Unicompartmental knee osteoarthritis can be treated with either Total Knee Arthroplasty (TKA) or Unicompartmental Knee Arthroplasty (UKA) and controversy remains as to which treatment is best. UKA has been reported to offer a variety of advantages, however many still see it as a temporary procedure with higher revision rates. We aimed to clarify the role of UKA and evaluate the long-term and revision outcomes. We retrospectively reviewed the pain, function and total Knee Society Score (KSS) for 602 UKA and 602 TKA in age and gender matched patients over ten years. The total pre-operative KSS scores were not significantly different between UKA and TKA (42.67 vs 40.54 P=0.021). KSS (pain) was significantly better in the TKA group (44.39 vs 41.38 P= 0.007) at one year and at five years post-operatively (45.33 vs 43.12 P=0.004). There was no statistically significant difference for KSS (total) in TKA and UKA during the study period. 16.3% of UKA and 20.1% of TKA had a documented complication. 79 UKA (13%) and 36 TKA (6%) required revision surgery. Despite the higher revision rate, pre-operative KSS (total) before revision was not significantly different between UKA and TKA (42.94 vs 42.43 P=0.84). Performance for UKAs was inferior to TKAs in Kaplan-Meier cumulative survival analysis at 10 years (P<0.001). Both UKA and TKA are viable treatment options for unicompartmental knee osteoarthritis, each with their own merits. UKA is associated with fewer complications whereas TKA provides better initial pain relief and is more durable and less likely to require revision

The objective of this study was to compare differences in alignment following robotic arm-assisted bi-unicompartmental knee arthroplasty (Bi-UKA) and conventional total knee arthroplasty (TKA). This was a prospective, randomised controlled trial of 70 patients. 39 TKAs were implanted manually, as per standard protocol at our institution, and 31 Bi-UKA patients simultaneously received fixed-bearing medial and lateral UKAs, implanted using robotic arm-assistance. Preoperative and 3-month postoperative CT scans were analysed to determine hip knee ankle angle (HKAA), medial distal femoral angle (MDFA), and medial proximal tibial angle (MPTA). Analysis was repeated for 10 patients by a second rater to validate measurement reliability by calculating the intra-class correlation coefficient (ICC). Mean change in HKAA towards neutral was 2.7° in TKA patients and 2.3° in Bi-UKA patients (P=0.6). Mean change in MDFA was 2.5° for TKA and 1.0° for Bi-UKA (P<0.01). Mean change in MPTA was 3.7° for TKA and 0.8° for Bi-UKA (P<0.01). Mean postoperative MDFA and MPTA for TKAs were 89.8° and 89.6° respectively, indicating orientation of femoral and tibial components perpendicular to the mechanical axis. Mean postoperative MDFA and MPTA for Bi-UKAs were 91.0° and 86.9° respectively, indicating a more oblique joint line orientation. Inter-rater agreement was excellent (ICC>0.99). Early functional activities, according to the new Knee Society Scoring System, favoured Bi-UKAs (P<0.05). Robotic arm-assisted, cruciate-sparing Bi-UKA better maintains the natural anatomy of the knee in the coronal plane and may therefore preserve normal joint kinematics, compared to a mechanically aligned TKA. This has been achieved without significantly altering overall HKAA

Anterior knee pain (AKP) is the commonest complication of total knee arthroplasty (TKA). This study aims to assess whether sagittal femoral component position is an independent predictor of AKP after cruciate retaining single radius TKA without primary patellofemoral resurfacing. From a prospective cohort of 297 consecutive TKAs, 73 (25%) patients reported AKP and 89 (30%) reported no pain at 10 years. Patients were assessed pre-operatively and at 1, 5 and 10 years using the short form 12 and Oxford Knee Score (OKS). Variables assessed included demographic data, indication, reoperation, patella resurfacing, and radiographic criteria. Patients with AKP (mean age 67.0 (38–82), 48 (66%) female) had mean Visual Analogue Scale (VAS) Pain scores of 34.3 (range 5–100). VAS scores were 0 in patients with no pain (mean age 66.5 (41–82), 60 (67%) female). Femoral component flexion (FCF), anterior femoral offset ratio, and medial proximal tibial angle all differed significantly between patients with AKP and no pain (p<0.001), p=0.007, p=0.009, respectively). All PROMs were worse in the AKP group at 10 years (p<0.05). OKSs were worse from 1 year (p<0.05). Multivariate analysis confirmed FCF and Insall ratio <0.8 as independent predictors of AKP (R. 2. = 0.263). Extension of ≥0.5° predicted AKP with 87% sensitivity. AKP affects 25% of patients following single radius cruciate retaining TKA, resulting in inferior patient-reported outcome measures at 10 years. Sagittal plane positioning and alignment of the femoral component are important determinants of long-term AKP with femoral component extension being a major risk factor

