Aims

Between 2002 and 2011, 81 patients with a traumatic total brachial plexus injury underwent reconstruction by double free muscle transfer (DFMT, 47 cases), single muscle transfer (SMT, 16 cases) or nerve transfers (NT, 18 cases).

Methods

They were evaluated for functional outcome and quality of life (QoL) using the Disability of Arm, Shoulder and Hand questionnaire, both pre- and post-operatively. The three groups were compared and followed-up for at least 24 months.

We report our experience with glenohumeral arthrodesis as a salvage procedure for epilepsy-related recurrent shoulder instability. A total of six patients with epilepsy underwent shoulder fusion for recurrent instability and were followed up for a mean of 39 months (12 to 79). The mean age at the time of surgery was 31 years (22 to 38). Arthrodesis was performed after a mean of four previous stabilisation attempts (0 to 11) in all but one patient in whom the procedure was used as a primary treatment. All patients achieved bony union, with a mean time to fusion of 2.8 months (2 to 7). There were no cases of re-dislocation. One revision was undertaken for loosening of the metalwork, and then healed satisfactorily. An increase was noted in the mean subjective shoulder value, which improved from 37 (5 to 50) pre-operatively to 42 (20 to 70) post-operatively although it decreased in two patients. The mean Oxford shoulder instability score improved from 13 pre-operatively (7 to 21) to 24 post-operatively (13 to 36). In our series, glenohumeral arthrodesis eliminated recurrent instability and improved functional outcome. Fusion surgery should therefore be considered in this patient population. However, since the majority of patients are young and active, they should be comprehensively counselled pre-operatively given the functional deficit that results from the procedure.

Gunshot injuries to the shoulder are rare and difficult to manage. We present a case series of seven patients who sustained a severe shoulder injury to the non-dominant side as a result of a self-inflicted gunshot wound. We describe the injury as ‘suicide shoulder’ caused by upward and outward movement of the gun barrel as the trigger is pulled. All patients were male, with a mean age of 32 years (21 to 48). All were treated at the time of injury with initial repeated debridement, and within four weeks either by hemiarthroplasty (four patients) or arthrodesis (three patients). The hemiarthroplasty failed in one patient after 20 years due to infection and an arthrodesis was attempted, which also failed due to infection. Overall follow-up was for a mean of 26 months (12 to 44). All four hemiarthroplasty implants were removed with no feasible reconstruction ultimately possible, resulting in a poor functional outcome and no return to work. In contrast, all three primary arthrodeses eventually united, with two patients requiring revision plating and grafting. These patients returned to work with a good functional outcome. We recommend arthrodesis rather than replacement as the treatment of choice for this challenging injury.

In adults with brachial plexus injuries, lack of active external rotation at the shoulder is one of the most common residual deficits, significantly compromising upper limb function. There is a paucity of evidence to address this complex issue. We present our experience of isolated latissimus dorsi (LD) muscle transfer to achieve active external rotation. This is a retrospective review of 24 adult post-traumatic plexopathy patients who underwent isolated latissimus dorsi muscle transfer to restore external rotation of the shoulder between 1997 and 2010. All patients were male with a mean age of 34 years (21 to 57). All the patients underwent isolated LD muscle transfer using a standard technique to correct external rotational deficit. Outcome was assessed for improvement in active external rotation, arc of movement, muscle strength and return to work. The mean improvement in active external rotation from neutral was 24° (10° to 50°). The mean increase in arc of rotation was 52° (38° to 55°). Mean power of the external rotators was 3.5 Medical Research Council (MRC) grades (2 to 5).

A total of 21 patients (88%) were back in work by the time of last follow up. Of these, 13 had returned to their pre-injury occupation. Isolated latissimus dorsi muscle transfer provides a simple and reliable method of restoring useful active external rotation in adults with brachial plexus injuries with internal rotational deformity.

