Aims

Ankle fractures are common, mainly affecting adults aged 50 years and over. To aid recovery, some patients are referred to physiotherapy, but referral patterns vary, likely due to uncertainty about the effectiveness of this supervised rehabilitation approach. To inform clinical practice, this study will evaluate the effectiveness of supervised versus self-directed rehabilitation in improving ankle function for older adults with ankle fractures.

Methods

This will be a multicentre, parallel-group, individually randomized controlled superiority trial. We aim to recruit 344 participants aged 50 years and older with an ankle fracture treated surgically or non-surgically from at least 20 NHS hospitals. Participants will be randomized 1:1 using a web-based service to supervised rehabilitation (four to six one-to-one physiotherapy sessions of tailored advice and prescribed home exercise over three months), or self-directed rehabilitation (provision of advice and exercise materials that participants will use to manage their recovery independently). The primary outcome is participant-reported ankle-related symptoms and function six months after randomization, measured by the Olerud and Molander Ankle Score. Secondary outcomes at two, four, and six months measure health-related quality of life, pain, physical function, self-efficacy, exercise adherence, complications, and resource use. Due to the nature of the interventions, participants and intervention providers will be unblinded to treatment allocation.

The etiology of the flexion contracture is related to recurrent effusions present in a knee with end-stage degenerative joint disease secondary to the associated inflammatory process. These recurrent effusions cause increased pressure in the knee causing pain and discomfort. Patients will always seek a position of comfort, which is slight flexion. Flexion decreases the painful stimulus by reducing pressure in the knee and relaxing the posterior capsule. Unfortunately, this self-perpetuating process leads to a greater degree of contracture as the disease progresses. Furthermore, patients rarely maintain the knee in full extension. Even during the gait cycle the knee is slightly flexed. As their disease progresses, patients limit their ambulation and are more frequently in a seated position. Patients often report sleeping with a pillow under their knee or in the fetal position. All of these activities increase flexion contracture deformity. Patients with excessive deformity >40 degrees should be counseled regarding procedural complexity and that increasing constraint may be required. Patients are seen preoperatively by a physical therapist and given a pre-arthroplasty conditioning program. Patients with excessive flexion contracture are specifically instructed on stretching techniques, as well as quadriceps rehabilitation exercises. The focus in the postoperative physiotherapy rehabilitation program continues toward the goal of full extension. Patients are instructed in appropriate stretching regimes. Patients are immobilised for the first 24 hours in full extension with plaster splints, such as with a modified Robert Jones dressing. This dressing is removed on postoperative day one. The patient is then placed in a knee immobiliser and instructed to wear it at bed rest, during ambulation and in the evening, only removing for ROM exercises. In cases of severe flexion deformity >30 degrees, patients are maintained in full extension for 3–4 weeks until ROM is begun. Patients are encouraged to use a knee immobiliser for at least the first 6 weeks postoperatively. Treating patients with flexion contracture involves a combination of bone resection and soft tissue balance. One must make every effort to preserve both the femoral and tibial joint line. In flexion contracture the common error is to begin by resecting additional distal femur, which may result in joint line elevation and mid-flexion instability. The distal femoral resection should remove that amount of bone being replaced with metal. Attention should be directed at careful and meticulous balance of the soft tissues and release of the contracted posterior capsule with re-establishment of the posterior recess, which will correct the majority of flexion contractures. Inability to achieve ROM after TKA represents a frustrating complication for both patient and surgeon. Non-operative treatments for the stiff TKA include shoe lift in contralateral limb, stationery bicycle with elevated seat position, extension bracing, topical application of hand-held instruments to areas of soft tissue-dysfunction by a trained physical therapist over several outpatient sessions, and use of a low load stretch device. Manipulation under anesthesia is indicated in patients after TKA having less than 90 degrees ROM after 6 weeks, with no progression or regression in ROM. Other operative treatments range from a downsizing exchange of the polyethylene bearing to revision with a constrained device and low-dose irradiation in cases of severe arthrofibrosis

