Aim. To investigate self-reported quality of life (QoL) in patients with osteomyelitis referred to a specialist centre in the UK and investigate the relationship between QoL and BACH classification. Method. All patients newly referred to a specialist bone infection clinic at a single tertiary centre within the UK between January 2019 and February 2020 were prospectively included. Diagnosis of osteomyelitis was made according to the presence of clinical and radiological criteria for ≥6 months. An EQ-5D-5L questionnaire and visual analogue score (VAS) were completed during the initial clinic appointment. Long-bone osteomyelitis was classified by the attending orthopaedic surgeon using the BACH classification system as either uncomplicated, complex or with limited options available.1 Patients managed non-operatively were subclassified into those who were (i) unfit to receive an operation or (ii) fit and well with stable disease. EQ-5D index scores were compared to a published UK value set of 41 chronic health conditions within the UK.2. Results. 201 patients were referred during the study period, with 159 (79.1%) patients diagnosed with long-bone osteomyelitis and 16 (8.0%) with osteomyelitis of the pelvic bones. Patients with pelvic osteomyelitis reported lower EQ-5D index scores compared to long-bone osteomyelitis (EQ-5D: 0.097 vs. 0.435, p<0.001) but similar VAS (60.2 vs. 54.6, p=0.37). Long-bone and pelvic osteomyelitis gave the 40th and 41st lowest EQ-5D scores respectively when compared to 41 other chronic health conditions including stroke, chronic obstructive pulmonary disease, kidney disease, liver disease and malignancy. Patients classified as having uncomplicated long-bone osteomyelitis reported significantly higher QoL compared to those classified as complex osteomyelitis (EQ-5D: 0.527 vs. 0.401, p<0.05; VAS: 66.9 vs. 58.4, p<0.05). Patients who were not fit for surgery due to co-morbidity reported similar QoL scores compared to those patients with complex osteomyelitis (EQ-5D: 0.293, p=0.07; VAS: 46.6, p=0.06). Patients with stable disease who did not require surgery, gave significantly better QoL scores when compared to the other classifications of osteomyelitis (EQ-5D: 0.746, p<0.01; VAS: 81.9, p<0.01). Conclusions. Patient reported QoL in osteomyelitis correlates with disease complexity as classified according to the BACH classification system. Patients with pelvic and long-bone osteomyelitis rate their QoL lower than patients with other chronic diseases

