Aims. The aim of this study was to compare the incidence of anterior knee pain after antegrade tibial nailing using suprapatellar and infrapatellar surgical approaches. Patients and Methods. A total of 95 patients with a tibial fracture requiring an intramedullary nail were randomized to treatment using a supra- or infrapatellar approach. Anterior knee pain was assessed at four and six months, and one year postoperatively, using the Aberdeen Weightbearing Test – Knee (AWT-K) score and a visual analogue scale (VAS) score for pain. The AWT-K is an objective patient-reported outcome measure that uses weight transmitted through the knee when kneeling as a surrogate for anterior knee pain. Results. A total of 53 patients were randomized to a suprapatellar approach and 42 to an infrapatellar approach. AWT-K results showed a greater mean proportion of weight transmitted through the injured leg compared with the uninjured leg when kneeling in the suprapatellar group compared with the infrapatellar group at all timepoints at all follow-up visits. This reached significance at four months for all timepoints except 30 seconds. It also reached significance at six months at 0 seconds, and for one year at 60 seconds. Conclusion. The suprapatellar surgical approach for antegrade tibial nailing is associated with less anterior knee pain postoperatively compared with the infrapatellar approach. Introduction. Anterior knee pain is a common presentation of patello-femoral dysfunction and patients with this disorder represent a significant proportion attending a specialist knee clinic. There is an on-going debate as to the cause and best treatment for such patients. Previous studies on patella-femoral morphology have suggested patella maltracking plays an important part in the aetiology but there had been no studies correlating maltracking with articular cartilage change. Methods. We studied 147 consecutive patients (294 knees) aged between 10 and 63 presenting with anterior knee pain. All underwent MRI tracking scan of their knees as part of the routine investigations. We analysed the prevalence of maltracking with respect to gender, laterality and age groups, as well as patello-femoral articular cartilage changes. Results. 52% of patients were found to have maltracking, of which 75% were bilateral. Furthermore, 66% of patients with maltracking had radiological evidence of patellar articular cartilage changes, corresponding to 61% of 294 knees examined. While majority of these occur at lateral facet, a proportion of medial facet changes (16%) is also seen. More significantly, while 25% of knees from patients with maltracking under the age of 20 are found to have changes on the patella cartilage, this increases to 93% by the age of 50 or above, with step-wise increment per decade of age (p<0.01). Conclusion. Our results demonstrate a strong correlation between anterior knee pain symptoms, patella maltracking and changes in patello-femoral joint cartilages of varying severity. This has implications on the management of these patients and would suggest early correction of maltracking is indicated to prevent deterioration in the patello-femoral joint

Anterior knee pain (AKP) is the commonest complication of total knee arthroplasty (TKA). This study aims to assess whether sagittal femoral component position is an independent predictor of AKP after cruciate retaining single radius TKA without primary patellofemoral resurfacing. From a prospective cohort of 297 consecutive TKAs, 73 (25%) patients reported AKP and 89 (30%) reported no pain at 10 years. Patients were assessed pre-operatively and at 1, 5 and 10 years using the short form 12 and Oxford Knee Score (OKS). Variables assessed included demographic data, indication, reoperation, patella resurfacing, and radiographic criteria. Patients with AKP (mean age 67.0 (38–82), 48 (66%) female) had mean Visual Analogue Scale (VAS) Pain scores of 34.3 (range 5–100). VAS scores were 0 in patients with no pain (mean age 66.5 (41–82), 60 (67%) female). Femoral component flexion (FCF), anterior femoral offset ratio, and medial proximal tibial angle all differed significantly between patients with AKP and no pain (p<0.001), p=0.007, p=0.009, respectively). All PROMs were worse in the AKP group at 10 years (p<0.05). OKSs were worse from 1 year (p<0.05). Multivariate analysis confirmed FCF and Insall ratio <0.8 as independent predictors of AKP (R. 2. = 0.263). Extension of ≥0.5° predicted AKP with 87% sensitivity. AKP affects 25% of patients following single radius cruciate retaining TKA, resulting in inferior patient-reported outcome measures at 10 years. Sagittal plane positioning and alignment of the femoral component are important determinants of long-term AKP with femoral component extension being a major risk factor

