A primary goal of revision Total Knee Arthroplasty (rTKA) is restoration of the Joint Line (JL) and Posterior Condylar Offsets (PCO). The presence of a native contralateral joint allows JL and PCO to be inferred in a way that could account for patient-specific anatomical variations more accurately than current techniques. This study assesses bilateral distal femoral symmetry in the context of defining targets for restoration of JL and PCO in rTKA. 566 pre-operative CTs for bilateral TKAs were segmented and landmarked by two engineers. Landmarks were taken on both femurs at the medial and lateral epicondyles, distal and posterior condyles and hip and femoral centres. These landmarks were used to calculate the distal and posterior offsets on the medial and lateral sides (MDO, MPO, LDO, LPO respectively), the lateral distal femoral angle (LDFA), TEA to PCA angle (TEAtoPCA) and anatomic to mechanical axis angle (AAtoMA). Mean bilateral differences in these measures were calculated and cases were categorised according to the amount of asymmetry. The database analysed included 54.9% (311) females with a mean population age of 68.8 (±7.8) years. The mean bilateral difference for each measure was: LDFA 1.4° (±1.0), TEAtoPCA 1.3° (±0.9), AAtoMA 0.5° (±0.5), MDO 1.4mm (±1.1), MPO 1.0mm (±0.8). The categorisation of asymmetry for each measure was: LDFA had 39.9% of cases with <1° bilateral difference and 92.4% with <3° bilateral difference, TEAtoPCA had 45.8% <1° and 96.6% <3°, AAtoMA had 85.7% <1° and 99.8% <3°, MDO had 46.2% <1mm and 90.3% <3mm, MPO had 57.0% <1mm and 97.9% <3mm. This study presents evidence supporting bilateral distal femoral symmetry. Using the contralateral anatomy to obtain estimates for JL and PCO in rTKA may result in improvements in intraoperative accuracy compared to current techniques and a more patient specific solution to operative planning

Introduction. This study reports on minimum 2 year follow up outcomes on functional and quality of life of patients after undergoing bilateral osseointegration in comparison to traditional socket prosthesis. Materials & Methods. This is a prospective pilot study of 35 patients, consisting of 30 males and 5 females, aged 22–66 (mean 36) years at surgery, with minimum two-year follow-up. Selection criteria were age over 18 years, bilateral amputees who had socket-related problems or were wheelchair-bound with short stumps and non-reconstructable limb pathology. Principle outcome measures included the Questionnaire for persons with a Trans-Femoral Amputation (Q-TFA), Short Form Health Survey 36 (SF-36), Six Minute Walk Test (6MWT), Timed Up and Go (TUG), and K-levels. Adverse events were recorded including infection, revision surgery, fractures, and implant failures. Results. Comparisons were made using differences between the mean pre-operative and mean post-operative values for each outcome measure. Significant improvements in all validated outcome measures were observed. All patient who were wheelchair bound prior to the surgery were able to mobilise post operatively. The occurrence levels of adverse events, including the infection rate and revision rate within this patient cohort, were similar to other established trans-femoral osseointegration studies. Conclusions. These preliminary results indicate that osseointegration surgery is a safe and effective alternative treatment for bilateral amputees experiencing socket-related discomfort. Compared to the suboptimal outcomes of socket prostheses, osseointegration currently provides one of the best chances for any bilateral amputee to walk again and regain the ability to perform daily activities

Simultaneous bilateral total hip arthroplasty (THA) in patients with bilateral hip osteoarthritis is gradually becoming attractive, as it requires a single anesthesia and hospitalization. However, there are concerns about the potential complications following this surgical option. The purpose of this study is to compare the short-term major and minor complications and assess the readmission rate, between patients treated with same-day bilateral THA and those with staged procedures within a year. We retrospectively reviewed the charts of all patients with bilateral hip osteoarthritis that underwent simultaneous or staged (within a year) bilateral total THA in our institution, between 2016-2020. Preoperative patient variables between the two groups were compared using the 2-sample t-test for continuous variables, the Fisher's exact test for binary variables, or the chi-square test for multiple categorical variables. Similarly, differences in the 30-day major and minor complications and readmission rates were assessed. A logistic regression model was also developed to identify potential risk factors. A total of 160 patients (mean age: 64.3 years, SD: ±11.7) that underwent bilateral THA was identified. Seventy-nine patients were treated with simultaneous and eighty-one patients with staged procedures. There were no differences in terms of preoperative laboratory values, gender, age, Body Mass Index (BMI), or American Society of Anesthesiologists Scores (ASA) (p>0.05) between the two groups. Patients in the simultaneous group were more likely to receive general anesthesia (43% vs 9.9%, p0.05). After controlling for potential confounders, the multivariable logistic regression analysis showed similar odds of having a major (odds ratio 0.29, 95% confidence interval [0.30-2.88], p=0.29) or minor (odds ratio 1.714, 95% confidence interval [0.66-4.46], p=0.27) complication after simultaneous compared to staged bilateral THA. No differences in emergency department visits or readmission for reasons related to the procedure were recorded (p>0.05). This study shows that similar complication and readmission rates are expected after simultaneous and staged THAs. Simultaneous bilateral THA is a safe and effective procedure, that should be sought actively and counselled by surgeons, for patients that present with radiologic and clinical bilateral hip disease

