Patients with a high comorbidity burden (high-risk) can achieve similar improvements in quality of life compared to low-risk patients, but greater morbidity may deter surgeons from operating on these patients. Whether surgeon volume influences THA outcomes in high-risk patients has not been investigated. This study aimed to compare complication rates and implant survivorship in high-risk patients operated on by high volume (HV) and non-HV THA surgeons. Patients with Charlson Comorbidity Index ≥ 5 and American Society of Anesthesiologist Classification of 3 or 4 undergoing primary, elective THA between 2013 and 2021 were retrospectively reviewed. Patients were separated into groups based on whether they were operated on by a HV surgeon (defined as the top 25% of surgeons at our institution by number of primary THAs per year) or a non-HV surgeon. Groups were propensity matched 1:1 to control for demographic variables. A total of 1,134 patients were included in the matched analysis. Ninety day readmissions and revisions were compared between groups, and Kaplan-Meier analysis was used to evaluate implant survivorship within the follow-up period. Years of experience were comparable between Non-HV and HV surgeons (p=0.733). The HV group had significantly shorter surgical times (p<0.001), and shorter length of stay (p=0.009) compared to the Non-HV group. The HV group also had significantly fewer 90-day readmissions (p=0.030), all-cause revisions (p=0.023) and septic revisions (p=0.020) compared to the non-HV group at latest follow-up. The HV group had significantly greater freedom from all-cause (p=0.023) and septic revision (p=0.020) compared to the non-HV group. High-risk THA patients have fewer 90-day readmissions, all-cause revisions, septic revisions, as well as shorter length of stay when treated by HV surgeons. THA candidates with a high comorbidity burden may benefit from referral to high-volume surgeons to reduce procedural risk and improve postoperative outcomes

