Background. Non-invasive hemoglobin measurement was introduced to potentially eliminate blood draws postoperatively. We compared the accuracy and effectiveness of a non-invasive hemoglobin measurement system with a traditional blood draw in patients undergoing total joint arthroplasty. Methods. After IRB approval, 100 consecutive patients undergoing primary total hip or knee arthroplasty had their hemoglobin level tested by both traditional blood draw and a non-invasive hemoglobin monitoring system. Results were analyzed for the entire group, further stratifying patients based on gender, race, surgery (THA versus TKA), and post-operative hemoglobin level. Finally, we compared financial implications and patient satisfaction with the device. Paired t-test with 0.05 conferring significance was used. Stratified analyses of the absolute difference between the two measures were assessed using Mann- Whitney test. To assess the level of agreement between the two measures, the concordance correlation coefficient (CCC) was calculated. Results. Mean blood-draw hemoglobin value on POD1 was 11.063 ± 1.39 g/dL and 11.192 ± 1.333 g/dL with the non-invasive device. For all patients, the mean absolute difference between the two methods was 0.13 g/dL (p = 0.30). The CCC between the two methods was 0.58, conferring a moderate to strongly positive linear relationship (Figure 1). Non-invasive measurement was preferred by 100% of patients with a mean VAS score of 0/10. Additionally, the cost savings with the non-invasive system was $16.50 per patient. Discussion. Overall, there was no significant difference between the hemoglobin level obtained by traditional laboratory methods versus the Masimo Radical-7 system on post-operative day #1 in patients who underwent total joint arthroplasty. In the minority of patients (19%) who had a hemoglobin level of less than 10 g/dL, the difference between the two methods was statistically significant. Additionally, 100% of patients preferred the Masimo device to a traditional blood draw and the Masimo device was substantially cheaper. While further investigation of non-invasive hemoglobin monitoring systems is necessary, particularly in patients with a post-operative hemoglobin of less than 10 g/dL, our study shows that the Masimo Radical-7 device provides an accurate, preferable, and less expensive alternative to a traditional blood draw after total joint replacement. Conclusion. Overall, the non-invasive hemoglobin monitoring system offered a similar hemoglobin reading to the standard lab-draw reading, while improving satisfaction and lowering cost. The system relies on adequate perfusion for measurement, and our study demonstrated that lower hemoglobin values may reduce finger-tip perfusion and affect the hemoglobin reading


Shoulder replacement surgery is a well-established orthopaedic procedure designed to significantly enhance patients’ quality of life. However, the prevailing preoperative admission practices within our tertiary shoulder surgery unit involve a two-stage group and save testing process, necessitating an admission on the evening before surgery. This protocol may unnecessarily prolong hospital stays without yielding substantial clinical benefits. The principal aim of our study is to assess the necessity of conducting two preoperative group and save blood tests and to evaluate the requirement for blood transfusions in shoulder arthroplasty surgeries. A secondary objective is to reduce hospital stay durations and the associated admission costs for patients undergoing shoulder arthroplasty. We conducted a retrospective observational study covering the period from 1st January 2023 to 31st August 2023, collecting data from shoulder arthroplasty procedures across three hospitals within the Aneurin Bevan University Health Board. Our analysis included 21 total shoulder replacement cases and 13 reverse shoulder replacement cases. Notably, none of the patients required postoperative blood transfusions. The mean haemoglobin drop observed was 14 g/L for total shoulder replacements and 15 g/L for reverse shoulder replacements. The mean elective admission duration was 2.4 nights for total shoulder replacements and 2 nights for reverse shoulder replacements. Our data indicated that hospital stays were extended by one night primarily due to the preoperative group and save blood tests. In light of these findings, we propose a more streamlined admission process for elective shoulder replacement surgery, eliminating the need for the evening-before-surgery group and save testing. Hospital admissions in these units incur a cost of approximately £500 per night, while the group and save blood tests cost around £30 each. This revised admission procedure is expected to optimise the use of healthcare resources and improve patient satisfaction without compromising clinical care

