Background

FORECAST is a prospective longitudinal cohort study exploring mechanism-based prognostic factors for pain persistence in sciatica. Here, we share an update on this largest deeply-phenotyped primary care sciatica cohort.

Methods/results

Our cohort includes 180 people with sciatica (score >4 on Stynes’ Sum Score), aged 18–85, within 3 months of symptom onset. Psychosocial factors, self-reported sensory profiling, clinical examination, quantitative sensory testing (QST), biological samples (blood and skin samples), and Magnetic Resonance Neurography of lumbar nerve roots were collected at baseline. Pain persistence was determined at three and twelve months with the Sciatica Bothersomeness Index (SBI) and a numeric pain rating scale (NRS) as primary outcomes. Recruitment nears completion, with 160 participants enrolled to date. 127 and 96 participants have completed 3 and 12 months follow-up respectively.

Overall, 56% of our cohort are female, with a mean age (SD) of 54.14yrs (16.57). Ethnicity data approximates local populations.

SBI at baseline was (median [IQR]) 13[10-17], and interim longitudinal data shows stepwise improvement at 3 and 12 months. Baseline ‘average’ pain intensity was 5.56 (2.15) for leg pain, and 4.14(2.82) for low back pain (LBP). Overall, pain scores decreased at 3 and 12 months, with greater reductions in leg pain than LBP at 12 months. However, around 55–80% and 40–65% of people reported persistent pain at 3 and 12 months respectively.

Management of highly displaced acromioclavicular joint (ACJ) injuries remain contentious. It is unclear if delayed versus acute reconstruction has an increased risk of fixation failure and complications. The primary aim of this was to compare complications of early versus delayed reconstruction. The secondary aim was to determine modes of failure of ACJ reconstruction requiring revision surgery.

A retrospective study was performed of all patients who underwent operative reconstruction of ACJ injuries over a 10-year period (Rockwood III-V). Reconstruction was classed as early (<12 weeks from injury) or delayed (≥12 weeks). Patient demographics, fixation method and post-operative complications were noted, with one-year follow-up a minimum requirement for inclusion. Fixation failure was defined as loss of reduction requiring revision surgery.

104 patients were analysed (n=60 early and n=44 delayed). Mean age was 42.0 (SD 11.2, 17–70 years), 84.6% male and 16/104 were smokers. No difference was observed between fixation failure (p=0.39) or deep infection (p=0.13) with regards to acute versus delayed reconstruction. No patient demographic or timing of surgery was predictive of fixation failure on regression modelling. Overall, eleven patients underwent revision surgery for loss of reduction and implant failure (n=5 suture fatigue, n=2 endo-button escape, n=2 coracoid stress fracture and n=2 deep infection).

This study suggests that delayed ACJ reconstruction does not have a higher incidence of fixation failure or major complications compared to acute reconstruction. For those patients with ongoing pain and instability following a trial of non-operative treatment, delayed reconstruction would appear to be a safe treatment approach.

The aim of this study was to define the long-term outcome following adolescent clavicle fracture.

We retrospectively reviewed all adolescent fractures presenting to our region (13–17years) over a 10-year period. Patient reported outcomes were undertaken at a minimum of 4 years post-injury (QuickDASH and EQ-5D) in completely displaced midshaft fractures (Edinburgh 2B, >2cm displacement, n=50) and angulated midshaft fractures (Edinburgh 2A2, >30 degrees angulation, n=32).

677 clavicle fractures were analysed. The median age was 14.8 (IQR 14.0–15.7) and 89% were male. The majority were midshaft (n=606, 89.5%) with either angulation (39.8%) or simple fully displaced (39.1%). Only 3% of midshaft fractures underwent acute fixation (n=18/606), all of which were fully displaced. The incidence of refracture following non-operative management of midshaft fractures was 3.2% (n=19/588), all united with non-operative management. Fracture type, severity of angulation or displacement was not associated with risk of refracture. There was one case of non-union encountered following non-operative management of all displaced midshaft fractures (0.4%, n=1/245). At a mean of 7.6 years following injury, non-operative management of both displaced and angulated fractures had a median QuickDASH was 0.0 (IQR 0.0–2.3), EQ-5D was 1.0 (IQR 1.0–1.0). 97% of angulated fractures and 94% of displaced fractures were satisfied with their final shoulder function.

We conclude that Non-operative management of adolescent midshaft clavicle fractures result in excellent functional outcomes with a low rate of complications at long-term follow up. The relative indications for surgical intervention for clavicle fractures in adults do not appear to be applicable to adolescents.

This prospective, randomized, controlled trial compares patient outcome after non-operative care versus open reduction and tunneled suspension device fixation (ORTSD) for grade III or IV acromioclavicular joint disruptions.

Sixty patients aged between sixteen and thirty-five years with an acute grade III or IV AC joint disruption were randomized to receive ORTSD fixation or non-operative treatment. Functional assessment was conducted at six weeks, three months, six months, and one year using the Disabilities of the Arm, Shoulder and Hands (DASH), Oxford Shoulder Scores (OSS) and Short Form (SF-12). Reduction was evaluated using radiographs. Complications were recorded, and an economic evaluation performed.

There was no significant difference in DASH or OSS at one year between non-operative and ORTSD groups (DASH score, 4.67 versus 5.63; OSS, 45.72 versus 45.63). Patients undergoing surgery had inferior DASH scores at 6 weeks (p<0.01). Five patients who failed non-operative management subsequently received surgery. Overall cost of treatment was significantly greater after ORTSD fixation (£796.22 vs £3359.73 (p<0.01)).

ORTSD fixation confers no functional benefit over non-operative treatment at one year. While patients managed non-operatively generally recover faster, a significant group remain dissatisfied following non-operative treatment requiring delayed surgical reconstruction.

To evaluate if clinical recovery following midshaft clavicle fracture is associated with nonunion and determine if this has superior predictive value compared to estimation at time of injury.

A prospective study of all patients (≥16 years) who sustained a displaced midshaft clavicle fracture was performed. We assessed patient demographics, injury factors, functional scores and radiographic predictors with a standardized protocol at six-weeks. Conditional-stepwise regression was used to assess which factors independently predicted nonunion at six-months post-injury determined by CT. The nonunion predictor six-week model (NUP6) was compared against a previously validated model based on factors available at time of injury (NUP0-smoking, comminution and fracture displacement).