Radiological evidence of post-traumatic osteoarthritis (PTOA) after fracture of the tibial plateau is common but end-stage arthritis which requires total knee arthroplasty is much rarer. The aim of this study was to examine the indications for, and outcomes of, total knee arthroplasty after fracture of the tibial plateau and to compare this with an age and gender-matched cohort of TKAs carried out for primary osteoarthritis. Between 1997 and 2011, 31 consecutive patients (23 women, eight men) with a mean age of 65 years (40 to 89) underwent TKA at a mean of 24 months (2 to 124) after a fracture of the tibial plateau. Of these, 24 had undergone ORIF and seven had been treated non-operatively. Patients were assessed pre-operatively and at 6, 12 and > 60 months using the Short Form-12, Oxford Knee Score and a patient satisfaction score. Patients with instability or nonunion needed total knee arthroplasty earlier (14 and 13.3 months post-injury) than those with intra-articular malunion (50 months, p < 0.001). Primary cruciate-retaining implants were used in 27 (87%) patients. Complication rates were higher in the PTOA cohort and included wound complications (13% vs 1% p = 0.014) and persistent stiffness (10% vs 0%, p = 0.014). Two (6%) PTOA patients required revision total knee arthroplasty at 57 and 114 months. The mean Oxford knee score was worse pre-operatively in the cohort with primary osteoarthritis (18 vs 30, p < 0.001) but there were no significant differences in post-operative Oxford knee score or patient satisfaction (primary osteoarthritis 86%, PTOA 78%, p = 0.437). Total knee arthroplasty undertaken after fracture of the tibial plateau has a higher rate of complications than that undertaken for primary osteoarthritis, but patient-reported outcomes and satisfaction are comparable. The prevalence of Parkinson's disease (PD) is expected to rise however reports of the outcomes of total knee arthroplasty (TKA) in patients with PD in the literature are sparse. We present the first study to compare short to medium term outcomes of TKA in patients with and without PD. We performed a retrospective analysis of data from our regional arthroplasty database. In our PD group 32 TKAs were implanted. In our age-matched control group 33 TKAs were implanted. Mean age at operation was 73 years and the primary indication was osteoarthritis in both groups. Data was collected pre-operatively and at routine 1, 3 and 5 year follow-up attendances. Median in-patient stay was comparable in both groups (P=0.714). Pre-operatively, there were no between-group differences in range of movement, Knee Society Function Score (KSFS), Knee Society Score (KSS) or Pain score taken as an independent variable (P=0.108, 0.079, 0.478 and 0.496). KSS improved in both groups post-operatively with no significant between-group differences (P=0.707). Improvement was maintained to Year 5 (median 30 points pre-operatively and 91 points at Year 5 in PD group). Pain score also improved in both groups. There was no functional improvement following TKA in the PD group. In the controls, an increase in KSFS at Year 1 was followed by a return to pre-operative values at Year 5. Complications in the PD group included 1 case of bilateral quadriceps tendon avulsion and 1 dislocation requiring revision. Patients with PD benefit from excellent pain relief following TKA for at least 5 years after surgery

