Abstract. Introduction. Mid-flexion instability may cause poor outcomes following TKA. Surgical technique, patient-specific factors, and implant design could all contribute to it, with modelling and fluoroscopy data suggesting the latter may be the root cause. However, current implants all pass the preclinical stability testing standards, making it difficult to understand the effects of implant design on instability. We hypothesized that a more physiological test, analysing functional stability across the range of knee flexion-extension, could delineate the effects of design, independent of surgical technique and patient-specific factors. Methods. Using a SIMvitro-controlled six-degree-of-freedom robot, a dynamic stability test was developed, including continuous flexion and reporting data in a trans-epicondylar axis system. 3 femoral geometries were tested: gradually reducing radius, multi-radius and single-radius, with their respective tibial inserts. 710N of compression force (body weight) was applied to the implants as they were flexed from 0–140° with three levels of anterior/posterior (AP) tibial force applied (−90N,0N,90N). Results. While in static tests, the implants performed similarly, functional stability testing revealed different paths of motion and AP laxities throughout the flexion cycle. Some designs exhibited mid-flexion instability, while others did not: the multi-radius design allowed increased AP laxity as it transitioned to each arc of reduced femoral component radius; the single-radius design had low tibial bearing conformity, allowing 16mm difference in the paths of mid-flexion versus extension motion. Conclusions. Preclinical lab testing reveals functional differences between different design philosophies. Implant design impacts kinematics and mid-flexion stability, even before factoring in surgical technique and patient-specific factors

Abstract. Introduction. Derby technique for posterolateral corner (PLC) reconstruction uses a doubled gracilis autograft to reconstruct the popliteo-fibular ligament (PFL) and a split biceps tendon transfer to reconstruct the lateral collateral ligament (LCL). We report midterm outcomes of a case series who underwent PLC reconstruction. Methodology. A retrospective review of 27 patients who underwent PLC reconstruction from 2012 until 2018 was performed (6 females, 21 males, median age 26). Median time interval from injury to primary procedure was 10 months. Outcomes were recorded as per clinical assessment and outcome scores. Results. All patients had no varus laxity in full knee extension. Similarly, at 30° of knee flexion, 25 patients (92.5%) had no varus laxity and 2 patients (7.5%) had grade 1 laxity. No patients demonstrated abnormal external rotation at final follow up. Outcome scores were obtained from 17 patients who underwent primary ligament reconstruction at a mean follow up of 72 months postoperatively. The mean Lysholm score was 94 +/-5.5 (72–100) and mean subjective IKDC score was 78/87 (89.6%) (51–87). 90% of our patients had gained normal or nearly normal IKDC scores on returning to routine activities of daily living and sports following surgery compared to their pre-injury status. Conclusion. Our technique enables an anatomical LCL & PFL reconstruction in isolation or in combination with ACL/PCL reconstruction or to augment an acute repair without contralateral graft harvest or allograft. The functional knee scores demonstrated in this cohort are comparable and in fact superior to previously published knee scores after PLC reconstruction

