Aims

A functional anterior cruciate ligament (ACL) or posterior cruciate ligament (PCL) has been assumed to be required for patients undergoing unicompartmental knee arthroplasty (UKA). However, this assumption has not been thoroughly tested. Therefore, this study aimed to assess the biomechanical effects exerted by cruciate ligament-deficient knees with medial UKAs regarding different posterior tibial slopes.

Methods

ACL- or PCL-deficient models with posterior tibial slopes of 1°, 3°, 5°, 7°, and 9° were developed and compared to intact models. The kinematics and contact stresses on the tibiofemoral joint were evaluated under gait cycle loading conditions.


Aims. To compare time dependent functional improvement for patients with medial, respectively lateral knee osteoarthritis (OA) after treatment with opening wedge osteotomy relieving the pressure on the osteoarthritic part of the knee. Methods. In all, 49 patients (52 knees) with a mean age of 47 years (31 to 64) underwent high tibial osteotomies (HTO), and 24 patients with a mean age of 48 years (31 to 62) low femoral osteotomies (LFO) with opening wedge technique due to medial, respectively lateral knee OA with malalignment. All osteotomies were stabilized with a Puddu plate and bone grafting performed in the same time period (2000 to 2008). The patients were evaluated by the Knee Injury and Osteoarthritis Outcome Score (KOOS) pre-operatively and at six months, and at one, two, five, and ten years postoperatively. The knee OA was graded according to the Ahlbäck and Kellgren-Lawrence radiological scoring systems. Results. The mean angular corrections were 8.0° (4° to 12°) for the HTO and 9.6° (4° to 20°) for the LFO. Both the pre-operative KOOS and the osteoarthritic gradings were similar for the two patient groups. The five subscores of KOOS increased significantly during the postoperative period (p < 0.001 to 0.029) levelling out after one year in both groups. The KOOS subscore symptoms was significantly higher for patients with HTO than those with LFO at all follow-up times, for sport and recreation in the period one to five years, and for pain and quality of life at two to five years (p < 0.001 to 0.009). Eight HTOs (15%) and five LFOs (21%) were converted to total knee arthroplasty after mean 6.7 years (2.0 to 9.8) and 5.4 years (4.0 to 8.0) respectively. The ten-year osteotomy survival rates were 88% for the HTO and 79% for the LFO (p = 0.745). Conclusion. Patients with unicompartmental knee OA improved after a corrective opening wedge osteotomy, but four of the five subscores of KOOS were significantly higher for those with medial than those with lateral OA in most of the ten-year follow-up period. Medial knee OA effects approximately 4.1 million people in England. Non-surgical strategies to lower knee joint loading is commonly researched in the knee OA literature as a method to alleviate pain and discomfort. Medial knee OA is much more prevalent than lateral knee OA due to the weight bearing line passing medial to the knee causing an external knee adduction moment (KAM). Numerous potential gait retraining strategies have been proposed to reduce either the first and/or the second peak KAM, including: toe-in gait, toe-out gait, lateral trunk lean and medial thrust gait. Gait retraining has been researched with little regard to the biomechanical consequences at the hip and ankle joints. This systematic review aimed to establish whether gait retraining can reduce medial knee loading as assessed by first and second peak KAMs, establish what are the biomechanical effects a reduced KAM has on other lower limb joint biomechanics and outline patient/participant reported outcomes on how easy the gait retraining style was to implement. The protocol for this systematic review was registered with PROSPERO on the 23rd January 2018 (registration ID: CRD42018085738). 13 databases were searched by one author (J.B.B). Additionally, PROSPERO was searched for ongoing or recently completed systematic reviews. Risk of bias was assessed using the Downs and Black quality index. Search: Group one consisted of keywords “walk” OR “gait”. Keywords “knee” OR “adduction moment” built up the second group. Group three consisted “osteoarthriti” OR “arthriti” OR “osteo arthriti”, OR “OA”. Group four included “hip” OR “ankle”. the searched results of each group were combined with conjunction “AND” in all fields. Out of the eight different gait retraining strategies identified, trunk lean reduced first peak KAM the most, which was evaluated in 3 studies, reducing first peak KAM by 20%-65%. There was a lack of collective pelvic, hip and/or ankle joint biomechanical variables reported across all 11 studies. Of eight gait retraining styles identified, the strategy that reduced first peak KAM the most was an increased lateral trunk lean, which was evaluated in 3 different studies. This is the first systematic review that has highlighted that there is limited evidence of the biomechanical consequences of a reduced knee joint load has on the pelvic, hip and/or ankle joints when undertaking gait retraining protocols. Future studies assessing gait retraining strategies should provide biomechanical outputs for other lower limb joints other than the knee joint, as well as providing participant perceptions on the level of difficulty the gait style is to perform

