Aims. Day-case arthroplasty is gaining popularity in Europe. We report outcomes from the first 12 months following implementation of a day-case pathway for unicompartmental knee arthroplasty (UKA) and total hip arthroplasty (THA) in an NHS hospital. Methods. A total of 47 total hip arthroplasty (THA) and 24 unicompartmental knee arthroplasty (UKA) patients were selected for the day-case arthroplasty pathway, based on preoperative fitness and agreement to participate. Data were likewise collected for a matched control group (n = 58) who followed the standard pathway three months prior to the implementation of the day-case pathway. We report same-day discharge (SDD) success, reasons for delayed discharge, and patient-reported outcomes. Overall length of stay (LOS) for all lower limb arthroplasty was recorded to determine the wider impact of implementing a day-case pathway. Results. Patients on the day-case pathway achieved SDD in 47% (22/47) of THAs and 67% (16/24) of UKAs. The most common reasons for failed SDD were nausea, hypotension, and pain, which were strongly associated with the use of fentanyl in the spinal anaesthetic. Complications and patient-reported outcomes were not significantly different between groups. Following the introduction of the day-case pathway, the mean LOS reduced significantly by 0.7, 0.6, and 0.5 days respectively in THA, UKA, and total knee arthroplasty cases (p < 0.001). Conclusion. Day-case pathways are feasible in an NHS set-up with only small changes required. We do not recommend fentanyl in the spinal anaesthetic for day-case patients. An important benefit seen in our unit is the so-called ‘day-case effect’, with a significant reduction in mean LOS seen across all lower limb arthroplasty. Aims

Open reduction in developmental dysplasia of the hip (DDH) is regularly performed despite screening programmes, due to failure of treatment or late presentation. A protocol for open reduction of DDH has been refined through collaboration between surgical, anaesthetic, and nursing teams to allow same day discharge. The objective of this study was to determine the safety and feasibility of performing open reduction of DDH as a day case.

Methods

A prospectively collected departmental database was visited. All consecutive surgical cases of DDH between June 2015 and March 2020 were collected. Closed reductions, bilateral cases, cases requiring corrective osteotomy, and children with comorbidities were excluded. Data collected included demographics, safety outcome measures (blood loss, complications, readmission, reduction confirmation), and feasibility for discharge according to the Face Legs Activity Cry Consolidability (FLACC) pain scale. A satisfaction questionnaire was filled by the carers. Descriptive statistics were used for analysis.