Introduction. There is a paucity of studies analyzing the rates of revision total knee arthroplasty in diabetic patients stratified by glycated hemoglobin levels. The purpose of this study was to: 1) determine the incidence of revision TKA; 2) correlate the percent of glycated hemoglobin with incidence of revision; and 3) determine the cause of revision in diabetic patients stratified by glycated hemoglobin level. Methods. We utilized a national private payer dataset within the PearlDiver database from 2007 to 2015 quarter 1 to determine who had diabetes and underwent TKA. There were 424,107 patients who were included in the analysis. We determined the incidence of revision TKA in the overall cohort, in addition to stratifying the incidence by glycated hemoglobin levels. To determine the effect of glycated hemoglobin levels on revision TKA rate, we performed a correlation analysis between the level of glycated hemoglobin and the incidence of revision TKA. We performed descriptive statistics of the underlying cause of revision TKA in both the overall and stratified cohorts. Results. There was a 3.2% incidence of revision in the overall cohort. When stratified by glycated hemoglobin levels, the cohort in the 6.6 to 7.0% category had the lowest incidence of revision (2.9%). The cohorts in the 8.6 to 9%, 5.1 to 5.5%, and 4.6 to 5%, glycated hemoglobin categories had the highest revision rates of 3.7, 3.7 and 4.7%, respectively. There was a significantly positive correlation between rate of revision and ascending glycated hemoglobin levels, and significantly negative correlation between descending glycated hemoglobin levels and revision incidence. The most common cause of revision was infection in the overall and stratified groups. Conclusion. Sub-optimal glycated hemoglobin levels in diabetic patients correlated with increase revision rates in those who underwent TKA. This may imply that management of blood glucose levels should be optimized before undergoing total knee arthroplasty to minimize revision surgery risk

Introduction. Allogeneic blood transfusion (ABT) remains a widely used therapeutic intervention in patients undergoing total knee arthroplasty (TKA). There is mounting evidence that tranexamic acid (TXA), a powerful antifibinolytic, can significantly reduce perioperative blood loss with a concomitant lower ABT rate. In May 2012, TXA intravenous infusion was introduced as standard therapy in all patients undergoing major hip and knee arthroplasty. The TXA protocol included infusing 1 gm prior to incision and 1 gm after lowering the tourniquet. Nadir hemoglobin (Hb) level has been shown to be the single most important predictor of ABT in patients undergoing TKA. It is often used as the main trigger for ABT and in research trials examining restrictive transfusion trials. There is a paucity of information regarding the impact of TXA on Hb levels in patients undergoing primary TKA. The purpose of this retrospective study was to examine the impact of TXA on hemoglobin levels in primary TKA patients. Methods. Patients undergoing primary single, or bilateral, TKA from a single orthopedic surgeon from the years 2009–2010 before TXA infusion (n=78) were compared to patients undergoing the same operation after TXA was introduced as a therapeutic intervention (n=97). TKA is a very standardized operation that has stayed consistent over the convening years in terms of surgical technique and intra-operative management. The following Hb values were selected for analysis between the two groups: pre-surgical Hb value, immediate post-operative Hb, nadir Hb, and discharge Hb. Paired t-test was used for analysis with p-value set at 0.05. Additional data analysis included: length of stay (LOS) and rate of ABT. Results. Demographically, the control group was younger compared to the experimental group (60 vs. 64 years). Table 1 shows the difference in the selected Hb values between the two groups. There was no difference in Hb values going into surgery between the two groups. For all other Hb values, there was a significant difference between the control group and the TXA group throughout the postoperative period. In addition, Hb drift was significantly lower in the TXA group compared to the control group by 0.7 g/dl. ABT rate was 4% for the TXA group and 50% for the control group. The control group had a higher LOS compared to the TXA, 4.9 vs. 4.3 days. Conclusion. TXA infusion in the intraoperative period is an effective therapeutic intervention for reducing the downward drift of Hb levels throughout the postoperative period in patients undergoing TKA, and in turn, significantly impacts ABT rate and resource utilization

There is a narrow line between the benefits and risks of anticoagulant therapy. Many factors influence a patient’s response to warfarin, and careful monitoring is required to ensure that the therapeutic level of anticoagulation is achieved. The purpose of this retrospective review was to examine the relationship between the postoperative response to warfarin and the preoperative level of haemoglobin. The results showed that lower preoperative levels of haemoglobin are associated with an increased response to warfarin (p = 0.01)