Aims. We aimed to compare the implant survival, complications, readmissions, and mortality of Vancouver B2 periprosthetic femoral fractures (PFFs) treated with internal fixation with that of B1 PFFs treated with internal fixation and B2 fractures treated with revision arthroplasty. Methods. We retrospectively reviewed the data of 112 PFFs, of which 47 (42%) B1 and 27 (24%) B2 PFFs were treated with internal fixation, whereas 38 (34%) B2 fractures underwent revision arthroplasty. Decision to perform internal fixation for B2 PFFs was based on specific radiological (polished femoral components, intact bone-cement interface) and clinical criteria (low-demand patient). Median follow-up was 36.4 months (24 to 60). Implant survival and mortality over time were estimated with the Kaplan-Meier method. Adverse events (measured with a modified Dindo-Clavien classification) and 90-day readmissions were additionally compared between groups. Results. In all, nine (8.01%) surgical failures were detected. All failures occurred within the first 24 months following surgery. The 24-month implant survival was 95.4% (95% confidence interval (CI) 89.13 to 100) for B1 fractures treated with internal fixation, 90% (95% CI 76.86 to 100) for B2 PFFs treated with osteosynthesis-only, and 85.8% (95% CI 74.24 to 97.36) for B2 fractures treated with revision THA, without significant differences between groups (p = 0.296). Readmissions and major adverse events including mortality were overall high, but similar between groups (p > 0.05). The two-year patient survival rate was 87.1% (95% CI 77.49 to 95.76), 66.7% (95% CI 48.86 to 84.53), and 84.2% (95% CI 72.63 to 95.76), for the B1 group, B2 osteosynthesis group, and B2 revision group, respectively (p = 0.102). Conclusion. Implant survival in Vancouver B2 PFFs treated with internal fixation was similar to that of B1 fractures treated with the same method and to B2 PFFs treated with revision arthroplasty. Low-demand, elderly patients with B2 fractures around well-cemented polished femoral components with an intact bone-cement interface can be safely treated with internal fixation. Aims. Previous studies have reported an increased risk for postoperative complications in the Medicaid population undergoing total hip arthroplasty (THA). These studies have not controlled for the surgeon’s practice or patient care setting. This study aims to evaluate whether patient point of entry and Medicaid status plays a role in quality outcomes and discharge disposition following THA. Methods. The electronic medical record at our institution was retrospectively reviewed for all primary, unilateral THA between January 2016 and January 2018. THA recipients were categorized as either Medicaid or non-Medicaid patients based on a visit to our institution’s Hospital Ambulatory Care Center (HACC) within the six months prior to surgery. Only patients who had been operated on by surgeons (CML, JV, JDS, RS) with at least ten Medicaid and ten non-Medicaid patients were included in the study. The patients included in this study were 56.33% female, had a mean age of 60.85 years, and had a mean BMI of 29.14. The average length of follow-up was 343.73 days. Results. A total of 426 hips in 403 patients were included in this study, with 114 Medicaid patients and 312 non-Medicaid patients. Medicaid patients had a significantly lower mean age (54.68 years (SD 12.33) vs 63.10 years (SD 12.38); p < 0.001), more likely to be black or ‘other’ race (27.19% vs 13.46% black; 26.32% vs 12.82% other; p < 0.001), and more likely to be a current smoker (19.30% vs 9.29%; p = 0.001). After adjusting for patient risk factors, there was a significant Medicaid effect on length of stay (LOS) (rate ratio 1.129, 95% confidence interval (CI) 1.048 to 1.216; p = 0.001) and facility discharge (odds ratio 2.010, 95% CI 1.398 to 2.890; p < 0.001). There was no Medicaid effect on surgical time (exponentiated β coefficient 1.015, 95% CI 0.995 to 1.036; p = 0.136). There was no difference in 30-day readmission, 90-day readmission, 30-day infections, 90-day infections, and 90-day mortality between the two groups. Conclusion. After controlling for patient variables, there was a statistically significant Medicaid effect on LOS and facility discharge. These results indicate that clinically similar outcomes can be achieved for Medicaid patients; however, further work is needed on maximizing social support and preoperative patient education with a focus on successful home discharge. Hip arthroscopy (HA) is an effective treatment for various hip conditions but has a steep learning curve and its effect on long-term joint preservation is unclear. This study uses population-level data to assess (1) the 90-day complication rate, and (2) the frequency and timing of revision HA, total hip replacement (THR), and pelvic osteotomy (PO) following primary HA. We performed a retrospective analysis of the National Hospital Episode Statistics database, examining all patients who underwent primary HA in NHS hospitals in England from 2010 to 2023 using relevant OPCS-4 codes. We evaluated patient demographics, 90-day complications, and reoperation rates for revision HA, THR, and PO. Descriptive statistical analyses were performed to calculate frequencies and average time to reoperations. We included 22,401 HA procedures in the study. The mean LOS was 0.82±2.04 days. The 90-day readmission rate was 0.17% at a mean of 54.4±8.1 days. The most common reasons for readmission were reoperation (0.071%), followed by infection (0.031%), pulmonary embolism (0.027%), pain (0.022%), bleeding (0.018%), and deep vein thrombosis (0.004%). One patient died within 90 days. Overall, 4942 patients (22.1%) required further surgery at a mean of 2.71±2.27 years. The rates of revision HA, conversion to THR, and PO were 6.94%, 14.6%, and 0.50% at a mean of 2.39±1.79, 2.87±2.46, and 2.26±1.80 years respectively. Female patients had higher rates of reoperation than males for conversion to THR (9.99% vs 4.63%), revision HA (4.92% vs 2.02%), and subsequent PO (0.43% vs. 0.06%) (p<0.001). This study demonstrates a low short-term complication rate after primary HA, supporting existing literature. However, a large proportion of patients required further surgery, especially females. These findings highlight the need for careful patient selection and counselling before HA to optimise outcomes, as well as further research on factors influencing longer-term outcomes and cost-effectiveness