Cephalasporin antibiotics have been commonly used for prophylaxis against surgical site infection. To prevent Clostridium difficile, the preferential use of agents such as flucloxacillin and gentamicin has been recommended. The aim of this study was to investigate the bone penetration of these antibiotics during hip and knee arthroplasty, and their efficacy against Staphylococcus aureus and S. epidermidis. Bone samples were collected from 21 patients undergoing total knee arthroplasty (TKA) and 18 patients undergoing total hip replacement (THA). The concentration of both antibiotics was analysed using high performance liquid chromatography. Penetration was expressed as a percentage of venous blood concentration. The efficacy against common infecting organisms was measured using the epidemiological cut-off value for resistance (ECOFF). The bone penetration of gentamicin was higher than flucloxacillin. The concentration of both antibiotics was higher in the acetabulum than the femoral head or neck (p=0.007 flucloxacillin; p=0.021 gentamicin). Flucloxacillin concentrations were effective against S. aureus and S. epidermis in all THAs and 20 (95%) TKAs. Gentamicin concentrations were effective against S.epidermis in all bone samples. Gentamicin was effective against S. aureus in 11 (89%) femoral samples. Effective concentrations of gentamicin against S. aureus were only achieved in 4 (19%) femoral and 6 (29%) tibial samples in TKA. Flucloxacillin and gentamicin was found to effectively penetrate bone during arthroplasty. Gentamicin was effective against S. epidermidis in both THA and TKA, while it was found to be less effective against S. aureus during TKA. Bone penetration of both antibiotics was less in TKA than THA

Component malalignment has long been implicated in poor implant survival in Total Knee Arthroplasty (TKA). Malalignment can occur in orientation of bony cuts, and in component cementation/implantation. Several systems exist to aid bony cut alignment (navigation, shape matching), but final implantation technique is common to all TKA. Correction of errors in bony cut alignment at cementation/implantation by surgeons has been described. Changes in alignment at this stage are likely to result in asymmetrical cement penetration, which is implicated in early failure. This study reviewed a consecutive series of 150 primary cemented TKAs using an imageless navigation system (aiming for neutral overall limb alignment). Deviation at implantation was calculated by comparing limb alignment recorded using the trial components with limb alignment recorded with the final implanted components, prior to closure. 136 patients (91%) had a final overall limb alignment within 2° of neutral. Three patients (2%) had a final overall limb alignment greater than 3° from neutral. Deviation occurring at implantation is shown in Figure 1 with deviations distributed around zero (mode 0, median 0.3, range −2 to +4,)

Summary. Revision TKA using CCK prosthesis showed comparable outcome to PS prosthesis in clinical and radiological results. Introduction. In revision total knee arthroplasty (TKA), the goal should be to obtain good motion, function and most importantly stability. The stability depends on remaining soft tissue and implant design. The more the ligaments retain function, the less the implant constraint is needed to achieve stability. With increased constraint, the transfer of joint reaction forces to implant-bone interface may lead to mechanical loosening of the implant. Constrained condylar knee (CCK) prosthesis provides more constraint compared with posterior stabilized (PS) prosthesis. The purpose of this study was to compare the clinical, radiological outcome and survivorship of CCK and PS prosthesis in revision TKA. Materials and Methods. One hundred and twenty-one consecutive revision TKAs using CCK (79 knees) and PS (42 knees) were included. The mean follow-up period was 63.2 months for CCK and 64.8 months for PS. The mean age was 69.5 years and 70.6 years in CCK and PS, respectively. Range of motion (ROM), American Knee Society (AKS) score, Hospital for Special Surgery (HSS) score, complications and failure rate were assessed. Radiographic measurements included tibiofemoral angle and radiolucent lines. The Kaplan-Meier survivorship analysis was performed with an end point of re-revision surgery for any reason and compared between CCK and PS using the Log-rank test. Results. The mean range of motion improved from 97.5° to 115.9° in PS, from 89.5° to 110.1° in CCK. The mean Knee Society knee and functional scores improved from 47.6 and 36.4 to 89.7 and 66.1 with PS, from 53.5 and 41.7 to 79.2 and 66.8 with CCK. The mean Hospital for Special Surgery knee score also improved from 57 to 76.9 and 59.7 to 77.6, respectively. The complication rate was 7.5% (4 cases; 3 recurred infections, 1 instability) in PS and 10.3% (7 cases; 4 recurred infections, 1 periprosthetic fracture, 2 stem tip pains) in CCK. The Kaplan–Meier survivorship analysis revealed that ten year survival of the components was 85.4% for PS and 80.0% for CCK. In all aspects, there were no statistical difference (a p-value of more than 0.05) between PS and CCK. Conclusion. Revision TKA using CCK prosthesis showed comparable outcome to PS prosthesis in clinical and radiological results. CCK prosthesis is a reliable and successful option for prosthesis selection in revision TKA when PS prosthesis is not enough for management of instability

