Meniscal tears are the most common knee injuries, occurring in acute ruptures or in chronic degenerative conditions. Meniscectomy and meniscal repair are two surgical treatment options. Meniscectomy is easier, faster, and the patient can return to their normal activities earlier. However, this procedure has long-term consequences in the development of degenerative changes in the knee, potentially leading to knee replacement. On the other hand, meniscal repair can offer prolonged benefits to the patients, but it is difficult to perform and requires longer rehabilitation. Sutures are used for meniscal repairs, but they have limitations. They induce tissue damage when passing through the meniscus. Furthermore, under dynamic loading of the knee, they can cause tissue shearing and potentially lead to meniscal repair failure. Our team has developed a new technology of resistant adhesive hydrogels to coat the suture used to repair meniscal tissue. The objective of this study is to biomechanically compare two suture types on bovine menisci specimens: 1) pristine sutures and 2) gel adhesive puncture sealing (GAPS) sutures, on a repaired radial tear under cyclic tensile testing. Five bovine knees were dissected to retrieve the menisci. On the 10 menisci, a complete radial tear was performed. They were separated in two groups and repaired using either pristine (2-0 Vicryl) or GAPS (2-0 Vicryl coated with adhesive hydrogels) with a single stitch and five knots. The repaired menisci were clamped on an Instron machine. The specimens were cyclically preconditioned between one and 10 newtons for 10 cycles and then cyclically loaded for 500 cycles between five and 25 newtons at a frequency of 0.16 Hz. The gap formed between the edges of the tear after 500 cycles was then measured using an electronic measurement device. The suture loop before and after testing was also measured to ensure that there was no suture elongation or loosening of the knot. The groups were compared statistically using Mann-Whitney tests for nonparametric data. The level of significance was set to 0.05. The mean gap formation of the pristine sutures was 5.61 mm (SD = 2.097) after 500 cycles of tensile testing and 2.38 mm (SD = 0.176) for the GAPS sutures. Comparing both groups, the gap formed with the coated sutures was significantly smaller (p = 0.009) than with pristine sutures. The length of the loop was equal before and after loading. Further investigation of tissue damage indicated that the gap was formed by suture filament cutting into the meniscal tissue. The long-term objective of this research is to design a meniscal repair toolbox from which the surgeon can adapt his procedure for each meniscal tear. This preliminary experimentation on bovine menisci is promising because the new GAPS sutures seem to keep the edges of the meniscal tear together better than pristine sutures, with hopes of a clinical correlation with enhanced meniscal healing

Meniscal tears commonly co-occur with ACL tears, and many studies address their side, pattern, and distribution. Few studies assess the patient's short-term functional outcome concerning tear radial and circumferential distribution based on the Cooper et al. classification. Meniscal tears require primary adequate treatment to restore knee function. Our hypothesis is to preserve the meniscal rim as much as possible to maintain the load-bearing capacity of the menisci after meniscectomy. The purpose of this study is to document the location and type of meniscal tears that accompany anterior cruciate ligament (ACL) tears and their effect on patient functional outcomes following arthroscopic ACL reconstruction and meniscectomy. This prospective cross-sectional observational study was conducted at AL-BASRA Teaching Hospital in Iraq between July 2018 and January 2020 among patients with combined ipsilateral ACL injury and meniscal tears. A total of 28 active young male patients, aged 18 to 42 years, were included. All patients were subjected to our questionnaire, full history, systemic and regional examination, laboratory investigations, imaging studies, preoperative rehabilitation, and were followed by Lysholm score 6 months postoperatively. All 28 patients were males, with a mean age of 27 ± 0.14 years. The right knee was the most commonly affected in 20/28 patients (71.4%). The medial meniscus was most commonly injured in 11 patients, 7 patients had lateral meniscal tears, and 10 patients had tears in both menisci. The most common tear pattern of the medial meniscus was a bucket handle tear (36.4%), while longitudinal tears were the most frequent in the lateral meniscus (71.4%) (P-value = 0.04). The most common radial tear location was zone E-F (5/28, 17.8%), and the most common circumferential zone affected was the middle and inner third, reported in 50% of tears. Good and excellent outcomes using the Lysholm score after 6 months were obtained in 42.9% and 17.9% of patients, respectively. Better functional scores were associated with lateral meniscal tears, bucket handle tears, tears extending to a more peripheral vascular area, and if no more than one-third of the meniscus was resected (P-value = 0.002). Less favourable outcomes were reported in smokers, posterior horn tears, and when surgery was delayed more than 1 year (P-value = 0.03). We conclude that there is a negative correlation between the amount of meniscus resected and functional outcome. Delayed ACL reconstruction increases the risk of bimeniscal tears. Bucket handle tears are the most common tears, mostly in the medial meniscus, while longitudinal tears are most common in the lateral meniscus. We recommend performing early ACL reconstruction within 12 months to reduce the risk of bimeniscal injuries

