As there is currently no evidenced-based and systematic way of prioritising people requiring JRS we aimed to develop a clinically relevant system to improve prioritisation of people who may require JRS. An important challenge in this area is to accurately assign a queue position and improve list management. To identify priority criteria areas eight workshops were held with surgeons and patients. Domains derived were pain, activity limitations, psychosocial wellbeing, economic impact and deterioration. Draft questions were developed and refined through structured interviews with patients and consultation with consultants. 38 items survived critical appraisal and were mailed to 600 patients. Eleven items survived clinimetric and statistical item reduction. Validation then included co-administration with standardised questionnaires (960 patients), verification of patient MAPT scores through clinical interview, examination of concordance with surgeon global ratings and test-retest. Ninety-six Victorian surgeons weighted items using Discrete Choice Experiments (DCEs). The DCE scaling generated a scale, which clearly ranked patients across the disease continuum. The MAPT differentiated people on or not on waiting lists (p<0.001), and was highly correlated with other questionnaires, e.g., unweighted-MAPT vs WOMAC (r=0.78),
The Oxford mobile bearing knee prosthesis (Zimmer Biomet Inc, Warsaw, Ind) is considered a good treatment option for isolated medial compartment knee arthrosis. From February 2001 until August 2016, 1719 primary Oxford medial unicompartmental knee replacement procedures were completed at our center by a group of seven surgeons. We undertook this study to examine the long-term survivorship of the Oxford unicompartmental knee replacement looking at survivorship and reasons for failure. A retrospective consecutive case series review was completed, and all revisions and re-operations were identified. Conversion to total knee replacement (TKA) was considered a failure. Kaplan-Meier survival analysis was used to calculate the 15-year survivorship of the group overall. We specifically looked at age, gender, BMI and surgeon caseload in addition to the reasons for failure. A statistical analysis was performed and differences in survivorship were compared for the variables listed. A logistic cox regression was performed to explore predictors of revision. Overall 15-year survivorship was 89.9%. Female survivorship of 88.1% was statistically worse than the male group at 91.8% (p=0.018). Younger patients (75yrs of age (p= 0.036). There was a large range in surgical case load by individual surgeons (range 17–570 knees). There were no statistically significant differences in age, BMI, or gender when comparing the individual surgeon groups. There was a large range in 15-year survivorship between individual surgeons (range 78.3% – 95%). Overall the most common reason for revision was due to wear of the unreplaced portion of the knee (lateral and/or patella-femoral joint) followed by aseptic loosening, polyethylene dislocation, infection or persistent pain. The 15-year survivorship results of the Oxford medial unicompartmental knee replacement at our center compares favourably to other published series and large registry data series. We found a reduction in survivorship in female patients and younger patients (< 5 5yrs). There were also significant differences in survivorship based on the individual surgeon. A more selective patient approach yielded the best long-term survivorship and equivalent to that of total knee replacement. We therefore suggest using a more selective approach when choosing patients for a medial unicompartmental knee replacement with the Oxford mobile bearing prosthesis in order to enhance long-term survivorship.

Background. Obesity has been linked with increased rates of knee osteoarthritis. Limited information is available on the survival and functional outcome results of rTKR in the obese patients. This registry-based study aimed to identify whether BMI is an independent risk factor for poorer functional outcomes and /or implant survival in rTKA. Methods. New Zealand Joint Registry (NZJR) data of patients who underwent rTKA from 1st January 2010 to January 2023 was performed. Introduction. With advances in mobile application, digital health is being increasingly used for remote and personalised care. Patient education, self-management and tele communication is a crucial factor in optimising outcomes. Aims. We explore the use of a smartphone app based orthopaedic care management system to deliver personalised surgical experience, monitor patient engagement and functional outcomes of patients undergoing knee arthroplasty. Little information exists regarding optimal tibial stem usage in revision total knee arthroplasty (rTKA) utilising a tibial trabecular metal (TM) cone. The purpose of this study was to compare 1) functional outcomes, 2) radiographic outcomes, and 3) implant survivorship in rTKA utilising TM cones combined with either short stems (SS) or long stems (LS) at minimum two-years clinical follow-up. In this retrospective, multi-centre study, patients undergoing TM cone utilising rTKA between 2008 and 2019 were included. Patients were divided into: SS group (no diaphyseal engagement), and LS group (diaphyseal engagement). All relevant clinical charts and post-operative radiographs were examined.

The use of rotating hinge (RH) prostheses for severe primary as well as revision arthroplasty is widely established. Aim of this study was to investigate long term results of a new RH prosthesis (EnduRo®, B Braun, Germany), which uses carbon-fiber reinforced poly-ether-ether-ketone (CFR PEEK) as a new bearing material, first time used in knee arthroplasty.
