Aims. To evaluate if, for orthopaedic trainees, additional cadaveric simulation training or standard training alone yields superior radiological and clinical outcomes in patients undergoing dynamic hip screw (DHS) fixation or hemiarthroplasty for hip fracture. Methods. This was a preliminary, pragmatic, multicentre, parallel group randomized controlled trial in nine secondary and tertiary NHS hospitals in England. Researchers were blinded to group allocation. Overall, 40 trainees in the West Midlands were eligible: 33 agreed to take part and were randomized, five withdrew after randomization, 13 were allocated cadaveric training, and 15 were allocated standard training. The intervention was an additional two-day cadaveric simulation course. The control group received standard on-the-job training. Primary outcome was implant position on the postoperative radiograph: tip-apex distance (mm) (DHS) and leg length discrepancy (mm) (hemiarthroplasty). Secondary clinical outcomes were procedure time, length of hospital stay, acute postoperative complication rate, and 12-month mortality. Procedure-specific secondary outcomes were intraoperative radiation dose (for DHS) and postoperative blood transfusion requirement (hemiarthroplasty). Results. Eight female (29%) and 20 male trainees (71%), mean age 29.4 years, performed 317 DHS operations and 243 hemiarthroplasties during ten months of follow-up. Primary analysis was a random effect model with surgeon-level fixed effects of patient condition, patient age, and surgeon experience, with a random intercept for surgeon. Under the intention-to-treat principle, for hemiarthroplasty there was better implant position in favour of cadaveric training, measured by leg length discrepancy ≤ 10 mm (odds ratio (OR) 4.08 (95% confidence interval (CI) 1.17 to 14.22); p = 0.027). There were significantly fewer postoperative blood transfusions required in patients undergoing hemiarthroplasty by cadaveric-trained compared to standard-trained surgeons (OR 6.00 (95% CI 1.83 to 19.69); p = 0.003). For DHS, there was no significant between-group difference in implant position as measured by tip-apex distance ≤ 25 mm (OR 6.47 (95% CI 0.97 to 43.05); p = 0.053). No between-group differences were observed for any secondary clinical outcomes. Conclusion. Trainees randomized to additional cadaveric training performed hip fracture fixation with better implant positioning and fewer postoperative blood transfusions in hemiarthroplasty. This effect, which was previously unknown, may be a consequence of the intervention. Further study is required. Cite this article: Bone Jt Open 2023;4(8):602–611

Aims. The primary aim of the survey was to map the current provision of simulation training within UK and Republic of Ireland (RoI) trauma and orthopaedic (T&O) specialist training programmes to inform future design of a simulation based-curriculum. The secondary aims were to characterize; the types of simulation offered to trainees by stage of training, the sources of funding for simulation, the barriers to providing simulation in training, and to measure current research activity assessing the educational impact of simulation. Methods. The development of the survey was a collaborative effort between the authors and the British Orthopaedic Association Simulation Group. The survey items were embedded in the Performance and Opportunity Dashboard, which annually audits quality in training across several domains on behalf of the Speciality Advisory Committee (SAC). The survey was sent via email to the 30 training programme directors in March 2019. Data were retrieved and analyzed at the Warwick Clinical Trials Unit, UK. Results. Overall, 28 of 30 programme directors completed the survey (93%). 82% of programmes had access to high-fidelity simulation facilities such as cadaveric laboratories. More than half (54%) had access to a non-technical skills simulation training. Less than half (43%) received centralized funding for simulation, a third relied on local funding such as the departmental budget, and there was a heavy reliance on industry sponsorship to partly or wholly fund simulation training (64%). Provision was higher in the mid-stages (ST3-5) compared to late-stages (ST6-8) of training, and was formally timetabled in 68% of prostgrammes. There was no assessment of the impact of simulation training using objective behavioural measures or real-world clinical outcomes. Conclusion. There is currently widespread, but variable, provision of simulation in T&O training in the UK and RoI, which is likely to expand further with the new curriculum. It is important that research activity into the impact of simulation training continues, to develop an evidence base to support investment in facilities and provision