Abstract. Aims. The aim of this study was to evaluate the indications for patients presenting with knee pain undergoing magnetic resonance imaging of the knee prior to referral to the orthopaedic department and to ascertain whether plain radiograph imagining would be more beneficial prior to an MRI scan. Method. A retrospective review of all referrals received by the hospital over a 6-month period was performed. Patients with knee pain that underwent an MRI scan were classified into two age groups, under 50 years and over 50 years old. Patients having undergone Magnetic resonance imaging (MRI) prior to referral were identified, and findings of the scan were recorded. These patients were reviewed further to see if a plain radiograph had been completed prior to or after the MRI. Results. A total of 414 patients were referred. In the over 50's 228 MRI scans were performed. Of these 103/228 patients the predominate finding was meniscal tears and 72/228 had a finding of osteoarthritis. A plain radiograph of the knee was completed 99/228 cases, 28/99 before the MRI. In the under 50's 186 scans were performed and of these 85/186 the predominate finding was meniscal tears. A plain radiograph of the knee was completed in only eighty-three (83/186) cases, (69/83) before the MRI. Conclusion. In over 50s, 57% of patients (129 of 228) did not have a knee X-ray before having an MRI. For a single hospitals referral over 6 months these unnecessary MRI of knee cost the National Health Services £7,500

Meniscal repairs are commonly performed during anterior cruciate ligament (ACL) reconstruction. This study aimed to identify the risk factors for meniscal repair failure following concurrent primary ACL reconstruction. Primary ACL reconstructions with a concurrent repair of a meniscal tear recorded in the New Zealand ACL Registry between April 2014 and December 2018 were analyzed. Meniscal repair failure was defined as a patient who underwent subsequent meniscectomy, and was identified after cross-referencing data from the ACL Registry with the national database of the Accident Compensation Corporation (ACC). Multivariate Cox regression was performed to produce hazard ratios (HR) with 95% confidence intervals (CI) to identify the patient and surgical risk factors for meniscal repair failure. 2041 meniscal repairs were analyzed (medial = 1235 and lateral = 806). The overall failure rate was 9.4% (n = 192). Failure occurred in 11.1% of medial (137/1235) and 6.8% of lateral (55/806) meniscal repairs. The risk of medial failure was higher with hamstring tendon autografts (adjusted HR = 2.00, 95% CI 1.23 – 3.26, p = 0.006) and in patients with cartilage injury in the medial compartment (adjusted HR = 1.56, 95% CI 1.09 – 2.23, p = 0.015). The risk of lateral failure was higher when the procedure was performed by a surgeon with an annual case volume of less than 30 ACL reconstructions (adjusted HR = 1.92, 95% CI 1.10 – 3.33, p = 0.021). Age, gender, time from injury-to-surgery and femoral tunnel drilling technique did not influence the risk of meniscal repair failure. When repairing a meniscal tear during ACL reconstruction, the use of a hamstring tendon autograft or the presence of cartilage injury in the medial compartment increases the risk of medial meniscal repair failure. Lower surgeon case volume increases the risk of lateral meniscal repair failure

Traditionally, sports Injuries have been sub-optimally managed through Emergency Departments (ED) in the public health system due to a lack of adequate referral processes. Fractures are ruled out through plain radiographs followed by a reactive process involving patient initiated further follow up and investigation. Consequently, significant soft tissue and chondral injuries can go undiagnosed during periods in which early intervention can significantly affect natural progression. The purpose of this quality improvement project was to assess the efficacy of an innovative Sports Injury Pathway introduced to detect and treat significant soft tissue injuries. A Sports Injury Pathway was introduced at Fiona Stanley Hospital (WA, Australia) in April 2019 as a collaboration between the ED, Physiotherapy and Orthopaedic Departments. ED practitioners were advised to have a low threshold for referral, especially in the presence of a history of a twisting knee injury, shoulder dislocation or any suggestion of a hip tendon injury. All referrals were triaged by the Perth Sports Surgery Fellow with early follow-up in our Sports Trauma Clinics with additional investigations if required. A detailed database of all referrals was maintained, and relevant data was extracted for analysis over the first 3 years of this pathway. 570 patients were included in the final analysis. 54% of injuries occurred while playing sport, with AFL injuries constituting the most common contact-sports injury (13%). Advanced Scope Physiotherapists were the largest source of referrals (60%). A total of 460 MRI scans were eventually ordered comprising 81% of total referrals. Regarding Knee MRIs, 86% identified a significant structural injury with ACL injuries being the most common (33%) followed by isolated meniscal tears (16%) and multi-ligament knee injuries (11%). 95% of Shoulder MRI scans showed significant pathology. 39% of patients required surgical management, and of these 50% were performed within 3 months from injury. The Fiona Stanley Hospital Sports Injury Pathway has demonstrated its clear value in successfully diagnosing and treating an important cohort of patients who present to our Emergency Department. This low threshold/streamlined referral pathway has found that the vast majority of these patients suffer significant structural injuries that may have been otherwise missed, while providing referring practitioners and patients access to prompt imaging and high-quality Orthopaedic sports trauma services. We recommend the implementation of a similar Sports Injury Pathway at all secondary and tertiary Orthopaedic Centres