Aims. This study aimed to investigate patients’ attitudes towards day-case hip and knee arthroplasty and to describe patient characteristics associated with different attitudes, with the purpose of providing an insight into the information requirements for patients that surgeons should address when informing patients about day-case surgery. Methods. A total of 5,322 patients scheduled for hip or knee arthroplasty between 2016 and 2022 were included in the study. Preoperatively, patients were asked if they were interested in day-case surgery (‘Yes’, ‘Do not know’, ‘No’). Patient demographics including age, BMI, sex, and patient-reported outcome measures (PROMs) such as the EuroQol five-dimension three-level questionnaire (EQ-5D-3L) were examined within each attitude group. Additionally, changes in attitude were assessed among patients who had completed the questionnaire in association with prior hip or knee arthroplasty. Results. Of the surveyed patients, 41.8% were interested in day-case surgery (n = 2,222), 20.8% responded ‘Do not know’ (n = 1,105), and 37.5% were not interested (n = 1,995). Patients who were not interested had a higher mean age (‘No’, 70.2 years (SD 10.0) vs ‘Yes’, 65.2 years (SD 10.7)), with a majority being female (‘No’, 71.9% female (n = 1,434) vs ‘Yes’, 48.6% female (n = 1,081)). Approximately 20% of patients responded ‘Do not know’ regardless of age, sex, and PROMs. Patients reporting anxiety/depression based on EQ-5D-3L more frequently answered ‘No’ (56.9%; 66/116) compared to those not experiencing anxiety/depression (34.9%; 1,356/3,890). Among patients who responded ‘Do not know’ before their first surgery, over 70% changed their attitude to either ‘Yes’ (29.9%; 38/127) or ‘No’ (40.9%; 52/127) at their subsequent surgery. Conclusion. From 2016 to 2022, 58.3% of hip and knee arthroplasty patients expressed uncertainty or no interest in day-case surgery. In connection with current initiatives to increase the number of day-case arthroplasty procedures, there should be a focus on informing patients to address the prevalent negative or uncertain attitude. Further research is needed to investigate what preoperative information patients consider crucial in their decision-making process regarding day-case surgery. Aims. The rate of day-case total knee arthroplasty (TKA) in the UK is currently approximately 0.5%. Reducing length of stay allows orthopaedic providers to improve efficiency, increase operative throughput, and tackle the rising demand for joint arthroplasty surgery and the COVID-19-related backlog. Here, we report safe delivery of day-case TKA in an NHS trust via inpatient wards with no additional resources. Methods. Day-case TKAs, defined as patients discharged on the same calendar day as surgery, were retrospectively reviewed with a minimum follow-up of six months. Analysis of hospital and primary care records was performed to determine readmission and reattendance rates. Telephone interviews were conducted to determine patient satisfaction. Results. Since 2016, 301/7350 TKAs (4.1%) in 290 patients at our institution were discharged on the day of surgery. Mean follow-up was 31.4 months (6.2 to 70.0). In all, 28 patients (9.3%) attended the emergency department or other acute care settings within 90 days of surgery, most often with wound concerns or leg swelling; six patients (2.0%) were readmitted. No patients underwent a subsequent revision procedure, and there were no periprosthetic infections. Two patients (0.7%) underwent secondary patella resurfacing, and one patient underwent arthroscopic arthrolysis after previous manipulation under anaesthetic (MUA). Three patients (1.0%) underwent MUA alone. Primary care consultation records, available for 206 patients, showed 16 patients (7.8%) contacted their general practitioner within two weeks postoperatively; two (1.0%) were referred to secondary care. Overall, 115/121 patients (95%) telephoned stated they would have day-case TKA again. Conclusion. Day-case TKA can be safely delivered in the NHS with no additional resources. We found low incidence of contact with primary and secondary care in the postoperative period, and high patient satisfaction. Cite this article: Bone Jt Open 2023;4(8):621–627

Aims. We present the development of a day-case total hip arthroplasty (THA) pathway in a UK National Health Service institution in conjunction with an extensive evidence-based summary of the interventions used to achieve successful day-case THA to which the protocol is founded upon. Methods. We performed a prospective audit of day-case THA in our institution as we reinitiate our full capacity elective services. In parallel, we performed a review of the literature reporting complication or readmission rates at ≥ 30-day postoperative following day-case THA. Electronic searches were performed using four databases from the date of inception to November 2020. Relevant studies were identified, data extracted, and qualitative synthesis performed. Results. Our evaluation and critique of the evidence-based literature identifies day-case THA to be safe, effective, and economical, benefiting both patients and healthcare systems alike. We further validate this with our institutional elective day surgery arthroplasty pathway (EDSAP) and report a small cohort of successful day-case THA cases as an example in the early stages of this practice in our unit. Conclusion. Careful patient selection and education, adequate perioperative considerations, including multimodal analgesia, surgical technique and blood loss management protocols and appropriate postoperative pathways comprising reliable discharge criteria are essential for successful day-case THA. Abstract. Introduction. Knee osteotomy, both high tibial and distal femoral osteotomy, is a well-recognised treatment for young, active patients with unicompartmental knee osteoarthritis. Osteotomy around the knee is usually performed as an inpatient procedure. The aim of this study was to assess the effectiveness and patient satisfaction of our day-case protocol for knee osteotomy. Methodology. All patients who underwent day-case knee osteotomy at the study unit, over a three-year period, were reviewed to assess the success of ambulatory care for knee osteotomy. Patients were sent questionnaires to assess functional outcome and patient satisfaction with our day-case process. Results. Thirty-three knee osteotomies were performed as a day-case protocol, of which same day discharge was achieved in 24 patients (73%) and discharge within 24 hours achieved in 32 patients (97%). The mean post-operative Knee Osteoarthritis Outcome Score (KOOS) was 67.1 and 79% of patients rated their care as good or excellent. Return to sporting activities was achieved in 75% of patients, and 88% of patients reported they would be happy to undergo day-case knee osteotomy again. Conclusion. Knee osteotomy, both high tibial and distal femoral osteotomy, can be successfully performed as a day-case procedure with similar improvements in functional outcomes and no increased complication rate, compared to in-patient osteotomy

