Summary Statement. The purpose is to evaluate the effects of internet usage on new patient referral patterns to identify optimal patient recruitment and communication. Overall, social networking and internet may be an effective way for surgeons to recruit a wider patient population. Introduction. Prior studies in other medical specialties have shown that social networking and internet usage has become an increasingly important means of patient communication and referral. However, this information is lacking in the orthopaedics literature. In this study, we evaluate the means by which new patients arrive at orthopaedic clinics in a major academic center. The purpose is to evaluate the effects of internet or social media usage on new patient referral patterns to identify avenues to optimise patient recruitment and communication. Patients and Methods. New patients were recruited in a major academic orthopaedic clinic to complete a 15-item questionnaire with demographic information, social media use/networking and referral method. Data was collected for all orthopaedic sub-specialties and analyzed accordingly. Statistical analysis was performed. Results. Of the 752 responses, there were 66% female and 34% male responses. Responses were obtained from hand (142), sports medicine (303), foot and ankle (129), joints/tumor (95) and trauma (83) services. Overall, 51% report using social networking sites such as Facebook or Twitter. Of the patients that report not using social network sites, 92% are over the age of 40. Joints/tumor patients most commonly had seen another orthopaedic surgeon prior to their visit (59%) and had prior surgery (42%). Most patients traveled under 60 miles and were referred by their primary care physicians. Between 18–26% of all patients used a physician review website before consultation. The majority of the patients prefer communicating with their physician via the phone(68%) compared to email(32%). Independent associations found that sports medicine patients tend to be higher social networking users (35.9%) relative to other services (9.8–17.9%) and was statistically higher when compared to the joints/tumor service (P<.0001). The multivariate logistic regression model showed that the sports service was generally more likely to have social networking users with the exception of the foot/ankle service), however these differences were not statistically significant. The biggest indicator predicting social media usage in the orthopaedic population was age. The older the patient population, the less likely patients will use social networking sites. Non-doctorate patients were more likely to be social media users compared to doctorate level individuals, but was not statistically significant. Patients that lived from 120 to 180 miles from the hospital were significantly more likely to be social media users, as were patients that did research on their condition prior to their new patient appointment. Discussion and Conclusion. Orthopaedic patients who use social media are more likely to be younger, research their condition prior to their appointment and undergo an average day's travel (120–180 miles) to see a physician. Up to 26% of all patients have seen or used a physician review site prior to their visit. Despite the increased social media usage, most orthopaedic patients still prefer telephone communication with their physicians. Overall, social networking and internet may be an effective way for surgeons to recruit a wider patient population. In an increasingly competitive market, surgeons with younger patient populations (Sports Medicine) will need to utilise social networking and the internet to capture new patient referrals

Congenital talipes equinovarus (CTEV), also known as club foot or talipes is a common congenital disorder. Parents are using the Internet more and more as a source of information about health care. Unfortunately, the quality of health care information on the Internet varies. This study looked at information available to parents using two instruments for judging the equality of information on the internet. The top five search engines were searched on Google. Three of these were also included in the top 50 sites in Ireland so these 3 sites were used. The phrases CTEV and club foot were searched from all 3 platforms. Websites were then scrutinized using the HON code and the DISCERN tool. 54 organic sites were found for the 3 search engines using the key word club foot. For the key word CTEV 55 matches were returned for the three search engines. 4 websites displayed the HON code. Using the discern tool CTEV websites had a mean score of 60 with a standard deviation of 17. While club foot had a mean score of 56.8 with a standard deviation of 13. Max score 80. Large volumes of information are available to parents on the Internet. Often parents find comfort in sharing experiences and feel empowered by learning about their children's illnesses. However, information provided on the interned can also be ambiguous and disingenuous. Practitioners should be aware of a number of key websites that parents can be directed towards

