This study aims to compare the outcomes of Volar locking plating (VLP) versus percutaneous Kirschner wires (K-wire) fixation for surgical management of distal radius fractures.

We systematically searched multiple databases, including MEDLINE for randomized controlled trials (RCTs) comparing outcomes of VLP fixation and K-wire for treatment of distal radius fracture in adults. The methodological quality of each study was assessed by the Cochrane Risk of Bias tool. Patient-reported outcomes, functional outcomes, and complications at 1 year follow up were evaluated. Meta-analysis was performed using random-effects models and results presented as risk ratios (RRs) or mean differences (MDs) with 95% confidence interval (CI).

13 RCTs with 1336 participants met the inclusion criteria. Disabilities of the Arm, Shoulder and Hand (DASH) scores were significantly better for VLP fixation (MD= 2.15; 95% CI, 0.56-3.74; P = 0.01; I2=23%). No significant difference between the two procedures for grip strength measured in kilograms (MD= −3.84; 95% CI,-8.42-0.74; P = 0.10; I2=52%) and Patient-Rated Wrist Evaluation (PRWE) scores (MD= −0.06; 95% CI,-0.87-0.75; P = 0.89; I2=0%). K-wire treatment yielded significantly improved extension (MD= −4.30; P=0.04) but with no differences in flexion, pronation, supination, and radial deviation (P >0.05). The risk of complications and rate of reoperation were similar for the two procedures (P >0.05).

This meta-analysis suggests that VLP fixation improves DASH score at 12 months follow up, however, the difference is small and unlikely to be clinically important. Existing literature does not provide sufficient evidence to demonstrate the superiority of either VLP or K-wire treatment in terms of patient-reported outcomes, functional outcomes, and complications.

Treatment of periprosthetic joint infection (PJI) can include local delivery of antibiotics. A frequently used medium is absorbable calcium sulphate beads. The aims of this study were to:

identify how often organisms in infected THRs are sensitive to the added antibiotics

establish the incidence of persistent wound discharge and hypercalcaemia

All patients who received an antibiotic loaded calcium sulphate carrier (Stimulan, Biocomposites, Keele, UK) for either confirmed infection, presumed infection or for prophylaxis between July 2015 and July 2020 were included. Stimulan use was at the discretion of the surgeon, and between 10 and 40cc was used. In the absence of a known organism we routinely used 1g vancomycin and 240mg gentamicin per 10 cc of calcium sulphate. Post-operative sensitivities for all organisms cultured were compared to the antibiotics delivered locally. Persistent wound drainage was defined as discharge beyond the third postoperative day. Patients had serum calcium measured if they developed symptoms consistent with hypercalcaemia (Ca >2.6 mmol/L) or the clinical team felt they were at high risk.

189 patients (mean age 66.9 years, mean BMI 28.9, 85 male, 104 female) were included. 11 patients had a native joint septic arthritis, 42 presented with acute PJI and 136 presented with chronic PJI. 133 patients grew an organism, of which 126 were sensitive to the added antibiotics. Of the seven patients with resistant growth five had vancomycin-resistent Enterococcus, one Pseudomonas and one multi-organism growth including coagulase negative Staphylococcus. 40 patients experienced persistent wound discharge, with eight requiring re-operation. All other cases settled with dressing management. 12 patients developed hypercalcaemia (3/64 10cc, 7/117 20cc, 0/2 30cc and 2/6 40cc). The peak calcium reading ranged between the second and twelfth post-operative day.

The addition of vancomycin and gentamicin to absorbable calcium sulphate covers the majority of organisms found in culture positive infection in our cohort. It also appears safe, with an acceptable incidence of hypercalcaemia or wound discharge. Further work is required to identify patients at greatest risk of culturing resistant organisms or delayed wound healing.

Surgical biopsies are still considered the gold standard in obtaining tumor tissue samples. In this study, we will analyze the core needle biopsy in the evaluation of musculoskeletal tumors focusing on the accuracy, effectiveness, and safety of this technique in comparison to an open biopsy procedure.

