The presence of hip osteoarthritis is associated with abnormal spinopelvic characteristics. This study aims to determine whether the pre-operative, pathological spinopelvic characteristics “normalize” at 1-year post-THA. This is a prospective, longitudinal, case-control matched cohort study. Forty-seven patients underwent pre- and post- (at one-year) THA assessments. This group was matched (age, sex, BMI) with 47 controls/volunteers with well-functioning hips. All participants underwent clinical and radiographic assessments including lateral radiographs in standing, upright-seated and deep-flexed-seated positions. Spinopelvic characteristics included change in lumbar lordosis (ΔLL), pelvic tilt (ΔPT) and hip flexion (pelvic-femoral angle, ΔPFA) when moving from the standing to each of the seated positions. Spinopelvic hypermobility was defined as ΔPT>30° between standing and upright-seated positions. Pre-THA, patients illustrated less hip flexion (ΔPFA −54.8°±17.1° vs. −68.5°± 9.5°, p<0.001), greater pelvic tilt (ΔPT 22.0°±13.5° vs. 12.7°±8.1°, p<0.001) and greater lumbar movements (ΔLL −22.7°±15.5° vs. −15.4°±10.9°, p=0.015) transitioning from standing to upright-seated. Post-THA, these differences were no longer present (ΔPFApost −65.8°±12.5°, p=0.256; ΔPTpost 14.3°±9.5°, p=0.429; ΔLLpost −15.3°±10.6°, p=0.966). The higher prevalence of pre-operative spinopelvic hypermobility in patients compared to controls (21.3% vs. 0.0%; p=0.009), was not longer present post-THA (6.4% vs. 0.0%; p=0.194). Similar results were found moving from standing to deep-seated position post-THA. Pre-operative, spinopelvic characteristics that contribute to abnormal mechanics can normalize post-THA following improvement in hip flexion. This leads to patients having the expected hip-, pelvic- and spinal flexion as per demographically-matched controls, thus potentially eliminating abnormal mechanics that contribute to the development/exacerbation of hip-spine syndrome

Functional outcomes are important for patients with bone tumors undergoing lower extremity endoprosthetic reconstruction; however, there is limited empirical evidence evaluating function longitudinally. The objective of this study was to determine the changes in function over time in patients undergoing endoprosthetic reconstructions of the proximal femur, distal femur and proximal tibia. We conducted a secondary analysis of functional outcome data from the Prophylactic Antibiotic Regimens in Tumor Surgery (PARITY) trial. Patient function was assessed with the Musculoskeletal Tumor Society Score 93 (MSTS) and the Toronto Extremity Salvage Score (TESS), which were administered preoperatively and at 3, 6 and 12 months postoperatively. Both instruments are scored from 0-100, with higher scores indicated greater function. Mean functional scores were evaluated over time and we explored for differences among patients undergoing proximal femur reconstructions (PFR), distal femur reconstructions (DFR) and proximal tibia reconstructions (PTR). The patient-importance of statistically significant differences in function was evaluated utilizing the minimally important difference (MID) of 12 for the MSTS and 11 for the TESS. We explored for differences in change scores between each time interval with paired t-tests. Differences based on endoprosthetic reconstruction undertaken were evaluated by analysis of variance and post-hoc comparisons using the Tukey test. A total of 573 patients were included. The overall mean MSTS and TESS scores were 77.1(SD±21) and 80.2(SD±20) respectively at 1-year post-surgery, demonstrating approximately a 20-point improvement from baseline for both instruments. When evaluating change scores over time by type of reconstruction, PFR patients experienced significant functional improvement during the 3-6 and 6-12 month follow-up intervals, DFR patients demonstrated significant improvements in function at each follow-up interval, and PTR patients reported a significant decrease in function from baseline to 3 months, and subsequent improvements during the 3-6 and 6-12 month intervals. On average, patients undergoing endoprosthetic reconstruction of the lower extremity experience important improvements in function from baseline within the first year. Patterns of functional recovery varied significantly based on type of reconstruction performed. The results of this study will inform both clinicians and patients about the expected rehabilitation course and functional outcomes following endoprosthetic reconstruction of the lower extremity

