The medical model of history, examination and investigation forms the bedrock of diagnosis and management of all patients. The essence is the recognition of patterns of symptoms and signs. In the modern era there are an increasing number of non-medical resources ranging from web-based information, computer diagnostic aids and non-specialist healthcare professionals to provide a diagnosis and commence management of a wide range of conditions, including knee problems. We analysed the quality and patterns of clinical presentation in order to answer the question how closely clinical symptoms and examination findings correlate to diagnosis based on MRI scan and/or arthroscopic findings. The analysis was a dataset of a consecutive series of patients, aged 18 to 45, with no past history of knee problems or end stage arthritis, presenting to a single specialist triage physiotherapist, working within an integrated knee service, who fully completed a standardised knee assessment proforma of presenting symptoms and signs at a large district general hospital. The study comprises 86 patients and 98 knees. We analysed this data based on diagnostic findings of MRI scan or arthroscopy to provide definitive intra-articular diagnosis. Based on standard textbook descriptions of common presentations, we went on to define the patients' presentation history and examination as typical or atypical, with typical meaning the symptoms and signs correlated with the diagnosis. The null hypothesis is that patients have a high chance of typical presentations for common knee conditions. In the 75% of patients with a significant intra-articular pathology we found the majority had chondral rather than meniscal tears 1.7 to 1. Forty four percent of patients had atypical symptoms and 71% had atypical clinical signs, 30% and only 26% of the cohort had both typical symptoms and signs together, reflecting a surprisingly low positive predictive probability of symptoms and signs in this group of patients, particularly those with chondral lesions which was 44%. In this cohort, 57% of the cohort has 3 or more multiple diagnoses. In the diagnostically normal group, 43% had symptoms and signs typical for a meniscal tear. We conclude that clinical symptoms and signs surprisingly inaccurate in guiding intra-articular pathology within the knee, even in a sub-set considered the easy and accurate to assess. The number of multiple diagnoses and the incidence of false positive results also means that simplistic interpretations of non-definitive diagnoses and linear causation of pain pathways should be treated cautiously

Aim. Patients with late acute periprosthetic joint infections (PJI) and treated with surgical debridement have a high failure rate. Previous studies have shown that rheumatoid arthritis (RA) is an independent risk factor for treatment failure. We conducted a case-control study to identify predictors for failure in late acute PJI treatment in RA patients. We hypothesize that patients with RA have a higher failure rate compared to controls due to the use of immunosuppressive drugs. Method. Data of an international multicenter retrospective observational study was used. Late acute PJI was defined as a sudden onset of symptoms and signs of a PJI, more than 3 months after implantation. Failure of treatment was defined as persistent signs of infection, relapse with the same or reinfection with a different micro-organism, need for prosthesis removal or death. Cases with RA were matched with cases without RA based on the affected joint. A Cox survival analyses, stratified for RA, was used to calculate hazard ratio's (HR) for failure. Subgroup analyses were used to explore other predictors for treatment failure in RA patients. Results. A total of 40 patients with RA and 80 controls without RA were included. Treatment failure occurred in 65% patients with RA compared to 45% for controls (p= .052). 68% of patients with RA used immunosuppressive drugs at time of PJI diagnosis. The use or continuation of immunosuppressive drugs in PJI was not associated with a higher failure rate; neither were the duration of symptoms and causative microorganism. The time between implantation of the prosthetic joint and diagnosis of infection was longer in RA patients: median 110 (IQR 41-171) vs 29 months (IQR 7.5–101.25). Exchange of mobile components was associated with a lower risk of treatment failure (HR 0.489, 95% CI 0.242–0.989, p-value .047). Conclusions. The use of immunosuppressive drugs does not seem to be associated with a higher failure rate in patients with RA. Mobile exchange in RA patients is associated with a lower risk of failure. This might be due to the significantly older age of the prosthesis in RA patients. Future studies are needed to explore these associations and its underlying pathogenesis