There is a lack of information about the association between patellofemoral osteoarthritis (PFOA) and both adolescent Anterior Knee Pain (AKP) and previous patellar dislocations. This case-control study involved 222 participants from our knee arthroplasty database answering a questionnaire. 111 patients suffering PFOA were 1:1 matched with a unicompartmental tibiofemoral arthritis control group. Multivariate correlation and binary logistic regression analysis was performed, with odds ratios (ORs) and 95% confidence intervals (CIs) calculated. This analysis helps us assess the effect of both variables whilst adjusting for major confounders, such as previous surgery and patient-reported instability. An individual is 7.5 times more likely to develop PFOA if they have suffered adolescent AKP (OR 7.5, 95% CIs 1.51–36.94). Additionally, experiencing a patellar dislocation increases the likelihood of development of PFOA, with an adjusted odds ratio of 3.2 (95% CIs 1.25–8.18). A 44-year difference in median age of first dislocation was also observed between the groups. This should bring into question the traditional belief that adolescent anterior knee pain is a benign pathology. Patellar dislocation is also a significant risk factor. These patients merit investigation, we encourage clinical acknowledgement of the potential consequences when encountering patients suffering from anterior knee pain or patellar dislocation

Open or closed fracture of the tibial shaft is a common injury. There is no long-term outcome data of patients after tibial shaft fracture utilising modern treatment methods. This study assessed pain and function of 1509 consecutive patients with a tibial shaft fracture at 12–22 years following injury. Secondary outcomes included: effect on employment, effect of social deprivation, necessity for hardware removal and comparative morbidity following fasciotomy. Prospective study of 1509 consecutive adult patients with a tibial shaft fracture (1990–1999) at a high-volume trauma unit. 1034 were male, and the mean age at injury was 40 years. Fractures were classified according to AO, and open fractures graded after Gustillo and Anderson. Time to fracture union, complication rate, hardware removal and incidence of anterior knee pain were recorded. Employment and assessment of social deprivation were detailed. Function was assessed at 12 to 22 years post injury using the Short Musculoskeletal Functional Assessment and Short Form 12 questionnaires. 87% of fractures united without further intervention. Social deprivation was associated with higher incidence of fracture and poorer functional and economic outcomes. 11.5% patients underwent fasciotomy which correlated with poorer long-term outcome. Tibial shaft fracture had high mortality in the elderly. At long-term follow-up 25% of patients have anterior knee pain and 20% ankle discomfort after IM nailing. This is the largest and longest study assessing functional and economic outcomes of tibial shaft fracture. This is the first paper to describe ankle pain following tibial IM nailing at long-term follow-up

This ongoing prospective RCT compares two surgical approaches with respect to accuracy of guidewire and tibial nail position. 29 patients with tibial fractures were randomised to semi-extended (SE) or standard (S) approaches of tibial nail insertion. Fluoroscopy obtained guidewire and final nail position. The SE approach is more proximal with the guidewire inserted posterior to the patella, theoretically allowing more accurate wire and nail placement. Measurements were taken in both planes of the nail and guidewire to determine deviation from optimum angle of insertion (relative to the long axes of the tibial shaft). 14 and 15 patients were treated with semi-extended and standard approaches respectively. The semi-extended approach results in better guidewire and nail placement in both planes with mean deviation from the optimal angle of insertion as below:. Guidewire AP 4.5° (SE) versus 4.04° (S) Lateral 24.59° (SE) versus 33.36° (S). Nail AP 3.21° (SE) versus 3.68° (S) Lateral 17.73° (SE) versus 24.04° (S). Anterior knee pain may be due to excessive anterior placement of the nail. We assessed anterior placement of the guidewire and nail in the lateral plane. This was expressed as a percentage from the anterior cortex of the tibia. Mean results are below:. Guidewire 9.7% (SE) versus 9.3% (S). Nail 19.5% (SE) versus 16.3% (S). Semi-extended nailing may allow for better guidewire position and as such nail placement. This may reduce anterior knee pain

Anterior knee pain post Total knee Replacement (TKR) has been reported to be as high as 49%. The source is poorly understood; both the peripatellar soft tissues and the infrapatellar fat pad have been implicated. Immunohistochemical studies demonstrate hyperinnervation of the peripatellar soft tissues. In theory circumferential electrocautery denervates the patella. However there is little evidence that this practice translates into improved clinical outcomes. This study aimed to find the effect on clinical outcome, of intraoperative circumpatellar electrocautery in patients undergoing TKR. 200 patients undergoing primary TKR were randomised to circumferential circumpatellar electrocautery or nothing. Patients were assessed for Visual (VAS) for anterior knee pain and Oxford Knee Score (OKS) preoperatively, 3 months, 6 months and 1 year post-procedure. Patients and assessors were blinded to treatment allocation until the end of the study. There were 91 patients in the electrocautery group and 94 controls. The mean VAS improvement from pre-op to one year was 3.8 in both groups. The mean improvement in OKS was 16.6 points in the control and 17.7 in the electrocautery group (p= 0.40). There were no significant differences between the two groups in terms of VAS or OKS at any other time. Although previous studies have conflicting outcomes, to our knowledge this is the first prospective randomised controlled blinded trial of significant power, to evaluate the effect of peripatellar diathermy in TKR. We conclude that denervation electrocautery of the patella makes no difference to the clinical outcomes of TKR

