We aimed to determine the rate of and risk factors for post-traumatic osteoarthritis (PTOA) and total knee arthroplasty (TKA) requirement after operative management of tibial plateau fractures (TPF) in older adults. We conducted a retrospective cohort study of 182 operatively managed TPFs in 180 patients ≥60 years old over a 12-year period with minimum follow up 1 year. Data including patient demographics, clinical frailty scores, mechanism of injury, management, reoperation and mortality were recorded. Radiographs were reviewed for: Schatzker classification; pre-existing knee osteoarthritis (KOA); severe joint depression >15mm; and development of PTOA. Kaplan Meier survival analysis was performed. Regression analysis was used to identify risk factors for radiographic indication for TKA and actual TKA. Forty-seven percent were Schatzker II fractures. Radiographic KOA was present at fracture in 32.6%. Fracture fixation was performed in 95.6% cases and acute TKA in 4.4%. Thirteen patients underwent late TKA (7.5%). At five-years, 11.8% (6.0-16.7 95% CI) had required TKA and 20.9% (14.4-27.4 95% CI) had a radiographic indication for TKA. Severe joint depression and pre-existing KOA were associated with worse survival for endpoints radiographic indication for TKA and actual TKA. Severe joint depression (HR 2.49(1.35-4.61 95% CI), p=0.004), pre-existing KOA (HR 2.23(1.17-4.23), p=0.015) and inflammatory arthropathy (HR 2.4(1.04-5.53), p=0.039) were independently associated with radiographic indication for TKA. In conclusion, severe joint depression and pre-existing arthritis are independent risk factors for both severe PTOA and TKA after TPFs in older adults. These features should be considered as an indication for primary management with acute TKA

Though knee osteoarthritis (OA) is diagnosed and monitored radiographically, full thickness cartilage loss (FTCL) has rarely been correlated with radiographic classification. This study aims to analyse which classification system correlates best with FTCL and assessing their reliability. Prospective study of 300 consecutive patients undergoing total knee arthroplasty (TKA) for OA. Two blinded examiners independently graded preoperative radiographs using 5 systems: Kellgren-Lawrence (KL); International Knee Documentation Committee (IKDC); Fairbank; Brandt; and Ahlback. Interobserver agreement was assessed using the intraclass correlation coefficient. Intraoperatively, anterior cruciate ligament (ACL) status and FTCL in 16 regions of interest were recorded. Radiographic classification and FTCL were correlated using the Spearman correlation coefficient. On average, each knee had 6.8±3.1 regions of FTCL, most common medially. The commonest patterns of FTCL were medial with patellofemoral (48%) and tricompartmental (30%). ACL status was associated with pattern of FTCL (p=0.02). All classification systems demonstrated moderate ICC, but this was highest for IKDC: whole knee 0.68 (95%CI 0.60–0.74); medial compartment 0.84 (0.80–0.87); and lateral compartment 0.79 (0.73–0.83). Correlation with FTCL was strongest for Ahlback (Spearman rho 0.27–0.39) and KL (0.30–0.33), though all systems demonstrated medium correlation. The Ahlback was the most discriminating in severe OA. Osteophyte presence in the medial compartment had high positive predictive value for FTCL, but not in the lateral compartment. The Ahlback and KL systems had the highest correlation with confirmed cartilage loss. However, the IKDC system displayed best interobserver reliability, with favourable correlation with FTCL in medial and lateral compartments, though less discriminating in severe disease