BACKGROUND. Injuries of the both menisci and complete ACL tear is quite common. In the literature, functional outcome in these patients is often variable and less than satisfactory. We studied the functional outcome of this group of patients (retrospective study of prospectively followed case series). MATERIAL AND METHODS. All patients who were diagnosed with ACL injury by arthroscopy at an tertiary hospital in southern India between January2013 and june2017 were enrolled in the study. From the total of 189 consecutive patients who had ACL tears with either one meniscus or both meniscus tear, 41 patients had injuries of both menisci of the same knee with ACL tear. 6 patients were lost to follow up. All patients underwent repair/balancing of one/both meniscus and reconstruction of ACL. There were 29 males and 6 females with age ranging from 18 – 60yrs (mean 25.2) years available for final analysis. RESULTS. In 45.71% of patients (n=16) both meniscal tear was diagnosed by arthroscopic examination only. Bucket handle tear was observed in 5 knees (medial: 4 knees, lateral: 1 knee) in fresh group, and 4 knees (medial: 3 knees, lateral: 1 knees) in delayed presentation. PMMRT was noted in 6 patients and PLMRT was noted in 3 patients. All patients were evaluated using lysholm score. CONCLUSION. The incidence of both meniscal tear associated with ACL injury has significant effect on the post operative management and the overall functional outcome. The incidence of injuries of both menisci is higher in acute injuries in our series

The suggestion of a meniscal tear produces a pavlovian response in the orthopaedic surgeon. However, meniscal signal anomalies and associated changes become common with age in symptom free knees. T he issue for the IME requested to assess workers with painful knees is to determine if the MRI changes represent a painful injury and if the treatment planned (usually arthroscopy) may, in fact, be harmful. MRI signal changes are assessed on the likelihood they predict for unstable meniscal tears. Some patterns of meniscal tears are benign. Associated changes such as baker's cyst and ligament thickening are also common but are poor predictors of symptomatic tears. Preclinical osteoarthritis has a high incidence of associated meniscal change and arthroscopic menisectomy may accelerate osteoarthritis progression. Clinical tests have variable specificity and sensitivity but in combination with an understanding of the patterns of MRI signal can be combined to predict which meniscal tears would benefit from arthroscopic surgery, which injuries would do as well with non-operative treatment and which patterns predict deterioration after surgery. As the views of the IME are often contrary to the surgeon, a comprehensive bibliography is provided for any who need to argue their case. Introduction. Meniscal tears in middle-aged patients are common. There is a lack of consensus regarding the optimum management of these injuries. Although arthroscopic partial meniscectomy (APM) is a frequently performed surgical option, literature has failed to prove its effectiveness over conservative approaches. Materials and Methods. We retrospectively reviewed 102 middle-aged (age > 40 years) patients who had been treated for meniscal tears between January 2010 and December 2012. We followed these patients up telephonically to assess knee outcome satisfaction rates, any early (6 weeks) improvements in pain and knee function and their pre-morbid and post-treatment activity levels (Tegner Activity Scale). Results. There were 65 male and 37 female patients with an average age of 58 years (range 40–79). After their first consultation 72 patients were treated conservatively and 30 patients were treated with an APM. There were no reported complications following surgery. We managed to contact 87 patients who were followed up at an average of 17.5 months (range 5–34 months). Overall, 72.4% of patients managed were satisfied with their knee outcomes. APM had a 78.9% satisfaction rate and 67.3% of patients treated conservatively were satisfied. There were no significant differences in early or long term pain or knee function improvements between APM or conservative modalities. 89.7% of patients returned to the same or better activity levels after treatment. There was a 1.5 times greater risk of worse activity level following APM. There was a 79.3% satisfaction rate in patients who remained at the same activity level (p=0.00). Analysis showed 21 of the 72 conservatively treated patients failed this approach. APM was performed on 18 of them resulting in a 75% satisfaction rate for this group. Conclusion. APM and conservative management produce equivalently good results for meniscal tears in middle-aged patients. Delayed surgery does not negatively influence the outcome of these patients. Surgery performed in low-volume centres has been associated with longer operating time, longer hospital stays, lower functional outcomes, and higher rates of revision surgery, complications, and mortality. This has been reported consistently in the arthroplasty literature, but there is a paucity of data regarding the relationship between surgical volume and outcome following anterior cruciate ligament (ACL) reconstruction. The purpose of this study was to compare the ACL reconstruction failure rate between hospitals performing different annual surgical volumes. The hypothesis was that ACL reconstructions performed at low-volume hospitals would be associated with higher failure rates than those performed at high-volume centres. This level-II cohort study included all patients from the Norwegian Knee Ligament Registry that underwent isolated primary autograft ACL reconstruction between 2004 and 2016. Hospital volume was divided into quintiles based on the number of ACL reconstructions performed annually, defined arbitrarily as: 1–12 (V1), 13–24 (V2), 25–49 (V3), 50–99 (V4), and ≥100 (V5) annual procedures. Kaplan-Meier estimated survival curves and survival percentages were calculated with revision ACL reconstruction as the end point. Mean change in Knee Injury and Osteoarthritis Outcome Score (KOOS) Quality of Life and Sport subsections from pre-operative to two-year follow-up were compared using t-test. 19,204 patients met the inclusion criteria and 1,103 (5.7%) underwent subsequent revision ACL reconstruction over the study period. Patients in the lower volume categories (V1-3) were more often male (58–59% vs. 54–55% p=<0.001) and older (27 years vs. 24–25 years, p=<0.001) compared to the higher volume hospitals (V4-5). Concomitant meniscal injuries (52% vs. 40%) and participation in pivoting sports (63% vs. 56%) were most common in V5 compared with V1 (p=<0.001). Median operative time decreased as hospital volume increased, ranging from 90 minutes at V1 hospitals to 56 minutes at V5 hospitals (p=<0.005). Complications occurred at a rate of 3.8% at low-volume (V1) hospitals versus 1.9% at high-volume (V5) hospitals (p=<0.001). Unadjusted 10-year survival with 95% confidence intervals for each hospital volume category were: V1 – 95.1% (93.7–96.5%), V2 – 94.1% (93.1–95.1%), V3 – 94.2% (93.6–94.8%), V4 – 92.6% (91.8–93.4%), and V5 – 91.9% (90.9–92.9%). There was no difference in improvement between pre-operative and two-year follow-up KOOS scores between hospital volume categories. Patients having ACL reconstruction at lower volume hospitals did not have inferior clinical or patient reported outcomes, and actually demonstrated a lower revision rate. Complications occurred more frequently however, and operative duration was longer. The decreased revision rate is an interesting finding that may be partly explained by the fact that patients being treated in these small, often rural hospitals, may be of lower demand as suggested by the increased age and decreased participation in pivoting sports. In addition, patients with more complicated pathology such as meniscal tears were more commonly treated in the larger volume hospitals. The most significant limitation of this study is that provider volume was not assessed, and the number of surgeons dividing up the surgical volume at each hospital is not known

