We conducted a case-control study to examine the merit of silver-coated tumour prostheses. We reviewed 85 patients with Agluna-treated (silver-coated) tumour implants treated between 2006 and 2011 and matched them with 85 control patients treated between 2001 and 2011 with identical, but uncoated, tumour prostheses. . In all, 106 men and 64 women with a mean age of 42.2 years (18.4 to 90.4) were included in the study. There were 50 primary reconstructions (29.4%); 79 one-stage revisions (46.5%) and 41 two-stage revisions for infection (24.1%). The overall post-operative infection rate of the silver-coated group was 11.8% compared with 22.4% for the control group (p = 0.033, chi-square test). A total of seven of the ten infected prostheses in the silver-coated group were treated successfully with debridement, antibiotics, and implant retention compared with only six of the 19 patients (31.6%) in the control group (p = 0.048, chi-square test). Three patients in the silver-coated group (3.5%) and 13 controls (15.3%) had chronic periprosthetic infection (p = 0.009, chi-square test). The overall success rates in controlling infection by two-stage revision in the silver-coated group was 85% (17/20) compared with 57.1% (12/21) in the control group (p = 0.05, chi-square test). The Agluna-treated endoprostheses were associated with a lower rate of early periprosthetic infection. These silver-treated implants were particularly useful in two-stage revisions for infection and in those patients with incidental positive cultures at the time of implantation of the prosthesis. . Debridement with antibiotic treatment and retention of the implant appeared to be more successful with silver-coated implants. Cite this article: Bone Joint J 2015;97-B:252–7

Introduction: We have previously described modification of Ti that renders the implant surface bactericidal and prevents bacterial colonization in vitro. This study evaluates the efficacy of the same surface in preventing periprosthetic infection in a small mammal model. Methods: Control or VancTi rods were incubated with S. aureus (Ci=104CFU/mL) in TSB containing 0, 5×10-3, 4, or 100 μg/mL vancomycin at 37°C for 24h. Bacteria were detected by fluorescence (Live/Dead BacLight) and imaged by confocal microscopy. Resistance was tested by incubating control or VancTi rods with S. aureus for 0–8 weeks. Adherent bacteria were tested every 7 days on vancomycin screening agar (6μg/mL). Results: Using a percutanous approach, the intercondylar region of the knee in rats were identified. The intrameduallry canal of the femur was retrograde reamed using an 18-gauge needle. Infection was induced by injection of 103 CFU S. aureus in 150uL of saline into the femoral canal followed by insertion of a 20 mm Ti implant. Animals were harvested at various time points. At harvest, animals were euthanized with CO2.. Detailed analysis including radiographic, micro-CT, histological, bacteriological, and clinical evaluation was performed. Results: All animals showed signs of infection within the first few post-operative days with increased soft tissue swelling and limited mobility. At 1 week 75% of the animals showed radiographic signs of periprosthetic infection including periosteal reaction, abscess formation, widened canal, bone destruction, and formation of involucrum. PPI could be prevented in 92% of cases when modified Ti-Van was used. In one animal despite the use of antibiotic-tethered implant, PPI occurred partially due to the pin insertion penetrating the bone cortex. Discussion: Biologically modified implants with bactericidal surface can have a promising role in management of periprosthetic infection. The modified implant described herein contains a nanoscale surface of covalently linked antibiotics that can potentially confer bactericidal properties throughout the life of the implant

Background. Total hip arthroplasty (THA) has become one of the most commonly performed elective procedures. Today, there are nearly 50 000 annual hospitalizations for hip replacement surgery in Canada. This number is projected to increase significantly with the aging population. Periprosthetic joint infection (PJI) is the 3rd leading cause of failure following THA and is reported to occur at an incidence of 1–3%. A two-stage revision THA is the current gold standard treatment and this has a tremendous economic impact on the healthcare system. The purpose of this study is to create an accurate cost estimate of two-stage revision THA and, in turn, evaluate the economic burden of PJI as it compares to primary THA in a Canadian healthcare context. Methods. We conducted a retrospective review of primary THA cases and two-stage revision THA for PJI at our institution. Patients were matched for age and BMI. We recorded all costs associated with each procedure, including: OR time, equipment, length of hospital stay, readmission rates, and any other inpatient resource use. Unit costs were obtained using administrative data from the case costing department at London Health Sciences Centre. Billing fees associated with the procedure were obtained from the Ontario Schedule of Benefits. Descriptive statistics were used to summarize the demographic characteristics of patients, hospital costs and resource use data. Patients with PJI were compared to the matched cohort of primary THA using the t-test (for continuous variables), and the chi-square test (for categorical variables). Results. Fifty consecutive cases of revision THA were matched to 50 patients who underwent uncomplicated primary THA between 2006 and 2014. Periprosthetic infection was associated with a significant increase in hospital stay (26.5 vs. 2.0; t=4.95, p<0.001), clinic visits (9.5 vs. 3.8; T= 6.49, p<0.001), readmission rates (12 vs. 1; X. 2. 11.1, p<0.001) and overall cost ($39 953 vs. $7 460; T=73, p<0.001) in comparison to the primary arthroplasty cohort. Conclusion. Two-stage revision for infected THA is a significant economic burden to the healthcare system. Our data suggests a 5-fold increase in healthcare cost when compared to primary THA. This may be an important consideration when distributing resources among Canadian tertiary care centres

Aims. The diagnosis of periprosthetic joint infection (PJI) remains demanding due to limitations of all the available diagnostic tests. The synovial fluid marker, α-defensin, is a promising adjunct for the assessment of potential PJI. The purpose of this study was to investigate the qualitative assessment of α-defensin, using Synovasure to detect or exclude periprosthetic infection in total joint arthroplasty. Patients and Methods. We studied 50 patients (28 women, 22 men, mean age 65 years; 20 to 89) with a clinical indication for revision arthroplasty who met the inclusion criteria of this prospective diagnostic study. The presence of α-defensin was determined using the qualitative Synovasure test and compared with standard diagnostic methods for PJI. Based on modified Musculoskeletal Infection Society (MSIS) criteria, 13 cases were categorised as septic and 36 as aseptic revisions. One test was inconclusive. Results. The Synovasure test achieved a sensitivity of 69% and a specificity of 94%. The positive and negative likelihood ratios were 12.46 and 0.33, respectively. A good diagnostic accuracy for PJI, with an area under the curve of 0.82, was demonstrated. Adjusted p-values using the method of Hochberg showed that Synovasure is as good at diagnosing PJI as histology (p = 0.0042) and bacteriology with one positive culture (p = 0.0327). Conclusion. With its ease of use and rapid results after approximately ten minutes, Synovasure may be a useful adjunct in the diagnosis of PJI. Cite this article: Bone Joint J 2017;99-B:66–72

Periprosthetic infection (PPI) is one of the most devastating complications of total knee arthroplasty (TKA). It is widely accepted that resection arthroplasty supplemented with intravenous antibiotics and delayed exchange arthroplasty is the treatment modality of choice for infected TKA. However, the outcome after reimplantation has varied and unpredictable results have been reported. This study evaluates the outcome of this treatment strategy in a single high volume specialised center. Furthermore, our study aims to identify the factors that lead to failure of this treatment. A thorough review of our joint registry database revealed that 80 patients with an infected TKA underwent resection arthroplasty at our institution during 2000–2005. Sixty-five patients underwent two-stage exchange arthroplasty while the remaining 15 failed to have the second stage reimplantation due to ill health or underwent arthrodesis or amputation. The latter 15 were excluded from the analysis. All patients were followed up prospectively for at least two years. Detailed data including demographics, comorbidities, surgical history, and medication intake was collected. Intraoperative data, organism profile, and complications were also documented. Failure was defined as patient requiring additional surgical procedure for control of infection or loosening. Two-stage exchange arthroplasty successfully eradicated infection in 45 patients (31%) without need for further treatment. Twelve patients (18%) had recurrent infection that necessitated another resection arthroplasty. Eleven (17%) patients required irrigation and debridement for postoperative purulent drainage which successfully treated infection in 5 cases (46%). The remaining 6 patients failed and required resection arthroplasty. Three additional patients had early loosening of components and required revision arthroplasty. The exact cause of loosening in these patients could not be determined, and despite lack of isolation of organisms infection was suspected. Our analysis identified that irrigation and debridement prior to resection arthroplasty are major risk factors for failure. Current strategies to treat periprosthetic infection remain imperfect. Two-stage exchange arthroplasty with all its inherent problems and inconveniences imparted a modest success in treatment of PPI at our high volume specialised center. The rise in the number of resistant and virulent organisms, increase in the number of patients with severe medical comorbidities who develop infection may account for the decline in the success of two-stage resection arthroplasty. Novel strategies for treatment of PPI are desperately needed