It is estimated that approximately 3.1 – 7.7% of the general population suffers from primary osteoarthritis of the hip, with up to 42% of these cases being bilateral. The odds of undergoing a contralateral THA after index unilateral THA range from 16–85%. Up to 20% of these patients have the contralateral THA within 5 years. For this patient population, simultaneous bilateral THA may be an appealing option but it remains controversial. Proponents of bilateral simultaneous THA cite advantages such as a single anesthetic exposure, overall shorter length of hospital stay, quicker recovery, earlier return to function, less time off of work, and potential economic advantages. Only recently has there been more data emerging on patients undergoing simultaneous bilateral THA through the direct anterior approach (DAA). The DAA has the distinct advantage of supine positioning that facilitates easy exposure to both hips without the need to reposition the patient onto a fresh surgical incision while performing the second operation. Recent publications suggest that bilateral simultaneous DAA is a safe procedure and may have economic benefits as well. At our institution between 2010 and 2016, a consecutive series of 105 patients (210 hips) undergoing simultaneous bilateral DAA THA and a matched group of 217 patients undergoing unilateral DAA THA by the same surgeon at a single institution were reviewed. The two groups were matched by gender, age, body mass index and date of surgery. There were no significant differences between the two groups in terms of early complications. There were 2 complications in the unilateral group that were intraoperative nondisplaced calcar fractures that were treated with a single cerclage cable and 50% weight bearing for four weeks. There were 6 in-hospital systemic complications in the unilateral group compared to 7 in the bilateral group (p = 0.129). In-hospital systemic complications were similar between the two groups and included urinary retention, cardiopulmonary abnormalities, alcohol withdrawal, and nausea / vomiting. There were a total of 14 30-day follow-up hip-related complications in the unilateral group compared to 5 in the bilateral group (p = 0.06) These complications were similar between the two groups and included wound healing issues, tendinitis / bursitis, deep infection, nerve palsy, stem subsidence, and instability. Intraoperative estimated blood loss (EBL) was 360cc in the unilateral group compared to 555cc in the bilateral group (p < 0.001). The bilateral group had lower postoperative day one (POD1) hemoglobin (9.5 g/dl vs. 10.2 g/dl; p < 0.001). Four percent of unilateral patients required blood transfusion compared to 11% in the bilateral group. There were significant differences between the two groups in terms of distance ambulated on POD1 and length of stay (LOS). On average, the unilateral patients walked 235 feet on POD1 compared to 182 feet for the bilateral patients (p < 0.001). Length of stay was significantly longer in the bilateral group (1.95 days vs. 1.12 days; p < 0.001). All 322 patients involved in the study were discharged to home except for a single patient in the bilateral group who was discharged to a skilled nursing facility. In conclusion, we found no difference in in-hospital or 30-day complication rates when comparing the simultaneous bilateral group to the unilateral group. The main difference when compared to unilateral surgery is increased blood loss yet this did not directly result in specific complications. Simultaneous bilateral DAA THA can be performed safely and without an unacceptably high perioperative complication rate

Simultaneous bilateral total hip arthroplasties (THAs) present unique and unwarranted dangers to the patient and surgeon alike. These include a significantly increased risk of blood transfusion (up to 50% in contemporary series even with the use of tranexamic acid), longer operative times, longer length of stays, and higher mortality rates in patients with minimal risk factors (age > 75 years, rheumatoid arthritis, higher ASA class, and/or male sex). This is even in light of the fact that the vast majority of literature has a substantial selection bias in which only the healthiest, youngest, non-obese, and most motivated patients are included. Traditionally, simultaneous bilateral THAs were completed in the lateral decubitus position. This required the surgeon and surgical team to reposition the patient onto a fresh wound, as well as additional prepping and draping. To mitigate these additional limitations of simultaneous bilateral THAs, there has been a recent trend towards utilizing the direct anterior approach. However, this particular approach presents its own unique set of complications such as an increased risk of periprosthetic femoral fracture and early femoral failure, an increased risk of impaired wound healing (particularly in obese patients), potential injury to the lateral femoral cutaneous nerve with subsequent neurogenic pain, and traction-related neurologic injuries. When compounded with the risks of simultaneous bilateral THAs, the complication profile becomes prohibitive for an elective procedure with an otherwise very low morbidity

