The royal victoria hospital is a tertiary trauma centre receiving pelvic injury referrals for a population of 1.7 million. The use of ilio-sacral screw fixation with low anterior frame stabilisation has been adopted as the principle treatment for unstable pelvic ring injuries in our institution. We aim to describe our practice and outcomes following the use of percutaneous screw fixation of the pelvis. The review included standardised assessment of health-related quality of life (SF-36) as well as the Iowa pelvic score and Majeed pelvic injury outcome scores. Data was also collected on associated injuries, post-operative complications, nerve injury and pain scores. A total case series of 45 patients undergoing percutaneous ilio-sacral screw fixation following traumatic pelvic injury were identified over a 5 year period. Of these 23 were contactable to follow-up or responded to questionnaire review. The mean follow up was 680 days (range 151–1962). The mean age was 33 (range 18–57). The mean SF-36 physical and mental scores were 38 and 46 respectively. The mean Majeed score was 69 and Iowa pelvic score was 65. The mean pain score was 3.5 (range 0–7). There were no incidences of deep infection, post-operative PE or nerve injury related to screw insertion. Patients with isolated pelvic injuries performed better on outcome scoring however the low SF-36 scores highlight the severity of pelvic injuries

Introduction: Percutaneous cannulated screw placement (PCSP) is a safe method of internal fixation, indicated for pelvic ring fractures. Due to the close proximity of neurovascular structures to the path of the screw placed in either the Posterior elements (PE) or Anterior column (AC), pre-operative planning is needed to prevent injury.

This study aims to develop a pre-operative protocol for the Australian population, regarding the safe number of screws and size of screw that may be placed. Additionally, results from the study may help identify patients at increased risk of injury during PCSP.

Methods: All patients were from the Australian population and had been admitted into the emergency department at The Royal Melbourne Hospital. Control patients had no pathology in the pelvis, while treated group patients had pelvic ring fractures and were treated with PCSP.

Safe corridor measurements of the PE and AC were taken in the control patients. Pelvic CT scans, taken as part of trauma protocol, were reconstructed using 3D modelling and the dimensions of the whole (3 dimensional) safe corridor measured.

The accuracy of screw placement was determined in each treated patient. Accuracy was assessed by the screenshot method, the post-operative CT method or by both methods. In both methods, accuracy was taken as the deviation between the positions of the actual screw and planned screw.

Results: There were 22 control patients and 12 treated patients.

The mean ± (standard deviation, SD) minimum measurement of the safe corridor at the PE was 15.6 ± 2.3 mm (range 11.6 mm to 20.2 mm) and at the AC was 5.9 ±1.6 mm (range 3.0 mm to 10.0 mm).

The mean ± (SD) accuracy of screw placement was 6.1 ± 5.3 mm and ranged from a displacement of 1.3 mm to 16.1 mm.

Conclusion: The minimum dimensions of the safe corridor and the accuracy of screw placement occurred over a wide range. We recommend that dimensions of the safe corridor be assessed pre-operatively in every patient using 3D modelling to determine the safe number and size of screw that can be placed.

We reviewed 110 patients with an unstable fracture of the pelvic ring who had been treated with a trapezoidal external fixator after a mean follow-up of 4.1 years. There were eight open-book (type B1, B3-1) injuries, 62 lateral compression (type B2, B3-2) and 40 rotationally and vertically unstable (type C1-C3) injuries.

The rate of complications was high with loss of reduction in 57%, malunion in 58%, nonunion in 5%, infection at the pin site in 24%, loosening of the pins in 2%, injury to the lateral femoral cutaneous nerve in 2%, and pressure sores in 3%. The external fixator failed to give and maintain a proper reduction in six of the eight open-book injuries, in 20 of the 62 lateral compression injuries, and in 38 of the 40 type-C injuries. Poor functional results were usually associated with failure of reduction and an unsatisfactory radiological appearance. In type-C injuries more than 10 mm of residual vertical displacement of the injury to the posterior pelvic ring was significantly related to poor outcome. In 14 patients in this unsatisfactory group poor functional results were also affected by associated nerve injuries. In lateral compression injuries the degree of displacement of fractures of the pubic rami caused by internal rotation of the hemipelvis was an important prognostic factor.

External fixation may be useful in the acute phase of resuscitation but it is of limited value in the definitive treatment of an unstable type-C injury and in type-B open-book injuries. It is usually unnecessary in minimally displaced lateral compression injuries.

