Aims. This study aimed to determine outcomes of isolated tibial insert exchange (ITIE) during revision total knee arthroplasty (TKA). Methods. From 1985 to 2016, 270 ITIEs were performed at one institution for instability (55%, n = 148), polyethylene wear (39%, n = 105), insert fracture/dissociation (5%, n = 14), or stiffness (1%, n = 3). Patients with component loosening, implant malposition, infection, and extensor mechanism problems were excluded. Results. Survivorship free of any re-revision was 68% at ten years. For the indication of insert wear, survivorship free of any re-revision at ten years was 74%. Re-revisions were more frequent for index diagnoses other than wear (hazard ratio (HR) 1.9; p = 0.013), with ten-year survivorships of 69% for instability and 37% for insert fracture/dissociation. Following ITIE for wear, the most common reason for re-revision was aseptic loosening (33%, n = 7). For other indications, the most common reason for re-revision was recurrence of the original diagnosis. Mean Knee Society Scores improved from 54 (0 to 94) preoperatively to 77 (38 to 94) at ten years. Conclusion. After ITIE, the risk and reasons for re-revision correlated with preoperative indications. The best results were for polyethylene wear. For other diagnoses, the re-revision rate was higher and the failure mode was most commonly recurrence of the original indication for the revision TKA. Those with loosening of any of the components, history of infection, severe knee stiffness or problems with the extensor mechanism were excluded. There were 29 males (1 bilateral) and 26 female patients with a mean age of 66 years (range 35 to 83 years) at the time of revision surgery. Twenty-seven inserts were exchanged based on ligamentous instability, 24 because of insert wear or breakage including two cases of polyethylene dislodgment from the tibial base-plate and 5 for other reasons. Twelve knees had one to three prior revisions. Surveillance from index arthroplasty averaged 8.3 years (range 1.6 to 16.2 years) and since revision 4.6 years (range 2 to 14 years). Knee Society and Function Scores improved from 56 and 50.9 prior to revision to 76 and 59 at final surveillance, respectively. Fourteen (25%) of the 56 knees subsequently required re-revision after a mean of only three years (range 0.5 to 6.8) from tibial insert exchange. The cumulative survival at 5.5 years was 63.5% (95% CI=+/−14.4, n=19). Of the 27 patients with preoperative instability, eight were revised and another four were considered as failures due to severe pain. Of the 24 failed inserts, five were re-revised, one was amputated as a result of chronic ankle osteomyelitis concomitant to a chronically painful arthroplasty, and another two inserts failed due to severe pain. Isolated tibial insert exchange led to a surprisingly high early failure rate. Tibial insert exchange should therefore be undertaken with caution as an isolated method of total knee revision surgery even in those circumstances for which the modular insert was designed and felt to be of greatest value

Total knee replacement is one of the most successful procedures in orthopaedic surgery. Although originally limited to more elderly and less active individuals, the inclusion criteria for TKA have changed, with ever younger, more active and heavier patients receiving TKA. Currently, wear debris related osteolysis and associated prosthetic loosening are major modes of failure for TKA implants of all designs. Initially, tibial components were cemented all-polyethylene monoblock constructs. Subsequent long-term follow up studies of these implants have demonstrated excellent durability in survivorship studies out to twenty years. Aseptic loosening of the tibial component was one of the main causes of failure in these implants. Polyethylene wear with osteolysis around well fixed implants was rarely (if ever) observed. Cemented metal-backed nonmodular tibial components were subsequently introduced to allow for improved tibial load distribution and to protect osteoporotic bone. Long-term studies have established that many one-piece nonmodular tibial components have maintained excellent durability. Eventually, modularity between the polyethylene tibial component and the metal-backed tray was introduced in the mid-80s mainly to facilitate screw fixation for cementless implants. These designs also provided intra-operative versatility by allowing interchange of various polyethylene thicknesses, and to also aid the addition of stems and wedges. Other advantages included the reduction of inventory, and the potential for isolated tibial polyethylene exchanges as a simpler revision procedure. However, since the late 1980's, the phenomena of polyethylene wear and osteolysis have been observed much more frequently when compared with earlier eras. The reasons for this increased prevalence of synovitis, progressive osteolysis, and severe polyethylene wear remain unclear, but it is likely associated with the widespread use of both cementless and cemented modular tibial designs. Backside wear between the metal tray and polyethylene has been implicated. Recent RSA studies comparing fixation of all-polyethylene to modular components has shown that their RSA migration patterns are superior and fixation is in fact better with the all-polyethylene construct. Further, in a recent meta-analysis, all-polyethylene components were equivalent to metal-backed components regarding revision rates and clinical scores. The promise of modular tibial components affording a simple liner exchange to revise a knee has not borne out in the literature. Several studies have revealed that the effectiveness of isolated tibial insert exchange in revision TKR is of limited value. Isolated tibial insert exchange led to a surprisingly high rate of early failure. Tibial insert exchange as an isolated method of total knee revision should therefore be undertaken with caution even in circumstances for which the modular insert was designed and believed to be of greatest value. Because of the modularity, extra materials, and extra processing, modular tibial components are significantly more expensive than all-polyethylene components

