After a severe ankle sprain the incidence of residual complaints, particularly on the medial side of the joint, is high. We studied a consecutive series of 30 patients who had operative repair of acute ruptures of lateral ligaments. During operation, arthroscopy revealed a fresh injury to the articular cartilage in 20 ankles, in 19 at the tip and/or anterior distal part of the medial malleolus as well as on the opposite medial facet of the talus. In six patients, a loose piece of articular cartilage was found.

We conclude that in patients with a rupture of one or more of the lateral ankle ligaments after an inversion injury, an impingement occurs between the medial malleolus and the medial facet of the talus. Patients with a lesion of the lateral ankle ligament caused by a high-velocity injury (a faulty landing during jumping or running) had a higher incidence of macroscopic cartilage damage (p < 0.01), medially-located pressure pain (p = 0.06) and medially-located complaints at one-year follow-up (p = 0.02) than those with a low-velocity injury (a stumble).

Introduction. Osteochondral lesions (OCLs) of the talus are a challenging and increasingly recognized problem in chronic ankle pain. Many novel techniques exist to attempt to treat this challenging entity. Difficulties associated with treating OCLs include lesion location, size, chronicity, and problems associated with potential graft harvest sites. Matrix associated stem cell transplantation (MAST) is one such treatment described for larger lesions >15mm2 or failed alternative therapies. This cohort study describes a medium-term review of the outcomes of talar lesions treated with MAST. Methods. A review of all patients treated with MAST by a single surgeon was conducted. Preoperative radiographs, MRIs and FAOS outcome questionnaire scores were conducted. Intraoperative classification was undertaken to correlate with imaging. Postoperative outcomes included FAOS scores, return to sport, revision surgery/failure of treatment and progression to arthritis/fusion surgery. Results. 58 MAST procedures in 57 patients were identified in this cohort. The mean follow up was 5 years. There were 20 females and37males, with a mean age of 37 years (SD 9.1). 22 patients had lateral OCLS were and 35 patients had medial OCLs. Of this cohort 32patients had previous surgery and 25 had this procedure as a primary event. 15 patients had one failed previous surgery, 9 patients had two, four patients had three previous surgeries and three patients had four previous surgeries. 12 patients had corrective or realignment procedures at the time of surgery. In terms of complications 3 patients of this cohort went on to have an ankle fusion and two of these had medial malleolar metal work taken out prior to this, 5 patients had additional procedures for arthrofibrotic debridements, 1 patient had a repeat MAST procedure, 1 additional patients had removal of medial malleolar osteotomy screws for pain at the osteotomy site, there were 2 wound complications one related to the ankle and one related to pain at the iliac crest donor site. Conclusion. MAST has demonstrated positive results in lesions which prove challenging to treat, even in a “ failed microfracture” cohort. RCT still lacking in field of orthobiologics for MAST. Longer term follow up required to evaluate durability

Fractures of the tibial shaft are common injuries, but there are no long-term outcome data in the era of increased surgical management. The aim of this prospective study was to assess the clinical and functional outcome of this injury at 12 to 22 years. Secondary aims were to determine the short- and long-term mortality, and if there were any predictors of clinical or functional outcome or mortality. From a prospective trauma database of 1502 tibial shaft fractures in 1474 consecutive adult patients, we identified a cohort of 1431 tibial diaphyseal fractures in 1403 patients, who fitted our inclusion criteria. There were 1024 men, and mean age at injury was 40.6 years. Fractures were classified according to the AO system, and open fractures graded after Gustilo and Anderson. Requirement of fasciotomy, time to fracture union, complications, incidence of knee and ankle pain at long-term follow-up, changes in employment and the patients’ social deprivation status were recorded. Function was assessed at 12 to 22 years post-injury using the Short Musculoskeletal Function Assessment and short form-12 questionnaires. Long-term functional outcome data was available for 568 of the surviving patients, 389 were deceased and 346 were lost to follow-up. Most fractures (90.7%, n = 1363) united without further intervention. Fasciotomies were performed in 11.5% of patients; this did not correlate with poorer functional outcome in the long term. Social deprivation was associated with a higher incidence of injury but had no impact on long-term function. The one-year mortality in those over 75 years of age was 29 (42%). At long-term follow-up, pain and function scores were good. However, 147 (26%) reported ongoing knee pain, 62 (10%) reported ankle pain and 97 (17%) reported both. Such joint pain correlated with poorer functional outcome. Cite this article: Bone Joint J 2014;96-B:1370–7