There is no universally agreed definition of cauda equina syndrome (CES). Clinical signs of CES including direct rectal examination (DRE) do not reliably correlate with cauda equina (CE) compression on MRI. Clinical assessment only becomes reliable if there are symptoms/signs of late, often irreversible, CES. The only reliable way of including or excluding CES is to perform MRI on all patients with suspected CES. If the diagnosis is being considered, MRI should ideally be performed locally in the District General Hospitals within one hour of the question being raised irrespective of the hour or the day. Patients with symptoms and signs of CES and MRI confirmed CE compression should be referred to the local spinal service for emergency surgery. CES can be subdivided by the degree of neurological deficit (bilateral radiculopathy, incomplete CES or CES with retention of urine) and also by time to surgical treatment (12, 24, 48 or 72 hour). There is increasing understanding that damage to the cauda equina nerve roots occurs in a continuous and progressive fashion which implies that there are no safe time or deficit thresholds. Neurological deterioration can occur rapidly and is often associated with longterm poor outcomes. It is not possible to predict which patients with a large central disc prolapse compressing the CE nerve roots are going to deteriorate neurologically nor how rapidly. Consensus guidelines from the Society of British Neurological Surgeons and British Association of Spinal Surgeons recommend decompressive surgery as soon as practically possible which for many patients will be urgent/emergency surgery at any hour of the day or night. . Aims: We aim to investigate how often we needed extensive investigations to reach a speciþc diagnosis and whether we could identify symptoms and signs predictive of such diagnosis. Methods: We analysed the records of 60 children referred to our specialist clinics complaining of back pain. We statistically analysed the speciþcity of 9 parameters from history and clinical examination to predict the þnal diagnosis and checked the correlation between initial clinical diagnosis and þnal diagnosis following extensive investigations such as bone scan, CT and MRI scan. Results: The mean age of our group of patients was 12.3 years (range=3–18 years). We failed to reach a speciþc initial diagnosis after history taking, clinical examination and simple X-rays of the spine in 33 patients (55%). By the completion of investigations a speciþc diagnosis was not reached in only 18 (35%), and the difference was signiþcant compared to the other 65% (chi square(1)=4.41, p< 0.05). In 15 children specialist investigations clariþed a diagnosis of spondylolysis, Scheuermannñs disease and tumour. A hypothesised set of 9 clinical symptoms and signs was not predictive of speciþcity of initial clinical diagnosis (RL 2=0.12, chi square (9)=10.07, p> 0.05), but was predictive of þnal diagnosis (RL 2=0.35, chi square(9)=22.88, p< 0.01). A speciþc diagnosis was less likely in the presence of activity related pain or intermittent pain. There was a high degree of correlation between initial clinical diagnosis and þnal diagnosis. (phi=0.70, chi square(1)=25.07, p< 0.001). Conclusions: For about half our group of children, back pain was not a speciþc symptom. A set of common set of clinical symptoms and signs did not initially predict speciþcity. Based on these initial þndings we should continue to investigate in depth any referred child with back pain

Introduction: Anterior knee pain is a commonly encountered orthopaedic symptom. Textbooks list a number of clinical symptoms and physical signs that are specific for the PF joint in the assessment of the patient with knee pain. Radiological evaluation of the PF joint is rarely helpful. The authors have investigated the value of these symptoms and signs in predicting the presence of PF pathology. Patients and Methods: All patients presenting for diagnostic knee arthroscopy were enrolled prospectively. A detailed proforma was completed with each patient pre-operatively recording the presence or absence of specific PF symptoms and clinical signs. A blinded independent observer recorded the operative findings of the PF joint, the presence / absence of a plica and / or anterior synovitis. Chondromalacia (CM) was graded using the Outerbridge classification and grade 2 or greater was considered a positive finding. Pre-operative symptoms and signs were correlated with arthroscopic findings and their positive predictive value (PPV) was determined. Results: Thirty patients were enrolled over a 2-month period. Pure anterior knee pain was present in 10 patients. The remainder had medial (15), lateral (2) or generalised pain (3). 20 of the thirty patients had degenerative changes in the PF joint. One patient had a plica and no patients had significant anterior synovitis. 20 of the 30 patients had pathological findings elsewhere in their joint. In the prediction of PF degenerative changes, the PPV of symptoms exacerbated by squatting was 0.53; stairs – 0.55; kneeling – 0.57; rising from low chair – 0.6 and night pain – 0.5. Analysis of specific signs in predicting PF changes showed that the PPV of PF crepitus was 0.5; pain exacerbated by patellar compression 0.6; and patellar facet compression was 0.62. Conclusion: Degenerative changes of the PF joint are common in our patient population. The value of knee arthroscopy in patients with isolated PF symptoms is questionable. Our assessment of these patients with specific questions and physical signs has been shown to have a low PPV in predicting ultimate arthroscopic findings in the PF joint