Background. Chronic pain is a complex condition that demonstrates better outcomes in multidisciplinary rehabilitation, typically delivered to groups of patients by tertiary healthcare teams. An inter-disciplinary pain management course for individual patients was developed to increase the scope of physical therapists working in primary care by integrating osteopathic manual therapy with psychological interventions from Acceptance and Commitment Therapy (ACT), a form of ‘3rd wave’ Cognitive Behaviour Therapy. Method and Results. A single cohort study with pre-course (n=180) and post-course (n=79) self-report measures (44% response rate) evaluated six week interventions which combined individual manual therapy with self-management, delivered by teams of qualified and student osteopaths. Data included: quality of life (European Quality of Life Questionnaire); pain, mood and coping (Bournemouth Questionnaire); psychological flexibility (Revised Acceptance and Action Questionnaire); and mindfulness (Freiburg Mindfulness Inventory). Participants were predominantly female (68%), unemployed (59%), with an average age of 49 and pain duration of more than 12 months (86%). Commonly reported symptoms were low back pain (82%), neck pain (60%) and multiple sites (86%). At six months, there were statistically significant improvements in all four outcome measures (p<0.0005), with promising effect sizes in quality of life and pain coping (r=0.52) which appeared to be mediated by changes in psychological flexibility. Conclusions. This innovative, integrated, patient-centred chronic pain management course demonstrated promising outcomes when delivered by osteopaths with varying experience. Randomised clinical trials are now needed to assess outcomes in comparison with standard care, and optimal ways of training physical therapists to deliver effective psychological interventions. Background. Early mobilization after total joint arthroplasty decreases postoperative complications and optimizes future functional outcomes. The purpose of this study was to evaluate the effectiveness of a supervised patient ambulation program following total joint arthroplasty. Methods. In 2014, our institution initiated a mandatory supervised patient ambulation program termed “No One Walks Alone” (NOWA). The program requires that all patients who mobilize are accompanied by a nurse or physical therapist. Gait belts are worn whenever a patient ambulates. Bed and chair alarms are utilized to alert if a patient tries to ambulate alone. If a patient experiences a fall, he or she is immediately evaluated by the nursing staff, and details regarding the fall and any injury sustained are documented. A retrospective review of patients undergoing total knee or hip arthroplasty between 2011–2017 was conducted. Patients who had surgery between 2011–2013, before the program, were included in the pre-implementation group (N= 3,069) and those having surgery between 2016–2017, after the program started, were included in the post-implementation group (N=3,947). The incidence of patient falls, fall-related complications were compared between groups. Results. The number of patient falls was 23 in the pre-implementation group (0.7%) and 9 in the post-implementation group (0.2%). Fall-related complications occurred in 14 of 23 falls in the pre-implementation group (0.5% overall) and 2 of 9 falls in the post-implementation group (0.05% overall) (p<0.001). Patients who experienced a fall were more likely to have undergone total knee arthroplasty (81.3%) compared to those who did not fall (58%) (p=0.008). Conclusion. A supervised patient ambulation program can be successful at reducing the incidence of patient falls and complications related to falls in the immediate postoperative setting

Introduction. Traditional Total Knee Arthpolasty (TKA) replaces all 3 compartments of the knee for patients diagnosed with OA. There might be functional benefit to replacing only damaged compartments, and retaining the normal ligamentous structures. There is a long history of performing multi-compartment arthroplasty with discrete components. Laskin reported in 1976 that good pain relief and acceptable clinical results were achieved at two years in patients with bi-unicondylar knee replacement [Laskin 1976]. Other authors also have reported on bi-unicompartmental knee arthroplasty achieving successful clinical outcomes [Stockley 1990; Confalonieri 2005]. Banks et al. reported that kinematics of bi-unicompartmental arthroplasties during gait demonstrated some of the basic features of normal knee kinematics [Banks 2005]. These reports suggest that a modular approach to resurfacing the knee can be successful and achieve satisfactory clinical and functional results. Objective. The primary objective of this study is to compare the functional outcomes of three patient groups treated for osteoarthritis. Methods. Subjects received either a modular, multicompartment knee arthroplasty (MKA) implanted with robotic-arm assistance(MAKO Surgical Corp., Fort Lauderdale, FL), a computer assisted TKA (TKA CAS) or a TKA implanted using traditional manual instrumentation (TKA T). Patients that were eligible to receive a TKA were randomly selected to receive computer assisted or traditional surgical technique and blinded to the type of TKA surgical technique utilized. We report post-operative functional outcomes including Range of Motion (ROM), Timed-up and go(TUG), and Quad strength at time intervals of 2 weeks, 6 weeks, 3 months and 6 months. The TUG test is a validated measure of patient mobility where a patient is asked to stand up from a chair, walk three meters turn around and sit back down [Boonstra, 2008]. The Quad strength assessment is measured with a hand held dynamometer (Lafayette Instruments, Lafayette, IN) while patient was seated with leg at 90 degrees flexion. The patient is asked to extend their knee while a physical therapist provides resistive forces to maintain static knee flexion. All tests were administered by one physical therapist. Results. Patients that underwent MKA saw significant increase in ROM post-operatively when compared to TKA CAS patients (P<0.009) and TKA T patients (p<0.003), Figure 1. Patients that underwent MKA also saw an increase in Quad Strength, however this was only statistically significant between the MKA and TKA CAS groups, (P<0.04), Figure 2. Patients that underwent MKA saw a reduction in TUG which indicated an improved mobility post-operatively, Figure 3. The reduced TUG was only statistically significant for MKA patients compared to TKA T patients (P<0.005). There was no statistical significance seen between the two TKA groups for any functional measure. Discussion. Initial findings do indicate a short term improvement in functional outcomes for MKA patients when compared to TKA patients. Additional data clinical and functional data is being collected and enrollment is continuing for this study