Hip dysplasia is a common cause of hip pain in young adults. Pelvic osteotomy (PO) techniques are the gold standard for treating symptomatic patients, albeit technically demanding. This study aimed to (1) evaluate the 90-day complication rate, and (2) investigate the reasons, frequency and timing of reoperations following primary PO procedures. We retrospectively analysed the National Hospital Episode Statistics database, examining all patients aged over 14 who underwent PO in NHS England hospitals from 2010 to 2023. We identified index procedures and reoperations using relevant OPCS-4 codes. We analysed patient demographics, 90-day complications, and readmission rates for ipsilateral metalwork removal, revision PO, hip arthroscopy (HA), and THR conversion, and calculated the mean time to reoperations. This study included 1,348 PO cases (mean age: 28.7±9.1 years, 89.5% female). The mean hospital stay was 5.4±3.9 days, with a 90-day readmission rate of 0.52% at a mean of 51.0±17.2 days. The most common causes were infection (0.22%) and reoperation (0.15%). The 90-day rate of pulmonary embolism and deep vein thrombosis was 0.074%. One patient died within 90 days. Overall, 810 patients (60.1%) were readmitted for a subsequent hip procedure at a mean of 2.12±1.90 years following their primary PO. Metalwork removal was required in 616 patients (45.7%) at a mean of 1.70±1.19 years. Readmission rates for revision PO, HA, and THR, were 4.23% (mean time: 2.89±0.82 years), 4.15% (mean time: 2.91±2.28 years) and 6.01% (mean time: 5.24±3.08 years) respectively. This study highlights a low 90-day complication rate following primary PO, but a high reoperation rate, mainly for metalwork removal. We provide the most up-to-date report of revision PO, HA and THR conversion rates in England. These findings provide valuable insight that can facilitate informed decision-making, expectation-setting, and post-operative planning, also establishing a benchmark for future quality improvement

Following a total hip arthroplasty (THA), early hospital readmission rates of 3–8% are considered as ‘acceptable’ in terms of medical care cost policies. Surprisingly, the impact of readmissions on mortality has not been priorly portrayed. Therefore, we aimed to analyse the mortality of unplanned readmissions after primary THA at a high-volume Argentinian center. We prospectively analysed 90-day readmissions of 815 unilateral, elective THA patients operated between 2010–2014 whose medical insurance was the one offered by our institution. Mean follow-up was 51 months (range, 37–84). Median age was 69 (IQR, 62–77). We stratified our sample into readmitted and non-readmitted cohorts. Through a Cox proportional hazard model, we compared demographic characteristics, clinical comorbidities, surgical outcomes and laboratory values between both groups in order to determine association with mortality. We found 37 (4.53%) readmissions at a median time of 40.44 days (IQR: 17.46–60.69). Factors associated with readmission were: hospital stay (p=0.00); surgical time (p=0.01); chronic renal insufficiency (p=0.03); ASA class 4 (p=0.00); morbid obesity (p=0.006); diabetes (p=0.04) and a high Charlson Index (p=0.00). Overall mortality rate of the series was 3.31% (27/815). Median time to mortality was 455.5 days (IQR: 297.58–1170.65). One-third (11/37) of the readmitted patients died, being sepsis non-related to the THA the most common cause of death. After adjusting for confounders, 90-day readmissions remained associated with mortality with an adjusted HR of 3.14 (CI95%: 1.05–9.36, p=0.04). Unplanned readmissions were an independent risk factor for future mortality, increasing 3 times the risk of a decease eventuality

