This study aimed to evaluate the effectiveness of a novel intraoperative navigation platform for total knee arthroplasty (TKA) in restoring native knee joint kinematics and strains in the medial collateral ligament (MCL) and lateral collateral ligament (LCL) during squatting motions. Six cadaver lower limbs underwent computed tomography scans to design patient-specific guides. Using these scans, bony landmarks and virtual single-line collateral ligaments were identified to provide intraoperative real-time feedback, aided in bone resection, implant alignment, tibiofemoral kinematics, and collateral ligament elongations, using the navigation platform. Assessment of the humeral head translation with respect to the glenoid joint, termed humeral head migration (HHM), is crucial in total shoulder arthroplasty pre-operative planning. Its assessment informs current classification systems for shoulder osteoarthritis as well as the evaluation of surgical correction. In current clinical practice, HHM assessment relies on computed-tomography (CT) imaging. However, the associated supine position might undermine its functional relevance as it does not reflect the weight-bearing condition with active muscle engagement associated with the upright standing position of most daily activities. Method
Mechanical alignment (MA) in total knee arthroplasty (TKA), although considered the gold standard, reportedly has up to 25% of patients expressing post-operative dissatisfaction. Biomechanical outcomes following kinematic alignment (KA) in TKA, developed to restore native joint alignment, remain unclear. Without a clear consensus for the optimal alignment strategy during TKA, the purpose of this study was to conduct a paired biomechanical comparison of MA and KA in TKA by experimentally quantifying joint laxity and medial collateral ligament (MCL) strain. 14 bilateral native fresh-frozen cadaveric lower limbs underwent medially-stabilised TKA (GMK Sphere, Medacta, Switzerland) using computed CT-based subject-specific guides, with KA and MA performed on left and right legs, respectively. Each specimen was subjected to sensor-controlled mediolateral laxity tests. A handheld force sensor (Mark-10, USA) was used to generate an abduction-adduction moment of 10Nm at the knee at fixed flexion angles (0°, 30°, 60°, 90°). A digital image correlation system was used to compute the strain on the superficial medial collateral ligament. A six-camera optical motion capture system (Vicon MX+, UK) was used to acquire kinematics using a pre-defined CT-based anatomical coordinate system. A linear mixed model and Tukey's posthoc test were performed to compare native, KA and MA conditions (p<0.05). Unlike MA, medial joint laxity in KA was similar to the native condition; however, no significant difference was found at any flexion angle (p>0.08). Likewise, KA was comparable with the native condition for lateral joint laxity, except at 30°, and no statistical difference was observed. Although joint laxity in MA seemed lower than the native condition, this difference was significant only for 30° flexion (p=0.01). Both KA and MA exhibited smaller MCL strain at 0° and 30°; however, all conditions were similar at 60° and 90°. Medial and lateral joint laxity seemed to have been restored better following KA than MA; however, KA did not outperform MA in MCL strain, especially after mid-flexion. Although this study provides only preliminary indications regarding the optimal alignment strategy to restore native kinematics following TKA, further research in postoperative joint biomechanics for load bearing conditions is warranted.
This study aims to create a novel computational workflow for frontal plane laxity evaluation which combines a rigid body knee joint model with a non-linear implicit finite-element model wherein collateral ligaments are anisotropically modelled using subject-specific, experimentally calibrated Holzpfel-Gasser-Ogden (HGO) models. The framework was developed based on CT and MRI data of three cadaveric post-TKA knees. Bones were segmented from CT-scans and modelled as rigid bodies in a multibody dynamics simulation software (MSC Adams/view, MSC Software, USA). Medial collateral and lateral collateral ligaments were segmented based on MRI-scans and are modelled as finite elements using the HGO model in Abaqus (Simulia, USA). All specimens were submitted varus/valgus loading (0-10Nm) while being rigidly fixed on a testing bench to prevent knee flexion. In subsequent computer simulations of the experimental testing, rigid bodies kinematics and the associated soft-tissue force response were computed at each time step. Ligament properties were optimised using a gradient descent approach by minimising the error between the experimental and simulation-based kinematic response to the applied varus/valgus loads. For comparison, a second model was defined wherein collateral ligaments were modelled as nonlinear no-compression spring elements using the Blankevoort formulation. Models with subject-specific, experimentally calibrated HGO representations of the collateral ligaments demonstrated smaller root mean square errors in terms of kinematics (0.7900° +/− 0.4081°) than models integrating a Blankevoort representation (1.4704° +/− 0.8007°). A novel computational workflow integrating subject-specific, experimentally calibrated HGO predicted post-TKA frontal-plane knee joint laxity with clinically applicable accuracy. Generally, errors in terms of tibial rotation were higher and might be further reduced by increasing the interaction nodes between the rigid body model and the finite element software. Future work should investigate the accuracy of resulting models for simulating unseen activities of daily living.