200 patients completed follow-up at six months. The nonunion rate was 14% (27/200). Of the functional scores, the QuickDASH had the highest accuracy on receiver-operator-characteristic (ROC) curve analysis with a 39.8 threshold, above which was associated with nonunion (Area Under Curve (AUC) 76.8%, p<0.001).

On regression modelling QuickDASH ≥40 (p=0.001), no callus on radiograph (p=0.004) and fracture movement on examination (p=0.001) were significant predictors of nonunion. If none were present the predicted nonunion risk was 3%, found in 40% of the cohort (n=80/200). Conversely if two or more were present, found in 23.5% of the cohort, the predicted nonunion risk was 60%.

The NUP6 model appeared to have superior accuracy when compared to the NUP0 model on ROC curve analysis (AUC 87.3% vs 64.8% respectively).

Delayed assessment at six-weeks following displaced clavicle fracture enables a more accurate prediction of fracture healing.


Fractures of the lateral clavicle with complete displacement have a high non-union rate and are associated with poor functional outcomes following non-operative treatment. Various operative techniques are available but preliminary studies of open reduction and tunnelled suspensory device (ORTSD) fixation report good early functional outcomes with a low rate of complications. This study assesses the functional outcomes in a large series of patients treated using ORTSD.

After surgical reconstruction in 67 patients, outcomes were assessed using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire and Oxford score at six weeks, and three, six and twelve months post-operatively. 55 of 64 surviving patients were contacted at a mean of 69 (27–120) months to complete DASH and Oxford scores, evaluate overall satisfaction, and document any complications.

At one year post-operatively, the mean Oxford score was 46.4 and mean DASH score was 2.4 points (59/67 patients assessed). At a mean of 69 months after surgery, the mean Oxford score was 46.5 and mean DASH score was 2.2 (55 surviving and contactable patients). There were no significant differences between the one-year functional scores and those at the latest follow-up. Two patients developed symptomatic non-union requiring re-operation, and two developed an asymptomatic fibrous union not requiring surgery. The five-year survival when considering only obligate revision for implant-related complications was 97.0%.

ORTSD fixation for isolated displaced lateral-end clavicle fractures in medically-fit patients is associated with good functional outcomes, and a low rate of medium-term complications. Routine removal of the implant was not necessary.

The primary aim of this study was to undertake a cost-effectiveness analysis (CEA) of acute fixation versus conservative management of displaced midshaft clavicle fractures. The secondary aim was to conduct a sensitivity analysis of patient characteristics that may influence a threshold of £20,000 per quality-adjusted life year gained (QALY).

A CEA was conducted from a randomised control trial comparing conservative management (n=92) to acute plate fixation (n=86) of displaced midshaft clavicle fractures. The incremental cost effectiveness ratio (ICER) was used to express the cost per QALY. The short form 6-dimensional (SF-6D) score was the preference based index to calculate the cost per QALY.

The 12-month SF-6D advantage of acute fixation over conservative management was 0.0085 (p=0.464) with a mean cost difference of £4,096.22 and resultant ICER of £481,908.24/QALY. For a threshold of £20,000/QALY the benefit of acute fixation would need to be present for 24.1 years. Linear regression analysis identified nonunion as the only independent factor to influence the SF-6D at 12-months (p<0.001). Conservatively managed fractures that resulted in a nonunion (n=16) had a significantly worse SF-6D compared to acute fixation (0.0723, p=0.001) with comparable healthcare cost at 12-months (£170.12 difference). Modelling the ICER of acute fixation against those complicated by a nonunion proved to be cost effective at £2,352.97/QALY at 12-months.

Routine plate fixation of displaced midshaft clavicle fractures is not cost-effective. Patients with nonunion after conservative management have increased morbidity with comparable expense to those undergoing acute fixation which suggests targeting these patients is a more cost-effective strategy.

This study aimed to evaluate the effect of clavicular shortening, measured by three-dimensional computerized tomography (3DCT), on functional outcomes and satisfaction in patients with healed, displaced, midshaft clavicle fractures up to one year following injury.

The data used in this study were collected as part of a multicenter, prospective randomized control trial comparing open reduction and plate fixation with nonoperative treatment for displaced midshaft clavicle factures. Patients who were randomized to nonoperative treatment and who had healed by one year were included. Clavicle shortening relative to the uninjured contralateral clavicle was measured on 3DCT. Outcome analysis was conducted at six weeks, three months, six months and one year following injury and included the Disabilities of the Arm, Shoulder and Hand (DASH), Constant and Short Form-12 (SF-12) scores, and patient satisfaction.

48 patients were included. The mean shortening of injured clavicles, relative to the contralateral side, was 11mm (+/− 7.6mm) with a mean proportional shortening of 8percnt;. Proportional shortening did not significantly correlate with the DASH (p>0.42), Constant (p>0.32) or SF-12 (p>0.08) scores at any time point. There was no significant difference in the mean DASH or Constant scores at any followup time point both when the cut off for shortening was defined as one centimeter (p>0.11) or two centimeters (p>0.35). There was no significant difference in clavicle shortening between satisfied and unsatisfied patients (p>0.49).

This study demonstrated no association between shortening and functional outcome or satisfaction in patients with healed, displaced, midshaft clavicle fractures up to one year following injury.

The natural history of primary anterior glenohumeral dislocation in adolescent patients remains unclear and no consensus exists for management of these patients. The study objectives were to report the natural history following primary anterior glenohumeral joint dislocation in adolescent patients and to identify risk factors for repeat dislocation.

We reviewed prospectively-collected clinical and radiological data of 133 adolescent patients (mean age 16.3 years (range 13–18); 115 male patients (86.5%)) diagnosed with primary anterior glenohumeral joint dislocation and managed nonoperatively from 1996 to 2008 at our institution (mean follow-up 95.2 months (range 1–215)).