To determine the relationship between advancing months from ACL rupture and the incidence of intra-articular meniscal and chondral damage. From a prospectively collected database 5086 patients undergoing primary ACL reconstruction, using hamstring graft, carried out between January 2000 and August 2010 were identified. Data collected included the interval between injury and surgery, type and location of meniscal tears (requiring meniscectomy) and location and severity of chondral damage (ICRS grading system). Patients were grouped according to time interval and age. The median time from ACL injury to ACL reconstruction was 3 months (range 0.25 to 480). Overall, an increasing incidence of medial meniscal injury and chondral damage occurred with advancing chronicity of ACL deficiency. The incidence of medial meniscal injury requiring meniscectomy increased from 18% of patients undergoing ACL reconstruction within 4 months of injury to 59% of patients if ACL reconstruction was delayed more than 12 months (p<0.001). The incidence of lateral meniscal tears did not increase significantly over time. The increasing incidence of secondary pathology with advancing chronicity was more pronounced in the younger age groups. The risk of a medial meniscal tear requiring resection was significantly less if surgery was performed before 5 months in the <17 years group (Odds Ratio 2) and 17–30 years group (OR 1.9), but less so in the 31–50 years group (OR 1.5) and >50 years group (OR 1.5). Advancing age was associated with a greater incidence of chondral damage and medial meniscal injury, but not lateral meniscal injury. Males had a greater incidence of lateral meniscal tears (34% vs. 20%), but not medial (28% vs. 25%) or chondral damage (35% vs. 36%), compared to females. The incidence of chondral damage and medial meniscal tears increases with advancing time after ACL injury. Particularly in younger patients, ACL reconstruction should be performed within 4 months of ACL injury in order to minimise the risk of irreversible damage to meniscal and chondral structures

INTRODUCTION. The menisci play a fundamental biomechanical role in the knee and also help in the maintaining of the articular homeostasis; thus, either a lesion or the complete absence of the menisci can invalidate the physiological function of the knee causing important damages, even at long term. Unfortunately, meniscal tears are often found during the ordinary orthopaedic practice while the regenerative potential of this kind of tissue is very low and limited to its peripheral-vascularized part; this is why the majority of these common arthroscopic findings are not reparable and often the surgeon is almost forced to perform a partial, subtotal or even total meniscectomy, regardless of the well-known consequences of this kind of surgery. MATERIALS AND METHODS. Recently a porous, biodegradable scaffold made of an aliphatic polyurethane (Actifit(tm),Orteq Ltd) has been developed for the arthroscopic treatment of partial and irreparable meniscal tears; thanks to its particular structure, this scaffold facilitates the regeneration of the removed meniscal part, preventing the potential cartilage damage due to its complete or partial lack. We performed a prospective clinical study on 17 patients affected by a massive loss of meniscal substance either medial or lateral associated with intraarticular or global knee pain and/or swelling. We analyzed the patient both clinically and by using the International Knee Document Committee's (IKDC) Subjective and Objective Knee Evaluation Form. We also assessed the sport activity resumption by comparing the Tegner score at the time of the very first visit with the presurgery and prelesional ones. Finally, we also organized a control MRI at 6 and 12 months after surgery. DISCUSSION. Our preliminary results are encouraging and they confirm the clinical experiences of other study groups. Apparently, the properties of this scaffold help in vessels formation and tissue regeneration potentially allowing the restoration of the surgically removed portion and preventing, or delaying at least, both chondral and articular degeneration. We also performed some biopsy associated arthroscopic “second-looks” that reinforced the already good clinical results; the biopsies also confirmed the new tissue ingrowth into the biomaterial, potentially leading to the replacement of the previously removed damaged tissue. CONCLUSIONS. Preliminary results suggest that this surgical procedure can be considered a really promising method for the treatment of both inveterate and symptomatic meniscal tears; however, other randomized studies with a longer follow-up should be done to confirm its reliability and potentialities