Introduction. Osteoarthritis (OA) represents a leading cause of disability and a growing burden on healthcare budgets. OA is particularly vexing for young, active patients who have failed less invasive therapies but are not yet candidates for arthroplasty. Often, patients suffering in this wide therapeutic gap face a debilitating spiral of disease progression, increasing pain, and decreasing activity until they become suitable arthroplasty patients. An implantable load absorber was evaluated for the treatment of medial knee OA in this patient population. Joint overload has been cited as a contributor to OA onset or progression. In response, the KineSpring® System (Moximed, Inc, USA) has been designed to reduce the load acting on the knee. The absorber is implanted in the subcutaneous tissue without violating the joint capsule, thus preserving the option of future arthroplasty. The implant is particularly useful for young, active patients, given the reversibility of the procedure and the preservation of normal flexibility and range of motion. Methods and Results. The KineSpring System was implanted in 55 patients, with the longest duration exceeding two years. The treated group had medial knee OA, included younger OA sufferers (range 31–68 years), with a mean BMI > 30kg/m2. Acute implant success, adverse events, and clinical outcomes using validated patient reported outcomes tools were recorded at baseline, post-op, 2 and 6 weeks, and 3, 6, 12 and 24 months post-op. All patients were successfully implanted with a mean procedure time of 76.4 min (range 54–153 minutes). Mean hospital length of stay was 1.7 days (range 1–3 days), and patients recovered rapidly, achieving full weight bearing within 1–2 wks and normal range of motion by 6 weeks. Most patients experienced pain relief and functional improvement with 85% (35/41) reporting none or mild pain on the WOMAC pain subscale and 90% (37/41) reporting functional impairment as none on mild on the WOMAC function subscale at the latest follow-up visit (mean 9.3 ± 3.5 months). Clinically meaningful and statistically significant pain reduction and functional improvement were noted with baseline WOMAC pain scores (0–100 scale) improving from 42.4 to 16.1 (p<0.001) and WOMAC function (0–100 scale) improving from 42.0 to 14.7 (p<0.001) at latest follow-up. Patients reported satisfaction with the implant and its appearance. Conclusions. The KineSpring System preserves natural knee anatomy and kinematics while providing pain reduction and resumption of high activity levels that have proven durable. This device, with these excellent results, fills a major gap in treatment options for young and active OA patients

Introduction. Revision type arthroplasties for the treatment of knee osteoarthritis is an effective and secure procedure. It has different indications, but the most relevant is the revision of a failured primary arthroplasty. In our study, we reviewed the results of another indication, the implantation of a revision type arthroplasty as a primary procedure in cases of severe deformities. Objectives. To assess the radiological, clinical and functional situation and the quality of life of those patients in whom a revision knee arthroplasty had been implanted in the past years. Materials and Methods. We did a retrospective study of 108 knee arthroplasties (80% women) implanted between 1999 and 2005 with a mean follow up of 7.8 years. The mean age of the patients at the time of surgery was 75 years old (60–87). The most frequent indication for a revision type arthroplasty was an important valgum deformity osteoarthritis. We assess the functional and clinical situation using the Knee Society Score, both clinical and functional; the radiological situation using the Knee Society Roentgenographic evaluation and the quality of life using the Short Form 12 (SF12). We have analyzed the survivorship rate of our arthroplasties as well. Results. We could contact 75 patients. 20 had died in the past years without any new surgeries in the knee; we could not contact 13 patients due to lost of follow up. The survivalship rate of the arthroplasties is 91% at the time of follow up. In the cases of severe valgum or varum malalignment, a phisiological valgum angle (5.47°) was achieved in all of the patients. 77.4% of the patients declared themselves as satisfied or very satisfied in the last follow up. The results of the KSS-Clinical were excellent or good in 87.1% of the patients while in the KSS- Functional was 77.5%. The mean SF12 score was 20.68 out of 60 points. Conclusions. The clinical and functional results are overall good or very good, as well as the quality of life, in those patients in which a revision arthroplasty had been implanted instead of a conventional primary arthroplasty, when it was correctly indicated. So a revision type arthroplasty should be considered for the treatment of knee osteoarthritis in cases of severe bone deformities or severe malalignment

Introduction. Viscosupplementation is used widely to provide symptomatic relief to patients with knee OA. This study aimed to compare the efficacy and safety of the standard (3×2ml) and single (1×6ml) dosing regimens of hylan G-F 20. Methods. Prospective, randomized, blinded (reviewers), comparative independent study. Inclusion criteria was OA knee pain e 60mm on a 100mm VAS; no prior intra articular (IA) injection. Patients were randomised to recieve 1 × 6mL or 3 × 2mL hylan G-F 20. Follow-up at 1, 6, 12, 26 and 52 weeks. Analgesics prohibited for 24 hours prior to follow-up assessments and NSAID_s for 26 wks. All adverse events (AE) were recorded. Primary outcome measure: Target knee pain (VAS) at 26 weeks. Secondary outcome measures included WOMAC, Oxford knee score, SF12. Results. 412 patients were randomized (1 × 6mL N=194, 3 × 2 mL N=218). There were no significant differences in age, gender or grade of OA. Injection and/or treatment-related AE (all mild/moderate) were reported in 10.5% (3 × 2 mL) and 9.1% (1 × 6 mL) of patients Significant pain reduction was observed in both groups at 26 weeks (3 × 2 mL: 50% (mean) decrease, 1 × 6mL: 45%). There were no significant differences between groups for any of the primary or secondary outcome measures at 26 weeks. Discussion and Conclusion. Single dose of 6 mL hylan G-F 20 offer comparable safety and efficacy to 3 × 2 mL at 26 weeks. It provides both the patients and physicians a choice with the potential additional benefits of reduced operational costs

