Objectives. Different criteria for assessing the reduction quality of trochanteric fractures have been reported. The Baumgaertner reduction quality criteria (BRQC) are relatively common and the Chang reduction quality criteria (CRQC) are relatively new. The objectives of the current study were to compare the reliability of the BRQC and CRQC in predicting mechanical complications and to investigate the clinical implications of the CRQC. Methods. A total of 168 patients were assessed in a retrospective observational study. Clinical information including age, sex, fracture side, American Society of Anesthesiologists (ASA) classification, tip-apex distance (TAD), fracture classification, reduction quality, blade position, BRQC, CRQC, bone quality, and the occurrence of mechanical complications were used in the statistical analysis. Results. A total of 127 patients were included in the full analysis, and mechanical complications were observed in 26 patients. The TAD, blade position, BRQC and CRQC were significantly associated with mechanical complications in the univariate analysis. Only the TAD (p = 0.025) and the CRQC (p < 0.001) showed significant results in the multivariate analysis. In the comparison of the receiver operating characteristic curves, the CRQC also performed better than the BRQC. Conclusion. The CRQC are reliable in predicting mechanical complications and are more reliable than the BRQC. Future studies could use the CRQC to assess fracture reduction quality. Intraoperatively, the surgeon should refer to the CRQC to achieve good reduction in trochanteric fractures. Cite this article: Bone Joint Res 2019;8:502–508

Lag screw cut-out is a serious complication of dynamic hip screw fixation in trochanteric hip fractures. Lag screw position is recognised as a crucial factor influencing the occurrence of lag screw cut-out. We propose a modification of the Tip Apex Distance (TAD) and hypothesize that it could enhance the reliability of predicting lag screw cut-out in these injuries. A retrospective study of hip fracture cases was conducted from January 2018 to July 2022. A total of 109 patients were eligible for the final analysis. The modified TAD was measured in millimetres, based on the sum of the traditional TAD in the lateral view and the net value of two distances in the anteroposterior (AP) view. The first distance is from the lag screw tip to the opposite point on the femoral head along the lag screw axis, while the second distance is from that point to the femoral head apex. The first distance is a positive value, whereas the second distance is positive if the lag screw is superior and negative if it is inferior. Receiver operating characteristic (ROC) curve analysis was used to assess the reliability of various parameters for evaluating the lag screw position within the femoral head. Factors such as reduction quality, fracture pattern according to the AO/OTA classification, TAD, Calcar-Referenced TAD, Axis Blade Angle, Parker’s ratio in the AP view, Cleveland Zone 1, and modified TAD were statistically associated with lag screw cut-out. Among the tested parameters, the novel parameter exhibited 90.1% sensitivity and 90.9% specificity for predicting lag screw cut-out at a cut-off value of 25 mm, with a p-value < 0.001. The modified TAD demonstrated the highest reliability in predicting lag screw cut-out. A value of 25 mm may potentially reduce the risk of lag screw cut-out in trochanteric hip fractures

Introduction. Unstable ankle fractures are routinely managed operatively. Due to soft-tissue and implant related complications, there has been recent literature reporting on the non-operative management of well-reduced medial malleolus fractures following fibular stabilisation, but with limited evidence supporting routine application. This trial assessed the superiority of internal fixation of well-reduced (displacement ≤2mm) medial malleolus fractures compared with non-fixation following fibular stabilisation. Methods and participants. Superiority, pragmatic, parallel, prospective randomised clinical trial conducted over a four year period. A total of 154 adult patients with a bi- or trimalleolar fractures were recruited from a single centre. Open injuries and vertical medial malleolar fractures were excluded. Following fibular stabilisation, patients were randomised intra-operatively on a 1:1 basis to fixation or non-fixation after satisfactory fluoroscopic fracture reduction was confirmed. The primary outcome was the Olerud Molander Ankle Score (OMAS) at one-year post-randomisation. Complications and radiographic outcomes were documented over the follow-up period. Results. Among 154 participants (mean age, 56.5 years; 119 women [77%]), 144 [94%] completed the trial. At one-year the median OMAS was 80 (IQR, 60–90) in the fixation group compared with 72.5 (IQR, 55–90) in the non-fixation group (p=0.17). Complication rates were comparable. Significantly more patients in the non-fixation group developed a radiographic non-union (20% vs 0%; p<0.001), with the majority (n=8/13) clinically asymptomatic and one patient required surgical re-intervention for this. Fracture type and reduction quality appeared to influence fracture union and patient outcome. Conclusions. In this randomised clinical trial comparing internal fixation of well-reduced medial malleolus fractures with non-fixation, following fibular stabilisation, fixation was not superior according to the primary outcome. However, 1 in 5 patients following non-fixation developed a radiographic non-union and whilst the re-intervention rate to manage this was low, the future implications require surveillance. These results may support selective non-fixation of anatomically reduced medial malleolus fractures