Progressive collapsing foot deformity (PCFD) is a complex foot deformity with varying degrees of hindfoot valgus, forefoot abduction, forefoot varus, and collapse or hypermobility of the medial column. In its management, muscle and tendon balancing are important to address the deformity. Peroneus brevis is the primary evertor of the foot, and the strongest antagonist to the tibialis posterior. Moreover, peroneus longus is an important stabilizer of the medial column. To our knowledge, the role of peroneus brevis to peroneus longus tendon transfer in cases of PCFD has not been reported. This study evaluates patient reported outcomes including pain scores and any associated surgical complications for patients with PCFD undergoing isolated peroneus brevis to longus tendon transfer and gastrocnemius recession. Patients with symptomatic PCFD who had failed non-operative treatment, and underwent isolated soft tissue correction with peroneus brevis to longus tendon transfer and gastrocnemius recession were included. Procedures were performed by a single surgeon at a large University affiliated teaching hospital between January 1 2016 to March 31 2021. Patients younger than 18 years old, or undergoing surgical correction for PCFD which included osseous correction were excluded. Patient demographics, medical comorbidities, procedures performed, and pre and post-operative patient related outcomes were collected via medical chart review and using the appropriate questionnaires. Outcomes assessed included Visual Analogue Scale (VAS) for foot and ankle pain as well as sinus tarsi pain (0-10), patient reported outcomes on EQ-5D, and documented complications. Statistical analysis was utilized to report change in VAS and EQ-5D outcomes using a paired t-test. Statistical significance was noted with p<0.05. We analysed 43 feet in 39 adults who fulfilled the inclusion criteria. Mean age was 55.4 ± 14.5 years old. The patient reported outcome mean results and statistical analysis are shown in Table one below. Mean pre and post-operative foot and ankle VAS pain was 6.73, and 3.13 respectively with a mean difference of 3.6 (p<0.001, 95% CI 2.6, 4.6). Mean pre and post-operative sinus tarsi VAS pain was 6.03 and 3.88, respectively with a mean difference of 2.1 (p<0.001, 95% CI 0.9, 3.4). Mean pre and post-operative EQ-5D Pain scores were 2.19 and 1.83 respectively with a mean difference of 0.4 (p=0.008, 95% CI 0.1, 0.6). Mean follow up time was 18.8 ± 18.4 months. Peroneus brevis to longus tendon transfer and gastrocnemius recession in the management of symptomatic progressive collapsing foot deformity significantly improved sinus tarsi and overall foot and ankle pain. Most EQ-5D scores improved, but did not reach statistically significant values with the exception of the pain score. This may have been limited by our cohort size. To our knowledge, this is the first report in the literature describing clinical results in the form of patient reported outcomes following treatment with this combination of isolated soft tissue procedures for the treatment of PCFD. Patients with pelvic osteomyelitis reported lower EQ-5D index scores compared to long-bone osteomyelitis (EQ-5D:0.097 vs. 0.435, p<0.001) but similar VAS (60.2 vs. 54.6, p=0.37). Long-bone and pelvic osteomyelitis gave the lowest EQ-5D scores respectively when compared to 41 other chronic health conditions including stroke, chronic obstructive pulmonary disease, kidney disease, liver disease and malignancy. Introduction. Management of Vancouver type B1 and C periprosthetic fractures in elderly patients requires fixation and an aim for early mobilisation but many techniques restrict weightbearing due to re-fracture risk. We present the clinical and radiographic outcomes of our technique of total femoral plating (TFP) to allow early weightbearing whilst reducing risk of re-fracture. Methods. A single-centre retrospective cohort study was performed including twenty-two patients treated with TFP for fracture around either hip or knee replacements between May 2014 and December 2017. Follow-up data was compared at 6, 12 and 24 months. Primary outcomes were functional scores (Oxford Hip or Knee score (OHS/OKS)), Quality of Life (EQ-5D) and satisfaction at final follow-up (Visual Analogue Score (VAS)). Secondary outcomes were radiographic fracture union and complications. Results. Mean OHS and OKS was 50.25, EQ-5D score was >4 for all modalities, VAS was 64.4/100. Radiographs demonstrated bony union in 58% at 3 months and 76% at 6 months. We identified no case of re-fracture however non-union occurred in 4 patients. No other operative complications were identified. Conclusion. These results suggest that TFP may be a safe, viable option for management of periprosthetic fractures around stable implants allowing the benefit of early weightbearing, satisfactory outcomes and low re-fracture risk

Introduction. Pain after trauma has received relatively little research attention compared with surgical techniques and functional outcomes, but is important to patients. We aimed to describe nerve dysfunction and pain characteristics using tibial fractures as a model. We hypothesized that early nerve dysfunction was associated with neuropathic and chronic pain. Materials and Methods. Adult patients with isolated open or closed tibial diaphyseal fractures were prospectively observed for 1-year in 5 Major Trauma Centres. Nerve dysfunction was assessed using Semmes-Weinstein monofilaments, acute pain with the visual numerical rating scale (VNRS), neuropathic pain with the doleur neuropathique-4 score and quality of life (QOL) using the EQ-5D score. Results. Of 77 included patients, twenty-six (33.8%) had Gustilo-Anderson grade II or worse injuries. Forty-six (63.0%) had operative repair; 0 operation notes reported nerve injury. Mean VNRS pain scores one week post-injury were 4.6/10 (SD 2.4). Seventeen patients (23.3%) presented with impaired sensation, persisting in 11 (15.1%) at 3-months. Neuropathic pain affected 16 (20.8%) patients one week post-injury; a further 20 (26.0%) had >2 symptoms of nerve pain. Only four (11.1%) received anti-neuropathic drugs. At 6-months, twenty-three (31.5%) reported mobility problems, 21 (28.8%) difficulty completing normal activities, 25 (34.2%) pain and 15 (20.5%) anxiety and depression. Conclusions. Nerve dysfunction and neuropathic pain are common early features in patients with tibial fractures, but are poorly recognised and inadequately treated. Pain has long-term effects on patients’ QOL. Future work should aim to improve identification and management of neuropathic pain