Background and Objective. Patients undergoing hip fracture surgery have a high peri-operative mortality rate. We performed a retrospective study to ascertain if there is any relation between postoperative haemoglobin (Hb) decrease and cardiac related events following the surgery. Methodology. We carried out a retrospective study in this University Hospital's trauma unit. All patients operated for fracture neck of femur (hemiarthroplasty and DHS – Dynamic Hip Screw) between July 2006 and August 2008 were included in the study. Electronic records from the trauma unit, pathology portal, operating theatre and blood bank were obtained to identify the pre-operative and post-operative Hb levels, amount of blood transfused and Troponin T (TnT) level. Results. A total of 632 patients were operated for fracture neck of femur surgery during the study period of which 616 had complete perioperative blood results (DHS: 341; 80 male and 261 females and Hemiarthroplasty: 275; 68 male and 207 females). 60 patients had TnT levels performed within 10 days of the operation, thus suggesting possible cardiac related symptoms. Of these, 25 patients had a raised TnT (= 0.03). 24 (96%) of these patients had a post-operative Hb decrease compared with 550/591 (93%) patients without TnT (mean 2.3, range 0.1-5.6 g/dl compared with a mean of 2.4 and a range of 0-7 g/dl). Scatter diagram illustrated a positive correlation between post-operative Hb drop and TnT rise. Linear regression analysis concluded that a post-operative Hb drop is significantly correlated to TnT rise at the 10% level (p = 0.064). Conclusions. Post-operative Hb decline correlates with a raised TnT. As a standard protocol, post-operative bloods including Hb are performed the day after surgery. Given the correlation demonstrated, we recommend Hb levels to be analysed on day of surgery, to effectively manage low Hb levels before cardiac symptoms can develop

To determine whether pre-operative cessation of anticoagulant or antiplatelet medication is necessary for patients undergoing total shoulder arthroplasty (TSA) or reverse total shoulder arthroplasty (RTSA). A prospectively maintained database was used to identify 213 consecutive patients treated with TSA or RTSA performed by a single surgeon across 3 centres. This cohort included 24 patients on an anticoagulant agent (warfarin, apixaban, rivaroxaban, dabigatran), 52 patients on an antiplatelet agent (aspirin, clopidogrel), and a control group of 137 patients not on anticoagulant or antiplatelet medication. Patients on anticoagulant or antiplatelet medications continued these agents peri-operatively. Outcomes included haemoglobin drop, intra-operative blood loss, operative time, transfusion requirements and post-operative complications. The mean age of the cohort was 74.3 years (range 47 – 93) and 75 (35.2%) of the patients were male. TSA was performed in 63 cases and RTSA in 150 cases. The mean haemoglobin drop in the control group was 17.3 g/L, compared to 19.3 g/L in the anticoagulant group (p = 0.20) and 15.6 g/L in the anti-platelet group (p = 0.14). The mean intra-operative blood loss in the control group was 107.8 mL, compared to 143.0 mL in the anticoagulant group (p = 0.03) and 134.3 mL in the anti-platelet group (0.02). The mean operative time in the control group was 49.3 minutes, compared to 47.1 minutes in the anticoagulant group (p = 0.56) and 50.3 minutes in the anti-platelet group (p = 0.78). Post-operatively no patients developed a wound infection or haematoma requiring intervention. Three patients not on anticoagulant or antiplatelet medication developed pulmonary embolism. Continuing anticoagulant or antiplatelet medication was associated with higher intra-operative blood loss, but produced no statistically significant differences in haemoglobin drop, operative time, transfusion requirements or post-operative complications. We now do not routinely stop any anticoagulant or antiplatelet medication for patients undergoing total shoulder arthroplasty