The interest in day-case hip arthroplasty is increasing; however, there are conflicting results regarding readmission risk, and little is known about patients’ attitude towards day-case surgery. We aimed to investigate differences in 30-day readmission rates between day-case patients and single-overnight-stay patients following total hip arthroplasty (THA) and explore patients’ attitude towards day-case surgery. From the Danish National Patient Register we identified 29,486 THAs (1353 day-case THAs and 28,133 single-overnight-stay THAs) performed between 2010 and 2020. Day-case surgery was defined as discharge on the day-of-surgery. Overnight admissions within 30 days of surgery were considered readmissions. Differences in readmission rates between day-case and single-overnight-stay patients were investigated using logistic regression adjusted for patient characteristics and year of surgery. In a single-center descriptive study, 2395 hip arthroplasty patients from 2016 to start-2023 answered a questionnaire on patient characteristics, attitude towards day-case surgery and patient-reported outcome measures (PROM). The overall 30-day readmission rate was 4.4% (CI: 4.2–4.6%) with no difference between day-case (4.4%) and single-overnight-stay THAs (4.4%) (odds-ratio: 1.2 [CI: 0.91–1.6]). In the descriptive study answers to the question whether patients were interested in being discharged on the day of surgery, were: “Yes” = 41%, “Do not know” = 20%, “No” = 39%. Patients responding “No” had lower preoperative PROM-scores, were older (“No” = 70.2 y vs. “Yes” = 65.3 y), and more often female (“No” = 72% vs. “Yes” = 52% female). Based on nationwide data from 2010–2020, day-case THA patients were not more likely to be readmitted compared to single-overnight-stay patients. However, most patients were not positive towards day-case surgery. While surgeons may consider day-case surgery to be safe, patients are not intuitively positive, indicating an attitude mismatch. To achieve high success rate and patient satisfaction in day-case surgery, shared-decision making initiatives within day-case surgery are needed

Abstract. Introduction. Uni-compartmental knee replacement (UKR) has become popularised due to quicker recovery times, reduced postoperative pain, and blood loss. The desire to increase bed capacity and reduce costs, while preserving safety and patient satisfaction, has led to increased interest in day-case arthroplasty. This study observes the feasibility of UKR as a day-case procedure and whether this affects short and long-term postoperative outcomes. Methodology. Between 2018 and 2021, at a single institution and operated by a single orthopaedic surgeon, seventy-seven patients received a UKR on an elective basis. The patients were divided into two groups: ‘day-case’ for those discharged on the same day, and ‘non day-case’ group. Results. 31 patients were identified as day case procedures with the remaining 46 requiring between one to three days before discharge. Mean age, sex, modal ASA score, BMI, Charlson co-morbidity index, and pre-op oxford knee score showed no statistically significant difference between the two groups (p>0.05). No significant difference between the post-op oxford knee score at 1 year was found for patients treated and discharged as a day case procedure (37.8 +/− 6.88) and those who remained as an inpatient postoperatively (37.8 +/− 10.7); t(df) = −0.0007, p=0.994. No patients in either group suffered any complications beyond the peri-operative period. 30-day and 90-day readmission rates were equal. Conclusions. With no significant differences in post-op knee scores, complication, and readmission rates, we feel UKR can be performed as a viable day case procedure in a planned elective setting. This will result in significant cost savings