Introduction. Despite several studies, controversies prevailed about the rate of complications following one-stage and two-stage bilateral total hip arthroplasty (THA). In current prospective study, we compared the complications and functional outcomes of one-stage and two-stage procedures. Methods. One hundred and eighty patients (ASA class I or II) with bilateral hip osteoarthritis were assigned randomly to two equal groups. Two groups were matched in term of age and sex. All of the surgeries were performed through the Harding approach using uncemented implants. In two-stage procedures, surgeries were performed with 6 months to one year interval. All patients were evaluated one year postoperatively. Results. The Harris hip score averaged 84.1±12.6 and 82.6±15.3 in one-stage and two-stage groups, respectively (p=0.528). The hospital stay was significantly longer in two-stage group (9.8±1.1 versus 4.9±0.8 days). The cumulative hemoglobin drop and number of transfused blood units were the same. One patient in each group developed symptomatic deep venous thrombosis and managed successfully. There was no patient with perioperative death, pulmonary embolism, infection, dislocation, periprosthetic fracture or heterotrophic ossification. No patient required reoperation. Two patients in one-stage group developed unilateral temporary peroneal nerve palsy resolved after 3 and 4 months. Conclusion. The current study showed that one-stage bilateral THA can be used successfully for patients who require bilateral hip arthroplasty without increased rate of complications. The functional and clinical outcomes are comparable and hospital stay is significantly shorter. However, the authors recommend to perform one-stage bilateral THA for healthy patients with ASA class I or II

Reconstruction of the anterior cruciate ligament (ACL) allows to restore stability of the knee, in order to facilitate the return to activity (RTA). Although it is understood that the tendon autograft undergoes a ligamentous transformation postoperatively, knowledge about longitudinal microstructural differences in tissue integrity between types of tendon autografts (ie, hamstring vs. patella) remains limited. Diffusion tensor imaging (DTI) has emerged as an objective biomarker to characterize the ligamentization process of the tendon autograft following surgical reconstruction. One major limitation to its use is the need for a pre-injury baseline MRI to compare recovery of the graft, and inform RTA. Here, we explore the relationship for DTI biomarkers (fractional anisotropy, FA) between knees bilaterally, in healthy participants, with the hypothesis that agreement within a patient's knees may support the use of the contralateral knee as a reference to monitor recovery of the tendon autograft, and inform RTA. Fifteen participants with no previous history of knee injuries were enrolled in this study (age, 26.7 +/− 4.4 years; M/F, 7/8). All images were acquired on a 3T Prisma Siemens scanner using a secured flexible 18-channel coil wrapped around the knee. Both knees were scanned. A 3D anatomical Double Echo Steady State (DESS) sequence was acquired on which regions of interest (ROI) were placed consistent with the footprints of the ACL (femur, posteromedial corner on medial aspect of lateral condyle; tibia, anteromedial to intercondylar eminence). Diffusion images were acquired using fat saturation based on optimized parameters in-house. All diffusion images were pre-processed using the FMRIB FSL toolbox. The footprint ROIs of the ACL were then used to reconstruct the ligament in each patient with fiber-based probabilistic tractography (FBPT), providing a semi-automated approach for segmentation. Average FA was computed for each subject, in both knees, and then correlated against one another using a Pearson correlation to assess the degree of similarity between the ACLs. A total of 30 datasets were collected for this study (1/knee/participant; N=15). The group averaged FA (+/− standard deviation) for the FBPT segmented ACLs were found to equal 0.1683 +/− 0.0235 (dominant leg) and 0.1666 +/− 0.0225 (non-dominant leg). When comparing both knees within subjects, reliable agreement was found for the FBPT-derived ACL with a linear correlation coefficient (rho) equal to 0.87 (P < 0 .001). We sought to assess the degree of concordance in FA between the knees of healthy participants with hopes to provide a method for using the contralateral “healthy” knee in the comparison of autograft-dependent longitudinal changes in microstructural integrity, following ACL reconstruction. Our results suggest that good agreement in anisotropy can be achieved between the non-dominant and dominant knees using DTI and the FBPT segmentation method. Contralateral anisotropy of the ACL, assuming no previous injuries, may be used as a quantitative reference biomarker for monitoring the recovery of the tendon autograft following surgical reconstruction, and gather further insight as to potential differences between chosen autografts. Clinically, this may also serve as an index to supplement decision-making with respect to RTA, and reduce rates of re-injuries

