The aim of this study was to evaluate the suitability of the tapered cone stem in total hip arthroplasty (THA) in patients with excessive femoral anteversion and after femoral osteotomy. We included patients who underwent THA using Wagner Cone due to proximal femur anatomical abnormalities between August 2014 and January 2019 at a single institution. We investigated implant survival time using the endpoint of dislocation and revision, and compared the prevalence of prosthetic impingements between the Wagner Cone, a tapered cone stem, and the Taperloc, a tapered wedge stem, through simulation. We also collected Oxford Hip Score (OHS), visual analogue scale (VAS) satisfaction, and VAS pain by postal survey in August 2023 and explored variables associated with those scores.Aims

The primary objective of this study was to develop a validated classification system for assessing iatrogenic bone trauma and soft-tissue injury during total hip arthroplasty (THA). Dual Mobility (DM) Total Hip Replacements (THRs), are becoming widely used but function in-vivo is not fully understood. The aim of this study was to compare the incidence of impingement of a modular dual mobility with that of a standard cup. A geometrical model of one subject's bony anatomy \[1\] was developed, a THR was implanted with the cup at a range of inclination and anteversion positions (Corail® stem, Pinnacle® cup (DePuy Synthes)). Two DM variants and one STD acetabular cup were modelled. Joint motions were taken from kinematic data of activities of daily living associated with dislocation \[2\] and walking. Custom triflange acetabular components (CTACs) play an important role in reconstructive orthopaedic surgery, particularly in revision total hip arthroplasty (rTHA) and pelvic tumour resection procedures. Accurate CTAC positioning is essential to successful surgical outcomes. While prior studies have explored CTAC positioning in rTHA, research focusing on tumour cases and implant flange positioning precision remains limited. Additionally, the impact of intraoperative navigation on positioning accuracy warrants further investigation. This study assesses CTAC positioning accuracy in tumour resection and rTHA cases, focusing on the differences between preoperative planning and postoperative implant positions. A multicentre observational cohort study in Australia between February 2017 and March 2021 included consecutive patients undergoing acetabular reconstruction with CTACs in rTHA (Paprosky 3A/3B defects) or tumour resection (including Enneking P2 peri-acetabular area). The aim of this study was to identify the optimal lip position for total hip arthroplasties (THAs) using a lipped liner. There is a lack of consensus on the optimal position, with substantial variability in surgeon practice. A model of a THA was developed using a 20° lipped liner. Kinematic analyses included a physiological range of motion (ROM) analysis and a provocative dislocation manoeuvre analysis. ROM prior to impingement was calculated and, in impingement scenarios, the travel distance prior to dislocation was assessed. Iliopsoas tendonitis occurs in up to 30% of patients after hip resurfacing arthroplasty (HRA) and is a common reason for revision. The primary purpose of this study was to validate our novel computational model for quantifying iliopsoas impingement in HRA patients using a case-controlled investigation. Secondary purpose was to compare these results with previously measured THA patients. We conducted a retrospective search in an experienced surgeon's database for HRA patients with iliopsoas tendonitis, confirmed via the active hip flexion test in supine, and control patients without iliopsoas tendonitis, resulting in two cohorts of 12 patients. The CT scans were segmented, landmarked, and used to simulate the iliopsoas impingement in supine and standing pelvic positions. Three discrete impingement values were output for each pelvic position, and the mean and maximum of these values were reported. Cup prominence was measured using a novel, nearest-neighbour algorithm. The mean cup prominence for the symptomatic cohort was 10.7mm and 5.1mm for the asymptomatic cohort (p << 0.01). The average standing mean impingement for the symptomatic cohort was 0.1mm and 0.0mm for the asymptomatic cohort (p << 0.01). The average standing maximum impingement for the symptomatic cohort was 0.2mm and 0.0mm for the asymptomatic cohort (p << 0.01). Impingement significantly predicted the probability of pain in logistic regression models and the simulation had a sensitivity of 92%, specificity of 91%, and an AUC ROC curve of 0.95. Using a case-controlled investigation, we demonstrated that our novel simulation could detect iliopsoas impingement and differentiate between the symptomatic and asymptomatic cohorts. Interestingly, the HRA patients demonstrated less impingement than the THA patients, despite greater cup prominence. Aims. Iliopsoas impingement occurs in 4% to 30% of