Unstable pelvic injuries in young children with an immature pelvis have different modes of failure from those in adolescents and adults. We describe the pathoanatomy of unstable pelvic injuries in these children, and the incidence of associated avulsion of the iliac apophysis and fracture of the ipsilateral fifth lumbar transverse process (L5-TP). We retrospectively reviewed the medical records of 33 children with Tile types B and C pelvic injuries admitted between 2007 and 2014; their mean age was 12.6 years (2 to 18) and 12 had an immature pelvis. Those with an immature pelvis commonly sustained symphyseal injuries anteriorly with diastasis, rather than the fractures of the pubic rami seen in adolescents. Posteriorly, transsacral fractures were more commonly encountered in mature children, whereas sacroiliac dislocations and fracture-dislocations were seen in both age groups. Avulsion of the iliac apophysis was identified in eight children, all of whom had an immature pelvis with an intact ipsilateral L5-TP. Young children with an immature pelvis are more susceptible to pubic symphysis and sacroiliac diastasis, whereas bony failures are more common in adolescents. Unstable pelvic injuries in young children are commonly associated with avulsion of the iliac apophysis, particularly with displaced SI joint dislocation and an intact ipsilateral L5-TP.

Determine the infection risk of nonoperative versus operative repair of extraperitoneal bladder ruptures in patients with pelvic ring injuries. Pelvic ring injuries with extraperitoneal bladder ruptures were identified from a prospective trauma registry at two level 1 trauma centers from 2014 to 2020. Patients, injuries, treatments, and complications were reviewed. Using Fisher's exact test with significance at P value < 0.05, associations between injury treatment and outcomes were determined. Of the 1127 patients with pelvic ring injuries, 68 (6%) had a concomitant extraperitoneal bladder rupture. All patients received IV antibiotics for an average of 2.5 days. A suprapubic catheter was placed in 4 patients. Bladder repairs were performed in 55 (81%) patients, 28 of those simultaneous with ORIF anterior pelvic ring. The other 27 bladder repair patients underwent initial ex-lap with bladder repair and on average had pelvic fixation 2.2 days later. Nonoperative management of bladder rupture with prolonged Foley catheterization was used in 13 patients. Improved fracture reduction was noted in the ORIF cohort compared to the closed reduction external fixation cohort (P = 0.04). There were 5 (7%) deep infections. Deep infection was associated with nonoperative management of bladder rupture (P = 0.003) and use of a suprapubic catheter (P = 0.02). Not repairing the bladder increased odds of infection 17-fold compared to repair (OR 16.9, 95% CI 1.75 – 164, P = 0.01). Operative repair of extraperitoneal bladder ruptures substantially decreases risk of infection in patients with pelvic ring injuries. ORIF of anterior pelvic ring does not increase risk of infection and results in better reductions compared to closed reduction. Suprapubic catheters should be avoided if possible due to increased infection risk later. Treatment algorithms for pelvic ring injuries with extraperitoneal bladder ruptures should recommend early bladder repair and emphasize anterior pelvic ORIF