Bilateral simultaneous total knee replacement surgery remains controversial with arguments for and against its use. Doing sequentially staged TKR's is a safer procedure and may have additional benefits as set out below. If both knees need to be replaced we have often seen that the symptoms of the contralateral knee improve after the one knee is replaced and that patients wait some time before having the opposite knee replaced. Materials:. 333 of 2084 patients having primary total knee replacements needing bilateral replacements were reviewed retrospectively. Results. 245 patients were seen initially with bilateral arthritis of the knee and needed bilateral TKR, while 88 patients developed arthritis in the contralateral knee following TKR. No patients had simultaneous bilateral TKR's; operations were done sequentially and the average time between the TKRs was 20.77 months with a range between 1.5–111 months. Most patients had the contralateral knee replaced within two years of the first knee replacement but 81 patients actually waited between 2 and 10 years before coming in for the second TKR. Conclusion:. It is possible to wait some time before it becomes necessary to replace the contralateral knee in patients who need bilateral TKRs and avoid the increased risk of bilateral simultaneous surgery. The delay not only reduces the cost for the first operation but also gives the second knee a longer time to failure. This is especially important in the typical patients who qualify for bilateral simultaneous TKR's i.e. the younger fitter patients

Background. Pre-operative autologous blood donation is recommended as a means of reducing the need for allogeneic transfusion before simultaneous bilateral total hip arthroplasty (THA). However, there have been few reports on the optimal amount of autologous donation for this procedure. In this study we sought to determine the amount of autologous blood required for patient undergoing simultaneous bilateral THA using the direct anterior approach. Methods. We retrospectively enrolled 325 consecutive patients (650 hips) underwent simultaneous bilateral primary THA from January 2012 to June 2014. Thirty-three patients were men and 290 patients were women. The patients’ mean age at THA was 59.1 years. All THAs were performed using the direct anterior approach. Intraoperative blood salvage was applied for all patients and postoperative blood salvage was not applied for any patients. Results. The mean intraoperative blood loss and the mean operative time for the bilateral procedure were 413±165 g and 87.2±12.3 minutes, respectively. Two hundreds and forty-one of the 325 patients (74.2%) donated an average of 1.9 (range, 1–2) units of autologous blood before the operation. The mean hemoglobin levels on the preoperative day, postoperative day 1 and postoperative day 5 were 12.5g/dl, 10.5 g/dl and 9.5 g/dl, respectively. Only 1 patient (0.3%) required postoperative transfusions of allogeneic blood. All of the autologous units collected were transfused, and no units were wasted. Conclusion. Simultaneous bilateral THA can be performed without allogenic blood transfusion in 99.7% of patients. We could not find out significant effectiveness of an average of 1.9 units of autologous blood donation for this procedure in this study. We concluded that simultaneous bilateral THA can be performed without autologous blood donation in healthy patients without severe hip deformity. Whereas, preoperative donation of autologous blood might be suitable for patients with low body weight or patients with severe hip deformity. The minimally invasive aspect of the direct anterior approach seems to allow a low rate of allogeneic blood transfusion in the study

Background. Fifteen to twenty percent of patients presenting for total hip arthroplasty (THA) have bilateral disease. While simultaneous bilateral THA is of interest to patients and surgeons, debate persists regarding its merits. The majority of previous reports on simultaneous bilateral THA involve patients in the lateral decubitus position, which require repositioning, prepping and draping, and exposure of a fresh wound to pressure and manipulation for the contralateral THA. The purpose of this study was to compare complications, component position, and financial parameters for simultaneous versus staged bilateral THAs using the direct anterior approach (DAA). Methods. Medical records were reviewed for patient demographics, medical history, operative time, estimated blood loss (EBL), change in hemoglobin, transfusion, tranexamic acid (TXA) use, length of stay (LOS), discharge disposition, leg length discrepancy, acetabular cup position, and perioperative complications. Cost and reimbursement data were analyzed. Results. Forty-four patients were included in the sequential group and fifteen patients in the simultaneous group (Figure 1). Operative time was significantly longer for simultaneous DAA THA, with procedures lasting 260 +/− 48 minutes compared to 132 +/− 30 minutes for a single arthroplasty in the sequential group (p<0.001). Patients undergoing simultaneous bilateral DAA THA also had a significantly higher EBL (p<0.001), hemoglobin drop (p<0.001), and blood transfusion rate (p<0.01) compared to parameters for a single arthroplasty in the sequential group. This was despite TXA being used in a significantly higher proportion of simultaneous procedures compared to sequential procedures (p<0.01). The LOS was significantly longer in patients undergoing simultaneous bilateral DAA THA (2.9 +/− 1.0 days) compared to sequential (2.2 +/− 0.6 days) (p<0.001). No deep venous thrombosis (DVT) or pulmonary embolism (PE) was detected in either group during the observation period. No significant difference was detected regarding perioperative complications or whether patients were able to be discharged home instead of to a post-inpatient facility (Figure 2). There was no significant difference in component position, complications, or readmissions between groups. Total cost per hip was significantly less for the simultaneous ($15,565 +/− 1,470) compared to the sequential group ($19,602 +/− 3,094) (p<0.001). There was no significant difference in total payments between the simultaneous group ($25,717 +/− 4,404) and the sequential group ($24,926 +/− 8,203) (p=0.93). Thus, with lower cost and similar reimbursement, profit per hip was significantly higher for the simultaneous ($9,606 +/− 5,060) compared to the sequential group ($5,324 +/− 7,997) (p<0.05). (Figure 3). Conclusions. Significant data regarding simultaneous bilateral THA has been published but results are conflicting and different surgical approaches were used. To our knowledge only four previous reports have been published examining simultaneous bilateral THA performed via the DAA. While simultaneous DAA THA presents challenges, our results suggest that simultaneous DAA THA may add value to the healthcare system without resulting in increased complications compared to sequential hip arthroplasty