patients after undergoing total hip arthroplasty (THA). Despite a relatively high incidence, there are few attempts at modelling impingement between the iliopsoas and acetabular component, and no attempts at modelling this in a representative cohort of subjects. The purpose of this study was to develop a novel computational model for quantifying the impingement between the iliopsoas and acetabular component and validate its utility in a case-controlled investigation. Methods. This was a retrospective cohort study of patients who underwent THA surgery that included 23 symptomatic patients diagnosed with iliopsoas tendonitis, and 23 patients not diagnosed with iliopsoas tendonitis. All patients received postoperative CT imaging, postoperative standing radiography, and had minimum six months’ follow-up. 3D models of each patient’s prosthetic and bony anatomy were generated, landmarked, and simulated in a novel iliopsoas impingement detection model in supine and standing pelvic positions. Logistic regression models were implemented to determine if the probability of pain could be significantly predicted. Receiver operating characteristic curves were generated to determine the model’s sensitivity, specificity, and area under the curve (AUC). Results. Highly significant differences between the symptomatic and asymptomatic cohorts were observed for iliopsoas impingement. Logistic regression models determined that the impingement values significantly predicted the probability of groin pain. The simulation had a sensitivity of 74%, specificity of 100%, and an AUC of 0.86. Conclusion. We developed a computational model that can quantify iliopsoas impingement and verified its accuracy in a case-controlled investigation. This study aimed to evaluate the accuracy of implant placement with robotic-arm assisted total hip arthroplasty (THA) in patients with developmental dysplasia of the hip (DDH). The study analyzed a consecutive series of 69 patients who underwent robotic-arm assisted THA between September 2018 and December 2019. Of these, 30 patients had DDH and were classified according to the Crowe type. Acetabular component alignment and 3D positions were measured using pre- and postoperative CT data. Hip arthroplasty does not always restore normal anatomy. This is due to inaccurate surgery or lack of stem sizes. We evaluated the aptitude of four total hip arthroplasty systems to restore an anatomical and medialized hip rotation centre. Using 3D templating software in 49 CT scans of non-deformed femora, we virtually implanted: 1) small uncemented calcar-guided stems with two offset options (Optimys, Mathys), 2) uncemented straight stems with two offset options (Summit, DePuy Synthes), 3) cemented undersized stems (Exeter philosophy) with three offset options (CPT, ZimmerBiomet), and 4) cemented line-to-line stems (Kerboul philosophy) with proportional offsets (Centris, Mathys). Methods
Introduction. Lumbar spine fusion in patients undergoing THA (total hip arthroplasty) is a known risk factor for hip dislocation with some studies showing a 400% increased incidence compared to the overall THA population. Reduced spine flexibility can effectively narrow the cup anteversion safe zone while alterations in pelvic tilt can alter the center of the anteversion safe zone. The use of precision cup alignment technology combined with patient-specific cup alignment goals based on preoperative assessment has been suggested as a method of addressing this problem. The current study assess the dislocation rate of THA patients with stiff or fused lumbar spines treated using surgical navigation with patient-specific cup orientation goals. Methods. Seventy-five THA were performed in 54 patients with a diagnosis of lumbar fusion, lumbar disc replacement, and scoliosis with Cobb angles greater than 40 degrees were treated by the senior author (SM) as part of a prospective, non-randomized study of surgical navigation in total hip arthroplasty. All patients were treated using a smart mechanical navigation tool for cup alignment (HipXpert System, Surgical Planning Associates, Inc., Boston, MA). Cup orientation goals were set on a patient-specific basis using supine pelvic tilt as measured using CT. Patients with increased pelvic tilt had a goal for increased cup anteversion and patients with decreased pelvic tilt had a goal for decreased cup anteversion (relative to the anterior pelvic plane coordinate system). Each patient's more recent outpatient records were assessed for history of dislocation, instability, mechanical symptoms, decreased range of motion or progressive pain. Additionally, last clinic radiographs were reviewed to confirm lumbar pathology in the form of spinal surgical hardware. Results. Seventy-five total hip arthroplasties with stiff lumbar spine were reviewed with and average follow up of 6.04 years. The average number of levels of lumbar fusion was 2.3 levels. Appropriate acetabular component placement has been proposed