Liner exchange and bone grafting are commonly used in cases of wear and osteolysis around well- fixed acetabular components in revision total hip arthroplasty. However, in total knee revision, liner exchange is a more rare option. In a multicenter study, we evaluated 22 TKAs that were revised with liner exchange and bone grafting for wear and osteolysis. All knees were well-fixed and well-aligned, and all components were modular tibial components. Osteolytic areas averaged 21.1cm2 and 7.6cm2 on AP projections of the femur and tibia, respectively, and averaged 21.6cm2 and 5.7cm2 on lateral projections of the femur and tibia, respectively, with the largest area being 54cm2 on a single projection. Follow up was minimum 2 years and average 40 months. No knees were revised and radiographically, all osteolytic lesions showed evidence of complete or partial graft incorporation. In addition, there was no radiographic evidence of loosening at final follow up. The Mayo Clinic evaluated 56 isolated tibial insert exchange revisions at their institution. Cases of loosening, infection, knee stiffness, or extensor mechanism problems were excluded. At minimum 2-year follow up (average 4.6 years), 14 knees (25%) required re-revision. Baker et al. evaluated 45 total knees undergoing isolated tibial insert exchange. At minimum 2 years, 4 knees (9%) required revision. Significant improvement was seen in clinical outcomes questionnaires, but only 58% had clinical successful global WOMAC scores. In summary, isolated liner exchange in the revision total knee setting has variable results. It can be successful but it is indeed a rare option and should be limited to cases were the total knee arthroplasty is both well-fixed and well-aligned

Patients ≤ 55 years have a high primary TKA revision rate compared to patients >55 years. Guided motion knee devices are commonly used in younger patients yet outcomes remain unknown. In this sub-group analysis of a large multicenter study, 254 TKAs with a second-generation guided motion knee implant were performed between 2011–2017 in 202 patients ≤ 55 years at seven US and three European sites. Revision rates were compared with Australian Joint Registry (AOANJRR) 2017 data. Average age 49.7 (range 18–54); 56.4% females; average BMI 34 kg/m2; 67.1% obese; patellae resurfaced in 98.4%. Average follow-up 4.2 years; longest follow-up six years; 27.5% followed-up for ≥ five years. Of eight revisions: total revision (one), tibial plate replacements (three), tibial insert exchanges (four). One tibial plate revision re-revised to total revision. Revision indications were mechanical loosening (n=2), infection (n=3), peri-prosthetic fracture (n=1), and instability (n=2). The Kaplan-Meier revision estimate was 3.4% (95% C.I. 1.7% to 6.7%) at five years compared to AOANJRR rate of 6.9%. There was no differential risk by sex. The revision rate of the second-generation guided motion knee system is lower in younger patients compared to registry controls

A modular tibial insert exchange is a seemingly attractive benign and simple surgical alternative when compared to full knee revision. Unfortunately, the results have been less than satisfactory with modular insert exchange for polyethylene wear and knee instability. Babis et al reported the results of 56 isolated insert exchanges performed for wear or instability. The re-revision rate was 25% at a mean follow-up of 3-years and the cumulative survival rate was only 63.5% at 5.5 years. In another study, 27% (6/22) insert exchanges for wear required re-revision within 5 years. Bert et al reported scoring and damage in 89% of 55 retained components considered candidates for isolated insert exchange. Such damage could account for accelerated wear of a new insert. These studies are misleading. The new insert must be polyethylene not prone to oxidation and accelerated wear. In a recent study of 177 revisions for wear and osteolysis, the survivorship of insert exchange using non-irradiated poly was 100%. Insert exchange does not correct the problem of a poor tibial locking mechanism. Whiteside and Katerberg reported 3 failures in 49 insert exchanges, fabrication of the tibial locking mechanism was used to address this problem. With revision for instability, insert exchange must provide full stability in both flexion and extension. Tibial insert exchange must correct the underlying cause of failure that led to the revision surgery. Full knee revision is a complex procedure that brings with it increased risks of perioperative complications such as infection and should be reserved only for cases that will not do well with simple insert exchange