Unicompartmental knee osteoarthritis can be treated with either Total Knee Arthroplasty (TKA) or Unicompartmental Knee Arthroplasty (UKA) and controversy remains as to which treatment is best. UKA has been reported to offer a variety of advantages, however many still see it as a temporary procedure with higher revision rates. We aimed to clarify the role of UKA and evaluate the long-term and revision outcomes. We retrospectively reviewed the pain, function and total Knee Society Score (KSS) for 602 UKA and 602 TKA in age and gender matched patients over ten years. The total pre-operative KSS scores were not significantly different between UKA and TKA (42.67 vs 40.54 P=0.021). KSS (pain) was significantly better in the TKA group (44.39 vs 41.38 P= 0.007) at one year and at five years post-operatively (45.33 vs 43.12 P=0.004). There was no statistically significant difference for KSS (total) in TKA and UKA during the study period. 16.3% of UKA and 20.1% of TKA had a documented complication. 79 UKA (13%) and 36 TKA (6%) required revision surgery. Despite the higher revision rate, pre-operative KSS (total) before revision was not significantly different between UKA and TKA (42.94 vs 42.43 P=0.84). Performance for UKAs was inferior to TKAs in Kaplan-Meier cumulative survival analysis at 10 years (P<0.001). Both UKA and TKA are viable treatment options for unicompartmental knee osteoarthritis, each with their own merits. UKA is associated with fewer complications whereas TKA provides better initial pain relief and is more durable and less likely to require revision

Aims

To investigate health-related quality of life (HRQoL) of older adults (aged ≥ 60 years) after tibial plateau fracture (TPF) compared to preinjury and population matched values, and what aspects of treatment were most important to patients.

Methods

We undertook a retrospective, case-control study of 67 patients at mean 3.5 years (SD 1.3; 1.3 to 6.1) after TPF (47 patients underwent fixation, and 20 nonoperative management). Patients completed EuroQol five-dimension three-level (EQ-5D-3L) questionnaire, Lower Limb Function Scale (LEFS), and Oxford Knee Scores (OKS) for current and recalled prefracture status. Propensity score matching for age, sex, and deprivation in a 1:5 ratio was performed using patient level data from the Health Survey for England to obtain a control group for HRQoL comparison. The primary outcome was the difference in actual (TPF cohort) and expected (matched control) EQ-5D-3L score after TPF.

We aimed to analyse complication rates following medial opening-wedge high tibial osteotomy (OWHTO) for knee OA. A regional retrospective cohort study of all patients who underwent HTO for isolated medial compartment knee OA from 2003–2013. 115 OWHTO were performed. Mean age = 47 (95%CI 46–48). Mean BMI = 29.1 (95%CI 28.1–30.1). Implants used: 72% (n=83) Tomofix, 21% (n=24) Puddu plate, 7% (n=8) Orthofix Grafts used: 30% (n=35) autologous, 35% (n=40) artificial and 35% (n=40) no graft. 25% (n=29) of patients suffered 36 complications. Complications included minor wound infection 9.6%, major wound infection 3.5%, metalwork irritation necessitating plate removal 7%, non-union requiring revision 4.3%, vascular injury 1.7%, compartment syndrome 0.9%, and other minor complications 4%. Apparent higher rates of non-union occurred with the Puddu plate (8.3%) relative to Tomofix (3.6%) but was not statistically significant. No other significant differences existed in complication rates relative to implant type, bone graft used, patient age or BMI. Serious complications following HTO appear rare. The Tomofix has an apparent lower rate of non-union compared to older implants but greater numbers are required to determine significance. There is no significant difference in union rate relative to whether autologous graft, artificial graft or no graft is used