We reviewed 87 patients who underwent revision anterior cruciate ligament (ACL) reconstruction. The incidence of meniscal tears and degenerative change was assessed and related to the timing from primary ACL graft failure to revision ACL reconstruction. Patients were divided into either an early group (revision surgery within 6 months of graft failure) or a delayed group. Degenerative change was scored using the French Society of Arthroscopy system. There was a significantly higher incidence of articular cartilage degeneration in the delayed group compared to the early group (53.2% vs 24%, p < 0.01, Mann- Whitney U test). No patients in the early group had advanced degenerative change (SFA grades 3 or 4), compared with 12.9% of patients in the delayed group. There was no significant difference in the incidence of meniscal tears between the two groups. In conclusion, the findings of the study support the view that patients with a failed ACL reconstruction and symptomatic instability should have an early revision reconstruction procedure carried out to minimise the risk of articular degenerative change

Introduction. Menisci performs multiple functions in the knee.'These depend largely on the structural integrity of the meniscus. Arthroscopic partial menisectomy is the treatment of choice for meniscal tears in adults. There is conflicting evidence about the progression of degenerative changes in the medial or lateral compartment of the knee following menisectomy. Aim. The aim of our study was to demonstrate the subjective, objective and radiographic outcome of arthroscopic partial lateral menisectomy in the intermediate term and to identify any association between age, sex, activity level, the type of meniscal tear, pre operative articular surface damage and the amount of meniscus resected on the outcome. Materials and Methods. Between 1999 and 2003,152 patients in the18 to 40 year age group underwent arthroscopic partial lateral menisectomy. A senior consultant orthopaedic surgeon performed all procedures. 72 patients were available for final clinical and radiological review. Patient's subjective & objective assessments were undertaken using validated scoring systems. Musculoskeletal physiotherapist & radiologists performed clinical & radiological assessments independently. Progression of degenerative changes was recorded. Results. All patients demonstrated initial improvement of symptoms lasting upto 2 ½ years followed by gradual deterioration of subjective symptoms. There was statistically significant deterioration in the IKDC & Lysolm scores. Radiological changes developed or progressed in 36.6% of the patients (P value < 0.005). These changes were most marked in the 35 to 40 year age group. There was no statistical correlation between clinical symptoms and radiological changes. No other statistically significant associations were demonstrated. Conclusions. Arthroscopic partial lateral menisectomy leads to progressive deterioration in clinical and radiological outcome in the Intermediate term. However there is no correlation between clinical & radiological outcomes