Background. Computer-assisted navigation systems for total knee arthroplasty (TKA) were introduced to improve implantation accuracy and to optimize ligament balancing. Several comparative studies in the literature confirmed an effect on the component position and other studies could not confirm these results. For ligament balancing most studies found no significant influence on the clinical outcome using a navigation system for TKA. In the literature there were no reports of mid-term results after navigated TKA. With our study we wanted to show if the use of a navigation system for TKA will have an influence on the component's position and on the clinical results at 5-year follow up. Methods. We enrolled 200 patients in a prospective randomized study with a minimum follow up of 5 years. 100 TKA were operated on without using a navigation system (Group A) and 100 surgeries (Group B) were done with computer assistance. Radiological investigation by standard radiographs including a long-leg X-ray was performed with a follow up rate in Group A of 86.2% versus 80.2% in Group B. We measured the mechanical axis of the leg, lateral distal femoral angel (LDFA), medial proximal tibial angle (MPTA), tibial slope and the alpha-angle of the patella. Clinical investigation was performed with a follow up rate in Group A of 85.7% versus 79.8% in Group B including the parameters for the range of motion (ROM), ligament balancing, anterior drawer test, feeling of instability, anterior knee pain, effusion, WOMAC Score, Insall Score and HSS Knee Score. Results. In both groups there was no aseptic loosening or difference in TKA survival rate (Group A 95.4% versus Group B 98.85% 5-year survival rate, p = 0.368). With the navigation technique the mechanical axis of the limb in the frontal plane was improved (p = 0.015; Group B: 1.67 ° ± 1.6° versus 2,44 ° ± 2.2 ° in Group A). 90% of the Group B and 81% of the Group A were within 3 ° varus/valgus deformity of the mechanical axis of the limb (p = 0.157). The accuracy of tibial slope was higher in the Group B (p = 0.001). More patients of the Group B (95% versus 79%) were within a deviation of 3 °, −7 ° tibial slope (p = 0.007). The mean deviation of 90 ° LDFA was higher (p = 0.034) in the Group A (1.89 ° versus 1.36 ° in Group B). Mean deviation of 90 ° MPTA, mean MPTA, mean LDFA and patella alpha angle were similar in both groups (p 0.253). There was no difference in ROM, ligament balancing, anterior drawer test, anterior knee pain or feeling of instability (p 0.058). Insall Knee Score total (181 Group A/191 Group B) and HSS Knee Score total (91 Group A/93 Group B) was higher with the navigated procedure in Group B (p 0.026). WOMAC total and HSS grades were similar in both groups (p 0.070). Conclusions. The accuracy of the mechanical axis of the limb and the tibial slope was higher with the navigated procedure. TKA survival rate and clinical outcomes were similar in both groups at 5-year follow up

The purpose of this study was to validate the usefulness of SPECT/CT as an evaluation method for determining treatment and prognosis for patellofemoral cartilage lesions, based on MRI and arthroscopic findings. From August 2009 to February 2010, SPECT/CT was performed on 80 patients (24 male and 56 female, average 53.3 years old) with chronic anterior knee pain who had no degenerative changes or patellar malalignment on plain radiographs. Radioisotope uptake findings on patellofemoral joint from SPECT/CT were classified into three grades by one nuclear medicine specialist. In the patients who demonstrated poor improvements after conservative treatments, MRI and arthroscopy were performed. Patellofemoral cartilage conditions under arthroscopic finding (ICRS grades) were compared with SPECT/CT grading. In all cases, there were increased uptakes on patellofemoral joints. Eighteen cases showed low uptake (Group 1), 22 cases showed medium uptake (Group 2) and 40 cases high uptake (Group 3). At the follow-up examination after mean 1.7 months, 16 cases of Group 1 (89%), 14 cases of Group 2 (64%) and 28 cases of Groups3 (70%) responded positively to the conservative treatments. For those 22 cases showing poor improvement, MRI and arthroscopy were done. Damage or thinning of patellofemoral cartilage, subchondral cyst, edema and fissuring were observed in every case that underwent MRI. Every arthroscopic case was found that there was patellofemoral cartilage damage. Each of ICRS grade I and grade II was found in 2 cases from Group 1. Two cases of grade II, 4 cases of grade III, and 2 cases of grade IV were found in Group 2. Two cases of grade II, 4 cases of grade III, and 6 cases of grade IV were found in Group 3. There was significant difference in the success rate of conservative treatment between SPECT/CT grade 1 and 2 or 3. And there was significant association between SPECT/CT grade and ICRS grade (P=0.034). SPECT/CT could be a useful modality in determining the treatment and prognosis for patellofemoral cartilage lesions compared with MRI and arthroscopic findings