The primary purpose of this study was to assess whether patients presenting with clinical graft laxity following primary anatomic anterior cruciate ligament (ACL) reconstruction using hamstring autograft reported a significant difference in disease-specific quality-of-life (QOL) as measured by the ACL-QOL questionnaire. Clinical ACL graft laxity was assessed in a cohort of 1134/1436 (79%) of eligible patients using the Lachman and Pivot-shift tests pre-operatively and at 12- and 24-months following ACL reconstruction. Post-operative ACL laxity was assessed by an orthopaedic surgeon and a physical therapist who were blinded to each other's examination. If there was a discrepancy between the clinical examination findings from these two assessors, then a third impartial examiner assessed the patient to ensure a grading consensus was reached. Patients completed the ACL-QOL questionnaire pre-operatively, and 12- and 24-months post-operatively. Descriptive statistics were used to assess patient demographics, rate of post-operative ACL graft laxity, surgical failures, and ACL-QOL scores. A Spearman rho correlation coefficient was utilised to assess the relationships between ACL-QOL scores and the Lachman and Pivot-shift tests at 24-months post-operative. An independent t-test was used to determine if there were differences in the ACL-QOL scores of subjects who sustained a graft failure compared to the intact graft group. ACL-QOL scores and post-operative laxity were assessed using a one-way analysis of variance (ANOVA). There were 70 graft failures (6.17%) in the 1134 patients assessed at 24-months. A total of 226 patients (19.9%) demonstrated 24-months post-operative ACL graft laxity. An isolated positive Lachman test was assessed in 146 patients (12.9%), an isolated positive Pivot-shift test was apparent in 14 patients (1.2%), and combined positive Lachman and Pivot-shift tests were assessed in 66 patients (5.8%) at 24-months post-operative. There was a statistically significant relationship between 24-month post-operative graft laxity and ACL-QOL scores (p < 0.001). Specifically, there was a significant correlation between the ACL-QOL and the Lachman test (rho = −0.20, p < 0.001) as well as the Pivot-shift test (rho = −0.22, p < 0.001). There was no significant difference between the scores collected from the graft failure group prior to failure occurring (mean = 74.38, SD = 18.61), and the intact graft group (mean = 73.97, SD = 21.51). At 24-months post-operative, the one-way ANOVA demonstrated a statistically significant difference between the ACL-QOL scores of the no laxity group (mean = 79.1, SD = 16.9) and the combined positive Lachman and Pivot-shift group (mean = 68.5, SD = 22.9), (p = 0, mean difference = 10.6). Two-years post ACL reconstruction, 19.9% of patients presented with clinical graft laxity. Post-operative graft laxity was significantly correlated with lower ACL-QOL scores. The difference in ACL-QOL scores for patients with an isolated positive Lachman or Pivot-shift test did not meet the threshold of a clinically meaningful difference. Patients with clinical laxity on both the Lachman and Pivot-shift tests demonstrated the lowest patient-reported ACL-QOL scores, and these results exceeded the minimal clinically important difference