This study aimed to identify factors associated with increased rates of blood transfusion in patients with adolescent idiopathic scoliosis (AIS) treated with posterior spinal fusion (PSF). A retrospective case-control study was performed for AIS patients treated at a large children's hospital between August 2018 and December 2020. All patients with a diagnosis of AIS were evaluated. Data on patient demographics, AIS, and transfusion parameters were collected. Univariate regression and multivariate logistic modeling were utilized to assess risk factors associated with requiring transfusion. Odds ratios (OR) and 95% confidence interval (CI) were calculated. Surgeries were done by three surgeons and thirty anesthesiologists. To quantify the influence of anesthesia practice preferences a categorical variable was defined as “higher-transfusion practice preference”, for the provider with the highest rate of transfusion. A total of 157 AIS patients were included, of whom 56 were transfused RBC units (cases), and 101 did not receive any RBC transfusion (controls). On univariate analysis, the following variables were significantly correlated with receiving RBC transfusion: “higher-transfusion practice preference,” “administration of crystalloids,” “receiving fresh frozen plasma (FFP),” “receiving platelets,” “pre-operative hemoglobin,” “cell saver volume,” and “surgical time.” On multiple regression modeling, “pre-operative hemoglobin less than 120 g/L” (OR 14.05, 95% CI: 1.951 to 135.7) and “higher-transfusion practice preference” (OR 11.84, 95% CI: 2.505 to 63.65) were found to be meaningfully and significantly predictive of RBC transfusion. In this cohort, we identified pre-operative hemoglobin of 120 g/L as a critical threshold for requiring transfusion. In addition, we identified significant contribution from anesthesia transfusion practice preferences. Our multivariate model indicated that these two factors are the major significant contributors to allogenic blood transfusion. Although further studies are required to better understand factors contributing to transfusion in AIS patients, we suggest standardized, peri-operative evidence-based strategies to potentially help reduce variations due to individual provider preferences

Abstract. Background. The aim of this study is to analyse the radiological outcomes and predictors of avascular necrosis following 2-hole DHS in Garden I and II neck of femur fractures in patients >60 years with a minimum follow up of one year. Methods. We retrospectively reviewed 51 consecutive patients >60 years who underwent DHS fixation for Garden I and II fractures. Demographics, fracture classification, time to surgery, pre-operative AMTS, preoperative posterior tilt angle, quality of reduction, pre and post-operative haemoglobin (hb), creatinine and comorbidities were analysed. Results. There were 40 (78.4%) females and the mean age was 77 years. 28 and 23 were Garden I and II NOF fractures respectively. Union was observed in all our patients except one. 12/51(23.5%) developed AVN of the femoral head. Statistically significant higher incidence of AVN was noted in patients with a pre-op tilt angle > 20. 0. (p = 0.006). The mean drop in Hb was higher in patients who developed AVN (21.5 g/L) versus the non-AVN group (15.9 g/L) (p = 0.001). There was no difference in AVN with respect to laterality, mean time to surgery, pre-operative AMTS and Charlson comorbidity index. 4/52 (7.6%) had re-operations. The 30-day and one year mortality were 1.9 % and 11.7 % respectively. Conclusion. In our series a preoperative posterior tilt angle of >20. 0. and a drop in haemoglobin were found to correlate with the progression to AVN. No correlation was observed between AVN and time to surgery, laterality, quality of reduction and comorbidities

Abstract. Background. The use of tourniquet in knee arthroplasty is common but in recent times, it has initiated a debate on its use. Complications from use of tourniquet are well documented in literature but there is less evidence on thigh pain, blood loss and length of stay post arthroplasty. Methods. We included 62 patients undergoing knee arthroplasty either Unicondylar knee arthroplasty or total knee arthroplasty. Patients were allocated randomly in tourniquet and without tourniquet groups. Half of the patients had UKA and other half TKA. Thigh pain was recorded using Visual analogue scale on day 1, 2 and on discharge. A drop in pre-operative and post operative haemoglobin level was recorded too. Independent sample t-test was done to compare the difference between the 2 groups mainly, drop in haemoglobin, thigh pain, knee pain and length of stay. Results. The mean drop in haemoglobin was comparable and was not statistically significant. Thigh pain on day 1 in no tourniquet group had a mean value of 0 and statistically significant difference in thigh pain at day 1,2 and discharge. In subgroup analysis between TKA and UKA the latter had improved results. Conclusions. Knee arthroplasty surgery without the use of tourniquet provides less thigh pain in the post-operative period and there isn't any significant difference in post op haemoglobin drop. None of the patient required any blood transfusion. A trend of early discharge was noted too but probably due to sample size, it wasn't statistically significant