Aims. This study aimed to analyze the correlation between transverse process (TP) fractures of the fourth (L4) and fifth (L5) lumbar vertebrae and biomechanical and haemodynamic stability in patients with a pelvic ring injury, since previous data are inconsistent. Patients and Methods. The study is a retrospective matched-pair analysis of patients with a pelvic fracture according to the modified Tile AO Müller and the Young and Burgess classification who presented to a level 1 trauma centre between January 2005 and December 2014. Results. A total of 728 patients with pelvic ring injuries were included, of whom 183 (25.1%) had a biomechanically unstable pelvic fracture. Of these patients, 84 (45.9%) had a fracture of a TP of L4 and/or L5. A total of 73 patients (13.4%) with a stable pelvic ring injury (p < 0.001) had a fracture of a TP. Patients with a fracture of a TP of L4 and/or L5 had a 5.5-fold risk (odds ratio (OR)) of having a biomechanically unstable pelvic injury. TP fractures (OR 1.6, p = 0.2) could not be confirmed as an independent predictor of haemodynamic instability. Conclusion. This is the first study that has demonstrated a positive correlation between a TP fracture of L4 and/or L5 and a biomechanically unstable pelvic ring injury. The presence of transverse process fractures of L4 and/or L5 indicates increased severity of pelvic injury and therefore can help in the planning of emergency treatment. Aims: Evaluation of: 1/ type and incidence of pelvic ring injuries, 2/ type and incidence of associated injuries, 3/ type of pelvic ring injuries pathomechanism, 4/ influence of pelvic ring injuries on trauma severity score, 5/ analysis of clinical long term results. Material and methods: Retrospective evaluation of 257 patients with pelvic ring injuries treated between 1989–2003 had been made. Mean patients age was: 65 y.o. (Range: 32–61 y.o.). Mean follow-up time: 57,5 months (range: 18–192 m.). All patients were treated no operatively: bed-rest, hamaque suspension, and skeletal traction. Analysis of pelvic injuries was made by Tiles classification. Evaluation of pelvic injuries pathomechanism was made by Young-Burgess classification. Clinical end results were evaluated by Iowa Pelvic Score. In polytraumatized patients trauma severity was evaluated by AIS and ISS. Results: Incidence of pelvic injury in hospitalized patients was 1,3%. In study group there were following pelvic injury types: type A – 14 %, type B – 77 % and type C – 9%. Pathomechanisms of pelvic injuries in study group were following: type LC – 61%, type APC – 30%, type VS – 2%, type CMI – 7%. Long term clinical results in Iowa pelvic Score were for pelvic ring injuries: type A – 92 p., type B – 86 p., type C – 67 p. Mean value of ISS index in patients with pelvic ring injuries was 23p, mean value of ISS index in patients without pelvic ring injuries was 19p. Conclusions: 1/ pelvic ring injuries in standard orthopedic and traumatic department are rare, 2/ pelvic ring injuries are frequent in polytraumatized patients, 3/ type C according to Tile’s classification is the most frequent type of pelvic ring injury, 4/ type LC according to Young-Burgess classification is the most frequent type of pelvic ring injury pathomechanism, 5/ incidence of pelvic ring injury in polytraumatized patient increases ISS index, 6/ long term clinical results suggested that most pelvic ring injuries can be successfully treated nonoperatively

Purpose of study. To review the treatment and outcomes of paediatric pelvic ring injuries in the UK. Methods and results. We performed a retrospective review of all pelvic fracture admissions to an English paediatric major trauma centre (MTC) from 2012 to 2016. A total of 29 patients were admitted with pelvic ring injuries with a mean age of 11 years (4- 16yrs). Road traffic accident was the mechanism in majority (72%), followed by fall from height (24%). Femoral shaft fracture was present in 5 (17%), head injury in 5 (17%), chest injuries in 5 (17%) and bladder injury in one child. 48% patients needed surgical procedures for fractures or associated injuries. We differentiated injuries according to the classification system of Torode and Zeig. 17% were Type A, 3% Type B, 48% Type C and 31% Type D. Almost all (93%) patients were treated conservatively. 51% of patients were allowed to mobilize full weight bearing after a period of bed rest. Non-weight bearing mobilization was recommended for fractures extending into the acetabulum, sacral fractures, unstable fracture patterns or associated fractures (neck of femur, femoral shaft and tibial shaft). Surgical fixation occurred in two patients. Both of these patients had significantly displaced Type D fractures. Only 44% of patients were back to sports at six months. Conclusions. Pelvic ring injuries are rare within the paediatric population and are associated with a high incidence of concomitant injury and significant functional morbidity. Their treatment should involve a multidisciplinary approach, which includes specialist in the care of pelvic trauma

To evaluate efficacy and outcome of embolisation following pelvic ring injuries in patients presented with ongoing hypovolaemic shock. Between 2000 and 2003, 200 poly-trauma patients presented in our institutions following pelvic ring injuries. Those with ongoing hypovolaemic shock who were treated within 24h of admission with embolisation were included in this study. Demographics, mechanism of injury, ISS, type of pelvic ring fracture, arterial source of bleeding, hours from injury to embolisation, and outcome were all recorded prospectively. Out of the 200 treated in our institutions 17 (8.5%) underwent angio-embolisation. The mean age of the patients was 37 (14-70) and the mean ISS was 29. Distribution of pelvic ring injuries included: 3LC, 7APC, 7VS. The mean time from injury to embolisation was 15 hours. 8/17 patients were initially treated with an external fixator. The distribution of arterial injuries was: 7 superior gluteal arteries, 8 internal iliac arteries, 1 obturator artery and 1 internal pudental artery. The mean number of units transfused prior to embolisation was 22 (range 6-50). Mortality rate was 4 (23%) out of 17 embolised patients. Angio-embolisation for pelvic ring injuries occurred in 8.5% of our study population. This study indicates that only a small proportion of patients required embolisation secondary to arterial bleeding. The overall survival rate was in accordance to published international experience. Embolisation should be considered as a valid adjunct in some selected group of patients with pelvic fractures where ongoing bleeding refractory to other treatment modalities is present