Background. Patient reported outcomes measures are a fundamental part of the NHS. Since 2009, they have been used to measure quality from the patient's perspective. PROMS2.0 is a semi-automated web based system, which allows collection and analysis of outcome data. This study looks at the factors, which can influence PROMS. These include looking at general trends which affect reported outcomes such as surgeon, age and gender. We also look to assess the reasons for non-uptake in the study. Methods. Data was collected from October 2012 to March 2015. Scores used to asses outcome measures included EQ-5D VAS, EQ-5D Health Index, and MOxFQ, collected pre-operatively and post-operatively. Results. 97/350 (27.8%) (69F+28M) patients consented and provided pre-op and post op scores. Average age was 57.2 years (Range-19–89). 69 Rt Vs 36 Lt. Surgeon A-51, B-31, C-8 procedures. MOxFQ- all three domains improved on average- Pain- 51.2 to 28.2. Walking/standing- 53.5 to 30.4 and Social interaction- 46.1 to 27.3. EQ5D scores also improved. VAS- 76.0 to 79.7, Index- 0.70 to 0.74. Patients over 70 showed the greatest improvement in MOxFQ, over 70s and under 50s did the same in EQ5D. Male patients reported better outcomes overall in both scores. All statistically significant. No significant patterns found between different surgeons. Left sided procedures did slightly better than right. Non-participation reasons- 20/30 lack of time, 7/30- reduced internet access, 3/30- non specified. Conclusions. Our results suggest improvement in PROMS. Better outcomes are linked to increased age, male sex, left sided procedures but not individual surgeons. Non-responding patients placed forgetfulness/time and lack of Internet access as reasons for non-participation Simplification and integration of PROMS will be key to increased participation. Driven by increasing emphasis on problem-based and self-directed learning, medical students and doctors in orthopedic specialty training rely increasingly on the internet as learning resource. As students or residents performance on physical examination may be less supervised in comparison to other clinical skills (for example surgical competence), online videos may provide a valuable source for education of physical examination skills. Cognitive psychological research has shown that videos can help viewers to understand techniques and manage the sequential steps of physical examination and approach to patients. YouTube is the largest open-access video platform available and provides access to thousands of educational videos on orthopedics-related topics. VuMedi, G9MD, and Orthobullets are examples of online platforms requiring user-registration with video content that is more directly focused on orthopedic topics. The objective of this study was to investigate the accuracy and quality of instructional videos on the physical examination of the elbow and identify factors influencing the educational usefulness. A YouTube, VuMedi, Orthobullets, and G9MD search was performed on October 7, 2018 for videos on the physical examination of the elbow. We included both basic examination and disease specific tests. The included videos were rated for accuracy and quality by two independent authors using a modified version of a validated scoring system. Inter-rater reliability was analyzed using mean difference and intra-class correlation coefficient. Twenty-three out of 126 videos were indicated as useful for educational purposes. Accuracy, quality and total scores were statistically significant higher for videos from specialized platforms compared to YouTube: 16.5 (95% CI 16 to 17) vs. 12.816 (95% CI 12.3 to 13.3) respectively. Video accuracy and quality were highly variable and did not correlate. The number of days online, views, and likes showed no or weak correlation with accuracy and quality. For the total score, our assessment tool showed excellent inter-rater reliability of 0.93 (95% CI 0.09–0.95) and a mean difference of 0.024 point between the two observers (p=0.871). There is considerable variation in accuracy and quality of online available videos on the physical examination of the elbow. We indicated 23 educationally useful videos and provided an assessment method. This assessment method can be useful for both viewers to assess reliability of a video and educators interested in creating videos

Patients living with type 1 diabetes mellitus (T1DM) can develop early onset osteoporosis and are exposed to an increased risk of fracture. Skeletal health can be influenced easily with diet and exercise. However, diabetes mellitus (DM)-related osteopathy is not emphasized in the public information campaigns on the American Diabetes Association, Diabetes UK, Diabetes Ireland or International Diabetes Federation websites. This investigation aims to assess the perceptions of patients regarding living with T1DM and their baseline knowledge on DM-related osteopathy. A survey was administered to 102 consenting individuals living with T1DM in attendance at the Galway University Hospital Diabetes Centre. Of the respondents, 44% were female and 56% male (mean age of 43). Respondents had T1DM for a mean of 21 years. Participants were asked to identify DM-related complications, including bone thinning and bone fractures. Respondents were primarily concerned about developing DM-related blindness, kidney damage and amputations, but not osteopathy. 49% of respondents did not identify osteopathy as a potential DM-related complication, 28% of respondents related DM with bone thinning and bone fractures, and 22% individuals only identified bone thinning or bone fractures. When asked for their primary source of DM-related information, endocrinologists and internet where identified. When comparable questions were asked of DM-related healthcare professionals, 56% did not recognize osteopathy as a complication of T1DM. This study demonstrated a low-level awareness of the impact living with T1DM has on bone health. The deployment of patient-interactive activities or educational modules may enhance the future health of individuals living with T1DM