for prevention of postoperative dislocation in total hip arthroplasty (THA). Manual placements often cause outliers in spite of attempts to insert the component within the intended safe zone; therefore, some surgeons routinely evaluate intraoperative pelvic radiographs to exclude excessive acetabular component malposition. However, their evaluation is often ambiguous in case of the tilted or rotated pelvic position. The purpose of this study was to develop the computational analysis to digitalize the acetabular component orientation regardless of the pelvic tilt or rotation. Intraoperative pelvic radiographs of 50 patients who underwent THA were collected retrospectively. The 3D pelvic bone model and the acetabular component were image-matched to the intraoperative pelvic radiograph. The radiological anteversion (RA) and radiological inclination (RI) of the acetabular component were calculated and those measurement errors from the postoperative CT data were compared relative to those of the 2D measurements. Methods
Introduction. Previous research defines the existence of a “safe zone” (SZ) pertaining to acetabular cup implantation during total hip arthroplasty (THA). It is believed that if the cup is implanted at 40°±10° inclination and 15°±10° anteversion, risk of dislocation is reduced. However, recent studies have documented that even when the acetabular cup is placed within the SZ, high incidence dislocation and instability remains due to the combination of patient-specific configuration, cup diameter, head size, and surgical approach. The SZ only investigates the angular orientation of the cup, ignoring translational location. Translational location of the cup can cause a mismatch between anatomical hip center and implanted cup center, which has not been widely explored. Objective. The objective of this study is to define a zone within which the implanted joint center can be altered with respect to the anatomical joint center but will not increase the likelihood of post-operative hip separation or dislocation. Methods. A theoretical forward solution hip model, previously validated by telemetric devices and fluoroscopy data of existing implants, was used for analysis. The model allows for modifications of implant geometries/placement and soft tissue resection to simulate various surgical conditions. For the baseline simulation, the cup center was matched to the anatomical hip joint center, calculated as the center of the best fit sphere mapping the acetabulum, and the orientation of the cup was 40°/15° (inclination/anteversion). Keeping cup orientation the same, the location of the cup was moved in 1 mm increments in all directions to identify the region where a mismatch between the two centers did not lead to separation or instability in the joint. Results. During both swing and stance phase, when the acetabular cup was placed within the optimal conic with a slant height of 5±1 mm, no hip instability or dislocation risk occurred. As the acetabular cup was translated to the boundary of the optimal conic, hip instability increased. When the acetabular cup was placed at the boundary of the optimal conic, up to 2 mm of hip separation in the lateral direction occurred during swing phase, resulting in a decrease in contact area and an increase in contact stress. As the cup was placed outside the optimal conic, severe edge loading and hip separation up to 3.5 mm occurred during swing phase. In general, this resulted in large increases in cup stress, resulting in increased risk of wear leading to early complications. Discussion. This study introduces the concept of an optimal conic in the hip joint space to reduce the incidence of dislocation and hip instability after THA. Placing the cup center within the optimal conic reduces hip instability. Moving the cup further from the anatomical hip center increases the occurrence of hip instability.

Pre-operative knowledge. Knowledge-based total hip arthroplasty is becoming increasingly recognised for improved safety, efficiency, and accuracy. Pre-operative knowledge of native and planned femoral anteversion, the exact size of implants, neck length and offset, and head lengths can serve to safely accelerate surgery and reduce the need for intra-operative imaging. Pre-operative knowledge of the effect on change in leg length and offset effected by specific implant combinations can serve to minimise undesired changes. The use of a smart mechanical navigation tool superimposed on this knowledge, can serve to easily and swiftly achieve optimal component position. Cost savings. Economic data from Q1 2013 to Q2 2016 demonstrate that CMS-insured patients treated by knowledge-based surgery using the HipXpert mechanical navigation system combined with the superior hip approach have the lowest cost of all patients treated in Massachusetts by an average of more than $7,000 over 90 days for Medicare Part A expenditure (HipXpert System, Surgical Planning Associates, Boston, MA).