Abstract. Background. Blood transfusion requirement after primary total hip replacements (THR) and total knee replacements (TKR) was found to be related to increased post-operative complications rate and length of hospital stay. Pre-operative haemoglobin level remains the single most important factor determining the requirement for post-operative blood transfusion. Methods. We carried out a local retrospective audit of 977 THRs and TKRs in 2019. Pre-operative and pre-transfusion haemoglobin levels for transfused group of patients were recorded. Results. A total of 977 patients had THRs and TKRs of which 34 (3.5%) had blood transfusions. From the 437 THRs, 24(5.5%) had bloods transfusions of which 19 were female. From the 540 TKRs 10(1.8%) were transfused, 9 were female. The average length of stay (LOS) for those transfused was 6.8 days and the average LOS for our trust is 3.2 days. Conclusion. The incidence of blood transfusion after primary THR or TKR was 3.5% in 2019. The majority of patients who received post-operative blood transfusion were found to have pre-operative haemoglobin level below 12 g/dl representing 61.7% of all the transfused patients. Transfused patients had more than double the average expected length of stay. The incidence of blood transfusion and associated risks can be improved by early detection and proper management of pre-op anaemia. Pre-assessment clinic has got a pivotal role but needs reminding of pre-op haemoglobin management strategies. Pre-operative optimisation of patients using either oral, intravenous iron supplements or erythropoietin according to the blood management protocol by the Royal College of Surgeons

Abstract. Aim. This study was aimed to look into factors responsible for delayed mobilization after lower limb arthroplasty and effect on length of stay. Methods. It is an observational study conducted at Kings Mill Hospital from August to October 2021. All patients undergoing primary knee or hip arthroplasty were included in the study, while patients with revision surgeries were excluded. A proforma was designed to record demographics and different variables including medications, type of anaesthesia, orthostatic hypotension, dizziness, preop and post op pain score, fall in haemoglobin, analgesia and length of stay. Patients were assessed on day one and data was recorded in the proforma. Data was analysed using SPSS. Results. There were 60 patients (32 females and 28 males) which were included in the study. Mean age was 69.62 years. Thirty patients underwent TKA while in the rest of 30 patients THA was done. Fifty patients were mobilized on day one while 10 patients failed to mobilise. Out of different variables assessed orthostatic hypotension, dizziness, pre mobilization pain score and pain score during mobilization were found to be significant. Mean length of stay was longer in patients with delayed mobilization (P=0.018). Conclusion. Pain, dizziness and orthostatic hypotension were independent factors affecting mobilization after lower limb arthroplasty and indirectly increasing the length of stay

The aim of this investigation is to firstly quantify the burden of disease, and secondly qualify the organisms being cultured during debridement to establish their sensitivities to available antibiotics. This study will also look at the concomitant burden of Human Immunodeficiency Virus (HIV) and Diabetes Mellitus (DM) in cases of hand sepsis, to establish whether these two disease processes require special consideration and treatment tailoring. The method employed to collect the data will be retrospective collection of patient information, using random sampling. Included patients will be adult patients who undergo debridement and have a sample registered on the National Health Laboratory System (NHLS). The daily intake sheet will be used to collect patients details and these details will then be used to collect results of intra-operative specimens using the NHLS. These patient details will also be used to check patients’ HIV results and HBA1c (Glycated Haemoglobin A1c) results. Majority of the specimens revealed Methicillin Sensitive Staphylococcus Aureus (MSSA), with a reassuringly low rate of Methicillin Resistant Staphylococcus Aureus (MRSA) when compared to international data. The yield of patients tested for HIV or DM was lower than expected, despite this being the standard of care. The projected impact of this study will be assessment of the current burden of disease and then clarification of our current management strategy, in order to assess if it would be possible to change to a more cost-effective antibiotic with a narrower spectrum of bacteria coverage. The potential exists to not only implement cost saving measures, but also promote antibiotic stewardship by decreasing the practice of empiric broad-spectrum antibiotic use