During follow-up, 102 (absolute incidence of 76.7%) patients experienced repeat dislocation. Median time interval between primary and repeat dislocation was 10 months (CI: 7.4 – 12.6). On survival analysis, 59% (CI: 51.2 – 66.8%) of patients remained stable one year following initial injury, 38% (CI: 30.2 – 45.8%) after two years, 21% (CI: 13.2- 28.8%) after five years, and 7% (CI: 1.1–12.9%) after 10 years. Neither age nor gender significantly predicted repeat dislocation during follow-up.

In conclusion, adolescent patients with primary anterior glenohumeral joint dislocations have a high rate of repeat dislocation, which usually occurs within two years of initial injury, and these patients should be considered early for operative stabilisation.

First-time anterior dislocation of the shoulder is associated with the development of recurrent instability. It is recognised that patients with recurrent instability often have osseous defects. Using 3D computerised tomography (3DCT) it is possible to quantify these defects. Whether these defects are present after the primary dislocation or occur progressively from multiple dislocations is unclear. We correlated the presence of Hill-Sachs lesions and anterior glenoid bone loss with evidence of recurrent dislocation and clinical outcomes.

78 patients were followed up for two years. All underwent a 3DCT within a week of injury. Standardised images of the humeral head and glenoid were produced. Using standardised digital techniques bone loss was measured.

39% of the patients developed further instability. Average Hill-Sachs circumferential length = 15.23%. Average Hill-Sachs surface area = 5.53%. The length and surface area of the Hill-Sachs lesions were significantly associated with further instability. (p=0.019 and p=0.003). Average en face glenoid surface area loss=1.30% with no association to instability (p=0.685). There was poor correlation between the size of the glenoid lesion and the size of the Hill-Sachs lesion.

Results showed that age and increasing size of the Hill-Sachs lesions result in a higher rate of instability. Interestingly glenoid bone loss was relatively low and did not predict recurrent instability. The size of the Hill-Sachs lesion does not have a linear relationship with glenoid bone loss. Further work defining the morphology of the Hill-Sachs lesion and its engagement with a glenoid defect is required.

Non-union has traditionally been considered a rare complication following the non-operative management of clavicle fractures. A growing body of evidence has demonstrated higher rates of non-union in adults with displaced fractures. However, the variables that predict non-union in these patients remain unclear. We evaluated the prevalence and risk factors for non-union following displaced midshaft clavicle fractures in a large consecutive series of patients managed non-operatively in our Unit.

Materials and Methods

1097 consecutive adults (mean age 26.1yrs) with displaced midshaft clavicle fractures treated non-operatively in our Unit were included. All patients were interviewed and examined by an orthopaedic trauma surgeon and underwent radiological assessment within a week of injury. All patients were managed in a simple sling for two weeks followed by early mobilization. All patients were followed-up until clinical and radiological confirmation of union. Non-union was defined clinically as the presence of pain or mobility of the fracture segments on stressing, and radiologically as failure of cortical bridging by 6 months.

Background

The functional outcome and risk of recurrence following arthroscopic stabilisation for recurrent anterior shoulder instability is poorly defined in large prospective outcome studies. This is the first study to prospectively evaluate these outcomes in patients who have been treated using this technique.

Methods

We performed a prospective study of a consecutive series of 302 patients (265 men and 37 women, mean age 26.4 years) who underwent 311 (9 bilateral) arthroscopic Bankart repairs for recurrent anterior instability. Patients were evaluated preoperatively and postoperatively at 6 months, and annually thereafter. The chief outcome measures were risk of recurrence and the two-year functional outcomes (assessed using the WOSI and DASH scores).

Background

Proximal humeral fractures are common and a minority develop non-union, which can result in pain and disability. We aimed to identify the risk factors and quantify the prevalence of non-union.

Methods

A thirteen-year retrospective study of 7039 patients with proximal humeral fractures was performed and a database created. 246 patients with non-union were compared to a control group to identify risk factors. Logistic regression analyses were performed to identify significant variables obtained at presentation to predict non-union.

Aims

The aim of this study was to compare biomechanical properties of pre-contoured plate fixation using different screw fixation modes in a mid-shaft clavicle fracture model.

Methods

Fourth generation biomechanical clavicle sawbones with a mid-shaft osteotomy were plated in one of three modes: nonlocking bicortical, locking bicortical and locking unicortical mode. The specimens were then tested to failure in four-point bending and pull-off tests.

Background

Acute anterior dislocation of the glenohumeral joint may be complicated by injury to neighboring structures. These injuries are best considered a spectrum of injury ranging from an isolated dislocation (unifocal injury), through injuries associated with either nerve or osteoligamentous injury (bifocal injury), to injuries where there is evidence of both nerve and osteoligamentous injury. The latter combination has previously been described as the “terrible triad,” although we prefer the term “trifocal,” recognizing that this is the more severe end of an injury spectrum and avoiding confusion with the terrible triad of the elbow. We evaluated the prevalence and risk factors for nerve and osteoligamentous injuries associated with an acute anterior glenohumeral dislocation in a large consecutive series of patients treated in our Unit.

Materials and Methods

3626 consecutive adults (mean age 48yrs) with primary traumatic anterior shoulder dislocation treated at our unit were included. All patients were interviewed and examined by an orthopaedic trauma surgeon and underwent radiological assessment within a week of injury. Where rotator cuff injury or radiologically-occult greater tuberosity fracture was suspected, urgent ultrasonography was used. Deficits in neurovascular function were assessed clinically, with electrophysiological testing reserved for equivocal cases.

Background

Hyperlaxity is associated with a high incidence of shoulder dislocations. Collagen V regulates the diameter of fibrils of the abundant collagen type I. Decorin and biglycan are members of the small leucine rich proteoglycans(SLRP's)family and play important roles in the regulation of collagen fibrillogenesis. The aim of this study was to identify if there was a link in hyperlaxity, capsule strength, collagen V and SLRP's expression.

Methods

Data was collected for 10 patients undergoing open shoulder stabilization for recurrent instability. Beighton score was used to assess hyperlaxity. Localization of Collagen V and SLRP's was studied by immunohistochemical staining of paraffin embedded sections of shoulder capsule. Grading of the stain was done on a 0-4 scale(0=no staining and 4=strong staining>50% of the slide)by three observers. Shoulder capsules were mounted on a material testing system and vertical load was applied to reach yield.