INTRODUCTION. The purpose of this study is to elucidate longitudinal kinematic changes of the hip joint during heels-down squatting after THA. METHODS. 66 patients with 76 primary cementless THAs using a CT-based navigation system were investigated using fluoroscopy. An acetabular component and an anatomical femoral component were used through the mini-posterior approach with repair of the short rotators. The femoral head size was 28mm (9 hips), 32mm (12 hips), 36mm (42 hips), and 40mm (12 hips). Longitudinal evaluation was performed at 3 months, 1 year, and 2≤ years postoperatively. Successive hip motion during heels-down squatting was recorded as serial digital radiographic images in a DICOM format using a flat panel detector. The coordinate system of the acetabular and femoral components based on the neutral standing position was defined. The images of the hip joint were matched to 3D-CAD models of the components using a2D/3D registration technique. In this system, the root mean square errors of rotation was less than 1.3°, and that of translation was less than 2.3 mm. We estimated changes in the relative angle of the femoral component to the acetabular component, which represented the hip ROM, and investigated the incidence of bony and/or prosthetic impingement during squatting (Fig.1). We also estimated changes in the pelvic posterior tilting angle (PA) using the acetabular component position change. In addition, when both components were positioned most closely during squatting, we estimated the minimum angle (MA) up to theoretical prosthetic impingement as the safety margin (Fig.2). RESULTS. No prosthetic or bony impingement and no dislocation occurred in any hips. The mean maximum hip flexion ROM was 92.4° (range, 76.6° – 107.9°) at 3 months, 103.4° (range, 81.5° – 115.2°) at 1 year, and 102.4° (range, 87.1° – 120.6°) at 2≤ years (3 months vs 1 year, p<0.05; 1 year vs 2≤ years, p>0.05, paired t-test). The mean PA was 26.7° (range, 0.9° – 49.8°) at 3 months, 21.7° (range, 3.4° – 43.8°) at 1 year, and 21.2° (range, −0.7° – 40.4°) at 2≤ years (3 months vs 1 year, p<0.05; 1 year vs 2≤ years, p>0.05). The mean flexion ROM and MA at 2≤ years were 98.4±20.8° and 14.3±7.3° in 28 mm heads, 102.3±10.7° and 15.6±4.8° in 32 mm heads, 102.8±14.5° and 20.3±9.6° in 36 mm heads, and 103.2±16.9° and 23.4±10.9° in 40 mm heads, respectively. There were no significant differences in the hip flexion ROM between 28, 32, 36, and 40 mm head cases, whereas MA significantly increased as the femoral head diameter was larger (p<0.05, unpaired t-test). DISCUSSION AND CONCLUSION. Three-dimensional assessment of dynamic squatting motion after THA using the 2D/3D registration technique enabled us to elucidate longitudinal kinematic change of the hip joint. Longitudinal kinematic analysis indicated that hip flexion ROM and posterior tilt during squatting changed significantly by 1 year postoperatively, and there were no significant changes after 1 year while safety margin kept > 10°. Title. Longitudinal Intravital Imaging to Quantify the “Race for the Surface” Between Host Immune Cell and Bacteria for Orthopaedic Implants with S. aureus Colonization in a Murine Model. Aim. To assess S. aureus vs. host cell colonization of contaminated implants vis intravital multiphoton laser scanning microscopy (IV-MLSM) in a murine model. Method. All animal experiments were approved by IACUC. A flat stainless steel or titanium L-shaped pin was contaminated with 10. 5. CFU of a red fluorescent protein (RFP) expressing strain of USA300LAC, and surgically implanted through the femur of global GFP-transgenic mice. IV-MLSM was performed at 2, 4, and 6 hours post-op. Parallel cross-sectional CFU studies were performed to quantify the bacteria load on the implant at 2,4,6,12,18 and 24 hours. Results. 1) We developed a high-fidelity reproducible IV-MLSM system to quantify S. aureus and host cell colonization of a bone implant in the mouse femur. Proper placement of all implants were confirmed with in vivo X-rays, and ex vivo photos. We empirically derive the ROI during each imaging session by aggregating the imaged volume which ranges from (636.4um × 636.4um × 151um) = 0.625 +/- 0.014 mm. 3. of bone marrow in a global GFP-transgenic mouse. 2) IV-MLSM imaging acquisition of the “race for the surface”.In vitro MPLSM images of implants partially coated with USA300LAC (RFP-MRSA) were verified by SEM image. Results from IV-MLSM of RFP-MRSA and GFP. +. host cell colonization of the contaminated implants illustrated the mutually exclusive surface coating at 3hrs, which to our knowledge is the first demonstration of “the race for the surface” between bacteria and host cells via intravital microscopy. 3) Quantifying the “race for the surface” with CFU verification of S. aureus on the implant. 3D volumetric rendering of the GFP. +. voxels and RFP+ voxels within the ROI were generated in Imaris. The voxel numbers suggeste that the fight for the surface concludes ∼3hrs post-infection, and then transitions to an aggressive MRSA proliferation phase. The results of WT control demonstrate a significant increase in CFU by 12hrs post-op for both stainless steel (P<0.01) and titanium (P<0.01). Conclusions. We developed IV-MLSM to quantify the “Race for the Surface” between host cells and contaminating S. aureus in a murine femur implant model. This race is remarkably fast, as the implant surface is completely covered with 3hrs, peak bacterial growth on the implant occurs between 2 and 12 hours and is complete by 12hrs