Reported wound complication in below knee surgery can be quite high. Recent study demonstrated that increased blood loss and hematoma formation increase wound complications especially in foot and ankle surgeries. Despite the evidence on the benefit of TXA on blood loss in TKA and THA it is not routinely used by surgeon in below knee surgery. To assess the efficacy and safety of this medication in reducing wound complication and blood loss and the risk of thromboembolic complications in patients undergoing below knee surgery. A systematic literature search of PubMed, Embase, Ovid, the Cochrane Library and AAOS and AOFAS conference proceedings was conducted. The primary outcome was the rate of wound complications. Data were analyzed using the Review Manager 5.3 software. Nine studies involving 861 patients met the inclusion criteria. The meta-analysis indicated that TXA, when compared to a control group, reduced wound complications (OR, 0.54; 95% IC, 0.31 to 0.95, p = 0,03), blood loss (MD = −149,4 ml; 95% CI, −205,3ml to −93,6ml), post-operative drainage (MD = −169,8 ml; 95% CI, −176,7 to −162,9 ml) and hemoglobin drop (MD = −8,75 g/dL; 95% IC, −9,6 g/dL to −7,8 g/dL). There was no significant difference in thromboembolic events (RR 0,53; 95% CI, 0,15 - 1,90; p = 0,33). This study demonstrated that TXA could be use in below knee surgery to reduce wound complication and blood loss without increased thromboembolic complications. The small number of studies limit the findings interpretation. Further studies are needed to sustain those resutls

Routine post-operative bloods following all elective arthroplasty may be unnecessary. This retrospective cohort study aims to define the proportion of post-operative tests altering clinical management. Clinical coding identified all elective hip or knee joint replacement under Hawkes Bay District Health Board contract between September 2019-December 2020 (N=373). Uni-compartmental and bilateral replacements, procedures performed for cancer, and those with insufficient data were excluded. Demographics, perioperative technique, and medical complication data was collected. Pre- and post-operative blood tests were assessed. Outcome measures included clinical intervention for abnormal post-operative sodium (Na), creatinine (Cr), haemoglobin (Hb), or potassium (K) levels. A cost-benefit analysis assessed unnecessary testing. 350 patients were Included. Median age was 71 (range 34-92), with 46.9% male. Only 26 abnormal post-operative results required intervention (7.1%). 11 interventions were for low Na, 4 for low K, and 4 for elevated Cr. Only 7 patients were transfused blood products. Older age (p=0.009) and higher ASA (p=0.02) were associated with intervention of any kind. Abnormal preoperative results significantly predicted intervention for Na (p<0.05) and Cr (p<0.05). All patients requiring treatment for K used diuretic medication. Preoperative Hb level was not associated with need for transfusion. Overall, there were 1027 unnecessary investigations resulting in $18,307 excess expenditure. Our study identified that the majority of elective arthroplasty patients do not require routine postoperative blood testing. We recommend investigations for patients with preoperative electrolyte abnormality, those taking diuretics, and patients with significant blood loss noted intra-operatively. In future, a larger, randomised controlled trial would be useful to confirm these factors

The aim of this study is to analyse the radiological outcomes and predictors of avascular necrosis following 2-hole DHS in Garden I and II neck of femur fractures in patients more than 60 years with a minimum follow up of one year. We retrospectively reviewed 51 consecutive patients aged more than 60 years who underwent DHS fixation for Garden I and II fractures. Demographics, fracture classification, time to surgery, pre-operative AMTS, preoperative posterior tilt angle, quality of reduction, pre and post-operative haemoglobin(hb), creatinine and comorbidities were analysed for correlation with AVN using Chi-Square test, Independent Sample and paired t test. There were 40 (78.4%) females and the mean age of the cohort was 77 years. 28 and 23 were Garden I and II NOF fractures respectively. Union was observed in all our patients except one(kappa =1). 12/51(23.5%) developed AVN of the femoral head. Statistically significant higher incidence of AVN was noted in patients with a pre-op tilt angle > 200 (p = 0.006). The mean drop in Hb was higher in patients who developed AVN (21.5 g/L) versus the non-AVN group (15.9 g/L) (p = 0.001). There was no difference in AVN rates with respect to laterality, mean time to surgery, pre-op AMTS and Charlson comorbidity index. 4/52 (7.6%) had re-operations (one hardware prominence, two conversions to arthroplasty, one fixation failure during the immediate post-op period). The 30-day and one year mortality rates were 1.9 % and 11.7 % respectively. 2-hole DHS fixation in undisplaced NOF fractures has excellent union rates. A pre-operative posterior tilt angle of >200 and a greater difference in pre and post operative haemoglobin were found to correlate positively with the progression to AVN . No correlation was observed between AVN and time to surgery, laterality, quality of reduction and comorbidities