Worldwide, it is expected that 6.3 million patients will sustain a hip fracture in 2050. Hemiarthroplasty is commonly practiced for displaced femoral neck fractures. The choice between unipolar (UH) or bipolar (BH) hemiarthroplasty is still controversial. The objective of this study was to assess the effect on hip function of BH compared to UH for a displaced femoral neck fracture in elderly patients. We conducted a systematic review and meta-analysis of randomized controlled trials comparing BH to UH. Data sources were Medline, Embase, Cochrane Library and Web of Science. All data was pooled in Review Manager (RevMan) version 5.3 software. Selection of the studies included, data abstraction, data synthesis, risk of biais and quality of evidence evaluation was done independently by two authors. Our primary outcome was postoperative hip function. Secondary outcomes were health-related quality of life (HRQoL), acetabular erosion and postoperative complications. 13 randomized controlled trials (n=2256) were eligible for the meta-analysis. There was no difference in hip function scores (standardized mean difference of 0.33 [−0.09–0.75, n=864, I. 2. = 87%,]). Patients with bipolar heads had higher Health-Related Quality of Life scores than patients with unipolar heads (mean difference in EQ-5D scores of 0.12 [0.04–0.19, n=550, I. 2. = 44%]). The use of BH decreased the incidence of acetabular erosion (relative risk of 0.37 [0.17–0.83, n=525, I. 2. = 0%]). There was no relative risk difference for mortality, dislocation, revision and infection. Due to the high heterogeneity between the studies included, it is still unclear whether patients undergoing BH have better hip function than patients undergoing UH. Although, health-related quality of life (HRQoL) may be improved. Future research could be conducted to determine whether a BH offers a better quality of life than UH to geriatric patients undergoing surgery. More precise assessment scores could be developed to better evaluate postoperative outcomes

Proximal humerus fracture dislocations are amongst the most severe proximal humerus injuries, presenting a challenging management problem. The aim of this study was to report on the long-term outcomes of the management of proximal humerus fracture dislocations. Patients with a proximal humerus fracture dislocation managed at a Level 1 trauma centre from January 2010 to December 2018 were included. Patients with an isolated tuberosity fracture dislocation or a pathological fracture were excluded. Outcome measures were the Oxford Shoulder Score (OSS), EQ-5D-5L, return to work, and radiological outcomes. Complications recorded included further surgery, loss of position/fixation, non-union/malunion, and avascular necrosis. A total of 69 patients were included with a proximal humerus fracture dislocation in the study period; 48 underwent surgical management and 21 were managed with closed reduction alone. The mean (SD) age of the cohort was 59.7 (±20.4), and 54% were male. Overall patients reported a mean OSS of 39.8 (±10.3), a mean EQ-5D utility score of 0.73 (±0.20), and 78% were able to return to work at a median of 1.2 months. There was a high prevalence of complications in both patients managed operatively or with closed reduction (25% and 38% respectively). In patients undergoing surgical management, 21% required subsequent surgery. Patient reported outcome measures post proximal humerus fracture dislocations do not return to normal population levels. These injuries are associated with a high prevalence of complications regardless of management. Appropriate patient counselling should be undertaken before embarking on definitive management

In this study we compare survivorship and patient reported outcome measures in robotically assisted versus conventional Total Hip Arthroplasty (THA). This paper investigates the hypothesis that implant survival and PROMS following THAs performed with robotic assistance were not different to outcomes following conventional THAs. Data included all patients undergoing THA for osteoarthritis between 19 April 2016 and 31 December 2020. Analysis of PROMS outcomes was restricted to those who had completed PROMS data preoperatively and at 6 months postoperatively. There were 157,647 procedures, including 3567 robotically assisted procedures, available for comparison of revision rates. 4557 procedures, including 130 robotically assisted procedures, had PROMS data available. The revision rate of primary THA performed with robotic assistance was not statistically different from THA performed by conventional methods (4 year cumulative percent revision 3.1% v 2.7%; HR = 1.05, p=0.67). The Oxford Hip Score, VAS for pain and the EQ-VAS score for overall health showed no statistically significant difference between the groups. The EQ-5D Utility Score showed an improved score (median score 1 v 0.88; OR = 1.58, p=0.007) for the robotically assisted group compared to the conventional group. Robotic assisted THA was not associated with significant improvement in early revision or joint-specific PROMs. The findings may have been biased, in either direction, by unmeasured patient, surgeon, hospital and prosthesis factors. The findings (including the difference in health-related quality of life) may have also been influenced by lack of blinding. Future research should include methods to minimise these biases