The purpose of this study was to compare the clinical and radiological outcome of patients with intact, broken and removed syndesmosis screws after Weber B or C ankle fracture with an associated injury to the syndesmosis. We hypothesised that there would be no difference. Of a possible 142 patients who fulfilled our inclusion criteria, 52 returned for clinical and radiological assessment at least one year after surgery. Of these, 27 had intact syndesmosis screws, ten had broken screws, and 15 had undergone elective removal of the screw. The mean American Orthopaedic Foot and Ankle Society ankle/hindfoot score was 83.07 (. sd. 13.59) in the intact screw group, 92.40 (. sd. 12.69) in the broken screw group, and 85.80 (. sd. 11.33) in the removed screw group (p = 0.0466). There was no difference in clinical outcome of patients with intact or removed syndesmotic screws. Paradoxically, patients with a broken syndesmosis screw had the best clinical outcome. Our data do not support the removal of intact or broken syndesmosis screws, and we caution against attributing post-operative ankle pain to breakage of the syndesmosis screw

Background. Severe infantile Blount's disease can result in a multiplanar deformity of the proximal tibia with both intra-articular and metaphyseal components. Correction can represent a significant surgical challenge. We describe our results using the Taylor spatial frame for acute tibial hemiplateau elevation combined with gradual metaphyseal correction in patients with severe infantile blounts with an associated physeal bony bar. Methods. Eight patients (10 knees) underwent tibial hemiplateau elevation and metaphyseal correction with use of the Taylor Spatial Frame between 2012–2016. We undertook a retrospective case note and radiographic review of all patients to assess clinical and radiographic outcomes. Mean age at the time of surgery of was 11.7 years and mean length of follow up was 16.8 months. Results. At time of latest follow up all patients reported no hip, knee or ankle pain. All knees were clinically stable without lateral thrust during gait. Improvement in radiographic parameters was seen in all patients. The mean tibiofemoral angle improved from −28.3 to 5.9 degrees post operatively. The angle between femoral condyles and the tibial shaft improved from a mean of 56.3 degrees to 90.3 degrees. The joint depression angle was also seen to improve from mean 47.4 degrees to 9.8 degrees. No significant complications were seen. Conclusion. This technique has been shown to be an effective method of correction of the complex deformity encountered in severe infantile Blount's disease. Use of the Taylor spatial frame may provide certain advantages in comparison to previously described approaches. Level of Evidence: Level IV (Case Series)

Open or closed fracture of the tibial shaft is a common injury. There is no long-term outcome data of patients after tibial shaft fracture utilising modern treatment methods. This study assessed pain and function of 1509 consecutive patients with a tibial shaft fracture at 12–22 years following injury. Secondary outcomes included: effect on employment, effect of social deprivation, necessity for hardware removal and comparative morbidity following fasciotomy. Prospective study of 1509 consecutive adult patients with a tibial shaft fracture (1990–1999) at a high-volume trauma unit. 1034 were male, and the mean age at injury was 40 years. Fractures were classified according to AO, and open fractures graded after Gustillo and Anderson. Time to fracture union, complication rate, hardware removal and incidence of anterior knee pain were recorded. Employment and assessment of social deprivation were detailed. Function was assessed at 12 to 22 years post injury using the Short Musculoskeletal Functional Assessment and Short Form 12 questionnaires. 87% of fractures united without further intervention. Social deprivation was associated with higher incidence of fracture and poorer functional and economic outcomes. 11.5% patients underwent fasciotomy which correlated with poorer long-term outcome. Tibial shaft fracture had high mortality in the elderly. At long-term follow-up 25% of patients have anterior knee pain and 20% ankle discomfort after IM nailing. This is the largest and longest study assessing functional and economic outcomes of tibial shaft fracture. This is the first paper to describe ankle pain following tibial IM nailing at long-term follow-up