Aim of study: To evaluate infectious complications in patients operated for pelvic ring injury (level 1 trauma-centre). Method: Retrospective study of patients treated for pelvic ring injury in 5 years (July 1999 – June 2004), in whom wound infection was diagnosed clinically (purulent secretion) or by microbiological culture. Results: In the period 102 patients with pelvic ring injury as part of polytrauma were treated in trauma-centre. Thirteen patients died during few hours after admission without any surgical treatment of pelvis fracture and were excluded from the study group. Type A fracture of AO/ASIF classification was diagnosed in 18 (20 %), type B in 48 (54 %), type C in 23 patients (26 %). Forty three patients were operated (25 ORIF, 18 external fixation). Postoperative infection was diagnosed in 4 patients (9 % of all surgically treated) with mixed bacterial flora isolated in 3 patients and negative culture in 1 patient. In the same time period the frequency of infectious complications after the surgical treatment of musculoskeletal injuries was 1,3 % and mixed bacterial flora was detected in only 40 % of them. Conclusion: In patients surgically treated for pelvic ring injury the occurence of infectious complications is significantly higher than in patients surgically treated for other skeletal injury. Traditionally, unstable anterior pelvic ring injuries have been stabilised with an external fixator or by internal fixation. Recently, a new percutaneous technique of placement of bilateral supraacetabular polyaxial screws and subcutaneous connecting bar to assemble an “internal fixator” has been described. We present the surgical technique and early clinical results of using this technique in twenty-five consecutive patients with a rotationally unstable pelvic ring injury and no diastasis of the symphysis pubis treated between April 2010 and December 2013. Additional posterior pelvic stabilisation with percutaneous iliosacral screws was used in 23 of these patients. The anterior device was routinely removed after three months. Radiological evidence of union of the anterior pelvic ring was seen in 24 of 25 patients at a minimum 6 month follow-up. Thirteen patients developed sensory deficits in the lateral femoral cutaneous nerve (five bilateral) and only one fully recovered. The anterior pelvic internal fixator is a reliable, safe and easy percutaneous technique for the treatment of anterior pelvic ring injuries, facilitating the reduction and stabilisation of rotational displacement. However, lateral femoral cutaneous nerve dysfunction is common. The technique is recommended in cases with bilateral or unilateral pubic rami fractures and no diastasis of the symphysis pubis

Sacral fractures are often underdiagnosed, but are frequent in the setting of pelvic ring injuries. They are mostly caused by high velocity injuries or they can be pathological in aetiology. We sought to assess the clinical outcomes of the surgically treated unstable sacral fractures, with or without neurological deficits. unstable sacral fractures were included in the study. Single centre, prospectively collected data, retrospective review of patients who sustained vertically unstable fractures of the sacrum who underwent surgical fixation. out of a total of 432 patients with pelvis and acetabulum injuries. fifty six patients met the inclusion criteria. 18 patients had sustained zone one injuries. 14 patients had zone 2 injuries and 10 patients had zone 3 injurie. Operative fixation was performed percutaneously using cannulated screws in 18 patients.. Open fixation of the sacrum using the anterior approach in 6 patients. Posterior approach was indicates in all 10 of the zone 3 injuries of the sacrum. While in 4 patients, combined approaches were used. 3 patients had decompression and spinopelvic fixation. Neurological deficits were present in 16% of the patients. 2 patients presented with neurgenic bladder. Of the 4 patients who had neurological fall out, 3 resolved with posterior decompression and posterior fixation. All 4 neurological deficits were due to taction or compression of the nerve roots. No hardware failures or non unions observed. The rate of neurological deficit was related more to the degree of pelvic ring instability than to a particular fracture pattern. Low rates of complications and successful surgical treatment of sacral fractures is achiavable. Timeous accurate diagnosis mandatory

