Lisfranc injuries were previously described as fracture-dislocations of the tarsometatarsal joints. With advancements in modern imaging, subtle Lisfranc injuries are now more frequently recognized, revealing that their true incidence is much higher than previously thought. Injury patterns can vary widely in severity and anatomy. Early diagnosis and treatment are essential to achieve good outcomes. The original classification systems were anatomy-based, and limited as tools for guiding treatment. The current review, using the best available evidence, instead introduces a stability-based classification system, with weightbearing radiographs and CT serving as key diagnostic tools. Stable injuries generally have good outcomes with nonoperative management, most reliably treated with immobilization and non-weightbearing for six weeks. Displaced or comminuted injuries require surgical intervention, with open reduction and internal fixation (ORIF) being the most common approach, with a consensus towards bridge plating. While ORIF generally achieves satisfactory results, its effectiveness can vary, particularly in high-energy injuries. Surgical reconstruction of deformed Charcot feet carries a high risk of nonunion, metalwork failure, and deformity recurrence. The primary aim of this study was to identify the factors contributing to these complications following hindfoot Charcot reconstructions. We retrospectively analyzed patients who underwent hindfoot Charcot reconstruction with an intramedullary nail between January 2007 and December 2019 in our unit. Patient demographic details, comorbidities, weightbearing status, and postoperative complications were noted. Metalwork breakage, nonunion, deformity recurrence, concurrent midfoot reconstruction, and the measurements related to intramedullary nail were also recorded.Aims
Methods
Aim. Anatomical reduction and Stable fixation of Lisfranc injuries is considered the gold standard. There is controversy about how it is best achieved. Some surgeons would advocate routine open anatomical reduction, which as a concept was popular in 1980s but the same anatomical reduction and fixation can be achieved percutaneously. We describe our method of close reduction and percutaneous fixation and present our results. Materials and methods. 22 patients with a minimum follow up of 12 months were included. We achieved satisfactory anatomical reduction percutaneously in all patients and internal fixation was performed using cannulated screws for medial and middle columns. Functional outcome was evaluated using Foot and Ankle Disability Index (FADI) and components of this score were analysed individually to assess which domain was most affected. Vertical ground reaction forces were measured using a force plate in a walking platform. Results. The average age at operation was 48 years (17–67). Mean follow up was 20 months (13–60). The average Foot & Ankle Disability Index at final follow up was 79 (66–94). No loss of reduction or metal breakage was noted. Walking on uneven surface, going down stairs, heavy work and pain first thing in the morning were the domains of functional Index that showed poor recovery. None of the patients had pain at rest. Only three patients found it extremely hard to return to recreational activities. None of the patients had problems related to wound. Gait analysis showed a prolonged push-off (p=0.22) and significantly prolonged pre-swing phase (p=0.015) of the affected limb. Conclusions. Percutaneous reduction and