BACKGROUND. Early dislocation is a foremost complication of total hip arthroplasty through a postero-lateral approach. The extra-articular impingement of the anterior part of the great trochanter with ileum bone, with or without soft tissue interposition is a well recognized but underestimated etiopathogenetic cause reported in literature. In this retrospective study through the assessment of clinical and radiographic follow-up at a minimum of six months, the effectiveness of an antero- longitudinal osteotomy of the great trochanter for early dislocation prevention is evaluated. MATERIALS AND METHODS. 209 patients (48.3% males and 51,7% females) underwent a total hip arthroplasty from June 2011 to September 2015, with surgery being performed by the same surgeon. A modified posterolateral approach was used according to the tissue-sparing criteria, in all the cases an anterior longitudinal osteotomy of the great trochanter has been performed at 90° to the antiversion angle of the implant and aligned posteriorly with the prosthesis. All the patients underwent a clinical and radiological follow up at one, three, and six months. RESULTS. In this study, only one patient reported dislocation of THA. One patient suffered from a wound infection which was subsequently treated with antibiotics and had complete remission. All patients demonstrated a fast recovery of ROM and walking, starting from pre-op Harris Hip Score 42.24pts and obtaining a score of 81.52pts at three months, and 92.03 at six months post-op. After surgery and during the follow up period, there were no trochanteric fractures detected. DISCUSSION. The correct positioning of the implants, the head diameter, offset, soft tissues repair, absence of impingement, and patients compliance are all elements that define the prosthetic stability. Literature shows and incidence of primary total hip arthroplasty dislocation between 0.80% to 10%. The incidence of dislocation reported in a preliminary study in our Institute is 0.48%, demonstrating the effectiveness of the trochanteric osteotomy. CONCLUSIONS. The osteotomy of the great trochanter is an effective surgical technique used to decrease the anterior impingement and early dislocation incidence. It is particularly effective on patients with good compliance and correctly implanted prosthetic components

Radial head fractures are common and mainly require a functional conservative treatment. About 20% of patients will present an unsatisfactory final functional result. There is, however, little data allowing us to predict which patients are at risk of bad evolve. This makes it difficult to optimize our therapeutic strategies in these patients. The aim of this study is to determine the personal and environmental factors that influence the functional prognosis of patients with a radial head fracture. We realized over a 1-year period a prospective observational longitudinal cohort study including 125 consecutive patients referred for a fracture of the radial head in a tertiary trauma center. We originally collected the factors believed to be prognostic indicators: age, sex, socioeconomic status, factors related to trauma or fracture, alcohol, tobacco, detection of depression scale, and financial compensation. A clinical and radiological follow-up took place at 6 weeks, 3 months, 6 months, and 1 year. The main functional measurement tool is the Mayo Elbow Performance Score (MEPS) and the Disabilities of the Arm, Shoulder and Hand (DASH). 123 patients were included in the study. 114 patients required nonsurgical management. 102 patients completed the 1-year follow-up for the main outcome (89 for the DASH score). Two patients required an unplanned surgery and were excluded from analyses. At 1 year, the average MEPS was 96.5 (range, 65–100) and 81% of subjects had an excellent result (MEPS ≥90). The most constant factor to predict an unsatisfactory functional outcome (MEPS <90 or DASH >17) is the presence of depressive symptoms at the initial time of the study (P = 0.03 and P = 0.0009, respectively). This factor is present throughout the follow-up. Other observed factors include a higher socioeconomic status (P = 0.009), the presence of financial compensation (P = 0.027), and a high-velocity trauma (P = 0.04). The severity of the fracture, advanced age, female sex, and the nature of the treatment does not influence the result at 1 year. No factor has been associated with a reduction in range of motion. Most of the radial head fractures heal successfully. We identified for the first time, with a valid tool, the presence of depressive symptoms at the time of the fracture as a significant factor for an unsatisfactory functional result. Early detection is simple and fast and would allow patients at risk to adopt complementary strategies to optimize the result

Introduction. Subtle deformities of the acetabulum and proximal femur are recognised as biomechanical risk factors for the development of hip osteoarthritis (OA) as well as a cause of hip and groin pain. We undertook this study to examine relationships between a number of morphological measurements of the acetabulum and proximal femur and the hip pain in a 20-year longitudinal study. Methods. In 1989 women of 45–64 years of age were recruited. Each had an AP-Pelvis radiograph at Year-2. These radiographs were analysed using a validated programme for measuring morphology. All morphological measurements were read blinded to outcome. At year 3 all participants were asked whether they experienced hip pain (side specific). This was repeated at visits up to and including 20-years. Logistic regression analysis (with robust standard errors and clustering by subject identifier) was performed using hip pain as a binary outcome. The model adjusted for baseline age, BMI and joint space and included only participants who were pain free on initial questioning. Results. 743 participants were included in the analysis. Median age 74.0. Pain was reported in 14.2% of hips. Logistic regression analyses revealed that extrusion index and LCE were significantly associated with hip pain before and after adjusting for covariates (OR 4.88[95%CI 1.32–17.97, p=0.017] and 0.84[95%CI 0.74–0.96, p=0.012] respectively). Modified triangular index height (MTIH) was also significantly associated after adjusting for covariates (OR 1.10[95%CI 1.01–1.20, p=0.022]). Extrusion index and MTIH were independently associated with hip pain at 20-years when used in the same model. No significant interaction was identified. Conclusions. This study provides evidence that measurements of hip morphology characteristic of previously undiagnosed dysplasia and FAI are predictive of hip pain in a 20-year longitudinal study. MTIH, LCE and Extrusion index were significant predictors of pain. This is the first study to describe these associations between hip morphology and pain in a longitudinal cohort