Fracture neck of femur (NOF) in elderly is a serious debilitating injury and has been presenting in increasing proportions. Some of these patients are unfortunate to come back with a contralateral injury. We attempted at looking into the incidence of these episodes in a cohort attending our trust and compared various parameters. We retrospectively assessed our hospital theatre data for fracture NOF in patients over 60 years in the last 3 years. We reviewed their demographics, mode of injury, time to contralateral injury, incidence of any other insufficiency fracture, operative procedure and any complications. There were 1435 patients who underwent surgery for fracture NOF over the 3 years. Forty-three of these had bilateral fractures. Females had 3 times more incidence as compared to males; average age at first injury was 84 years and at contralateral side was 85 years. Time between the two injuries ranged from 20 to 855 days (Median 242 days). Almost equal incidence of intra / extracapsular fractures was noted. Contralateral fracture pattern (Intra vs Extracapsular) for the was similar in 34 patients. Twelve patients had an associated insufficiency fracture. Fracture NOF in elderly is a rising epidemic. Only 3% of these patients suffer a contralateral fracture NOF which usually occurs within a year. The fracture pattern is frequently similar to the first fracture in and hence similar implants have been used. Only 21% patients were on bone protection medications. It is rather difficult to identify this small group and hence prevent a second contralateral incident

Introduction. The purpose of this retrospective study was to review the outcome of THA in the treatment of bilateral hip ankylosis of different causes; surgical, septic or spontaneous. Methods & Material. 20 THA procedures in 10 patients were included in the study, 5 males and 5 females all had bilateral fusion. Previous pathologies included: ankylosing spondylitis, AVN, septic arthritis and surgical arthrodesis. Flexion deformity ranged (10°-45°). Shortening as compared to normal anatomy was up to 6 cm and leg length discrepancy (LLD) ranged from 1 cm to 2.5 cm. Most unified X-ray finding was massive osteophytes formation with 3 patients showing severe narrowing of the femoral canal. Operative time averaged 147 minutes (70–210) and lateral approach was used in all patients, anesthesia was general with only 3 undergoing spinal anesthesia. Results. Serious complications were reported and were related to the correction of LLD; 1 incidence of sciatic nerve injury that recovered in one year, and another incidence of femoral nerve injury (sensory > motor) that recovered within 3 months, and one case of incomplete correction of LLD. At 5 years follow up (minimum 6 month), there is no loosening or revision. Discussion. The conversion of bilateral fused hip joints to THA is a very rewarding surgery but with higher risk of complications

Background:. The use of reverse total shoulder arthroplasty (RTSA) has been increasing around the world. However, because of concerns over lack of internal rotation with the reverse prosthesis and the resultant difficulties with activities of daily living (ADLs), many have recommended against performing bilateral RTSA. Methods:. We performed a retrospective review of prospectively obtained clinical data on 15 consecutive patients (30 shoulders) that underwent staged bilateral primary RTSA for the diagnosis of cuff tear arthropathy (CTA) between 2004 and 2012. All operations were performed by a single surgeon. The mean follow-up was 29.6 months from the second RTSA (range 12–65 months). The mean age of the patients at the time of the first operation was 72.9 years (range 63–79 years), and the mean duration between arthroplasties was 21.6 months (range 8–50 months). Patients were evaluated preoperatively and postoperatively at 2 weeks, 6 weeks, 3 months, 6 months, 1 year, and yearly with standardized clinical exams and outcome measures questionnaires including Constant, ASES, UCLA, Simple Shoulder Test, SPADI, and SF-12 scores. Results:. On both operative sides, elevation showed significant improvement from preoperative values (p = 0.002 and p = 0.021); however, external rotation, abduction, and internal rotation did not show significant differences at an average follow-up of 29.6 months. The SPADI, Constant, ASES, UCLA, and SST scores of both RTSA shoulders showed significant improvement from preoperative values (p ≤ 0.001 for all scores); however, the SF-12 scores on either shoulder did not show significant improvement. Evaluation of the outcome measures questionnaire revealed that all 15 patients in the cohort were able to perform perineal hygiene after their reverse arthroplasty. Conclusions:. Bilateral RTSA results in marked improvement in forward elevation, pain, and functional outcomes, and carries a high rate of satisfaction in subjective patient assessment. In addition, common ADLs that require significant internal rotation, such as perineal care, were not problematic in the patients studied