During shoulder arthroplasty the native functionality of the diseased shoulder joint is restored, this functionality is strongly dependent upon the native anatomy of the pre-diseased shoulder joint. Therefore, surgeons often use the healthy contralateral scapula to plan the surgery, however in bilateral diseases such as osteoarthritis this is not always feasible. Virtual reconstructions are then used to reconstruct the pre-diseased anatomy and plan surgery or subject-specific implants. In this project, we develop and validate a statistical shape modeling method to reconstruct the pre-diseased anatomy of eroded scapulae with the aim to investigate the existence of predisposing anatomy for certain shoulder conditions. The training dataset for the statistical shape model consisted of 110 CT images from patients without observable scapulae pathologies as judged by an experienced shoulder surgeon. 3D scapulae models were constructed from the segmented images. An open-source non-rigid B-spline-based registration algorithm was used to obtain point-to-point correspondences between the models. The statistical shape model was then constructed from the dataset using principle component analysis. The cross-validation was performed similarly to the procedure described by Plessers et al. Virtual defects were created on each of the training set models, which closely resemble the morphology of glenoid defects according to the Wallace classification method. The statistical shape model was reconstructed using the leave-one-out method, so the corresponding training set model is no longer incorporated in the shape model. Scapula reconstruction was performed using a Monte Carlo Markov chain algorithm, random walk proposals included both shape and pose parameters, the closest fitting proposal was selected for the virtual reconstruction. Automatic 3D measurements were performed on both the training and reconstructed 3D models, including glenoid version, critical shoulder angle, glenoid offset and glenoid center position. The root-mean-square error between the measurements of the training data and reconstructed models was calculated for the different severities of glenoid defects. For the least severe defect, the mean error on the inclination, version and critical shoulder angle (°) was 2.22 (± 1.60 SD), 2.59 (± 1.86 SD) and 1.92 (± 1.44 SD) respectively. The reconstructed models predicted the native glenoid offset and centre position (mm) an accuracy of 0.87 (± 0.96 SD) and 0.88 (± 0.57 SD) respectively. The overall reconstruction error was 0.71 mm for the reconstructed part. For larger defects each error measurement increased significantly. A virtual reconstruction methodology was developed which can predict glenoid parameters with high accuracy. This tool will be used in the planning of shoulder surgeries and investigation of predisposing scapular morphologies.