Aims. Ideal component sizing may be difficult to achieve in unicompartmental knee arthroplasty (UKA). Anatomical variants, incremental implant size, and a reduced surgical exposure may lead to over- or under-sizing of the components. The purpose of this study was to compare the accuracy of UKA sizing with robotic-assisted techniques versus a conventional surgical technique. Methods. Three groups of 93 medial UKAs were assessed. The first group was performed by a conventional technique, the second group with an image-free robotic-assisted system (Image-Free group), and the last group with an image-based robotic arm-assisted system, using a preoperative CT scan (Image-Based group). There were no demographic differences between groups. We compared six parameters on postoperative radiographs to assess UKA sizing. Incorrect sizing was defined by an over- or under-sizing greater than 3 mm. Results. There was a higher rate of tibial under-sizing posteriorly in the conventional group compared to robotic-assisted groups (47.3% (n = 44) in conventional group, 29% (n = 27) in Image-Free group, 6.5% (n = 6) in Image-Based group; p < 0.001), as well as a higher rate of femoral under-sizing posteriorly (30.1% (n = 28) in conventional group, 7.5% (n = 7) in Image-Free group, 12.9% (n = 12) in Image-Based group; p < 0.001). The posterior femoral offset was more often increased in the conventional group, especially in comparison to the Image-Based group (43% (n = 40) in conventional group, 30.1% (n = 28) in Image-Free group, 8.6% (n = 8) in Image-Based group; p < 0.001). There was no significant overhang of the femoral or tibial implant in any groups. Conclusion. Robotic-assisted surgical techniques for medial UKA decrease the risk of tibial and femoral under-sizing, particularly with an image-based system using a preoperative CT scan. Aims. In-hospital length of stay (LOS) and discharge dispositions following arthroplasty could act as surrogate measures for improvement in patient pathways, and have major cost saving implications for healthcare providers. With the ever-growing adoption of robotic technology in arthroplasty, it is imperative to evaluate its impact on LOS. The objectives of this study were to compare LOS and discharge dispositions following robotic arm-assisted total knee arthroplasty (RO TKA) and unicompartmental arthroplasty (RO UKA) versus conventional technique (CO TKA and UKA). Methods. This large-scale, single-institution study included patients of any age undergoing primary TKA (n = 1,375) or UKA (n = 337) for any cause between May 2019 and January 2023. Data extracted included patient demographics, LOS, need for post anaesthesia care unit (PACU) admission, anaesthesia type, readmission within 30 days, and discharge dispositions. Univariate and multivariate logistic regression models were also employed to identify factors and patient characteristics related to delayed discharge. Results. The median LOS in the RO TKA group was 76 hours (interquartile range (IQR) 54 to 104) versus 82.5 (IQR 58 to 127) in the CO TKA group (p < 0.001) and 54 hours (IQR 34 to 77) in the RO UKA versus 58 (IQR 35 to 81) in the CO UKA (p = 0.031). Discharge dispositions were comparable between the two groups. A higher percentage of patients undergoing CO TKA required PACU admission (8% vs 5.2%; p = 0.040). Conclusion. Our study showed that robotic arm assistance was associated with a shorter LOS in patients undergoing primary UKA and TKA, and no difference in the discharge destinations. Our results suggest that robotic arm assistance could be advantageous in partly addressing the upsurge of knee arthroplasty procedures and the concomitant healthcare burden; however, this needs to be corroborated by long-term cost-effectiveness analyses and data from randomized controlled studies. Aims. Metaphyseal cones with cemented stems are frequently used in revision total knee arthroplasty (TKA). However, if the diaphysis has been previously violated, the resultant sclerotic canal can impair cemented stem fixation, which is vital for bone ingrowth into the cone, and long-term fixation. We report the outcomes of our solution to this problem, in which impaction grafting and a cemented stem in the diaphysis is combined with an uncemented metaphyseal cone, for revision TKA in patients with severely compromised bone. Methods. A metaphyseal cone was combined with diaphyseal impaction grafting and cemented stems for 35 revision TKAs. There were two patients with follow-up of less than two years who were excluded, leaving 33 procedures in 32 patients in the study. The mean age of the patients at the time of revision TKA was 67 years (32 to 87); 20 (60%) were male. Patients had undergone a mean of four (1 to 13) previous knee arthroplasty procedures. The indications for revision were aseptic loosening (80%) and two-stage reimplantation for prosthetic joint infection (PJI; 20%). The mean follow-up was four years (2 to 11). Results. Survival free from revision of the cone/impaction grafting construct due to aseptic loosening was 100% at five years. Survival free from any revision of the construct and free from any reoperation were 92% and 73% at five years, respectively. A total of six patients (six TKAs, 17%) required a further revision, four for infection or wound issues, and two for periprosthetic fracture. Radiologically, one unrevised TKA had evidence of loosening which was asymptomatic. In all unrevised TKAs the impacted diaphyseal bone graft appeared to be incorporated radiologically. Conclusion. When presented with a sclerotic diaphysis and substantial metaphyseal bone loss, this technique combining diaphyseal impaction grafting with a metaphyseal cone provided near universal success in relation to implant fixation. Moreover, radiographs revealed incorporation of the bone graft and biological fixation of the cone. While long-term follow-up will be important, this technique provides an excellent option for the management of complex revision TKAs. Cite this article: Bone Joint J 2020;102-B(6 Supple A):116–122