Periprosthetic infection with extensive bone loss is a complex situation. The appropriate management of large bone defects has not been established. Without reconstruction amputation/disarticulation is the likely outcome. Aim of the study was to Analyse preliminary results of direct exchange endoprosthetic reconstruction for periprosthetic infection associated with segmental bone defects. Study of patients with periprosthetic infection and severe osteolysis treated by direct exchange tumour prostheses between June, 2005 and May, 2008 (4 – Distal femoral & 2 – Total femoral Replacements). Microbiological evidence of infection was confirmed with regular monitoring of radiograph, crp, esr and wcc. Community based antibiotic therapy was provided by infectious disease team based in our institution. The mean age and follow up were 74.2 years and 26.5 months respectively. Mean duration of antibiotics was 6 weeks intravenous(community based) and 3.5 months oral. 1 patient required intervention by plastic surgeons at index procedure. Radiographs at 6, 12 & 24 months showed no changes from immediate post-op. CRP, ESR and WBC count were within normal limits at the end of antibiotic therapy. One patient required prolonged pain relief with poor mobility due to instability in the opposite knee. One patient had infection recurrence. Knee range of movements averaged full extension to 95 degrees. The mean oxford knee scores pre and post operatively were 58 and 39.4 respectively. We conclude that salvage endoprosthetic reconstruction has provided effective pain relief, stability and improved mobility in our experience. It has provided an oppourtunity to avoid amputation. Multidisciplinary support from plastic surgeons and specialist microbiologists is essential

Two-stage exchange arthroplasty successfully eradicated infection in 36 patients (67%) without need for further treatment. Seven patients (13%) had recurrent infection that necessitated resection arthroplasty. Eleven (20%) patients required irrigation and debridement for postoperative purulent drainage which successfully treated infection in 8 of the cases. The remaining 3 patients failed and required resection arthroplasty. Three additional patients had early loosening of components and required revision arthroplasty. The exact cause of loosening in these patients could not be determined and despite lack of isolation of organisms infection was suspected. Multivariate analysis identified previous medical comorbidity and postoperative allogenic transfusion as risk factors for failure. Current strategies to treat periprosthetic infection remain imperfect. Two-stage exchange arthroplasty with all its inherent problems and inconveniences imparted a modest success in treatment of PPI at our high volume specialized center. With the increase in the number of virulent and resistant organisms, and the rise in arthroplasties being performed in infirm patients with medical comorbidities the success of this procedure is likely to be jeopardized. Novel treatment modalities to combat this dreaded condition is needed

Aims. We hypothesised that the synovial white blood cell (WBC) count in patients with a late periprosthetic joint infection (PJI) of the hip would depend on the duration of a patient’s symptoms, and that the optimal diagnostic threshold would also depend on this period of time. Patients and Methods. The synovial WBC count and percentage of polymorphonuclear cells (%PMN), and the serum CRP and ESR levels obtained > six weeks after primary THA were compared between 50 infected and 88 non-infected THAs, and in patients with symptoms for more than or less than two weeks. Diagnostic thresholds for the synovial WBC count were calculated using area under the curve calculation. Results. The synovial WBC count was significantly higher in patients with symptoms for ≤ two weeks compared with those with symptoms for > two weeks (p = 0.03). The optimal threshold for diagnosing PJI for the synovial WBC count was 5750 cells/µL (sensitivity 94; specificity 100; PPV 100%; NPV 89%; AUC 99%) and 1556 cells/µL (sensitivity 91; specificity 94; PPV 87% and NPV 97%; AUC 95%), respectively. The thresholds for the cut-offs based on duration of symptoms improved the diagnostic performance of this test. Conclusion. This study shows that the diagnostic thresholds for synovial fluid analysis in late periprosthetic infection following THA may depend on duration of symptoms. Aims. It remains difficult to diagnose early postoperative periprosthetic joint infection (PJI) following total knee arthroplasty (TKA). We aimed to validate the optimal cutoff values of ESR, CRP, and synovial fluid analysis for detecting early postoperative PJI in a large series of primary TKAs. Methods. We retrospectively identified 27,066 primary TKAs performed between 2000 and 2019. Within 12 weeks, 169 patients (170 TKAs) had an aspiration. The patients were divided into two groups: those evaluated ≤ six weeks, or between six and 12 weeks postoperatively. The 2011 Musculoskeletal Infection Society (MSIS) criteria for PJI diagnosis in 22 TKAs. The mean follow-up was five years (two months to 17 years). The results were compared using medians and Mann-Whitney U tests and thresholds were analyzed using receiver operator characteristic curves. Results. Within six weeks, the median CRP (101 mg/l vs 35 mg/l; p = 0.011), synovial WBCs (58,295 cells/μl vs 2,121 cells/μl; p ≤ 0.001), percentage of synovial neutrophils (91% vs 71% (p < 0.001), and absolute synovial neutrophil count (ANC) (50,748 cells/μl vs 1,386 cells/μl (p < 0.001) were significantly higher in infected TKAs. Between six and 12 weeks, the median CRP (85 mg/l vs 5 mg/l (p < 0.001)), ESR (33 mm/hr vs 14 mm/hr (p = 0.015)), synovial WBCs (62,247 cells/μl vs 620 cells/μl (p < 0.001)), percentage of synovial neutrophils (93% vs 54% (p < 0.001)), and ANC (55,911 cells/μl vs 326 cells/μl (p < 0.001)) were also significantly higher in infected TKAs. Optimal thresholds at ≤ six weeks were: CRP ≥ 82 mg/l (sensitivity 70%, specificity 77%), synovial WBCs ≥ 8,676 cells/μl (83%, 90%), percentage of synovial neutrophils ≥ 88% (67%, 78%), and ANC ≥ 8,346 cells/μl (83%, 91%). Between six and 12 weeks, thresholds were: CRP ≥ 34 mg/l (90%, 93%), synovial WBCs ≥ 1,983 cells/μl (80%, 85%), percentage of synovial neutrophils ≥ 76% (80%, 81%), and ANC ≥ 1,684 cells/μl (80%, 87%). Conclusion. Early PJI after TKA should be suspected within six weeks if the CRP is ≥ 82 mg/l, synovial WBCs are ≥ 8,676 cells/μl, the percentage of synovial neutrophils is ≥ 88%, and/or the ANC is ≥ 8,346 cells/μl. Between six and 12 weeks, thresholds include a CRP of ≥ 34 mg/l, synovial WBC of ≥ 1,983 cells/μl, a percentage of synovial neutrophils of ≥ 76%, and/or an ANC of ≥ 1,684 cells/μl. Two of the major complications of total hip and knee arthroplasty is periprosthetic infection and aseptic loosening. The serum marker Procalcitonin (PCT) has been shown to be a sensitive indicator of bacterial infection, but very little is known of its behaviour in Orthopaedic practice. The diagnosis of periprosthetic infection still remains a difficult diagnosis. A prospective study over 6 months of 59 patients undergoing either primary total hip or knee arthroplasty was performed, which included 32 hips. The mean age was 70 years. There were no exclusion criteria. Serum blood samples for PCT, CRP, ESR and WCC were taken pre- operatively and on days 1, 3 and 5 post- operatively. Patient records were reviewed after their routine 6 week follow- up. There was no clinical suspicion of infection at this stage, or during their hospital stay. Fifty patients (85%) had PCT concentrations within the normal range (< 0.5ng/ml) on all 4 days, and only 5 recorded a value > 1.0ng/ml. Only 1 patient had an elevated level on day 5. The other markers all showed sporadic elevation over the 3 post- operative days. The usefulness of PCT in diagnosing surgical infection has been frequently researched in cardio- thoracic and abdominal surgery. However, there is debate as to what cut- off value should represent infection, ranging from 1– 10ng/ml. This is largely because the natural acute phase response caused by these operations elevates PCT levels. This study convincingly shows that PCT, unlike the routine inflammatory markers, is not significantly stimulated by primary hip and knee arthroplasty. This would imply that PCT may be useful in diagnosing periprosthetic infection. A large multicentre study involving patients undergoing revision surgery would help validate this assumption

Report of a case of migrating periprosthetic infection from a hip replacement to a contralateral knee joint undergoing a total knee replacement. We present a 74-year old female patient who underwent a total hip arthroplasty of the left hip after a subcapital fracture of the femur. Four months after the index procedure the patient presented with signs and symptoms of infection of the operated joint. Staph aureus and Enterococcus faecalis were recognized as the infecting bacteria. The implants were removed, cement spacers were placed and a total hip arthroplasty was performed again after three months. Unfortunately, infection ensued again and the patient underwent three more procedures until the joint was considered clean and t he hip remained flail without implants. The patient elected to undergo a total knee arthroplasty due to severe osteoarthritis of right knee. Intraoperatively tissue samples were taken and sent for cultures which identified Enterococcus faecalis present in the knee joint. Enterococcus migrated from the infected hip to nonoperated knee joint. Intravenous antibiotics were administered for three weeks but the knee presented with infection of the arthroplasty ten months after its insertion. The implants were removed the joint was debrided and cement spacers were inserted. The patient decided not to proceed with another procedure and she remains with the cement spacers in her knee. Rare report of migrating periprosthetic infection. Nosocomial enterococci acquired resistance cannot be ruled out. Unique characteristics in enterococci antibiotic resistance and biofilm formation