The influence of patient age on functional outcomes after periacetabular osteotomy (PAO) for acetabular dysplasia remains unclear, with previous studies utilising scores developed for older, arthritic patients. The purpose of this study is to assess the influence of patient age on International Hip Outcome Tool (iHOT-12) scores, two years after PAO. Eighty-six patients (72 female, 14 male; mean age 26.9 years) who underwent PAO by a single surgeon and had completed a minimum of two years follow up were identified. Patients were categorised into three groups according to age at the time of surgery: adolescent (11–20 years; n=29), young adult (21–30 years; n=29), and mature (≥31 years; n=28). The three groups were compared with respect to baseline variables and functional outcomes (iHOT-12, EQ-5D quality of life score, University of California Los Angeles (UCLA) activity score, pain visual analogue score (VAS)). Multivariate regression was undertaken to assess for independent predictors of two-year iHOT-12 score. The adolescent group demonstrated a lower BMI (p=0.004) while the mature group had a greater American Society of Anaesthesiology (ASA) score (p=0.049). There were otherwise no significant differences in baseline variables between the three groups (all p>0.05). The three groups demonstrated significant post-operative improvements across all functional outcome measures. There were no significant differences in two-year iHOT-12, EQ-5D, UCLA score or pain VAS between the three groups (all p>0.05). Regression modelling found that patient age at the time of surgery did not have an independent effect on two-year iHOT-12 score (p=0.878). We found that patient age at the time of surgery did not influence functional outcomes two years after PAO. Our results suggest that patient age should not be considered in isolation as an indication or contraindication for PAO

There is very limited literature describing the outcomes of management for proximal humerus fractures with more than 100% displacement of the head and shaft fragments as a separate entity. This study aimed to compare operative and non-operative management of the translated proximal humerus fracture. A prospective cohort study was performed including patients managed at a Level 1 trauma centre between January 2010 to December 2018. Patients with 2, 3 and 4-part fractures were included based on the degree of translation of the shaft fragment (≥100%), resulting in no cortical contact between the head and shaft fragments. Outcome measures were the Oxford Shoulder Score (OSS), EQ-5D-5L, and radiological outcomes. Complications recorded included further surgery, loss of position/fixation, and non-union/malunion. Linear and logistic regression models were used to compare management options. There were 108 patients with a proximal humerus fracture with ≥100% translation; 76 underwent operative management and 32 were managed non-operatively with sling immobilisation. The mean (SD) age in the operative group was 54.3 (±20.2) and in the non-operative group was 73.3 (±15.3) (p<0.001). There was no association between OSS and management options (mean 38.5(±9.5) operative vs mean 41.3 (±8.5) non-operative, p=0.48). Operative management was associated with improved health status outcomes; EQ-5D utility score adjusted mean difference 0.16 (95%CI 0.04-0.27, p=0.008); EQ-5D VAS adjusted mean difference 19.2 (95%CI 5.2-33.2, p=0.008). Operative management was further associated with a lower odds of non-union (adjusted OR 0.30, 95%CI 0.09-0.97, p=0.04), malunion (adjusted OR 0.14, 95%CI 0.04-0.51, p=0.003) and complications (adjusted OR 0.07, 95%CI 0.02-0.32, p=0.001). Translated proximal humerus fractures with ≥100% displacement demonstrate improved health status and radiological outcomes following surgical fixation. Patients with this injury should be considered for operative intervention

Introduction. The Constant Score (CS) and the Oxford Shoulder Score (OSS) are shoulder scoring systems routinely used in the UK. Patients with Acromio-Clavicular Joint (ACJ) and Sterno-Clavicular Joint (SCJ) injuries and those with clavicle fractures tend to be younger and more active than those with other shoulder pathologies. While the CS takes into account the recreational outcomes for such patients the weighting is very small. We developed the Nottingham Clavicle Score (NCS) specifically for this group of patients. Methods. We recruited 70 patients into a cohort study in which pre-operative and 6 month post-operative evaluations of outcome were reviewed using the CS, the OSS the Imatani Score (IS) and the EQ-5D scores which were compared with the NCS. Reliability was assessed using Cronbach's alpha. Reproducibility of the NCS was assessed using the test/re-test method. Each of the 10 items of the NCS was evaluated for their sensitivity and contribution to the total score of 100. Validity was examined by correlations between the NCS and the CS, OSS, IS and EQ-5D scores pre-operatively and post-operatively. Results. Significant correlations were demonstrated post-operatively between the NCS and OSS (p< 0.001), CS (p=0.001), IS (p< 0.001) and the ‘self-care’ (p=0.013), ‘pain’ (p< 0.001) and ‘usual activities’ (p< 0.001) sub-categories of EQ-5D. Internal consistency was excellent (Cronbach's alpha=0.87). Removal of an item measuring cosmetic satisfaction improved the alpha to 0.90. Significant agreement was found on test/re-test examination. Differences in NCS were directly related to differences in all 4 comparative outcome measures and 91% of patients with improved post-op NCS values reported improvements in their symptoms. Conclusions. The NCS has been proven to be a valid, reliable and sensitive outcome measure that can accurately measure the level of function and disability in the joint, SC joint and clavicle. We recommend its future use for clinical evaluation