Purpose: Difficult nonunions of the humerus are frequently treated with free vascularized bone, usually the fibula, but donor site morbidity from the lower leg can be significant. The lateral border of the scapula is a 7–12 cm tubular length of vascular bone that can be easily transferred to the humerus. Method: Nineteen established nonunions of the humerus, including mid-shaft (11), proximal (4), distal (3), and a failed shoulder fusion, were treated with a segment of scapula on a vascular pedicle. Patients (13 females, 6 males with average age of 63) had undergone 3 previous surgeries on average in attempts to heal the fracture. The time from injury to procedure averaged 28 months (range, 5–120 months). The surgical team consisted of an orthopedist and a plastic surgeon working simultaneously with the patient in a lateral position. The scapular graft was juxtaposed to the humerus through an axillary tunnel, then rigidly fixed with plate and screws and grafted with a calcium sulfate and bio-assayed demineralized bone matrix product (Allomatrix. ™. , Wright Medical Technology, Arlington TN). Results: All nonunions healed, achieving radiographic evidence of consolidation between 8–24 weeks (average, 13 weeks). One patient with a previously infected non-union had recurrent infection and required further surgery before healing. Another patient fractured through the mid-portion of the healed graft in a fall two years after surgery and was revised to an endoprosthesis. Patient-based Musculoskeletal Tumor Society scores averaged 83% at 41 months (range, 24–66). Conclusion: Pedicled transfer of the lateral border of scapula with circumflex scapular artery is an excellent choice for treating recalcitrant humeral nonunions. Inability to actively supinate the forearm makes common activities of daily living and certain vocational activities awkward or impossible to perform. A total of 11 patients with deficient supination of the arm underwent transfer of the tendon of flexor carpi ulnaris to the split tendon of brachioradialis with its bony insertion into the radial styloid left intact. Active supination beyond neutral rotation was a mean of 37.2° (25° to 49.5°) at a minimum follow-up of three years, representing a significant improvement (95% confidence interval 25 to 50, p < 0.001). Functional evaluation of the hand after this transfer showed excellent and good results in ten patients and fair in one.

The split tendon of brachioradialis as an insertion for transfer of the flexor carpi ulnaris appears to provide adequate supination of the forearm without altering the available pronation and avoids the domination of wrist extension sometimes associated with transfers of the flexor carpi ulnaris to the radial extensors of the wrist.

The classification of complex fractures of the proximal humerus has long been an area of dispute reflecting an inability to agree on the anatomy of these injuries based on conventional X-rays alone. We demonstrated here that 3-dimensional CT reconstructions, when viewed in a systematic fashion, can yield superior understanding and an enhanced concurrence among observers as to the nature of these fractures. This has lead to a modification of the Neer classification diagram of proximal humeral fractures to reflect their true 3-dimensional anatomy. A 3-dimensional understanding is crucial in and of itself during any process of surgical reconstruction, but a 3-dimensional classification is additionally useful insofar as it informs other aspects of clinical decision making. For example, in a particular category of injury what if any surgery is indicated? In this regard one must first know the natural history of the specific fracture type without the benefit of operative intervention. Towards an answer to this basic question we have categorized non-operated proximal humeral fracture patients according to the new 3-dimensional classification and have followed their clinical progress. We present here the Natural History in unoperated patients with the types of Complex injuries who historically have been the ones commonly recommended to surgery. Results: Over an 8 year period, 63 Complex Fractures treated non-operatively were evaluated with standardized indices. We conclude that overall motion, function and pain status of Complex Fractures of proximal humerus treated conservatively is similar to that of a successful surgical Shoulder Fusion. Motion is considerably compromised but pain is minimal and functional status is acceptable to most patients. Contrary to common belief avascular necrosis is rare even in severely displaced injuries. Additional new observations concerning Valgus/Varus, Head Split, and rotational injuries will also be presented. Future studies based on this 3-dimensional classification system need to be done to compare these natural history results with various types of surgical interventions

Scapular instability is a disabling deformity that results in pain and influences the overall upper limb function ; for which scapular stabilization may be necessary. Aim: To review the results of this procedure. Methods: 9 patients who underwent this procedure could be contacted and were assessed. We used the Disability of Arm, shoulder and Hand score(DASH) and the constant score for shoulder function including subjective assessment of patients pain and overall satisfaction and radiographs for evidence of unionwith a mean age of 36 yrs(range 21–57 yrs), performed in the period between July 1996 and October 2002 with a mean follow up period of 35.7 months,(range 10–72 months).6 of them were primary procedures and two were revisions for failed primary stabilisation . The main pre-operative complaint of these patients was dragging pain, scapular winging, painful forward flexion and abduction and sense of instability. The underlying pathology was Fascio-scapulo-humeral dystrophy in 3, sprengels shoulder in 2,brachial plexus palsy in 1, following trapezius muscle excision in 1, residual winging following shoulder fusion in 1. The technique used for fusion was plate and wires in 6, Rush pin and wires in 2. All of them had bone grafting . At the last follow up the mean DASH score was 37 and the mean constant score was 74.67. All patients had mild or no pain,felt their scapula stable and were satisfied with the outcome. The main post operative complaint was wire breakage and migration that necessitated removal in 3 patients. We conclude that scapulothoracic fusion is a valuable procedure and can restore reasonable shoulder function and relieve pain in patients with scapular instability

Between March 1994 and June 2003, 80 patients with brachial plexus palsy underwent a trapezius transfer. There were 11 women and 69 men with a mean age of 31 years (18 to 69). Before operation a full evaluation of muscle function in the affected arm was carried out. A completely flail arm was found in 37 patients (46%). Some peripheral function in the elbow and hand was seen in 43 (54%). No patient had full active movement of the elbow in combination with adequate function of the hand. Patients were followed up for a mean of 2.4 years (0.8 to 8). We performed the operations according to Saha’s technique, with a modification in the last 22 cases. We demonstrated a difference in the results according to the pre-operative status of the muscles and the operative technique.