Aim. The most frequent mechanical failure in the osteosynthesis of intertrochanteric fractures is the cut-out. Fracture pattern, reduction quality, tip-apex distance or the position of the cervico-cephalic screw are some of the factors that have been associated with higher cut-out rates. To date, it has not been established whether underlying bacterial colonization or concomitant infection may be the cause of osteosynthesis failure in proximal femur fractures (PFF). The primary objective of this study is to assess the incidence of infection in patients with cut-out after PFF osteosynthesis. Method. Retrospective cohort study on patients with cut-out after PFF osteosynthesis with endomedullary nail, from January 2007 to December 2020. Demographic data of patients (such as sex, age, ASA), fracture characteristics (pattern, laterality, causal mechanism) and initial surgery parameters were collected (time from fall to intervention, duration of surgery, intraoperative complications). Radiographic parameters were also analyzed (tip-apex distance and Chang criteria). In all cut-out cases, 5 microbiological cultures and 1 anatomopathological sample were taken and the osteosynthesis material was sent for sonication. Fracture-related infection (FRI) was diagnosed based on Metsemakers et al (2018) and McNally et al (2020) diagnostic criteria. Results. Of the 67 cut-out cases, 16 (23.9%) presented clinical, analytical or microbiological criteria of infection. Of these sixteen patients, only in 3 of them the presence of an underlying infection was suspected preoperatively. A new osteosynthesis was performed in 24 cases (35.8%) and a conversion to arthroplasty in the remaining 43 (64.2%). A comparative analysis was performed between cases with and without infection. The groups were comparable in terms of demographic data and postoperative radiological data (using Chang criteria and tip-apex distance). Patients with underlying infection had a higher rate of surgical wound complication (56.3% vs 22%, p = 0.014), higher rates of leukocytes counts (11.560 vs 7.890, p = 0.023) and time to surgery (5.88 vs 3.88 days, p = 0.072). Conclusions. One out of four osteosynthesis failure in PFF is due to underlying FRI and in almost 20% were not unsuspected before surgery. In PFF osteosynthesis failures, underlying infection should be taken into account as a possible etiological factor and thus a preoperative and intraoperative infection study should be always performed

Introduction. This study aimed to compare MIPO and IMNr in the treatment of supracondylar femur fracture following TKA in respect of fracture healing, complications and functional results. Materials and Methods. A retrospective analysis was made of 32 supracondylar femur fractures classified according to the Rorabeck classification, comprising 20 cases treated with MIPO and 12 with IMNr. The two techniques were compared in respect of ROM, KSS, SF-12 scores, intraoperative blood loss, surgery time, and radiological examination findings. Results. No significant difference was determined between the two groups in respect of age, gender and fracture type, or in the median time to union (MIPO 4.3 months, IMNr 4.2 mths) (p >0.05). In the MIPO group, 2 patients had delayed union, so revision surgery was applied. The mean postoperative ROM was comparable between IMNr and MIPO (86.2 °vs 86 °, p > 0.05). The mean Knee Society Score (KSS) and SF-12 score did not differ between the IMN and MIPO groups. (p>0.05). Reduction quality in the sagittal plane was better in the MIPO group and no difference was determined in coronal alignment. Greater shortening of the lower extremity was seen in the IMNr group than in the MIPO group. (20.3 vs 9.3mm, p<0.05). Perioperative blood loss was greater (2 units vs.1.2 units) and mean operating time was longerin the MIPO group. (126.5 min vs 102.2 min, p<0.05). Conclusion. In patients with good bone stock, supracondylar femur fracture following TKA can be treated successfully with IMN or MIPO. IMN has the advantage of less blood loss and a shorter operating time. Reduction quality may be improved with the MIPO technique. Both surgery techniques can be successfully used by orthopaedic surgeons taking a case-by-case approach