Aims. Infection following total hip or knee arthroplasty is a serious complication. We noted an increase in post-operative infection in cases carried out in temporary operating theatres. We therefore compared those cases performed in standard and temporary operating theatres and examined the deep periprosthetic infection rates. Patients and methods. A total of 1223 primary hip and knee arthroplasties were performed between August 2012 and June 2013. A total of 539 (44%) were performed in temporary theatres. The two groups were matched for age, gender, body mass index and American Society of Anesthesiologists grade. Results. The deep infection rate for standard operating theatres was 0 of 684 (0%); for temporary theatres it was eight of 539 (1.5%) (p = 0.001). Conclusion. Use of a temporary operating theatre for primary hip and knee arthroplasty was associated with an unacceptable increase in deep infection. We do not advocate the use of these theatres for primary joint arthroplasty. Cite this article: Bone Joint J 2017;99-B:917–20

Problem: Deep infection after hip- or knee-replacement is a severe complication that may lead to implant removal or arthrodesis. Aim: In our hospital intraoperativ subcoutanues culture samples were taken before wound closure. We wanted to know if positive cultures are of any predicitive value in relation to early or late periprosthetic infection. If so, is there a consequence in treatment ? Also costs were analysed. Material and methods: In 2002 we performed 167 primary hip and knee replacements. We retrospectivaly analysed the outcome of 159 cases over a follow-up period of up to 28 month. Results: In 96.8% of the cases a culture sample was taken. Of these 5.8% showed germ growth. In only 4 cases positive cultures were followed by a change in treatment, either antibiotics or revision. Of nine patients with revision surgery only one had a positive culture. Four cases showed germ growth during revision surgery after sterile cultures during implantation. Most often different bacterias were found in primary and revision surgery. In 1.25% deep periprosthetic infection occured. One required second stage knee-replacement, one case ended in resection arthroplasty. Conclusion: Intraoperative culture sample is of no predictive value in primary joint replacement. There is no correlation between positiv cultures and indication for revision surgery. However costs for culture analysis and antibiotics are low compared to the increase of expense caused by periprosthetic infections

Introduction: Periprosthetic infection remains a main complication in arthroplasty. In case of a possible infection the surgeon has to have a concept of treatment which can be individually adjusted. Materials and Methods: To increase the success of implant retaining surgery VAC Instill therapy combining instillation of an antiseptic solution and VAC therapy was performed in 23 patients with periprosthetic hip infections. 19 patients had an early and 4 a late infection of the arthroplasty. Lavasept was used for irrigation. Results: Definitive wound closure was possible in all patients following 2.1 VAC exchange operations in average. The follow up was 19 months in average. In five of the 23 patients (22%) there was recurrent infection which made the explantation of the implant necessary. In early infection the success rate was 84%, in late infection 50%. Conclusion: VAC Instill therapy can be successfully used for salvage of infected endoprosthesis especially in case of early infection. The success rate seems to be higher than irrigation alone or suction drainage

Results: In general it shows that the most desolate cases in all categories line up for a primary failure. The ASA rating is remarkably higher and local findings like fistulas are present in nearly all patients. The bacteria involved are staphylococci in 70 %. Over 50 % had a combination of two or more pathogens and two patients showed an additional pathogen in the samples taken during the operation. The pattern of the involved bacteria is remarkably severe. But there are also three cases where no sign of higher risk or lower chances could be detected. Conclusions: Knowing about causes of failure in the specific one stage exchange situation does lead to adaptation in the concept of treatment for those specific cases. With successful application, better results through individual therapy-concepts will be possible

Infection following total hip or knee arthroplasty is a serious complication. We noted an increase in post-operative infection in cases carried out in a temporary operating theatre. We therefore compared those cases performed in standard and temporary operating theatres and examined the deep periprosthetic infection rates. A total of 1233 primary hip and knee arthroplasties were performed between August 2012 and June 2013. 44% were performed in temporary theatres. The two groups were matched for age, sex, BMI and ASA grade. The deep infection rate for standard operating theatres was 0/684 (0%); for temporary theatres it was 8/539 (1.5%); p=0.001. Use of a temporary operating theatre for primary hip and knee arthroplasty was associated with an unacceptable increase in deep infection. We do not advocate the use of these theatres for primary joint arthroplasty

Background. Additive manufacturing (AM) has created many new avenues for material and manufacturing innovation. In orthopaedics, metal additive manufacturing is now widely used for production of joint replacements, spinal fusion devices, and cranial maxillofacial reconstruction. Plastic additive manufacturing on the other hand, has mostly been utilized for pre-surgical planning models and surgical cutting guides. The addition of pharmaceuticals to additively manufactured plastics is novel, particularly when done at the raw material level. The purpose of this study was to prove the concept of antibiotic elution from additively manufactured polymeric articles and demonstrate feasibility of application in orthopaedics. Methods. Using patented processes, three heat-stable antibiotics commonly used in orthopaedics were combined with six biocompatible polymers (2 bioresorbable) into filament and powder base materials for fused deposition modeling (FDM) and selective laser sintering (SLS) AM processes. Raw materials of 1%, 2%, and 5% antibiotic concentrations (by mass) were produced as well as a blend of all three antibiotics each at 1% concentration. Thin disks of 25 mm diameter were manufactured of each polymer with each antibiotic at all concentrations. Disks were applied to the center of circular petri dishes inoculated with a bacterium as per a standard zone of inhibition, or Kirby-Bauer disk diffusion tests. After 72 hours incubation, the zone of inhibited bacterial growth was measured. Periprosthetic joint infection (PJI) of the knee was selected as the proof-of-concept application in orthopaedics. A series of tibial inserts mimicking those of a common TKR system were manufactured via SLS using a bioresorbable base material (Figure 1). Three prototype inserts were tested on a knee wear simulator for 333,000 cycles following ISO 14242–1:2014 to approximate 2–4 months of in vivo use between surgeries of a 2-stage procedure for PJI. Gravimetric measurement and visual damage assessment was performed. Results. Bacterial growth was inhibited to a mean diameter of 32.3 mm (FDM) and 42.2 mm (SLS) for nearly all combinations of polymers and concentrations of antibiotics. Prototype tibial inserts experienced an average of 200 mg of wear during testing and demonstrated no evidence of cracking, delamination or significant deformation (Figure 2). Conclusion. Bench-level testing of these novel antibiotic-eluting polymers demonstrates feasibility for their application in orthopaedic medicine. In particular, treatment of stubborn PJI with potential for increased and sustained antibiotic elution, patient-specific cocktailing, and maintenance of knee joint structure and function compared to existing PJI products and practices. Debridement of an infected total joint arthroplasty with retention of mechanically stable components is often performed for acute cases of periprosthetic infection (PPI). However, the reported success of such a procedure to fully eradicate infection has varied widely. The objective of this study was to elucidate the efficacy of debridement in both infected THA and TKA and attempt to identify risk factors responsible for failure. During the years 2000–2005, 71 TKA and 69 THA underwent irrigation and debridement for acute PPI (< 4 weeks). All patients were followed up prospectively for at least two years. Detailed data including demographics, comorbidities, surgical history, and medication intake was collected. Intraoperative data, organism profile, and complications were also documented. Failure was defined as patient requiring additional surgical procedure for control of infection or loosening. Of the 140 patients, 24% required repeat irrigation and debridement for postoperative drainage, hema-toma formation, or systemic symptoms. One third of these revision debridement patients underwent multiple consecutive debridements. Two-stage resection arthroplasty was required in 65 patients (46%) of the entire cohort. Fifty-eight percent of the patients with resection required revision of their cement spacer block due to continuous drainage and systemic symptoms indicative of persistent infection. We noted a total of 86 failures (61%) that required either an additional debridement or resection arthroplasty after the first debridement procedure. The failure rates of THA (62%) and TKA (55%) individually were similar (p=0.253). Although the concept of conservative management of PPI with debridement and retention of components is an attractive alternative to resection arthroplasty, we have found that 60% of patients undergoing this procedure will inevitably undergo two-stage arthroplasty. Furthermore, more than half of the patients that required resection arthroplasty developed infection of their spacer that entailed revision of the cement block. Therefore, we can conclude that this procedure has a high failure rate and should be implemented in only a select group of patients