Background. Well-fixed cementless stems are sometimes needed to be extracted in patients with complications including periprosthetic infection, stem-neck breakage or trunnionosis. The purpose of this study was to report the clinical outcome in patients undergoing re-implantation surgery following removal of a well-fixed porous-coated cementless stem by the femoral longitudinal split (FLS) procedure(Fig.1, Fig.2). Methods. We conducted a retrospective study and radiographic review of 16 patients who had undergone re-implantation following the FLS procedure to remove a well-fixed stem due to periprosthetic infection, stem-neck breakage or trunnionosis. The study group consisted of 2 men and 14women with an average age of 68.4 years. Mean follow-up was 33.1± 25.0 months. Operation time, intraoperative bleeding, complications, causes of re-operation and clinical score were evaluated and the Kaplan-Meier method was used to evaluate the longevity of the stem. Results. The average operation time was 272±63 minutes and intraoperative bleeding was 420±170 ml. Although postoperative dislocation occurred in 5 hips and sinking of the stem was found in 3 hips after surgery, no progression of the stem sinking was observed and the clinical JOA and JHEQ scores were both improved after re-implantation surgery. Re-implantation surgery with Zweymüller-type stems, which are shorter than those removed, revealed evidence of osseointegration of the stem without femoral fracture. Kaplan-Meier survival analysis of stem revision for any reason as the end point revealed 70.3% survival at 9 years (Fig.3). Conclusion. The FLS procedure is expected to confer successful clinical results without loosening of the stem, following safe extraction of well-fixed porous-coated cementless stems without fracture and will allow re-implantation with shorter cementless stems than those removed. Objectives

Post-infective arthritis is an important sequalae of septic arthritis(SA). While total knee arthroplasty(TKA) is an effective treatment for said arthritis, previous SA brings challenges for treatment planning. Using prospectively collected data from a cohort of patients with knee SA, this study aims to determine the proportion of patients requiring eventual TKA, and risk factors of developing prosthetic joint Infection(PJI).

Methods

All cases of 1st episode knee SA from 01/01/2000 to 31/12/2020 were identified in the Auckland region. Patient records and NZJR records of all cases were searched to identify subsequent TKA. PJI following arthroplasty was identified using ICM criteria.

Univariate and multivariate analysis was performed to determine risk factors for developing PJI.

Objectives. Delayed gadolinium enhanced MRI of cartilage (dGEMRIC) is a novel MRI-based technique with intravenous contrast agent that allows an objective quantification of biochemical cartilage properties. It enables a ‘monitoring' of the loss of cartilage glycosaminoglycan content which ultimately leads to osteoarthritis. Data regarding the longitudinal change of cartilage property after joint preserving hip surgery is sparse. We asked (1) if and how the dGEMRIC-index changes in patients undergoing open/arthroscopic treatment of femoroacetabular impingement (FAI) one year postoperatively compared to a control group of patients with non-operative treatment; (2) and if a change correlates with the clinical short term outcome. Methods. IRB-approved prospective comparative longitudinal study of two groups involving a total of 61 hips in 55 symptomatic patients with FAI. The ‘operative' group consisted of patients that underwent open/arthroscopic treatment of their pathomorphology. The ‘non-operative' group consisted of conservatively treated patients. Groups were comparable for preoperative radiographic arthritis (Tönnis score), preoperative HOOS- and WOMAC-scores and baseline dGEMRIC indices. All patients eligible for evaluation had preoperative radiographs and dGEMRIC scans at baseline and repeated dGEMRIC scans using the same scanner and protocol. (1) dGEMRIC indices of femoral and acetabular cartilage were assessed separately on the initial and follow-up dGEMRIC scans. Radial images were reformatted from a 3D T1 map for measurements. Regions of interest were placed manually peripherally and centrally within the cartilage based on anatomical landmarks at the 12 ‘hour' position of the clcok-face with the help of radial high-resolution PD-weighted MR images. (2) Patient-reported outcome was evaluated at baseline and at 1 year follow-up: Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and Hip disability and Osteoarthritis Outcome Score (HOOS). Statistical analysis included Student's t-Tests, Mann-Whitney U-tests and Wilcoxon signed-rank tests (p<0.05). Results. On the acetabular side, the dGEMRIC index decreased significantly (p<0.05) in 17/20 (85%) zones respectively in 21/24 (88%) of femoral zones in the operated group [Fig. 1]. In the non-operative group, no acetabular zone and 2/24 (8%) femoral zones presented with a significant drop [Fig. 2]. After one year the WOMAC and the HOOS scores significantly improved (58±42 to 33±42; p= 0.007 respectively 63±16 to 74±18; p= 0.028) for the operative group, while there was no change (55±45 to 48±50; p= 0.825 respectively 63±14 to 66±19; p= 0.816) for the non-operative group. Discussion. Interestingly joint-preserving surgery for FAI led to a decline in biochemical cartilage properties on MRI at a one year follow-up despite the significant improvement of patient outcome. This short-term phenomenon was described after periacetabular osteotomy for correction of hip dysplasia in literature with a normalization of the dGEMRIC values at 2 years