Aims. While interdisciplinary protocols and expedited surgical treatment improve the management of hip fractures in the elderly, the impact of such interventions on patients specifically undergoing arthroplasty for a femoral neck fracture is not clear. We sought to evaluate the efficacy of an interdisciplinary protocol for the management of patients with a femoral neck fracture who are treated with an arthroplasty. Methods. In 2017, our institution introduced a standardized interdisciplinary hip fracture protocol. We retrospectively reviewed adult patients who underwent hemiarthroplasty (HA) or total hip arthroplasty (THA) for femoral neck fracture between July 2012 and March 2020, and compared patient characteristics and outcomes between those treated before and after the introduction of the protocol. Results. A total of 157 patients were treated before the introduction of the protocol (35 (22.3%) with a THA), and 114 patients were treated after its introduction (37 (32.5%) with a THA). The demographic details and medical comorbidities were similar in the two groups. Patients treated after the introduction of the protocol had a significantly reduced median time between admission and surgery (22.8 hours (interquartile range (IQR) 18.8 to 27.7) compared with 24.8 hours (IQR 18.4 to 43.3) (p = 0.042), and a trend towards a reduced mean time to surgery (24.1 hours (SD 10.7) compared with 46.5 hours (SD 165.0); p = 0.150), indicating reduction in outliers. Patients treated after the introduction of the protocol had a significantly decreased rate of major complications (4.4% vs 17.2%; p = 0.005), decreased median hospital length of stay in hospital (4.0 days vs 4.8 days; p = 0.008), increased rate of discharge home (26.3% vs 14.7%; p = 0.030), and decreased one-year mortality (14.7% vs 26.3%; p = 0.049). The 90-day readmission rate (18.2% vs 21.7%; p = 0.528) and 30-day mortality (3.7% vs 5.1%; p = 0.767) did not significantly differ. Patients who underwent HA were significantly older than those who underwent THA (82.1 years (SD 10.4) vs 71.1 years (SD 9.5); p < 0.001), more medically complex (mean Charlson Comorbidity Index 6.4 (SD 2.6) vs 4.1 (SD 2.2); p < 0.001), and more likely to develop delirium (8.5% vs 0%; p = 0.024). Conclusion. The introduction of an interdisciplinary protocol for the management of elderly patients with a femoral neck fracture was associated with reduced time to surgery, length of stay, complications, and one-year mortality. Introduction. The association between preoperative opioid use and adverse outcomes after total hip arthroplasty (THA) has been reported. However, a quantitative assessment used to evaluate the combined effect of controlled substance use and define a threshold for adverse outcomes after THA has not been established. The current study aimed to identify the association between the NarxCare Score (NCS) (indicative of controlled substance use) and length of stay (LOS), discharge disposition, 90-day emergency department (ED) visits, readmission, and reoperation after primary THA, necessary to identify a preoperative NCS threshold for adverse outcomes. Methods. A total of 2,787 THAs (57.3% female; mean age: 64.3±11.14 years; NCS range: 0–800) were performed between November 2018-January 2020 at an integrated healthcare system with preoperative NCSs. Outcomes assessed included LOS, discharge disposition, 90-day ED visits, readmission (all-cause, procedure and non-procedure-related) and reoperation. The association between NCS category (in 100-point increments) and was analyzed through multivariate logistic regression accounting for risk factors. Results. NCS of zero, 1–99 and 100–199 were similar regarding all investigated outcomes (p>0.05 for all). Compared to a NCS of zero, NCS of 300–399 was independently associated with higher odds of a LOS >2 days (odds ratio [OR]:2.27; 95% confidence interval [CI][1.59–3.24]; p<0.001), non-home discharge (OR: 2.0; 95% CI [1.34–2.99]; p=0.001), 90-day all-cause readmission (OR: 2.0; 95% CI [1.16–3.36]; p=0.012). Furthermore, NCS of 300–399 exhibited higher odds of a procedure-related readmission (OR: 3.74; 95% CI [1.48–8.13]; p=0.004). There were no differences between the NCS strata in 90-day reoperation and non-procedure related readmission rates (p>0.05 for each). Conclusion. Higher NCS was associated with greater LOS and 90-day readmission, as well as lower odds of discharge to home. A NCS threshold of 300 can potentially designate high-risk patients for adverse outcomes after THA. This score may help stratify patients in the current alternative payment environment