Purpose:. Two-stage revision is the gold standard treatment for chronic periprosthetic infection of the hip or knee. Implantation of a functional antibiotic spacer allows patients the option of two stage revision or living with the spacer in place. Some patients may be satisfied with the activity level that they attain with the functional antibiotic spacer and may avoid the morbidity of a second surgery. Methods:. We retrospectively examined the outcomes of 30 joints in 27 patients treated for chronic periprosthetic infection of the hip or knee at a mean follow-up of 27 months (range 11–46 months). Comparing the patients treated with two-stage revision to those patients living with a functional antibiotic spacer, we used University of California Los Angeles activity scores, modified Harris hip scores, and patient satisfaction measures to answer two main questions: (1) Is there a significant difference in activity level? (2) Is there a significant difference in patient satisfaction? Patient satisfaction was measured on a 1–100 scale with 100 being completely satisfied. Results:. There were no statistically significant differences in the American Society of Anesthesiologists scores between the groups—mean 2.69 in the two-stage group and mean 3.00 in the spacer group (p value 0.255). Statistically significant differences were found between the groups for University of California Los Angeles activity scores—mean 5.3 in the two-stage group and mean 2.8 in the spacer group (p value 0.027). There were no statistically significant differences in modified Harris hip scores—mean 55.6 in the two-stage group and mean 41.6 in the spacer group (p value 0.34). In the subgroup analysis of the modified Harris hip score questions, there was a statistically significant difference in the need for support when ambulating—mean 4.21 in the two-stage group and mean 2.45 in the spacer group (p value 0.024)—and in the distance that can be walked—mean 6.15 in the two-stage group and mean 2.0 in the spacer group (p value 0.005). No statistically significant differences were found in patient satisfaction measures—mean 70.0 in the two-stage group and mean 76.6 in the spacer group (p value 0.65)—though the mean satisfaction measure was higher in the spacer group. Conclusion:. Patients living with a functional antibiotic spacer are less active than patients undergoing a two-stage revision for chronic periprosthetic infection of the hip or knee. Specifically, patients living with a functional antibiotic spacer need more support when ambulating and ambulate shorter distances than patients, who undergo two-stage revision surgery. Despite the decreased activity level, patients living with a functional antibiotic spacer are just as satisfied with the outcome of their joint as those patients treated with a two-stage revision. Patients with modest activity goals may be satisfied to live with a functional antibiotic spacer and to avoid a second surgery

Serological tests including erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) are frequently used in the preoperative workup to screen for periprosthetic infection (PPI) in total hip arthroplasty (THA). The cut-off points reported in the literature are arbitrarily chosen by investigators. Similarly, the values used in laboratories to distinguish elevated results vary from one institute to another. Therefore, we intended to define the appropriate cut-off points of ESR and CRP that can be used to differentiate infection from aseptic failure of THA. A review of our joint registry database revealed that 515 THA revisions (131 infected cases) were performed during 2000–2005. Intraoperative samples for culture were taken in all cases. The criteria used for diagnosis of infection were a positive intraoperative culture on solid media, presence of an abscess or sinus tract that communicated with the joint, positive preoperative aspiration culture, and/or elevated fluid cell count and neutrophil differential of the aspirated fluid. Non-infected patients with confounding factors that can elevate ESR and CRP including collagen vascular disease, inflammatory arthropathy, malignancy, and urinary tract infection were excluded. Receiver operator curves were used to determine the ideal cut-off point for both ESR and CRP. The mean value of ESR in the infected group (77mm/ hr) was significantly higher compared to that of the non-infected cohort (29mm/hr) (p=0.0001). Similarly, infected patients presented with a greater mean CRP (9.8 mg/dl) than their non-infected cohort (1.48 mg/ dl) (p=0.0001). The infection threshold for ESR was 45mm/hr with a sensitivity of 85% and specificity of 79%, while the optimal cut-off value for CRP was defined as 1.6 mg/dl which yielded a sensitivity of 86% and specificity of 83%. The optimal threshold values we determined are higher than the arbitrarily chosen values cited in the literature for ESR (30mm/hr) and CRP (1mg/dl). Although it has been previously reported that the sensitivity and specificity of CRP are far greater than that of ESR, we found that the two tests have comparable diagnostic value

Aim: The purpose of our study is to present the survival results, clinical outcome and complications from the use of APC in cases with a history of periprosthetic infection. Materials and Methods: Between 1986 and 1999, twenty-two patients (twenty-two hips) 11 male and 11 female (mean age 57.5 years – range 38 to 77 years) with massive bone loss (Paprosky IIIA 2 cases, IIIB 4 cases, and IV 16 cases) were included to our study. They all had a history of periprosthetic infection after an average of 3.3 (range 1 to 5) revision hip arthroplasties and were submitted to a two stage revision arthroplasty using an allograft-prosthesis composite. Results: At an average follow-up of eleven years (range, eight to twenty years), 14 patients were alive, 7 patients died, and 1 patient was lost to follow-up. The ten year survival of the allograft-prosthesis composites was 74.9 per cent (95 per cent confidence interval 55.1 to 94.7 per cent, 4 cases remaining at risk). Seven cases presented with APC failure needing re-revision, 2 due to re-infection (4 and 23 months from revision by the same microorganism species as for the initial infection (Staph aureus to both cases), 3 due to allograft non union (at 21, 43, 79 months) and 2 cases due to graft resorption (164, 175 months post revision). Delayed healing and wound drainage occurred to 2 more cases. Conclusion: Reconstruction of massive proximal femoral bone loss with an allograft-implant composite is a demanding procedure. Biologic means of reconstruction is a major advantage preserving bone stock for future surgery. However, high complication rate should be considered

Aims. We aimed to report the mid- to long-term rates of septic and aseptic failure after two-stage revision surgery for periprosthetic joint infection (PJI) following total hip arthroplasty (THA). Methods. We retrospectively reviewed 96 cases which met the Musculoskeletal Infection Society criteria for PJI. The mean follow-up was 90 months (SD 32). Septic failure was assessed using a Delphi-based consensus definition. Any further surgery undertaken for aseptic mechanical causes was considered as aseptic failure. The cumulative incidence with competing risk analysis was used to predict the risk of septic failure. A regression model was used to evaluate factors associated with septic failure. The cumulative incidence of aseptic failure was also analyzed. Results. There were 23 septic failures at final follow-up, with a cumulative incidence of 14% (95% confidence interval (CI) 8% to 22%) at one year, 18% (95% CI 11% to 27%) at two years, 22% (95% CI 14% to 31%) at five years, and 23% (95% CI 15% to 33%) at ten years. Having at least one positive culture (hazard ratio (HR) 2.38 (interquartile range (IQR) 1.19 to 4.74); p = 0.013), or a positive intraoperative frozen section (HR 2.55 (IQR 1.06 to 6.15); p = 0.037) was significantly associated with septic failure after reimplantation. With dislocation being the most common cause of aseptic revision (5.2%), the cumulative incidence of aseptic failure was 1% (95% CI 0% to 5%) at one year, 6% (95% CI 1% to 8%) at five years, and 8% (95%CI 3% to 17%) at ten years. Conclusion. If there is no recurrent infection in the five years following reimplantation, the chances of further infection thereafter are remote. While the results of a frozen section may be a reliable guide to the timing of reimplantation, intraoperative culture has, currently, only prognostic value. Introduction: Revision surgery in periprosthetic infection often encounters defects in bone stock caused by the loosening procedures, through continuous revisions or by explantation techniques. Since bony reconstruction in the presence of infection is critical, if no antibiotic impregnated bone grafts are available, metal implants are the last resort. Lately tantalum wedges and cones have been introduced as a stable augmentation device. Material and Methods: Since 2007 we have used 14 tantalum implants to fill bone defects in 13 cases of severe purulent periprosthetic infection (4 cones, 10 wedges). All patients are in permanent control on an outpatient basis. Results: Primary and continuous stability was achieved in all patients and no signs of recurrent or persistent infection were found. Conclusion: So far this concept proves to be successful and can be recommended. The stability of the implant is recorded for numerous aseptic reconstructions. Research is needed towards the surface properties of tantalum in contact with bacteria. Positive findings like in silver coating would be desirable. Further research towards impregnation possibilities of the porous structure with antibiotics for continuous elution like from cement or fleece could – in a positive result – improve septic surgery substantially

Aims. There is a paucity of long-term studies analyzing risk factors for failure after single-stage revision for periprosthetic joint infection (PJI) following total hip arthroplasty (THA). We report the mid- to long-term septic and non-septic failure rate of single-stage revision for PJI after THA. Methods. We retrospectively reviewed 88 cases which met the Musculoskeletal Infection Society (MSIS) criteria for PJI. Mean follow-up was seven years (1 to 14). Septic failure was diagnosed with a Delphi-based consensus definition. Any reoperation for mechanical causes in the absence of evidence of infection was considered as non-septic failure. A competing risk regression model was used to evaluate factors associated with septic and non-septic failures. A Kaplan-Meier estimate was used to analyze mortality. Results. The cumulative incidence of septic failure was 8% (95% confidence interval (CI) 3.5 to 15) at one year, 13.8% (95% CI 7.6 to 22) at two years, and 19.7% (95% CI 12 to 28.6) at five and ten years of follow-up. A femoral bone defect worse than Paprosky IIIA (hazard ratio (HR) 13.58 (95% CI 4.86 to 37.93); p < 0.001) and obesity (BMI ≥ 30 kg/m. 2. ; HR 3.88 (95% CI 1.49 to 10.09); p = 0.005) were significantly associated with septic failure. Instability and periprosthetic fracture were the most common reasons for mechanical failure (5.7% and 4.5%, respectively). The cumulative incidence of aseptic failure was 2% (95% CI 0.4 to 7) at two years, 9% (95% CI 4 to 17) at five years, and 12% (95% CI 5 to 22) at ten years. A previous revision to treat PJI was significantly associated with non-septic failure (HR 9.93 (95% CI 1.77 to 55.46); p = 0.009). At the five-year timepoint, 93% of the patients were alive (95% CI 84% to 96%), which fell to 86% (95% CI 75% to 92%) at ten-year follow-up. Conclusion. Massive femoral bone loss was associated with greater chances of developing a further septic failure. All septic failures occurred within the first five years following the one-stage exchange. Surgeons should be aware of instability and periprosthetic fracture being potential causes of further aseptic revision surgery. Cite this article: Bone Joint J 2021;103-B(7):1247–1253