This is a retrospective case series at King Abdulaziz Medical City (KAMC). All medical records from all patients who had a core needle biopsy (CNB) for a musculoskeletal mass and eventually underwent excisional biopsy between January 2010 and December 2016 at KAMC were included. Besides patient demographic data, the data extracted included the locations of the suspected mass, type of tissue acquired (bone or soft tissue), number of biopsies, complications reported during the procedure, histopathological report of core needle biopsy.

A total of 262 patients who were suspected to have a musculoskeletal tumor were identified. Female to male ratio was (1:1.4) and paediatrics (of 93.1%. The AUC of CNB in comparison to excisional biopsy was 0.86. The most common site of tumor extraction was in lower extremities (47.3%), followed by upper extremities (23.7%), pelvis and gluteal area (19.5%) and spine (9.5%).

In conclusion, CNB is cost-effective, safe and minimally invasive in bony and soft tissue lesions in comparison to an open biopsy procedure. Therefore, initiatives are required to implement this procedure to the majority of health care centers.

The scaphotrapeziotrapezoid (STT) joint is one of the key link joints between the proximal and the distal carpal rows. We assessed the relationship between the scaphotrapezium (STm) andscaphotrapezoid (STd) joints using computerised tomographyand hypothesised the ratio of STm is =/< STd joint due to which, the possibility of failure of trapeziectomy due to metacarpal collapse is insignificant. We reviewed CT scans of wrist joints of 113 eligible patientsfrom our wrist database between 2009 and 2014 for our study. 31 patients were randomised for interobserver correlation. Reformatted multi-planar sequences were analysed. The ratio of theSTm: STdin sagittal and coronal measurementswas evaluated. Interobserver variations were assessed using the Pearson coefficient.

The sex distribution included 68 males and 29 females, 49 left and 64 right wrists. The STm area was larger in 86 (76%) as compared to STd in 27(24%). Average trapezium to trapezoid ratio was 1:1.5. Ratio of area of trapezium: trapezoid joint is 0.30. The anatomic ratio of the STm in the coronal and sagittal planesis 0.3 and that of the STd joint is 0.2. Ratio of the STm: STd in the coronal plane is 0.29. Pearson's coefficient > 0.8.

A small subset of patients undergoing trapeziectomy alonefor stage II- IV carpometacarpal arthritis of the thumb are at risk of impingement of the first metacarpal due to collapse. Our assessment of the anatomical relationship of the STT joint with CT scan proves that although the area the STm joint is generally larger than the STd joint, there is no significant correlation on the whole.

We reported the outcomes of patients with Charcot neuropathy who underwent hind foot deformity correction using retrograde intramedullary nail arthrodesis. Twenty one feet in 20 patients, aged 45 to 83 years, with a mean BMI of 32.7 and a median ASA score of 3, were included in this study.

All patients presented with severe hindfoot deformities and 15 had recurrent ulceration. All patients were treated with hindfoot corrective fusion and seven patients also underwent simultaneous mid foot fusion using a bolt or locking plate. After a mean follow up of 26 months, none of the cases required any form of amputation. Eighty percent of patients with ulceration achieved healing and all but one patient returned to independent ambulation. One nail fracture and three mid foot metal work failure resulted in fixation failure requiring revision surgery. Distal locking screw displacement occurred only with standard screws but not with hydroxyapatite-coated screws. The AAOS-FAO score improved from 50.7 to 65.2 (p=0.015). The EQ-5D-5L improved from 0.63 to 0.67 (p=0.012) and the SF-36 PCS from 25.2 to 29.8 (p=0.003). Single stage deformity correction with intramedullary hindfoot arthrodesis nail is a viable treatment option for severe Charcot hindfoot deformity with ulceration and instability.