Introduction. Patients with FNF may be treated by either total hip arthroplasty (THA) or hemiarthroplasty (HA). Utilizing American Joint Replacement Registry (AJRR) data, we aimed to evaluate outcomes in FNF treatment. Methods. Medicare patients with FNF treated with HA or THA reported to the AJRR database from 2012–2019 and CMS claims data from 2012–2017 were analyzed in this retrospective cohort study. “Early” was defined as less than 90 days from index procedure. A logistic regression model, including index arthroplasty, age, sex, stem fixation method, hospital size. 1. , hospital teaching affiliation. 1. , and Charlson comorbidity index (CCI), was utilized to determine associations between index procedure and revision rates. Results. Of 75,333 FNF procedures analyzed, 82.2% had HA. 8.4% had cemented fixation. 36.9% had cementless fixation. Fixation was unknown for 41,225 (54.7%) patients. 90-day readmissions rates were 1.3% for both cohorts. Both the early revision rate (0.9% HA vs. 1.3% THA, p<0.0001) as well as the overall revision rate (1.5% HA vs. 2.3% THA, p<0.0001) were higher in the THA cohort. The three most common reasons for any revision were instability (26%), infection (21%), and periprosthetic fracture (15%). Higher rates of any revision were associated with cementless fixation (OR=1.37, 95% CI 1.20–1.57) and increased age (OR=0.96, 95% CI 0.96–0.97). THA and increased age were risk factors for early and any revision for instability. Cementless fixation, female sex, and decreased age were associated with lower revision rates for infection. Conclusion. The most common surgical treatment for FNF reported to the AJRR was cementless stem fixation and hemiarthroplasty. The higher revision rates for FNF treatment with THA warrant further investigation. The use of cemented femoral fixation in this patient population may be under-utilized. These data provide a benchmark of US practice for reference and comparison to other practices throughout US and the world

Introduction. While interdisciplinary protocols and expedited surgical treatment improve management of geriatric hip fractures, the impact of such interventions on patients undergoing specifically arthroplasty for femoral neck fracture (FNF) has not been well studied. The aim of this study is to evaluate the efficacy of an interdisciplinary hip fracture protocol for patients undergoing arthroplasty for acute FNF. Methods. In 2017, our tertiary care institution implemented a standardized interdisciplinary hip fracture protocol. We conducted a retrospective review of adult patients who underwent hemiarthroplasty (HA) or total hip arthroplasty (THA) for FNF from July 2012 – March 2020, and compared patient characteristics, hospitalization characteristics, and outcomes between those treated before and after protocol implementation. Results. 157 patients were treated before implementation (22.3% THA), and 114 patients were treated after implementation (32.5% THA). Demographics and medical comorbidities were similar before and after protocol implementation. Patients who underwent HA were older (82.1 vs 71.1 years, p<0.001), more medically complex (Charlson Comorbidity Index 6.4 vs 4.1, p<0.001), and more likely to develop delirium (8.5% vs 0%, p=0.024) than those who underwent THA. Patients treated after implementation had reduced time between admission and operative management (24.1 vs 46.5 hours, p=0.042), decreased rate of major complications (0% vs 12.7%, p<0.001), decreased hospital length of stay (LOS) (5.2 vs 6.4 days, p=0.008), increased rate of discharge to home (26.3% vs 14.7%, p=0.03), and decreased 365-day mortality (14.6% vs 26.1%, p=0.049). There were no significant differences between post-protocol and pre-protocol, respectively, 90-day readmission (18.2% vs 21.7%, p=0.53) or 30-day mortality (3.7% vs 5.1%, p=1.0). Conclusion. This interdisciplinary protocol reduced time to operative management, hospital LOS, in-hospital complications, and one-year mortality for patients who underwent arthroplasty for FNF, without increasing readmission or non-home discharge. Such interventions are critical in improving outcomes and reducing costs for an aging population