Introduction:. Periprosthetic joint infection (PJI) is a devastating diagnosis that carries a significant rate of associated mortality and places a large burden on health care systems. Treatment protocols often include combined intravenous antibiotics and staged revision surgery with locally-delivered antibiotics via PMMA cement spacers and/or beads. One disadvantage of PMMA is the need for later removal. Antibiotic releasing Calcium Sulphate beads (CaSO. 4. ) have had promising results in revision joint surgery and are absorbable, making later removal unnecessary. We report on use in a tertiary referral centre in the UK and present our initial findings. Methods & Results:. CaSO. 4. beads containing 1 gram of Vancomycin and 240 mg of tobramycin per 10 cc was implanted in 12 patients between August 2012 and December 2012, all having undergone revision joint surgery for PJI. Of these patients; 7 were men and 5 women, mean age was 57 years (range 39–72) with a mean ASA grade of 2 (1–4). Indications were infected Total Hip Replacement (n = 7), infected Total Knee Replacement (n = 4) and infected metal on metal hip resurfacing (n = 1). Three procedures were emergencies, with the remainder being semi-elective procedures. One patient had single-stage revision THR. At latest follow up 10 patients had made a full recovery, with normal function and inflammatory markers. Two patients were awaiting a second stage revision procedure. Mean follow up was 2 months (1–4). Conclusion:. Implantable calcium sulphate beads are a new therapeutic agent for use in periprosthetic infection, with improved drug eluting properties and total absorption radiographically within two to three weeks. This study is the first to report its use in the UK, with encouraging data supporting its use in revision arthroplasty surgery

One of the routinely used intraoperative tests for diagnosis of periprosthetic infection (PPI) is Gram stain that is reported to carry a very high specificity and a poor sensitivity. However, it is not known if the result of this test can vary according to the type of joint affected or the number of specimen samples collected. This study intended to examine the role of this diagnostic test in a large cohort of patients from single institution. A review of our joint registry database revealed that 453 total knee arthroplasty (TKA) and 551 total hip arthroplasty (THA) of which 171 and 150 cases were respectively infected underwent revision surgery during 2000–2005 and had intraoperative cultures available for interpretation. A positive gram stain was defined as the visualisation of bacterial cells or ‘many leukocytes’ (> 5 per high power field) under the smear. The sensitivity, specificity, and predictive values of each individual diagnostic arm of Gram stain were determined. Combinations were performed in series that require both tests to be positive to confirm infection and in parallel that necessitate both tests to be negative to rule out infection. This analysis was performed for THA and TKA separately and later compared for each joint type. The presence of organism cells and ‘many’ neutrophils on a Gram smear had high specificity (98%–100%) and positive predictive value (89%–100%) in both THA and TKA. The sensitivities (30%–50%) and negative predictive values (70%–79%) of the two tests were low as expected among both joint types. When the two tests were combined in series the specificity and positive predictive value were absolute (100%). The sensitivity (43%–64%) and the negative predictive value (82%) improved among both THA and TKA. The presence of organisms or ‘many’ leukocytes on the Gram smear can confirm PPI in TJA. As expected, the sensitivity and negative predictive value of the two tests were low, and therefore infection could not be safely ruled out. Although the two diagnostic arms of Gram stain can be combined to achieve improved negative predictive value (82%), Gram stain continues to have poor value in ruling out PPI. With the advances in the field of molecular biology, novel diagnostic modalities need to be designed that can replace these traditional and poor tests

Background. The diagnosis of Periprosthetic Joint Infection (PJI) is a considerable challenge in total joint arthroplasty. The mainstay for diagnosis of PJI is a combination of serological markers, including C-reactive protein (CRP), along with joint aspirate for white cell count, differential and culture. The aim of this study was to examine the use of synovial fluid CRP in the diagnosis of PJI. Material & Methods. Synovial fluid samples were collected prospectively from patients undergoing primary and revision knee arthroplasty. Samples were assessed for CRP, cell count and differential. Three groups were analyzed; those undergoing primary knee arthroplasty, aseptic knee arthroplasties and infected arthroplasties. Demographic data, along with associated medical co-morbidities, were collected,. Statistical analysis was performed. Synovial fluid CRP was correlated with serum CRP values. Sensitivity and specificity were calculated. Results. 50 synovial fluid samples were collected from 50 patients. Synovial fluid CRP was 0.3 +/− 0.4 in native knees, 0.2 +/− 0.5 in aseptic knee arthroplasties, 4.3+/−4.1 in patients with infected knee arthroplasties. Synovial fluid CRP was significantly higher in septic total knee arthroplasties by comparison to both aseptic total knees (p< 0.001) and native knees (p=0.006). The specificity was 100% and the sensitivity was 89% at a CRP of 1. Conclusion. While this is preliminary data, synovial fluid CRP was found to be significantly elevated in patients with infected total knee arthroplasties. This test is easily performed under routine hospital laboratory conditions without specific, expensive assays. We believe synovial CRP assay holds great promise as a new, potentially low-cost, diagnostic marker for PJI

Although bovine carriers of antibiotics are generally accepted in the treatment of local infections, a literature search in PubMed, Medline and Embase up to April 2006 did not reveal any information on the human pharmacokinetics on Garacol®, a bovine carrier containing the equivalent of 130 mg gentamicin sulphate, on the market since 1995. The purpose of this study was to describe the human in vivo pharmacokinetics of Garacol®. A cohort of 19 consecutive patients with an acute periprosthetic infection to which 2 to 5 fleeces were applied in each case. Initially, the concentration in blood increased to 3.2–7.2 mg/L depending on the number of fleeces that were applied. The serum peak concentrations resulted in peak/MIC ratios of 2.5–36 for P. aeruginosa, S. aureus and Klebsiella spp. Peak gentamicin levels in the exudate are bactericidal for several days, even for gentamicin-resistant micro-organisms. Subsequently, the serum values decreased almost linearly below 0.3 mg/L in 18 to 62 hours. After 24 hours the gentamicin serum levels dropped below the threshold for toxicity of 2 mg/L. Comparison is made between the difference in pharmacokinetic behaviour of the Garacol® drug with Septocoll® and conventional and mini PMMA beads. The conclusion is that collagen-loaded fleeces may be useful as an adjuvant treatment of implant-related infections

BACKGROUND. Periprosthetic infection is the most challenging complication following total knee arthroplasty (TKA). Poor oral hygiene has been assumed as an important risk factor for TKA infection. We aimed to investigate whether the improvement of oral hygiene through dental scaling could reduce the risk of TKA infection. METHODS. A nested case control study was conducted and enrollees in the National Health Insurance Research Database (NHIRD) aged above 40 years who had received total knee arthroplasty (TKA) between 1999–2002 were included as the TKA cohort. The cases were patients who underwent resection arthroplasty for infected TKA, and each case was matched by 4 controls from the TKA cohort by gender, using incidence density sampling method. The frequency of dental scaling before the index date was analyzed and compared between the case and the control groups. Multiple conditional logistic regression was used to assess the frequency of dental scaling and the risk of TKA infection. RESULTS. The percentages of patients who ever received dental scaling before the end of follow-up were higher in the control group than that in the TKA infection group. As compared with patients never received dental scaling, there was a trend that patients who received dental scaling once within 2 years, may have 9% lower risk in TKA infection (adjusted odds ratio, 0.91; 95% confidence interval, 0.76–1.08). Moreover, the risk of TKA infection can be reduced significantly in patients who had dental scaling at least once per year, with an odds ratio of 0.71 (95% confidence interval, 0.53–0.95). CONCLUSIONS. Improvement of oral hygiene by more frequent and regular dental scaling (at least once per year) may reduce the risk of TKA infection. TKA Patients should be advised to check their oral hygiene regularly

