Peripheral nerve injury is an uncommon but serious
complication of hip surgery that can adversely affect the outcome.
Several studies have described the use of electromyography and intra-operative
sensory evoked potentials for early warning of nerve injury. We
assessed the results of multimodal intra-operative monitoring during
complex hip surgery. We retrospectively analysed data collected
between 2001 and 2010 from 69 patients who underwent complex hip
surgery by a single surgeon using multimodal intra-operative monitoring
from a total pool of 7894 patients who underwent hip surgery during
this period. In 24 (35%) procedures the surgeon was alerted to a
possible lesion to the sciatic and/or femoral nerve. Alerts were
observed most frequently during peri-acetabular osteotomy. The surgeon
adapted his approach based on interpretation of the neurophysiological changes.
From 69 monitored surgical procedures, there was only one true positive
case of post-operative nerve injury. There were no false positives
or false negatives, and the remaining 68 cases were all true negative.
The sensitivity for predicting post-operative nerve injury was 100%
and the specificity 100%. We conclude that it is possible and appropriate
to use this method during complex hip surgery and it is effective
for alerting the surgeon to the possibility of nerve injury.

Aims. The aim of this study was to assess the clinical and radiological results of patients who were revised using a custom-made triflange acetabular component (CTAC) for component loosening and pelvic discontinuity (PD) after previous total hip arthroplasty (THA). Methods. Data were extracted from a single centre prospective database of patients with PD who were treated with a CTAC. Patients were included if they had a follow-up of two years. The Hip Disability and Osteoarthritis Outcome Score (HOOS), modified Oxford Hip Score (mOHS), EurQol EuroQoL five-dimension three-level (EQ-5D-3L) utility, and Numeric Rating Scale (NRS), including visual analogue score (VAS) for pain, were gathered at baseline, and at one- and two-year follow-up. Reasons for revision, and radiological and clinical complications were registered. Trends over time are described and tested for significance and clinical relevance. Results. A total of 18 females with 22 CTACs who had a mean age of 73.5 years (SD 7.7) were included. A significant improvement was found in HOOS (p < 0.0001), mOHS (p < 0.0001), EQ-5D-3L utility (p = 0.003), EQ-5D-3L NRS (p = 0.013), VAS pain rest (p = 0.008), and VAS pain activity (p < 0.0001) between baseline and final follow-up. Minimal clinically important improvement in mOHS and the HOOS Physical Function Short Form (HOOS-PS) was observed in 16 patients (73%) and 14 patients (64%), respectively. Aims. Improvements in functional results and long-term survival are variable following conversion of hip fusion to total hip arthroplasty (THA) and complications are high. The aim of the study was to analyze the clinical and functional results in patients who underwent conversion of hip fusion to THA using a consistent technique and uncemented implants. Methods. A total of 39 hip fusion conversions to THA were undertaken in 38 patients by a single surgeon employing a consistent surgical technique and uncemented implants. Parameters assessed included Harris Hip Score (HHS) for function, range of motion (ROM), leg length discrepancy (LLD), satisfaction, and use of walking aid. Radiographs were reviewed for loosening, subsidence, and heterotopic ossification (HO). Postoperative complications and implant survival were assessed. Results. At mean 12.2 years (2 to 24) follow-up, HHS improved from mean 34.2 (20.8 to 60.5) to 75 (53.6 to 94.0; p < 0.001). Mean postoperative ROM was flexion 77° (50° to 95°), abduction 30° (10° to 40°), adduction 20° (5° to 25°), internal rotation 18° (2° to 30°), and external rotation 17° (5° to 30°). LLD improved from mean -3.36 cm (0 to 8) to postoperative mean -1.14 cm (0 to 4; p < 0.001). Postoperatively, 26 patients (68.4%) required the use of a walking aid. The Bernese periacetabular osteotomy (PAO) is not indicated for growing hips as it crosses the triradiate cartilage in its posterior branch, and experimental work has shown this can induce substantial deformations, similar to posttraumatic dysplasia, which is observed after pelvis crash injuries in childhood. Upon examination, all injuries in the 19 cases of posttraumatic dysplasia described in literature plus 16 hips of our personal collection took place before the age of 6, which