The ideal management of acute syndesmotic injuries in elite athletes is controversial. Among several treatment methods used to stabilize the syndesmosis and facilitate healing of the ligaments, the use of suture tape (InternalBrace) has previously been described. The purpose of this study was to analyze the functional outcome, including American Orthopaedic Foot & Ankle Society (AOFAS) scores, knee-to-wall measurements, and the time to return to play in days, of unstable syndesmotic injuries treated with the use of the InternalBrace in elite athletes. Data on a consecutive group of elite athletes who underwent isolated reconstruction of the anterior inferior tibiofibular ligament using the InternalBrace were collected prospectively. Our patient group consisted of 19 elite male athletes with a mean age of 24.5 years (17 to 52). Isolated injuries were seen in 12 patients while associated injuries were found in seven patients (fibular fracture, medial malleolus fracture, anterior talofibular ligament rupture, and posterior malleolus fracture). All patients had a minimum follow-up period of 17 months (mean 27 months (17 to 35)).Aims
Methods
Introduction. We aimed to retrospectively identify risk factors for delayed / non-union for first metatarsophalangeal joint fusion. Methods. Case notes and radiograph analysis was performed for operations between April 2014 and April 2016 with at least 3 months post-operative follow up. Union was defined as bridging bone across the fusion site on AP and lateral radiographic views with no movement or pain at the MTPJ on examination. If union was not certain, CT scans were performed. All patients operations were performed/supervised by one of three consultant foot surgeons. Surgery was performed through a dorsal approach using the Anchorage compression plate. Blinded pre-operative AP radiographs were analysed for the presence of a severe hallux valgus angle equal or above 40 degrees. Measurement intra-observer reliability was acceptable (95%CI:1.6–2.3 degrees). Smoking and medical conditions associated with non-union underwent univariate analysis for significance. Results. 73 patients, 9 male, 64 female with a mean age of 61 years (range, 29 to 81) comprised the patient group. Mean follow up time was 13 months for both union vs non-union groups (range 3 to 24 months). 7 patients were identified as non / delayed union (9.6%). All smokers healed (n = 17), age, diabetes, COPD and rheumatoid arthritis did not show significant associations with non-union. Pre-operative hypothyroidism (relative risk 6.9, p = 0.05) and severe hallux valgus (relative risk 9.9, p = 0.002) were significantly associated with non / delayed union. Conclusion. Although overall bone mineral density is unaffected, studies have demonstrated abnormal bone remodelling in patients with hypothyroidism which may account for this unexpected finding. When inserting a lag-screw across an arthrodesis, stress is concentrated under the screw head risking asymmetrical force distribution and fracture of the cortical bone bridge. The IO FiX (Extremity Medical, NJ USA) is a new intraosseous device comprising an X-Post on one side of and parallel to the arthrodesis and a lag-screw inserted through the head of the X-Post which reinforces the cortical bone bridge. The X-Post behaves as an internal washer improving force distribution across the arthrodesis. Being intraosseous, near to the neutral axis of bend also means the device is fatigue-resistant and soft tissue irritation is reduced. The IO FiX has not been independently verified and therefore we analysed its performance in a human cadaveric ankle model. Our null hypothesis was there is no difference in force generation and contact area in an ankle arthrodesis when the IO FiX is compared with partially-threaded lag-screws. We used ten randomized cadaver ankles with a mean age of seventy-one years (44–84 years) prepared with flat arthrodesis cuts. A Tek-scan (Boston, USA) pressure transducer was used to measure force and contact area produced when the IO FiX was compared with a standard lag-screw and washer. The median average force in the IO FiX group was 3.95 kg and 2.35 kg in the lag-screw group (p=<0.01 Wilcoxon signed-rank). The IO FiX was able to create a more uniform contact area within the arthrodesis with a median average of 3.41 cm. 2. compared with 2.42 cm. 2. in the lag-screw group (p=<0.03 Wilcoxon signed rank). In developed nations Charcot arthropathy is most commonly caused by diabetes mellitus. Worldwide, leprosy remains the primary cause. All evidence points to a relationship between neurologic loss, continued loading activities and the development of unrecognized bone fragmentation. In type 2 diabetes, dysregulation of leptin biology causes bone loss and may be an important factor in precipitating Charcot events. Bone density studies show massive loss of bone in patients with ankle and hindfoot Charcot problems, but not midfoot problems. This suggests a different mechanism for collapse. Introduction. We describe our experience with a minimally invasive Chevron and Akin (MICA) technique for hallux valgus correction. This technique adheres to the same principles as open surgical correction but is performed using a specialized high-speed cutting burr under image intensifier guidance via tiny skin portals. Methods. All patients undergoing minimally invasive hallux valgus correction between November 2009 and April 2010 were included in this study and were subject to prospective clinical and radiological review. Patients were scored using the Kitaoka score as well as radiological review and patient satisfaction survey. Surgery was performed under general anaesthetic and included distal soft tissue release, Chevron and Akin osteotomies, with the same indications as for open surgery. All osteotomies were internally fixed with cannulated compression screws. Results. 83 operations were performed on 70 patients (2 male 65 female, mean age 54 years (27-78)). The pre-operative mean HVA was 34° and IMA 14°. Post-operative mean HVA was 9° and IMA 9.5°. Kitaoka score improved significantly at 3-12 months follow-up. There were no delayed or non- s and no osteonecrosis. MEDLINE, Web of Science, Embase, and Scopus databases were searched for studies reporting outcomes following ankle arthrodesis, in which standardized postoperative rehabilitation regimens were employed. Eligible studies were grouped according to duration of postoperative nonweightbearing: zero to one weeks (group A), two to three weeks (group B), four to five weeks (group C), or six weeks or more (group D). Anatomical atlases document classical safe corridors for the
placement of transosseous fine wires through the calcaneum during
circular frame external fixation. During this process, the posterior
tibial neurovascular bundle (PTNVB) is placed at risk, though this
has not been previously quantified. We describe a cadaveric study
to investigate a safe technique for posterolateral to anteromedial
fine wire insertion through the body of the calcaneum. A total of 20 embalmed cadaveric lower limbs were divided into
two groups. Wires were inserted using two possible insertion points
and at varying angles. In Group A, wires were inserted one-third
along a line between the point of the heel and the tip of the lateral
malleolus while in Group B, wires were inserted halfway along this
line. Standard dissection techniques identified the structures at
risk and the distance of wires from neurovascular structures was measured.

The aim of this retrospective study was to compare the functional
and radiological outcomes of bridge plating, screw fixation, and
a combination of both methods for the treatment of Lisfranc fracture
dislocations. A total of 108 patients were treated for a Lisfranc fracture
dislocation over a period of nine years. Of these, 38 underwent
transarticular screw fixation, 45 dorsal bridge plating, and 25
a combination technique. Injuries were assessed preoperatively according
to the Myerson classification system. The outcome measures included
the American Orthopaedic Foot and Ankle Society (AOFAS) score, the
validated Manchester Oxford Foot Questionnaire (MOXFQ) functional
tool, and the radiological Wilppula classification of anatomical

The aim of this study was to report a single surgeon series of
consecutive patients with moderate hallux valgus managed with a
percutaneous extra-articular reverse-L chevron (PERC) osteotomy. A total of 38 patients underwent 45 procedures. There were 35
women and three men. The mean age of the patients was 48 years (17
to 69). An additional percutaneous Akin osteotomy was performed
in 37 feet and percutaneous lateral capsular release was performed
in 22 feet. Clinical and radiological assessments included the type
of forefoot, range of movement, the American Orthopedic Foot and
Ankle (AOFAS) score, a subjective rating and radiological parameters. The mean follow-up was 59.1 months (45.9 to 75.2). No patients