Aims. Periprosthetic hip-joint infection is a multifaceted and highly detrimental outcome for patients and clinicians. The incidence of prosthetic joint infection reported within two years of primary hip arthroplasty ranges from 0.8% to 2.1%. Costs of treatment are over five-times greater in people with periprosthetic hip joint infection than in those with no infection. Currently, there are no national evidence-based guidelines for treatment and management of this condition to guide clinical practice or to inform clinical study design. The aim of this study is to develop guidelines based on evidence from the six-year INFection and ORthopaedic Management (INFORM) research programme. Methods. We used a consensus process consisting of an evidence review to generate items for the guidelines and online consensus questionnaire and virtual face-to-face consensus meeting to draft the guidelines. Results. The consensus panel comprised 21 clinical experts in orthopaedics, primary care, rehabilitation, and healthcare commissioning. The final output from the consensus process was a 14-item guideline. The guidelines make recommendations regarding increased vigilance and monitoring of those at increased risk of infection; diagnosis including strategies to ensure the early recognition of prosthetic infection and referral to orthopaedic teams; treatment, including early use of DAIR and revision strategies; and postoperative management including appropriate physical and psychological support and antibiotic strategies. Conclusion. We believe the implementation of the INFORM guidelines will inform treatment protocols and clinical pathways to improve the treatment and management of periprosthetic hip infection. Aims. This study evaluated the definitions developed by the European Bone and Joint Infection Society (EBJIS) 2021, the International Consensus Meeting (ICM) 2018, and the Infectious Diseases Society of America (IDSA) 2013, for the diagnosis of periprosthetic joint infection (PJI). Methods. In this single-centre, retrospective analysis of prospectively collected data, patients with an indicated revision surgery after a total hip or knee arthroplasty were included between 2015 and 2020. A standardized diagnostic workup was performed, identifying the components of the EBJIS, ICM, and IDSA criteria in each patient. Results. Of 206 included patients, 101 (49%) were diagnosed with PJI with the EBJIS definition. IDSA and ICM diagnosed 99 (48%) and 86 (42%) as infected, respectively. A total of 84 cases (41%) had an infection based on all three criteria. In 15 cases (n = 15/206; 7%), PJI was present when applying only the IDSA and EBJIS criteria. No infection was detected by one definition alone. Inconclusive diagnoses occurred more frequently with the ICM criteria (n = 30/206; 15%) compared to EBJIS (likely infections: n = 16/206; 8%) (p = 0.029). A better preoperative performance of the EBJIS definition was seen compared with the ICM and IDSA definitions (p < 0.001). Conclusion. The novel EBJIS definition identified all PJIs diagnosed by any other criteria. Use of the EBJIS definition significantly reduced the number of uncertain diagnoses, allowing easier clinical decision-making. Cite this article: Bone Joint Res 2022;11(9):608–618

Aims. The diagnosis of periprosthetic joint infection (PJI) can be challenging as the symptoms are similar to other conditions, and the markers used for diagnosis have limited sensitivity and specificity. Recent research has suggested using blood cell ratios, such as platelet-to-volume ratio (PVR) and platelet-to-lymphocyte ratio (PLR), to improve diagnostic accuracy. The aim of the study was to further validate the effectiveness of PVR and PLR in diagnosing PJI. Methods. A retrospective review was conducted to assess the accuracy of different marker combinations for diagnosing chronic PJI. A total of 573 patients were included in the study, of which 124 knees and 122 hips had a diagnosis of chronic PJI. Complete blood count and synovial fluid analysis were collected. Recently published blood cell ratio cut-off points were applied to receiver operating characteristic curves for all markers and combinations. The area under the curve (AUC), sensitivity, specificity, and positive and negative predictive values were calculated. Results. The results of the analysis showed that the combination of ESR, CRP, synovial white blood cell count (Syn. WBC), and polymorphonuclear neutrophil percentage (PMN%) with PVR had the highest AUC of 0.99 for knees, with sensitivity of 97.73% and specificity of 100%. Similarly, for hips, this combination had an AUC of 0.98, sensitivity of 96.15%, and specificity of 100.00%. Conclusion. This study supports the use of PVR calculated from readily available complete blood counts, combined with established markers, to improve the accuracy in diagnosing chronic PJI in both total hip and knee arthroplasties. The sensitivity, specificity, and predictive values of ESR and CRP were determined. Combinations were performed in parallel that necessitate both tests to be negative to rule out infection. The cost of each serological test was compared to that of other commonly used screening modalities. One hundred and sixteen patients (39%) were classified as infected and 180 patients (61%) were considered non-infected. The sensitivity, specificity, positive predictive value, and negative predictive value for the ESR were: 91%, 72%, 68%, and 93%. The sensitivity, specificity, positive predictive value, and negative predictive value for the CRP were: 94%, 74%, 70%, and 95%. The sensitivity and negative predictive value for the combined studies were 96% and 95%, respectively. However, five infected patients (4%) had a normal. ESR and CRP. An organism was cultured on solid media in 4 of the 5 cases. ESR and CRP were the least costly of all the preoperative tests including radionuclide imaging and joint fluid analysis. ESR and CRP are important preoperative tests in diagnosis of PPI and their ability to clench the diagnosis in the majority of cases should not be underestimated. When combined, these simple serological tests have improved sensitivity and negative predictive value to rule out infection

Aims. Periprosthetic joint infection (PJI) in total hip arthroplasty in the elderly may occur but has been subject to limited investigation. This study analyzed infection characteristics, surgical outcomes, and perioperative complications of octogenarians undergoing treatment for PJI in a single university-based institution. Methods. We identified 33 patients who underwent treatment for PJIs of the hip between January 2010 and December 2019 using our institutional joint registry. Mean age was 82 years (80 to 90), with 19 females (57%) and a mean BMI of 26 kg/m. 2. (17 to 41). Mean American Society of Anesthesiologists (ASA) grade was 3 (1 to 4) and mean Charlson Comorbidity Index was 6 (4 to 10). Leading pathogens included coagulase-negative Staphylococci (45%) and Enterococcus faecalis (9%). Two-stage exchange was performed in 30 joints and permanent resection arthroplasty in three. Kaplan-Meier survivorship analyses were performed. Mean follow-up was five years (3 to 7). Results. The two-year survivorship free of any recurrent PJI was 72% (95% confidence interval (CI) 56 to 89; 18 patients at risk). There were a total of nine recurrent PJIs at a mean of one year (16 days to eight years), one for the same pathogen as at index infection. One additional surgical site infection was noted at two weeks, resulting in a 69% (95% CI 52 to 86; 17 patients at risk) survivorship free of any infection at two years. There were two additional revisions for dislocations at one month each. As such, the two-year survivorship free of any revision was 61% (95% CI 42 to 80; 12 patients at risk). In addition to the aforementioned revisions, there was one additional skin grafting for a decubitus ulcer, resulting in a survivorship free of any reoperation of 54% (95% CI 35 to 73; ten patients at risk) at two years. Mean Clavien-Dindo score of perioperative complications was two out of five, with one case of perioperative death noted at six days. Conclusion. Octogenarians undergoing surgery for PJI of the hip are at low risk of acute mortality, but are at moderate risk of other perioperative complications. One in two patients will undergo a reoperation within two years, with 70% attributable to recurrent infections. Cite this article: Bone Joint J 2023;105-B(2):135–139

Introduction: One stage and two stage exchange procedures are acknowledged as gold standard for revision surgery in periprosthetic infection. Since the one stage concept is ethically, medically and economically superior, the last necessary argument is a fair rate of success on a large scale. The two stage procedure could then be limited to its genuine indications only. Material and Methods: In three years (2005/06/07) 555 one stage exchanges of the hip and the knee were carried out at our clinic. A first homogenous group of 282 cases has now been analysed and followed-up. Detailed data regarding case history, clinical situation, treatment course and complications will be presented. All cases were investigated through a telephone protocol or examination on an outpatient basis by the same person. The group contained 180 hips and 102 knees. The median age was 68, (22/91). The median follow-up time was 31 months, (51/15). Results: The primary success rate (dismissal from hospital) was 99% (1 death, 1 disarticulation). The revised rate of success in the course of the follow-up was 92,7 %. Conclusion: With a success rate of 92,7 % this example of the one stage performance proves the standard realisation possibility of this concept. Advantages like no impairment in the interval without prosthesis, reduced complication and risk possibilities and reduced costs speak for this procedure. Nevertheless prerequisites have to be respected. But after all the one stage procedure should be made available to all patients with the appropriate indication

