Summary. A rotational limit for screw insertion may improve screw purchase and plate compression by reducing stripping, as compared to a torque based limit. Introduction. Over-tightening screws results in inadvertent stripping of 20% of cortical bone screws. The current method of “two-fingers tight” to insert screws relies on the surgeon receiving torque feedback. Torque, however, can be affected by screw pitch, bone density and bone-thread friction. An alternative method of tightening screws is the “turn-of-the-nut” model, commonly used in engineering applications. In the “turn-of-the-nut” method, nuts used to fasten a joint are rotated a specific amount in order to achieve a pre-specified bolt tension. When applied to orthopaedics, bone assumes the role of the nut and the screw is the bolt. The screw is turned a set angular rotation that is independent of torque feedback. Potentially the “turn-of-the-nut” method provides an easier way of screw insertion that might lessen inadvertent screw stripping. The purpose of the current study was to use the “turn-of-the-nut” method to determine the angular rotation that results in peak plate compression and peak screw pullout force. Methods. Three pairs of human humeri in each of three groups (osteopenic, osteoporotic, and normal) underwent plate compression and pullout protocols. For plate compression, 3.5-mm screws were tightened into strain gauge instrumented plate until screw stripping occurred. Insertion torque, plate compression, and screw rotation were measured. For pullout, 3.5-mm screws were inserted until the head contacted the plate, additionally rotated (90, 180, 270, or 360 degrees), and then pulled out. A generalised linear and latent mixed model was used to check for significant associations (P < 0.05). Results. Mean (95% CI) peak plate compression occurred at 286 degrees (range, 261 – 311 degrees) beyond screw seating. Plate compression significantly increased at 90 to 135 degrees but not after 180 degrees. At 270 degrees, 39% of the screws had already reached their peak ability to compress. Peak screw torque lagged behind peak plate compression by 31 ± 50 degrees, and in seeking peak screw torque, a loss of 104 ± 115 N in plate compression resulted. Screw pullout force was greatest at 90 degrees, but it was not significantly different from that of the other angle groups. Conclusions. Screw rotation at 180 degrees provides plate compression and pullout strength statistically similar to those at greater rotations but without the loss of purchase associated with greater rotations

Background. Aseptic loosening of cemented femoral stems results from migration of wear particles along the bone-cement interface, producing a foreign body reaction. After cement insertion, blood back pressure can disrupt the bone-cement interface, enabling this spread of wear particles. Our study investigates whether altering timing and speed of stem insertion can reduce this risk. Methods. We inserted mock “C-Stem” femoral components (De Puy-Synthes), using Smartset HV cement (De Puy-Synthes) into artificial femora, fitted with proximal and distal pressure transducers. Cement insertion began two, three or four minutes after mixing. Cement pressures were then allowed to settle for one minute and the stems were then inserted over durations of 25, 60 or 90 seconds. Results. Quicker insertion led to high peak pressures; however insertion over 90 seconds at 4 and 5 minutes achieved more sustained pressures above blood back pressure. Slower insertion particularly improved proximal pressurisation. Furthermore when the stem was inserted over 90 seconds at 5 minutes then the cement pressures remained high after full insertion of the stem. Conclusions. Commonly, femoral stems are inserted rapidly. Our study demonstrates that by inserting the femoral stem more slowly, cement pressure can be maintained above blood back pressure for longer. We believe that after slower stem insertion, the higher cement viscosity enables the surgeon to maintain cement pressure, via the stem, sufficient to resist blood back pressure without inadvertently over inserting the stem. This safe and simple modification of cementing technique generates higher and more sustained cement pressures. Level of evidence. 3. Disclosures. Study purpose and background. There is growing concern that current outcome measures of back pain trials do not comprehensively capture what is important to patients. Some researchers believe we need to incorporate patients in the development of ‘next-generation’ outcomes. As a preliminary step to patient-interviews, we aimed to articulate ‘important change’ from the patients' perspective, as represented in reports of qualitative investigations inadvertently or directly exploring this. Summary of methods used and results. We adopted a multi-strand search of electronic databases, and citation and reference tracking. Two researchers identified qualitative investigations relating to low back pain. Data were abstracted and synthesised using meta-ethnographic processes. Provisional results, based on 41 studies, indicate few studies have directly addressed this issue, but that data regarding experience and expectations may be useful. Whilst results suggest that practically, patients are concerned with (re-)engagement in meaningful activities, the more experientially focused literature suggests that patients want to be believed and have validated their experiences and identity as someone ‘doing battle’ with pain. Patients seek not only diagnoses, treatment and cure, but simultaneously reassurance of the absence of pathology. In the absence of tenable diagnoses, some feel they must not adopt a ‘sick role’. Some struggle, but manage to meet others' expectations; thereby undermining the credibility of their pain/disability claims. Others withdraw, fearful of disapprobation and unable or unwilling to accommodate social demands. Patients generally seek to regain their pre-pain healthy, and emotionally robust state. Conclusion. Measurement of treatment effect must move beyond individual function to encompass the multidimensional impact of pain on identity and social participation. Conflicts of Interest. None. Introduction. Hand tumors are usually rare and there is not much literature about series of cases. We have studied a series of 110 cases. Hand tumors do consists of both benign and malignant cases. Methods. We studied series of 110 cases at Karnataka Institute of Medical Sciences, Hubli and Mysore Medical College & Research Institute, Mysore. We retrospectively reviewed the records of 110 patients who underwent double ray amputations at our center over few years: few had amputations of the fourth and fifth rays and others amputation of the second and third rays. Mean age at surgery was 34 years (range, 10–45 years), and minimum follow up was 64 months (mean, 98 months; range, 64–136 months). Some patients had high-grade soft tissue sarcomas of the hand, synovial sarcomas, malignant peripheral nerve sheath tumors, and undifferentiated sarcoma. No patients had detectable metastases at surgery. Results. All patients were completely disease-free at latest follow up. One patient was alive with lung metastases detected 32 months after surgery. No patients developed local tumor recurrence. Functional assessment showed a mean Musculoskeletal Tumor Society score of 24 (range, 19–28) and mean grip strength 24% of the contra lateral side (range, 17%–35%). Conclusions. The majority of osseous tumors of the hand are benign. The surgeon who evaluates and treats osseous tumors of the hand has to be familiar with limb anatomy, tumor biology, various presentations of the tumors and the range of treatment possibilities and their limitations. Lesions in the hand more often present earlier in their course than those at other sites, just because they are more likely to superficial and easily noticed. Ganglion cyst is the most frequently encountered comprising 50–70% of benign tumors of hand. Enchondroma was the next common benign bone tumour followed by osteoid osteoma, osteoblastoma, aneurismal bone cyst, giant cell tumor, epidermoid cyst, and osteochondroma. Although malignant neoplasms in the hand that arise from tissues other than the skin are very rare, the hand may be the site of distant breast, lung, kidney, esophagus, or colon adenocarcinoma metastases, most of which have a predilection for the distal phalanges. Malignant tumours of the hand are rare, although there remain many instances in which marginal excisions are performed for unsuspected malignant hand lesions. Suboptimal biopsy incisions and inadvertent contamination during these excisions may result in larger resections or amputations being necessary to ensure complete removal of the tumour with negative margins