Aims

Robotic-assisted total knee arthroplasty (RA-TKA) is theoretically more accurate for component positioning than TKA performed with mechanical instruments (M-TKA). Furthermore, the ability to incorporate soft-tissue laxity data into the plan prior to bone resection should reduce variability between the planned polyethylene thickness and the final implanted polyethylene. The purpose of this study was to compare accuracy to plan for component positioning and precision, as demonstrated by deviation from plan for polyethylene insert thickness in measured-resection RA-TKA versus M-TKA.

Methods

A total of 220 consecutive primary TKAs between May 2016 and November 2018, performed by a single surgeon, were reviewed. Planned coronal plane component alignment and overall limb alignment were all 0° to the mechanical axis; tibial posterior slope was 2°; and polyethylene thickness was 9 mm. For RA-TKA, individual component position was adjusted to assist gap-balancing but planned coronal plane alignment for the femoral and tibial components and overall limb alignment remained 0 ± 3°; planned tibial posterior slope was 1.5°. Mean deviations from plan for each parameter were compared between groups for positioning and size and outliers were assessed.

However, if the diaphysis has been previously violated (as in revision of a failed stemmed implant), the resultant sclerotic canal can impair cemented stem fixation, which is vital for cone ingrowth and long-term fixation. We report the outcomes of our novel solution to this problem, in which impaction grafting and a cemented stem in the diaphysis was combined with an uncemented metaphyseal cone for revision TKAs with severely compromised bone. Methods. A metaphyseal cone was combined with diaphyseal impaction grafting and cemented stems in a novel fashion for 35 revision TKAs. Mean age at revision TKA was 70 years, with 63% being male. Patients had a mean of 4 prior knee arthroplasty procedures. Indications for the revision with this construct were aseptic loosening (80%) and two-stage re-implantation for periprosthetic infection (PJI; 20%). Mean follow-up was 3 years. Results. Survivorship free from revision of the cone/impaction grafting construct due to aseptic loosening was 100% at 5 years. Survivorships free from any revision of the cone/impaction grafting construct and free from any reoperation were 92% and 73% at 5 years, respectively. Six knees (17%) required a reoperation (4 for infection/wound issues and 2 for periprosthetic fractures). Radiographically, 97% of cones were ingrown (1 loose cone in setting of PJI). In all but one case, impacted diaphyseal bone graft appeared to have incorporated radiographically. Conclusions. When presented with a sclerotic diaphysis and substantial metaphyseal bone loss, this innovative technique combining diaphyseal impaction grafting with a metaphyseal cone provided near universal success in regards to implant fixation. Moreover, radiographs revealed incorporation of the bone graft, and ingrowth of the cones. Objectives. The lack of effective treatment for cartilage defects has prompted investigations using tissue engineering techniques for their regeneration and repair. The success of tissue-engineered repair of cartilage may depend on the rapid and efficient adhesion of transplanted cells to a scaffold. Our aim in this study was to repair full-thickness defects in articular cartilage in the weight-bearing area of a porcine model, and to investigate whether the CD44 monoclonal antibody biotin-avidin (CBA) binding technique could provide satisfactory tissue-engineered cartilage. Methods. Cartilage defects were created in the load-bearing region of the lateral femoral condyle of mini-type pigs. The defects were repaired with traditional tissue-engineered cartilage, tissue-engineered cartilage constructed with the biotin-avidin (BA) technique, tissue-engineered cartilage constructed with the CBA technique and with autologous cartilage. The biomechanical properties, Western blot assay, histological findings and immunohistochemical staining were explored. Results. The CBA group showed similar results to the autologous group in biomechanical properties, Moran’s criteria, histological tests and Wakitani histological scoring. Conclusions. These results suggest that tissue-engineered cartilage constructed using the CBA technique could be used effectively to repair cartilage defects in the weight-bearing area of joints. Intro/Purpose. Lateral partial knee replacement is indicated as an alternative to total knee replacement for isolated end stage lateral compartment degenerative joint disease. The purpose of this study is to report the surgical technique and clinical results of a large series of lateral partial knee replacements from two institutions. Methods. A retrospective review identified 205 lateral unicompartmental arthroplasty procedures performed in two practices over a six-year period (Aug 2011 – June 2017). Patients indicated for surgery met specific preoperative clinical and radiographic criteria confirming lateral compartment arthritic disease with a correctable deformity, intact full-thickness medial cartilage, competent anterior cruciate ligament, and minimal disease in the patellofemoral compartment. A single cemented fixed bearing tibial component design was used in all cases specifically designed for lateral compartment anatomy. Results. The average age was 65 years old and 76% were female. Average follow-up in the lateral partials was 1.7 years (range 0.5 years to 6 years). Knee Society Scores improved from 45 (pre-op) to 88 points (post-op). Range of motion improved from 6–115o (pre-op) to 0–122 o (post-op). No knees were revised to a TKA. One knee had an additional medial partial knee placed at 1.1 years post lateral partial knee replacement for medial disease progression. Two knees required an irrigation and debridement for traumatic wound dehiscence. Conclusion. This is the largest series of lateral partial knee replacements reported. Clinical improvements, range of motion, and survivorship were excellent at short-term follow-up. Long-term follow-up is needed for additional understanding of a novel implant designed specifically for the lateral compartment and the survivorship of the unreplaced compartments. We believe the lateral partial knee replacement to be a viable option for isolated lateral compartment disease in approximately 4% of patients