The aim of this study was to produce estimates of specificity and predictive value of presenting symptoms and signs of paediatric bone cancer, a rare and frequently misdiagnosed condition, to aid clinical decision-making in primary care. A systematic literature review plus questionnaire to primary care physicians were carried out to determine frequency of bone cancer symptoms in both cancer and the benign conditions as which cancer is misdiagnosed. Literature sources – Ovid MEDLINE (1950-May 2008), EMBASE (1980-May 2008) and AMED (Allied and Alternative Medicine) (1985-May 2008). Literature review methods – We included systematic reviews, cohort studies or case series (where n ≥ 10), reporting frequency of symptoms and signs at initial presentation, as originally recorded in case notes or observed by the authors, in subjects aged 0–18 years. Disease incidence data was taken from retrospective and prospective studies from 1980 onwards which recorded incidence over a defined time period, in a large pre-defined population within Europe, North America or Australia. Questionnaire respondents – 32 general practitioners and paediatric Accident & Emergency physicians throughout Scotland and England. Positive predictive values (PPVs) for bone cancer symptoms range from 0.003 to 0.034% (percentage of children presenting with symptom who have cancer). Specificity (percentage of children without cancer who do not have the symptom) varies considerably between symptoms and ranges from 24% (tenderness) to 95% (weight loss). Specificity can be improved by looking for combinations of symptoms. Weight loss and fever are the features with both highest specificity and highest PPV. Bone cancer symptoms, even those with high specificity for cancer, have low positive predictive value. We suggest that diagnosis based on initial presentation to primary care is intrinsically difficult and that delay in diagnosis is not unreasonable if it is to make use of time as a diagnostic aid

Aims. The aim of the British Association for Surgery of the Knee (BASK) Meniscal Consensus Project was to develop an evidence-based treatment guideline for patients with meniscal lesions of the knee. Materials and Methods. A formal consensus process was undertaken applying nominal group, Delphi, and appropriateness methods. Consensus was first reached on the terminology relating to the definition, investigation, and classification of meniscal lesions. A series of simulated clinical scenarios was then created and the appropriateness of arthroscopic meniscal surgery or nonoperative treatment in each scenario was rated by the group. The process was informed throughout by the latest published, and previously unpublished, clinical and epidemiological evidence. Scenarios were then grouped together based upon the similarity of clinical features and ratings to form the guideline for treatment. Feedback on the draft guideline was sought from the entire membership of BASK before final revisions and approval by the consensus group. Results. A total of 45 simulated clinical scenarios were refined to five common clinical presentations and six corresponding treatment recommendations. The final guideline stratifies patients based upon a new, standardized classification of symptoms, signs, radiological findings, duration of symptoms, and previous treatment. Conclusion. The 2018 BASK Arthroscopic Meniscal Surgery Treatment Guidance will facilitate the consistent identification and treatment of patients with meniscal lesions. It is hoped that this guidance will be adopted nationally by surgeons and help inform healthcare commissioning guidance. Validation in clinical practice is now required and several areas of uncertainty in relation to treatment should be a priority for future high-quality prospective studies. Objective. To determine on radiographs the presence of Basilar Impression (BI) in children with Osteogenesis Imperfecta (OI). To confirm this sign and altered geometrical relationships of the craniocervical junction in course of time with magnetic resonance imaging (MRI). Methods and patients: In a cohort study of 130 patients with OI (OI type I:85; OI type III: 21; OI type IV: 24) lateral radiographs of the skull and cervical spine were made in a standardized way. MRI scans were performed when BI was suspected based upon protrusion of the odontoid above Chamberlain’s line. Intracranial abnormalities as well as the basal angle were described. Neurological examination was performed in patients with conclusive BI at MRI scan. Results and Discussion: In eight patients BI could be confirmed by MRI scan. None of the children had or developed in time neurological symptoms or signs. In follow up no alteration of intracranial findings were seen, although in one child Chamberlain’s line increased from 8 mm (first MRI) to 15 mm (last MRI). Conclusion: In our cohort study no alteration of the intracranial contents was seen at subsequent MRI scans. Although anatomic deformations exist in BI, no neurological symptoms or signs were present in our study and no operative reconstruction had to be performed. Periodical MRI – scan has not been of influence on the clinical decision making process. At the moment we perform a MRI-scan if BI is suspected on lateral skull radiographs