Background. Improving quality and efficiency is a priority in health policy. Several studies have shown a correlation between high hospital volume and improved results of surgery. In Norway, orthopaedic surgeons operate a relatively low number of total knee replacements compared with other countries. The number of total knee replacements has, however, increased significantly over the past 10 years. Some studies have also shown an association between surgeon volume and outcomes after total knee arthroplasty (TKA). Purpose. We wanted to study a possible correlation between prosthesis survival and surgery volume of TKA, both with respect to hospital volume and surgeon volume. Materials and Methods. To examine the annual surgery volume per hospital we used registry data from the Norwegian Arthroplasty Register (NAR) for cemented TKA without patella component from 1994–2009. A questionnaire regarding surgeon volume in 2000 and 2009 was sent to all 58 operating hospitals in Norway, as this is not registered in the NAR. We received responses from 29 hospitals. Surgeon volume was defined by the percentage of surgeons at the hospital who operated 1–10 TKA over a year. Cox regression (without adjustment) was used to estimate proportions without revision and relative differences (RR). Analyses were also done for a subset of commonly used prostheses (AGC, LCS, LCS Complete, Duracon, NexGen, Profix), and for the prosthesis LCS and Profix separately. Results. Median annual hospital volume increased from 19 knees in 1995 to 83 in 2009. Prosthesis survival at 10 years was 92.5% (95% CI: 91.6–93.4) with 1–24 operations annually and 95.3% (93.7–96.8) with 100–149 operations. We found a lower risk of revision for hospitals with 25–49 (RR = 0.86, p = 0.06), 50–99 (RR = 0.81, p = 0.01, 100–149 (RR = 0.59, p<0.001 and 150 or more (RR = 0.48, p <.001) operations per year, compared to the hospitals with 1–24 operations. The analyses for subgroups of prosthesis brands provided similar results, except for the Profix prosthesis where there was no difference between hospital volumes. In 2000, 47% of the Norwegian hospitals had two thirds of the surgeons operating 1–10 TKA annually, but this was reduced to 24% in 2009. High annual surgeon volume gave the best results in our analyses. Conclusion. There has been an increase in the number of TKAs over time, in Norway as in other countries. Surgeon volume has also increased over time. Previously a high number of surgeons performed a low number of total knee arthroplasties, but this has been significantly reduced over the last 10 years. Survival curves from the NAR show a gradual improvement over time regarding the results of TKA. Our study indicated better results for TKA both at higher hospital volume and surgeon volume. Further analyses should investigate whether other factors may have an influence on this result

Aims

To determine whether there is any difference in infection rate at 90 days between trauma operations performed in laminar flow and plenum ventilation, and whether infection risk is altered following the installation of laminar flow (LF).

Patients and Methods

We assessed the impact of plenum ventilation (PV) and LF on the rate of infection for patients undergoing orthopaedic trauma operations. All NHS hospitals in England with a trauma theatre(s) were contacted to identify the ventilation system which was used between April 2008 and March 2013 in the following categories: always LF, never LF, installed LF during study period (subdivided: before, during and after installation) and unknown. For each operation, age, gender, comorbidity, socio-economic deprivation, number of previous trauma operations and surgical site infection within 90 days (SSI90) were extracted from England’s national hospital administrative Hospital Episode Statistics database. Crude and adjusted odds ratios (OR) were used to compare ventilation groups using hierarchical logistic regression. Subanalysis was performed for hip hemiarthroplasties.