Introduction. There are numerous surgical techniques for medial patellofemoral ligament (MPFL) reconstruction. Problems with certain techniques include patellar fracture and re-rupture. Aim. To investigate the functional outcomes of MPFL reconstructions performed using a free gracillis tendon graft, oblique medial patella tunnel and interference screw femoral fixation. Patients were selected for MPFL reconstruction if they had recurrent patellar dislocations, and with the use of clinical and radiographic evaluation. Methods. A prospective case series of 31 consecutive knees in 28 patients underwent reconstruction of the MPFL using a free gracillis tendon graft, oblique medial patella tunnel and interference screw femoral fixation. Our primary outcome measure was the Kujala patellofemoral questionnaire. This was assessed preoperatively and postoperatively at 6 weeks and 3, 6, 9, 12, 18, 24 months and at final follow up. Secondary outcome measures included Fulkerson patellofemoral scores at the same time intervals, return to work, return to preoperative sport and complications. Results. The median follow up time was 25 months (range 12 – 44 months). Kujala scores improved from 58 to 93 (p < 0.002) and Fulkerson scores improved from 58 to 95 (p < 0.002) pre- and postoperatively respectively. The median return to work was 8 weeks and return to preoperative sport was 11 weeks. There were 2 revisions as a result of trauma. There were no instances of patellar fracture in this case series. Conclusions. MPFL reconstruction using a free gracillis tendon graft, oblique medial patella tunnel and interference screw femoral fixation appears to be an effective surgical intervention for patellar instability. This technique also reduces the risk of patellar fracture, a common shortcoming with many other techniques

