Diabetic foot problems are a common cause for hospitalisation in this group and up to 25% of diabetic patients will be affected. Prevalence of diabetes is rising, currently affecting 680000000 people worldwide. The enormity of this problem mandates any strategy that shortens therapeutic period and enhances success rates. Cerament G has been used in our unit as a treatment adjunct in diabetic foot treatment. Successful treatment is viewed as eradication of infection and a functional foot.

Retrospective review of 40 months practice with 115 patients. Inclusion: all diabetic feet requiring surgery Cerament G used, protocol driven Microbiology pathway. Exclusion: Primary closure not possible. Cerament G not used. Outcome assessed in three groups: Total failure (further surgery required); slow to heal (healing by secondary intention); healed without problems.

Healed 99 (eradication of infection and return to function), failure to heal 16 (success rate: 86.1%). Infection was the cause of failure in only in 2.6% (13 failures due to patient noncompliance or poor vascularity). Accepted success rate in treating osteomyelitis in diabetic feet is 68% (medical treatment only), combination of surgery and medical is 86%. Eradication of infection is the only end point return to function is not addressed. This study shows Cerament G with surgery/systemic antibiotics provides a 97.4% success rate.

Therapeutic drivers in this field have been determined traditionally by Physicians and Vascular Surgeons (resection rather than reconstructive surgery.) Our assertion is that eradicating infection in a functionally useless foot is a waste of health resources. Our strategy is always the delivery of an intact functional foot residuum. Cerament G as an adjunct allows this goal in a cost-effective manner.

Anterior cruciate ligament (ACL) rupture with associated meniscal pathology commonly occurs in a young, active population. Preserving a greater proportion of the meniscus may improve long-term outcomes by maintaining shock absorption and knee stability. However, meniscal repair procedures involve longer healing and rehabilitation than meniscal debridement, which could affect return to work and activity. The purpose of this study was to examine the functional outcomes and quality of life scores through two years in patients undergoing ACL reconstruction (ACL-R) who had meniscal repair, meniscal debridement, or no meniscal damage at the time of reconstruction.

Data for 1814 skeletally mature patients with isolated primary ACL-R was prospectively collected at a single centre from January 2010 to December 2015. Functional testing of operative to non-operative limb performance was completed at one- and two-years following surgery and included single-leg balance, single-leg hop for distance, timed six-meter hop, triple-hop for distance, and triple cross-over hop for distance. ACL-Quality-of-life questionnaires (ACL-QoL) were completed pre-operatively and at 1- and 2-years post-operatively. Descriptive statistics were completed for patient demographics and intra-operative pathology. Unpaired t-tests using 95% confidence intervals were conducted to compare groups.

The patient cohort was 45% female, with a mean age of 31 years (SD 11, range 14–66). Meniscal injury was detected in 1229/1814 knees (67.8%). There were 729 debridements and 538 repairs performed. Graft choice was hamstring autograft in 85.8% of cases, bone-patellar-tendon-bone autograft in 2.5%, allograft in 10.1% and other graft types in 1.5%. Pre-operative ACL-QoL scores were 29 and 28.5 for knees without and with meniscal damage, respectively (p>0.05).

Of 1814 patients, 1269 (69.9%) completed the ACL-QoL at the two-year appointment, and 1225 (67.5%) completed the functional testing. At two years post-operative, patients with no meniscal damage at surgery demonstrated superior limb symmetry performance on triple-hop for distance compared to patients with meniscal damage (98.4% vs 97.1%, p < 0 .05, CI 0.1–2.5%). No other functional testing parameters showed statistical significance. There was no difference in functional outcome between patients undergoing an isolated meniscal repair versus debridement at one- or two-years. ACL-QoL scores were statistically significantly higher at one- and two-years post-operative for patients without meniscal damage (1-year: 73 vs 70.2, p < 0 .05, CI 0.51–5.1, 2-years: 79.2 vs 76.1, p < 0 .05, CI 0.79–5.4). ACL-QoL scores were minimally higher for isolated meniscal debridement compared to isolated meniscal repair at both time points (1-year: 71.4 vs 68, p < 0 .05, CI 0.4–6.4, 2-years: 78.3 vs 74, p < 0 .05, CI 1.3–7.3).

Functional outcomes do not differ at one or two years post-operatively for patients undergoing meniscal repair versus debridement concomitant with ACL-R. Quality of life scores were statistically significantly higher for the patients with no meniscal pathology at both one- and two-years post-operative. ACL-QoL scores were also statistically significantly different for the meniscal repair and debridement groups however these differences are unlikely to be clinically significant. Extended follow-up is needed to determine if the differences detected in ACL-QoL scores are sustained over time, as well as the long-term role of meniscal injury on functional outcomes.

Aim

This study presents the early results of a novel procedure, both in timing and surgical technique, aimed to treat those cases of congenital hip dysplasia that present late or fail conservative treatment.