The meniscus is at the cornerstone of knee joint function, imparting stability and ensuring shock absorption, load transmission, and stress distribution within the knee joint. However, it is very vulnerable to injury and age-related degeneration. Meniscal tears are reported as the most common pathology of the knee with a mean annual incidence of 66 per 100,000. Knee osteoarthritis progresses more rapidly in the absence of a functional meniscus. Historically, tears extending to the avascular inner portion of the meniscus (white-white zone, “WW”), such as radial tears were considered as untreatable and were often resected, due to the lack of vascularity in the WW zone. Perfusion-based anatomical studies performed on cadaveric menisci in the 1980s shaped the current dogma that human meniscus has poor regenerative capacity, partly due to limited blood supply that only reaches 10 to 25% of the meniscus, commonly referred to as red-red zone (“RR”). Previous studies, including those utilizing animal models have shown mobilization of Mesenchymal Stem Cells (MSCs) upon injury into the WW zone, and successful MSC recruitment when administered externally to the injury site. We and others have recently reported positive outcomes of repaired tears in the inner zone of patients. We hypothesized that the “avascular” white-white zone of the meniscus possesses regenerative capacity due to a resident stem/progenitor cell population. Further, we sought to redefine the presence of microvessels in all meniscal zones using advanced stereology and imaging modalities. Fifteen menisci from fresh human cadaveric knees (mean age: 21.53±6.53 years) without evidence of previous injury were obtained from two tissue banks (JRF, Centennial, CO) and Biosource Medical (Lakeland, FL) and utilized for this study. The use of cadaveric specimens for research purposes was approved by the institutional review board. Tibial plateaus were dissected to harvest medial and lateral menisci along their entire length. The RR, red-white (RW) and WW zones were dissected and separated into three thirds from the inner aspect to the marginal border of the meniscus and their wet weights recorded (Fig.1A). Meniscus tissue cellular content in each zone was obtained from dissociation of meniscus tissue using 0.02% w/v pronase (Millipore) for 1h at 37oC, followed by 18h 0.02% w/v collagenase II (Worthington) at 37oC with shaking. Isolated cells were characterized immediately after harvest using flow cytometry with antibodies against MSCs surface markers (CD105, CD90, CD44 and CD29) as well as respective isotype controls. Further, meniscal cells were cultured and split twice when confluence was reached, characterized at P2 and compared to bone marrow-derived MSCs (BM-MSCs) using the same markers. Self-renewal of cells was assessed using colony forming unit (CFU) assay. Differentiation assays were performed to assess whether colony-forming cells retained multilineage potential. For morphological examination of bigger vessels, samples were fixed in 10% formalin for 1 week, paraffin embedded, sectioned (4 μm thick) and stained with H&E and Masson's trichrome. Presence of microvessels was assessed by CD31 immunofluorescence staining. Further, menisci were cleared using the uDisco protocol labeled with the TO-PRO®-3 stain, a fluorescent dye that stains cell nuclei and imaged using light-sheet microscopy. All continuous data are presented as mean ±standard deviation. Non-repeated measures analysis of variance (ANOVA) and Tukey-Kramer HSD post hoc analysis were performed on sample means for continuous variables. Statistical significance was set at p < 0 .05. Menisci were successfully cleared using a modified uDISCO procedure, imaged and analyzed for total cell density. As expected, bigger vessels were observed in RR but not in WW. However, immunofluorescent staining for CD31 showed a subset of CD31+endothelial cells present in the WW zone, indicating the presence of small vessels, most likely capillaries. In order to assess whether enzymatic digestion had a differential result depending on meniscus zone due to cellular content, we analyzed yields per meniscus per zone. The wet weight of different zones (WW:RW:RR) was at a ratio of ∼1:3:5 respectively, however, the ratio of cells isolated from each zone was at ∼1:4:20, indicating that RR has a denser population of mononuclear cells. However, the difference between all zones in cell yields was not significant. The clonogenic potential of isolated cells was shown to be non-significantly different between the three zones. Differentiation of isolated cells to osteogenic lineage using osteogenic media in vitroshowed no difference between the three zones. Flow cytometry analysis of cells from the three meniscal zones displayed presence of two distinct subpopulations of cells immediately after isolation. One subpopulation was positive to MSC surface markers and the other negative. Additionally, flow cytometry of cultured meniscal cells at P2 displayed that the entire cell population was CD44+CD105+CD29+CD90+, suggesting that culturing meniscal cells results in selection of stem/progenitor cells (plastic adherence). Surface marker expression analysis showed differential expression patterns between markers depending on zone. Similar fraction of cells was detected to express both MSC markers CD90 and CD105 (7–10%) and similar fraction of cells expressed both MSC markers CD29 and CD44 (1–2%) in all three zones, indicating similar density of resident stem/progenitor cells in each zone. Importantly, WW showed significantly higher expression for all four MSC markers compared to the RR zone, indicating higher relative density of stem/progenitor resident cells in the WW zone. Our results determine that CD31-expressing microvessels were present in all zones, including the WW zone, which was previously considered completely avascular. Additionally, stem/progenitor cells were shown to be present in all three zones of the menisci, including the WW zone, showcasing its regenerative potential. Lateral meniscal failure and secondary valgus with lateral compartment arthrosis is quite common in the developed world. The varus knee is the common phenotype of the ‘jock’ of both genders, while the valgus knee is a common consequence of lateral meniscal tear, skiing or ‘catwalk’ life. Occurring more commonly in ‘flamingo’ phenotypes, lateral meniscal failure can be disabling, entirely preventing high heels being worn for instance. Indications. Lateral UKA is indicated for most valgus knees, and is substantially safer than TKA. ACL integrity is not essential in older people, as the patello-femoral mechanism is in line with the lateral compartment. Severe valgus with substantial bone loss is not a contraindication, if the deformity is simply angular. As long as there is not marked subluxation, fixed flexion deformity invariably corrects after notch osteophyte removal from femur and tibia. Combinations. Lateral UKA can be combined safely with PFJA: performed through a lateral approach, this is a safe and conservative procedure. ACL integrity is not essential – reconstruction can be undertaken simultaneously, if necessary. Combining lateral UKA with medial UKA is only rarely needed, and sometimes needs ACL reconstruction too. Adding a medial UKA in under 5 years usually results from overcorrection of the valgus. Mid Term Results, at a median of 7 years postop: Between 2005 to 2009, 64 knees in 58 patients had a lateral UKA using a device designed for the lateral compartment. This included 41 females and 17 males with a mean age of 71 years at the time of surgery (range 44–92). Thirty-nine patients underwent surgery on the right knee and 6 underwent bilateral procedures, of which four were performed under a single anesthetic. Primary lateral compartment osteoarthritis was the primary diagnosis in 63 cases with secondary osteoarthritis to a lateral tibial plateau fracture the indication in one patient. At 119 months follow up, the predicted cumulative survival was 0.97. With re-operation as an endpoint, 11% of patients within the study had undergone re-operation with a predicted cumulative survival of 0.81 at 119 months. This compares well with historic fixed bearing series. Preoperative OKS scores were available for 50 knees, scores were available for 63 knees at 9–48 months and 52 knees at 61–119 months post index operation. There was a significant improvement in the OKS between the preoperative scores (median 26 range 9–36) and early postoperative time points of 9–48 months, (median 42 range 23–48) (p<0.001). At the later postoperative time point of 61–119 months the score had been maintained (42 range 10–48). Conclusion. Lateral UKA is a small and safe procedure, with clinical outcomes that are equivalent to a medial UKA and are maintained at a median of 7 years postoperatively