Reconfiguration of elective orthopaedic surgery presents challenges and opportunities to develop outpatient pathways to reduce surgical waiting times. Dupuytren's disease (DD) is a benign progressive fibroproliferative disorder of the fascia in the hand, which can be disabling. Percutaneous-needle-fasciotomy (PNF) can be performed successfully in the outpatient clinic. The Aberdeen hand-service has over 10 years' experience running dedicated PNF clinics. NHS Grampian covers a vast area of Scotland receiving over 11749 referrals to the orthopaedic unit yearly. 250 patients undergone PNF in the outpatient department annually. 100 patients who underwent PNF in outpatients (Jan2019–Jan2020). 79M, 21F. Average age 66 years range (29–87). 95 patients were right hand dominant. DD risk factors: 6 patients were diabetic, 2 epileptic, 87 patients drank alcohol. 76 patients had a family history of DD. Disease severity, single digit 20 patients, one hand multiple digits in 15 patients, bilateral hands in 65 patients of which 5 suffered form ectopic manifestation suggestive of Dupuytren's diasthesis. Using Tubiana Total flexion deformity score pre and post fasciotomy. Type 1 total flexion deformity (TFD) between 0–45 degrees pre PNF n=60 post N= 85, Type 2 TFD 45–90 degrees pre PNF n=18 post N=9, Type 3 TFD 90–135 pre PNF n=15 post N= 5, Type 4 TFD >135 pre PNF n=1 post PNF N=1. Using Chi-square statistical test, a significant difference was found at the p<0.05 between the pre and post PNF TFD. Complication: 8 recurrence, 1 skin tear. No patients sustained digital nerve injury. Outpatients PNF clinics are a valuable resource

Objectives. The aim of this systematic literature review was to assess the clinical level of evidence of commercially available demineralised bone matrix (DBM) products for their use in trauma and orthopaedic related surgery. Methods. A total of 17 DBM products were used as search terms in two available databases: Embase and PubMed according to the Preferred Reporting Items for Systematic Reviews and Meta Analyses statement. All articles that reported the clinical use of a DBM-product in trauma and orthopaedic related surgery were included. Results. The literature search resulted in 823 manuscripts of which 64 manuscripts met the final inclusion criteria. The included manuscripts consisted of four randomised controlled trials (level I), eight cohort studies (level III) and 49 case-series (level IV). No clinical studies were found for ten DBM products, and most DBM products were only used in combination with other grafting materials. DBM products were most extensively investigated in spinal surgery, showing limited level I evidence that supports the use Grafton DBM (Osteotech, Eatontown, New Jersey) as a bone graft extender in posterolateral lumbar fusion surgery. DBM products are not thoroughly investigated in trauma surgery, showing mainly level IV evidence that supports the use of Allomatrix (Wright Medical, London, United Kingdom), DBX (DePuy Synthes, Zuchwil, Switzerland), Grafton DBM, or OrthoBlast (Citagenix Laval, Canada) as bone graft extenders. Conclusions. The clinical level of evidence that supports the use of DBM in trauma and orthopaedic surgery is limited and consists mainly of poor quality and retrospective case-series. More prospective, randomised controlled trials are needed to understand the clinical effect and impact of DBM in trauma and orthopaedic surgery. Cite this article: J. van der Stok, K. A. Hartholt, D. There is an ongoing revolution in the use of data within orthopaedics and medicine in general, with an imperative for surgeons to be involved from the bottom up and better define the data collection culture. The use of registries plays a major role in the development of “big data” in orthopaedics. There are multiple examples that are already set up and running, both those inspired and set up by clinicians or those where the main stakeholders may lay people, with some input from clinicians. The British Limb Reconstruction society is no exception, with registries for lengthening nails and pilon fractures due to roll out imminently. The BLRS has tasked this years BOA clinical leadership fellow to investigate the current state of registries among the specialist orthopaedic societies. In particular, comparison of the already well established registries and national audits with the development of registry projects in the smaller societies. The issues of funding, consent, infrastructure and governance each infer particular challenges when translating the methods of the larger registries to the needs and resources of a smaller group. We have aimed to consider these challenges in relation to the set-up of the BLRS registries in order to better understand the potential future pitfalls and opportunities

COVID 19 led to massive disruption of elective services across Scotland. This study was designed to assess the impact on elective service that the COVID-19 pandemic had, to what extent services have been restarted and the associated risks are in doing so.

This is a retrospective observational study. The primary outcomes are the number of operations completed, 30-day mortality, 30-day complication rates and nosocomial infection with COVID-19 compared to previous years. Data was collected from 4 regions across Scotland from 27th March 2020 - 26th March 2021. This was compared to the same time period the previous year.