Variations in pelvic anatomy are a major risk factor for misplaced percutaneous sacroiliac screws used to treat unstable posterior pelvic ring injuries. A better understanding of pelvic morphology improves preoperative planning and therefore minimises the risk of malpositioned screws, neurological or vascular injuries, failed fixation or malreduction. Hence a classification system which identifies the clinically important anatomical variations of the sacrum would improve communication among pelvic surgeons and inform treatment strategy. 300 Pelvic CT scans from skeletally mature trauma patients that did not have pre-existing posterior pelvic pathology were identified. Axial and coronal transosseous corridor widths at both S1 and S2 were recorded. Additionally, the S1 lateral mass angle were also calculated. Pelvises were classified based upon the sacroiliac joint (SIJ) height using the midpoint of the anterior cortex of L5 as a reference point. Four distinct types could be identified:. Type-A – SIJ height is above the midpoint of the anterior cortex of the L5 vertebra. Type-B – SIJ height is between the midpoint and the lowest point of the anterior cortex of the L5 vertebra. Type-C – SIJ height is below the lowest point of the anterior cortex of the L5 vertebra. Type-D – a subgroup for those with a lumbosacral transitional vertebra, in particular a sacralised L5. Differences in transosseous corridor widths and lateral mass angles between classification types were assessed using two-way ANOVAs. Type-B was the most common pelvic type followed by Type-A, Type-C, and Type-D. Significant differences in the axial and coronal corridors was observed for all pelvic types at each level. Lateral mass angles increased from Types-A to C, but were smaller in Type-D. This classification system offers a guide to surgeons navigating variable pelvic anatomy and understanding how it is associated with the differences in transosseous sacral corridors. It can assist surgeons’ preoperative planning of screw position, choice of fixation or the need for technological assistance

Introduction: Injuries to the bony pelvis are a significant group of injuries with often serious implications. The close proximity of the bony pelvis to vital organs, it’s involvement in weight bearing and ambulation, and the high energy mechanisms usually required to produce these injuries produces a variety of injury patterns, associated injuries and possible outcomes. Aims: To determine the long-term functional outcomes of displaced pelvic ring injuries treated surgically in the United Kingdom. Methods: All patients with pelvic ring fractures treated surgically were contacted by postal questionnaire. Outcomes were measured using the SF-36, as well as ad-hoc tools including return to work and sporting activities. Injuries were classified using the Young and Burgess (YB) classification system. Results: There was a response rate of 70% (145 of 209 patients). Average age at injury was 40 yrs (16–74 yrs). Average follow up was 5.3yrs (1–12 yrs). There were 45 Antero-Posterior Compression (APC) injuries, 51 Lateral Compression (LC) injuries and 49 Vertical Shear (VS) injuries. 69% of the LC injuries had returned to some form of employment, compared with 58% of the APC injuries and 51% of the VS. 39% of the LC injuries had returned to their pre-injury sporting activities, compared with 27% of the APC and 33% of the VS. The average Physical Functioning Score of the SF-36 outcome tool was 73.2 for the LC injuries, 61.7 for the APC injuries and 63.3 for the VS injuries (scale 0–100, 100 representing best status). These trends were mirrored in the other outcome domains of the SF-36 tool. Conclusions: These results illustrate the long-term morbidity associated with pelvic ring injuries and relationship with injury subtypes. LC injuries appeared to perform better than APC and VS injuries in all outcome measures utilized. These findings may aid in determining the prognosis and provision of services for patients with pelvic ring injuries