is striking as pelvic injuries in children increase with age. Based on this observation, we started to extend the PAO indication to severe dysplasias in children with open growth plate, initially aged 9 years and older. Following the positive results, it was extended further, our youngest patient being 5 years old. We retrospectively examined radiographic outcomes of 23 hips (20 patients), aged 10.6±1.8 years [range 5.0 – 13.2], operated by us in four centers. Pre- and 3-months postoperative, and the latest FUP radiograph at growth plate closure were measured. We evaluated the acetabular index (AI), lateral center-edge (LCE), ACM-value and compared them with reference values adjusted for age. The age at triradiate cartilage closure was compared with the non-operated side. The follow-up time was 5.4±3.7 years [0.8 - 12.7]. In 5 hips, growth plate closure was delayed by a few months. All angles significantly normalized after PAO (LCE: 14±8° → 38±11°, AI: 20±8° → 7±4°, ACM: 53±5° → 48±4°), with >80% of them severe pathological pre-PAO, none afterwards. Aims. Severe acetabular bone loss and pelvic discontinuity (PD) present particular challenges in revision total hip arthroplasty. To deal with such complex situations, cup-cage reconstruction has emerged as an option for treating this situation. We aimed to examine our success in using this technique for these anatomical problems. Patients and Methods. We undertook a retrospective, single-centre series of 35 hips in 34 patients (seven male, 27 female) treated with a cup-cage construct using a trabecular metal shell in conjunction with a titanium cage, for severe acetabular bone loss between 2011 and 2015. The mean age at the time of surgery was 70 years (42 to 85) and all patients had an acetabular defect graded as Paprosky Type 2C through to 3B, with 24 hips (69%) having PD. The mean follow-up was 47 months (25 to 84). Results. The cumulative five-year survivorship of the implant with revision for any cause was 89% (95% confidence interval (CI) 72 to 96) with eight hips at risk. Total hip joint replacement (THJR) for high riding congenital hip dislocation (CDH) is often performed in young patients, and presents unique problems with acetabular cup placement and leg length inequality. A database and the NZ Joint Registry were used to identify 76 hips in 57 patients with a diagnosis of CDH who underwent THJR in the Wellington region between 1994 and 2015. Records and radiographs of 46 hips in 36 patients classified pre-operatively as Crowe II, III or IV were reviewed. approaches were used to expose the lateral cutaneous nerve of the
thigh and the femoral, obturator and sciatic nerves in order to
study the risk of injury to these structures during the dissection,
osteotomy, and acetabular reorientation stages of a Bernese peri-acetabular
osteotomy. Injury of the lateral cutaneous nerve of thigh was less likely
to occur if an osteotomy of the anterior superior iliac spine had
been carried out before exposing the hip. The obturator nerve was likely to be injured during unprotected
osteotomy of the pubis if the far cortex was penetrated by >
5 mm.
This could be avoided by inclining the osteotome 45° medially and
performing the osteotomy at least 2 cm medial to the iliopectineal

Sciatic nerve palsy following total hip arthroplasty
(THA) is a relatively rare yet potentially devastating complication.
The purpose of this case series was to report the results of patients
with a sciatic nerve palsy who presented between 2000 and 2010,
following primary and revision THA and were treated with neurolysis.
A retrospective review was made of 12 patients (eight women and
four men), with sciatic nerve palsy following THA. The mean age
of the patients was 62.7 years (50 to 72; standard deviation 6.9).
They underwent interfascicular neurolysis for sciatic nerve palsy,
after failing a trial of non-operative treatment for a minimum of
six months. Following surgery, a statistically and clinically significant
improvement in motor function was seen in all patients. The mean
peroneal nerve score function improved from 0.42 (0 to 3) to 3 (1
to 5) (p <
0.001). The mean tibial nerve motor function score
improved from 1.75 (1 to 4) to 3.92 (3 to 5) (p = 0.02).The mean
improvement in sensory function was a clinically negligible 1 out
of 5 in all patients. In total, 11 patients reported improvement
in their pain following surgery. . We conclude that neurolysis of the
Total hip replacement (THR) after acetabular
fracture presents unique challenges to the orthopaedic surgeon.