Introduction. The authors recognised that patients presenting to the Orthopaedic Spinal Rapid Access Service with symptoms and or signs of cauda equina syndrome may not have the diagnosis confirmed radiologically. Altered sensation in the ‘saddle area’, bilateral sciatica, urinary incontinence or retention, altered bowel habit, and sexual dysfunction are well recognised symptoms of cauda equina syndrome. Recognised side-effects of neuropathic medications commonly prescribed for radicular pain include: altered sensation, urinary incontinence or retention, and sexual dysfunction. We have undertaken a retrospective cohort analysis in order to identify the relationship between prescribed medications and presenting symptoms and signs. Method. 151 patients were referred to the service within a 6 month period. Case notes of 34 patients presenting with symptoms and or objective signs of CES in absence of positive radiological findings were reviewed. Data collected included the patient's age, sex, prescribed medications and presenting symptoms. Results. Of these 34, 9(26%) presented with altered bladder function and ‘saddle area’ sensation and 25(74%) with isolated bladder symptoms. Mean age was 47 in both female and males, 26 females and 8 males. 16(47%) were taking neuropathic medications, 7(22%) anti-depressants and 9(28%) anti-convulsant medications. Conclusion. 16(47%) of patients presenting with CES in the absence of radiological evidence were prescribed neuropathic medications with known side effects that may contribute to their symptoms. Therefore clinicians should take due consideration of prescribed medications as a possible cause of CES signs and symptoms. Further work is required to analyse data from a larger patient population in order to identify if particular medications carry a higher risk. Conflicts of interest: No conflicts of interest. Aims

People with severe, persistent low back pain (LBP) may be offered lumbar spine fusion surgery if they have had insufficient benefit from recommended non-surgical treatments. However, National Institute for Health and Care Excellence (NICE) 2016 guidelines recommended not offering spinal fusion surgery for adults with LBP, except as part of a randomized clinical trial. This survey aims to describe UK clinicians’ views about the suitability of patients for such a future trial, along with their views regarding equipoise for randomizing patients in a future clinical trial comparing lumbar spine fusion surgery to best conservative care (BCC; the FORENSIC-UK trial).

Methods

An online cross-sectional survey was piloted by the multidisciplinary research team, then shared with clinical professional groups in the UK who are involved in the management of adults with severe, persistent LBP. The survey had seven sections that covered the demographic details of the clinician, five hypothetical case vignettes of patients with varying presentations, a series of questions regarding the preferred management, and whether or not each clinician would be willing to recruit the example patients into future clinical trials.

Trunnion corrosion in metal-on-polyethylene THA is poorly understood, with multifactorial etiology, and the patients present with “hip pain”. We analysed the presenting symptoms and signs, intraoperative findings and the early results and complications of operative treatment. One surgeon treated 9 patients (6 male, 3 female), mean age 74 years, with the onset of symptoms at a mean of 7 years (range 3–18) after index surgery. The taper size was 12/14 in seven, 14/16 in one, and 6 degree in one hip. The preoperative mean cobalt level was 7.1 ppb (range, 2.2–12.8) and mean chromium level was 2.2 ppb (range, 0.5–5.2). MARS MRI showed fluid collection and pseudotumor in 5, fluid collection only in two, and synovitis/debris in one hip. In one patient, there was no preoperative MRI. There were a myriad of clinical presentations: thigh rash alone in one; diffuse leg pain and hip rash in one; acute pseudo-sepsis in one; iliopsoas tendinitis and diffuse rash in one; trochanteric bursitis in one; groin pain only in one; thigh-buttock pain in two; and diffuse hip pain and limp in one patient. Intraoperatively, 6 patients had liner and ceramic (or oxidized zirconium) head exchange only. Three patients had concurrent acetabular revision: one for broken locking mechanism; one because liner was unavailable, and one had acetabular loosening. The postoperative metal levels decreased in all patients: mean cobalt 0.5 ppb (range, 0–1.8) and mean chromium 0.9 ppb (range, 0–2.6). Seven patients had good pain relief and no complications. There were two major complications requiring reoperation: acute infection at 6 weeks and patient required 2-stage reimplantation; and second patient had recurrent dislocation and was revised to a dual mobility component. Trunnion corrosion in metal-on-polyethylene THA has several clinical presentations, including local skin rash, iliopsoas tendinitis, and other limb dysfunction. There should be a high index of suspicion and serum cobalt/chromium levels are recommended for diagnosis. The patients should be counseled about possible postoperative complications

Aims

The aim of this study was to determine the consensus best practice approach for the investigation and management of children (aged 0 to 15 years) in the UK with musculoskeletal infection (including septic arthritis, osteomyelitis, pyomyositis, tenosynovitis, fasciitis, and discitis). This consensus can then be used to ensure consistent, safe care for children in UK hospitals and those elsewhere with similar healthcare systems.