Purpose of Study. 1- To introduce a new graft technique for ACL reconstruction using the patellar tendon with its attachment on the tibial tuberosity as bone-tendon (B PT) auto graft sparing the patella. 2-To assess the above described technique prospectively in a group of patients with ACL deficient knees. Type of study. Prospective case series of a newly describedACL graft technique. Material and methods. 44 patients with ACL deficient knee (with no other ligament injuries) were recruited to have ACL reconstruction using the above mentioned graft technique, there was 39 males and 5 females, average age at the index surgery was 24,4 year (range 20-40), the average time from injury to operation was 4 months (range 1 week to 1 year), the average follow up period was 30 months (ranged 24-35), there was 19 patients with isolated ACL injury and 25 with associated intra articular pathology including 22 meniscal tear, 8 with articular cartilage injury (3 of them associated with the meniscal tears). Those injuries were treated with meniscal repair (12 cases), partial menisectomy (10 cases), debridement and shaving +/− micro fracture technique for the articular cartilage injuries. Results. at the final follow up all patients were satisfied with their knees, the IKDC improved from 68 +- 2.8(pre-op) to 92 +- 2.7 (CI-95%), and Lysholm score from 65 +- 3.2 to 93 +- 3(CI-95%) at the final visit. Anterior knee pain was reported in 4 patients (9%) on moderate to strenuous activities. 7 patients (16%) could not return to their pre-injury activity level. The mean difference in KT-1000 measurement was 2 +- 0.8. Conclusion. our technique is effective with compatible results with other ACL graft's technique with less complications rate when compared with PBTB auto graft, it avoids the remote incidence of patellar fractures, and reducing the incidence of anterior knee pain

Aims. This non-blinded randomised controlled trial compared the effect of patient-controlled epidural analgesia (PCEA) versus local infiltration analgesia (LIA) within an established enhanced recovery programme on the attainment of discharge criteria and recovery one year after total knee arthroplasty (TKA). The hypothesis was that LIA would increase the proportion of patients discharged from rehabilitation by the fourth post-operative day but would not affect outcomes at one year. Patients and Methods. A total of 242 patients were randomised; 20 were excluded due to failure of spinal anaesthesia leaving 109 patients in the PCEA group and 113 in the LIA group. Patients were reviewed at six weeks and one year post-operatively. Results. There was no difference in the proportion of patients discharged from rehabilitation by the fourth post-operative day, (77% in the PCEA group, 82% in the LIA group, p = 0.33), mean length of stay (four days in each group, p = 0.540), day of first mobilisation (p = 0.013) or pain (p = 0.278). There was no difference in mean Oxford Knee Scores (41 points in each group, p = 0.915) or the rate of complications in the two groups. Conclusion. Both techniques provided adequate pain relief, enabled early mobilisation and accelerated rehabilitation and good patient-reported outcomes up to one year post-operatively. We have compared a new technique of computer-assisted knee arthroplasty with the current conventional jig-based technique in 70 patients randomly allocated to receive either of the methods. Post-operative CT was performed according to the Perth CT Knee Arthroplasty protocol and pre- and post-operative Maquet views of the limb were taken. Intra-operative and peri-operative morbidity data were collected and blood loss measured. Post-operative CT showed a significant improvement in the alignment of the components using computer-assisted surgery in regard to femoral varus/valgus (p = 0.032), femoral rotation (p = 0.001), tibial varus/valgus (p = 0.047) tibial posterior slope (p = 0.0001), tibial rotation (p = 0.011) and femorotibial mismatch (p = 0.037). Standing alignment was also improved (p = 0.004) and blood loss was less (p = 0.0001). Computer-assisted surgery took longer with a mean increase of 13 minutes (p = 0.0001)

