Background. Peri-prosthetic humerus fractures are relatively uncommon occurrences that can be difficult to manage non-operatively. Locking plate technology has enhanced the surgical management of these fractures. We describe an osteosynthesis technique utilizing a locking plate with eccentrically placed screw holes to place “skive screws” in the proximal end of the plate to achieve fixation around the stem of the implant. Methods. A retrospective review of prospectively collected data was performed for a consecutive series of patients treated with this skive screw technique from May 2011 to September 2014. Seven patients presented with postoperative type B peri-prosthetic humerus fractures. Average follow-up was 24 months. Radiographic analysis was performed on most recent postoperative imaging. Clinical outcomes were assessed using VAS pain, ASES total score, ASES functional score, SST, SANE, range of motion and strength. Results. At an average of follow-up of 24 months, all patients demonstrated fracture healing. Functional outcomes were limited with only two patients achieving forward elevation above 90 degrees and average ASES Function score was 27.5. The choice of whether to perform antegrade intramedullary nailing (IMN) or plate fixation (PF) poses a conundrum for the surgeon who must strike the balance between anatomical restoration while reducing elbow and shoulder functional impairment. Most humeral middle third shaft fractures are amenable to conservative management given the considerable acceptable deformity and anatomical compensation by patients. This study is concerned with the patient reported outcomes regarding shoulder and elbow function for IMN and PF respectively. A prospective cohort study following up all the cases treated surgically for middle third humeral fractures from 2016 to 2022 at a single centre. Telephonically an analogue pain score, an American Shoulder and Elbow Society (ASES) score for shoulder function and the Oxford Elbow score (OES) for elbow function were obtained. One hundred and three patients met the inclusion criteria. Twenty four patients participated in the study, fifteen had IMN (62.5%) and nine had PF (37.5%.). The shoulder function outcomes showed no statistical difference with an average ASES score of sixty-six for the IMN group and sixty-nine for the PF group. Women and employed individuals expressed greater functional impairment. Hand dominance has no impact on the scores of elbow and shoulder function post operatively. The impairment of abduction score post antegrade nailing was higher in the antegrade nailing group than the plated group. The OES demonstrated greater variance in elbow function in the PF group with the IMN group expressing greater elbow disfunction. This study confirms that treatment of middle third humerus shaft fractures by plate fixation is marginally superior to antegrade intramedullary nailing in preserving elbow function and abduction ability.
Fractures of the shaft of the humerus are often treated conservatively in a hanging cast or a humeral brace. The conservative management of this fracture is often prolonged and quite uncomfortable for the patient. Some of the patients will need an operative fixation after a trial of conservative management. We retrospectively looked at 72 consecutive patients with fractures of the shaft of the humerus that presented in our institution over a period of two years. The fracture pattern, treatment modality time to union and the number that needed operative fixation following a trial of conservative treatment was analysed. Of the 72 patients 4 were lost to follow-up. 45 patients had a 1.2.B or 1.2.C type of fracture and 23 had a 1.2.A type of fracture. 29 (41%) were successfully treated conservatively, 11 (16%) patients were operated as the primary procedure and 15 (22%) patients were operated due to delayed or non union. 13 (19%) patients were operated within 4 weeks of the fracture as their alignment was not acceptable on their weekly follow-up. The average time to union in the patients treated conservatively was 22 weeks, while that of the patients treated primarily by open reduction and plating was 14 weeks (p-value<0.05). Patients who needed operation after initial conservative management required prolonged period of rehabilitation and union time was 32.2 weeks. At the time of fracture union 72% of the patients who had been treated conservatively had joint stiffness requiring physiotherapy, while only 18% of those who had an open reduction and internal fixation had stiffness and required physiotherapy. (p-value < 0.05). In conclusion careful consideration should be given before it is decided to treat this fracture conservatively especially in the case of 1.2.A fracture pattern.
Peri-prosthetic fractures around implants in the proximal humerus can present substantial challenges. Most individuals who undergo upper limb arthroplasty tend to be osteopenic to begin with, and the anatomy of the proximal humerus does not provide an excess of bone to work with. Therefore, peri-prosthetic fractures pose difficulties to rotator cuff function and implant stability. There are multiple classification systems, but series are small and the classification does not always lead to treatment algorithms. Risk factors for humeral fractures after shoulder arthroplasty include endosteal notching, cortical perforation, varus malalignment, stem perforation, ipsilateral shoulder and elbow arthroplasties, and loose stems. Many of these risk factors are directly related to technical errors at the time of surgery. Poor exposure can lead to aberrant starting point and errors in reaming. Oversized prostheses can lead to cortical perforation or even stem perforation. Proper positioning of the patient on the table and surgical releases help avoid these technical errors. Peri-prosthetic fractures should be carefully evaluated radiographically for stability. Two important considerations: 1. Is the implant stable? 2. Is the fracture stable? Generally, if the implant is unstable, the implant must be revised. In the setting of a stable implant, many humeral fractures can be treated nonoperatively. Many fractures at or below the level of the tip of the implant can be treated as typical humeral fractures. Options for fixation include plates with cables or long stem prostheses which bypass the fracture. Displaced tuberosity fractures are treated with suture or wire fixation. Risk factors for a poor outcome include increased time to union, skin breakdown, and stiff shoulder.