Introduction. Instability following total knee arthroplasty is a leading cause of failure and is often treated with component revision. The goal of this study was to determine if isolated tibial polyethylene insert exchange (ITPIE) to a higher-level constraint would afford similar outcomes to component revision in the properly selected patient. Methods. We retrospectively evaluated 176 consecutive patients between 2016–2017 who were revised for symptomatic instability at a single institution. Demographic information and level of constraint preoperatively and postoperatively were documented. Radiographic parameters were also recorded for patients undergoing ITPIE. Outcome measures included all cause re-revision rates as well as patient reported outcome measures (PROMs) obtained preoperatively and at minimum 1-yr follow up. Descriptive analysis including sample t-test and chi square test were performed with statistical significance set at p <0.05. Results. 100 patients underwent component revision. 76 patients underwent ITPIE. Patients undergoing ITPIE were not found to have gross malalignment, malrotation, compromised fixation or insufficient collateral ligaments. No statistically significant differences were noted in terms of demographic characteristics between the groups. Similar increases in constraint as well as the thickness of the polyethylene were noted at the time of revision surgery in both groups. Further, patients underwent similar rates of re-revision (15.9% for component revision and 11.8% for ITPIE), p=0.8. In analyzing PROMs, no statistically significant differences were noted. Radiographic parameters of those who underwent ITPIE showed majority of patients to have well aligned components. The average follow-up was 2.3(±1.0)yrs for the component revision group and 2.2(±1.1)yrs for the ITPIE group. Conclusion. In the appropriately selected patient, ITPIE offers similar rates of success to component revision including similar re-revision rates and similar improvement in PROMs. Degree of constraint is generally increased and recommended when patients are treated for instability regardless of component revision or ITPIE. Instability after TKA can result from ligament imbalance, attenuation of soft tissues, or ligament disruption. Flexion instability has been reported after both CR and PS TKA. However, the clinical manifestations of flexion instability can be quite variable. Symptoms of flexion instability include pain and swelling after activity. Bracing occasionally can be helpful. Revision options to treat flexion instability include tibial insert exchange and revision to increase constraint. However, more favorable results have been reported using implants with varus-valgus constraint. Constrained mechanisms include a varus-valgus constrained PS post or hinge. The constrained post relies on the mechanical function of the post to provide stability which may deform or wear in-vivo leading to recurrent instability if used for a completely deficient collateral ligament. The hinge, which provides more rigid constraint, is indicated for collateral ligament deficiency. However, the additional constraint also results in greater bone-implant interface stresses, which may be mitigated by use of stem fixation to minimise risk of loosening