198 (18%) of patients had evidence of non-union at 6 months. Patient factors associated with non-union included increasing age, smoking and the presence of medical comorbidities (p<0.05). Injury-related factors associated with non-union included increasing fragment translation and displacement, and injury pattern (Edinburgh 2B2: comminuted segmental fracture)(p<0.01).

We present the largest series reporting prevalence and risk factors for non-union following conservatively treated, displaced midshaft clavicle fractures. These fractures can no longer be viewed as a single clinical entity, but as a spectrum of injuries each requiring individualized assessment and treatment. Increased understanding of the outcomes of these injuries will enable clinicians to better identify those patients that may be better served with primary operative reconstruction.

Damage Control techniques involve primary external fracture fixation to reduce the ‘second hit’ of surgery. This study used a large animal (ovine) trauma model to compare pathophysiological responses of primary external femoral fixation and intramedullary stabilisation.

Under terminal anaesthesia bilateral femoral fractures and hypovolaemia were produced using a pneumatic ram. 24 sheep were randomised into 4 groups and monitored for 24 hours. Group 1 – Control; Group 2 – Trauma only; Group 3 – Trauma and external fixation; Group 4 – Trauma and reamed intramedullary stabilisation. Outcome measures: pulmonary embolic load (transoesophageal echocardiography); plasma coagulation markers; bronchoalveolar lavage differential cell counts (neutrophils, lymphocytes and macrophages).

Total embolic load was significantly higher (p<0.001) in the intramedullary fixation group (median score 42 versus 20). All trauma groups had a significant increase (p < 0.05) in prothrombin time with a fall in antithrombin III and fibrinogen levels. No significant differences occurred between trauma groups with any coagulation or alveolar lavage marker.

Intramedullary femoral fracture fixation produced a relatively higher pulmonary embolic load. However, the initial fracture fixation method did not affect any of the changes seen in the measured coagulation or inflammatory markers during the first 24 hours of injury.

Our purpose was to study the functional outcome and electrophysiologically to assess the axially nerve function in patients who have undergone surgery using a deltoid-splitting approach to treat complex proximal humeral fractures.

This was a prospective observational study and was carried out in the Shoulder injury clinic at a university teaching hospital. Over a one-year period we treated fourteen locally-resident patients (median age 59 years) who presented with a three- or four-part proximal humeral fracture. All patients were treated using the extended deltoid-splitting approach, with open reduction, bone grafting and plate osteosynthesis. All patients were prospectively reviewed and underwent functional testing using the DASH, Constant and SF-36 scores as well as spring balance testing of deltoid power, and dynamic muscle function testing. At one year after surgery, all patients underwent EMG and nerve latency studies to assess axillary nerve function.

Thirteen of the fourteen patients united their fractures without complications, and had DASH and Constant score that were good, with comparatively minor residual deficits on assessment of muscle power. Of these thirteen patients, only one had evidence of slight neurogenic change in the anterior deltoid. This patient had no evidence of anterior deltoid paralysis and her functional scores, spring balance and dynamic muscle function test results were indistinguishable from the patients with normal electrophysiological findings. One of the fourteen patients developed osteonecrosis of the humeral head nine months after surgery and had poor functional scores, without evidence of nerve injury on electrophysiological testing.

Reconstruction through an extended deltoid-splitting approach provides a useful alternative in the treatment of complex proximal humeral fractures. The approach provides good access for reduction and implant placement and does not appear to be associated with clinically-significant adverse effects.

Background: Complex anterior fracture-dislocations of shoulder are rare injuries and little is known about the functional outcomes following operative treatment. The aim of our study was to evaluate the functional outcomes following open reduction and internal fixation for these injuries.

Methods: Over a ten and a half year period, we studied thirty-two consecutive patients who presented to shoulder injury clinic at the Royal Infirmary of Edinburgh with an acute Type-I complex anterior fracture-dislocation of the shoulder. The average age of the 19 men and 13 women was 58 years (range 21–81 years). All patients were treated with open reduction and internal fixation and were followed up for at least two years. The functional outcomes were assessed by three scoring systems (Disabilities of the Arm, Shoulder and Hand questionnaire, the Constant questionnaire, and the Short Form-36 general health questionnaire).

Results: At two years post injury, the median DASH score was 26 points and the median Constant score was 83.5 points. The range of motion of the shoulder joint showed significant improvement throughout the two year period and degree of forward flexion and abduction continued to show significant improvement at five years. At two years, the mean forward flexion was 162°, mean extension was 152°, mean internal rotation was 109° and mean external rotation was 73°. All but one patient returned to their job at two years.

Conclusions: Type I complex anterior fracture-dislocations of the shoulder are rare and occur in the middle age and elderly group of population. Following open reduction and internal fixation, the functional outcomes have been shown to be satisfactory. We recommend that open reduction and internal fixation should be the first choice of treatment in Type-I complex anterior fracture-dislocation of shoulder.

Introduction: This study was performed to assess the incidence of generalized ligament laxity in patients presented with 1st time anterior shoulder dislocation.

Patients and Methods: Prospective data was collected for patients presented with 1st time anterior shoulder dislocation and clavicle fracture as a control group between Aug 2008 and Feb 2009 under the care of a specialist shoulder surgeon. Data included demographic details, mechanism of injury and generalized ligament laxity using Beighton score. Laxity is scored on a 0–9 scale. Scores of 4 or above are indicative of generalized ligament laxity. Brighton criteria was used to diagnose Benign Joint Hypermobility Syndrome (BJHS)

Results: Data was collected for 44 patients with first time anterior shoulder dislocation and 43 patients with clavicle fracture. There was no difference in the demographics of the groups. There were 40 male (91%) and 4 (9%) female patients in the dislocation group. Mean age was 25 years with a range from 15–55. Most common cause of shoulder dislocation was sports related injuries in 26 patients (60%). The average Beighton score for dislocation group was 3.6 with a range from 0–9 as compared to 2.1 with a range from 0–7 in the control group. Twenty one patients (48%) in the dislocation group had a Beighton score of 4 or more indicating generalized ligament laxity as compared to 12 patients (28%) in the control group. This difference was statistically significant with a P value of 0.009. Six patients (14%) fulfilled the Brighton criteria for BJHS in the dislocation group as compared to 3 patients (7%) in the control group.