The aim of this study was to analyse the gait pattern, muscle force and functional outcome of patients who had undergone replacement of the proximal tibia for tumour and alloplastic reconstruction of the extensor mechanism using the patellar-loop technique. Between February 1998 and December 2009, we carried out wide local excision of a primary sarcoma of the proximal tibia, proximal tibial replacement and reconstruction of the extensor mechanism using the patellar-loop technique in 18 patients. Of these, nine were available for evaluation after a mean of 11.6 years (0.5 to 21.6). The strength of the knee extensors was measured using an Isobex machine and gait analysis was undertaken in our gait assessment laboratory. Functional outcome was assessed using the American Knee Society (AKS) and Musculoskeletal Tumor Society (MSTS) scores. The gait pattern of the patients differed in ground contact time, flexion heel strike, maximal flexion loading response and total sagittal plane excursion. The mean maximum active flexion was 91° (30° to 110°). The overall mean extensor lag was 1° (0° to 5°). The mean extensor muscle strength was 25.8% (8.3% to 90.3%) of that in the non-operated leg (p < 0.001). The mean functional scores were 68.7% (43.4% to 83.3%) (MSTS) and 71.1 (30 to 90) (AKS functional score). In summary, the results show that reconstruction of the extensor mechanism using this technique gives good biomechanical and functional results. The patients’ gait pattern is close to normal, except for a somewhat stiff knee gait pattern. The strength of the extensor mechanism is reduced, but sufficient for walking. . We assessed the reliability, accuracy and variability of closed-wedge high tibial osteotomy (HTO) using computer-assisted surgery compared to the conventional technique. A total of 50 closed-wedge HTO procedures were performed using the navigation system, and compared with 50 HTOs that had been performed with the conventional technique. In the navigation group, the mean mechanical axis prior to osteotomy was varus 8.2°, and the mean mechanical axis following fixation was valgus 3.6°. On the radiographs the mean pre-operative mechanical axis was varus 7.3°, and the mean post-operative mechanical axis was valgus 2.1°. There was a positive correlation between the measured data taken under navigation and by radiographs (r > 0.3, p < 0.05). The mean correction angle was significantly more accurate in the navigation group (p < 0.002). The variability of the correction was significantly lower in the navigation group (2.3° vs 3.7°, p = 0,012). We conclude that navigation provides reliable real-time intra-operative information, may increase accuracy, and improves the precision of a closed-wedge HTO

Introduction

Infection following TKA can be a catastrophic complication that can cause significant pain, morbidity and jeopardize limb viability. The integrity of the soft tissue envelope is critical to successful treatment and infection control. While local tissue flaps can provide adequate coverage for most soft tissue defects around the knee, there are cases that require salvage using microvascular free tissue transfers. The purpose of this study is to evaluate the 1) rate of limb salvage; 2) infection control; and 3) clinical function following free flap coverage for salvage of the infected TKA.

Materials and Methods

We retrospectively reviewed 23 microvascular free tissue transfers for management of soft tissue defects in infected TKA. There were 16 men and 7 women with a mean age of 61.2 years (range 39–81). The median number of procedures performed prior to soft tissue coverage was 5 (range 2–9) and all patients had failed at least one 2 stage reimplantation procedure. Clinical outcomes were measured using the Knee Society Scoring system for pain and function. The rate of limb salvage and infection control were recorded.

The dysplastic Trochlear is a developmental condition characterised by an abnormally flat or dome-shaped trochlea and it is recognised as a significant cause of patella instability, with the increased sulcus angle being is the most common finding. Surgical correction of the shape of the Trochlear Groove is frequently performed. The described methods in the literature involve open arthrotomy to normalise and maintain the trochlear morphology achieving normal alignment and tracking of the patella. Open procedures carries a significant risk of arthrofibrosis. The technique was developed in human cadaveric knees at the Donjoy Clinical Education and Research Facility (CERF) in Vista California. We describe an arthroscopic procedure to create a neo-trochlea using gouges, spherical and conical hooded burrs. We studied prospectively a series of 4 consecutive patients with patello-femoral instability secondary to trochlear dysplasia, who were treated by an Arthroscopic trochleoplasty by a single surgeon between 2007 and 2008. Postoperatively the patients were rehabilitated in accordance with our routine Patello-Femoral microfracture Protocol, allowing weight bearing and ROM 0-20 degrees in a long lever brace for 6 weeks. CT scanning at one year showed a complete neo-cortex and cartilage sequenced MRI at 12 months showed complete fill with fibro cartilage. Pre- and post-operative scores (KOOS, Kujala) were assessed by the patients and a satisfaction questionnaire was completed. The results showed a statistical improvement in the outcome at the 2 year follow up. Overall, patients (100%) were satisfied with the outcome of their procedure and there have been no adverse events. To our knowledge this technique has not been described before in the English literature and the early results of arthroscopic trochleoplasty are encouraging and offer an alternative to open approaches. Larger numbers and longer follow ups are needed to confirm the long term benefit