3431 elective operations were completed in the year post-pandemic compared with 12255, demonstrating a reduction of 72%. Both groups had comparable demographics. Major joint arthroplasty saw a 72% reduction, with TKR seeing a reduction of 82%. Each of the 4 health boards were affected in a similar fashion. Nosocomial COVID-19 infection was 0.4% in the post covid group. 30 day mortality was the same at 0.1%. Total complications rose from 5.7% to 10.1% post covid.

This study shows that there has been a substantial reduction in elective activity across Scotland that is disproportionate to the level of COVID-19. The risk of developing COVID-19 from elective surgery is low at 0.4%, however all complications saw a significant rise. This is likely multifactorial. This study will inform decision makers in future pandemics, that it is safe to continue elective orthopaedic surgery and of the potential impact of cessation of services.

‘Primum non nocere’ is one of the most well known moral principles associated with the medical profession. Often, in our bid to maintain and improve quality of life, we neglect to recognise those patients who are in fact nearing the end of theirs. Thus, our aim was to ascertain if we are recognising the ‘dying’ orthopaedic patient and whether key elements of management in accordance with SIGN are being addressed. All hip-fracture deaths occurring at a District General Hospital over a 4-year period (2012–2015) were included. Paper and electronic notes were used to record patient demographics, days from admission to death, diagnosis of ‘dying’ and discussions regarding DNACPR and ceiling of care. Total numbers of investigations undertaken during the week prior to death were noted. 89 hip-fracture deaths occurred between 2012–2015, of which 57 were female with a mean age at death of 84 years. The number of days post-admission to death was 17.5 (range 0–109). 45 patients had a new DNACPR recorded and 13 were longstanding. 43 patients (48.3%) were diagnosed as dying at a mean of 7.2 days following admission, 31 of whom (72.1%) had ceiling of care discussed. Of this cohort, 32 had futile investigations during their last week of life and astoundingly 10 on the day of death. Although some effort is being made to recognise the ‘dying’ orthopaedic patient, further work is needed to establish a clear ceiling of care pathway, which maintains and respects patient comfort and dignity during their last days of life

Evidence suggests as little as 32percnt; of those with a displaced intracapsular hip fracture who meet the NICE eligibility criteria currently undergo a total hip replacement (THR). The reason for this discrepancy is not clear. This study therefore set out to examine the reasons behind this lack of adherence to these guidelines through the use of a questionnaire to current Trauma & Orthopaedic surgery consultants across Scotland. An invitation to take part in the survey was distributed through the Scottish Committee for Orthopaedics & Trauma (SCOT) email address list. A series of 10 questions were designed to determine the background of participants, their experience at performing hip fracture surgery (including THR) and their thoughts regarding its use in the hip fracture setting. Results were collated at the end of the study period and quantitatively analysed where possible. There were 91 responses in total. 53percnt; of individuals said they would offer those meeting the NICE criteria a THR less than 76percnt; of the time. The most commonly used alternative was a cemented bipolar hemiarthroplasty (51percnt;). Hip surgeons were more likely to perform or supervise THR for hip fracture than non-hip surgeons (p<0.0001). There were a wide variety of reasons why people would not offer a THR including dislocation rate, technical complexity and inadequate evidence for use. Overall this study highlights current trends and barriers in the provision of THR to hip fracture patients. This knowledge can be used to ascertain research priorities to maximise the quality of care in this setting


Aims

Ankle fracture fixation is commonly performed by junior trainees. Simulation training using cadavers may shorten the learning curve and result in a technically superior surgical performance.

Methods

We undertook a preliminary, pragmatic, single-blinded, multicentre, randomized controlled trial of cadaveric simulation versus standard training. Primary outcome was fracture reduction on postoperative radiographs.