Conclusion: We found that there is a high incidence (48%) of generalized ligament laxity in patients presented with first time anterior shoulder dislocation. Appropriate advice should be given to these patients about rehabilitation, risk of recurrent dislocations and timing of shoulder stabilization.

Purpose: This study was performed to assess the incidence of generalized ligament laxity in patients undergoing revision shoulder stabilization.

Methods and Results: Prospective data was collected for 21 patients undergoing revision shoulder stabilization and 43 patients with clavicle fractures as a control group, between 2004 and 2009 under the care of orthopaedic surgeons at the Royal Infirmary of Edinburgh including demographic details, type of primary stabilization and causes of failure.

Clinical examination was used to assess the ligament laxity using the Beighton score. Laxity is scored on a 0–9 scale. Scores of 4 or above are indicative of generalized ligament laxity. Brighton criteria is used to diagnose Benign Joint Hypermobility Syndrome (BJHS) and use signs and symptoms along with Beighton score. The most common primary procedure for shoulder instability was open stabilization in 15 patients (75%).

The most common cause of failure was trauma in 14 patients (67%). The mean age was 29 years with a range from 22–58 years. There were 16 males and 5 females. The average Beighton score for patients undergoing revision shoulder stabilization was 2.8 with a range from 0–7. 13 patients (61%) in this group had a Beighton score of 4 or more as compared to 12 patients (27%) in the control group. This difference was statistically significant with a P value of 0.018. 11 patients (52%) fulfilled the Brighton criteria for BJHS.

Conclusion: We found that there is a high incidence (61%) of generalized ligament laxity in patients undergoing revision shoulder stabilization. Trauma is a common (67%) cause of failure in these patients. Patients with generalized ligament laxity should be made aware about the high failure rate after shoulder stabilization at their primary surgery and appropriate advice about rehabilitation should be given.

Background and Aims: Plate fixation of acute mid-shaft clavicle fractures is becoming increasingly popular. However limb and life threatening complications such as injury to the subclavian vessels have been reported. One possible solution to reduce the risk of this complication is the use of unicortical screw fixation.

The aim of this study was to compare biomechanical properties of pre-contoured plate fixation using different screw fixation modes in a mid-shaft clavicle fracture model.

Methods: Fourth generation biomechanical clavicle sawbones with a mid-shaft osteotomy were plated in one of three modes: nonlocking bicortical, locking bicortical and locking unicortical mode. The specimens were then tested to failure in four-point bending and pull-off tests.

Results: Failure due to fracture through the sawbone was more common in nonlocking bicortical mode while plate bending was more common in the locking bicortical group. The ultimate load at failure was significantly lower in the locking bicortical group compared to the nonlocking bicortical group, however there was no significant difference between the locking unicortical group and nonlocking bicortical group.

In the pull-off tests 100% of nonlocking bicortical and locking bicortical plates failed by fracture of the sawbone. 100% of the locking unicortical plates failed by plate and screw pull-off from the sawbone. The load at failure was highest for the locking unicortical plate but this was not significantly different to the other groups.

Conclusion: This study shows that specimens fixed with locking unicortical screw fixation withstood comparable or superior loads in four-point bending and pull-off test when compared to nonlocking bicortical and locking bicortical screw fixation. In addition both locking screws and unicortical screws appear to provide a protective effect against periprosthetic sawbone fracture. Locking unicortical screw fixation of pre-contoured plates may be a viable alternative in the fixation of mid-shaft clavicle fractures.

Background: Transcranial Doppler Ultrasound has been used to detect cerebral micremboli following long bone fractures and intramedullary stabilization. However the clinical effects in terms of cognitive function remain unclear. We aim to measure the cerebral embolic load and to clarify clinical cognitive function following lower limb long bone fractures stabilised by reamed intramedullary fixation.

Methods: 27 femoral and tibial diaphyseal fractures (median age 36 years) were cognitively assessed 3 days following surgery and compared to the normal age and intelligence matched population. A wide range of cognitive tests assessed: global cognitive function; verbal fluency and speed; immediate and delayed memory recall; attention and mental processing speeds. 20 patients had intra-operative transcranial Doppler ultrasound monitoring of the middle cerebral artery for embolic signals. In addition a marker of neuronal injury (S-100B protein) was measured pre-operatively and at 0, 24 and 48 hours following surgery. One sample Wilcoxon signed rank test compared median (percentile) cognitive scores for the fracture patient cohort to a value of 50 representing the normal population.

Results: A significant deterioration in immediate memory recall of unstructured material was noted following surgery. Using established criteria, 4 patients had detectable cerebral emboli with a median count of 3 (range 2–9). Scatter plot graphs indicated no correlation between cerebral embolic events and clinical cognitive dysfunction. S-100B protein levels increased from a pre-operative median (interquartile range) of 0.20 (0.23) to a peak immediately following surgery of 0.51 (0.97) with no correlation to clinical cognitive dysfunction

Conclusions: A small number of cerebral embolic events occur during intramedullary fracture stabilisation but with no direct correlation made to cognitive dysfunction on detailed testing. Recent concerns over the specificity of S100B protein due to extracerebral tissue release appear to be confirmed.

Significance: Clarify cognitive function following intramedullary fracture stabilisation and correlate with cerebral (systemic) embolic load.

Study Purpose To assess clinical cognitive function and measure cerebral embolic load following primary cemented hip arthroplasty.

Methods 34 primary cemented hip arthroplasty patients (mean age 69.9 (SD 11.0)0 and no history of cerebrovascular disease, underwent cognitive assessment before and after (day 4) surgery. Testing included: Verbal fluency and speed (Control Oral Word Association Test); Working memory to assess immediate and delayed recall (Weschler Memory Scale III); Attention and mental processing speeds (Colour Trails 1& 2). A one sample Wilcoxon signed rank test compared median differences before and after surgery. A sub-group of 20 patients had intra-operative transcranial Doppler ultrasound monitoring of the middle cerebral artery for embolic signals. A marker of neuronal injury (S-100B protein) was measured pre-operatively and at 0, 24 and 48 hours following surgery.