Aims. The number of revision arthroplasties being performed in the elderly is expected to rise, including revision for infection. The primary aim of this study was to measure the treatment success rate for octogenarians undergoing revision total hip arthroplasty (THA) for periprosthetic joint infection (PJI) compared to a younger cohort. Secondary outcomes were complications and mortality. Methods. Patients undergoing one- or two-stage revision of a primary THA for PJI between January 2008 and January 2021 were identified. Age, sex, BMI, American Society of Anesthesiologists grade, Charlson Comorbidity Index (CCI), McPherson systemic host grade, and causative organism were collated for all patients. PJI was classified as ‘confirmed’, ‘likely’, or ‘unlikely’ according to the 2021 European Bone and Joint Infection Society criteria. Primary outcomes were complications, reoperation, re-revision, and successful treatment of PJI. A total of 37 patients aged 80 years or older and 120 patients aged under 80 years were identified. The octogenarian group had a significantly lower BMI and significantly higher CCI and McPherson systemic host grades compared to the younger cohort. Results. The majority of patients were planned to undergo two-stage revision, although a significantly higher proportion of the octogenarians did not proceed with the second stage (38.7% (n = 12) vs 14.8% (n = 16); p = 0.003). Although there was some evidence of a lower complication rate in the younger cohort, this did not reach statistical significance (p = 0.065). No significant difference in reoperation (21.6% (n = 8) vs 25.0% (n = 30); p = 0.675) or re-revision rate (8.1% (n = 3) vs 16.7% (n = 20); p = 0.288) was identified between the groups. There was no difference in treatment success between groups (octogenarian 89.2% (n = 33) vs control 82.5% (n = 99); p = 0.444). Conclusion. When compared to a younger cohort, octogenarians did not show a significant difference in complication, re-revision, or treatment success rates. However, given they are less likely to be eligible to proceed with second stage revision, consideration should be given to either single-stage revision or use of an articulated spacer to maximize functional outcomes. Introduction. Serum erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP), and synovial fluid white blood cell (WBC) count and differential are effective in diagnosing periprosthetic joint infection (PPJI); however their utility in patients with inflammatory arthritis is unknown. The purpose of this study is to determine the utility of these tests in patients with inflammatory arthritis. Methods. 934 Consecutive revision hip and knee arthroplasties were prospectively evaluated for PPJI. 202 Cases were excluded due to acute post-operative or hematogenous infection. 690 Patients had non-inflammatory and 42 had inflammatory arthritis. Receiver operating characteristic (ROC) curves were used to establish optimal ESR, CRP, WBC, and % neutrophil values for diagnosis of PPJI, and the area under the curve (AUC) was calculated to determine the overall accuracy. Results. The optimal thresholds for predicting PPJI were ESR 30mm/hr, CRP 17mg/L, WBC 2667, and differential 75% neutrophils in inflammatory arthritis, and ESR 32mm/hr, CRP 15mg/L, WBC 4000, and 78% neutrophils in non-inflammatory arthritis. The efficacy of these tests was similar in both populations (AUC for inflammatory ESR=86.2%, CRP=86.2%, WBC=93.8, 93.6% neutrophils; AUC for non-inflammatory ESR=85.2%, CRP=90.2%, WBC=94.5, 95% neutrophils); there was no significant difference between groups (ESR p = 0.861, CRP p= 0.549, WBC p=0.8315, % neutrophils p=0.7021). The rate of PPJI was significantly higher in patients with inflammatory (33.3%) than non-inflammatory (18.8%) arthritis (p-value=0.013). Conclusions. These results suggest that the ESR and CRP are useful in diagnosing PPJI in patients with inflammatory as well as non-inflammatory arthritis with similar optimal cut-off values

Aims. This study aimed to evaluate the clinical application of the PJI-TNM classification for periprosthetic joint infection (PJI) by determining intraobserver and interobserver reliability. To facilitate its use in clinical practice, an educational app was subsequently developed and evaluated. Methods. A total of ten orthopaedic surgeons classified 20 cases of PJI based on the PJI-TNM classification. Subsequently, the classification was re-evaluated using the PJI-TNM app. Classification accuracy was calculated separately for each subcategory (reinfection, tissue and implant condition, non-human cells, and morbidity of the patient). Fleiss’ kappa and Cohen’s kappa were calculated for interobserver and intraobserver reliability, respectively. Results. Overall, interobserver and intraobserver agreements were substantial across the 20 classified cases. Analyses for the variable ‘reinfection’ revealed an almost perfect interobserver and intraobserver agreement with a classification accuracy of 94.8%. The category 'tissue and implant conditions' showed moderate interobserver and substantial intraobserver reliability, while the classification accuracy was 70.8%. For 'non-human cells,' accuracy was 81.0% and interobserver agreement was moderate with an almost perfect intraobserver reliability. The classification accuracy of the variable 'morbidity of the patient' reached 73.5% with a moderate interobserver agreement, whereas the intraobserver agreement was substantial. The application of the app yielded comparable results across all subgroups. Conclusion. The PJI-TNM classification system captures the heterogeneity of PJI and can be applied with substantial inter- and intraobserver reliability. The PJI-TNM educational app aims to facilitate application in clinical practice. A major limitation was the correct assessment of the implant situation. The use of megaprostheses is accompanied with periprosthetic infection in up to 15% of cases. Among metals with antimicrobial activity, silver has raised the interest of investigators because of its good antimicrobial activity. The aim of this study was to determine the infection rate of silver-coated megaprostheses in comparision to uncoated titanium prostheses. We prospectively identified 40 patients who were treated with a silver-coated proximal femur (n=17) or proximal tibia (n=23) replacement (Mutars. ®. , Implantcast, Germany). Patients with a silver-coated tumor endoprosthesis were compared with 74 (proximal femur replacement n=33, proximal tibia n=41) retrospectively assessed patients with a titanium endoprosthesis regarding the number of infections. In the titanium group a proximal femur replacement was associated with the highest infection rate (18.2%; time of infection in mean 15 months postoperatively). In the silver-group infection could be reduced to 5.9% (time of infection 12 months postoperatively). In patients with a proximal tibia replacement the infection rate could be reduced from 17.1% (time of infection in mean 28 months postoperatively) to 4.3% (time of infection 4 months postoperatively) in the silver group. Regarding the final, successful treatment of infection it can be stated that in the silver group the patients could be treated either by intravenous antibiotics only or by a one-stage exchange of the prosthetic body. In the titanium group seven patients (53%) were treated by a two-stage reimplantation of the prosthesis, in 4 patients (31%) an amputation and in one patient rotationplasty was performed. We conclude that silver-coated megaendoprostheses can reduce the risk of infection on a short-term followup. Importantly, minor revisions in the case of infection in patients with a silver-coated prostheses were more often successful. Further studies with more patients and a longer followup are necessary in order to evaluate the possible benefit of silver exactly

Periprosthetic joint infection (PJI) remains an extremely challenging complication. We have focused on this issue more over the last decade than previously, but there are still many unanswered questions. We now have a workable definition that everyone should align to, but we need to continue to focus on identifying the organisms involved. Surgical strategies are evolving and care is becoming more patient-centred. There are some good studies under way. There are, however, still numerous problems to resolve, and the challenge of PJI remains a major one for the orthopaedic community. This annotation provides some up-to-date thoughts about where we are, and the way forward. There is still scope for plenty of research in this area. Aims. Despite numerous studies focusing on periprosthetic joint infections (PJIs), there are no robust data on the risk factors and timing of metachronous infections. Metachronous PJIs are PJIs that can arise in the same or other artificial joints after a period of time, in patients who have previously had PJI. Methods. Between January 2010 and December 2018, 661 patients with multiple joint prostheses in situ were treated for PJI at our institution. Of these, 73 patients (11%) developed a metachronous PJI (periprosthetic infection in patients who have previously had PJI in another joint, after a lag period) after a mean time interval of 49.5 months (SD 30.24; 7 to 82.9). To identify patient-related risk factors for a metachronous PJI, the following parameters were analyzed: sex; age; BMI; and pre-existing comorbidity. Metachronous infections were divided into three groups: Group 1, metachronous infections in ipsilateral joints; Group 2, metachronous infections of the contralateral lower limb; and Group 3, metachronous infections of the lower and upper limb. Results. We identified a total of 73 metachronous PJIs: 32 PJIs in Group 1, 38 in Group 2, and one in Group 3. The rate of metachronous infection was 11% (73 out 661 cases) at a mean of four years following first infection. Diabetes mellitus incidence was found significantly more frequently in the metachronous infection group than in non-metachronous infection group. The rate of infection in Group 1 (21.1%) was significantly higher (p = 0.049) compared to Groups 2 (6.2%) and 3 (3%). The time interval of metachronous infection development was shorter in adjacent joint infections. Concordance between the bacterium of the first PJI and that of the metachronous PJI in Group 1 (21/34) was significantly higher than Group 2 (13/38; p = 0.001). Conclusion. The findings of this study suggest that metachronous PJI occurs in more than one in ten patients with an index PJI. Female patients, diabetic patients, and patients with a polymicrobial index PJI are at significantly higher risk for developing a metachronous PJI. Furthermore, metachronous PJIs are significantly more likely to occur in an adjacent joint (e.g. ipsilateral hip and knee) as opposed to a more remote site (i.e. contralateral or upper vs lower limb). Additionally, adjacent joint PJIs occur significantly earlier and are more likely to be caused by the same bacteria as the index PJI. Cite this article: Bone Joint J 2023;105-B(3):294–300