We report an unusual case of knee disease where calcific tendonitis occurring in both quadriceps and patellar tendon simultaneously in the same knee. A 47 year old female presented to orthopaedics outpatient clinic with acute onset of swelling and knee pain with no history of trauma. She was found to have a moderate effusion of the knee joint with mild tenderness over the mid quadriceps tendon. Active flexion of the knee joint was painful with a range of motion between 0-90 degrees. She is otherwise healthy with no past medical history. Plain radiographs and Magnetic Resonance Imaging (MRI) Scan revealed calcification of both tendons.

Calcific tendonitis is classically found in the supraspinatus tendon of the shoulder. In addition, it has been described in other areas of the body such as the wrist, thigh, hip, knee and ankle. This condition usually occurs in the quadriceps or patellar tendons separately and rarely affecting both tendons in the same knee simultaneously. The patients condition improved significantly with physiotherapy, anti-inflammatory medications and ultrasound therapy.

Calcific tendinitis of both quadriceps and patellar tendon is a very rare cause of knee pain. Most of the time it is treated conservatively with non-steroidal anti-inflammatory drugs and ultrasound therapy and some times steroid injection. However; patient may require surgical intervention especially in refractory cases to resolve the condition.

Background: There remains no clear guidance from the British Orthopaedic Association regarding the use of closed suction drains after primary total knee replacement (TKR). Previous studies suggest no benefit in their use and no significant difference between the type of drain used.

Aims: To assess whether allogenic transfusion requirements are altered by the use of closed suction drainage and to establish if drains reduce post-operative complications and length of stay in hospital.

Materials and Methods: 60 patients undergoing TKR in Eastbourne between December 2008 and June 2009 were evaluated pre- and post-operatively. The population was divided into those with no drain, a Redivac drain (fluid discarded) and a re-transfusion drain (fluid filtered and transfused into the patient).

Results: 23 patients had Redivac drains and 5 (22%) required an allogenic blood transfusion. 37 patients had no drain and 2 (5%) required allogenic blood. For the 14 re-transfusion drain patients none received allogenic blood. There was no significant difference in the length of stay across all 3 groups (p=> 0.25). There was a significantly higher incidence of wound ooze and cellulitis in patients with no drain (50%; p=< 0.01). This is in comparison to the other two groups where only 30% of patients with a Redivac drain and 7% of patients with a re-transfusion drain experienced these complications. There was no significant difference in the average haemoglobin drop between the 3 groups (p=> 0.1).

Conclusions: The results suggest a benefit in using drains to reduce the risk of post-operative wound ooze and cellulitis after TKR. Re-transfusion drains also appear to reduce the requirement for allogenic blood transfusion in the post-operative period.

A prospective study of 72 patients with Morton’s neuroma was carried out outlining presenting symptoms, significance of clinical examination and the beneficial effect of various treatment modalities. They were followed up for at least 6 weeks. There were 51 females (70%) and 21 males (30%) with average age of 52 years.

Bilateral symptoms were present in 15% cases with remaining 85% cases having unilateral symptoms. Commonest symptom observed was pain in the web space, commonest being 3rd space (70%) and others being 2nd space (18%), 4th space (4%) and combination of two spaces (8%). In 90% of these cases, pain was aggravated by walking and wearing closed shoes; and relieved by taking rest.

Paraesthesia in adjacent toes was present in 46% cases. Clinically palpable Mulder’s click was seen in 54% cases.

Shoe modification was tried in 33% patients, with little benefit. All 72 patients underwent corticosteroid and local anaesthetic injection in the outpatient clinic. Fair to good pain relief was obtained in 76% cases with average duration of pain relief of 2.8 weeks (range (0–8 weeks)). No pain relief was achieved in 24% cases.

Twenty-eight patients (38%) who either had inadequate pain relief at 6 weeks following injection; or had recurrence of pain eventually underwent surgical excision/decompression using plantar approach. None of them had any complication related to surgery. All patients had excellent pain relief at a minimum of 6 months follow up after the surgery. 90 % of the patients who underwent surgery had VAS pain score of 0 at 6 months follow up.

Thus, single injection treatment is a very useful treatment modality achieving satisfactory results in 76% of patients. Surgical excision/decompression should be reserved for patients with no pain relief/recurrence after the injection.