The rate of arterial injury in trauma patients with pelvic ring fractures has been cited as high as 15%. Addressing this source of hemorrhage is essential in the management of these patients as mortality rates are reported as 50%. Percutaneous techniques to control arterial bleeding, such as embolization and REBOA, are being employed with increasing frequency due to their assumed lower morbidity and invasiveness than open exploration or cross clamping of the aorta. There are promising results with regards to the mortality benefits of angioembolization. However, there are concerns with regards to morbidity associated with embolization of the internal iliac vessels and its branches including surgical wound infection, gluteal muscle necrosis, nerve injury, bowel infarction, and thigh / buttock claudication. The primary aim of this study is to determine whether pelvic arterial embolization is associated with surgical site infection (SSI) in trauma patients undergoing pelvic ring fixation. This observational cohort study was conducted using US trauma registry data from the American College of Surgeons (ACS) National Trauma Database for the year of 2018. Patients over the age of 18 who were transported through emergency health services to an ACS Level 1 or 2 trauma hospital and sustained a pelvic ring fracture treated with surgical fixation were included. Patients who were transferred between facilities, presented to the emergency department with no signs of life, presented with isolated penetrating trauma, and pregnant patients were excluded from the study. The primary study outcome was surgical site infection. Multivariable logistic regression was performed to estimate treatment effects of angioembolization of pelvic vessels on surgical site infection, adjusting for known risk factors for infection. Study analysis included 6562 trauma patients, of which 508 (7.7%) of patients underwent pelvic angioembolization. Overall, 148 (2.2%) of patients had a surgical site infection, with a higher risk (7.1%) in patients undergoing angioembolization (unadjusted odds ratio (OR) 4.0; 95% CI 2.7, 6.0; p < 0 .0001). Controlling for potential confounding, including patient demographics, vitals on hospital arrival, open fracture, ISS, and select patient comorbidities, pelvic angioembolization was still significantly associated with increased odds for surgical site infection (adjusted OR 2.0; 95% CI 1.3, 3.2; p=0.003). This study demonstrates that trauma patients who undergo pelvic angioembolization and operative fixation of pelvic ring injuries have a higher surgical site infection risk. As the use of percutaneous hemorrhage control techniques increase, it is important to remain judicious in patient selection

Introduction: Early mortality associated with unstable pelvic ring injuries is often secondary to continuous pelvic bleeding. Hemostatic measures such as pelvic binders or external fixation may help to control low pressure bleeding from lacerated veins or broad fracture surfaces, while control of high pressure arterial bleeding may require embolization. Purpose: Evaluate our experience with the control of hemorrhagic shock associated with pelvic ring injuries during initial patient management. Methods: From January 2003 until December 2006, all [105] patients admitted to our level I trauma center with a pelvic or an acetabular fracture were prospectively entered into our polytrauma data base. Of 105 patients, 67 were classified with a type B or C pelvic fracture. All these patients received a pelvic strap belt by the paramedic team at the scene of the accident. Pelvic fractures were diagnosed on the initial anteroposterior pelvic radiograph and computed tomography. From this initial group of 67 patients, we identified 38 as unstable requiring blood transfusion and intensive care monitoring. The results and survival rate were evaluated according to the initial sequence of surgical procedures and the patients were divided into 3 groups, X,Y, and Z Follow-up physical examination and radiographs was performed for all survivors at an average of 10 months post-injury (range, 6 months to 3 years). Results: The average age of the 38 patients was 38.6 years (range, 24–51 years) and their average ISS was 53 (range 21–75).All were injured in a high velocity motor vehicle accident or a fall from a height. The patients were managed in the emergency department by a multidisciplinary team according A.T.L.S. guidelines. Of the 38 patients, five died shortly after arrival in the emergency department despite resuscitation efforts. Within the first 24 hours, pelvic stabilization was performed in 27 patients with either an anterior external fixator frame (n=13), pelvic clamp (n=11) or primary open reduction internal fixation (n=3). In group X, of 19 patients initially treated with external fixation and eventual arterial embolization without laparotomy, 18 (94 %) survived. In group Y, there were 8 patients treated by external fixation, eventual arterial embolization and laparotomy, and 7 (87 %) survived. In group Z, all 6 patients in whom a scratch laparotomy with packing prior to any skeletal fixation was attempted,no patient survived ! All survivors underwent definitive open reduction and plate and screw fixation, with an average ICU stay of 10 days (3–15). Conclusion: This study shows that optimal control of bleeding associated with pelvic ring injuries is achieved by initial skeletal fixation prior to any other surgical procedures. Immediate laparotomy was associated with a high rate of intraoperative death due to the failure to control bleeding