Background. Total Knee Arthroplasty (TKA) provides patients with significant improvements in quality of life. Subjective patient reported outcome measures (PROMs) are traditionally used to measure preoperative functional status and postoperative outcomes. However, there are limitations to PROMs. In particular, they provide virtually no functional information in the first 3 weeks after surgery, which could be used to guide the patient's recovery. Newly available wearable electronic sensors make it possible to: 1) measure important functional outcomes following TKA; 2) guide the patient's physical therapy (PT); and 3) provide real-time functional and clinical information to the provider. Compliance with PT after TKA is a challenge. Patients cite time, transportation, and cost as deterrents to PT appointments. However, an intensive PT program is essential in TKA. Surface sensor devices may be able to increase PT compliance by guiding patients through exercises at home. Additionally, these devices can transmit PT progress in real-time to the providers, allowing them to monitor and assist the patient's recovery. Our study investigates the feasibility of using a surface sensor device (TracPatch™) on patients following TKA. We sought to answer the following questions: 1) Will patients tolerate the device; 2) Will patients comply with device instructions; 3) Will patients be able to use the smart phone application; 4) Will the device collect, transmit, and store data as it was designed? We believe these fundamental questions must be answered as we enter the era of personal sensor-measured functional outcomes. Methods. 20 patients undergoing primary, unilateral TKA were enrolled in this IRB approved study. At the pre-surgical visit, patients were given instructions for the device and smart phone application. Each patient used the device in the week prior to surgery, and data was collected. The device was again applied in the operating room. For 3 weeks post-operatively, the device collected functional data, along with WOMAC, OKS, KSS, PROMIS, and VAS pain scores. A satisfaction survey was collected on the device. Results. The study results emphasize the importance of clear device instructions. Using the sensor and phone application prior to surgery was very helpful. The device was surprisingly well tolerated. Older patients were able to use the device without significant difficulty. Virtually all patients found the device helpful and, often fun. Physical therapists felt that the devices helped personalize the therapy program. The functional information from the device was much more helpful in guiding care in the first 3 weeks following surgery than PROM scores. Conclusion. It is anticipated that sensor devices of the kind tested in this study will have a major impact on the care of TKA patients. The purpose of this study was not to measure that impact. Our goal was to examine the factors that optimize the use of these devices. It is critical that clear device instructions be given to patients. Office procedures must be established to monitor use of the devices. If protocols are established for their use, surface sensors have the potential to provide invaluable information to TKA patients and caregivers

The aim of this study was to assess the incidence, management and survival of unstable pelvic ring injuries in patient aged 65 years or older. Prospectively kept data was analysed from April 2008 to October 2016. Information regarding the mechanism, fracture type, associated injuries, treatment and complications of the treatment were collected. Annual incidence was calculated and a Kaplan Meier survival analysis for carried out at 30 days, 1 year and 5 years. 404 patient records were available. 125 were 65 years or older (60 males and 65 females). 24 (19%) patients required surgical stabilisation to permit mobilisation the remaining 101 patients, treated conservatively were mobilised with immediate weight-bearing under the supervision of a physical therapist with assistive devices. Mean age was 73.5 years (SD 9.9 yrs). Fracture types were − 61.B2 47(37.6%), 61.B1 24(32%), 61.A2 17(13.6%), 61.C1 16(12.8%), 61.C2 5(4%), 61.A1 2(1.6%) and 61.C3 3(2.4%). Mechanisms of injury included fall from standing height − 41 (32%), road traffic collisions − 46(36.8%), fall from higher than standing height − 10(8%), fall from horse − 6(4.8%), jumped from bridge − 3(2.4%) & others 19(15%). Complications in surgical group included 1 death from PE and 1 wound infection treated with vacuum assisted dressing. Survivorship was 91.7%(30 days), 82.5%(1 year) and 49.7%(5 years). Most common fracture type is 61.B2. Over one third of fractures resulted from low energy mechanism. The majority 81% could be managed conservatively. One-year survival figure closely resembles the fracture neck of femur group, highlighting the frailty of this population