Objectives. Tranexamic acid (TXA), an inhibitor of fibrinolysis blocking the lysine-binding site of plasminogen to fibrin, has been reported to reduce intraoperative and postoperative blood loss in patients undergoing primary total hip arthroplasty (PTHA) both with and without cement. Both intravenous (IV) and topical (TOP) administration of TXA can effectively reduce blood loss in THA without increasing risk of deep venous thrombosis (DVT). However, there have been few reports investigating the combination of intravenous and topical administration of TXA in bilateral cementless PTHA. We investigated the effects of combined intravenous and topical administration of TXA on postoperative blood loss, drainage volume, and perioperative complications in patients with bilateral simultaneous cementless PTHA for hip osteoarthritis. Patients and methods. We retrospectively reviewed the demographic and clinical data of 41 patients who underwent bilateral simultaneous cementless PTHA for hip osteoarthritis from May 2015 to January 2017, of which there were 29 male (70.7%) and 12 female (29.3%) patients. Patients in IV group (n= 11) received only TXA (15 mg/kg) 10 min prior to the incision of each side; and patients in IV + TOP group (n=13) received i.v. TXA (15 mg/kg) combined with topical adiministration (1.0 g) of TXA during the each THA procedure; patients in control group (n=17) received the same dosage of normal saline both i.v. and topically. Outcome measures were total blood loss, hemoglobin, hematocrit value (HCT) changes preoperatively, and on the 1st, 3rd postoperative day, the amount of drainage, and perioperative complications. Results. On the 1st, 3rd postoperative day, patients in group IV and group IV + TOP had significantly higher haematological parameters (haemoglobin, hematocrit value (HCT)) than patients in control group (P < 0.05 (group IV vs control group), P < 0.01 (group IV + TOP vs control group), respectively), while no significant differences found between patients in group IV and group IV + TOP (P > 0.05). The postoperative drainage volume of patients in group IV and group IV + TOP were significantly less than those in control group (P < 0.01, P < 0.01, respectively), while no significant differences found between those in group IV and group IV + TOP (P > 0.05). No significant differences were found in the perioperative complications (DVT or PE) among all three groups. Conclusion. The combined administration of intravenous and topical TXA resulted in a significantly reduction in postoperative blood loss, compared with placebo group. No adverse perioperative complications were observed. This study supports the combined intravenous and topical administration of TXA in bilateral cementless PTHA

Using a computer-based quality assurance program, we analysed peri-operative data on 160 patients undergoing one-stage bilateral hip or knee arthroplasties under regional anaesthesia with routine anaesthetic monitoring and only using peripheral intravenous access for peri-operative safety. We monitored defined intra-operative adverse events such as hypotension, myocardial ischaemia, arrhythmias, hypovolaemia, hypertension and early post-operative complications. We also determined post-operative hip and knee function, and patient satisfaction with different aspects of the anaesthetic management. Those patients undergoing one-stage bilateral arthroplasties were matched according to a cross-stratification which used three variables (American Society of Anesthesiologists’ physical status scoring system, age and joint replaced) to patients undergoing unilateral hip or knee arthroplasties. Serious intra-operative adverse events were, with the exception of intra-operative hypotension, very infrequent in patients undergoing bilateral (nine adverse events) as well as unilateral arthroplasties (five adverse events). Early post-operative complications were also infrequent in both groups. However, the risks of receiving a heterologous blood transfusion (odds ratio 2.5; 95% confidence interval (CI) 1.3 to 5.0, estimated by exact conditional logistic regression) or vasoactive drugs (odds ratio 3.9; 95% CI 2.0 to 7.8) were significantly greater for patients undergoing bilateral operations. Patient satisfaction with anaesthesia was high; all patients who underwent the one-stage bilateral operation would choose the same anaesthetic technique again