The aim of this study was to validate the use of three models of fracture fixation in the assessment of technical skills. We recruited 21 subjects (six experts, seven intermediates, and eight novices) to perform three procedures: application of a dynamic compression plate on a cadaver porcine model, insertion of an unreamed tibial intramedullary nail, and application of a forearm external fixator, both on synthetic bone models. The primary outcome measures were the Objective Structural Assessment of technical skills global rating scale on video recordings of the procedures which were scored by two independent expert observers, and the hand movements of the surgeons which were analysed using the Imperial College Surgical Assessment Device.

The video scores were significantly different for the three groups in all three procedures (p < 0.05), with excellent inter-rater reliability (α = 0.88). The novice and intermediate groups specifically were significantly different in their performance with dynamic compression plate and intramedullary nails (p < 0.05). Movement analysis distinguished between the three groups in the dynamic compression plate model, but a ceiling effect was demonstrated in the intramedullary nail and external fixator procedures, where intermediates and experts performed to comparable standards (p > 0.6). A total of 85% (18 of 21) of the subjects found the dynamic compression model and 57% (12 of 21) found all the models acceptable tools of assessment.

This study has validated a low-cost, high-fidelity porcine dynamic compression plate model using video rating scores for skills assessment and movement analysis. It has also demonstrated that Synbone models for the application of and intramedullary nail and an external fixator are less sensitive and should be improved for further assessment of surgical skills in trauma. The availability of valid objective tools of assessment of surgical skills allows further studies into improving methods of training.

Anatomical descriptions of the lateral retinaculum have been published, but the attachments, name or even existence of its tissue bands and layers are ill-defined. We have examined 35 specimens of the knee. The deep fascia is the most superficial layer and the joint capsule is the deepest. The intermediate layer is the most substantial and consists of derivatives of the iliotibial band and the quadriceps aponeurosis. The longitudinal fibres of the iliotibial band merge with those of the quadriceps aponeurosis adjacent to the patella. These longitudinal fibres are reinforced by superficial arciform fibres and on the deep aspect by transverse fibres of the iliotibial band. The latter are dense and provide attachment of the iliotibial band to the patella and the tendon of vastus lateralis obliquus.

Our study identifies two important new findings which are a constant connection of the deep fascia to the quadriceps tendon superior and lateral to the patella, and, a connection of the deeper transverse fibres to the tendon of vastus lateralis obliquus.