Results A significant difference was noted in Colour Trails tests 1& 2 following hip arthroplasty with P values (C.I.) of 0.002 (−21, −4) and 0.023 (−15.5, −1.0) respectively.

Using established emboli criteria 10 (50%) patients had true cerebral emboli with a range from 1 to 550 signals (median 2.5 interquartile range (IQ) 2 to 12.5). S-100B levels increased from a pre-operative median (IQ) of 0.15 microg/L (0.12 to 0.20) to a peak immediately following surgery of 1.88(1.36 to 4.24) returning to 0.26(0.18 to 0.37) by 48 hours (normal range: 0.03–0.15). Plotted scatter charts indicated no correlation between embolic load and cognitive dysfunction or with S-100B levels following surgery.

Conclusion Cognitive testing indicates deterioration in early measured attention, visual searching and mental processing speed shortly following hip arthroplasty. No direct correlation was found between cognitive dysfunction and cerebral embolic load.

The purpose of this study was to prospectively audit the functional outcome and peri-operative complications associated with the use of a trochantericentry cephalomedullary nail to treat all low-energy subtrochanteric fractures.

Methods: Over a 95 month period, we used the Long Gamma Nail (LGN) to treat a consecutive series of 302 locally-resident patients who sustained subtrochanteric fractures during low-energy trauma. We prospectively assessed the mortality, prevalence of complications and functional outcome during the first year post-injury. We used survivorship methodology to assess the rate of re-operations and implant revision during the first year after surgery.

Results: By one year, 74 patients had died, 10 were untraceable and 7 refused to participate in follow-up. The remaining 211 patients had evaluation of their functional outcome and post-operative complications during the first year post-injury.

The prognosis following an operatively-treated subtrochanteric fracture was similar to other proximal femoral fractures, with a one-year mortality of 24.5%, and an increased level of social dependence, increased use of walking aids and reduction in mobility amongst survivors.

Re-operation was required in 27 patients (8.9%), although only 18 patients required nail revision. On survivorship analysis, 96.8% of fractures had healed by six months post-injury, and only five patients had confirmed non-unions, which were successfully treated with autogenous bone grafting. Lag-screw mechanical failure and fractures distal to the nail were seen in twelve (4%) and five (1.7%) patients respectively; all of these fractures subsequently healed after further treatment. Deep sepsis occurred in only five (1.7%) patients.

Conclusions: Subtrochanteric fractures caused by low-energy trauma have similar prognosis to other proximal femoral fractures. Trochanteric-entry cephalomedullary nails are a versatile treatment option for these injuries, and are associated with an acceptable rate of peri-operative complications and favourable functional outcome.

Background: Long bone fractures and intramedullary stabilisation can result in the extravasation of fat and marrow emboli into the venous circulation. The effects of these emboli can become systemic causing neurological features.

Aim: To establish the cerebral microembolic load following femoral and tibial diaphyseal fractures treated by intramedullary fixation and to specify any neurological impairment with the application of a series of cognitive tests and a serum marker of neuronal injury.

Methods: 20 femoral and tibial fractures treated with intramedullary fixation had intra-operative transcranial doppler ultrasound monitoring of the middle cerebral artery with emboli detection software set to established guidelines. Cognitive testing (day 3), following surgery with an I.Q. assessment (PFSIQ) allowing comparison with age specific normative data. This included: verbal fluency and speed (COWAT – Control Oral Word Association Test); working memory with assessment of immediate and delayed recall; mini-mental state examination; executive function, attention and mental processing speeds (Colour Trails 1& 2). Beta S-100 levels measured pre-operatively, 0, 24 and 48 hours following surgery as a marker of neuronal injury.

Statistical Analysis: One sample Wilcoxon signed rank test to compare median of the cognitive scores with age matched normative data. Multiple regression analysis used to correlate embolic load with cognitive function.

Results: Mean age (SD) for the group is 32 (5.8). Mean PFSIQ of 52.8%, SD 21.4 [median 59.5, IQ range 28.3, 71.3]. No significant difference detected in cognitive testing compared with normative data. Cerebral microemboli detected in 17 of 20 patients with a count median (range) of 6 (0, 29). The mean pre-operative beta S-100 level was 0.36 micro g/l (normal range 0–0.15). This increased to a peak mean of 0.88 micro g/l immediately following surgery with a poor correlation to cerebral embolic load.

Discussion: Detailed clinical testing indicates no significant deterioration in cognitive function following intramedullary stabilisation of these fractures. A variable cerebral micro-embolic load was seen but with no detectable clinical effect. No direct correlation was found between the elevated levels of Beta S-100 seen following surgery and cerebral embolic load. This appears to correlate with previous concerns in the literature regarding the specificity and sensitivity of this established marker of neuronal injury.

Background: Distal third fractures account for 10 to 15% of all clavicle fractures. Traditional management of displaced lateral third fractures has been with internal fixation. Several authors have reported higher rates of non-union and poor outcome in conservatively managed fractures. However, long term follow up of non-operated distal third fractures has shown comparable functional outcomes to those managed with internal fixation. The purpose of this retrospective study was to analyse the clinical and radiographic results of nonoperative treatment of displaced lateral clavicle fractures.

Methods: Eighty six patients with displaced lateral end clavicle fractures were treated primarily with a sling for comfort. The fractures were classified as Neer type IIa in fifty patients, type IIb in twenty nine and type III in seven. Physiotherapy was begun after the sling was removed at an average of two weeks after the injury. Patients were evaluated with regard to shoulder function and general health using a modification of the Constant score and SF-36 respectively. All patients had a repeat radiographic exam at follow up. The average duration of follow up was six years (range two to ten years).

Results: Fourteen patients developed symptoms severe enough to warrant surgery at between seven and twenty four months post-injury. Eleven had radiographically confirmed non-union and three had symptomatic osteoarthritis of the acromioclavicular joint. The remaining seventy two patients had not undergone any further surgery. Twenty one patients (29.2%) from the nonoperatively treated group had non-union of the clavicle fracture. The average adjusted Constant score in the non operated group was 94 (range 82 to 98). There was no significant difference in either Constant score or SF-36 between those with non-union and those fractures which had healed. There was also no significant difference in these scores between those treated nonoperatively and those treated by delayed surgery.