Abstract. By next summer the number of patients in the tranexamic acid group will be much higher, probably around 50–60. Purpose. Tranexamic acid has been extensively studied in single total knee and total hip replacement patients. It has been found to reduce blood loss and transfusion rates, with no increase in the rate of venous thromboembolism. This study was undertaken to determine whether tranexamic acid reduces blood loss and the rate of blood transfusion after bilateral total knee replacement, which has a much higher transfusion rate. Method. The preoperative haemoglobin and the lowest postoperative haemoglobin for 30 consecutive bilateral tkr patients who received tranexamic acid was recorded. These were compared with a a consecutive series of 262 historic controls that did not receive tranexamic acid. All patients were operated on by the same surgeon. The surgical procedure was essentially unchanged throughout the study period. The decision to transfuse was made by the hospitalist, who did not know whether the patient received tranexamic acid. Data calculated included the percentage drop in haemoglobin, and the transfusion rate for each group. A subset of patients who were anaemic preoperatively (Hb < 125) were compared for each group. Results. For the control group, which did not receive tranexamic acid, the average preoperative haemoglobin was 138, and dropped to 85 postoperatively. This represented a 38% drop in haemoglobin. Of the 262 patients, 105 required transfusion, giving a transfusion rate of 40%. The average number of units transfused was 1.8. For the study group, which received tranexamic acid, the average preoperative haemoglobin was 133, and dropped to 97 postoperatively. This represented a 27% drop in haemoglobin. Of the 30 patients, only one required transfusion, giving a transfusion rate of 4%. That one patient required only one unit of blood. For the patients who were anaemic preoperatively (Hb < 125) the transfusion rate for the control group was 97%, and for the study group was 20% (1/5). Conclusion. Tranexamic acid markedly reduces blood loss and the rate of transfusion for patients undergoing bilateral total knee replacement

Background. To determine the relative contributions of bilateral versus unilateral femoral shaft fracture plus injuries in other body regions to mortality after injury. Study design. A retrospective analysis of the prospectively recorded Trauma Registry data (TARN) from 1989 to 2003. Methods. Patients were divided into groups UFi (isolated unilateral femur injury),BFi (isolated bilateral femur injury) and UFa and BFa if an associated injury was present. Data collected for each patient included age, Injury Severity Scores, Glasgow Coma Scale, mortality, physiological parameters, the timing and extent of prehospital care, the time to arrival at the hospital, initial treatments, time to and type of surgery, length of ICU and hospital stay. Logistic regression data analysis was performed to determine variables that were associated with increased mortality. Results. Patients in group BFa had a significantly higher ISS (23 vs 9),reduced GCS (12 vs 15) and increased mortality rate (31.6% vs 9.8%) than patients in group BFi. Group BFa patients had an increased number of associated injuries than group UFa. Regression analysis of variables evident on admission revealed a significant correlation between bilateral femoral fractures with associated injuries and mortality. However bilateral fracture, even in isolation significantly increased the odds of mortality by 3.07(1.36-6.92). Intramedullary nailing (IMN) was the method of fracture fixation associated with the lowest patient mortality overall. However, when assessing patient mortality in the BFa group with an ISS of more than 40, three other fracture fixation regimens were associated with a lower mortality rate than the IMN group. Conclusions. The increase in mortality with bilateral femoral fractures is more closely associated with the presence of associated injuries and poor physiological parameters than to the presence of the bilateral femoral fracture alone. However contrary to ISS scoring the latter does convey a significant additional risk of mortality. The presence of bilateral femoral fractures should alert the clinician to the very high likelihood (80%) of significant associated injuries in other body systems and their life-threatening potential

Purpose. Bilateral simultaneous and staged total hip arthroplasty has been issues in terms of safety and costs with development of surgical technique, postoperative medical care. The purpose of this prospective study is to compare the outcomes of simultaneous and staged bilateral operations, in terms of outcome, safety, and socioeconomic-effectiveness. Patients and Methods. All patients(470 cases) that underwent simultaneous or staged bilateral THA using modified minimally invasive two-incision technique between January 2004 and November 2009 were registered, and after exclusion divided into two groups; simultaneous bilateral THA group (Group A: 171 patients) and staged bilateral THA group (Group B: 64 patients) by patient's condition and preference. Staged operations were performed at the time when patients want to get surgery due to undurable pain. For clinical evaluations, amounts of blood loss, blood transfusion, postoperative morbidity and mortality were compared. And for socioeconomic-effectiveness, costs for hospitalization and time for returning to previous job were investigated. Results. Overall complications were not significant between two groups and perioperative morbidity rates were similar in the two groups compared. (p=0.546). Patients in group A required more blood transfusions than those in group B (3.05 vs 1.93 unit, p=0.003), although blood losses in two groups were similar. (906 vs 936 cc, p=0.605) Average interval between staged operation in group B was 18.2 months (range, 2.2 ∼ 65.0 months). Average length of hospital stay was significantly shorter in group A (average 14.6 days) than in group B (average 24.2 days) (p<0.001). Group A showed 20% higher total medical cost than group B (9,240 US vs 11,107 US dollars) (p<0.05). Conclusions. There were no differences in clinical outcome, morbidity and mortality between simultaneous and staged bilateral THA. But simultaneous operation showed a cost-effectiveness with shorter hospitalization and early return to the previous job even though there still remained possibility of more blood transfusion

