Freiberg's Infraction; osteonecrosis of the metatarsal head, is the fourth most common intra-articular osteonecrosis in the body. Surgical intervention is usually reserved for late stage of the disease process (III-V) or failure of conservative management. We evaluated the outcomes of patients treated with primary Interpositional Arthroplasty technique using periosteum and fat for adequate surfacing and as a spacer for Freiberg's Disease.

Twenty-three cases (21 patients) were performed from February 2009 – March 2016 (18 women, 5 men). Mean age at surgery was 51.1 years (range 19 – 70.5 years) with 91% affecting the second metatarsal. Twenty-one cases were primary and two cases were revision. Five cases were stage III, 10 were in stage IV and 8 were stage V. All patients underwent Interpositional Arthroplasty using periosteum and fat graft from affected metatarsal inserted as joint spacer and secured with sutures. Patients were followed up by postal questionnaires using two validated questionnaires; MOXFQ and AOFAS. Mean follow-up was 3.7 years (0.6 – 7.6 years). Paired two-tailed student t tests were used to assess clinical significance.

The left and right foot was affected in 12 and 11 cases respectively. There were no postoperative infections, non-unions or transfer metatarsalgia. Surgery allowed 8 patients to wear normal footwear, 9 wearing fashion shoes, 5 wearing dress shoes and 5 patients returned to sporting activities. Mean pre-operative and post-operative VAS pain scores were 6.7 (range 4–10) and 3.2 (range 0–0) (p< 0.05). Mean peri-operative AOFAS scores were 43.8 (range 14–14) and 71.3 (range 10–10) (p< 0.05). Mean peri-operative MOXFQ scores were 62.9 (range 23–23) and 31.8 (range 0–0) (p< 0.05).

We recommend our novel Interpositional Arthroplasty using periosteum and fat spacer for late stage Freiberg's disease as it can result in significant improvement in pain, prevents donor site morbidity and produces significant functional improvement and patient satisfaction.

Background:

Previous attempts at small incision hallux valgus surgery have compromised the principles of bunion correction in order to minimise the incision. The Minimally Invasive Chevron/Akin (MICA) is a technique that enables an open modified Chevron/Akin to be done through a 3 mm incision, facilitated by a 2 mm Shannon burr.

Methodology:

This is a consecutive case series performed between 2009 and 2012. This includes the learning curve for minimally invasive surgery. All cases were performed by a single surgeon at two different sites, one centre where minimally invasive surgery is available and the other where it is not. The standard procedure in both centres is a modified Chevron osteotomy. Regardless of whether the osteotomy was performed open or minimally invasive two-screw fixation was performed. Retrospective analysis includes the IMA, HVA, M1 length, forefoot width and forefoot: hindfoot ratio. Clinical outcomes include the MOXFQ, AOFAS, and assessment of complications.

Introduction:

It has been shown that inadequate reduction of the sesamoids can lead to recurrent hallux valgus. It can be difficult however to assess the sesamoid position. We propose a simple method of grading sesamoid position; the sesamoid width ratio. We aim to assess for a difference in ratio between those with and without hallux valgus and subsequent correlation with increased deformity. The new grading system can then be tested for inter-observer reliability.

Methods:

277 (103 normal, 87 preoperative, 87 postoperative) AP weight bearing foot radiographs were analysed for hallux valgus angle (HVA), intermetatarsal angle (IMA), and both medial and lateral sesamoid width (mm). The sesamoid width ratio (SWR; lateral/medial width) was then calculated. Using statistical methods based upon HVA and IMA grading, three groups of increasing hallux valgus severity, in accordance with SWR, were defined; normal ≥1.30, moderate 1.29–0.95 and severe ≤0.94. Sixty images (10 normal, 25 preoperative, 25 postoperative) were then sent on disc to three separate reviewers to assess for inter-observer error.

Background:

The Chevron osteotomy is straightforward, requires less dissection and allows earlier rehabilitation than some other osteotomies. However it is generally perceived as unsuitable for severe deformities even though a 2012 meta-analysis and an earlier RCT failed to show any advantage of the scarf over the chevron. We aim to assess the correctability of severe HV comparing the correction, the clinical outcomes and complications of the Chevron osteotomy with other techniques employed in a consecutive series.

Methodology:

We reviewed a series of 92 cases of severe hallux valgus (IMA >17° regardless of the HVA). The follow-up period varied from 1 to 4 years. Pre-operative x-rays and final post-operative weight-bearing x-rays were performed. Outcome scores (MOXFQ and AOFAS), IMA, HVA and foot width were collected. Complications were monitored.

Introduction:

NICE guidelines state that every patient should be assessed for their VTE risk on admission to hospital. The aim of this study was to determine whether currently recommended risk assessment tools (Nygaard, Caprini, NICE and Plymouth) can correctly identify the patients at risk.

Methods:

In a consecutive series of over 750 trauma patients treated with cast immobilisation 23 were found to have suffered a VTE. Their notes were retrospectively reviewed to discover how many had been assessed for their VTE risk on admission. Additionally, the 4 most current Risk Assessment Tools were used to retrospectively score the patients for their VTE risk to determine whether they would have been identified as at risk of sVTE, had the RAMs been used at the time. We also identified a matched group of patients in the same cohort who had not suffered a VTE and they were also retrospectively risk assessed.