A prospective study of 276 TKA's performed in patients with valgus knee deformity ≥ 10° using a Modified Surgical Technique. MATERIALS AND METHODS. Bone cuts were used to balance the valgus knee and soft tissue release was confined to the postero-lateral capsule in severe deformity. The aim was to restore a “functional mechanical axis” as opposed to a “neutral” mechanical axis. All surgeries were performed between Jan2003 and Apr2007, under the care of a single surgeon using an LCS rotating platform. All patients had full length radiographs and outcome scores collected prospectively. RESULTS. The mean coronal alignment of the lower limb was corrected from 15.9°(10-45°) to 3.8°. 94% patients had their coronal alignment restored to = 7°. Sixteen knees with postoperative valgus ≥8° were analysed as a separate group. The mechanical axis deviation was corrected from 52.3mm to 8.8mm. The distal femoral cut was made at 5° valgus in 131 knees(47.5%), 6° in 111 knees(40.2%) and 7° in 24 knees(8.7%).78 knees(28%) were balanced only with bone cuts. 198 knees(72%) had release of posterolateral capsule. 16 knees(5.8%) also had release of IT band. Lateral patellar release was performed in 39 knees (14%) and 23 knees had patella contouring. 93%knees had central patello-femoral alignment postoperatively. One spinout was managed by closed reduction and a second patient had revision of tibial tray for collapse. Patellar subluxation occurred in two patients. The oxford knee score and the American Knee Society clinical score improved from 48.5 to 26 and 21.04 to 86.03 respectively. CONCLUSION. Adequate correction of valgus knee deformity was successfully achieved using our modified technique with satisfactory short term outcome. It is our opinion that the extensive release of soft tissues is not only unnecessary but also fraught with the potential for instability

We describe the results of 50 operations carried out on 46 patients with medial osteoarthritis of the knee of Ahlbäck grade 1 to 3. Patients were randomised either to a closed-wedge high tibial osteotomy (HTO) or an open-wedge procedure based on the hemicallotasis technique (HCO). Their median age was 55 years (38 to 68). The preoperative median hip-knee-ankle (HKA) angle was 171° (164 to 176) in the HTO group and 173° (165 to 179) in the HCO group. After six weeks, the median HKA angle was 185° (176 to 194) in the HTO group and 184° (181 to 188) in the HCO group. In the HTO group, seven patients were within the range of 182° to 186° compared with 21 in the HCO group (p < 0.001). One year later, ten HTO patients were within this range while the HKA angulation in the HCO group was unchanged. At two years the numbers were 11 and 18, respectively. We evaluated the clinical results on the Hospital for Special Surgery, Lysholm and Wallgren-Tegner activity scores, and patients completed part of the Nottingham Health Profile questionnaire. An impartial observer at the two-year follow-up concluded that all scores had improved, but found no clinical differences between the groups

Anterior cruciate ligament (ACL) reconstruction is commonly performed and has been for many years. Despite this, the technical details related to ACL anatomy, such as tunnel placement, are still a topic for debate. In this paper, we introduce the flat ribbon concept of the anatomy of the ACL, and its relevance to clinical practice.