Bearing diameter and acetabular component orientation have been shown to be important variables effecting blood metal ion levels following hip resurfacing arthroplasty. So far no studies on bilateral hip resurfacings have taken into account these variables. We examined the serum ion results of patients under the care of two experienced hip resurfacing surgeons who carry out ion analysis as part of routine post operative care. Surgeon 1: Patients were implanted exclusively with a “third generation” resurfacing device. Surgeon 2: Patients were implanted with the same “third generation” device and also a low clearance “fourth generation” resurfacing device. Only ion results from patients who were 12 months post surgery were included. Bilateral patients were matched to unilateral patients according to the surgeon performing the operation; the resurfacing system implanted and cup inclination and anteversion angles. The ion data from each bilateral group was tested against the corresponding unilateral groups using the Mann Whitney U test for non parametric data. Significance was drawn at p<0.05. Surgeon 1: There were 310 patients with unilateral joints and 50 with bilateral joints. There were no significant differences with regard to time to follow up, activity levels, joint sizes or cup orientations. Serum chromium (Cr) and cobalt (Co) concentrations were significantly greater in the bilateral group (p<0.001). Median ion levels were greater in the bilateral group by a factor of >2 in the smallest joint sizes and <2 in the largest joints. Surgeon 2: There were 11 patients with bilateral third generation resurfacing joints and 50 with unilateral joints of the same design. The same relationship as described above was identified. There was a notable difference in the fourth generation implant group (n=13 bilateral, n=100 unilateral). Median ion levels for patients receiving bilateral joints of sizes <47mm were ten times greater than in the corresponding unilateral group. Bearing diameter and component design are critical factors in determining metal ion levels following bilateral hip resurfacings. Surgeons must consider the potential implications of gross increases in metal ion levels prior to performing bilateral hip resurfacings in smaller patients

Background. One-stage bilateral total hip arthroplasty (THA) is twice as invasive as unilateral THA. Therefore, increases in bleeding, postoperative anemia, and complications are a concern. The purpose of this study was to investigate hemoglobin values and the use of autologous and allogenic blood transfusion after one-stage bilateral THA. Methods. Twenty-nine patients (7 men and 22 women; 58 hips) were treated with one-stage bilateral THA. The mean age of subjects at the time of surgery was 60.6 years. The average body mass index for patients was 21.7 kg/m. 2. The diagnoses were secondary osteoarthritis due to developmental dysplasia of the hip (n=25) and avascular necrosis (n=4). All patients had donated 800 ml of autologous blood in 2 stages preoperatively (1 to 4 weeks apart). All patients took iron supplements starting from 5 weeks preoperatively. For all patients, the procedure was performed under general anesthesia in the lateral decubitus position via a posterolateral approach. Intra-operative blood salvage was not used. Suction drains were inserted subfascially. As a general rule, pre-donated autologous blood was transfused back to the patients intra- or post-operatively. Allogenic blood transfusion was performed when clinical symptoms of anemia occurred (hypotension, low urinary output, tachycardia, etc.) rather than using a preset blood threshold (hemoglobin level <8 g/dl). To determine changes in blood pressure following surgery until the next morning, systolic and diastolic blood pressure were measured at 3-hr intervals. Results. The mean duration of surgery was 67.4 min for the procedure on the side that was operated on first, 32.7 min to change to the other side, and 68.4 min for the procedure on the other side. The mean blood loss was 576.1 ml. Hemoglobin values at baseline, at the time of autologous blood donation, and on the first day after surgery were 13.2, 12.7, and 8.7 g/dl respectively. Hemoglobin values were significantly different between the 2 weeks before surgery and the first day after surgery. Systolic and diastolic blood pressure were the lowest 3–6 hrs postoperatively (mean, 86/55). Blood reinfusion using autologous blood was performed for all patients. The allogenic blood transfusion rate was 25.0% (range, 2–8 units). In terms of complications, one patient developed an arrhythmia on postoperative day 5. This was the patient for whom autologous blood donation could not be performed due to pre-existing anemia. This patient also had right-sided sciatic nerve palsy. Discussion. With respect to one-stage bilateral THA, Gie showed that allogenic blood transfusion rate with or without autologous blood donation was 42% and 87% respectively. The allogenic blood transfusion rate was 25.0% in our study. Establishing a procedure to perform surgery in a shorter time period may further reduce the rate of allogenic blood transfusion. Although not used herein, intraoperative blood salvage may also be considered. Conclusions. In one-stage bilateral THA, autologous blood donation is effective in managing perioperative anemia and reducing the rate of allogenic blood transfusion. Perioperative blood management based on individual patients' situations are important for the safe performance of one-stage bilateral THA