Introduction. Maintaining posterior stability in total knee arthroplasty (TKA) may be achieved by using a posterior stabilized TKA, retaining and balancing the posterior cruciate ligament (PCL) using a traditional cruciate-retaining design (CR), or by increasing the sagittal plane conformity of the tibial insert. In the latter case, stability is achieved by the addition of an anterior buildup on the tibial polyethylene creating the so-called “anterior stabilized” (AS) design. We hypothesized that using an AS tibial insert would provide similar function and survivorship as compared to using a more traditional CR bearing when the PCL is either recessed or balanced. Methods. Between 2004 and 2016, 1,731 modular CR TKAs were implanted in 1,509 patients using the same CR TKA design. The diagnosis was osteoarthritis in 98%. 58% of patients were female. Average age of 64.9 years. Within this group, 868 TKAs (50.1%) had a standard CR tibial bearing (3-degree posterior slope and no posterior lip) implanted (CR-S). 480 TKAs (27.8%) had a lipped CR modular tibial bearing (2.5 mm elevated posterior lip) implanted (CR-L). Starting in 2013, 383 TKAs (22.1%) were implanted with an AS modular tibial bearing (9–11 mm anterior lip and a 5 mm posterior lip). If the PCL was considered non-functional or absent, an AS bearing was placed. If the PCL was considered functional, a standard bearing or lipped bearing was used. Clinical and radiographic analysis was performed according to the Knee Society (KS) grading system. The most recent clinical and radiographic evaluation was used for post-operative analysis. The average follow-up in the entire cohort of TKAs was 5.5 years (range 2 to 14.3 years). Kaplan-Meier analysis was used to determine prosthesis survivorship with failure defined as aseptic loosening of the prosthesis (with or without revision) or tibial insert exchange. Results. At final follow-up, there were no significant differences in knee flexion, pain, function, or stair scores. Walking scores were significantly lower in the AS group. Posterior instability was higher in the CR-S group, whereas the manipulation rate was highest in the CR-L group (1.7% versus 1.3% and 0% in the CR-S and AS groups, respectively). Kaplan-Meier survivorship at five years, excluding infection, demonstrated no significant difference between the three groups (CR-S, CR-L, and AS tibial insert groups, 99% 100% and 99% respectively). Conclusion. When the PCL is considered non-functional at operation, the AS insert provided similar final flexion, function, stair score, and five-year survivorship when compared to using the standard and lipped CR tibial insert when the PCL was balanced. Using an ultra-congruent AS dished tibial component appears to be a reasonable option when the PCL is completely released or found deficient at operation. Aims: We present the results of the PFC Sigma knee replacement at a mean follow-up of 2.75 years (range: 1 to 6 years). Clinician and radiographic evaluation was carried out using the American Knee Society Clinical Rating and Radiographic Scoring Systems. Patient outcome was assessed using the Oxford-12 and SF-12 health-status questionnaires. Methods: A consecutive series of 293 Total Knee Replacements (TKR) in 247 patients was followed prospectively between September 1996 and September 2002. Our series included 138 females and 109 males with a mean age of 73.2 years. We operated on 214 patients with osteoarthritis, 20 with rheumatoid arthritis, 12 with post-traumatic arthritis and on one patient with erosive arthritis. The patella was resurfaced in 36 knees. 11 patients died during the follow-up period. Results: Subjective patient satisfaction score was based on improvement in pain and functional status. It was marked as excellent in 183 knees, good in 75, fair in 26 and poor in 9. Active extension lag improved from a mean of 8.5 to 3.3 degrees and flexion from 98.5 to 107.1 degrees. The mean Oxford-12 score improved from 44.17 to 24.68 and the SF-12 from 30.05 to 34.99. Radiolucent zones were observed around 2 tibial components. Post-operative complications included wound-healing problems in 25 patients, wound infection in 12, cardio-respiratory in 12, DVT in 11, non-fatal PE in 3, resolved CPN palsy in 3 and 1 death. Prosthesis complications included stiffness in 31, chronic pain in 18 (4 CRPS) and instability in 3 knees. 13 patients underwent MUA and 6 arthroscopic excision of intra-articular adhesions. 2 prosthetic infections were confirmed by bone-scan. Revision surgery was undertaken in 3 patients, 1 for established infection, 1 patella replacement and 1 tibial insert exchange. Conclusions: The early results of the PFC Sigma Knee Replacement show a high degree of patient satisfaction with a low rate of serious complications. Survivorship analysis will be undertaken in the future

Aims

This study compared patient-reported outcomes of three total knee arthroplasty (TKA) designs from one manufacturer: one cruciate-retaining (CR) design, and two cruciate-sacrificing designs, anterior-stabilized (AS) and posterior-stabilized (PS).

Methods

Patients scheduled for primary TKA were included in a single-centre, prospective, three-armed, blinded randomized trial (n = 216; 72 per group). After intraoperative confirmation of posterior cruciate ligament (PCL) integrity, patients were randomly allocated to receive a CR, AS, or PS design from the same TKA system. Insertion of an AS or PS design required PCL resection. The primary outcome was the mean score of all five subscales of the Knee injury and Osteoarthritis Outcome Score (KOOS) at two-year follow-up. Secondary outcomes included all KOOS subscales, Oxford Knee Score, EuroQol five-dimension health questionnaire, EuroQol visual analogue scale, range of motion (ROM), and willingness to undergo the operation again. Patient satisfaction was also assessed.