INTRODUCTION. Meniscal tears are very common and treated surgically by suturing or partial or total meniscectomy. After meniscectomy, the tibiofemoral contact area is decreased whih leads to higher contact stresses associated with clinical symproms and a faster progression of tibiofemoral osteoarthritis. Besides meniscus allograft transplantation, artificial implants have been developed to replace the menisci after meniscectomy. AIM. We investigated the short- and medium-term clinical results and survivorship of two artificial meniscus implants used as a treatment for post-meniscectomy pain in young to middle-aged patients: the anchored polyurethane degradable Actifit® (2007–2013) and the non-anchored polycarbonate-urethane NUSurface® meniscal implants (2011–2013). PATIENTS AND METHODS. Sixty-seven Actifit were implanted in 67 patients with a mean age of 30.5 years (12 to 50) as a lateral meniscus replacement in 24 cases and medial in 43. Forty-one NUSurface were implanted as a medial meniscus replacement in 35 patients aged 31 to 61 at surgery. Clinical scores (KOOS, Lysholm, IKDC, VAS and EQ-5D) and MRI were obtained preoperatively and at 1, 2 and 5 years (if applicable) postoperatively. Complications and reinterventions were noted and cumulative implant survivorship computed. RESULTS. Seventeen Actifit had been removed at a mean of 22 months postop for persisting pain and/or extrusion of the implant on MRI. Three were converted to a Total Knee Arthroplasty (TKA), 7 replaced by a meniscal allograft and 7 were removed. The cumulative survivorship was 63.6% at 6 years. Forty-six patients with the Actifit in situ had a significant improvement of all clinical scores compared to preoperative scores (p<0.05) and were satisfied with the result. In the NUSurface group, 19 of the 41 implanted menisci had been removed at 2 to 26 months follow-up. Reasons for failure were radial tear or rupture of the meniscus in 7 cases, dislocation with or without tear in 4, synovitis and hydrops in 2 possibly related to synovial reaction to polymer particles, medial pressure caused by a too large size in 3 and persistent pain or OA evolution in 3. In 5 patients the implant was replaced by the same type artificial meniscus, unsuccessfully in 3. Besides, complications occurred in 32 patients including inflammation, effusion and squeaking. Nevertheless the KOOS, IKDC and VAS for pain were significantly improved at 12 months postoperatively (p<0.001). DISCUSSION. Short-term clinical evaluation of artificial meniscal replacement after meniscectomy showed an initial improvement of pain and knee function. However, both in the anchored degradable polyurethane meniscal implant group as in the non-anchored polycarbonate-urethane implant group the number of short-term failures was high and associated with important morbidity. The hard, non-degradable polycarbonate-urethane meniscal implant was torn or worn in 7 cases within 2 years postop. Three patients had synovial reactions possibly related to particulate debris. In conclusion, the currently available artificial meniscal transplants have a too high short-term failure rate to be advocated for widespread clinical use

Early stabilisation after an anterior cruciate ligament (ACL) rupture reduces future meniscal injury. We may therefore expect protection of articular cartilage from ACL reconstruction, but this has yet to be shown. Our aim wasto determine the effect of meniscal injury on the long term risk of osteoarthritis (OA) following ACL reconstruction using Single Photon Emission Computed Tomography (SPECT, a 3 dimensional radionuclide scan). We studied a prospective series of 31 patients (mean age at injury of 29 years) who had bone-patellar tendon-bone ACL reconstruction for unstable, ACL deficient knees. Mean follow-up was 10 years (range 9-13). Patients were separated into two groups according to the status of their menisci at the time of ACL reconstruction, those with intact menisci in group 1 (n=15) and those who required partial meniscectomy in group 2 (n=16). The contra-lateral normal knee was used as a control. All knees were clinically stable with high clinical scores (mean Lysholm score 93 and mean Tegner activity score 6). In group 1 (intact menisci) only one patient (7%) had clinical symptoms of OA and was the only patient with increased uptake on SPECT compatible with early OA. In group 2 (partial meniscectomy), two had clinical symptoms of osteoarthritis, and five patients (32%) had increased uptake on SPECT compatible with early OA. None of the control knees had early OA on SPECT. The prevalence of OA 10 years post ACL reconstruction, using the most sensitive investigation available, is very low in patients who had intact menisci (7%), but increases 5 fold (32%) if a meniscal tear was present. We recommend early ACL reconstruction to preserve the menisci to minimise the long term risk of OA