The transfer resulted in an increase of function in all patients and in 74 (95%) a decrease in multidirectional instability of the shoulder. The mean increase in active abduction was from 6° (0 to 45) to 34° (5 to 90) at the last review. The mean forward flexion increased from 12° (0 to 85) to 30° (5 to 90).

Abduction (41°) and especially forward flexion (43°) were greater when some residual function of the pectoralis major remained (n = 32). The best results were achieved in those patients with most pre-operative power of the biceps, coracobrachialis and triceps muscles (n = 7), with a mean of 42° of abduction and 56° of forward flexion. Active abduction (28°) and forward flexion (19°) were much less in completely flail shoulders (n = 34).

Comparison of the 19 patients with the Saha technique and the 15 with the modified procedure, all with complete paralysis, showed the latter operation to be superior in improving shoulder stability. In all cases a decrease in instability was achieved and inferior subluxation was abolished.

The results after trapezius transfer depend on the pre-operative pattern of paralysis and the operative technique. Better results can be achieved in patients who have some function of the biceps, coracobrachialis, pectoralis major and triceps muscles compared with those who have a complete palsy. A simple modification of the operation ensures a decrease in joint instability and an increase in function.

We have compared the functional outcome after glenohumeral fusion for the sequelae of trauma to the brachial plexus between two groups of adult patients reviewed after a mean interval of 70 months. Group A (11 patients) had upper palsy with a functional hand and group B (16 patients) total palsy with a flail hand. All 27 patients had recovered active elbow flexion against resistance before shoulder fusion. Both groups showed increased functional capabilities after glenohumeral arthrodesis and a flail hand did not influence the post-operative active range of movement. The strength of pectoralis major is a significant prognostic factor in terms of ultimate excursion of the hand and of shoulder strength. Glenohumeral arthrodesis improves function in patients who have recovered active elbow flexion after brachial plexus palsy even when the hand remains paralysed

Introduction: Arthrodesis of the shoulder joint is appropriate for several conditions, including paralysis, degenerative disease, infection, and salvage of failed arthroplasty. Two common complications of shoulder fusion, non-union and unacceptable arm position, may reflect failure to achieve rigid fixation during the surgical procedure. Numerous fixation techniques have been described, including plate fixation, external fixation, and screw fixation. Aim: To compare the biomechanics of five fixation techniques of shoulder fusion in a human cadaveric model. Methods: Twenty-five shoulder fusions were carried out in fresh-frozen human cadaveric specimens with the following five techniques: screw fixation alone (n=5), external fixation alone (n=5), external fixation supplemented with screw fixation (n=5), single plate fixation (n=5), and double plate fixation (n=5). Each specimen was tested on a servo-hydraulic machine under repeated physiologic loads to determine the bending and torsional stiffness. Results: There was a statistically significant difference in bending and torsional stiffness between all five fixation techniques (ANOVA, p< 0.05). Normalised bending (B) and torsional (T) stiffness, in descending order, were: double plate (B=1.0, T=1.0), single plate (B=0.77, T=0.89), external-fixation with screws (B=0.68, T=0.74), external-fixation alone (B=0.40, T=0.53), and screws alone (B=0.13, T=0.26). Discussion & Conclusion: Statistically significant differences in bending and torsional stiffness have been identified using five different techniques of shoulder fusion. The risk of the most common complications of this surgical procedure, non-union and unacceptable arm position, may be minimised if these biomechanical findings are applied to surgical decision-making

A new technique of shoulder fusion is presented using a posterior approach. After removal of the articular cartilage, a Rush pin is introduced from the spine of the scapula, through the glenoid into the medullary canal of the humerus. This is supplemented by tension-band wiring from the acromion to the neck of the humerus and a muscle pedicle graft attached to the acromion. A shoulder spica is applied for four to six weeks. Four patients with injuries to the upper brachial plexus and 14 with paralysis of the upper arm due to anterior poliomyelitis have been followed for three years. One of the 18 patients developed nonunion; she had removed her own cast prematurely. This method of fixation provides high shear resistance and low axial stiffness without deforming plastically. It does not affect bone growth in young patients, is effective in patients with osteoporosis, and gives a high rate of union