Introduction. In contrast to knee arthroplasty, there is no national register on high tibial osteotomy (HTO) in Sweden. The knowledge of the outcome of HTO as a treatment alternative for knee osteoarthritis (OA) is insufficient. The rate of revision to knee arthroplasty after HTO at 10 years has been reported to vary between 8 and 49%. The aim of the study was to evaluate the outcome of HTO performed in Sweden 1998–2007, expressed by rate of revision to knee arthroplasty. Patienter och Material: 3 196 HTO (69% men) 30 years or older operated on for knee OA in Sweden 1998–2007 were identified through the in- and out-patient care registers from the Swedish National Board of Health and Welfare. Side, left/right, that was operated, diagnosis and indication for surgery were verified through surgical records. Conversions of HTO to knee arthroplasty before 2010 were identified through the Swedish Knee Arthroplasty Register (SKAR). In about 10% of the cases it was unknown what side the HTO had been performed on and thus if the arthroplasty had been on the same knee. In these cases we assumed a worst case scenario of all having been on the same side as the HTO. A 10-year survival analysis was performed using revision to an arthroplasty as the end point. Result. The mean age was 52 years (range 30- 80) with 97% of the patients younger than 65 years. The cumulative revision rate (CRR) at 10 year was 29.4% (95% CI 27.2–32.9) after adjusting for age and gender. The risk of revision increased by increasing age and the risk of revision after adjusting for age was significantly higher in women than men RR 1.30 (95% CI 1.11–4.54). Most of the HTO's were performed by open wedge osteotomy using external fixation. The risk of revision was higher for closed wedge osteotomies when comparing the closed and open wedge methods RR1.29 (95% CI 1.08–1.55). Conclusion. 70% of the high tibial osteotomies performed in Sweden 1998–2007 had not been converted to a knee replacement, using the 10 year cumulative revision rate. If it is considering beneficial to have no implant in the knee, high tibial osteotomy may be considered as an alternative to TKA in the younger and physically active patients

The aim of this study was to determine if patient reported outcome scores for arthroscopic meniscectomy are adversely affected by the degree of knee osteoarthritis or patient body mass index (BMI). All patients who underwent arthroscopic meniscectomy within the NHS in Scotland between 6th February and 29th April 2012 were audited as part of the Scottish Government Musculoskeletal Knee Arthroscopy Audit and were eligible for inclusion within this study. A total of 270 patients returned both their pre-operative and post-operative EuroQol 5Q5D5L descriptive questionnaire and Knee injury and Osteoarthritis Outcomes Scores. Patients were stratified according to BMI, degree of osteoarthritis, history of injury, and duration of knee symptoms. Pre-operative to post-operative EuroQol index scores [0.642±0.253 to 0.735±0.277, median±SD] and Knee injury and Osteoarthrtis Outcome Scores [44.63±18.78 to 62.28±24.94, median±SD] improved across all patients (p<0.0001). This was irrespective of degree of BMI, history of injury, or duration of symptoms. There was no such improvement in patients with moderate to severe osteoarthritis. Those patients with a BMI >35 kg/m2 had lower post-operative scores than the pre-operative scores of those of BMI <30 kg/m2. Arthroscopic meniscectomy is beneficial regardless of patient BMI, duration of symptoms, history of injury, or in the presence of mild arthritis

The surgical treatment of unicompartmental knee osteoarthritis remains controversial. This study aims to compare the medium-term outcomes of age and gender matched patients treated with unicompartmental knee replacement (UKR) and total knee replacement (TKR). We retrospectively reviewed pain, function and total knee society scores (KSS) for every UKR and age and gender matched TKR in NHS Tayside, with up to 10 years prospective data from Tayside Arthroplasty Audit Group. KSS was compared at 1, 3 and 5 years. Medical complications and joint revision were identified. Kaplan-Meier with revision as end-point was used for implants survival analysis. 602 UKRs were implanted between 2001 and 2013. Preoperative KSS for pain and total scores were not significantly different between UKRs and TKRs whereas preoperative function score was significantly better for UKRs. Function scores remained significantly better in UKRs from preoperative until 3 years follow up. Further analysis revealed no statistically significant difference in the change of function scores in both groups over time. There was a trend for TKRs to perform better than UKRs in pain scores. Total KSS for both groups were not significantly different at any point of the 5-year study. Fewer medical complications were reported in the UKR group. Kaplan-Meier analysis showed a survival rate of 93.7% in UKRs and of 97% in TKRs (Log rank p-value = 0.012). The revision rate for UKR was twice as much as TKR. The theoretical advantages of UKR are not borne out by the findings in this study other than immediate postoperative complications