With an ageing population and an increasing number of primary arthroplasties performed, the revision burden is predicted to increase. The aims of this study were to 1. Determine the revision burden in an academic hospital over a 11-year period; 2. identify the direct hospital cost associated with the delivery of revision service and 3. ascertain factors associated with increased cost. This is an IRB-approved, retrospective, single tertiary referral center, consecutive case series. Using the hospital data warehouse, all patients that underwent revision hip or knee arthroplasty surgery between 2008-2018 were identified. 1632 revisions were identified (1304 patients), consisting of 1061 hip and 571 knee revisions. The majority of revisions were performed for mechanical-related problems and aseptic loosening (n=903; 55.3%); followed by periprosthetic joint infection (n=553; 33.9%) and periprosthetic fractures (176; 10.8%). Cost and length of stay was determined for all patient. The direct in-hospital costs were converted to 2020 inflation-adjusted Canadian dollars. Several patients- (age; gender; HOMR- and ASA-scores; Hemoglobin level) and surgical- (indication for surgery; surgical site) factors were tested for possible associations. The number of revisions increased by 210% in the study period (2008 vs. 2018: 83 vs. 174). Revision indications changed over study period; with prevalence of fracture increasing by 460% (5 in 2008 vs. 23 in 2018) with an accompanying reduction in mechanical-related reasons, whilst revisions for infection remained constant. The mean annual cost over the entire study period was 3.9 MMCAD (range:2.4–5.1 MMCAD). The cost raised 150% over the study period from 2.4 MMCAD in 2008 to 3.6 MMCAD. Revisions for fractured had the greatest length of stay, the highest mean age, HOMR-score, ASA and cost associated with treatment compared to other revision indications (p < 0 .001). Patient factors associated with cost and length of stay included ASA- and HOMR-scores, Charlson-Comorbidity score and age. The revision burden increased 1.5-fold over the years and so has the direct cost of care delivery. The increased cost is primarily related to the prolonged hospital stay and increased surgical cost. For tertiary care units, these findings indicate a need to identify strategies on improving efficiencies whilst improving the quality of patient care (e.g. efficient ways of reducing acute hospital stay) and reducing the raise of the economic burden on a publicly funded health system

Aim. Recurrence of bone and joint infection, despite appropriate therapy, is well recognised and stimulates ongoing interest in identifying host factors that predict infection recurrence. Clinical prediction models exist for those treated with DAIR, but to date no models with a low risk of bias predict orthopaedic infection recurrence for people with surgically excised infection and removed metalwork. The aims of this study were to construct and internally validate a risk prediction model for infection recurrence at 12 months, and to identify factors that predict recurrence. Predictive factors must be easy to check in pre-operative assessment and relevant across patient groups. Methods. Four prospectively collected datasets including 1173 participants treated in European centres between 2003 and 2021, followed up to 12 months after surgery for orthopaedic infections, were included in logistic regression modelling [1–3]. The definition of infection recurrence was identical and ascertained separately from baseline factors in three contributing cohorts. Eight predictive factors were investigated following a priori sample size calculation: age, gender, BMI, ASA score, the number of prior operations, immunosuppressive medication, glycosylated haemoglobin (HbA1c), and smoking. Missing data, including systematically missing predictors, were imputed using Multiple Imputation by Chained Equations. Weekly alcohol intake was not included in modelling due to low inter-observer reliability (mean reported intake 12 units per week, 95% CI for mean inter-rater error −16.0 to +15.4 units per week). Results. Participants were 64% male, with a median age of 60 years (range 18–95). 86% of participants had lower limb orthopaedic infections. 732 participants were treated for osteomyelitis, including FRI, and 432 for PJI. 16% of participants experienced treatment failure by 12 months. The full prediction model had moderate apparent discrimination: AUROC (C statistic) 0.67, Brier score 0.13, and reasonable apparent calibration. Of the predictors of interest, associations with failure were seen with prior operations at the same anatomical site (odds ratio for failure 1.51 for each additional prior surgery; 95% CI 1.02 to 2.22, p=0.06), and the current use of immunosuppressive medications (odds ratio for failure 2.94; 95% CI 0.89 to 9.77, p=0.08). Conclusions. This association between number of prior surgeries and treatment failure supports the urgent need to streamline referral pathways for people with orthopaedic infection to specialist multidisciplinary units


