Background

The Comprehensive Care for Joint Replacement (CJR) model was implemented in April-2016 to standardize cost and improve quality of care for two of the most commonly billed inpatient procedures for Medicare patients, total knee and total hip arthroplasty. The purpose of this study is to compare one institution's predicted savings and losses under the CJR model with actual savings and losses after two years of implementation and discuss new methods to maintain savings.

Methods

Using our institution's data, we calculated the mean cost per episode of care. We calculated the percent reduction in target price and percent savings or losses per case for the CJR and Bundle Payment Care Initiative (BPCI) for each Medicare Severity Diagnosis Related Group (MS-DRG) using mean cost per episode and CJR and BPCI target prices. We compared the target prices, annual savings, and losses per episode of care for both CJR and BPCI. All CJR savings, projected and actual, were computed by comparing CJR savings to that of 2018 BPCI savings.

Introduction

The use of intraoperative liposomal bupivacaine (LB) peri-articular injection has been highly debated for total knee arthroplasty (TKA). We evaluated the effect of an institutional-wide discontinuation of intraoperative LB on immediate postoperative pain scores, opioid consumption, and objective functional outcomes.

Material and Methods

Between July 1, 2019 and November 30, 2019, an institutional policy discontinued use of intraoperative LB, while the volume of non-LB with epinephrine was increased from 40-ml to 60-ml. A historical cohort was derived from patients undergoing TKA between January 1, 2019 and June 30, 2019. All patients received the same opioid sparing protocol, minimizing variability in prescribing habits. No adductor canal blocks/pumps were utilized.

Nursing documented verbal rating scale (VRS) pain scores were collected from our electronic data warehouse and averaged per patient per 12-hour interval. Opiate administration events were derived as Morphine Milligram Equivalences (MMEs) per patient per 24-hour interval. To assess immediate postoperative functional status, the validated Activity Measure for Post-Acute Care (AM-PAC) tool was utilized. All time events were calculated relative to TKA completion instant.

Summary

Narcotic administration within the inpatient setting is highly variable any may benefit from the implementation of standardized multi-modal pain management protocols.

Introduction

Total joint arthroplasty (TJA) candidates have historically received high doses of opioids within the perioperative period for the management of surgical pain. Healthcare systems have responded by improving opioid prescribing documentation and implementing narcotic-sparing pain protocols into TJA integrate care pathways (ICP). Despite these efforts, there are few technological platforms designed to curtail excessive inpatient narcotic administration. Here we present an early iteration of an inpatient narcotic administration reporting tool which normalizes patient narcotic consumption as an average daily morphine milligram equivalence (MME) per surgical encounter (MME/day/encounter) among total hip arthroplasty (THA) recipients. This information may help orthopaedic surgeons visualize their individual granular inpatient narcotic prescribing habits individually and compared to other surgeons, while taking into consideration patient and procedure specific variables.

Introduction

Despite the demonstrated success of total knee arthroplasty (TKA) and higher rates of arthritis and disability from arthritis among minorities, these populations utilize TKA at lower rates than non-Hispanic white patients in the United States. In addition, women present for TKA with more advanced disease prior to TKA than their male counterparts. In order to explore potential reasons for this, we examined the relationship between race, gender, and pre-operative expectations on postoperative outcomes among patients undergoing TKA.

Methods

134 patients scheduled for primary unilateral TKA secondary to moderate to severe osteoarthritis were enrolled in this two-institution prospective study. All patient demographics were collected and recorded in an electronic data registry. In addition, patient reported outcome (PRO) questionnaires, including the Knee Injury and Osteoarthritis Outcome Score (KOOS), HSS Patient Expectation Questionnaire, Social Support Survey, Pain Catastrophizing Scale, Visual Analogue Pain Scale (VAS), Geriatric Depression Scale and European Quality (EQ) of Life 5-Dimensions with EQ VAS were prospectively collected at 4 time points (baseline, 4–8 weeks, 9–14 months, 20–26 months postoperative). Statistical analysis was then conducted among patients of different gender and ethnic background to assess for the effect of these demographic variables on patient expectations and outcomes.