Conclusions: Nonoperative treatment of most displaced lateral third clavicle fractures can achieve good functional results comparable to those reported after surgical treatment. Surgery should be reserved for those with primary complications or for the minority who have painful non-union or acromioclavicular joint problems in the early stages of treatment.

Introduction Damage Control orthopaedic techniques have been proposed in the seriously injured with primary external fixation of long bone fractures, reducing the ‘second hit’ of surgery. We have developed a large animal (ovine) model for the study of major trauma.

Aim To clarify the sequence of pulmonary and systemic physiological responses over a 24-hour period following injury, comparing the effects of primary external femoral fixation to intramedullary stabilisation to better quantify the ‘second hit’ of these surgical techniques.

Methods Under terminal anaesthesia bilateral femoral diaphyseal fractures were produced using a mechanical pneumatic actuator (ram). Hypovolaemic shock was maintained for 4 hours before fluid resuscitation and surgical stabilisation.

24 sheep were randomised into 4 groups and monitored for 24 hours following injury:

Group 1 – Control Group (effects of general anaesthesia only)

Group 2 – Control Group for Trauma (injury but no long bone stabilisation)

Group 3 – Damage control group (Injury and external fixation)

Group 4 – Early total care (Injury and reamed intra-medullary stabilisation)

Outcome measures: Embolic load (Mayo score) using transoesophageal echocardiography; serum markers of coagulation (prothrombin time, activated partial thromboplastin time and fibrinogen levels) and inflammation (interleukin 6). Bronchoalveolar lavage to assess total cell count and cell differential to quantify the proportion of neutrophils present.

Results A sustained embolic shower was detected with each femoral fracture (mean Mayo score of 5 and 5.5 respectively). Intramedullary reaming and nailing produced further embolic events with a mean score of 2.5 and 1.5 respectively. Mean prothrombin time increased from a pre-fracture mean of 12 in each group to 18.8 (group 1) 20.7 (group 2); 24.8 (group 3); 31.1 (group 4). Alveolar lavage samples taken at 0, 4 and 24 hours following injury indicated a progressive neutrophilia developing in each group with a count pre-fracture of 4.3 increasing to 55.75 (group 1); 40 (group2); 49 (group3) and 31.7 (group 4) by 24 hours following injury.

Discussion The effects of damage control techniques in this model appear to be a reduced stimulation of the extrinsic coagulation system. An additional embolic hit was detected secondary to intramedullary reaming and nail insertion. Localised lung inflammation seems to develop in all groups with no significant differences seen due to treatment.

Survival was 96.4 percent at one year and 93.6 percent at five years, with no significant difference between prostheses. There were eight revisions, (one deep infection, four dislocations and three peri-prosthetic fractures), by 12 months. The average Constant score was 67.1 at one year.

Conclusion: Prognostic factors on presentation were age of the patient and their physiological index, and at three months any complication, the position of the implant, tuberosity union and persistent neurological deficit. Overall optimum outcome was in patients aged 55 to 60, with minimal co-morbidities and uncomplicated recovery.

Humeral hemi arthroplasty has become widely used as a form of surgical management for severe fractures. However there is still no consensus as to the role for prosthetic replacement in displaced proximal humeral fractures.

The aim was to assess shoulder hemi arthroplasty for un-reconstructable three and four part proximal humeral fractures at a minimum of twelve months and identify factors that guide to prognosis.

Criteria for inclusion were patients with a fracture that went onto shoulder hemi arthroplasty with Constant scoring at a minimum follow up of one year. Patients were treated using a Neer or Osteonics prosthesis, with the decision for hemi arthroplasty being made at the time of surgery. Post-operative management was standardised. An independent functional assessment, record review establishing a physiological index according to comorbidities, and a radiological analysis were carried out. A survival analysis was performed for the one and five year results and data was analysed by linear regression to identify prognostic factors.

Of 163 patients there were 138 fitting the criteria, 42 males and 96 females with an average age of 68.5 (range30–90) years and average follow up of 6.3 (range1–15) years. The fracture pattern was three or four part in 133 cases and 5 head split fractures; 58 were associated with a dislocation. Survival was 96.4% at 1 year and 93.6% at 5 years, with no significant difference between prostheses. There were 8 revisions, (1 deep infection, 4 dislocations and 3 peri-prosthetic fractures), most within 12 months. The average Constant score was 67.1 at one year.

Prognostic factors on presentation were the age of the patient and their physiological index. Factors at 3 months were any complication, the position of the implant, tuberosity union and persistent neurological deficit. Overall optimum outcome was gained by patients aged 55–60, with minimal comorbidities and an uncomplicated recovery.

Acute Respiratory Distress Syndrome (ARDS) is a rare but important complication of trauma, with a mortality of around 50%, and considerable morbidity amongst survivors. The treatment options currently available are supportive only. Although trauma is known to be an important risk factor, previous studies have been intensive care-based and the epidemiology of ARDS amongst trauma patients remains unknown.

We prospectively studied 7387 consecutive admissions to a single University Hospital, providing all trauma care to a well defined population, over an eight year period. Inclusion criteria were admission following trauma, age over thirteen and residence within the catchment area. Fifty five percent of all patients studied were male, the average age was fifty years and 97% of injuries were due to blunt trauma.

Thirty-eight (0.5%) patients developed ARDS following trauma, giving an incidence of 0.8 per 100 000 population per annum. The mortality rate was 26%. The incidence of ARDS after isolated thoracic, head, abdominal or extremity injury was less than one percent. The incidence was significantly higher amongst younger patients with a median age of 29 for those developing the condition. High energy trauma was also associated with an increased incidence, with 84% cases arising following a road traffic accident or a fall from a height. The highest incidence was observed amongst patients with multiple injuries. Patients with injuries to two anatomical regions had a higher incidence (up to 2.9%) than those with isolated injuries, and those with injuries to three anatomical regions had a higher incidence still (up to 8.2%). The combination of abdominal and extremity injury was shown on logistic regression to be especially significant.