Methods

A Delphi approach was used to determine consensus in three core aspects of care: 1) assessment, investigation, and diagnosis; 2) treatment; and 3) service, pathways, and networks. A steering group of paediatric orthopaedic surgeons created statements which were then evaluated through a two-round Delphi survey sent to all members of the British Society for Children’s Orthopaedic Surgery (BSCOS). Statements were only included (‘consensus in’) in the final agreed consensus if at least 75% of respondents scored the statement as critical for inclusion. Statements were discarded (‘consensus out’) if at least 75% of respondents scored them as not important for inclusion. Reporting these results followed the Appraisal Guidelines for Research and Evaluation.

Aims

Steroid injections are used for subacromial pain syndrome and can be administered via the anterolateral or posterior approach to the subacromial space. It is not currently known which approach is superior in terms of improving clinical symptoms and function. This is the protocol for a randomized controlled trial (RCT) to compare the clinical effectiveness of a steroid injection given via the anterolateral or the posterior approach to the subacromial space.

Methods

The Subacromial Approach Injection Trial (SAInT) study is a single-centre, parallel, two-arm RCT. Participants will be allocated on a 1:1 basis to a subacromial steroid injection via either the anterolateral or the posterior approach to the subacromial space. Participants in both trial arms will then receive physiotherapy as standard of care for subacromial pain syndrome. The primary analysis will compare the change in Oxford Shoulder Score (OSS) at three months after injection. Secondary outcomes include the change in OSS at six and 12 months, as well as the Pain Numeric Rating Scale (0 = no pain, 10 = worst pain), Disabilities of Arm, Shoulder and Hand questionnaire (DASH), and 36-Item Short-Form Health Survey (SF-36) (RAND) at three months, six months, and one year after injection. Assessment of pain experienced during the injection will also be determined. A minimum of 86 patients will be recruited to obtain an 80% power to detect a minimally important difference of six points on the OSS change between the groups at three months after injection.

Aims

Chronic osteomyelitis (COM) of the lower limb in adults can be surgically managed by either limb reconstruction or amputation. This scoping review aims to map the outcomes used in studies surgically managing COM in order to aid future development of a core outcome set.

Methods

A total of 11 databases were searched. A subset of studies published between 1 October 2020 and 1 January 2011 from a larger review mapping research on limb reconstruction and limb amputation for the management of lower limb COM were eligible. All outcomes were extracted and recorded verbatim. Outcomes were grouped and categorized as per the revised Williamson and Clarke taxonomy.

We live in an era where younger, fitter, more active patients are presenting with the symptoms and signs of degenerative joint disease and require total knee and total hip arthroplasty at a young age. At the same time, this population of patients is living longer and longer and is likely to create new and more complex failure modes for their implants. The ideal solution is a biological one, whereby we can either prevent joint degradation or catch it in its early stages and avoid further deterioration. There may also be advances along the way in terms of partial arthroplasty and focal resurfacing that will help us prevent the need for total joint arthroplasty. There are several tensions that need to be considered. Should we resurface / replace early, particularly now that we have access to navigation and robotics and can effectively customise the implants to the patient's anatomy and their gait pattern? This would allow good function at a young age. Or should we wait as long as possible and risk losing some function for the sake of preserving the first arthroplasty for the lifetime of the patient?. There are some key issues that we still do not fully understand. The lack of true follow-up data beyond 20 or 30 years is worrying. The data available tends to be from expert centers, and always has a dramatic loss to follow-up rate. We worry about bearing surfaces and how those materials will behave over time but we really do not know the effect of chronic metal exposure over several decades, nor do we really understand what happens to bone as it becomes more and more osteopenic and fragile around implants. We have largely recorded but ignored stress shielding, whereas this may become a very significant issue as our patients get older, more fragile, more sarcopaenic and more neurologically challenged. All the fixation debates that we have grappled with, may yet come back to the fore. Can ingrowth lead to failure problems later on? Will more flexible surfaces and materials be required to fit in with the elasticity of bone?. We have failed dramatically at translating the in vitro to the in vivo model. It seems that the in vitro model tells us when failure is going to occur but success in vitro does not predict success in vivo. We, therefore, cannot assume that long-term wear data from simulators will necessarily translate to the extreme situations in vivo where the loading is not always idealised, and can create adverse conditions. We must, therefore, consider further how to improve and enhance our interventions. There is no doubt that the avoidance of arthroplasty needs to be at the heart of our thinking but, ultimately, if arthroplasty is to be performed, it needs to be performed expertly and in such a way as to minimise later failure. It also, clearly, needs to be cost-effective. The next stage will no doubt involve close cooperation between surgeons, engineers and industry partners to identify individualised surgical targets, select an appropriate prosthesis to minimise soft-tissue strain and develop a reproducible method of achieving accurate implantation. An ideal outcome can only be achieved by an appropriately trained surgeon selecting the optimal prosthesis to implant in the correct position in the well-selected patient. In the longer term, our choice of implants and the way that they are inserted and fixed must take into account the evolving physiology of our patients, the nature of our devices and how to limit harm from them, and the long-term impact of the materials used which we sometimes still do not understand