The anterolateral ligament (ALL) has been recently recognized as a distinct stabilizer for internal rotation in the ACL-deficient knee and it has been hypothesized that ALL reconstruction may play an important role in improving anterolateral instability following ACL reconstruction. Both the gracilis tendon (GT) and a portion of the iliotibial band (ITB) have been suggested as graft materials for ALL reconstruction, however, there is an ongoing debate concerning whether GT or ITB are appropriate grafting materials. Furthermore, there is limited knowledge in how the mechanical properties of these potential grafts compare to the native ALL. Consequently, the aim of this study was to characterize the elastic (Young's modulus and failure load) and viscoelastic (dynamic and static creep) mechanical properties of the ALL and compare these results with the characteristics of the grafting materials (GT and ITB), in order to provide guidance to clinicians with respect to graft material choice. Fourteen fresh-frozen cadaveric knees (85.2±12.2 yr) were obtained. The ALL, ITB, and the distal (GTD) and proximal gracilis tendons (GTP) (bisected at mid portion) were harvested from each donor and tested with a dynamic material testing frame. Prior to testing, the cross-sectional area of each tissue was measured using a casting method and the force required to achieve a min-max stress (1.2–12 MPa) for the testing protocol was calculated (preconditioning (20 cycles, 3–6 MPa), sinusoidal cycle (200 cycles, 1.2–12 MPa), dwell at constant load (100 s, 12 MPa), and load to failure (3%/s)). Kruskall-Wallis tests were used to compare all tissue groups (p<0.05). The Young's modulus of both ALL (181.3±63.9 MPa) and ITB (357.6±94.4 MPa) are significantly lower than GTD (835.4±146.5 MPa) and GTP (725.6±227.1 MPa). In contrast, the failure load of ALL (124.5±40.9 N) was comparable with GTD (452.7±119.3 N) and GTP (433±133.7 N), however, significantly lower than ITB (909.6±194.7 N). Dynamic creep of the ALL (0.5±0.3 mm) and ITB (0.7±0.2 mm) were similar (p>0.05) whereas the GTD (0.26±0.06 mm) and GTP (0.28±0.1 mm) were significantly lower. Static creep progression of the ALL (1.09±0.4 %) was highest across all tissues, while GTD (0.24±0.05 %) and GTP (0.25±0.0.04 %) were lowest and comparable with ITB (0.3±0.07 %) creep progression. Since grafts from the ITB, GTD and GTP were comparable to the ALL only for certain mechanical properties, there was no clear preference for using one over another for ALL reconstruction. Therefore, further studies should be performed in order to evaluate which parameters play a vital role to determine the optimum grafting choice.
Poor soft tissue balance in total knee arthroplasty (TKA) is one of the most primary causes of dissatisfaction and reduced joint longevity, which are associated with postoperative instability and early implant failure1. Therefore, surgical techniques, including mechanical instruments and 3-D guided navigation systems, in TKA aim to achieve optimum soft tissue balancing in the knee to improve postoperative outcome2. Patella-in-Place balancing (PIPB) is a novel technique which aims to restore native collateral ligament behaviour by preserving the original state without any release. Moreover, reduction of the joint laxity compensates for the loss of the visco-elastic properties of the cartilage and meniscus. Following its clinical success, we aimed to evaluate the impact of the PIPB technique on collateral ligament strain and laxity behaviour, with the hypothesis that PIPB would restore strains in the collateral ligaments3. Eight fresh-frozen cadaveric legs were obtained (KU Leuven, Belgium, H019 2015-11-04) and CT images were acquired while rigid marker frames were affixed into the femur, and tibia for testing. After carefully removing the soft tissues around the knee joint, while preserving the joint capsule, ligaments, and tendons, digital extensometers (MTS, Minnesota, USA) were attached along the length of the superficial medial collateral ligament (MCL) and lateral collateral ligament (LCL). A handheld digital dynamometer (Mark-10, Copiague, USA) was used to apply an abduction or adduction moment of 10 Nm at fixed knee flexion angles of 0°, 30°, 60° and 90°. A motion capture system (Vicon Motion Systems, UK) was used to record the trajectories of the rigid marker frames while synchronized strain data was collected for MCL/LCL. All motion protocols were applied following TKA was performed using PIPB with a cruciate retaining implant (Stryker Triathlon, MI, USA). Furthermore, tibiofemoral kinematics were calculated4 and combined with the strain data. Postoperative tibial varus/valgus stresses and collateral ligament strains were compared to the native condition using the Wilcoxon Signed-Rank Test (p<0.05). Postoperative tibial valgus laxity was lower than the native condition for all flexion angles. Moreover, tibial valgus of TKA was significantly different than the native condition, except for 0° (p=0.32). Although, tibial varus laxity of TKA was lower than the native at all angles, significant difference was only found at 0° (p=0.03) and 90° (p=0.02). No significant differences were observed in postoperative collateral ligament strains, as compared to the native condition, for all flexion angles, except for MCL strain at 30° (p=0.02) and 60° (p=0.01). Results from this experimental study supported our hypotheses, barring MCL strain in mid-flexion, which might be associated with the implant design. Restored collateral ligament strains with reduced joint laxity, demonstrated by the PIPB technique in TKA In conclusion, to further prove the success of PIPB, further biomechanical studies are required to evaluate the success rate of PIPB technique in different implant designs.