Introduction. Long-term ankle pain, stiffness or swelling are frequent following ankle fracture. We investigated whether engineered compression stocking (ECS) influenced functional outcome and quality of life (QoL). Methods. Ninety patients < 72 hours following ankle fracture, (59 conservative and 31 operative fixation) were randomised to i) ECS + air-cast boot or ii) air-cast boot alone. Patients were followed at 2, 4, 8, 12 weeks and 6 months for Olerud Molander Ankle Score (OMAS), American Orthopaedic Foot and Ankle Score (AOFAS) and SF12v2 score with duplex imaging for deep vein thrombosis (DVT) at 4 weeks. 22 controls managed by plaster of paris (POP) were also assessed at 6 months only. Results. Of 90 patients recruited, 29 (14 ECS + boot and 15 boot alone) have completed 6 month follow up. The mean (95%CI) foot, ankle and calf circumferences of the injured leg as a percentage of the normal leg were 100%, 100% and 99% at 6 months for ECS + boot compared with 106%, 109% and 102% for boot alone (p<0.001) and 103%, 106% and 102% for POP. By 6 months, the mean OMAS score for ECS + boot was 94, significantly better than 65 for boot alone and 59 for POP (p<0.001). AOFAS scores were also significantly improved at 105 with ECS compared to 87 for boot alone and 78 for POP (p<0.001). Mean SF12v2 scores were 115 for ECS + boot and 99 for boot alone and 91 for POP (p = 0.001). Of the 78 patients completing duplex imaging at 4 week, 5 (13%) of 38 ECS + boot patients had a DVT compared with 10 (25%) of the 40 boot alone patients (p = 0.30). Conclusion. ECS improve functional outcome and quality of life following ankle fractures

Introduction. There is a need for a standardised guideline to assist in optimal decision-making in diabetics who have acquired an ankle fracture. Through a critical analysis of the literature, a diagnostic and management algorithm that incorporates a quantitative scoring system is proposed and presented for consideration. Methods. Publications were identified by conducting a comprehensive keyword search of Medline, EMBASE and CINAHL databases. Search terms included “diabetes,” “ankle,” and “fracture”. Articles published in the English language that were pertinent to the topic were included. Manual search of the references in these relevant papers were also completed to further identify publications for potential inclusion. Publications and conferences not published in the English language or not pertinent to the topic in the above databases were excluded. Duplicate results that occurred in different databases were truncated to a single result. Results. A total of 352 results were revealed using the above methods. Of these, 59 articles met our inclusion criteria. 293 articles met our exclusion criteria. A further 9 articles were included after reviewing the included articles and their respective references. From these results, we propose a detailed diagnostic algorithm that may provide a systematic approach to a diabetic patient who presents with acute ankle pain, swelling, warmth, and/or redness. If a fracture is noted on plain radiographs, a proposed management algorithm and scoring system can be used to assist in deciding whether to proceed with primary open reduction, internal fixation (ORIF) or primary arthrodesis/rigid fixation. This scoring system is based on factors acquired through history and examination. Major factors have been allocated a score of 2, which include: previous or coinciding history of Charcot arthropathy, peripheral neuropathy, insulin dependence with poor compliance, and a diabetic history of greater than 20 years. Minor factors have been allocated a score of 1, which include: age > 50, nephropathy, retinopathy, osteoporosis, vasculopathy, and presence of nutrition/diet deficiency. A score of greater than 7 suggests a primary limb-salvage arthrodesis/rigid fixation because of the likely poor outcome in a primary ORIF. A score of less than 7 suggests that an acceptable outcome will likely result with a primary ORIF. Discussion. Through the use of the AFDA algorithm and score, it will hopefully provide a standardised approach to diabetic ankle fractures and; a method of quantifying risks for both the patient and affected ankle joint, thus, allowing the surgeon to have confidence in achieving the best possible outcome. Although this score is based on a critical analysis of the current literature, further validation of both this algorithm and score is recommended. Once validated, the AFDA score in particular, can also potentially be used as a research tool for further follow-up and outcome studies on diabetic ankle fractures