In total knee arthroplasty (TKA), both intravenous (IV) and/or intra-articular (IA) administration of tranexamic acid (TXA) were showed to reduce blood loss. Moreover, research suggesting TXA decreases postoperative knee swelling, but it is unknown whether this results in improved postoperative rehabilitation outcome. Thus, the aim of this study was to evaluate whether combined IV and IA administration of TXA would associate with improved early rehabilitation outcomes. In this institutional review board approved randomized controlled trial, 179 patients scheduled for unilateral TKA were randomized to one of three regimens: (1) IA administration of 1gm TXA at end of procedure only, (2) additional preoperative IV dose of 15 mg/kg 30min before tourniquet inflation, and (3) additional postoperative dose 4hrs after preoperative dose. Primary outcomes included knee range of motion, Knee Society Score (KSS) at 6-month postoperatively, haemoglobin drop at day-2 post-operatively, and transfusion rate. Secondary outcome was venous thromboembolism (VTE) complications. Baseline characteristics were comparable between the allocation groups. Patients in regimen (3) showed statistically significant better knee extension range (6.2°, 5.9°, 2.9°, p=0.01), and KSS (88.5, 89.9, 93.0, p=0.02) at 6-month postoperatively, and lesser drop in haemoglobin at day-2 post-operatively (2.72, 2.47, 1.75 g/dL, p=0) when compared with patients in other regimens. No patients required transfusion, or complicated by VTE. The combined administration of IA and IV TXA, including both preoperative and postoperative doses, associated with statistically significantly improved early rehabilitation outcomes. Background. Tourniquets and tranexamic acid (TXA) are commonly used in total knee arthroplasty (TKA), but there is not consensus on how these interventions affect blood transfusion rates and total blood loss. Few studies examine the combined use of both interventions. We compared outcome measures and transfusion rates following TKA, with and without the use of tourniquet and TXA. Methods. Retrospective cohort study of 477 consecutive patients undergoing primary TKA at a single center between 2008 and 2013. There were 243 in the tourniquet-assisted (TA) and 234 in the tourniquet-unassisted (TU) group. Subanalysis was performed on those patients receiving TXA. Results. Mean operative duration was 66.4 minutes (30–135) in the TA group and 87.5 minutes (43–162) in the TU group (p<0.0001). Mean post-operative drop in hemoglobin was significantly greater in TU group (3.1g/dl vs. 2.8g/dl, p=0.002). The transfusion rate was 9.5% in TA compared to 11.5% in TU patients (p=0.46) with comparable mean units transfused (2.6 vs. 2.2, p=0.30). There was no significant difference in rate of wound infection (2% vs. 1.7%, p = 0.82) and total complication rate (4.1% vs. 2.5%, p=0.19) between groups. Mean length of stay was 5.8 days in TA and 7 days in the TU group (p=0.07). Those patients given TXA had a lower hemoglobin drop (2.6g/dl vs. 3.3g/dl, p=0.04) with similar transfusion (10.9% vs. 13.3%, p=0.61) and complication (3.3% vs. 5.2%, p=0.95) rates. Discussion. Tourniquet-unassisted TKA had a greater operative duration and post-operative drop in hemoglobin than tourniquet-assisted TKA. However, transfusion rates were similar between groups. TXA use reduced the operative decrease in hemoglobin with no effect on complication or transfusion rates. Key words. TKA, tourniquet, tranexamic acid, transfusion rates, outcomes.