Purpose. The purpose of this study was to assess the biomechanical stability of the a total ankle arthroplasty system using longitudinal migration (LM) and inducible displacement (ID) measures. This study is the first study of its kind to assess total ankle arthroplasty (TAA) implant micromotion using model-based radiostereometric analysis (MBRSA). Method. Twenty patients underwent TAA that implanted the Mobility(TM) (DePuy, Warsaw IN). The mean (SD) age was 60.4 (12.5) and BMI was 29.1 (2.8) kg/m. 2. One surgeon performed all surgeries. All patients included in this study had given informed consent. Capital Health Research Ethics Board had approved this study. Uniplanar medial-lateral RSA X-ray exams were taken postop (double exam), at six wk, three mth, six mth, one yr and two yr followup times using a supine, unloaded position. Standing medial-lateral exams were taken at three mth, six mth, one yr and two yr followup intervals. LM and ID micromotions were assessed using Model-based RSA 3.2 software (Medis specials, Leiden, The Netherlands). Implant micromotions (x, y, z, Rx, Ry, Rz, MTPM) were determined and assessed for each subject using model-based pose estimation, and the implant-based coordinate system. The Elementary Geometric Shapes module from the Model-based RSA 3.2 software was used to assess the micromotion of the tibial component spherical tip due to implant symmetry. Results. The median (range) maximum total point motion (MTPM) for the implants at 2 year followup were 1.23 mm (0.39–1.95 mm) for the talar implant and 0.96 mm (0.17–2.28 mm) for the spherical tip of the tibia implant. Generally for each subject and implant component, the slopes of the migration curves decreased over time. The talar and tibial implants mean LM showed initial subsidence in the y-direction (migration into the bone) followed by stabilization patterns at one year followup. The median (range) of two year MTPM ID for the talar component was 0.39 (0.27–1.06) mm. At the one year and two year followup times the ID were almost all below the detection limit of 0.85 mm. The highest measured displacement for any one talar component at either of these times was 1.06 mm. Hence, the implant was displaced at least 0.21 mm under loading. The median (range) of one year and two year MTPM ID for the tibial component spherical tip was 0.08 (0.03–0.19) mm. The tibial component spherical tip demonstrates no ID in terms of MTPM greater than the 0.22 mm detection limit. Conclusion. The implant subsides directly into the bone in the line of primary loading during standing or walking. For most of the patients the two year LM for the Mobility(TM) demonstrates a typical subsidence-stabilization behaviour seen in many RSA studies of orthopaedic implants. Based on the results of this study the Mobility(TM) components show no measurable ID. This is the first study of its kind internationally for total ankle arthroplasty and offers novel insight into the need for prosthetic design change

Introduction. Modern metal-on-metal bearing resurfacings have been in use for nearly two decades. Local and systemic metal ion exposure continues to cause concern. We could not find a prospective metal ion study in such patients with a 10-year follow-up. This is the first ten year prospective study of metal ion levels in blood and their release in urine following hybrid fixed metal-on-metal surface arthroplasty. Methods. Twenty six patients were included in an ongoing longitudinal metal ion study of patients with unilateral metal-on-metal hip resurfacings. Three of them were excluded due to subsequent contralateral resurfacing and one has relocated abroad. Cobalt and chromium levels were assessed in 12 hour urine collections before and periodically after operation (5 days to 10 years) using high resolution plasma mass spectrometry. Mean age at operation was 53 years and mean BMI 27.9. Results. Hip function questionnaires at the 10-year review showed that the patients have well-functioning pain-free resurfacings (mean Oxford hip score 14.3). Metal ion results show median cobalt and chromium release at 10 years were 8.3 μg/24 hr and 4.35 μg/24 hr respectively. A statistically significant early peak 6 months to 1 year after operation is followed by a steady decrease over the following seven years although the reductions are not statistically significant. Discussion and Conclusion. Elevated systemic metal ion exposure continues to cause concern in patients treated with metal-metal arthroplasties. Our results show that metal release in these bearings shows a reducing trend after an initial peak. The unknown significance of persistent metal ion elevation underlines the need for continued long-term monitoring