Introduction. Recent advances in minimally invasive surgery and improved post-operative pain management make it possible to perform major foot/ankle operations as day-case. This could have significant impact on length of stay, saving resources and is in keeping with government policy. However, there are theoretical concerns about complications and low patient satisfaction due to pain. Methods. The survey was developed following review of the literature and was approved for distribution by the BOFAS (British Orthopaedic Foot & Ankle Society) scientific committee. An online survey (19 questions) was sent to UK foot and ankle surgeons via the BOFAS membership list. Major foot/ ankle procedures were defined as surgery that is usually performed as an inpatient in majority of centres and day-case as same day discharge, with day surgery as the intended pathway. Results. A total of 132 surgeons responded, 80% from Acute NHS Trusts. The majority (78%) thought that more procedures could be performed as day-case at their centre. Currently 45% of respondents perform less than 100 day-case surgeries per year for these procedures. Despite post-operative pain and patient satisfaction being theoretical concerns for day-case surgery in this population; these outcomes were only measured by 34% and 10% of respondents respectively. The top perceived barriers to performing more major foot and ankle procedures as day-case were: Lack of physiotherapy input pre/post-operatively (23%), Lack of out of hours support (21%). Conclusions. There is consensus among surgeons to do more major foot/ ankle procedures as day-case. Despite theoretical concerns about post-operative pain and satisfaction this was only measured by a third of those surveyed. Out of hours support and physiotherapy input pre/ post-op were perceived as the main barriers. There is a need to scope the provision of physiotherapy pre/post-operatively and out of hours support at sites where this is a perceived barrier

Total knee arthroplasty (TKA) is a major orthopaedic intervention. The length of a patient's stay has been progressively reduced with the introduction of enhanced recovery protocols: day-case surgery has become the ultimate challenge. This narrative review shows the potential limitations of day-case TKA. These constraints may be social, linked to patient’s comorbidities, or due to surgery-related adverse events (e.g. pain, post-operative nausea and vomiting, etc.). . Using patient stratification, tailored surgical techniques and multimodal opioid-sparing analgesia, day-case TKA might be achievable in a limited group of patients. The younger, male patient without comorbidities and with an excellent social network around him might be a candidate. Demographic changes, effective recovery programmes and less invasive surgical techniques such as unicondylar knee arthroplasty, may increase the size of the group of potential day-case patients. . The cost reduction achieved by day-case TKA needs to be balanced against any increase in morbidity and mortality and the cost of advanced follow-up at a distance with new technology. Objective: The aim of this prospective observational study was to evaluate the feasibility of performing ACL reconstruction with femoral nerve block as a day-case procedure, implement the necessary changes and audit the outcomes following instigation of a day-case protocol. Methods: From November 2005 to April 2006 all patients undergoing ACL reconstruction were entered into a day-case feasibility study. Any issues preventing day-case discharge were identified, and in November 2006 changes were implemented to address these. Since then all ACL reconstruction patients have been treated as day-cases using femoral nerve blocks. Patients are discharged if safe, and contacted 1 and 7 days postoperatively to detect any problems and to assess patient satisfaction. Patients undergoing ACL reconstruction with femoral nerve block in the independent sector, by the same surgeon but without a strict protocol were used as a comparison group. Results: 18 patients have followed the day-case pathway, 15 (83%) being discharged as day-cases. The other cases had afternoon surgery delaying discharge. Only 25% (3/12) patients treated in the independent sector were discharged on the day of surgery. All patients had good working femoral nerve blocks, were able to mobilise safely with crutches and reported no adverse effects from their blocks at follow-up phone calls. All reported good analgesia at home and high satisfaction with being a day-case. Conclusions: With appropriate education of patients and staff, and following a day-case protocol, ACL reconstruction with femoral nerve block is a safe day-case procedure with high patient satisfaction