INTRODUCTION. Physical therapy(PT) is an integral component in the management of musculoskeletal conditions. On the other hand, there have been few reports exclusively dedicated to studying PT interventions on the same day of total hip arthroplasty(THA). In this study, we investigate the role of rehabilitation in the early postoperative period on length of stay (LOS), total medical cost, and physical recovery following total hip arthroplasty. METHODS. A prospective cohort study was carried out 104 consecutive patients who underwent 107 primary THA performed by two surgeons. Data were gathered on all patients who underwent operative management from June2016 to June 2017. Institutional review board approval was obtained before performing this study. Patient demographic, physical, and clinical dates were collected for all patients, including age, gender, body mass index (BMI), diagnosis, Japan Orthopedic Association (JOA) hip score, Japanese Orthopedic Association Hip-Disease Evaluation Questionnaire (JHEQ) score, 3min walk test, and Timed up and go (TUG) test. The patient population consisted of 5men and 99women, with an average age of 66.0 years (range, 50–84 years). There were no statistically significant differences between patients who did and did not receive PT with regard to demographic, medical, and surgical data, including gender, age, BMI, JOA hip score, JHEQ score, preoperative 3min walk test, preoperative TUG test(Table 1). All patients underwent direct anterior approach THA through navigation system. Postoperative day (POD) 0 was defined as the same day of surgery. There were no standardized criteria by which patients were selected for participation in rehabilitation with physical therapists. Patient selection for POD 0 rehabilitation was based on the end of surgery time. For instance, when the end of surgery time was in the forenoon, the patients were received POD 0 PT. In contrast, patients who ended operation in the afternoon were classified POD 1 PT. Rehabilitation protocol was adjusted based on surgical approach, and all patients were weight bearing as tolerated. TUG test and 3min walk test was done by a physiotherapist on the seventh day postoperatively. RESULTS. Patients who received PT on POD 0 were compared with patients who received PT on POD 1. (Table2) Using the operative start time to determine LOS, patients who received therapy on POD 0 stayed an average of 14.1±4.8 days, and those who received therapy on POD 1 stayed an average of 19.2±9.1 days. The LOS was statistically significantly different between groups (P = .01). In terms of physical recovery, the TUG test received therapy on POD 0 was taken an average of 14.0±6.0 seconds, and the TUG test received therapy on POD 1 was an average of 17.6±9.4 seconds. (P=.04) Furthermore, Total cost on POD0. Day 0 patients had a mean cost of ¥1,970,000±21,000 and Day 1 had a mean cost of 2,190,000±49,600, which remained significance difference(P=.01). CONCLUSION. This study suggests that early rehabilitation and patient mobilization on the date of surgery is important to shorten length of hospital stay, decrease total medical cost and to achieve faster physical recovery

Rehabilitation systems based on inertial measurement units (IMU) and bio-feedbacks are increasingly used in many different settings for patients with neurological disorders such as Parkinson disease or balance impairment, and more recently for functional recover after orthopedic surgical interventions or injuries especially concerning the lower limb. These systems claim to provide a more controlled and correct execution of the motion exercises to be performed within the rehabilitation programs, hopefully resulting in a better outcomes with respect to the traditional direct support of a physical therapists. In particular recruitment of specific muscles during the exercise is expression of its correct and finalized execution. The objective of this study was to compare muscular activation patterns of relevant lower limb muscles during different exercises performed with traditional rehabilitation and with a new validated system based on IMU and biofeedback (Riablo, Corehab, Trento, Italy). Twelve healthy subjects (mean age 28.1 ± 3.9, BMI 21.8± 2.1) were evaluated in a rehabilitation center. Muscular activation pattern of gluteus maximum, gluteus medium, rectus femoris and biceps femoris was recorded through surface EMG (Cometa; Milan) during six different motion tasks: hip abduction in standing position, lunge, hip flexion with extended knee in standing position, lateral lunge, hip abduction with extended knee in lateral decubitus, squat. Subjects performed 10 repetitions of each task for a total of 100 repetitions per motion task, with and without Riablo System as well as during standard rehabilitation. An additional IMU was positioned on the shank in order to detect beginning and end of each repetition. A single threshold algorithm was used to identify muscle activation timing. During hip abduction in standing position, gluteus maximum and rectus femoris showed a better and longer activation pattern while using Riablo compared to traditional rehabilitation. Gluteus medium showed a similar activation pattern whereas biceps femoris showed no activation from 30% to 80% using Riablo. During squat, rectus femoris and biceps femoris had a similar activation pattern with and without Riablo whereas gluteus maximum and gluteus medium showed a better activation pattern while using Riablo. The recent development of innovative rehabilitation systems meets the need of manageable, reliable and efficient instruments able to reduce rehabilitation costs but with the same good clinical outcomes. Muscular activation patterns of relevant lower limb muscles during selected motion tasks reveal their correct execution. The use of this new rehabilitation system based on IMU and biofeedback seems to allow a more selective and effective muscular recruitment, likely due to the more correct and controlled execution of the exercise, particularly for the identification and interdiction of possible compensation mechanisms