Methods

48 patients and 55 hips treated over the period from December 2004 to February 2011 were retrospectively reviewed. All were treated with adductor and psoas tenotomy, open reduction, capsulorrhaphy and acetabuloplasty by the senior author.

Introduction: Arthrodesis of the knee nowadays is used as a salvage procedure, commonly for patients with a failed TKR or in infected trauma cases. We present 4 patients with extensive bone defects following septic sequelae of trauma treated by Arthrodesis of the knee joint.

Materials and Methods: Four patients (avg. 46.5 years; range 37–57 years; three male and one female) with longstanding infected non-union fractures (3 months–2 years) at the knee joint (three Tibial plateau and one distal femur) were treated by initial debridement and removal of dead or infected bone. This led to substantial bone defects (6–12 cm) of the debrided bone at the knee joint. The patients then underwent bone transport with a circular frame to compensate for this bone defect before achieving an Arthrodesis of the knee joint. Three patients also had a free muscle flap for soft tissue coverage before bone transport was begun.

Results: Arthrodesis of the knee was achieved in all patients at an average time of 26 months (20–32 months). None of the patients have any active infection of the limb.

Discussion and Conclusions: Knutson et al (1984) said that massive bone loss may substantially reduce the success rate of Arthrodesis of the knee. Wilde and Stearns (1989) noted decreased fusion rates with greater degrees of bone loss. In our series the bone defects were a sequelae of infective non-union, this further complicates the healing process. However, using circular frame for Bone transport to overcome the defect and to achieve compression at the Arthrodesis site is a useful technique for such challenging cases.

The number of patients in the United Kingdom being admitted with Neck of Femur Fractures (NOF) is increasing each year. Primary first aid for these patients includes adequate analgesia. The commonest forms of analgesia are opioids and in some units regional blockade. However, both have limitations. Regional block is skill dependent while opiates are known to have many side effects.

Paracetamol is an analgesia that is safe and has an excellent side-effect profile within standard doses. Intravenous paracetamol has a far higher predictable bio-availibilty than oral, within standard dosage. This study is to assess the suitability of using intravenous Paracetamol as an alternative.

Method: Prospective study: a change in protocol resulted in all NOF’s admitted under the care of the senior author being prescribed regular intra-venous paracetamol within standard dosage. PRN opioids were available for breakthrough pain. NOF’s admitted under the care of other consultants remained on the established protocol. Opioid usage and pain scores (scale 0–10) were measured.

Results: results of 72 patients were collected, 44 in intravenous paracetamol group and 28 in the control group, having regular opiates and oral paracetamol. There is a 65% reduction in opiate usage in the intravenous paracetamol group (P value= 0.015). There is only a 0.5 difference in average pain score between the two groups (P value= 0.173).

Conclusion: The use of regular intra-venous paracetamol results in a significant reduction in the need for opioid analgesia. The pain relief within this group was comparable to that in the control group. The side-effects of opioids are dose dependent, a reduction in their usage therefore improves both pre and post-operative morbidity by reducing the side effects. A simple change in analgesia protocol to a safer, more predictive agent can result in an improved pre/postoperative period.

Introduction: Staphylococcus aureus is a major cause of chronic infections and causes particular problems in relation to implanted prostheses. Biofilm formation on abiotic surfaces affords bacteria innate protection from opsonophagocytosis and antibiotic agents and complicates the eradication of infection from bone and implanted prostheses. Increased concentrations of sodium, the major extracellular cation, have previously been implicated in increased biofilm formation in Staphylococcus aureus. In this study we demonstrate that increased concentrations of potassium, the major intracellular cation, also causes a significant increase in biofilm formation. Furthermore we also show that halide stress also leads to a primary increase in penicillin resistance in Staphylococcus aureus.

Methods: Staphylococcus aureus ATCC 9144 was cultured in broth supplemented with variable amounts of potassium chloride and sodium chloride. Biofilm formation was investigated in 96-well microtiter plates using a standard technique. Antibiotic resistance was investigated using graduated E-test strips.

Results: There was a positive correlation between bio-film formation and increased concentrations of sodium and potassium. Biofilm formation was noted to be even greater under potassium stress than under sodium stress. Sodium stress also lead to a five-fold increase in penicillin resistance in naïve Staphylococcus aureus cells.

Discussion: Cellular injury or insult can lead to cell necrosis and lysis. The intracellular concentration of potassium is 30 times higher than that of the surrounding extracellular fluid. Hence, cell necrosis leads to markedly increased local concentrations of potassium. These experiments show that an increase in potassium concentration leads to an increase in biofilm formation. This suggests that biofilm formation and hence infection of implanted pros-theses may be more likely in areas of major tissue trauma such as large resections and revisions. Furthermore, cellular stress leads to increased antibiotic resistance in naïve cells which may nullify prophylaxis and complicate bacterial eradication in vivo.