Aims

This study was performed to explore the effect of melatonin on pyroptosis in nucleus pulposus cells (NPCs) and the underlying mechanism of that effect.

Methods

This experiment included three patients diagnosed with lumbar disc herniation who failed conservative treatment. Nucleus pulposus tissue was isolated from these patients when they underwent surgical intervention, and primary NPCs were isolated and cultured. Western blotting, reverse transcription polymerase chain reaction, fluorescence staining, and other methods were used to detect changes in related signalling pathways and the ability of cells to resist pyroptosis.

Aims. To determine whether a correlation exists between the clinical symptoms and signs of impingement, and the severity of the lesions seen at bursoscopy. Methods. Fifty-five consecutive patients who underwent arthroscopic subacromial decompression were analysed. Pre-operatively patients completed an assessment form consisting of visual analogue pain score, and shoulder satisfaction. The degree of clinical impingement was also recorded. At arthroscopy impingement was classified according to the Copeland-Levy classification. Clinical assessment and scoring was performed at 6 months post-operatively. Linear regression coefficients were calculated to determine if the degree of impingement at arthroscopy correlated with pre-operative pain, satisfaction and clinical signs of impingement. Results. Pre-op pain levels, shoulder satisfaction and the degree of clinical impingement did not correlate with severity of the lesions of the acromion and cuff. (Average Correlation co-efficient r2 = 0.018). There was no correlation between the improvement in the shoulders post-operatively and the severity of lesions (r2 = 0.008). Conclusion. There is no significant correlation between pain, clinical signs, or satisfaction and the severity of lesions seen at arthroscopy

Introduction. Cauda equina syndrome represents the constellation of symptoms and signs resulting from compression of lumbosacral nerve routes. Combined with subjective neurological findings, a reduction in anal tone is an important sign deeming further imaging necessary. Our main objective was to investigate the validity of DRE for assessment of anal tone. Method. 75 doctors completed a questionnaire documenting their grade, speciality and experience in performing DRE. A model anus, using a pressure transducer surrounding an artificial canal, was assembled and calibrated. Participants performed 4 DREs on the model and predicted tone as ‘reduced’ or ‘normal’ (35 and 60 mmHg respectively), followed by a ‘squeeze’ test. 30 healthcare assistants (HCAs) with no training in DRE partook as a control group. Results. In each attempt 60%, 61%, 63% and 72% of doctors correctly identified the anal tone respectively (average accuracy 64%). HCAs had an identical average accuracy of 64%. 100% of participants correctly felt the squeeze test. For doctors no correlation was found between confidence in assessing anal tone using DRE and a correct result. 71% had received previous training in DRE with 64% of these taught how to assess anal tone. Conclusion. The results demonstrate that accuracy in assessing anal tone using DRE is limited with overall correctness of 64%. Poor correlation exists between perceived level of skill and study result. Doctors were not significantly more able than HCAs to detect correct tone. Therefore, DRE for the assessment of anal tone is not a wholly accurate tool. No conflicts of interest. No funding obtained. This work was presented at BritSpine