INTRODUCTION. Trabecular Titanium™ is an innovative material characterised by an high open porosity and composed by multi-planar regular hexagonal cells. It is not a traditional coating and its tri-dimensional structure has been studied to optimise osteointegration. Furthermore, it has excellent mechanical properties, as a very high tensile and fatigue resistance and an elastic module very similar to the that of the trabecular bone. The aim of this study is to evaluate the osteointegration and bone remodelling measuring the longitudinal pattern of change in BMD around a cementless acetabular cup made from Trabecular Titanium™ (Delta TT cup, Lima Corporate, Italy) in primary total hip arthroplasty (THA). METHODS. Dual-energy x-ray absorptiometry (DEXA) analysis, radiographic evaluation on standard AP and lateral views and clinical evaluation with Harris Hip Score (HHS) and SF-36 were performed at 1 week, 3, 6, 12 months after surgery. RESULTS. Between February 2009 and June 2010, 72 patients underwent primary THA with Delta TT cup at 4 centres. There were 36 (50%) female and 36 (50%) male with an average age of 63 (range 39-75). Preliminary results include 72 patients with 56 at 3 months, 42 at 6 months and 28 at 12 months. The mean HHS improved from 48 points (range 14-79) preoperatively to 88 (range 74-100) at 3 months, to 94 (range 71-100) at 6 months and to 97 (range 94-100) at 12 months. SF-36 reported a progressive improvement of all domains. An initial transient decrease of the bone mineral density (BMD) occurred between 1 week and 3 months after surgery, then bone mass returns to baseline values after 6 months and increased at 12 months. No radiolucent lines nor osteolysis have reported and no revision occurred. There were 2 complications not related to the implant on study: one dislocation solved with a close reduction and one dislocation solved with a revision of the neck and the head, but not compromising the cup. CONCLUSIONS. Delta TT acetabular cup allows to obtain a very good primary stability, thanks to the high friction coefficient of the Trabecular Titanium™, followed by a good and fast osteointegration, as reported by early DEXA outcomes. Preliminary results showed also a very good clinical and functional improvement. Long-term follow-up is ongoing in order to prove these promising results

Background. The optimal management of idiopathic clubfoot has changed over three decades. Recently there has been an enthusiastic embracing of the Ponseti technique. The purpose of this 14-year comparative prospective longitudinal study was to directly assess the differences in results between these two treatment methods. Methods. Over the period of this study there were 52,514 births in the local population and all newborns with clubfoot were referred directly to the Pediatric Orthopedic Surgeon. Patient demographics, the Harrold & Walker Classification, and associated risk factors for clubfoot were collected prospectively and analyzed. If conservative treatment failed to correct the deformity adequately, a radical subtalar release (RSR) was undertaken (the primary outcome measure of the study). Results. There were 114 feet (80 patients): 64 feet treated ‘traditionally’ and 50 feet with the Ponseti technique. Idiopathic clubfoot was present in 76.25% of patients. Mean time to RSR was 33.3 and 44.1 weeks for the traditional and Ponseti groups respectively. In the traditional group 65.6% (CI: 53.4 to 76.1%) of feet underwent RSR surgery compared to 25.5% (CI: 15.8 to 38.3%) in the Ponseti group. When idiopathic clubfoot alone was analysed, these rates reduce to 56.5% (CI: 42.3 to 69.8%) and 15.8% (CI: 7.4 to 30.4%) respectively. The Relative Risk of requiring RSR in traditional compared to Ponseti groups was 2.58 (CI: 1.59 to 4.19) for all patients and 3.58 (CI: 1.65 to 7.78) for idiopathic clubfoot. Conclusions. Introduction of the Ponseti technique into our institution significantly reduced the need for RSR in fixed clubfoot

Purpose. It is well known that meniscus extrusion is associated with structural progression of knee OA. However, it is unknown whether medial meniscus extrusion promotes cartilage loss in specific femorotibial subregions, or whether it is associated with a increase in cartilage thickness loss throughout the entire femorotibial compartment. We applied quantitative MRI-based measurements of subregional cartilage thickness (change) and meniscus position, to address the above question in knees with and without radiographic joint space narrowing (JSN). Methods. 60 participants with unilateral medial OARSI JSN grade 1–3, and contralateral knee OARSI JSN grade 0 were drawn from the Osteoarthritis Initiative. Manual segmentation of the medial tibial and weight-bearing medial femoral cartilage was performed, using baseline and 1-year follow-up sagittal double echo steady-state (DESS) MRI, and proprietary software (Chondrometrics GmbH, Ainring, Germany). Segmentation of the entire medial meniscus was performed with the same software, using baseline coronal DESS images. Longitudinal cartilage loss was computed for 5 tibial (central, external, internal, anterior, posterior) and 3 femoral (central, external, internal) subregions. Meniscus position was determined as the % area of the entire meniscus extruding the tibial plateau medially and the distance between the external meniscus border and the tibial cartilage in an image located 4mm posterior to the central image (a location commonly used for semi-quantitative meniscus scoring). The relationship between meniscus position and cartilage loss was assessed using Pearson (r) correlation coefficients, for knees with JSN and without JSN. Results. The percentage of knees showing a quantitative value of >3mm medial meniscus extrusion was 50% in JSN knees, and only 12% in noJSN knees. The 1-year cartilage loss in the medial femorotibial compartment was 74±182µm (2.0%) in JSN knees, and 26±120µm (0.8%) in noJSN knees. There was a significant correlation between cartilage loss throughout the entire femorotibial compartment (MFTC) and extrusion area in JSN knees but not for noJSN knees. Also, the extrusion distance measured 4mm posterior to the central slice was not significantly correlated with MFTC cartilage loss. The strongest (negative) correlation between meniscus position and subregional femorotibial cartilage loss (r=−0.36) was observed for the external medial tibia. In contrast, no significant relationship was seen in the central tibia. No significant relationship was found in other tibial subregions, except for the anterior medial tibia, but only in JSN knees (r=−0.27). Correlation coefficients for the femoral subregions were generally smaller than those for tibial subregions, with only the internal medial weight-bearing femur attaining statistical significance (r =−0.26). Conclusions. The current results show that the relationship between meniscus extrusion and cartilage loss differs substantially between femorotibial subregions. The correlation was strongest for the external medial tibia, a region that is physiologically covered by the medial meniscus. It was less for other tibial and femoral subregions, including the central medial tibia, a region that exhibited similar rates of cartilage loss as the external subregion. The findings suggest that external tibia may be particularly vulnerable to cartilage tissue loss once the meniscus extrudes and the surface is “exposed” to direct, non-physiological, cartilage-cartilage contact