Background:

Various angles have been used to grade the severity of hallux valgus deformity. They are useful in surgical planning but do not correlate with symptom severity or improvement. We feel that there is a fundamental mismatch between the width of the forefoot and the width of the hindfoot and that this is more clinically relevant, we describe two techniques for measuring this. We aim to measure the degree of foot narrowing after surgery and moreover how this correlates to the severity of pre- and post operative outcomes.

Methods:

200 consecutive bunion operations were assessed with weight bearing radiographs. The HVA and IMA were measured according to standard practice. We also assessed forefoot width using two methods we have described. The first is the ‘Forefoot Width’ measured as a perpendicular to the midfoot (a technique we have previously validated). The ‘Foot Ratio’ is calculated as a function of the calcaneal width. Clinical outcomes were assessed using the MOXFQ and AOFAS.

Background

The Weil osteotomy is successful in the management of metatarsalgia and toe deformity. The aim is to achieve controlled shortening whilst avoiding plantarflexion. Recognised complications of the Weil osteotomy include a ‘floating toe’ in up to 20% or more of cases.

Aim

Can modification of the Weil osteotomy avoid the metatarsal head plantarflexion and subsequent dorsal subluxation of the interosseous muscle that is implicated in the development of a ‘floating toe’.

Background

Despite the suggestion by Virchow in 1856 that thrombosis was the result of venous stasis, endothelial dysfunction and hypercoagulability there are some fundamental questions which remain to be answered. The published studies fail to provide specific details such as cast type and anatomical location of the thrombosis, but instead focus on the incidence of VTE and which chemical thromboprophylaxis is most effective. Previous studies of VTE in trauma patients have involved small numbers of patients and have not look at the risk medium to long term risk. Most importantly they have not looked at the site of the VTE. This makes interpretation of the link between cast and VTE even more complex.

Methodology

We analysed 1479 consecutive trauma cast applications and the incidence of symptomatic VTE in the six months following the injury. The diagonosis, cast type and site of the VTE was recorded.

Introduction

The muscles of the leg collectively comprise the calf pump, however the action of each muscle group on calf pump function is not known. Patients with foot or ankle injury or surgery are often advised to perform foot and ankle movements to help prevent deep venous thrombosis. Our aim was to determine which foot and ankle movements were most effective in stimulating the calf pump. Method: Nine healthy participants were enrolled in this research and ethics approved prospective study. Participants with a previous history of peripheral vascular disease, varicose veins, deep venous thrombosis or previous foot and ankle surgery were excluded. Each participant followed a standardized protocol of foot and ankle movements, starting with foot in neutral position and the baseline and movement peak systolic velocity within the popliteal vein was measured during each movement. The movements tested were toe dorsiflexion, toe plantar flexion, ankle dorsiflexion, ankle plantar flexion.

Results

The mean patient age was 34 years (range 28–58), the majority were female (n = 6). All movements resulted in statistically significant changes in peak systolic velocity (p = <0.05). In order of decreasing peak velocity the exercises which had greatest effect on calf pump function were: Ankle dorsiflexion (101cm/s), Ankle plantarflexion (84cm/s), Toe dorsiflexion (63cm/s), Toe plantarflexion (59cm/s).

We have shown that all four exercises significantly increased calf pump function. The greatest effect was seen with ankle movements.

Background

Metatarsus adductus is the most common forefoot deformity. Variable prevalence values were reported in literature using different techniques in different populations.

Numerous radiological measurements have been proposed to assess this deformity with a paucity of studies reporting the reliability of these methods.

The metatarsus adductus angle was shown to correlate with the severity of hallux abductovalgus in normal feet and preselected populations of juvenile hallux valgus.

Materials & Methods

Weight bearing dorsoplantar radiographs of 150 feet were examined for 5 angles commonly used in assessing metatarsus adductus: angle between the second metatarsus and the longitudinal axis of the lesser tarsus (using the 4th or 5th metatarso-cuboid joint as a reference), Engel's angle and modified angle's angle. The prevalence of metatarsus adductus was assessed according to published criteria for different techniques. Inter and intra-observer reliabilities of these angles were evaluated on 50 X-rays. Linear regression tests were used to assess the correlation between hallux valgus and different angles used in assessing metatarsus adductus.

Introduction

The incidence of deep venous thrombosis (DVT) in patients with lower limb cast immobilization occurs in up to 20% of patients. This may result from altered calf pump function causing venous stasis. Our aim was to determine the effects of below knee cast on calf pump function.

Method

Nine healthy participants were enrolled in this research and ethics approved prospective study. Aim

To assess the influence of posterior slope on Knee flexion and function in Asian and Caucasian populations.

Material & methods

We have conducted a prospective comparative study of 109 Asian and Caucasian posterior tibial slopes. All data has been collected prospectively and includes personal data (height, weight, tibial measurements), ASA grading, knees scores and range of movement. Analysis was performed for the whole group and comparisons were made between the two sets of patients. Minimum follow-up was two years.