Understanding the long-term effects of total knee arthroplasty (TKA) on joint kinematics is vital to assess the success of the implant design and surgical procedure. However, while Two fresh-frozen lower limbs from a single donor (male, age: 83yr, ht: 1.83m, wt: 86kg), who had undergone bilateral TKA (Genesis II, Smith&Nephew, Memphis, USA) 19 years prior to his demise, were obtained following ethical approval from the KU Leuven institutional board. The specimens were imaged using computed tomography (CT) and tested in a validated knee simulator3 replicating active squatting and varus-valgus laxity tests. Tibiofemoral kinematics were recorded using an optical motion capture system and compared to various studies in the literature using the same implant – experimental studies based on cadaveric specimens (CAD)1,4 and an artificial specimen (ART)5, and a computational study (COM)6. Maximum tibial abduction during laxity tests for the left leg (3.54°) was comparable to CAD (3.30°), while the right leg exhibited much larger joint laxity (8.52°). Both specimens exhibited valgus throughout squatting (left=2.03±0.57°, right=5.81±0.19°), with the change in tibial abduction over the range of flexion (left=1.89°, right=0.64°) comparable to literature (CAD=1.28°, COM=2.43°). The left leg was externally rotated (8.00±0.69°), while the right leg internally rotated (−15.35±1.50°), throughout squatting, with the change in tibial rotation over the range of flexion (left=2.61°, right=4.79°) comparable to literature (CAD=5.52°, COM=4.15°). Change in the femoral anteroposterior translation over the range of flexion during squatting for both specimens (left=14.88mm, right=6.76mm) was also comparable to literature (ART=13.40mm, COM=20.20mm). Although TKA was reportedly performed at the same time on both legs of the donor by the same surgeon, there was a stark difference in their post-operative joint kinematics. A larger extent of intraoperative collateral ligament release could be one of the potential reasons for higher post-operative joint laxity in the right leg. Relative changes in post-operative tibiofemoral kinematics over the range of squatting were similar to those reported in the literature. However, differences between absolute magnitudes of joint kinematics obtained in this study and findings from the literature could be attributed to different surgeons performing TKA, with presumable variations in alignment techniques and/or patient specific instrumentation, and the slightly dissimilar ranges of knee flexion during squatting. Five cadaveric legs with total knee implants (NH019 2017-02-03) were submitted to a varus (LCL) and valgus (MCL) ramped loading (0 – 40N). Ultrasound radiofrequency (rf) data and reference surface strains data, obtained with 3D digital image correlation (DIC), were collected synchronously. Prior to processing, US data were qualitatively assessed and specimens displaying substantial imaging artefacts were discarded, leaving five LCL and three MCL specimens in the analysis. Ultrasound rf data were processed in Matlab (The MathWorks, Inc., Natick, MA) with a custom-built speckle tracking approach adapted from a method validated on larger tendons and based on normalized cross-correlation. Digital image correlation data were processed with commercial software VIC3D (Correlated Solutions, Inc., Columbia, SC). To optimize speckle tracking, several tracking parameters were tested: kernel and search window size, minimal correlation coefficient and simulated frame rate. Parameters were ranked according to three comparative measures between US- and DIC-based strains: R2, mean absolute error and strains differences at 40N. Parameters with best average rank were considered as optimal. To quantify the agreement between US- and DIC-based strain of each specimen, the considered metrics were: R2, mean absolute error and strain differences at 40N. The LCL showed a good agreement with a high average R2 (0.97), small average mean absolute difference (0.37%) and similar strains at 40N (DIC = 2.92 ± 0.10%; US = 2.99 ± 1.16%). The US-based speckle tracking method showed worse performance for the MCL with a lower average correlation (0.55). Such an effect has been observed previously and may relate to the difficulty in acquiring sufficient image quality for tracking the MCL compared to the LCL, which likely arises due to structural or mechanical differences; notably MCL is larger, thinner, more wrapped around the bone and stretches less. However, despite these challenges, the MCL tracking still showed small average mean absolute differences (0.44%) and similar strains at 40N (DIC = 1.48 ± 0.06%; US = 1.44 ± 1.89%). Pre-operative 3D glenoid planning improves component placement in terms of version, inclination, offset and orientation. Version and inclination measurements require the position of the inferior angle. As a consequence, current planning tools require a 3D model of the full scapula to accurately determine the glenoid parameters. Statistical shape models (SSMs) can be used to reconstruct the missing anatomy of bones. Therefore, the objective of this study is to develop and validate an SSM for the reconstruction of the inferior scapula, hereby reducing the irradiation exposure for patients. The training dataset for the statistical shape consisted of 110 CT images from patients without observable scapulae pathologies as judged by an experienced shoulder surgeon. 