Introduction. This study reviews the orthopaedic care of the thirteen patients who were admitted and treated at Royal Manchester Children's' Hospital following the Manchester Arena Bomb blast. Methods. We included all children admitted to Royal Manchester Children's Hospital injured following the bomb blast who either suffered upper limb, lower limb or pelvic fractures, or penetrating upper or lower limb wounds. The nature of each patient's bone and soft tissue injuries, initial and definitive management, and outcome were assessed and documented. Main outcome measures were time to fracture union, time to definitive soft tissue/skin healing, and functional outcome. Findings. Thirteen children were admitted with orthopaedic injuries; 12 were female and mean age was 12.69. All patients had penetrating deep wounds with at least one large nut foreign body in situ, two patients suffered significant burn injury, one patient required amputation of two digits, and two patients required local flap reconstruction. There were a total of 29 upper and lower limb fractures in nine of the patients, with the majority managed without internal or external fixation. In only half of the patients all fractures showed full radiological union at 6 months follow up. There was significant morbidity with several patients suffering long term physical and psychological disability and one patient still in hospital. Conclusion. We found that stable fractures in children secondary to blast injuries can often be appropriately managed without metalwork, and penetrating wounds can be managed without the need for skin graft/flap reconstruction. Our study documents the severe nature of the injuries suffered by paediatric survivors of the Manchester Arena bomb blast. It highlights the demands on a trauma unit following such an event

General Duties Medical Officers (GDMO) deploy to singleton posts on a variety of platforms with, in some cases, limited orthopaedic exposure. The aim of this paper is to investigate the type of orthopaedic conditions presenting to a GDMO at sea onboard a Royal Fleet Auxiliary (RFA) platform to gain a better understanding of the breadth and chronicity of cases. This information can then be utilised when planning the New Entry Medical Officer programme to ensure GDMO pre-deployment teaching is pitched appropriately. All cases contained in a prospective database over a four-month period at sea were interrogated and data pertaining to orthopaedic cases was collected. Data analysis revealed that almost a fifth of cases were orthopaedic related with back and knee problems accounting for 50%. The case mix was similar to that seen by Primary Healthcare Practitioners ashore with only one case requiring medical evacuation from the ship. The data revealed that the comparably older civilian population on RFA platforms present with a wide breadth and chronicity of problems and with increasing numbers of GDMOs deploying on these platforms the data demonstrated the need to ensure teaching covers the management of common acute and chronic conditions as well as emergencies

Training time in Trauma & Orthopaedics is pressured. In this action research project, we develop a feedback/self-reflection model for trainers and trainees, emphasising the contribution both groups make to training, to maximise cohesion and efficacy. Starting in 2013, trainees completed anonymous feedback forms after each 6-month post. The 18-point quantitative questionnaire covers four training domains: WBA engagement, teaching/feedback, research/audit, operative training. Consultant trainers completed a once-off corresponding 18-point self-reflection questionnaire. Additionally, trainers were asked for their expectations of and advice for trainees. Individual trainer profiles were generated from trainee feedback questionnaires, allowing comparison between trainer-group-average, trainer-specific and trainer-self-reflection scores across 18 fields. Trainer profiles were uploaded to ISCP and used for recognition of trainer status for SOAR. This data provided basis for local service provision review with amendments to maximise training efficacy. Results of thematic analysis of trainer feedback was shared with the trainee group. This and subsequent group self-reflection formed the basis of our ‘Trainee Charter’. Trainee feedback illustrates high levels of satisfaction with local training (average global score 4.2/5). Strengths included ‘feedback’ and ‘operative teaching’; relative weaknesses included ‘research time’ and ‘OPD teaching’. The ‘Trainee Charter’ details specific desirable behaviours that embody eight trainee-qualities consistently identified by trainers as important, including ‘honesty’ and ‘being organised’. The charter emphasises trainee contribution to training. For the first time, trainers have the benefit of serial and individualised feedback. Trainees are better informed and empowered in relation to maximising their own training. Most importantly, both halves of the training-team are explicitly acknowledged

Introduction. No previous studies have attempted to measure parental satisfaction and service quality in regards to paediatric orthopaedic service inpatient care. We performed a prospective observational study to assess parental satisfaction with the level of service provided for paediatric orthopaedic inpatient care in our unit. Methods. We employed the validated Swedish parent satisfaction questionnaire to generate parental satisfaction data from 104 paediatric orthopaedic hospital inpatients between August 2009 and May 2010 (49 elective and 55 trauma paediatric orthopaedic admissions, median age range 2–6 years). Questions focused on eight domains of quality: Information on illness, information on routines, accessibility, medical treatment, care processes, staff attitudes, parent participation and staff work environment. Scores generated were a percentage of the maximum achievable for that quality index, for example 100% would correspond to a parent awarding all questions for that index the highest possible score. Results. Overall combined scores for the care indices were highest for parent's perception of ‘medical treatment’ (95%) and ‘staff attitudes’ (95%). The medical treatment index includes questions regarding staff member's skill and competence. Lowest scores corresponded to the index' information routines' (86%). Conclusion. Information routines applies to parental awareness of ward rounds, to whom questions should be directed and which doctors/nursing staff are responsible for their child's care. Lower scores in relation to this index were substantiated by comments from relatives requesting greater information provision. The types of information parents required was routinely provided suggesting that retention rather than lack of information is the main issue. Provision of information pamphlets tailored to common injuries or elective procedures might prove an effective method for improving this aspect of care and increasing overall parental satisfaction with paediatric orthopaedic inpatient service