The purpose of this study is to describe the use of the PHILOS plate (Synthes) in reverse configuration to treat complex distal humeral non-unions. Non-union is a frequent complication of distal humeral fracture. It is a challenging problem due to the complex anatomy of the distal humerus, small distal fragment heavily loaded by the forearm acting as a long lever arm with powerful forces increasing the chances of displacement. Rigid fixation and stability with a device of high “pull-out” strength is required. The PHILOS plate has been used in reverse configuration to achieve good fixation while allowing central posterior placement of the implant. 11 patients with established non-union of distal humeral fractures were included in this study. No patient in whom this implant was used has been excluded. Initial fixation was revised using the PHILOS plate in reverse configuration and good fixation was achieved. Bone graft substitutes were used in all cases. Patients were followed to bony union, and functional recovery. All fractures united. One required revision of plate due to fatigue failure. Average time to union was 8 months with excellent restoration of elbow function. A reversed PHILOS plate provides an excellent method of fixation in distal humeral non-union, often complicated by distorted anatomy and previous surgical intervention. It has a high “pull-out” strength and may be placed in the centre of the posterior humerus, allowing proximal extension of the fixation as far as is required. It provides secure distal fixation without impinging on the olecranon fossa. It is more versatile and easier to use than available pre contoured plating systems.
The use of shorter humeral stems in reverse shoulder arthroplasty has been reported as safe and effective. Shorter stems are purported to be bone preserving, easy to revise, and have reduced surgical time. However, a frequent radiographic finding with the use of uncemented short stems is stress shielding. Smaller stem diameters reduce stress shielding, however, carry the risk of varus or valgus malalignment in the metadiaphyseal region of the proximal humerus. The aim of this retrospective radiographic study was to measure the true post-operative neck-shaft (N-S) angle of a curved short stem with a recommended implantation angle of 145°. True anteroposterior radiographs of patients who received RTSA using an Ascend Flex short stem at three specialized shoulder centres (London, ON, Canada, Lyon, France, Munich, Germany) were reviewed. Radiographs that showed the uncemented stem and humeral tray in orthogonal view without rotation were included. Sixteen patients with proximal humeral fractures or revision surgeries were excluded. This yielded a cohort of 124 implant cases for analysis (122 patients, 42 male, 80 female) at a mean age of 74 years (range, 48 – 91 years). The indications for RTSA were rotator cuff deficient shoulders (cuff tear arthropathy, massive cuff tears, osteoarthritis with cuff insufficiency) in 78 patients (63%), primary osteoarthritis in 41 (33%), and rheumatoid arthritis in 5 (4%). The patterns of nerve and associated skeletal injury were reviewed in 84 patients referred to the brachial plexus service who had damage predominantly to the infraclavicular brachial plexus and its branches. Patients fell into four categories: 1. Anterior glenohumeral dislocation (46 cases); 2. ‘Occult’ shoulder dislocation or scapular fracture (17 cases); 3. Humeral neck fracture (11 cases); 4. Arm hyperextension (9 cases). The axillary (38/46) and ulnar (36/46) nerves were most commonly injured as a result of glenohumeral dislocation. The axillary nerve was ruptured in only 2 patients who had suffered high energy trauma. Ulnar nerve recovery was often incomplete. ‘Occult’ dislocation refers to patients who had no recorded shoulder dislocation but the history was suggestive that dislocation had occurred with spontaneous reduction. These patients and those with scapular fractures had a similar pattern of nerve involvement to those with known dislocation, but the axillary nerve was ruptured in 11 of 17 cases. Removal of a well-fixed humeral component during revision shoulder arthroplasty presents a challenging problem. If the humeral component cannot be extracted simply from above, an alternate approach must be taken that may include compromising bone architecture to remove the implant. Two potential solutions to this problem that allow removal of the well-fixed prosthesis are making a humeral window or creating a longitudinal split in the humerus. A retrospective review was performed at the Mayo Clinic to determine the complications associated with performing humeral windows and longitudinal splits during the course of revision shoulder arthroplasty. This study included 427 patients from 1994–2010 at Mayo Clinic undergoing revision shoulder arthroplasty. From this cohort, those who required a humeral window or a longitudinal split to assist removal of a well-fixed humeral component were identified. Introduction. Proximal humeral fracture occurs most frequently in elderly and usually unite within 12 weeks. Malunion is more common than nonunion. And even if nonunion occurs, we usually treat it by the blade plate. Norris et al. reported hemiarthroplasty is indicated for severe degenerative changes of the articular surface, osteonecrosis of the humeral head, or osteopenia severe enough to jeopardize fixation in order patients. Purpose. Locking plate constructs for proximal humerus fractures can fail due to varus collapse, especially in the presence of osteoporosis and comminution of the medial cortex. Augmentation using a fibular allograft as an intramedullary bone peg may strengthen fixation preventing varus collapse. Purpose. The presence of a Hill-Sachs lesion is a major contributor to failure of surgical intervention following anterior shoulder dislocation. The relationship between lesion size, measured on pre-operative MRI, and risk of recurrent instability after surgery has not previously been defined. Hypothesis: We hypothesized that the size of Hill-Sachs lesions on pre-op MRI would be greater among patients who failed soft tissue stabilization when compared to patients who did not fail. We also hypothesized that the existence of a glenoid lesion would lead to failure with smaller Hill-Sachs lesions. Method. Nested case-control analysis of 114 patients was performed to evaluate incidence of failure after soft tissue stabilization. Purpose. In total elbow arthroplasty (TEA), especially for elbows with condyle defect due to rheumatoid arthritis or trauma, determination of rotation alignment of implants is often difficult. To develop a navigation system for TEA, selecting bony landmarks that can be identified intraoperatively is important. Therefore, we developed a new roentgen free navigation system such as special alignment jigs for TEA based on CT data of normal elbows. The aim of this study was to evaluate alignments of implants after MIS-TEA using the new systems. And also, we reported that 6 bony landmarks on the elbow showed small variability in normal elbows by CT examinations and were considered to be usable as intraoperative landmarks for determining rotational position of implants last year.