Aims. The diagnosis of periprosthetic joint infection (PJI) continues to present a significant clinical challenge. New biomarkers have been proposed to support clinical decision-making; among them, synovial fluid alpha-defensin has gained interest. Current research methodology suggests reference methods are needed to establish solid evidence for use of the test. This prospective study aims to evaluate the diagnostic accuracy of high-performance liquid chromatography coupled with the mass spectrometry (LC-MS) method to detect alpha-defensin in synovial fluid. Methods. Between October 2017 and September 2019, we collected synovial fluid samples from patients scheduled to undergo revision surgery for painful total knee arthroplasty (TKA). The International Consensus Meeting criteria were used to classify 33 PJIs and 92 aseptic joints. LC-MS assay was performed to measure alpha-defensin in synovial fluid of all included patients. Sensitivity, specificity, positive predictive value, negative predictive value, and the area under the receiver operating characteristic curve (AUC) were calculated to define the test diagnostic accuracy. Results. The AUC was 0.99 (95% confidence interval (CI) 0.98 to 1.00). Receiver operating characteristic (ROC) analysis showed that the optimal cut-off value of synovial fluid alpha-defensin was 1.0 μg/l. The sensitivity of alpha-defensin was 100% (95% CI 96 to 100), the specificity was 97% (95% CI 90 to 98), the positive predictive value was 89.2% (95% CI 82 to 94), and negative predictive value was 100% (95% CI 96 to 100). ROC analysis demonstrated an AUC of 0.99 (95% CI 0.98 to 1.0). Conclusion. The present study confirms the utility of alpha-defensin in the synovial fluid in patients with painful TKA to select cases of PJI. Since LC-MS is still a time-consuming technology and is available in highly specialized laboratories, further translational research studies are needed to take this evidence into routine procedures and promote a new diagnostic approach. Aims. Periprosthetic joint infection (PJI) demonstrates the most feared complication after total joint replacement (TJR). The current work analyzes the demographic, comorbidity, and complication profiles of all patients who had in-hospital treatment due to PJI. Furthermore, it aims to evaluate the in-hospital mortality of patients with PJI and analyze possible risk factors in terms of secondary diagnosis, diagnostic procedures, and complications. Methods. In a retrospective, cross-sectional study design, we gathered all patients with PJI (International Classification of Diseases (ICD)-10 code: T84.5) and resulting in-hospital treatment in Germany between 1 January 2019 and 31 December 2022. Data were provided by the Institute for the Hospital Remuneration System in Germany. Demographic data, in-hospital deaths, need for intensive care therapy, secondary diagnosis, complications, and use of diagnostic instruments were assessed. Odds ratios (ORs) with 95% confidence intervals (CIs) for in-hospital mortality were calculated. Results. A total of 52,286 patients were included, of whom 1,804 (3.5%) died. Hypertension, diabetes mellitus, and obesity, the most frequent comorbidities, were not associated with higher in-hospital mortality. Cardiac diseases as atrial fibrillation, cardiac pacemaker, or three-vessel coronary heart disease showed the highest risk for in-hospital mortality. Postoperative anaemia occurred in two-thirds of patients and showed an increased in-hospital mortality (OR 1.72; p < 0.001). Severe complications, such as organ failure, systemic inflammatory response syndrome (SIRS), or septic shock syndrome showed by far the highest association with in-hospital mortality (OR 39.20; 95% CI 33.07 to 46.46; p < 0.001). Conclusion. These findings highlight the menace coming from PJI. It can culminate in multi-organ failure, SIRS, or septic shock syndrome, along with very high rates of in-hospital mortality, thereby highlighting the vulnerability of these patients. Particular attention should be paid to patients with cardiac comorbidities such as atrial fibrillation or three-vessel coronary heart disease. Risk factors should be optimized preoperatively, anticoagulant therapy stopped and restarted on time, and sufficient patient blood management should be emphasized. Aims. The aims of this study were to determine the incidence and factors for developing periprosthetic joint infection (PJI) following hemiarthroplasty (HA) for hip fracture, and to evaluate treatment outcome and identify factors associated with treatment outcome. Methods. A retrospective review was performed of consecutive patients treated for HA PJI at a tertiary referral centre with a mean 4.5 years’ follow-up (1.6 weeks to 12.9 years). Surgeries performed included debridement, antibiotics, and implant retention (DAIR) and single-stage revision. The effect of different factors on developing infection and treatment outcome was determined. Results. A total of 1,984 HAs were performed during the study period, and 44 sustained a PJI (2.2%). Multiple logistic regression analysis revealed that a higher CCI score (odds ratio (OR) 1.56 (95% confidence interval (CI) 1.117 to 2.187); p = 0.003), peripheral vascular disease (OR 11.34 (95% CI 1.897 to 67.810); p = 0.008), cerebrovascular disease (OR 65.32 (95% CI 22.783 to 187.278); p < 0.001), diabetes (OR 4.82 (95% CI 1.903 to 12.218); p < 0.001), moderate-to-severe renal disease (OR 5.84 (95% CI 1.116 to 30.589); p = 0.037), cancer without metastasis (OR 6.42 (95% CI 1.643 to 25.006); p = 0.007), and metastatic solid tumour (OR 15.64 (95% CI 1.499 to 163.087); p = 0.022) were associated with increasing PJI risk. Upon final follow-up, 17 patients (38.6%) failed initial treatment and required further surgery for HA PJI. One-year mortality was 22.7%. Factors associated with treatment outcome included lower preoperative Hgb level (97.9 g/l (SD 11.4) vs 107.0 g/l (SD 16.1); p = 0.009), elevated CRP level (99.1 mg/l (SD 63.4) vs 56.6 mg/l (SD 47.1); p = 0.030), and type of surgery. There was lower chance of success with DAIR (42.3%) compared to revision HA (66.7%) or revision with conversion to total hip arthroplasty (100%). Early-onset PJI (≤ six weeks) was associated with a higher likelihood of treatment failure (OR 3.5 (95% CI 1.2 to 10.6); p = 0.007) along with patients treated by a non-arthroplasty surgeon (OR 2.5 (95% CI 1.2 to 5.3); p = 0.014). Conclusion. HA PJI initially treated with DAIR is associated with poor chances of success and its value is limited. We strongly recommend consideration of a single-stage revision arthroplasty with cemented components. Cite this article: Bone Jt Open 2022;3(12):924–932

Aims. To investigate the optimal thresholds and diagnostic efficacy of commonly used serological and synovial fluid detection indexes for diagnosing periprosthetic joint infection (PJI) in patients who have rheumatoid arthritis (RA). Methods. The data from 348 patients who had RA or osteoarthritis (OA) and had previously undergone a total knee (TKA) and/or a total hip arthroplasty (THA) (including RA-PJI: 60 cases, RA-non-PJI: 80 cases; OA-PJI: 104 cases, OA-non-PJI: 104 cases) were retrospectively analyzed. A receiver operating characteristic curve was used to determine the optimal thresholds of the CRP, ESR, synovial fluid white blood cell count (WBC), and polymorphonuclear neutrophil percentage (PMN%) for diagnosing RA-PJI and OA-PJI. The diagnostic efficacy was evaluated by comparing the area under the curve (AUC) of each index and applying the results of the combined index diagnostic test. Results. For PJI prediction, the results of serological and synovial fluid indexes were different between the RA-PJI and OA-PJI groups. The optimal cutoff value of CRP for diagnosing RA-PJI was 12.5 mg/l, ESR was 39 mm/hour, synovial fluid WBC was 3,654/μl, and PMN% was 65.9%; and those of OA-PJI were 8.2 mg/l, 31 mm/hour, 2,673/μl, and 62.0%, respectively. In the RA-PJI group, the specificity (94.4%), positive predictive value (97.1%), and AUC (0.916) of synovial fluid WBC were higher than those of the other indexes. The optimal cutoff values of synovial fluid WBC and PMN% for diagnosing RA-PJI after THA were significantly higher than those of TKA. The specificity and positive predictive value of the combined index were 100%. Conclusion. Serum inflammatory and synovial fluid indexes can be used for diagnosing RA-PJI, for which synovial fluid WBC is the best detection index. Combining multiple detection indexes can provide a reference basis for the early and accurate diagnosis of RA-PJI. Cite this article: Bone Joint Res 2023;12(9):559–570

This meta analysis address the relationship between infection developing after total hip arthroplasty (THA) and heterotopic ossification (HO). To identify the gaps in available knowledge, we screened for full-length peer-reviewed research articles listed in PubMed, Embase, and Web of Science over the past 20 years. The following search terms and Boolean operators were used: heterotopic ossification AND infection AND (hip replacement OR hip arthroplasty). The search resulted in the identification of as few as 14 articles describing periprosthetic joint infection (PJI) and HO after THA. Data summarized from 6 studies suitable for further meta-analysis yielded a cumulative sample size of 753 observations, with 186 recorded events of HO. The pooled RR was estimated at 2.22 (95% CI: 1.00 to 4.91, p = 0.0497), suggesting a more than twofold risk of HO compared to the group without PJI. In conclusion, there is a clear association between a higher risk of HO and PJI. Basic research findings support the hypothesis that bacterial pathogen-associated molecular patterns (PAMPs) can lead to osteogenesis through a toll-like receptor (TLR) and nuclear factor kappa B (NF-κB) pathway in the course of HO development. Together, these results suggest that HO prophylaxis should always be prescribed in PJI after THA. Moreover, during revisions following THA for presumed non-septic reasons, the presence of HO warrants consideration for infection, as there is a potential heightened risk of pathologic ossification induced by PAMPs.