Background. Routine closed suction drainage and postoperative laboratory studies have long been tenets of most TJA protocols. However, recent literature has called into question whether either is necessary with modern outpatient TJA clinical pathways. Methods. Demographic, cost, and readmission data for 2,605 primary unilateral TJA cases was collected retrospectively and analyzed prior to and after a protocol change where routine closed suction drains and postoperative laboratory tests were eliminated. This protocol change was designed to treat all primary TJA like outpatients regardless of their admission status. Drain usage changed from routine to selective based on hemostasis. Lab studies changed from routine to selective for patients on warfarin for VTE prophylaxis (INR), with ASA 4 or 5 status (BMP), and with a hematocrit < 27% in the recovery room after THA (CBC). Results. Drain utilization decreased by 50% and postoperative labs decreased by 70%. Drain and lab test cost per patient decreased from $74.62 before to $19.91 after the change, while charges per patient decreased from $625.23 before to $77.15 after. The 30 and 90-day readmission rates decreased with no readmissions occurring before or after the change that could have been prevented by using a drain or obtaining labs. Conclusion. Selective elimination of routine closed suction drainage and postoperative laboratory studies as part of an outpatient TJA protocol is safe and provides cost savings for inpatient primary TJA. Per 100 cases, this change could decrease institution costs by $5,500 and patient charges by $55,000 without decreasing quality or safety

Introduction. Depression is a common co-morbidity in Total Hip Arthroplasty (THA). Chronic pain and depression influence one another. Postoperative outcomes and satisfaction rates are affected by depression. Objectives. The aim of the study was to determine the impact of pre-operative depression on patient functional outcomes and satisfaction rates after THA. A secondary objective was to assess potential change in the incidence of depression at mid-term post-operative follow-up. Study Design & Methods. We retrospectively reviewed 200 patients undergoing THA from 2015–2016 at a single academic institution.150 (75%) of patients were followed up at a minimum of 24 months. Patients pre-operative depression symptoms were assessed according to the Zung Self-reported depression score and the PHQ-9 scores. Functional scores, satisfaction rates, expectation rates and 30- and 90-day readmission rates were correlated with pre-operative and 2-year follow-up depression scores. Results. There were 103 females and 47 males with an average age of 60.1 years (22–89 years old). The Patient Health Questionnaire (PHQ-9) correlated with the Zung Self-reported depression scores. Overall, 73 patients (48.7%) were classified as being depressed pre-operatively. There were 48 (32.2%) with mild depression and 19 (12.8%) with marked depression. Females, HIV-infected and morbidly obese patients were more likely to be depressed. The mean pre-operative Harris Hip Score (HHS) was 35.95 (24–66) and Oxford Hip Score (OHS) 17.11 (0–48). The 30-day readmission rate was 2% and all patients were pre-operatively severely depressed. At a minimum of 2 years there was an improvement in Zung Self-Rating Depression Scale and PHQ-9 scores. Only 29 (18.8%) patients were depressed. There were 55 patients who progressed from depressed to non-depressed (p=0.00), while 11 patients (7.4%) became depressed after THA (p=0.00). The least improvement in both OHS and HHS was in the severely depressed group (p<0.05). The overall satisfaction rate was 89.7%. Depressed patient satisfaction rate was 86.9% with only 70.27% of the severely depressed group satisfied post-operatively. Age (>70) and BMI >40 correlated with dissatisfaction (p<0.05). Conclusions. There is a high prevalence of depression in THA. Incorporation of psychological management strategies may improve satisfaction rates and functional outcomes post-THA