Introduction. Patellofemoral complications remain a very common post-operative problem in association with total knee arthoplasty (TKA). As malrotation of the femoral component is often considered crucial for the outcome, we analyzed absolute rotational femoral alignment in relation to patellar tracking pre- and postoperatively and matched the results with the two year functional outcome. Methods. Femoral rotation and component rotation was assessed by axial radiography using condylar twist angle (CTA). The lateral patellar displacement, patellar tilt and Insall-Salvati index were measured on conventional radiographs. All assessments were done pre-operatively and at 2-year follow up. The series included 48 consecutive TKA (21 men, 27 women) performed at a single high-volume joint-replacement-center in 2008. All operations were performed using a tibia first-ligament balancing technique without patella resurfacing. The implant used was a condylar unconstrained ultracongruent rotating platform design. Outcome was assessed using the international knee society score (KSS) and the Kujala Score for anterior knee pain. Results. Preoperative CTA showed 6.4±2. 5° (X±SD) of internal femoral rotation (IR) (range, 1° of external rotation (ER) to 12° of IR) compared to postoperative CTA of 3.9°±2.98° (X±SD) of IR (range, 9.5° IR to 3.8°of ER) Preoperative patella lateral displacement showed a mean of 1.1mm (−2mm, 6mm), compared to postoperative patella lateral displacement with a mean of 1.7mm (−3mm, 6mm). Postoperative mean patella tilt was 6.65° (1.8°, 11.7°) postoperatively compared to 8.55° (4.3°,11.5°) preoperatively. No correlation was found between CTA post surgery and patella positioning (r=0.034, 95% CI). IR of the femoral component >3°did not show increased patella lateral displacement/tilt compared to 0° or ER. No correlation was found between the Kujala score and internal rotation of the component (r=0.082, p=0.05). At 2 year post OP KSS reached > 185 of max. 200 points in over 82% of patients. Conclusion. The influence of IR of the femoral component on patellofemoral kinematics remains controversial. As demonstrated, IR does not imperatively lead to patella maltracking and/or patellofemoral symptoms. Functional outcome in this series shows that relative rotation of the femoral component in accordance with natural variations as seen in the pre-operative assessment allows for good and excellent results

Understanding the cause of failure of total knee arthroplasties (TKA) is essential in guiding clinical decision making and adjusting treatment concepts for revision surgery. The purpose of the study was to determine current mechanisms of failure of TKA and to describe changes and trends in revision surgery over the last 10 years. A retrospective review was done on all patients who had revision total knee arthroplasty during a 10-year period (2000–2009) at one institution. The preoperative evaluation in conjunction with the intraoperative findings was used to determine causes of failure. All procedures were categorizes as Sharkey et al. described previously. The data was analyzed regarding the cause of failure and displaying the incidence and trends over the last 10 years. 1225 surgeries were done in the time period with a steady increase of procedures per year (34 procedures in 2000 to 196 in 2009). The most common cause of revision TKA was aseptic failure in 65% and septic failure in 31% of the reviewed cases. However, we could observe a steady proportional increase of the septic classified revisions over the time. Both categories could be subdivided to specific causes of failure including aseptic loosening (24%), anterior knee pain (20%), instability (6,4%), arthrofibrosis (4,9%), PE wear (3,6%), malpositioning/malrotation (2,7%) periprosthetic fracture (2,0%) and other (4,6%), or in early (12,9%), late (15,4%) or low-grade infection (3,3%), respectively. Complementary to the classification Sharkey et al. described in 2002 we identified new subcategories of failure: malrotation (since 2003), Low-Grade-Infection (since 2006), allergic failure/loosening (since 2006), Mid-Flexion-Instability (since 2007), soft tissue impingement (since 2009). The incidence of the classic aseptic loosening due to PE wear shows a clear decrease in the last 10 years whereas we could observe an increase of the new diagnosis of instability, malrotation or low-grade-infection as determined cause of failure. The detailed analysis of the failure mechanism in total knee arthroplasty is important to understand the clinical problem and to adjust treatment strategies. We were able to complement present classifications and give a first overview on the incidence for specific causes of failure. Our data shows changes in the indication for surgery over the time and compared to the collective of Sharkey et al. from 1997–2000. This might be due to new diagnostic methods and better implant materials as well as to a generally increased awareness of the specific mechanism of TKA failure