Aims. Hip displacement, common in patients with cerebral palsy (CP), causes pain and hinders adequate care. Hip reconstructive surgery (HRS) is performed to treat hip displacement; however, only a few studies have quantitatively assessed femoral head sphericity after HRS. The aim of this study was to quantitatively assess improvement in hip sphericity after HRS in patients with CP. Methods. We retrospectively analyzed hip radiographs of patients who had undergone HRS because of CP-associated hip displacement. The pre- and postoperative migration percentage (MP), femoral neck-shaft angle (NSA), and sphericity, as determined by the Mose hip ratio (MHR), age at surgery, Gross Motor Function Classification System level, surgical history including Dega pelvic osteotomy, and triradiate cartilage status were studied. Regression analyses using linear mixed model were performed to identify factors affecting hip sphericity improvement. Results. A total of 108 patients were enrolled. The mean preoperative MP was 58.3% (SD 31.7%), which improved to 9.1% (SD 15.6%) at the last follow-up. NSA and MHR improved from 156.5° (SD 11.5°) and 82.3% (SD 8.6%) to 126.0° (SD 18.5°) and 89.1% (SD 9.0%), respectively. Factors affecting the postoperative MHR were preoperative MP (p = 0.005), immediate postoperative MP (p = 0.032), and history of Dega osteotomy (p = 0.046). Conclusion. We found that hip sphericity improves with HRS. Preoperative MP, reduction quality, and acetabular coverage influence femoral head remodelling. We recommend that surgeons should consider intervention early before hip displacement progresses and that during HRS, definite reduction and coverage of the femoral head should be obtained. Aims. Proximal femur fractures treatment can involve anterograde nailing with a single or double cephalic screw. An undesirable failure for this fixation is screw cut-out. In a single-screw nail, a tip-apex distance (TAD) greater than 25 mm has been associated with an increased risk of cut-out. The aim of the study was to examine the role of TAD as a risk factor in a cephalic double-screw nail. Methods. A retrospective study was conducted on 112 patients treated for intertrochanteric femur fracture with a double proximal screw nail (Endovis BA2; EBA2) from January to September 2021. The analyzed variables were age, sex, BMI, comorbidities, fracture type, side, time of surgery, quality of reduction, pre-existing therapy with bisphosphonate for osteoporosis, screw placement in two different views, and TAD. The last follow-up was at 12 months. Logistic regression was used to study the potential factors of screw cut-out, and receiver operating characteristic curve to identify the threshold value. Results. A total of 98 of the 112 patients met the inclusion criteria. Overall, 65 patients were female (66.3%), the mean age was 83.23 years (SD 7.07), and the mean follow-up was 378 days (SD 36). Cut-out was observed in five patients (5.10%). The variables identified by univariate analysis with p < 0.05 were included in the multivariate logistic regression model were screw placement and TAD. The TAD was significant with an odds ratio (OR) 5.03 (p = 0.012) as the screw placement with an OR 4.35 (p = 0.043) in the anteroposterior view, and OR 10.61 (p = 0.037) in the lateral view. The TAD threshold value identified was 29.50 mm. Conclusion. Our study confirmed the risk factors for cut-out in the double-screw nail are comparable to those in the single screw. We found a TAD value of 29.50 mm to be associated with a risk of cut-out in double-screw nails, when good fracture reduction is granted. This value is higher than the one reported with single-screw nails. Therefore, we suggest the role of TAD should be reconsidered in well-reduced fractures treated with double-screw intramedullary nail. Cite this article: Bone Jt Open 2024;5(6):457–463

Aims

The localization of necrotic areas has been reported to impact the prognosis and treatment strategy for osteonecrosis of the femoral head (ONFH). Anteroposterior localization of the necrotic area after a femoral neck fracture (FNF) has not been properly investigated. We hypothesize that the change of the weight loading direction on the femoral head due to residual posterior tilt caused by malunited FNF may affect the location of ONFH. We investigate the relationship between the posterior tilt angle (PTA) and anteroposterior localization of osteonecrosis using lateral hip radiographs.