Introduction. The alternative kinematic alignment (KA) technique for total knee arthroplasty (TKA) aims at restoring the native joint line orientation and laxity of the knee. The goal is to generate a more physiological prosthetic knee enabling higher functional performance and satisfaction for the patient. KA TKA have only been reported so far with cruciate retaining and posterior-stabilised designs. Similarly, medial pivot design for TKA has been recently developed to enable more natural knee kinematics and antero-posterior stability. The superiority of KA technique and medial pivot implant design is still controversial when compared to current practice. Our study aims to assess the value of KA TKA when performed with medial pivot implants. Methods. We conducted a retrospectively matched case-control study. Clinical data was prospectively collected on patients as part of an ongoing ODEP study. Thirty-three non-selected consecutive KA TKAs performed by the lead author were matched to a control group of 33 measured resection with mechanically aligned (MA) TKAs performed by other consultant surgeons. Patients were matched for sex, age, BMI and pre-operative Oxford Knee Score (OKS). Pre-operative median OKS was 21 points (max 48), mean age was 69, mean BMI 31, and there were 21 female patients in both arms. The medial pivot GMK Sphere implant (Medacta, Switzerland) was used in all cases. OKS and EQ-5D scores were measured pre-operatively and at 1-year post-op. Patient outcome satisfaction scores were assessed at 1-year follow-up using a visual analogic scale (VAS). Pre- and post-operative knee radiographs were analysed using TraumaCad software. Results. No reoperation or revision was recorded in either group. KA patients were found to have higher OKS (median 44 Vs 42, p=0.78), satisfaction (median 99/100 Vs 90/100, p=0.28), and EQ-5D improvement (mean 0.34 Vs 0.28, p=0.21) compared to MA patients; however, none of the differences discovered were statistically significant. In addition, KA patients had a femoral component that was on average, 3.5° more valgus orientated (aLDFA 84° Vs 80°, p<0.05) and 2.1° more flexed (4.4° Vs 2.3°, p=NS), and a tibial component with 3.6° (aMPTA 86° Vs 89.6°, p<0.05) and 3.9° (5.5° Vs 1.6°, p<0.05) increased varus orientation and posterior slope, respectively. Conclusion/Discussion. KA TKA performed with medial pivot implant design has shown good safety and efficacy at early-term. The physiological implantation provided by the KA technique seems to be clinically beneficial compared to MA implantation, although, the measured differences did not show statistical significance. Having a low study power and high ceiling effect of outcome measure tools may partly explain our results. Early results for KA TKA are encouraging and longer follow-up is warranted to assess longevity of results. For any figures or tables, please contact authors directly

To report the clinical, functional and radiological outcome of consecutive primary hip arthroplasties using large diameter (36mm and above) ceramic bearing couples. We believe this to be one of the first independent series. We prospectively reviewed 519 consecutive primary THA using fully HAC coated acetabular shell and fully HAC coated stem (JRI Ltd) in 502 patients, with minimum follow-up of 32 months. A Biolox-Delta ceramic liner with an 18 deg taper and Biolox-Delta ceramic head (36mm and 40mm) were used in all cases, by 3 surgeons. None were lost to follow-up. Clinical outcome was measured using Harris, Charnley Oxford, EuroQol EQ-5D scores. Radiographs were systematically analysed for implant position, loosening, migration, osteolysis. Return to sports and hobbies were recorded. Mean age was 64.9 yrs (11–82yrs). There were no dislocations. 50–62mm acetabular shells were used. 36 mm head was used in 92% of cases. No acetabular revisions were performed for aseptic loosening. Other re-operations were for infection (1), peri-prosthetic fractures (1). The mean Harris and Oxford scores were 95 (88–97) and 14.1 (12–33) respectively. Harris and Oxford scores were 95 (88–97) and 14.1 (12–33) respectively. The Charnley score was 5.7 (5–6) for pain, 5.8 (4–6) for movement and 5.9 (4–6) for mobility. There was a significant improvement in the range of movement of the hip. There was no migration of acetabular component. Acetabular radiolucencies were present around one shell. No acetabular liner wear was demonstrated in CT Scans. Mean inclination was 7.4deg(37–65). Mean EQ-5D description scores and health thermometer scores were 0.84 (0.71–0.92) and 88 (66–96). With an end point of definite or probable loosening, the probability of survival was 100%. Overall survival with removal or repeat revision of either component for any reason as the end point was 99.1%. The results of this study show an excellent clinical and functional outcome and support the use of a fully coated prosthesis with ceramic bearing couples. We envisage monitoring and prospectively reporting the long-term outcome of this series of patients