Purpose: Neck of femur fractures increasingly form a large bulk of in-patient admissions to trauma units. These patients often require multi-disciplinary input before discharge. Delayed discharge not only exposes patients to nosocomial infections, it places strain on limited bed numbers. The use of a single screening question at time of admission to identify those patients suitable for fast-track discharge was investigated.

Methods: Prospective study. 122 patients in the study. Basic epidemiological data was collected. At admission the patient was asked whether they were able to do their own shopping or not. Following discharge: the length of stay and discharge destination was recorded. Results were analysed using standard statistical methods

Results: 122 patients were identified. 43 of the patients(35%) able to do their own shopping. There was a significant difference in length of stay between the two groups as well as mortality rate. Those able to do their own shopping stayed in hospital on average 4 days longer. In the group who could do their own shopping, 100% were discharged home from the trauma ward.

Conclusions: This study demonstrated that through the use of a single question it is possible to identify elderly patients suitable for home rehabilitation. All those patients who could do their own shopping were discharged home from the trauma ward without need for transfer to an in-patient rehabilitation ward. All patients who could do their own shopping were assessed as suitable for discharge home once they had completed a basic course of rehabilitation. This resulted in a longer length of stay in hospital. Identifying this group of patients it is possible to reduce their average length of stay in hospital from 14 days to 5 days. This is achieved using a fast track system for elective arthroplasty patients, based on a system of home rehabilitation. This results in improved care for the patients as well as financial savings on in-patient care.

A phenomenon of methicillin resistance in methicillin sensitive Staphylococcus aureus has been noted in organisms living in biofilm induced by the state of cell wall deficiency. The rate and the amount of biofilm formed by the cell wall deficient organisms far exceeds that of cell wall patent organisms. Once removed from the biofilm the S. aureus had the same sensitivities of the original organism. Cell wall deficient organisms outside the biofilm did not demonstrate the methicillin resistance. A known laboratory strain (ATCC 9144) was induced into a cell wall deficient state and allowed to form biofilm. The rate of formation and amount formed was compared with that formed by cell wall patent organisms. Before inducing cell wall deficiency sensitivity to methicillin was demonstrated using standard microbiological technique. Using an oxacillin containing plate as a culture medium: the biofilm, cell wall deficient organisms and the cell wall competent organisms were inoculated onto separate media. Organisms from the biofilm were isolated and grown free of the biofilm on blood agar. Any growth on the oxacillin containing plate would demonstrate methicillin resistance. There was no growth on the plates containing the cell wall competent or cell wall deficient organisms. There was however growth on the plate inoculated with bio-film, however when organisms were isolated from the biofilm, there was no growth on the media. Antibiotic sensitivities of the original inoculant and the organisms isolated from the biofilm were the same. The biofilm, induced as a result of cell wall deficiency, offers a form of structural protection to the Staphylococcus aureus without altering the resistance pattern of organism. Standard microbiological techniques would therefore report the organism as methicillin sensitive, however clinically the organism may behave as a methicillin resistant organism. The state of cell wall deficiency encourages the formation of biofilm in S. aureus. In-vitro the state of cell wall deficiency is induced using high osmolality media or sub-lethal doses of cell wall active antibiotics. Both these states are found in clinical practice.

The use of sub-lethal doses of cell wall active antibiotics to induce cell wall deficiency in S aureus has been described. Cell Wall Deficient S aureus show an increased in-vitro ability to form biofilm. Cephalosporins(cell wall active antibiotics.) are commonly used at time of arthroplasty surgery as antimicrobial prophylaxis. Adherence is fundamental step in biofilm formation.

The adherence of cell wall deficient S aureus versus ‘wild type’ S aureus to glass was investigated. Slides comparing the two types of organisms were analysed using fluoroscopy and J-image software. The ability to adhere to plastic was investigated using a micro-titre based absorption test. In a third investigation a centrifugal force was used to quantify the adherence ability of the cell wall deficient organisms to the glass slides.

The cell wall deficient organisms demonstrated an increased ability to adhere to glass compared to the ‘wild type’. After exposure, there was on average twenty times more cell wall deficient organisms per unit area compared to the ‘wild-type’. The micro-titre plates were similar. After incubation, the absorption of each well was measured. Compared to the ‘wild type’ there was a significantly increased absorption in wells containing the cell wall deficient organisms. Showing an increased ability to adhere to plastic. The third technique quantified the ability to adhere using a centrifugal force. The slides were exposed to ‘wild type’ and cell wall deficient organisms, however before staining they were placed in a centrifuge. On analysis there were five cell wall deficient S aureus per field of view, compared to 0.5 ‘wild-type’.

An increased ability of cell wall deficient S aureus to adhere to surfaces has been shown. Adherence is fundamental to biofilm formation. The significance to orthopaedics is that the inadequate use of Cephalosporins at time of operation may be facilitating chronic infections.