Introduction: Unilateral posterior Pelvic Ring injuries but especially bilateral sacral fractures or bilateral sacroiliac joint (SI) ruptures as well as lumbosacral dislocations and fracture dislocations remains a significant surgical challenge. 1. ,. 2. ,. 3. despite advances in surgical techniques. Although the true incidence of these fractures are unknown, 30% are identified late. 4. The treatment of those fractures varies from conservative treatment, posterior plate fixation, anterior plating as well as percutaneous and open Sacroiliac (SI) joint screws. However, screw pull-outs and loss of fixation in those methods are well described In the Alfred Hospital, Melbourne (Australia) a Level 1 Trauma Center a series of 14 patients were treated from 10/2006 to date with a multiaxial spinal system. Methods: Patients with posterior pelvic injuries separation were identified prospectively since October 2006. Data was extracted from the trauma registry database and medical record and diagnostic imaging. Since Ocober 2006, 10 patients with bilateral posterior pelvic ring injuries and 4 with unilateral injuries were identified for fixation. Technique: The patients were put supine and a incision medial/distalto the posterior iliac spine was made. The placement for the incision gives the surgeon the opportunity to estend the approach to an open reduction of the sacral fracture or SI Joint disruption if a closed reduction cannot be achieved. A pedicel screw from a multiaxial spinal system (Xia, Stryker or Pangea, Synthes) is placed percutaneously in the posterior iliac crest on both sides and the reduction is performed with the screws attached to the screw handles and with Image Intensifier. After the reduction the multiaxial screwheads are bent and transfixed with a bar which is tunneled epifacial. All patients underwent a multislice pelvic and lumbar spine CT and these patients were assessed clinically for neurovascular symptoms and stability. The follow-up included clinical assessment and CT imaging. Results: Since October 2006 14 patients (10 male, 4 female) with an average age of 32.4 years (range: 20–44 years, median 33 years) and an average ISS (Injury Severity Score) of 37 (range: 14–66, median 34). The mechanism of injury for these patients included: pedestrians versus car; motorcylce; paragliding and motor car collision. All patients had associated anterior pelvic ring injuries which were internally fixed in all but one case. The follow up time was one to 18 month. The patients were assessed clinically and with CT imaging. No complications or loss of fixation have been observed in this patient group in this short follow up time. Discussion: The fixation system is highly versaitle and the whole posterior iliac crest can be used for fixation. The posterior instrumentation provides also a good control of the reduction of anterior pelvic ring fractures which should be fixed when associated. In all cases but 3 the nature of the comminuted sacral fractures did not allow the use of SI-Joint screws or anterior SI-Joint plating. The construct provides initial stability and allows mobilization of the patient. It can be used in cases with sacral comminution and may offer advantages over posterior plate fixation, by reducing complications with prominent metalware

We report the functional and socioeconomic long-term outcome of patients with pelvic ring injuries. We identified 109 patients treated at a Level I trauma centre between 1973 and 1990 with multiple blunt orthopaedic injuries including an injury to the pelvic ring, with an Injury Severity Score (ISS) of ≥ 16. These patients were invited for clinical review at a minimum of ten years after the initial injury, at which point functional results, general health scores and socioeconomic factors were assessed. In all 33 isolated anterior (group A), 33 isolated posterior (group P) and 43 combined anterior/posterior pelvic ring injuries (group A/P) were included. The mean age of the patients at injury was 28.8 years (5 to 55) and the mean ISS was 22.7 (16 to 44). At review the mean Short-Form 12 physical component score for the A/P group was 38.71 (22.12 to 56.56) and the mean Hannover Score for Polytrauma Outcome subjective score was 67.27 (12.48 to 147.42), being significantly worse compared with the other two groups (p = 0.004 and p = 0.024, respectively). A total of 42 patients (39%) had a limp and 12 (11%) required crutches. Car or public transport usage was restricted in 16 patients (15%). Overall patients in groups P and A/P had a worse outcome. The long-term outcome of patients with posterior or combined anterior/posterior pelvic ring injuries is poorer than of those with an isolated anterior injury. There is controversy regarding the optimum method of stabilising traumatic anterior pelvic ring injuries. This study aimed to evaluate the role of pubic symphysis plating. Methods. All patients who underwent pubic symphysis plating in a regional pelvic and acetabular unit were studied. Fracture classification, type of fixation, complications, and incidence of metalwork failure were recorded. Results. Out of 178 consecutive patients, 159 (89%) were studied for a mean of 37.6 months. There were 121 males and 38 females (mean age 43 years). Symphysis pubic fixation was performed in 100 AO-OTA type B and 59 type C injuries using a Matta symphyseal plate (n=92), reconstruction plate (n=65), or DCP (n=2). Supplementary posterior pelvic fixation was performed in 102 patients. 5 patients required revision for failure of fixation or symptomatic instability of the pubic symphysis. A further 7 patients had metalwork removed for other reasons. Metalwork breakage occurred in 63 patients (40%). 62 of these 63 patients were asymptomatic and metalwork was left in situ. Conclusions. Plate fixation of the symphysis pubis is an effective method of stabilising anterior pelvic ring injuries with a low complication rate. There is a high rate of late metalwork breakage, but this is not clinically significant