Metatarsus adductus (MA) is associated with a medially facing distal facet of the medial cuneiform (with a normal first metatarsal) and varus/adducted deformities of the metaphysis of the lesser metatarsals. A number of patients with severe symptomatic metatarsus adductus do not improve with time. A number of surgical techniques have been described but the series are small and use radiological rather functional outcomes. It is clear however that the failure and complication rate with these procedures is high. A combined medial cuneiform and lesser metatarsal basal closing wedge osteotomy has potential advantages over more commonly used procedures (including the combined cuneiform-calcaneal) osteotomy, by correcting at the level of deformity.

We reviewed a consecutive series of 15 cases (11 severe idiopathic metatarsus adductus, 4 with history of clubfoot) (all Bleck’s grade severe) treated with combined cuneiform-metatarsal osteotomies. Patients were followed up for a mean of 30 months using child-, parent and clinician-based outcome measures as well as radiological assessment. Outcomes are also compared to currently used and historical procedures.

Bleck’s grade improved to 65% normal 35% mild post op; Radiographic improvements (all p< 0.001); 1stray angle 30°→62°, 1stMT-Talar angle 43°→9°, 2ndMT-Talar angle 41°→8°, 2ndMT-Calcaneal angle 48°→14°, 5thMT-Calcaneal angle improved from 13°→3°. Mean postop scores; Chesney - 14 (12–15); Utukari – 13 (10–18); Laaweg – 93 (81–100); Vitale – 13 (10–14). None of the radiographic scores correlated with the clinical scores. All children gained improved levels of activity.

Our findings indicate that this technique can be used effectively in children > 4 years and is a safe alternative to historical procedures, with excellent radiographic/ clinical outcomes, and a low complication rate.

Tibiotalocalcaneal fusion is generally reserved for complex cases such as severe deformity or bone loss, infection, Charcot and revision procedures. Subsequently published series have been small and there are no studies comparing plate fixation and intramedullary nailing. We present the outcomes in the largest series to date and have also compared the union rate and complication rate between blade plate fixation (36) and intramedullary nailing (46). Both groups were well matched for patient and disease factors.

There was a non-statistically significant trend towards better outcomes with nails. Further analysis was conducted comparing the two fixation methods by the indication for surgery, there were no statistically significant differences, the reasons for these findings are discussed.

Aims: To compare the preoperative knee function in patients of Asian origin and Caucasians living in the same community.

Background: The prevalence of osteoarthritis is high in all ethnic and demographic groups. The timing of surgery is important as poor preoperative functional status is related to poor postoperative function.

Methods: Prospective study of 63 Asian patients age and sex matched with Caucasian patients undergoing total knee arthroplasty. Pre operative Knee Society Clinical Rating System scores were recorded as a separate Knee Score and Knee Function.

Results: The mean preoperative Knee Score in Asian patients was 37.6 in comparison to 41.5 in Caucasians (p< 0.10) this difference was not statistically significant. The mean preoperative Knee Function in Asian patients was 32.5 in comparison to 45.0 in Caucasians (p< 0.0001) this difference was highly statistically significant.

Conclusions:Patients of Asian origin undergoing total knee arthroplasty have lower preoperative knee function to Caucasians. Cultural beliefs and social support explain part of this discrepancy but health care providers must also attempt to educate patients and close family members about the importance of timing the surgery to obtain the optimum benefits of pain relief and function.

Background: Aseptic loosening of the tibial component is the one of the commonest cause of failure following a TKR. Good cement penetration into bone can reduce this and this has led to the development of new devices and techniques to improve the pressurisation during the cementing process.

We have conducted a case controlled study on a novel cementing technique and compared the results with published optimum levels.

Methods: 104 patients that underwent TKR at our institution were studied. Half of these were cemented using a double mix of cement inserted under pressure via an inexpensive alternative to the cement gun. The data was collected prospectively and included patient information, knee outcome scores and radiological assessment and there was a minimum 4 years follow-up.

Results: We found a significant difference in the number of early lucent lines as well as the size of the cement mantle particularly in the most important Zones. Despite pressurisation 1 of the 14 zones did not improve and we have examined the reasons for this.

In order to achieve optimum cement penetration we recommend that attention is paid to good pressurization and cement penetration and describe our technique for doing this.

The importance of correctly identifying and treating ankle syndesmosis injuries is paramount to achieving a good functional result. Although it is clear that anatomical correction is essential the practical aspects remain disputed. Controversial issues include diagnosing and determining which injuries need treating, the number and size of screws for fixation, the number of cortices engaged, a screw removal strategy and the use of biodegradable screws or other forms of fixation. We report the results of a survey of 440 orthopaedic surgeons to determine current UK practice, this is then compared to best practice as determined by literature review.

When analysis was performed for the group and by sub-speciality (foot and ankle, trauma and general orthopaedics) we found large differences in practice. Furthermore it appeared that some aspects of treatment varied considerably from published evidence, for instance four out of five screws were removed much earlier than clinical studies have recommended.