Introduction. The obesity epidemic is a growing problem and must be considered with the projected increased demand for total hip arthroplasty (THA). Previous studies have reported increased complication rates after THA in the obese population, which has led to hesitation in offering surgery to this population. Moreover, some insurers are denying coverage for morbidly obese patients. While many consider obesity a “modifiable” risk factor, very few patients with advanced osteoarthritis have successfully lost substantial weight. The experience of centers that utilize systematic preoperative risk stratification tools and standardized postoperative total joint pathways may be underrepresented in prior studies. The aim of this study is to describe one surgeon's experience performing THA in morbidly and super-obese patient populations using an institutional preoperative Risk Stratification Tool (RST) and total joints pathway. Methods. We conducted a retrospective review of patients undergoing primary THA between May 2014 and December 2017 performed by a single surgeon at a tertiary care referral center. All patients were assessed preoperatively using an institutional RST and had a minimum of 90-day postoperative follow up. Patients were stratified by body mass index (BMI, kg/m. 2. ): non-obese (BMI < 30), obese (30–34), severely obese (35–39), morbidly obese (40–44), and super-obese (≥ 45). Primary outcomes were inpatient and 90-day complications. Continuous and binary parameters were analyzed by Kruskal-Wallis and Fisher exact tests. Logistic regression was additionally utilized to evaluate outcomes by BMI cohort. Results. A consecutive series of 368 patients met inclusion criteria across all BMI cohorts. There was significant variation with respect to age (P=0.001), BMI (P<0.001), diabetes (P=0.008), ASA class (P<0.001), and anesthesia type (P=0.003) (Table 1). Variation among BMI cohorts was also identified for several operative and postoperative parameters, including longer operative and in-room time and greater length of stay (P<0.001) (Table 2). Compared to non-obese patients, super-obese patients had 20.1 greater odds of return to OR within 90 days for superficial surgical site infection (SSI) or prolonged round drainage (P=0.008) (Table 3). Notably, morbidly and super-obese patients were not at significantly increased risk for inpatient intensive care unit (ICU) transfer, blood transfusion, 90-day emergency room visit, or 90-day readmission compared to their non-obese counterparts. For patients in whom 1-year follow-up was available, these differences between BMI cohorts remained insignificant. Conclusions. Patients with BMI>40 are more likely than non-obese patients to have increased postoperative rehabilitation needs but are not at increased risk for in-hospital complications. Super-obese patients have greater risk of superficial SSI or prolonged wound drainage than non-obese patients but are not at increased risk for revision or deep infection in any cohort. Use of a preoperative RST may help to mitigate postoperative complications and readmissions previously associated with morbid and super-obesity. We conclude that THA can be safely performed in super-obese patients and therefore care should not be denied to this population. Summary sentence. Total hip arthroplasty (THA) can be safely performed in morbidly and super-obese patients with the use of a preoperative risk stratification tool (RST) and total joints pathway. Aims

United Classification System (UCS) B2 and B3 periprosthetic fractures in total hip arthroplasties (THAs) have been commonly managed with modular tapered stems. No study has evaluated the use of monoblock fluted tapered titanium stems for this indication. This study aimed to evaluate the effects of a monoblock stems on implant survivorship, postoperative outcomes, radiological outcomes, and osseointegration following treatment of THA UCS B2 and B3 periprosthetic fractures.

Methods

A retrospective review was conducted of all patients who underwent revision THA (rTHA) for periprosthetic UCS B2 and B3 periprosthetic fracture who received a single design monoblock fluted tapered titanium stem at two large, tertiary care, academic hospitals. A total of 72 patients met inclusion and exclusion criteria (68 UCS B2, and four UCS B3 fractures). Primary outcomes of interest were radiological stem subsidence (> 5 mm), radiological osseointegration, and fracture union. Sub-analysis was also done for 46 patients with minimum one-year follow-up.

Aims

Obesity is associated with an increased risk of hip osteoarthritis, resulting in an increased number of total hip arthroplasties (THAs) performed annually. This study examines the peri- and postoperative outcomes of morbidly obese (MO) patients (BMI ≥ 40 kg/m2) compared to healthy weight (HW) patients (BMI 18.5 to < 25 kg/m2) who underwent a THA using the anterior-based muscle-sparing (ABMS) approach.

Methods

This retrospective cohort study observes peri- and postoperative outcomes of MO and HW patients who underwent a primary, unilateral THA with the ABMS approach. Data from surgeries performed by three surgeons at a single institution was collected from January 2013 to August 2020 and analyzed using Microsoft Excel and Stata 17.0.