The majority of patients can be treated with a standard THR, resulting

Introduction. Pelvic discontinuity is a challenging complication. One treatment option that has garnered enthusiasm is acetabular distraction. This method obtains stability via distraction of the discontinuity and placement of an oversized socket (± augments) and elastic recoil of the pelvis. The aims of this study were to report implant survivorship, radiographic results, clinical outcomes, and complications of acetabular distraction for pelvic discontinuity in the largest series to date. Methods. We retrospectively identified all revision THAs with a Paprosky 3B defect and pelvic discontinuity between 2005 and 2017. Of the 162 patients, 32 were treated with distraction. The mean distraction achieved was 5mm (range, 3–8mm). In addition to distraction with a hemispherical cup, augments were utilized in 3 and cages in 19. The mean age at revision was 68 years with 75% female. Mean follow-up was 3 years. Results. The 2-year survivorships free from revision for aseptic loosening, revision, and reoperation were 95%, 75%, and 69%, respectively. There were 3 revisions including one for instability, one for infection, and one for aseptic loosening. At last follow-up, 16 patients had radiographic evidence of cup osteointegration to ilium and ischium, 13 to ilium only, and 3 without evidence of osteointegration (two distracted 3mm and one distracted 6mm). 19 discontinuity lines persisted while 13 had radiographically healed. Only those with osteointegration to both ilium and ischium had evidence of bone graft consolidation and discontinuity line healing. Mean HHS improved from 43 preoperatively to 77 postoperatively (p<0.0001). The most common complication was a postoperative
In order to release the contracture band completely without damaging normal tissues (such as the sciatic nerve) in the surgical treatment of gluteal muscle contracture (GMC), we tried to display the relationship between normal tissue and contracture bands by magnetic resonance neurography (MRN) images, and to predesign a minimally invasive surgery based on the MRN images in advance. A total of 30 patients (60 hips) were included in this study. MRN scans of the pelvis were performed before surgery. The contracture band shape and external rotation angle (ERA) of the proximal femur were also analyzed. Then, the minimally invasive GMC releasing surgery was performed based on the images and measurements, and during the operation, incision lengths, surgery duration, intraoperative bleeding, and complications were recorded; the time of the first postoperative off-bed activity was also recorded. Furthermore, the patients’ clinical functions were evaluated by means of Hip Outcome Score (HOS) and Ye et al’s objective assessments, respectively.Aims

Evaluation of the anatomical features, details of surgical technique and results of the THA in patients with CDH (type C1 and C2 by G. Hartofilakidis). From 2001 to 2016 years one surgical team performed 683 THA in patients with CDH. We retrospectively studied 561 total hip arthroplasties in 349 patients, follow-up rate was 82.1%, from 12 to 188 months (mean 69.4). The results were evaluated by clinical examination, X-rays analysis, Harris Hip Score. Unilateral high hip dislocation was observed in 175 patients (31.2%), in these cases often have underdeveloped half of the pelvis on the side of the dislocation. Type C1 was observed in 326 cases and type C2 – in 235 cases. Type C1 in comparison with C2 has less leg length discrepancy, developed shape of proximal femur, presence of supraacetabular osteophyte. The mean displacement of femoral head was 47.6 mm (from 29 to 55) for C1 and 63.4 mm (from 41 to 78) for C2. Average offset in C1 was 50.1 mm (37–63) and in C2 − 44.3 mm (34–52). Shortening osteotomy by T. Paavilainen performed in 165 cases (50.6%) with C1 dysplasia and in 235 cases (100%) with C2. The features of surgical technique were small size of the cups with obligatory additional screw fixation of the cup and small offset of the stems. The cup was positioned into the true acetabulum in 99.1% cases of C2 type, for C1 – only 69.0%). The cups size 44 mm were used in 97.3% cases for type C2 and in 78.6% cases for type C1. For shortening osteotomy in 76.3% cases Wagner Cone stems were used. Early complications included 9 dislocations (1.6%), 8 femoral nerve neuropathies (1.4%) and 3 infections (0.5%). There is no sciatic nerve palsy. Late complications included dislocation in two hips (1.1%), nonunion of the greater trochanter (8.4%), aseptic loosening of the femoral component − 2 (0.8 %), aseptic loosening of the cup − 11 (1.6%). Average Harris Hip score improved from 39.5 to 83.6 with unsignificant diffence between types C1 and C2 (from 37.3 to 81.4 and from 40.4 to 85.1 consequently). Revision rate was 2.1% for type C1 and 5.5% for type C2. Hip replacement surgery in patients with high hip dislocation is very challenging. The aim of this study was to compare open reduction and internal fixation (ORIF) with revision surgery for the surgical management of Unified Classification System (UCS) type B periprosthetic femoral fractures around cemented polished taper-slip femoral components following primary total hip arthroplasty (THA). Data were collected for patients admitted to five UK centres. The primary outcome measure was the two-year reoperation rate. Secondary outcomes were time to surgery, transfusion requirements, critical care requirements, length of stay, two-year local complication rates, six-month systemic complication rates, and mortality rates. Comparisons were made by the form of treatment (ORIF vs revision) and UCS type (B1 vs B2/B3). Adult patients with history of childhood infection pose a surgical challenge for total hip arthroplasty (THA) due to distorted bony anatomy, soft-tissue contractures, risk of reinfection, and relatively younger age. Therefore, the purpose of the present study was to determine clinical outcome, reinfection rate, and complications in patients with septic sequelae after THA. A retrospective analysis was conducted of 91 cementless THAs (57 male and 34 female) performed between 2008 and 2017 in patients who had history of hip infection during childhood. Clinical outcome was measured using Harris Hip Score (HHS) and Modified Merle d’Aubigne and Postel (MAP) score, and quality of life (QOL) using 12-Item Short Form Health Survey Questionnaire (SF-12) components: Physical Component Score (PCS) and Mental Component Score (MCS); limb length discrepancy (LLD) and radiological assessment of the prosthesis was performed at the latest follow-up. Reinfection and revision surgery after THA for any reason was documented.Aims