To progress to a same day surgery program for arthroplasty, it is important that we examine and resolve the issues of why patients stay in the hospital. The number one reason is fear and anxiety of the unknown and of surgical pain. The need for hospital stay is also related to risk arising from comorbidities and medical complications. Patients also need an extended stay to manage the side effects of our treatment, including after effects of narcotics and anaesthesia, blood loss, and surgical trauma. The process begins pre-operatively with an appropriate orthopaedic assessment of the patient and determination of the need for surgery. The orthopaedic team must motivate the patient, and ensure that the expectations of the patient, family and surgeon are aligned. In conjunction with our affiliated hospitalist group that performs almost all pre-admission testing, we have established guidelines for patient selection for outpatient arthroplasty. The outpatient surgical candidate must have failed conservative measures, must have appropriate insurance coverage, and must be functionally independent. Previous or ongoing comorbidities that cannot be optimised for safe outpatient care may include: congestive heart failure, or valve disease; chronic obstructive pulmonary disease, or home use of supplemental oxygen; untreated obstructive sleep apnea with a BMI >40 kg/m2; hemodialysis or severely elevated serum creatinine; anemia with hemoglobin <13.0 g/dl; cerebrovascular accident or history of delirium or dementia; and solid organ transplant. Pre-arthroplasty rehabilitation prepares the patient for peri-operative protocols. Patients meet with a physical therapist and are provided with extensive educational materials before surgery to learn the exercises they will need for functional recovery. Enhancement of our peri-operative pain management protocols has resulted in accelerated rehabilitation. The operative intervention must be smooth and efficient, but not hurried. Less invasive approaches and techniques have been shown to decrease pain, reduce length of stay, and improve outcomes, especially in the short term. Between June 2013 and December 2015, 1957 primary knee arthroplasty procedures (1010 total, 947 partial) were performed by the author and his 3 associates at an outpatient surgery center. Seven percent of patients required an overnight stay, with a majority for reasons of convenience related to travel distance or later operative time. Importantly, no one has required overnight stay for pain management. Outpatient arthroplasty is safe, it's better for us and our patients, and it is here now. In an outpatient environment the surgeon actually spends more time with the patients and family in a friendly environment. Patients feel safe and well cared for, and are highly satisfied with their arthroplasty experience

We compared self-reported pain and function, complications and revision rates, and radiographic outcomes of hydroxylapatite(HA) or cemented tibial fixation in the first five years following primary total knee arthroplasty. This was a randomised clinical trial of eighty-one patients. Prospective, randomised clinical trial. Patients less than seventy years of age with non-inflammatory knee arthritis. Eighty-one patients were randomised at the time of surgery to receive HA or cemented tibial fixation. Subjects were evaluated preoperatively, six months, one and five years postoperatively by a physical therapist who was blinded to group allocation. X-rays were evaluated by an experienced arthroplasty surgeon who did not perform any of the surgeries. Self-reported pain and function, the primary outcomes, were measured by the Western Ontario McMaster (WOMAC) Osteoarthritis Index and the RAND 36-item Health Services Inventory (RAND-36). Complications and revision rates were determined through hospital record review and at each patient evaluation. The Knee Society Radiological Score was used to evaluate plain radiographs at each assessment. There was slightly more pain in HA group at six months as measured by both the WOMAC and RAND-36, a difference that disappeared by the one-year assessment. There were no differences in function, radiographic findings or complications at any time. Finally, no subjects required revision of the tibial prosthesis during the study. Overall, no significant differences were seen between groups. The initial difference in self-reported pain disappeared by twelve-months postoperatively. At five-years postoperatively, there is no advantage to HA tibial fixation over cemented tibial fixation