Introduction. In orthopaedics, clinical outcome assessment (COA) is mostly performed by questionnaires which suffer from subjectivity, a ceiling effect and pain dominance. Real life activity monitoring (AM) can objectively assess function and becomes now feasible as technology has become smaller, lighter, cheaper and easier to use. In this study we validated a custom made algorithm based on accelerometry using different orthopaedic patients with the aim to use AM in orthopaedic COA. Methods. A small, lightweight 3D-accelerometer taped to the lateral side of the affected upper leg served as the activity monitor. AM algorithms were programmed in Matlab to classify standing, sitting, and walking. For validation a common protocol was used; subjects were asked to perform several tasks for 5 or 10 seconds in a fixed order. An observer noted the starting time of each task using a stopwatch. Accuracy was calculated for the number of bouts per activity as well as total time per activity. 10 Subjects were chosen with different pathologies (e.g. post total knee/hip arthroplasty, osteoarthritis) since the difference in movement dynamics in each pathology poses a challenge to the algorithm. Results. In total the subjects performed 267 activities (99 standing, 80 sitting, 88 walking), 258 of which (99, 73, 87 resp.) were classified correctly by the algorithm, corresponding to a sensitivity of 97%. Sensor misplacement in 1 subject caused all missed instances in sitting, and exclusion of this subject increased sensitivity to 99.9%. 5 Instances of standing were incorrectly added by the algorithm, giving a specificity of 95% for standing. In total 80 sit-stand, and 78 stand-sit transitions were performed. Subjects were standing for 792 seconds, sitting for 764 s, and walking for 905 s. The algorithm found a total duration of 739, 583 and 1056 seconds for those activities respectively, and 83 seconds of lying (misclassification of sitting). Discussion. Sensor placement is an important factor to obtain reliable results. Even so sensitivity and specificity are comparable to values found in literature [85–99%]. The added instances of standing occurred when a subject did not immediately sit after a period of walking. It is doubtful if these instances should be considered false positives. The main difference in duration is also found in sitting, which is caused by the missed instances previously described, in combination with the fact that the duration of transitions are added to the walking period in the algorithm, whereas it is divided over sitting and walking by the observer. This corresponds to a difference of less than one second per transition. The algorithm produces reliable results when challenged with different movement patterns common with orthpaedic pathologies. The device may be used as as AM in objective assessment of clinical outcome after orthopaedic procedures

Competition ratios for Core Surgical Training (CST) and Higher Specialist Training in Trauma & Orthopaedic (T&O) surgery have decreased over the last 5 years. Whilst multifactorial, one reason thought to contribute to career decision- making, is junior doctors' experience whilst working in that specialty. This study aimed to identify ‘who’ is currently working on the “1st on call” tier in T&O in the UK, and what clinical activities are undertaken. Collaborators were recruited between 12/09/2015 – 17/01/2016 via the BOTA networks. Data was prospectively collected between 18/01/2015 and 22/01/2015. Each collaborator completed a coded clinical activity diary for all doctors on the “1st on call” rota for T&O in their hospital. Activity parameters included doctor grade, rota gaps, operative and clinic exposure, on call activity, and ward cover. 221 collaborators submitted clinical activity data regarding 933 junior doctors from 100 T&O departments in the UK. 30 rota gaps were identified. The mean number of junior tier doctors was 9 (range 1–23). The “Lost Tribe” comprised Foundation Year 2 (26%), Core Surgical Trainee (19%), Trust Grade (20%), and locum doctors (13%), amongst other grades. During the study period, 2.5% of the ‘Lost Tribe’s' time was allocated to clinic, 2.7% to theatre, 27% to ward cover and 34.6% to zero sessions. Doctors-in-training make up a minority of the workforce and as such, the T&O profession need to do more to ensure that junior doctors are exposed to clinics and operative lists to specifically address the balance between training and service delivery