Introduction. Isolated lateral compartment osteoarthritis (OA) occurs in 5–10% of knees with OA [1, 2]. Lateral unicompartmental knee arthroplasty (LUKA) emerged as a treatment to this disease in the early 80s but challenging surgical technique has limited the prevalence of this treatment option [1–3]. A robotic-arm assisted surgical technique (MAKO Surgical Corp.) has emerged as a way to achieve precise implant positioning which can potentially improve surgical outcomes. Objectives. The purpose of this study was to evaluate short term outcomes for patients that received LUKA using a novel robotic-arm assisted surgical technique. Methods. Thirty-seven (37) patients (12 male, 25 female - mean age 63.7 years) with lateral OA received a robotic-arm assisted LUKA between July 2011 and September 2013 from 3 surgeons. All patients were evaluated by an independent surgeon not involved in the treatment of these patients at an average follow-up of 15.9 months (8–27). Range of motion and limb alignment was compared pre- and post-operatively. Results. Lateral UKA using robotic-arm assistance improved the post-operative range of motion an average of 4.8 ± 7.1º (p<0.0001) from a starting value of 136.5 ± 8.6º to a post-operative value of 141.6 ± 8.0º. In addition, patients began with a pre-operative deformity of 3.1 ± 3.2º of valgus and resulted in a post-operative alignment of 0.8 ± 1.9º of valgus corresponding to an average correction of 2.4 ± 2.3º less valgus (p<0.000001). The average operative time was 44.0 ± 10.8 minutes with 97% of the cases completed within 60 minutes. Conclusion. These results suggest that LUKA with robotic-arm assistance provides excellent post-operative alignment and demonstrate a reliable option for management of isolated lateral knee OA

The lateral compartment is predominantly affected in approximately 10% of patients with osteoarthritis of the knee. The anatomy, kinematics and loading during movement differ considerably between medial and lateral compartments of the knee. This in the main explains the relative protection of the lateral compartment compared with the medial compartment in the development of osteoarthritis. The aetiology of lateral compartment osteoarthritis can be idiopathic, usually affecting the femur, or secondary to trauma commonly affecting the tibia. Surgical management of lateral compartment osteoarthritis can include osteotomy, unicompartmental knee replacement and total knee replacement. This review discusses the biomechanics, pathogenesis and development of lateral compartment osteoarthritis and its management.

The Oxford mobile-bearing unicompartmental knee replacement (UKR) is an effective and safe treatment for osteoarthritis of the medial compartment. The results in the lateral compartment have been disappointing due to a high early rate of dislocation of the bearing. A series using a newly designed domed tibial component is reported.

The first 50 consecutive domed lateral Oxford UKRs in 50 patients with a mean follow-up of three years (2.0 to 4.3) were included. Clinical scores were obtained prospectively and Kaplan-Meier survival analysis was performed for different endpoints. Radiological variables related to the position and alignment of the components were measured.

One patient died and none was lost to follow-up. The cumulative incidence of dislocation was 6.2% (95% confidence interval (CI) 2.0 to 17.9) at three years. Survival using revision for any reason and aseptic revision was 94% (95% CI 82 to 98) and 96% (95% CI 85 to 99) at three years, respectively. Outcome scores, visual analogue scale for pain and maximum knee flexion showed a significant improvement (p < 0.001). The mean Oxford knee score was 43 (sd 5.3), the mean Objective American Knee Society score was 91 (sd 13.9) and the mean Functional American Knee Society score was 90 (sd 17.5). The mean maximum flexion was 127° (90° to 145°). Significant elevation of the lateral joint line as measured by the proximal tibial varus angle (p = 0.04) was evident in the dislocation group when compared with the non-dislocation group.