3D scapulae models were constructed from the segmented images. An open-source non-rigid B-spline-based registration algorithm was used to obtain point-to-point correspondences between the models. A statistical shape model was then constructed from the dataset using principal component analysis. Leave-one-out cross-validation was performed to evaluate the accuracy of the predicted glenoid parameters from virtual partial scans. Five types of virtual partial scans were created on each of the training set models, where an increasing amount of scapular body was removed to mimic a partial CT scan. The statistical shape model was reconstructed using the leave-one-out method, so the corresponding training set model is no longer incorporated in the shape model. Reconstruction was performed using a Monte Carlo Markov chain algorithm, random walk proposals included both shape and pose parameters, the closest fitting proposal was selected for the virtual reconstruction. Automatic 3D measurements were performed on both the training and reconstructed 3D models, including glenoid version, inclination, glenoid centre point position and glenoid offset. In terms of inclination and version we found a mean absolute difference between the complete model and the different virtual partial scan models of 0.5° (SD 0.4°). The maximum difference between models was 3° for inclination and 2° for version. For offset and centre point position the mean absolute difference was 0 mm with an absolute maximum of 1 mm. The magnitude of the mean and maximum differences for all anatomic measurements between the partial scan and complete models is smaller than the current surgical accuracy. Considering these findings, we believe a SSM based reconstruction technique can be used to accurately reconstruct the glenoid parameters from partial CT scans.
Orthopaedic training sessions, vital for surgeons to understand post-operative joint function, are primarily based on passive and subjective joint assessment. However, cadaveric knee simulators, commonly used in orthopaedic research,1 could potentially benefit surgical training by providing quantitative joint assessment for active functional motions. The integration of cadaveric simulators in orthopaedic training was explored with recipients of the European Knee Society Arthroplasty Travelling Fellowship visiting our institution in 2018 and 2019. The aim of the study was to introduce the fellows to the knee joint simulator to quantify the surgeon-specific impact of total knee arthroplasty (TKA) on the dynamic joint behaviour, thereby identifying potential correlations between surgical competence and post-operative biomechanical parameters. Eight fellows were assigned a fresh-frozen lower limb each to plan and perform posterior-stabilised TKA using MRI-based patient-specific instrumentation. Surgical competence was adjudged using the Objective Structured Assessment of Technical Skills (OSATS) adapted for TKA.2 All fellows participated in the OSATS scores ranged from 79.6% to 100% (mean=93.1, SD=7.7). A negative correlation was observed between surgical competence and change in post-operative tibial kinematics over the entire range of motion during passive flexion – OSATS score vs. change in tibial abduction (r=−0.87; p=0.003), OSATS score vs. change in tibial rotation (r=−0.76; p=0.02). When compared to the native condition, post-operative tibial internal rotation was higher during passive flexion (p<0.05), but lower during squatting (p<0.033). Post-operative joint stiffness was greater in extension than in flexion, without any correlation with surgical competence. Although trained at different institutions, all fellows followed certain standard intraoperative guidelines during TKA, such as achieving neutral tibial abduction and avoiding internal tibial rotation,4 albeit at a static knee flexion angle. However, post-operative joint kinematics for dynamic motions revealed a strong correlation with surgical competence, i.e. kinematic variability over the range of passive flexion post-TKA was lower for more skilful surgeons. Moreover, actively loaded motions exhibited stark differences in post-operative kinematics as compared to those observed in passive motions. In conclusion, the inclusion of cadaveric simulators replicating functional joint motions could help quantify training paradigms, thereby enhancing traditional orthopaedic training, as was also the unanimous opinion of all participating fellows in their positive feedback.