The epidemiology of ARDS following trauma has not previously been defined. The incidence is highest following high energy trauma, in younger patients and in polytraumatised patients.

We have identified risk factors for the development of this rare but serious complication of trauma. Vigilant monitoring of those patients who are at increased risk will allow appropriate supportive measures to be instituted at an early stage.

Poor shoulder function may complicate the non-operative treatment of a humeral diaphyseal fracture. This has often been regarded as an unavoidable consequence of the relative immobility of the shoulder during brace treatment. Tears of the rotator cuff have not previously been recognised as an underlying cause of persistent shoulder pain and stiffness in this situation.

In this report, we identified six patients, from a consecutive series of 294 patients with humeral fractures treated in our Institution over a five-year period, who were found to have symptomatic rotator cuff tears following non-operative treatment of their humeral diaphyseal fracture. All had normal pre-injury upper limb function, but had persistent pain and loss of active shoulder movement after their fracture had united. Large, retracted cuff tears, affecting the cuff muscles attached to the greater tuberosity, were identified pre-operatively from imaging of the rotator cuff and intra-operatively at open surgery. Surgical reconstruction of the cuff was carried out in all cases and yielded satisfactory return of shoulder function, on assessment at least eighteen months after injury.

A pilot study of a physiotherapy led shoulder clinic was initiated in January 1999 as a direct response to increasing orthopaedic consultant out-patient clinic waiting times.

The orthopaedic consultant reviews letters from GP’s to the orthopaedic clinic. Patients with benign musculoskeletal shoulder problems are referred to Shoulder Assessment Clinic (SHAC). The physiotherapist completes a subjective and objective assessment and a shoulder score is recorded using Constant and Murley Shoulder (Constant and Murley 1985). The scoring system has a maximum of 100 points indicating perfect, pain-free movement and function. The Constant and Murley Shoulder score remains most widely used and validated scoring system in Europe for the shoulder.

A total of 130 patients were referred to SHAC between January 1999 and December 1999. Sixty-six patients were treated and discharged by physiotherapy alone with an average increase in shoulder score of 40 points. Twenty-seven had surgery, 19 had steroid injection. Overall waiting time to be seen in SHAC was 58 days. Since this initial pilot, 320 patients have been assessed and treated in the SHAC.

Our experience of physio-led shoulder clinic shows patients can be assessed and treated with a far shortened waiting time, and the improved shoulder scores suggest effective treatment. This allows more patients of an appropriate type to be seen in the outpatient orthopaedic clinic.

We reviewed the results of the treatment of 30 tibial fractures with minor to severe bone loss in 29 patients by early soft-tissue and bony debridement followed by primary locked intramedullary nailing. Subsequent definitive closure was obtained within the first 48 hours usually with a soft-tissue flap, and followed by bone-grafting procedures which were delayed for six to eight weeks after the primary surgery. The time to fracture union and the eventual functional outcome were related to the severity and extent of bone loss. Twenty-nine fractures were soundly united at a mean of 53.4 weeks, with delayed amputation in only one patient. Poor functional outcome and the occurrence of complications were usually due to a departure from the standard protocol for primary management. We conclude that the protocol produces satisfactory results in the management of these difficult fractures, and that intramedullary nailing offers considerable practical advantages over other methods of primary bone stabilisation.

We reviewed 63 patients with fractures of the distal tibial metaphysis, with or without minimally displaced extension into the ankle joint. The fractures had been caused by two distinct mechanisms, either a direct bending force or a twisting injury. This influenced the pattern of the fracture and its time to union. All fractures were managed by statically locked intramedullary nailing, with some modifications of the procedure used for diaphyseal fractures. There were few intra-operative complications. At a mean of 46 months, all but five patients had a satisfactory functional outcome. The poor outcomes were associated with either technical error or the presence of other injuries. We conclude that closed intramedullary nailing is a safe and effective method of managing these fractures.

We randomised 24 patients before they had a cemented hemiarthroplasty for hip fracture to receive either thorough or minimal saline lavage of the femoral canal. We then determined the effect in each group on the thromboembolic and cardiopulmonary responses to the pressurised insertion of cement, using transoesophageal echocardiography to show the echogenic embolic response. We found a statistically significant reduction in both the duration of the response and the number of large emboli in patients who had had thorough lavage as compared with the control group with minimal lavage. There was also less disturbance of pulmonary function, as assessed by the change in end-tidal CO2 levels and oxygen saturation, in patients who had thorough lavage. Three patients had a significant fall in blood pressure during cement insertion; all had only minimal lavage. We consider that thorough lavage should be an essential part of the preparation of the proximal femur before cement insertion.

We performed transoesophageal echocardiography in 111 operations (110 patients) which included medullary reaming for fresh fractures of the femur and tibia, pathological lesions of the femur, and hemiarthroplasty of the hip. Embolic events of varying intensity were seen in 97 procedures and measured pulmonary responses correlated with the severity of embolic phenomena. Twenty-four out of the 25 severe embolic responses occurred while reaming pathological lesions or during cemented hemiarthroplasty of the hip and, overall, pathological lesions produced the most severe responses. Paradoxical embolisation occurred in four patients, all with pathological lesions of the femur (21%); two died. In 12 patients large coagulative masses became trapped in the heart. Extensive pulmonary thromboembolism with reamed bone and immature clot was found at post-mortem in two patients; there was severe systemic embolisation of fat and marrow in one who had a patent foramen ovale and widespread mild systemic fat embolisation in the other without associated foraminal defect. Sequential analysis of blood from the right atrium in five patients showed considerable activation of clotting cascades during reaming.

We report the results of locked Seidel nailing for 30 fractures of the humerus. There were frequent technical difficulties at operation especially with the locking mechanisms. Protrusion of the nail above the greater tuberosity occurred in 12 cases, usually due to inadequate locking, and resulted in shoulder pain and poor function. Poor shoulder function was also seen in five patients with no nail protrusion, presumably because of local rotator cuff damage during insertion. Our results suggest that considerable modifications are required to the nail, and possibly to its site of insertion, before its use can be advocated.