Background The internet is an increasingly utilised resource for accessing information regarding a variety of heath conditions. YouTube is a popular video sharing platform used to both seek and distribute information online. Materials & Methods. A search for ‘scoliosis’ was carried out using YouTube's search engine and data was collected on the first fifty videos returned. A JAMA score (to determine currency, authorship, source and disclosure) and scoliosis specific score (that measures the amount of information on the diagnosis and treatment options as devised by Mathur et al in 2005; scored 0–32) was recorded for each video to measure quality objectively. Additionally the number of views, number of comments and feedback positivity was documented for each. Data analysis was conducted using R 3.1.4/R Studio 0.98 with control for the age of each video in analysis models. Results. The average number of views per video was 71,152 with an average length of 7 minutes 32 seconds. Thirty six percent of the videos fell under the authorship category of personal experience. The average JAMA score was 1.32/4 and average scoliosis specific score was 5.38/32. There was a positive correlation between JAMA score and number of views P=0.003. However in contrast there was a negative correlation between scoliosis specific score and number of views P=0.01. Conclusions. Online health information has historically been poor and our study shows that in an environment like YouTube which lacks a peer review process, the quality of scoliosis information is low. Further work is needed to determine whether accessing information on YouTube can play a role in patient care other than simple education pertaining to the disease and its management. Objectives

The patient-rated wrist evaluation (PRWE) and the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire are patient-reported outcome measures (PROMs) used for clinical and research purposes. Methodological high-quality clinimetric studies that determine the measurement properties of these PROMs when used in patients with a distal radial fracture are lacking. This study aimed to validate the PRWE and DASH in Dutch patients with a displaced distal radial fracture (DRF).

Methods

The intraclass correlation coefficient (ICC) was used for test-retest reliability, between PROMs completed twice with a two-week interval at six to eight months after DRF. Internal consistency was determined using Cronbach’s α for the dimensions found in the factor analysis. The measurement error was expressed by the smallest detectable change (SDC). A semi-structured interview was conducted between eight and 12 weeks after DRF to assess the content validity.

Objectives

Electronic forms of data collection have gained interest in recent years. In orthopaedics, little is known about patient preference regarding pen-and-paper or electronic questionnaires. We aimed to determine whether patients undergoing total hip (THR) or total knee replacement (TKR) prefer pen-and-paper or electronic questionnaires and to identify variables that predict preference for electronic questionnaires.

Methods

We asked patients who participated in a multi-centre cohort study investigating improvement in health-related quality of life (HRQoL) after THR and TKR using pen-and-paper questionnaires, which mode of questionnaire they preferred. Patient age, gender, highest completed level of schooling, body mass index (BMI), comorbidities, indication for joint replacement and pre-operative HRQoL were compared between the groups preferring different modes of questionnaire. We then performed logistic regression analyses to investigate which variables independently predicted preference of electronic questionnaires.

Objectives

Despite the fact that research fraud and misconduct are under scrutiny in the field of orthopaedic research, little systematic work has been done to uncover and characterise the underlying reasons for academic retractions in this field. The purpose of this study was to determine the rate of retractions and identify the reasons for retracted publications in the orthopaedic literature.

Methods

Two reviewers independently searched MEDLINE, EMBASE, and the Cochrane Library (1995 to current) using MeSH keyword headings and the ‘retracted’ filter. We also searched an independent website that reports and archives retracted scientific publications (www.retractionwatch.com). Two reviewers independently extracted data including reason for retraction, study type, journal impact factor, and country of origin.