Degeneration of the shoulder joint is a frequent problem. There are two main types of shoulder degeneration: Osteoarthritis and cuff tear arthropathy (CTA) which is characterized by a large rotator cuff tear and progressive articular damage. It is largely unknown why only some patients with large rotator cuff tears develop CTA. In this project, we investigated CT data from ‘healthy’ persons and patients with CTA with the help of 3D imaging technology and statistical shape models (SSM). We tried to define a native scapular anatomy that predesignate patients to develop CTA. Statistical shape modeling and reconstruction:
A collection of 110 CT images from patients without glenohumeral arthropathy or large cuff tears was segmented and meshed uniformly to construct a SSM. Point-to-point correspondence between the shapes in the dataset was obtained using non-rigid template registration. Principal component analysis was used to obtain the mean shape and shape variation of the scapula model. Bias towards the template shape was minimized by repeating the non-rigid template registration with the resulting mean shape of the first iteration. Eighty-six CT images from patients with different severities of CTA were analyzed by an experienced shoulder surgeon and classified. CT images were segmented and inspected for signs of glenoid erosion. Remaining healthy parts of the eroded scapulae were partitioned and used as input of the iterative reconstruction algorithm. During an iteration of this algorithm, 30 shape components of the shape model are optimized and the reconstructed shape is aligned with the healthy parts. The algorithm stops when convergence is reached.Background
Methods
Joint laxity assessments have been a valuable resource in order to understand the biomechanics and pathologies of the knee. Clinical laxity tests like the Lachman test, Pivot-shift test and Drawer test are, however, subjective of nature and will often only provide basic information of the joint. Stress radiography is another option for assessing knee laxity; however, this method is also limited in terms of quantifiability and one-dimensionality. This study proposes a novel non-invasive low-dose radiation method to accurately measure knee joint laxity in 3D. A method that combines a force controlled parallel manipulator device, a medical image and a biplanar x-ray system. As proof-of-concept, a cadaveric knee was CT scanned and subsequently mounted at 30 degrees of flexion in the device and placed inside a biplanar x-ray scanner. Biplanar x-rays were obtained for eleven static load cases. The preliminary results from this study display that the device is capable of measuring primary knee laxity kinematics similar to what have been reported in previous studies. Additionally, the results also display that the method is capable of capturing coupled motions like internal/external rotation when anteroposterior loads are applied. We have displayed that the presented method is capable of obtaining knee joint laxity in 3D. The method is combining concepts from robotic arthrometry and stress radiography into one unified solution that potentially enables unprecedented 3D joint laxity measurements non-invasively. The method potentially eliminates limitations present in previous methods and significantly reduces the radiation exposure of the patient compared to conventional stress radiography.
Despite high success rates following total knee arthroplasty (TKA), knee kinematics are altered following TKA. Additionally, many patients report that their reconstructed knee does not feel ‘normal’ [1], potentially due to the absence of the anterior cruciate ligament (ACL), an important knee stabilizer and proprioceptive mechanism. ACL-retaining implants have been introduced with the aim of replicating native knee kinematics, however, there has yet to be a detailed comparison between knee kinematics in the native knee and one reconstructed with an ACL-retaining implant. Six fresh-frozen right legs (77±10 yr, 5 male) were mounted in a kinematic rig and subjected to squatting (40°-105°) motions. The vertical positon of the hip was manipulated with a linear actuator to induce knee flexion while the quadriceps were loaded with an actuator to maintain a vertical load of 90 N at the ankle [2]. Medial/lateral hamstring forces were applied with 50 N load springs. During testing, an infrared camera system recorded the trajectories of spherical markers rigidly attached to the femur and tibia. Two trials were performed per specimen. Following testing on the native knee, specimens were implanted with an ACL-retaining TKA (Vanguard XP, Zimmer Biomet) and all trials were repeated. Three inlay thicknesses were tested to simulate optimal balancing as well as over- (1 mm thicker) and understuffing (1 mm thinner) relative to the optimal thickness. Pre-operative computed tomography scans allowed identification of bony landmarks and marker orientation, which were used define anatomically relevant coordinate systems. The recorded marker trajectories were transformed to anatomical translations/rotations and resampled at increments of 1° of knee flexion. Translations of the medial and lateral femoral condyle centers were scaled to maximum anterior-posterior (AP) width of the medial and lateral tibial plateau, respectively. For all kinematics, statistical analysis between knee conditions was conducted using repeated measures ANOVA in increments of 10° knee flexion. Internal rotation of the tibia was significantly lower (p<0.05) for the three reconstructed conditions relative to the native knee at flexion angles of 60° and below. No significant differences in tibial rotation were observed between the balanced, overstuffed, or understuffed conditions. The varus orientation was not significantly influenced by implantation, regardless of inlay thickness, for all flexion angles. At 40° flexion, the AP position of the femoral medial condyle was significantly more anterior for the native knee relative to the balanced and understuffed conditions. This finding was not significant for the other flexion angles. No significant differences were found for the lateral condyle center AP position at any flexion angle. Preservation of the cruciate ligaments during total knee arthroplasty may allow better physiologic representation of knee kinematics. The implants tested in this study were able to replicate kinematics of the native knee, except for tibial rotation and AP position of the medial femoral condyle in early knee flexion. Interestingly, the impact of inlay thickness was generally small, suggesting some tolerance in the choice of inlay thickness.