Background. Hip resurfacing arthroplasty (HRA) and total hip arthroplasty (THA) are treatments of end-stage hip disease. Gait analysis studies comparing HRA and THA have demonstrated that HRA results in a more normal gait than THA. The reasons may include the larger, more anatomic head diameter or the preservation of the neck of the femur with restoration of the anatomical position of the hip centre and normal proprioception. This study investigated (1) whether femoral head size diameter affects gait; (2) whether gait still differs between THA and HRA patients even with comparable head diameters. Methods. We retrospectively analysed the gait of 33 controls and 50 patients with a unilateral hip replacement, operated by the same surgeon. Follow-up ranged from 9–68 months. In 27 hips a small femoral head size was used (≤ 36mm); in 23 hips a large head size (>36mm). The small size group consisted of 11 long femoral stem THA and 16 short-stem THA and the large group of 5 long-stem THA, 8 short-stem THA and 10 HRA patients. There were 14 females/19 males in the control group; 22 females/5 males in the small size group; 13 females/10 males in the large size group. Results. (1) We found a significant difference in the step-length between the small head size group and the controls (p<0.01) at speeds ranging from 4.0 to 5.5 km/h but no difference between the larger head size and the controls. There was no significant difference in maximum speed, weight acceptance, push-off, mid-stance, impulse and cadence between the groups. (2) Analysis between THA and HRA in the large head size group revealed that there was a significant difference in the maximum speed (p=0.021) between the long-stem THA (6.338 km/h± 1.542) and HRA (7.756km/h± 0.7604) patients. At 5.5 km/h there was a significantly better weight acceptance (p=0.009) and mid-stance (p=0.041) of the HRA compared to short-stems. Impulse was significantly higher for HRA compared to long-stem THA (p<0.05) at all speeds ranging 4 to 5.5 km/h. (3) Males (7.1972 km/h ± .9700) had significantly higher maximum speeds compared to females (6.6524km/h± 1.019) (p=0.017) and lower gait impulse (p<0.01) at speeds ranging from 4 to 5.5km/h. (4) There was no significant difference in the Oxford Hip Score (OHS) and EQ-5D of patients in the small compared to the large head size group. Conclusions. Gait analysis demonstrated a significant difference in step length between THA patients with head size ≤ 36mm and normal controls. There was no difference in step length between normal controls and THA patients with larger head sizes. Compared to larger head size THA, the HRA still revealed higher maximum speeds and better weight acceptance. Males had significantly higher maximum speeds compared to females (controls and hip replacement patients). We could not demonstrate a correlation between better gait and Oxford scores or EQ-5D scores but these scores are known to have a ceiling effect. In a former study, better gait parameters such as longer step length and higher maximum speed have been associated with higher patient satisfaction


In the United Kingdom's National Joint Registry 2018 Annual Report, the combination of a POLARSTEM hip stem and R3 acetabular component has the lowest revision rate of any total hip arthroplasty (THA) construct combination at 7 years. Although revision rates remain a crucial measure of an implant combination's performance, there is increasingly more attention being given to patient-reported outcome measures (PROMs), which often reflect the endpoints that patients’ themselves consider of paramount importance in choosing to undergo THA. Therefore, the current analysis was undertaken to better understand the PROMs-based performance of this combination. Bespoke implant reports were requested for the POLARSTEM/R3 combination with OXINIUM™ heads and highly cross-linked polyethylene (XLPE) bearing. Reports used data from the National Health Service PROMs programme, which collected the Oxford Hip Score (OHS), EQ-5D and EQ-VAS. Health gain scores, calculated as differences between preoperative and 6-month post-operative scores, were adjusted to account for any differences in patient demographics between comparative groups. The mean OHS adjusted health gain score for the construct combination was 22.8 (95% confidence interval [CI]: 22.4 – 23.1; n = 1799 patients) compared to 21.2 (95% CI: 21.2 – 21.3; n = 111,055). For EQ-5D, the scores were 0.462 (95% CI: 0.451 – 0.473; n = 1685) for the construct and 0.434 (95% CI: 0.433 – 0.436; n = 102,448) for the class average. For EQ-VAS, the construct had adjusted scores of 14.2 (95% CI: 13.4 – 14.9; n = 1605) compared to the class average of 11.4 (95% CI: 11.3 – 11.5; n = 98,610). There were also more patients who rated their satisfaction as ‘excellent’ in the specific construct group. Comparisons were statistically significant in all cases (p < 0.001). In conclusion, in addition to excellent mid-term survivorship, the POLARSTEM/R3 construct combination has demonstrated superior PROMs that may improve patient outcomes

