Aims

To assess the feasibility of a randomized controlled trial (RCT) that compares three treatments for acetabular fractures in older patients: surgical fixation, surgical fixation and hip arthroplasty (fix-and-replace), and non-surgical treatment.

Methods

Patients were recruited from seven UK NHS centres and randomized to a three-arm pilot trial if aged older than 60 years and had a displaced acetabular fracture. Feasibility outcomes included patients’ willingness to participate, clinicians’ capability to recruit, and dropout rates. The primary clinical outcome measure was the EuroQol five-dimension questionnaire (EQ-5D) at six months. Secondary outcomes were Oxford Hip Score, Disability Rating Index, blood loss, and radiological and mobility assessments.


Displaced acetabular fractures in the older patient present significant treatment challenges. There is evidence the morbidity and mortality associated is similar to the fractured neck of femur cohort. Despite growing literature, there remains significant controversy regarding treatment algorithms; varying between conservative management, to fracture fixation and finally surgical fixation and simultaneous THA to allow immediate full weight bearing.

£250k NIHR, Research for Patient Benefit (Ref: PB-PG-0815-20054). Trial ethical approval (17/EE/0271). After national consultation, 3 arms included; conservative management, fracture fixation and simultaneous fracture fixation with THA. Statistical analysis required minimum 12 patients/3 arms to show feasibility, with an optimum 20/arm. Inclusion criteria; patients >60 years & displaced acetabular fracture. Exclusion criteria: open fracture, THA in situ, pre-injury immobility, polytrauma. Primary outcome measure - ability recruit & EQ-5D-5L at 6 months. Secondary outcome measures (9 months); OHS, Disability Rating Index, radiographs, perioperative physiological variables including surgery duration, blood loss, complications and health economics.

11 UK level 1 major trauma centres enrolled into the trial, commenced December 2017. Failure surgical equipoise was identified as an issue regarding recruitment. Full trial recruitment (60 patients) achieved; 333 patients screened. 66% male, median age 76 (range 63–93), median BMI 25 (range 18–37), 87% full mental capacity, 77% admitted from own home. 75% fall from standing height. 60% fractures; anterior column posterior hemi-transverse. Trial feasibility confirmed December 2020. Presented data- secondary outcomes that are statistically significant in improvement from baseline for only the fix and replace arm, with acceptable trial complications. Issues are highlighted with conservative management in this patient cohort.

Our unique RCT informs design and sample size calculation for a future RCT. It represents the first opportunity to understand the intricacies of these treatment modalities. This RCT provides clinicians with information on how best to provide management for this medically complex patient cohort.

Background

Hip fracture care has evolved, largely due to standardisation of practice, measurement of outcomes and the introduction of the Best Practice Tariff, leading to the sustained improvements documented by the National Hip Fracture Database (NHFD). The treatment of distal femoral fractures in this population has not had the same emphasis. This study defines the epidemiology, current practice and outcomes of distal femoral fractures in four English centres.

Methods

105 patients aged 50 years or greater with a distal femoral fracture, presenting to four UK major trauma centres between October 2010 and September 2011 were identified. Data was collected using an adapted NHFD data collection tool via retrospective case note and radiograph review. Local ethics approval was obtained.

Purpose

Traditional recommendations suggest that open fractures require urgent surgical debridement to reduce infection. Although many papers comparing early vs late debridement have found no difference in infection rates, these papers have not taken into account important confounding factors. We attempt to answer whether delay between injury and surgical debridement in open fractures is associated with a higher infection rate after accounting for these important confounders.

Method

Five hundred and twenty three open extremity fractures in 417 patients were identified using the Sunnybrook trauma and orthopaedic department registries. Thirty patients (36 fractures) did not have complete follow-up. Seven patients were excluded due to incomplete data (complete follow-up rate=91%). A further 14 patients died during their hospitalization. A total of 459 fractures in 364 patients were reviewed. Data was collected on demographics, ISS score, ASA, time to initial operative debridement, timing of antibiotic administration, mechanism of injury, presence of significant contamination, and Gustillo-Anderson fracture grade. Deep infection was defined as an unplanned return to the operating room for treatment of infection. The influence of time to initial debridement was examined in an unadjusted analysis as a continuous variable and at thresholds of 6 and 12 hours of delay. A multivariable logistic regression was used to analyze the effect of delay while controlling for important confounding variables.

Background

However, in many cases, these studies were not able to control for important confounding factors. The purpose of our study was to evaluate the relationship between delay to definitive surgical debridement while controlling for important confounders.

Method

364 patients with 459 open fractures treated at a level one trauma centre over four years were reviewed. Time to definitive surgical debridement was modelled as a predictor of infection while controlling for fracture grade, anatomic site of fracture, and presence of significant contamination. Time to debridement was modelled as both a continuous variable and a categorical variable with cut off points at 6 and 12 hours of delay.

Aims: To establish the best teaching method for medical students and ascertain the students’ preferred method of teaching.

Material and Methods: 30 medical students in were picked randomly and divided into two equal groups. Group 1 received Standard bedside teaching and Group 2 watched an interactive DVD. Each group then undertook a validated OSCE and the examiners were blinded as to which teaching method the students had received. The groups then received the other method of teaching followed by another OSCE. A questionnaire was given to all the students, to assess their satisfaction of the teaching session.

Results:

Conclusion: Interactive teaching method can be a useful technique for teaching medical students, however the students’ preferred method of teaching is standard bedside teaching. Efficiency of knowledge transfer can be improved if interactive teaching is followed by standard bed side teaching but not the other way around.