Aims

As our population ages, the number of octogenarians who will require a total hip arthroplasty (THA) rises. In a value-based system where operative outcomes are linked to hospital payments, it is necessary to assess the outcomes in this population. The purpose of this study was to compare outcomes of elective, primary THA in patients ≥ 80 years old to those aged < 80.

Methods

A retrospective review of 10,251 consecutive THA cases from 2011 to 2019 was conducted. Patient-reported outcome (PRO) scores (Hip disability and Osteoarthritis Outcome Score (HOOS)), as well as demographic, readmission, and complication data, were collected.

Aims

Thresholds for operative eligibility based on body mass index (BMI) alone may restrict patient access to the benefits of arthroplasty. The purpose of this study was to evaluate the relationship between BMI and improvements in patient-reported outcome measures (PROMs), and to determine how many patients would have been denied improvements in PROMs if BMI cut-offs were to be implemented.

Methods

A prospective cohort of 3,449 primary total hip arthroplasties (THAs) performed between 2015 and 2018 were analyzed. The following one-year PROMs were evaluated: hip injury and osteoarthritis outcome score (HOOS) pain, HOOS Physical Function Shortform (PS), University of California, Los Angeles (UCLA) activity, Veterans Rand-12 Physical Component Score (VR-12 PCS), and VR-12 Mental Component Score (VR-12 MCS). Positive predictive values for failure to improve and the number of patients denied surgery in order to avoid a failed improvement were calculated for each PROM at different BMI cut-offs.

Aims

The primary aim of this paper was to outline the processes involved in building the Partners Arthroplasty Registry (PAR), established in April 2016 to capture baseline and outcome data for patients undergoing arthroplasty in a regional healthcare system. A secondary aim was to determine the quality of PAR’s data. A tertiary aim was to report preliminary findings from the registry and contributions to quality improvement initiatives and research up to March 2019.

Methods

Structured Query Language was used to obtain data relating to patients who underwent total hip or knee arthroplasty (THA and TKA) from the hospital network’s electronic medical record (EMR) system to be included in the PAR. Data were stored in a secure database and visualized in dashboards. Quality assurance of PAR data was performed by review of the medical records. Capture rate was determined by comparing two months of PAR data with operating room schedules. Linear and binary logistic regression models were constructed to determine if length of stay (LOS), discharge to a care home, and readmission rates improved between 2016 and 2019.

Aims

We present the development of a day-case total hip arthroplasty (THA) pathway in a UK National Health Service institution in conjunction with an extensive evidence-based summary of the interventions used to achieve successful day-case THA to which the protocol is founded upon.

Methods

We performed a prospective audit of day-case THA in our institution as we reinitiate our full capacity elective services. In parallel, we performed a review of the literature reporting complication or readmission rates at ≥ 30-day postoperative following day-case THA. Electronic searches were performed using four databases from the date of inception to November 2020. Relevant studies were identified, data extracted, and qualitative synthesis performed.

Aims

Although good clinical outcomes have been reported for monolithic tapered, fluted, titanium stems (TFTS), early results showed high rates of subsidence. Advances in stem design may mitigate these concerns. This study reports on the use of a current monolithic TFTS for a variety of indications.

Methods

A multi-institutional retrospective study of all consecutive total hip arthroplasty (THA) and revision total hip arthroplasty (rTHA) patients who received the monolithic TFTS was conducted. Surgery was performed by eight fellowship-trained arthroplasty surgeons at four institutions. A total of 157 hips in 153 patients at a mean follow-up of 11.6 months (SD7.8) were included. Mean patient age at the time of surgery was 67.4 years (SD 13.3) and mean body mass index (BMI) was 28.9 kg/m2 (SD 6.5). Outcomes included intraoperative complications, one-year all-cause re-revisions, and subsidence at postoperative time intervals (two weeks, six weeks, six months, nine months, and one year).