We retrospectively reviewed 161 revision THAs with diaphyseal fitting, mid- modular femoral components performed by ten surgeons at two academic medical centers. The average follow-up was 6.1 years. At final follow-up, 4 patients required re-revision for failure of the femoral component; 3 (2%) for aseptic loosening and 1 for mechanical failure of stem in setting of periprosthetic fracture. There were a total of 24 (14.9%) revisions for any reason, with the most common reason being septic failure (10 of 24). To our knowledge, this is the largest reported series of mid-term survivorship and complications of revision THA with mid-modular femoral components. Our results show that these stems have a low rate of aseptic loosening, subsidence, and mechanical failure.

INTRODUCTION

Total Knee Arthroplasty (TKA) is a durable procedure which allows most patients to achieve a satisfactory functional level, but there can be instability under stressful conditions. Instability is one cause of early revision, often due to misalignment or inadequate ligament balancing. Persistent instability may cause elevated polyethylene wear. Lower levels of instability may cause patient discomfort with certain stressful activities. Hence quantifying instability may have an important role in the functional evaluation of TKA. Several previous studies showed that accelerometers have advantages in kinematic studies including low cost, ease of application, and application to any activity. The aim of this study was to demonstrate the use of an accelerometer attached to the anterior of the tibia, as an evaluation of knee stability of TKA patients. It was postulated that accelerations between TKAs and normal controls will be different, which could indicate abnormal TKA kinematics involving instability, especially for high intensity activities.

METHODS & MATERIALS

We tested 38 TKA knees in 27 patients, in the age range of 50–80 years, with a minimum follow up of 6 months; and 25 knees in 16 shoulder patients, who had no known knee pathology as age-matched controls. A tri-axial accelerometer was firmly attached to the anterior proximal tibia to measure 3-axis accelerations with a sample rate of 100 Hz. Four activities were tested;

Starting with the test leg, walk 3 steps then come to a sudden stop

Take one step forward with the non-tested leg and make a tight 90∗∗∗∗∗ turn towards the non- tested knee direction

Sit down for 3–4 seconds then stand back up

Step up on a 7″ inches high box with the test leg, followed by the non-test leg. Then step down from the box with the test leg, followed by the non-test leg.

During the activities, the patients responded to a questionnaire on instability and pain for each activity. For each test at the time of foot impact, there was a high/low peak acceleration, the peak-to-valley being taken as the indicator. The mean total magnitude of the acceleration was compared between the TKA and control groups in the anterior-posterior direction using the Student's t-test. Statistical significance was at p-value < 0.05.

Background: The rates of primary and revision knee arthroplasty in the United States have been increasing. Simultaneously, several studies have reported increased complication rates when these procedures are performed at low-volume centers. One innovation designed to improve knee arthroplasty outcomes is computer navigation, which aims to reduce revision rates by improving the alignment achieved at surgery. The purpose of this study was to examine the impact of hospital volume on the costeffectiveness of this new technology in order to determine its feasibility and the level of evidence that should be sought prior to its adoption.

Methods: A Markov decision model was used to evaluate the cost-effectiveness of computer-assisted knee arthroplasty, in relation to hospital volume. Transition probabilities were estimated from the arthroplasty literature, and costs were based on the average reimbursement for primary and revision knee arthroplasty at out institution. Outcomes were measured in quality adjusted life years.

Results: The results demonstrate that computer-assisted surgery becomes less cost-effective as the annual hospital volume decreases, as the cost of navigation increases, and as the impact on revision rates decreases. If a center performs 250 cases per year, computer navigation will be cost-effective if the annual revision rate is reduced by 2% per year over a twenty-year period. If a center performs 150 cases per year, computer navigation is cost-effective if it results in a 2.5% reduction in the annual revision rate over a twenty-year period. If a center performs only 25 cases per year, the annual reduction in revision rates must be 13% for computer navigation to be cost-effective.

Conclusion: This analysis demonstrates that computer navigation is not likely to be a cost-effective investment in health care improvement in low volume joint replacement centers, where its benefit is most likely to be realized. However, it may be a cost-effective technology for higher volume joint replacement centers, where the decrease in the rate of knee revision needed to make the investment cost-effective is modest, if improvements in revisions rates with the use of this technology can be realized. This illustrates that hospital volume can have a substantial impact on the cost-effectiveness of new technology in surgery, and this should be carefully considered by any center considering such a large capital investment.