Introduction. Surgical techniques for implant alignment in total knee arthroplasty (TKA) is a expanding field as manufacturers introduce patient-specific cutting blocks derived from 3D reconstructions of pre-operative imaging, commonly MRI or CT. The patient-specific OtisMed system uses a detailed MRI scan of the knee for 3D reconstruction to estimate the kinematic axis, dictating the cutting planes in the custom-fit cutting blocks machined for each patient. The resulting planned alignment can vary greatly from a neutral mechanical axis. The purpose of this study was to evaluate the early fixation of components in subjects randomized to receive shape match derived kinematic alignment or conventional alignment using computer navigation. A subset of subjects were evaluated with gait analysis. Methods. Fifty-one patients were randomized to receive a cruciate retaining cemented total knees (Triathlon, Stryker) using computer navigation aiming for neutral mechanical axis (standard of care) or patient-specific cutting blocks (OtisMed custom-fit blocks, Stryker). Pre-operatively, all subjects had MRI scans for cutting block construction to maintain blinding. RSA exams and health outcome questionnaires were performed post-operatively at 6 week, 3, 6, and 12 month follow-ups. A subset (9 subjects) of the patient-specific group underwent gait analysis (Optotrak TM 3020, AMTI force platforms) one-year post-TKA, capturing three dimensional (3D) knee joint angles and kinematics. Principal component analysis (PCA) was applied to the 3D gait angles and moments of the patient-specific group, a case-matched control group, and 60 previously collected asymptomatic subjects. Results. Five MRI scans for surgical planning were not useable due to motion artifacts, with 2 successfully rescanned. Ligament releases were performed in 62% of navigation cases and 32% of patient-specific cases. One patient-specific case was revised for failure of the cruciate ligament, resulting in a polyethylene liner exchange for a thicker, cruciate substituting insert. Implant migration at 1 year was 0.40±0.25 mm for the patient-specific group and 0.37±0.20 mm for the navigation group (maximum total point motions; t-test P=0.65). EQ-5D scores, Oxford Knee scores, satisfaction, pain, and range of motion were not different between groups at any follow-up to 1 year, including the polyethylene liner exchange case. The gait analysis showed that there were no statistical differences between groups. PCA captured a lower early stance phase flexion moment magnitude in the patient-specific group than the computer navigated recipients, bringing patterns further away from asymptomatic characteristics (flexion moment PC2, P=0.02). Conclusions. Implant migration was not different between groups at 1 year despite differences in implant alignment methods. Subject function and satisfaction were also not different between groups, despite significantly fewer ligament releases in the patient-specific group. However, gait analysis of a subgroup has not shown an improvement towards restoring asymptotic gait. It should be acknowledged that the production of patient-specific cutting blocks may not be possible for all patients due to the MRI scanning requirements. Continued evaluation with RSA to 2 years will be performed to monitor these subjects over the longer term

Background. The combination of patient-specific “just-in-time” implant manufacture and robotic technology has not yet been reported. The robot enables accurate placement of anatomically-matched implants. It should be cost-effective, simplify the procedure, and reduce instrumentation. The aims of this study were to determine whether the procedure was safe, radiographically accurate, and comparable in time and cost to conventional arthroplasty. Methods. All patients over 3 months post-op were included. Component position, orientation and size were determined from CT scans by the surgeon prior to manufacture. The implants were inserted using the Sculptor robot, which is supplied free of cost (Savile Row, Stanmore Implants, UK). Following registration, bone was milled away using a high-speed burr under haptic control of the robot. The implants were cemented and a mobile bearing inserted. Patients were followed up clinically and radiographically. Oxford and EQ-5D scores were obtained. Costs of the implant, instruments, and consumables were calculated and compared to published data for conventional UKA and TKA. Results. 17 patients (7M:10F) have reached 3 months follow-up. Mean age was 67 (range 49–84). Mean “robotic surgical time” was 25 minutes (range 14–39) contributing to a mean tourniquet time of 79 minutes (range 57–105). Median length of stay was 3 days (range 1–8). Post-op radiographs showed satisfactory component position. At a median follow-op of 4 months (range 3–6), mean Oxford score was 32 (range 23–41), and mean EQ-5DVAS was 70 (range 40–90). There were no serious complications. Total cost was £1665 per patient compared with £1334 for conventional UKA and £2620 for TKA. Conclusions. Patient-specific UKA can be made and placed safely, with radiographical accuracy, using a “no-cost” haptic robot. The time taken is comparable to other knee arthroplasty procedures, uses less instrumentation, and costs £1000 less than a typical TKA