Clinical results are excellent and short-term survival has improved when compared with earlier series. The risk of dislocation remains higher using a mobile-bearing UKR in the lateral compartment when compared with the medial compartment. Patients should be informed about this complication. To avoid dislocations, care must be taken not to elevate the lateral joint line.

Recently in Europe, Unicompartmental Knee Arthroplasty (UKA) has regained interest in the orthopedic community; however, based on various reports, results concerning UKA for isolated lateral compartment arthritis seemed to be not as good as for medial side. In 1988 our department started using Unicondylar Knee Pros-thesis with a fixed all polyethylene bearing tibial component and resurfacing of the distal femoral condyle. The aim of this study is to report on our personal experience using this type of implant for lateral osteoarthritis with a long follow-up period. Between January 1988 and October 2003, we performed 54 lateral UKAs (52 patients) and all were implanted for lateral osteoarthritis (3 cases of which were posttraumatic). 52 knees in 50 patients were available after a minimum duration of follow-up of five years (96.3 %). The mean age of the patients at the time of the index procedure was 72.2±1.5 years. The mean duration of follow-up was 100.9 months (range 64 – 189 months). At follow up, 4 underwent a second surgery: one conversion to TKA for tibial tray loosening at 2 years and 3 revisions for UKA in the medial compartment. No revision surgery was necessary for wear of either of the two components, nor for infection. The mean IKS knee score was 94.9 points, with mean range of motion 132.6° (range, 115–150) and a mean IKS function score totaling 81.8 points. The average femorotibial alignment was 1.8° (range −6° to 12°). Radiolucent lines in relation to the tibial component were appreciated in 6 knees and to the femoral component in 1 knee. Implant survival was 98.08% at ten years. The UKA with a fixed bearing tibial component and a femoral resurfacing implant is a reliable option for management of isolated lateral knee osteoarthritis. It offers excellent medium-term results for both functional level and implant survival which even currently enable us to widen our selection criteria to include younger patients or those associated with starting patellofemoral osteoarthritis

Purpose: The purpose of this study was to determine what differences exist in the knee flexion, rotation and adduction moments and periarticular knee muscle activation patterns between subjects with medial compartment knee osteoarthritis (OA) and those with lateral compartment knee OA. Method: Forty eight individuals with knee OA were studied. The group was divided into those with predominantly medial compartment involvement (38 subjects, age 63 ± 8 years) and those with lateral involvement (10 subjects, age 63 ± 9 years). Three-dimensional motion (Optotrak) and ground reaction force (AMTI) data were collected while the subjects walked at a self-selected velocity. The knee flexion, rotation and adduction moments, time normalized to the percentage of one gait cycle, were calculated using an inverse dynamics approach. Electromyograms (EMG) were also collected from the rectus femoris, vastus lateralis, vastus medialis, medial and lateral hamstrings, and medial and lateral gastrocnemius and normalized to maximum voluntary isometric contractions. Knee moments and waveforms for each muscle for one complete gait cycle were analyzed for group differences using principal component analysis (PCA) followed by Student’s t-tests (alpha-adj = 0.017) for the PCA scores. Results: The two groups were statistical similar in terms of age, height, weight, and walking velocity (p> 0.05). PCA analysis revealed statistically significant differences (p< 0.017) in patterns for the knee adduction moment, medial gastrocnemius, and lateral hamstrings between the two groups. Conclusion: As expected, there was a difference in the knee adduction moment between the two groups. What is novel is that the muscle activation patterns from the lateral site group are consistent with an attempt to unload that compartment. The results of this study provide evidence that biomechanical and neuromuscular differences do exist, depending on the OA site. This could have implications for developing site-specific conservative management approaches