Previous or ongoing comorbidities that contraindicate the outpatient setting include: cardiac – prior revascularization, congestive heart failure, or valve disease; pulmonary – chronic obstructive pulmonary disease, or home use of supplemental oxygen; untreated obstructive sleep apnea – BMI >40 kg/m2; renal disease – hemodialysis or severely elevated serum creatinine; gastrointestinal – history or post-operative ileus or chronic hepatic disease; genitourinary – history of urinary retention or severe benign prostatic hyperplasia; hematologic – chronic Coumadin use, coagulopathy, anemia with hemoglobin <13.0 g/dl, or thrombophilia; neurological – history of cerebrovascular accident or history of delirium or dementia; solid organ transplant. Pre-arthroplasty rehabilitation prepares the patient for peri-operative protocols. Patients meet with a physical therapist and are provided with extensive educational materials before surgery to learn the exercises they will need for functional recovery. Enhancement of our peri-operative pain management protocols has resulted in accelerated rehabilitation. The operative intervention must be smooth and efficient, but not hurried. In 2014, 385 primary partial knee arthroplasty procedures (7 patellofemoral replacement, 13 lateral, and 365 medial) were performed by the author and his 3 associates at an outpatient surgery center. Of those, 348 (95%) went home the same day while 17 (5%) required an overnight stay, with 11 for convenience related to travel distance or later operative time and 6 for medical issues. Outpatient arthroplasty is safe, it's better for us and our patients, and it is here now. In an outpatient environment the surgeon actually spends more time with the patients and family in a friendly environment. Patients feel safe and well cared for, and are highly satisfied with their arthroplasty experience

INTRODUCTION: Range of motion following TKA is a commonly assessed and important outcome parameter. The reported knee ROM is often measured using manual goniometers. The accuracy of goniometer in measuring ROM of the knee is not known. We compared the knee ROM measured with a manual goniometer and compared that to measurements obtained from computer assisted navigation system. METHODS: This prospective ongoing study has so far recruited 60 patients (71 knees) undergoing TKA by a single surgeon. Measurements of the ROM were performed intraoperatively using a 14 inch 360° Nexgen Baseline® goniometer, validated by physical therapists, and the Stryker knee navigation system. ™. Consistent anatomical landmarks were used to obtain flexion and extension measurements. Each goniometer and navigation measurement was performed twice by the same reader. RESULTS: Goniometer was more inaccurate in measuring the knee ROM than the navigation. There was a 5.07° difference between two measurements obtained with the goniometer compared to a mean discrepancy of 1.15° using the navigation system. Further, the ROM measured by navigation was on average 13.9° larger than that measured by the goniometer. BMI affected the ROM recording obtained by both the goniometer and the navigation system. DISCUSSION AND CONCLUSION: This study indicates that goniometer is not an accurate tool for measurements of knee ROM as there is a marked discrepancy between two goniometer readings. The navigation system seemed to produce more consistent, but markedly higher, readings than a manual goniometer. Reported results on knee ROM in the literature need to be interpreted with caution and scrutinized for potential inaccuracy of the measuring tool

Purpose of the study: Therapeutic options for retractile capsulitis ranges from therapeutic abstention to arthroscopic arthrolysis. The purpose of this work was to examine the efficacy of a simple therapeutic option (arthrodistention + self-mobilisation). Material and methods: This was a prospective study of a consecutive series of 41 patients (28 female, 13 male), dominant shoulder 57%. Inclusion criteria were deficient in passive range of motion ≥ 50% compared with the other side in at least two planes, without notion of trauma or surgery. Diagnosis and inclusion: one surgeon. Arthrodistension with corticosteroid injection: one radiologist. Recommendations for self-rehabilitation, the day of the arthrodistension: one physical therapist. Patients were reviewed at 30, 90 and 180 days to analyse pain (visual analogue scale, VAS), daily life activities (Constant score), range of motion. Results: From day 1 to 6 months –VAS regressed from 5.8 (2 – 9) to 0.8 (0/2). Constant daily activity score improved from 1/4 to 3.6/4; FA from 82 (60/115) to 170; (150/180); Re1 from 5 (−10/30) to 50 (20/70); RI from 12 (0 – 30) to 30 (10/60). Recovery was correlated with deficit in RI (p< 0.005). The greater the RI deficit the less rapid the recovery. Discussion: We did not use the overall Constant score because of the difficulty in evaluating force. An analysis of the literature shows that therapeutic abstention can provide recovery, but with a delay of about two years. Arthroscopic arthrolysis, interscalenic blocks provide a much quicker recovery (6 months). The results obtained here are comparable with those obtained with these more complex methods./. Conclusion: This study shows that a simple management strategy enables the same results as with more invasive and more costly techniques. The patient should be warned that an important deficit in RI will undoubtedly lead to a slower recovery