Aim. To assess the efficacy and ease of use of the Oxford Knee Score (OKS) in soft tissue knee pathology. Method. In a prospective study, we compared the OKS against the International Knee Documentation Committee 2000 (IKDC) and the Lysholm Scores (Lys). We also assessed the OKS with retrograde (Reversed OKS: 48=worst symptoms, 0=asymptomatic) and antegrade (as currently used in Oxford) numbering. All patients completed 3 questionnaires (OKS, Lys, and IKDC, or RevOKS, Lys, and IKDC) stating which was the simplest from their perspective. We recruited 93 patients from the orthopaedic and physiotherapy clinics. All patients between the ages of 15 and 45 with soft tissue knee derangements, such as ligamentous, and meniscal injuries were included. Exclusions were made in patients with degenerative and/or inflammatory arthritidis. Patients who had sustained bony injuries or underwent bony surgery were also excluded. Results. The distribution of the soft tissue injuries was: Meniscal tears (35%), anterior cruciate ligament injuries (23%), anterior knee pain (22%), other injuries (12%), and collateral ligament damage (8%). Linear regression analysis revealed no significant difference between all 3 scores (R squared=0.7823, P<0.0001). The OKS correlated best with the IKDC (r=0.7483), but less so with the Lys (r=0.3278). The reversed OKS did not correlate as well (R squared= 0.2603) with either the IKDC (r= -0.2978) or the Lys (r=-0.2586). ANOVA showed the OKS to be significantly easier than Lys to complete (p<0.0001), but not significantly easier than IKDC (p>0.05). Conclusion. The OKS is patient friendly and reliable in assessing soft tissue knee injury. This is particularly useful if the OKS is already in use within a department for assessment of degenerative disease. The Oxford Knee score should be used in an antegrade fashion (with a score of 48/48 corresponding to maximum symptoms) to give the best results in objective assessment

Introduction:. Unicompartmental knee arthroplasty (UKA) has gained renewed interest in recent years as an alternative to total knee arthroplasty (TKA) for limited degenerative disease of the knee. However, recurrence of symptoms may be seen, compromising long-term survivorship. Arthroscopy has been used to for diagnosing and treating symptoms in problematic TKA. The aim of this study was to investigate the outcome of arthroscopic procedures in symptomatic UKA. We hypothesized that arthroscopy is a viable tool in diagnosing and treating patients with recurrent symptoms after UKA and identifying patients that will benefit from revision surgery. Methods:. A search of the institution's joint registry was conducted to identify patients who underwent arthroscopy following UKA between 2003 and 2011. Medical records were analyzed for patient demographics and comorbidities. Clinical results following arthroscopy were evaluated using the Oxford Knee Score. All patient radiograph were assessed for component alignment and loosening. Results:. A total number of 76 arthroscopies were performed on 76 patients (45 females, 31 males) with an average age of 63.3(range 41–75) years. Patients were followed for a mean of 32 months (range, 8 to 84 months). The average duration of symptoms prior to arthroscopy was 13 months (range 1 month to 69 months) from primary UKA. Patients presented with the following symptoms: mechanical problems (instability, limited range of motion, locking and clicking) in 71%, painful prosthesis in 25%, and suspicion of periprosthetic infection in 4% of cases. Arthroscopic treatment consisted of irrigation, removal of impinging tissue or loose body as indicated. Operative diagnoses included, but were not limited to arthrofibrosis, impinging hypertrophic synovitis, chondromalacia, loose fragments, medial or lateral meniscal tear, prosthesis loosening or wear, partial anterior cruciate ligament rupture, and an infrapatellar spur. Eight patients required repeat arthroscopy and at an average follow-up of 18 months (range 1–58 months), 80% of patients had no recurrent symptoms. Fourteen patients required revision to a TKA (18%) for reasons diagnosed by arthroscopy (Table 1). One patient had an additional patellofemoral arthroplasty (1%) and another patient required revision of the femoral component (1%). At final follow-up, the mean Oxford Knee Score was 30 (range, 8–48). Discussion and conclusion:. With the help of arthroscopy after UKA, symptoms in 80% of patients were diagnosed and treated successfully. Furthermore, arthroscopy established the need for revision TKA or other additional procedures in 20% of cases. The use of arthroscopy in patients following problematic UKA in the current study proved to be an effective method of treating and identifying the underlying cause of patient symptoms