For some years, there has been vast international interest in creating models for joint efforts between geriatricians and orthopedic surgeons. We present data from two such models. For the first time in Denmark, the Department of Orthopedics Bispebjerg University Hospital (BUP) recruited two full-time geriatricians in September 2009. They were assigned an independent unit meant for severely ill orthopaedic patients with high comorbidity and polypharmacy. These two geriatricians had, during the previous two years, consulted another orthopaedic department at Gentofte University Hospital (GUH) in a neighbouring community three times a week. The aim of their intervention was then to optimize treatment for comorbidity, to clarify indication of acute fall-assessment, osteoporosis diagnosis and treatment, presence of delirium and dementia. Methods. A total of 1344 hip fracture patients (age 70 years) divided into three populations were included in this study. Mortality data were collected from the Danish Civil Registry. Population 1 (P1), n = 645 was included at GUH from January 1, 2006 to December 31, 2007. During the entire period, the patients had access to a senior consultant in geriatric medicine three times a week. In this population, the majority of patients were assessed for dementia (n = 636), delirium (n = 627) and Barthel Index (Barthel100) at admission (n = 394). Population 2 (P2), n = 381 included at BUH from September 1, 2009 until July 8, 2010 with orthogeriatric access. Population 3 (P3), n = 318 were included at BUH from September 28, 2008 until August 31, 2009 with no orthogeriatric access. Age mean (SD): P1 84.7 (6.8), P2 85.5 (7.3), P3 85.3 (14.3) P = 0.1(ANOVA) Sex ratio: females/males: P1 0.73/0.27, P2 0.80/0.20, P3 0.75/0.25 P = 0.09 Chi square). Results. In-hospital mortality rate: P1 4.8%, P2 6.3%, P3 9.1% P = 0.03 (Chi square). Three month mortality: In P1 dementia, delirium and Barthel Index (below 50 versus above 50) were all strong predictors: No dementia: 53/383 (13.8%) versus dementia present 68/253 (26.9%) died, P = 0001 (log-rank test). No delirium 69/456 (15.1%) versus delirium present 47/171 (27.5%) died F = 0.0004 (log-rank test) Barthel Index 50 38/372 (10.2%) versus Barthel<50 7/22 (31.8%) died P = 0.0004. Conclusion. This paper reports data from two different models with orthogeriatric service. Our data present delirium, dementia and Barthel Index to be very strong predictors for three month mortality (P1). Despite the time needed to implement a new orthogeriatric unit and the fact that the geriatricians only assessed a proportion of hip fracture patients, in-hospital mortality was reduced significantly for the total hip-fracture population (P2) within the first 10 months