We wished to assess the usefulness of Inspace balloon arthroplasty (IBA), in our Upper limb unit by regular patient reviews prospectively. This prospective study was started adhering to local approval process. Patients were identified in routine clinical practice by three upper limb consultants presenting with pain and disability and diagnosis of cuff tear was established clinically and on MRI. The patients were explained of ‘available’ treatment options and an information leaflet about balloon arthroplasty provided. Patients were seen in 4 weeks, in a dedicated clinic to find out their willingness to participate. Consultant shoulder surgeons carried out all Balloon arthroplasties and where the cuff was reparable or contraindications identified were excluded from study.

All the patients were reviewed by an independent clinician pre-operatively and post operatively using the assessment tools of Oxford Shoulder Scores (OSS), SF12 and VAS at 6 weeks, followed by 3, 6, 12 months and annually after.

We performed a total of 32 IBA procedures. At the latest follow-up we identified that 24 patients have completed 2 years post procedure. Of these patients 1 deceased after 6 months, 3 were lost to follow up one each at 6, 12 and 24 months. Three were revised to reverse TSA. The OSS reflects that the positive difference of 10 in the average scores (24 pre-operative versus 34 at 2 years) noted at one year follow up were maintained at 2 years. Similar observations made analysing at 12 months and 2 years scores for the VAS pain scores of 3 at 2 years compared to pre-operative score of 6 and so were improvements in both physical & mental components of SF12.

Our study observes that the IBA provides a sustainable pain relief and functional improvements over 2 years and may be a suitable alternative in physiologically compromised patients with irreparable RCT.

There are some reports that the invasive surgery of knee joint replacement repair static and dynamic balance. We investigated the changes in static and dynamic balance and muscle strength in pre- and postoperative of TKA and UKA for the purpose of assessing time dependent improvement.

A total of 168 patients (137 TKA; mean age 75.3, 31 UKA; mean age 78.1) were recruited to the study. These patients underwent static and dynamic balance assessment and muscle strength pre operation and 3, 6, 12 months post operation. The parameters of assessment were one leg standing time (open or close eyes), postural sway test (open or close eyes), 3m timed-up-and-go test, maximum stride and Isokinetic muscle strength. We have evaluated both the absolute value and the index which divided the value of the post-operation with the value of pre-operation.

Alignment had improved significantly after surgery in TKA and UKA. Isokinetic muscle strength (Fig. 1), one leg standing time with open eyes, 3m timed-up-and-go test (Fig. 2) and maximum stride showed better improvement than pre operation at 3, 6, and 12 months after surgery in TKA and UKA. On the other hand, one leg standing time with close eyes and postural sway test showed no improvement than pre operation at any time after operation (Fig. 3). Butpostural sway test in UKA showed the improvement trend at 3 months after surgery. In contrast, those test in TKA showed no improvement at 3 months after surgery (Fig. 3).

Our result showed the improvement of balance function correlated with muscle recovery and improvement of lower limb alignment than equilibrium function after the artificial knee joint replacement surgery. Because one leg standing time with close eyes and one leg postural sway test represent the equilibrium function than other tests. It is interesting that significant difference in the recovery of postural sway in three months after surgery in UKA compared with TKA.

Trabecular TitaniumTM is a tri-dimensional material composed by multi-planar regular hexagonal cells and characterised by a highly open porosity that has been studied to optimise bone osteointegration. The aim of this study is to evaluate bone remodelling measuring BMD changes around an acetabular cup made from Trabecular TitaniumTM in primary total hip arthroplasty (THA).

Between February 2009 and December 2010, 89 patients (91 hip) underwent primary THA with a modular acetabular cup in Trabecular TitaniumTM (DELTA-TT cup, Limacorporate, Villanova di San Daniele, Italy). The average age was 63.5± 9.4 years, the average height and weight were 75.9± 12.9 kg and 168.8± 8.9 cm, respectively (av. BMI 26.8± 4.2). There were 46 (51.7%) males and 43 (48.3%) females affected by primary coxarthrosis in 80 (87.9%) cases, avascular necrosis in 5 (5.5%), posttraumatic coxarthrosis in 3 (3.3%), dysplasia in 2 (2.2) and oligoarthritis in 1 (1.1%) case. The study includes the clinical evaluation with Harris Hip Score (HHS) and SF-36, radiographic evaluation and dual-energy x-ray absorptiometry (DEXA) analysis preoperatively and postoperatively at 1 week, 3, 6, 12 and 24 months.