As Total Hip Replacement (THR) rates increase healthcare providers have sought to reduce costs, while at the same time improving patient safety and satisfaction. Up to 50% of patients may be appropriate for Day Case THR, and in appropriately selected patients’ studies show no increase in complication rate while affording a significant cost saving and maintaining a high rate of patient satisfaction. Despite the potential benefits, levels of adoption of Day Case THR vary. A common cause for this is the perception that doing so would require the adoption of new surgical techniques, implants, or theatre equipment. We report on a Day-Case THR pathway in centres with an established and well-functioning Enhanced Recovery pathway, utilising the posterior approach and standard implants and positioning. We prospectively collected the data on consecutive THRs performed by a single surgeon between June 2018 and July 2021. A standardised anaesthetic regimen using short acting spinal was used. Surgical data included approach, implants, operative time, and estimated blood loss. Outcome data included time of discharge from hospital, post operative complications, readmissions, and unscheduled health service attendance. Data was gathered on 120 consecutive DCTHRs in 114 patients. 93% of patients were successfully discharged on the day of surgery. Four patients required re-admission: one infection treated with DAIR, one dislocation, one wound ooze admitted for a day of monitoring, one gastric ulcer. One patient had a short ED attendance for hypertension. Our incidence of infection, dislocation and wound problems were similar to those seen in inpatient THR. Out data show that the widely used posterior approach using standard positioning and implants can be used effectively in a Day Case THR pathway, with no increase in failure of same-day discharge or re-admission to hospital

High tibial osteotomy (HTO) is largely conducted as an inpatient procedure, imposing the significant cost of hospital admission. Here we examine patient-related outcomes and cost benefit of altering HTO to a day-case procedure. A retrospective questionnaire was conducted with patients that underwent HTO in the one and half years prior to the change to day-case, and those in the one and half years afterwards. We considered pre-operative and post-operative KOOS scores, patient pain-scoring and whether they would, given their experience, undergo HTO as an outpatient again. Thirty-three patients were included in the trial: twenty-three inpatient admissions and ten day-cases. Average KOOS score improvement for inpatients was 24.68 and outpatients was 31.8. Outcomes such as post-operative pain and nausea/vomiting were found to be similar between groups. The outpatient group unanimously agreed that if undergoing HTO again they would desire to be treated in the outpatient setting again. Currently HTO is conducted as an inpatient procedure in the majority of institutions. We have demonstrated that patients have comparable outcomes and agree that if undergoing HTO again they would wish to do so as day-case outpatient. This represents a significant cost saving for institutions conducting HTO as well as an improvement in service

Aim: To assess patients’ progress early after ACL reconstruction and to identity factors favouring outpatient surgery. Method: Eighty-three patients who had received a bone-patellar tendon-bone graft of whom 32 patients (38.5%) were treated as day-cases answered a telephone questionnaire. Information was gained on pain levels (scored from one to seven), medication received, complications and re-admission rates. Results: On the night of discharge 28% of day-case patients suffered pain greater than level 4 but all stated that oral non-steroidal anti-inflammatory drugs and non-opiate analgesics were sufficient to control their pain. Over the first 2 weeks after operation outpatients experienced statistically higher pain levels than inpatients (P = 0.03). Most patients in the study experienced their peak pain levels on the first and second days after the surgery rather than on the night of the surgery. Eighty-one percent of outpatients had their surgery started before 9:30am compared with 29% of inpatients. Drowsiness (n = 18), nausea (n = 11), unsuitable home conditions (n = 9) and pain (n = 7) were the most common reasons for patients choosing in-patient treatment. Six patients (five in-patients and one out-patient) were treated for superficial infections including the one patient who required re-admission (for intravenous anti-biotics). There were no other significant complications. Conclusion: Some patients may be treated safely as out-patients using oral pain relief with no significantly greater re-admission or complication rates than inpatients. An important factor in day-case treatment in this study was that having surgery early in the day allowed more time in hospital for recovery . Drowsiness and nausea after operation, and social factors at home were more important factors than pain

Purpose of study: To compare our practice of paediatric foot and ankle surgery with other hospitals in the UK and to assess the safety and patient satisfaction of day-case procedures on an afternoon list. Methods and Results: A postal questionnaire was sent to 135 consultant members of BSCOS to identify which foot and ankle operations were being performed as inpatients (“Major surgery”) and day-cases (“Minor surgery”). We received 87 (64%) replies over 3 months. The survey revealed that certain procedures were being performed as a day-case in fewer than 35% of centres in the UK. These included excision of tarsal coalitions, tendon transfers, metatarsal osteotomies and open posterior releases for equinus. These same procedures are performed routinely as day-cases at Birmingham Children’s Hospital. We focussed on a single surgeon series with a once weekly afternoon operating list. We identified 24 “major operations” on 19 patients that were performed as a day case over 21 months. The parents of each patient were contacted by telephone to complete a satisfaction survey. We demonstrate that there were no problems that should have warranted an inpatient stay. Conclusions: Most paediatric foot and ankle surgery can be performed satisfactorily as a day case which has obvious cost implications for the NHS. Although this is an accepted practice among adults, the majority of paediatric orthopaedic units are yet to accept this protocol