Background. Acetabular or pelvic ring injuries are invariably associated with high-energy trauma that could lead to a significant degree of disability. The purpose of this study was to investigate whether patients who had surgical treatment of isolated acetabular or pelvic injuries were able to return to their previous sporting activities. Patients and method. Between January 2001 and January 2002 90 patients were treated in our institution with pelvic (PF) and acetabular (AF) fractures. We excluded 22 of them who had sustained other associated injuries in order to eliminate the potential bias that the associated injuries could have on the results. Demographics, fracture classification, rehabilitation, outcome and complications were documented prospectively. Frequency, level of activity and sports participation before and after surgery, as well as EuroQol (EQ-5D) were also recorded. Results. There were 58 male and 10 female patients, with a mean age of 42 years (16-80) and mean follow-up of 30 months (24-36). 43 out of 53 (81%) patients in AF group and 13 out of 15 patients in PF group (83%) returned to a variable level of sports activity. Significant reduction was observed in those who sustained both column (BC) (p<0.04) and posterior wall (PW) (p<0.0009) fractures in the AF group. Significant reduction in frequency of sports practice was also found in the PW subgroup (p<0.0001). Patients <25 and >40 years of age in PF group and <40 or >65 in AF group had significant reduction in EQ-5D scores in comparison to the normal UK population. Conclusion. The majority of patients returned back to sports activities following surgery. The worst prognosis lies with BC and PW acetabular fractures. Middle aged patients do better as compared to younger or elderly patients in both groups

Background. Vertebral body compression fractures (VCFs) impair quality of life (QOL) and increase patient morbidity and mortality. The international, multicentre, randomised, controlled Fracture Reduction Evaluation (FREE) trial was initiated to compare effectiveness and safety of Balloon kyphoplasty (BKP) to non-surgical management (NSM) for the treatment of acute painful VCFs. We describe the primary endpoint of the ongoing 2-year study. Methods. Patients with 1-3 non-traumatic VCFs (< 3 months old) were randomised to either BKP or NSM. The primary endpoint was the change in QOL as measured by the SF-36 Health Survey Physical Component Summary (PCS) at one month, and device/procedure-related safety. Secondary endpoints included SF-36 subscales, the EQ-5D, self-reported back pain and function using the Roland Morris Disability Questionnaire (RMDQ). All patients were given osteoporosis medical therapy. Results. Among the BKP (N=149) and NSM (N=151) cohorts, mean patient age was 73 years and 77% were female. Most patients had VCFs due to primary osteoporosis; 8 patients due to corticosteroid-induced osteoporosis, and 4 had cancer-related fractures. Thirty-nine BKP (26%) and 36 NSM (24%) patients had >1 VCF treated. At one month follow-up, the mean improvement in the PCS was in favour of BKP over NSM (p<0.0001). All physical component SF-36 subscales and the total EQ-5D score were significantly improved for BKP compared to NSM. Mean improvements in back pain at 7 days and 1 month were significantly greater for BKP compared to NSM (p<0.0001 at both time points). The improvement in RMDQ for BKP over NSM was also significant (p<0.0001). There was one soft tissue haematoma and urinary tract infection, with no bone cement-related serious adverse events. Conclusions. Compared to non-surgical management, balloon kyphoplasty demonstrated superior short-term pain, function and quality of life outcomes with no difference in serious adverse events for the treatment of acute, painful vertebral compression fractures. Aims

COVID-19 has compounded a growing waiting list problem, with over 4.5 million patients now waiting for planned elective care in the UK. Views of patients on waiting lists are rarely considered in prioritization. Our primary aim was to understand how to support patients on waiting lists by hearing their experiences, concerns, and expectations. The secondary aim was to capture objective change in disability and coping mechanisms.

Methods

A minimum representative sample of 824 patients was required for quantitative analysis to provide a 3% margin of error. Sampling was stratified by body region (upper/lower limb, spine) and duration on the waiting list. Questionnaires were sent to a random sample of elective orthopaedic waiting list patients with their planned intervention paused due to COVID-19. Analyzed parameters included baseline health, change in physical/mental health status, challenges and coping strategies, preferences/concerns regarding treatment, and objective quality of life (EuroQol five-dimension questionnaire (EQ-5D), Generalized Anxiety Disorder 2-item scale (GAD-2)). Qualitative analysis was performed via the Normalization Process Theory.