Preliminary results are currently available for 47 patients at 12 months, 68 at 6 months and 80 at 3 months. The average HHS significantly improved from 48.7± 14.99 preoperatively to 93.8± 5.91 at 12 months, with a constant progression in the intermediate follow-ups. All patients showed a significant ROM increase, with an average flexion from 86.6°± 15.9° preoperatively to 105°±13.14 at 12 months. Sf-36 highlighted a satisfactory improvement of general health status from an average preoperative value of 50.8± 18.7 to 80.7± 12.9 at 12 months (from 42.9 to 80.1 for physical health; from 58.4 to 81.3 for mental one). All cups were stable at 12 months with no radiolucent lines. Preliminary DXA analysis reported an initial bone mineral density decrease from 1 week baseline values (BMD R1: 1.40± 0.37; R2: 1.20± 0.45; R3:1.16± 0.31) to 3 months (BMD R1: 1.31± 0.41; R2: 1.17± 0.3; R3: 1.06± 0.37) followed by BMD recovery up to initial values (BMD R1: 1.37± 0.3; R2:1.18± 0.34; R3: 1.12± 0.36) at 12 months.

Trabecular TitaniumTM demonstrates a good primary and secondary stability. Preliminary densitometric outcome confirms an optimal osseointegration of the DELTA-TT cup and early clinical and patient subjective results are very promising at a short term follow-up. However, the completions of follow-up evaluation are necessary to draw a conclusive analysis.

We sought to evaluate the early post-operative active range-of-motion (AROM) between robotic-assisted total knee arthroplasty (raTKA) and conventional TKA (cTKA). A secondary data analysis on a global prospective cohort study was performed. A propensity score method was used to select a matched control of cTKA from the same database using 1:1 ratio, based on age, sex, BMI, and comorbidity index. This resulted in 216 raTKA and cTKA matched cases. Multivariable longitudinal regression was used to evaluate difference in ROM over time and values are reported as least squares means (95% confidence interval). The longitudinal model tested the treatment effect (raTKA vs cTKA), time effect, and their interaction with control on covariance of patients ‘s age, sex, BMI, comorbidity and pre-operative flexion. Logistic regression was used to analyze the active flexion level at one month (cut by 90°) and three months (cut by 110°). At one-month postoperative the raTKA cases had more AROM for flexion by an average of 5.54 degrees (p<0.001). There was no difference at three months (p=0.228). The raTKA group had a greater improvement from pre-operative values at both one-month, with an average 7.07° (3.6°, 10.5°, p<0.001) more improvement, and at three-months with an average improvement of 4° more (1.61°, 7.24°, p=0.0115). AROM for extension was lower overall in the raTKA group by an average of 0.44° (p=0.029). The raTKA patients had higher odds of achieving ≥90° of flexion at one-month (OR 2.15, 95% CI 1.16, 3.99). raTKA resulted in greater AROM flexion gains in the early postoperative period than cTKA. Additional research is needed to understand if these earlier gains in AROM are associated with improved patient satisfaction and continued improvements with time

Lisfranc injuries account for 0.2% of all fractures and have been linked to poorer functional outcomes, in particular resulting in post-traumatic arthritis, midfoot collapse and chronic pain. This study assesses the longitudinal functional outcomes in patients with low and high energy Lisfranc injuries treated both operatively and non-operatively. Patients above 16 years with Lisfranc injuries from January 2008 and December 2017 were identified through the Victorian Orthopaedic Trauma Outcomes (VOTOR) registry. Follow-up performed at 6, 12 and 24 months through telephone interviews with response rate of 86.1%, 84.2% and 76.2% respectively. Longitudinal functional outcome data using Global Outcome Assessment, EQ-5D-5L, numerical pain scale, Short-Form 12, the WHO Disability Assessment Schedule and return to work status were collected. Univariate analysis was performed and variables showing a significant difference between groups (p < 0.25) were analysed with multivariable mixed effects regression model. 745 patients included in this retrospective cohort study. At 24 months, both the operative and non-operative groups demonstrated similar functional outcomes trending towards an improvement. Mixed effect regression models for the EQ items for mobility (OR 1.80, CI 0.91 – 3.57), self-care (OR 1.95, 95% CI 1.09-3.49), usual activities (OR 1.10, 95% CI 0.99-1.03), pain (OR 1.07, 95% CI 0.61-1.89), anxiety (OR 1.29, 95% CI 0.72-2.34) and pain scale (OR 1.07, 95% CI 0.51 – 2.22) and return to work (OR 1.28, 95% CI 0.56-2.91) between groups were very similar and not statistically significantly different. We concluded that there was no statistically significant difference between operative and non-operative patients with low and high energy Lisfranc injuries. Current clinical practices in Lisfranc injury management are appropriate and not inadvertently causing any further harm to patients. Future research comparing fracture patterns, fixation types and corresponding functional outcomes can help determine gold standard Lisfranc injury management