Introduction. Symptomatic instability following total knee arthroplasty (TKA) is a leading cause of early failure. Despite numerous reports on instability, standardized diagnostic and treatment protocols for these patients continue to remain unclear. Most reports recommend component revision as the preferred treatment, because of poor outcomes and high failure rates associated with isolated tibial polyethylene insert exchange (ITPIE). However, modern implant systems and standardized protocols may potentially change this teaching. Methods. We performed an IRB-approved, retrospective review of 90 consecutive patients with minimum 2 years follow-up who underwent revision TKA for instability by one of four arthroplasty surgeons at a single institution. Mean age was 62.0 years (range, 41 to 83 years), and 73% of patients were women. Charts were reviewed for relevant preoperative clinical and physical exam findings, as well as pertinent intraoperative findings. Radiographs were analyzed for femoral and tibial component positioning. Pre- and post-operative Knee Society Scores (KSS) were calculated. Results. Mean follow-up was 3.7 years. Using standardized criteria, 40% of patients were treated with ITPIE while 60% underwent revision of one or both components. In those treated with ITPIE, mean increase in polyethylene thickness was 4.4mm; level of constraint was also increased whenever allowed by the primary implants (47% of ITPIE cases). Total arc of motion improved from 117° to 123°. There were significant postoperative improvements in both KSS knee (48.4 to 82.6) and function (49.0 to 81.0) scores. Subgroup analysis demonstrated no significant differences in motion or KSS between those treated with ITPIE versus revision of one or both components. Combined failure rates were 19.4% in the ITPIE group versus 18.5% in the component revision group (p = 1.00). Conclusions. Using an algorithmic approach to patients with instability following TKA, symptoms and function can reliably be improved. Contrary to conventional teaching, ITPIE can be an effective strategy at addressing instability symptoms when specific preoperative and intraoperative criteria can be fulfilled

Aims

To describe the risk of periprosthetic joint infection (PJI) and reoperation in patients who have an acute, traumatic wound dehiscence following total knee arthroplasty (TKA).

Methods

From January 2002 to December 2018, 16,134 primary TKAs were performed at a single institution. A total of 26 patients (0.1%) had a traumatic wound dehiscence within the first 30 days. Mean age was 68 years (44 to 87), 38% (n = 10) were female, and mean BMI was 34 kg/m2 (23 to 48). Median time to dehiscence was 13 days (interquartile range (IQR) 4 to 15). The dehiscence resulted from a fall in 22 patients and sudden flexion after staple removal in four. The arthrotomy was also disrupted in 58% (n = 15), including a complete extensor mechanism disruption in four knees. An irrigation and debridement with component retention (IDCR) was performed within 48 hours in 19 of 26 knees and two-thirds were discharged on antibiotic therapy. The mean follow-up was six years (2 to 15). The association of wound dehiscence and the risk of developing a PJI was analyzed.

Aims

Currently, the US Center for Medicaid and Medicare Services (CMS) has been testing bundled payments for revision total joint arthroplasty (TJA) through the Bundled Payment for Care Improvement (BPCI) programme. Under the BPCI, bundled payments for revision TJAs are defined on the basis of diagnosis-related groups (DRGs). However, these DRG-based bundled payment models may not be adequate to account appropriately for the varying case-complexity seen in revision TJAs.

Methods

The 2008-2014 Medicare 5% Standard Analytical Files (SAF5) were used to identify patients undergoing revision TJA under DRG codes 466, 467, or 468. Generalized linear regression models were built to assess the independent marginal cost-impact of patient, procedural, and geographic characteristics on 90-day costs.

Aims

The aim of this study was to establish the results of isolated exchange of the tibial polyethylene insert in revision total knee arthroplasty (RTKA) in patients with well-fixed femoral or tibial components. We report on a series of RTKAs where only the polyethylene was replaced, and the patients were followed for a mean of 13.2 years (10.0 to 19.1).

Patients and Methods

Our study group consisted of 64 non-infected, grossly stable TKA patients revised over an eight-year period (1998 to 2006). The mean age of the patients at time of revision was 72.2 years (48 to 88). There were 36 females (56%) and 28 males (44%) in the cohort. All patients had received the same cemented, cruciate-retaining patella resurfaced primary TKA. All subsequently underwent an isolated polyethylene insert exchange. The mean time from the primary TKA to RTKA was 9.1 years (2.2 to 16.1).

We evaluated the survival of moulded monoblock and modular tibial components of the AGC total knee replacement in patients with rheumatoid arthritis. Between 1985 and 1995, 751 knees with this diagnosis were replaced at our institution. A total of 256 tibial components were of the moulded design and 495 of the modular design. The mean follow-up of the moulded subgroup was 9.6 years (0.5 to 14.7), and that of the modular group 7.0 years (0.1 to 14.7).

The groups differed significantly from each other in Larsen grade, cementing of components and patellar resurfacing, but no statistically significant difference between the survival of the components was found (Log rank test, p = 0.91). The cumulative success rate of the moulded group was 96.8% (95% confidence interval 93.6% to 98.4%) at five years and 94.4% (95% confidence interval 90.4% to 96.7%) at ten years, and of the modular group 96.2% (95% confidence interval 94% to 97.6%) and 93.6% (95% confidence interval 89.7% to 96%), respectively. Revision was required in 37 total knee replacements, the main causes were infection, pain, loosening of the tibial component and patellar problems. Survival rates for both components were satisfactory.