Tendinopathies are among the most common musculoskeletal injuries. Nowadays, part of its diagnosis is established through subjective qualitative evaluation of 2D ultrasound (US). This enables limited diagnostic differentiation or therapeutic optimization and has limited added value to diagnosis in an earlier stage. It is generally accepted that extra diagnostic information can be obtained via strain evaluation. The accurate validation of strain estimation is challenging due to the lack of a ground-truth. Therefore we evaluate the repeatability of displacement and strain estimations in the longitudinal direction, using an easy, fast and interactive application to estimate local strain during dynamic loading of the tendon. One healthy volunteer laid in a prone position with the foot fixed to an isokinetic device. Three sets of passive movement between −10° plantarflexion and +10° dorsiflexion were performed and repeated the following day. During this, US images with a spatial resolution of 0.02mm × 0.09mm were acquired at a frame-rate of 100Hz. The US system used was the Vevo2100 with a MS250 linear array transducer with a center frequency of 20MHz. After image collection, consecutive pairs of 2D images were registered in a multi-resolution scheme, using an affine and b-spline transformation optimized by the minimization of the sum-of-squared differences, to obtain deformation vector fields. Lastly the interactive application allows local analysis of tissue displacement and strain within selected regions of interest. Mean and standard deviation of the intra- and inter-day relative differences were calculated.Introduction

In patients with neural disorders such as cerebral palsy, three-dimensional marker-based motion analysis has evolved to become a well standardized procedure with a large impact on the clinical decision-making process. On the other hand, in knee arthroplasty research, motion analysis has been little used as a standard tool for objective evaluation of knee joint function. Furthermore, in the available literature, applied methodologies are diverse, resulting in inconsistent findings [1]. Therefore we developed and evaluated a new motion analysis framework to enable standardized quantitative assessment of knee joint function. The proposed framework integrates a custom-defined motion analysis protocol with associated reference database and a standardized post-processing step including statistical analysis. Kinematics are collected using a custom-made marker set defined by merging two existing protocols and combine them with a knee alignment device. Following a standing trial, a star-arc hip motion pattern and a set of knee flexion/extension cycles allowing functional, subject-specific calibration of the underlying kinematic model, marker trajectories are acquired for three trials of a set of twelve motor tasks: walking, walking with crossover turn, walking with sidestep turn, stair ascent, stair descent, stair descent with crossover turn, stair descent with sidestep turn, trunk rotations, chair rise, mild squat, deep squat and lunge. This specific set of motor tasks was selected to cover as much as possible common daily life activities. Furthermore, some of these induce greater motion at the knee joint, thus improving the measurement-to-error ratio. Kinetics are acquired by integrating two forceplates in the walkway. Bilateral muscle activity of 8 major muscles is monitored with a 16 channel wireless electromyography (EMG) system. Finally, custom-built software with an associated graphical user interface was created for automated and flexible analysis of gait lab data, including repeatability analysis, analysis of specific kinematic, kinetic and spatiotemporal parameters and statistical comparisons.INTRODUCTION