Purpose of the study: Recent data in the literature regarding intra-articular deliver of analgesics during the postoperative period have been encouraging. Patients benefit from optimal analgesia and earlier mobilisation, shortening rehabilitation time and hospital stay and limiting complications. In light of these encouraging results, our institution developed a programme designed to address all postoperative situations associated with implantation of a total knee arthroplasty (TKA). Material and methods: The programme combines pre-operative counselling and a postoperative programme for multimodal anaesthesia in addition to intra-articular analgesia for 24 hours and early mobilisation. We present here the results of this technique in patients undergoing first-intention TKA. We analysed information collected prospectively in all patients who had TKA from January to June 2008: 319 patients in six months. The operation was performed under peridural anaesthesia supplemented by intra-articular ropivacaine delivered by a catheter for 24 hours. Patients were mobilised, or verticalised, the day of surgery according to individual capacities. Data collected included: pain scores, date of the end of physical therapy, and data reviewed at six weeks. Results: A cohort of 305 patients was analysed; 36% of patients were mobilised the day of the operation and 93% on day 2. The rate of urinary catheters was 12% and administration of intravenous fluids 10%. Physical therapists determined that 58% of patients could be discharged on 3 after surgery and 85% on day 5. Eighty-percent of patients were free of nausea or vomiting and had well controlled pain. Regarding function, mean range of motion was 85° at discharge with 31% of patients requiring physical therapy. At six weeks, mean range of motion was 95° and only 5% of patients had lost amplitude (reduction > 10° of range of motion) compared to discharge values. Mean scores on the Oxford questionnaire improved from 43 preoperatively to 26 six weeks postoperatively. Discussion: This multidisciplinary approach guarantees excellent postoperative analgesia with early mobilisation and provides satisfactory results at six weeks. To this can be added the benefit of a lower rate of urinary catheters, administration of intravenous fluids, and physical therapy

Objective: Complex hand function problems in patients with rheumatic diseases may require the expertise of physicians and health professionals from multiple disciplines. The aim of the present study was to describe the characteristics, management strategies and outcomes of patients with rheumatic diseases who were referred to a multidisciplinary hand clinic. Methods: All consecutive patients with complex hand function problems who were referred to a multidisciplinary hand clinic including a rheumatologist, an orthopaedic surgeon, a rehabilitation specialist, an occupational therapist and a physical therapist were included. Of all patients, sociodemographic characteristics, general disease characteristics, the most troubling impairments and limitations regarding hand function and deformities were recorded at baseline. The following measures of hand function were assessed at baseline and 3 months after treatment: the Sequential Occupational Dexterity Assessment (SODA), the Michigan Hand Outcomes Questionnaire (MHQ), the hand/finger function subscale of the Arthritis Impact Measurement Scales (AIMS), grip strength (Jamar dynamometer) and pain (visual analogue scale, VAS, 0–10 cm). Results: Over a period of 28 months 69 patients were included. Basic characteristics, characteristics of hand function, impairments and limitations that were most frequently mentioned as well as the advised management strategies were recorded. In 38/54 patients (70%) the advised treatment was executed and 33 of them completed the follow-up assessment. Conclusion: The most frequently mentioned impairments and limitations of patients with rheumatic diseases and hand function problems pertain to grip ability and grip strength, pain and shaking hands. Management advices, including conservative and surgical treatment, are followed by two-thirds of the patients. On average, patients who are treated improve significantly with respect to grip strength and overall hand function as measured by a questionnaire