Introduction. Surgical site infections (SSI) are related to a surgical procedure and affect the surgical wound or deeper tissues. With continuing emphasis on clinical governance and quality control, there is increasing demand from both patients and government for methods of assessing surgical results. Rates of morbidity and mortality may play important roles in these assessments. When crude comparisons between hospitals in the incidence of SSI are made, these should at least be stratified by the type of procedure. The aim of this study is to fix SSI incidence in relation to surgical procedure. Methods. This report contains data of 19.948 procedures collected from 1996 to 2008 at a Specialist Orthopaedic Hospital and analyzed by a specific software designed for the study of infection in orthopaedic and trauma surgery. The SSI surveillance is focused on categories of surgical procedure (Hip Arthroplasty, Knee Arthroplasty, Spine Surgery and Hip Fracture Surgery) with each category containing a defined set of similar procedures. A basic of demographic data and details about operation itself are collected for each procedure. Patients are followed up throughout their hospital stay and after hospital discharge. We present the incidence of SSI by risk group and surgical procedure. SSI are categorized in type, moment of diagnosis and micro-organisms reported. Analysis of the data was performed (SPSSv15.0 ®). Statistical methods used to determine significance were the independent samples t test, Pearson X2 test, Odds ratio and Spearman correlation coefficient, with a significance level of p<0,05. Results. Rates of SSI are highest in hip hemiarthroplasty after fracture and in hip and knee revision procedures. Rates of SSI increase with the number of risk factors present in the patient, especially after fracture procedures. The most common infecting micro-organism was coagulase-negative staphylococcus, followed by Staphylococcus aureus, enterococci and streptococci. 38% of the infections arise after one year of surveillance. Conclusions. Our results confirm difference in rates of SSI depending of surgical procedure in orthopaedic and trauma surgery. We believe that our decision to monitor infection with a long term follow up it's the better way to avoid under-reporting of infection. Our study has also demonstrated the extent to which the emerging problem of infection due to MRSA has affected orthopaedic surgery. The risk of acquiring SSI caused by MRSA was particularly high in patients after hip fracture surgery. Rates of SSI increase with the number of risk factors present in the patient, especially after fracture procedures. Involvement in research forms a mandatory part of Trauma & Orthopaedic specialty training. Evidence of publication is a compulsory criterion for attaining Certificate of Completion of Training (CCT). The publishing behaviour of orthopaedic trainees from all four deaneries in Scotland was examined (East, North, South East and West of Scotland). A literature search was performed for Scottish orthopaedic trainees achieving CCT between July 2005–July 2010 using Knowledge Network and PubMed databases. Data collected included date of publication, article type, journal, publishing institute, number of authors and position of trainee within authors. There was no significant difference in mean number of publications/trainee prior to specialty training across the four deaneries (EOS 0.18; NOS 0.18; SES 0.25; WOS 0.73). The number of publications/trainee during training was statistically significantly higher in SES (mean 6.31; mode 9; median 4) compared to WOS (2.23;0;1), NOS (2.18;1;2) and EOS (1.72;1;1). However, there was no correlation between a trainee's number of publications during training and post–CCT. There was no significant difference for mean number of authors/trainee publication during training across the four deaneries (range 3.38–3.63), nor mean position of trainee in list of authors (range 1.37–1.67). This study highlights important differences and notable similarities in publishing behaviour during orthopaedic training across the Scottish deaneries. It suggests that rates of publication relate to factors during training and that publishing during training is not predictive of future behaviour. This research may be of interest to trainees, training committees and orthopaedic departments in future appointments

In biomaterial engineering the surface of an implant can influence cell differentiation, adhesion and affinity towards the implant. Increased bone marrow derived mesenchymal stromal cell (BMSC) differentiation towards bone forming osteoblasts, on contact with an implant, can improve osteointegration. The process of micropatterning has been shown to improve osteointegration in polymers, but there are few reports surrounding ceramics. The purpose of this study was to establish a co-culture of BMSCs with osteoclast progenitor cells and to observe the response to micropatterned zirconia toughened alumina (ZTA) ceramics with 30 µm diameter pits. The aim was to establish if the pits were specifically bioactive towards osteogenesis or were generally bioactive and would also stimulate osteoclastogenesis that could potentially lead to osteolysis. We demonstrate specific bioactivity of micropits towards osteogenesis with more nodule formation and less osteoclastogenesis. This may have a role when designing ceramic orthopaedic implants

This paper describes an audit loop. We studied patients undergoing hip and knee surgery (arthroplasty and revision arthroplasty). All the patients were ‘complex elective’. I.e. they were either ASA grade 3 or 4, or had a body mass index in excess of 40. We collected data concerning postoperative admissions to HDU, ICU and PACU (planned and unplanned rates of admission, length of stay). We also noted mortality. In the first part of the study (April 2005 to March 2006) we studied 298 patients. All patients were assessed independently by an anaesthetist on the day of surgery. A multidisciplinary preoperative assessment clinic commenced in April 2006. After this date all patients were assessed preoperatively by a multidisciplinary anaesthetic lead team (anaesthetist, orthopaedic senior house officer, nurse practitioner). The need for an HDU or ICU bed was assessed and the bed was booked at part of the pre-operative plan. In the second part of the study (May 2006 to April 2009) a further 1147 arthroplasty patients were studied. Data was again collected regarding HDU, ICU, PACU and mortality as noted above. We found statistically significant (p=0.001) reductions in the admissions to PACU (22% down to 10%) and in mortality (6.1% down to 1.2%) after the introduction of the pre assessment clinic. There was also statistically significant (p=0.01) reduction in the HDU length of stay(2.1 days to 1.6 days), ITU unplanned admissions (1.3% to 0.4%) and the ITU length of stay in days (2.3 to 1.9 days). We also estimated cost savings of nearly £50 000 in the second limb of the study. This is based on the average decrease in HDU and ICU length of stay. We recommend the use of a multidisciplinary pre assessment clinic for complex orthopaedic surgery