Introduction. Currently, a validate scale of ankle osteoarthritis (OA) is not available and different classifications have been used, making comparisons between studies difficult. In other joints as the hip and knee, the Kellgren-Lawrence (K&L) scale, chosen as reference by the World Health Organizations is widely used to characterize OA. It consists of a physician based assessment of 3 radiological features: osteophyte formation, joint space narrowing and bone end sclerosis described as follows: grade 0: normal joint; grade 1: minute osteophytes of doubtfull significance; grade 2: definite osteophytes; grade 3: moderate diminution of joint space; grade 4: joint space greatly impaired, subchondral sclerosis. Until now, the K&L scale has never been validated in the ankle. Our objective was to assess the usefulness of the K&L scale for the ankle joint, by determining its reliability and by comparing it to functional scores and to computerized minimal joint space width (minJSW) and sclerosis measurements. Additionally we propose an atlas of standardized radiographs for each of the K&L grades in the ankle. Methods. 73 patients 10 to 20 years post ankle ORIF were examined. Bilateral ankle radiographs were taken. Four physicians independently assessed the K&L grades and evaluated tibial and talar sclerosis on anteroposterior radiographs. Functional outcome was assessed with the AOFAS Hindfoot score. Bone density and minJSW were measured using a previously validated Ankle Image Digital Analysis software (AIDA). Results. The interobserver reliability, for the K&L stages was 0.60 (intraclass correlation coefficient) indicating moderate to good agreement. The mean AOFAS hindfoot score decreased substantially (p = 009) and linearly from 99.3 in K&L grade 0 to 79.5 points in K&L grade 4. The minJSW assessed by AIDA was similar among grades 0 to 2 (between 2 and 2.5mm), but significantly lower in grade 3 (1.8mm) and in grade 4 (1.1mm). A decreased minJSW less than 2mm, commonly used as a threshold for the assessment of hip and knee OA, was found in 77% of K&L grades 3–4 compared to 33% of grades 0–2, sensitivity 77.4% and specificity 66.7%. Physician based assessment revealed that subchondral sclerosis was present in 16% of K&L grade 1 patients, 52% of grade 2, 70% of grade 3 and 100% of grade 4 patients. No correlation could be found between physician based assessment and digital image analysis of subchondral sclerosis. Conclusions. Interobserver reliability in assessment of ankle OA using the K&L scale was similar to other previously described joints. OA progression correlated with functional diminution. Joint space narrowing assessed AIDA as well as the cut-off of 2mm correlated well with the K&L scale. Overall, we recommend the use of the K&L scale for the radiographic assessment of ankle OA

Introduction. Osteoarthritis (OA) of the hip and knee causes pain and functional limitations which lead to decreased physical activity level (1,2). Half of all male, and two thirds of all female, patients with early knee OA do not fulfill the recommendations for physical activity level (30 min/day of at least 5 days/week) (1). We tested the hypothesis that Scandinavian patients prior to total joint replacement had reduced actual physical activity and intensity levels compared with matched controls and that most patients did not fulfill the recommendations for physical activity. Methods. Physical activity and intensity were measured during 5 consecutive days with SenseWearTM Armband (SWA; BodyMedia, Inc., Pittsburgh, PA) in 53 patients with severe knee (n = 26) or hip (n = 27) OA prior to total joint replacement (51% women, age: 68 ± 5 years, BMI: 29.8 ± 10.2) and compared with data from 15 age-matched population-based controls (53% women, age: 68 ± 5 years, BMI: 26.9 ± 3.3). Sedentary, moderate, vigorous and very vigorous intensities were equivalent to Sense Wear Armband recordings of 3 MET, >36 MET, >69 MET, >9MET, respectively. In addition, total energy expenditure, average MET, numbers of steps/day, and the combined activity of moderate, vigorous, and very vigorous activity were calculated (min/day). Unpaired students t-test was used for between group comparisons (p < 0.05). Results. Participants wore the device for a mean ± SD of 98.5 ± 1.5% of the time. Patients spent significant less time being vigorously physically active (0.093 ± 0.082 vs. 0.156 ± 0.099 hours/day) and very vigorously active (0.015 ± 02 vs.0.028 ± 0.035 hours/day) than controls. However, no differences were observed for total energy expenditure, average MET, time spend being sedentary, and time spend being moderately physically active. The average activity of combined moderate, vigorous, and very vigorous activity for patients were 185 ± 1 min/day and 11.3% did not reach the recommended activity level. Finally, a tendency (p = 0.059) towards less steps/day was observed for patients (6778 ± 3196 vs. 8576 ± 2872 steps/day). Conclusions. Since Scandinavian OA patients prior to joint replacement show no or little decrease in physical activity, and that the majority fulfills recommendations for physical activity an increase in actual physical activity cannot be expected from surgery. This unexpected finding needs to be confirmed