Introduction

Day Case Surgery (defined as same day discharge) is a priority within the National Health Service and has been shown to provide beneficial outcomes for patients and hospitals. We report our experience developing a Day Case Programme for Total Ankle Replacement (TAR).

Methods

Prior to the introduction of a Day Case Programme, average length of stay following TAR in our unit was 3.5 days. Stakeholders were consulted about ways in which same day discharge could be facilitated. Patients' post-operative pain charts were reviewed prior to the introduction of this programme. Inclusion criteria included non-complex surgery (anticipated tourniquet < 2hrs), friend or relative support and pre-operative walking-aid assessment. An enhanced recovery protocol included long-acting popliteal block and dexamethasone. Patients were discharged with opiate analgesia and written pain instructions. Patients were asked to complete a pain and satisfaction questionnaire. Patient Reported Outcome Measures (PROMs) were recorded.

Abstract

There are numerous advantages of discharging patients early after any surgery. Day case arthroplasty in hip and knee is already brought into practice at many centres. We present our journey towards discharging elective shoulder arthroplasty patient on same after their surgery. An initial retrospective study of patients who underwent elective shoulder replacement between 2017 and 2020 were studied. It was identified that a selected group of patients could be discharged on the same of their surgery. The criteria to select a patient for this service was laid down that include ASA 1 or 2, good family support on discharge, personal wishes of patients and early identification of potential patients in the clinic and planning for day case shoulder arthroplasty56 consecutive patients underwent elective arthroplasty of shoulder. Among them 22 patients were discharges on the next day of surgery. The potential patients those could discharged on same were identified to be 11 out of 22 were under ASA 2 and had good family support at home on discharge. Average length of stay after surgery was 2.17 days. We have prospectively discharged 2 patients following the new criteria. This study demonstrates how outpatient elective shoulder could be implemented at other centres. Patient participation and selection with proper planning is key for success here.

To evaluate a peri-operative protocol developed to facilitate day case open shoulder procedures that historically have required overnight hospital admission. 75 consecutive day-case open shoulder procedures were performed in 75 patients (aged 18 – 65) followed up prospectively for a minimum of 6 months. The procedures included Open Primary Anterior Capsulo-Labral Reconstruction (ACLR) (24), open Revision ACLR (4), open Posterior Capsulo-Labral Reconstruction (2), mini-arthrotomy and rotator cuff repair (6), mini-arthrotomy and sub-acromial decompression (28), modified Weaver Dunn Reconstruction of Acromio-clavicular joint (ACJ) (2), decompression of ACJ (7), open release for frozen shoulder (2). Exclusion criteria included concomitant medical problems, and patients who would have no assistance in their care for the first 24 post operative hours. All patients received general anaesthesia, peri-operative analgesia using intravenous Fentanyl, and Diclofenac (PR), and local Bupivicaine 0.5% to incisions and intra-articular spaces; patients were discharged with oral analgesics. Patient satisfaction with overall experience, pain control, the incidence of nausea that was difficult to manage, the incidence of unplanned admission, attendance or delayed admission to hospital and postoperative complications were measured. 98% of patients were satisfied with their pain management. None of the patients suffered intractable post operative pain, nausea or vomiting; none required unplanned hospital admission or unexpected re-admission. All the patients were satisfied with their overall experience. There were no short or long term post operative complications. In conclusion, the anaesthetic protocol and surgical techniques used in this study permitted same day discharge for a wide variety of open shoulder procedures. For selected patients, open shoulder surgery as a day case appears to be safe effective and acceptable to the patient