Methods

Patients aged younger than 55 years diagnosed with ONFH after FNF were retrospectively reviewed. Overall, 65 hips (38 males and 27 females; mean age 32.6 years (SD 12.2)) met the inclusion criteria. Patients with stage 1 or 4 ONFH, as per the Association Research Circulation Osseous classification, were excluded. The ratios of anterior and posterior viable areas and necrotic areas of the femoral head to the articular surface were calculated by setting the femoral head centre as the reference point. The PTA was measured using Palm’s method. The association between the PTA and viable or necrotic areas of the femoral head was assessed using Spearman’s rank correlation analysis (median PTA 6.0° (interquartile range 3 to 11.5)).

Acetabular fractures can occur due to either low or high-energy trauma, and treatment can consist of non-operative management, open reduction and internal fixation (ORIF), or total hip arthroplasty in either the acute or chronic setting. These decisions are often based on the age of the patient, the fracture pattern, and the existence of pre-fracture hip debility. In the acute setting, younger patients should undergo ORIF with anatomic reduction of the fracture, while total hip arthroplasty (THA) may be considered for elderly patients with pre-existing hip arthritis. Several factors can expedite the onset of post-traumatic arthritis in the former, including difficult fracture patterns, fractures that are intra-articular in nature, or fractures involving the femoral head. A meta-analysis of seven studies with 685 patients from all age groups reported the incidence of post-traumatic arthritis following satisfactory reduction of acetabular fractures (≤2 mm) to be 13.2%. Unsatisfactory reductions (>2 mm) increased the incidence of post-traumatic arthritis to 43.5%. Factors affecting the reduction quality include fracture type, fracture characteristics (e.g. comminution, impaction), time to surgery, and experience level of the operative team. In such settings, salvage THAs can be considered. However, complications including aseptic loosening, instability, and periprosthetic infection are more common than for other indications leading to THA. In our experience, at 20 years, we found that THAs performed after operatively treated acetabular fractures still had excellent hip function, and a 70% survivorship free of aseptic acetabular revision. A more recent study of 30 primary THAs performed with highly porous acetabular components indicated excellent results as well. As such, if early complications can be avoided, patients can expect substantial pain relief and excellent durability

Aims

Despite limited clinical scientific backing, an additional trochanteric stabilizing plate (TSP) has been advocated when treating unstable trochanteric fractures with a sliding hip screw (SHS). We aimed to explore whether the TSP would result in less post operative fracture motion, compared to SHS alone.

Methods

Overall, 31 patients with AO/OTA 31-A2 trochanteric fractures were randomized to either a SHS alone or a SHS with an additional TSP. To compare postoperative fracture motion, radiostereometric analysis (RSA) was performed before and after weightbearing, and then at four, eight, 12, 26, and 52 weeks. With the “after weightbearing” images as baseline, we calculated translations and rotations, including shortening and medialization of the femoral shaft.

Purpose: To compare the accuracy of post-operative plain radiographs versus computed tomography (CT) scans for the assessment of acetabular fracture reduction. Method: A retrospective assessment of sixty-four fractures in sixty-two patients was performed independently by three orthopedic trauma surgeons. Pre-operative CT scans and three plain radiographs (one anteroposterior pelvis and two Judet views) were used to classify the fracture pattern and measure pre-operative articular step and gap. Post-operative reduction quality was assessed using three plain radiographs and an axial CT assessing for step, gap, intra-articular hardware or fragments and necessity to re-operate. Results: Fracture patterns were as follows; posterior wall (n=10), posterior column (n=1), anterior wall (n=4), anterior column (n=1), transverse (n=12), posterior column posterior wall (n=4), transverse posterior wall (n=8), T-type (n=6), anterior column posterior hemi-transverse (n=5) and associated both column (n=11). Pre-operatively, the average step and gap on plain radiographs was 8.7 mm (±SD) and 15.3 mm (±SD). Post-operatively, the average step and gap was 0.6 mm (SD) and 0.9 mm (SD) based on 46 patients. The assessment was not possible in 18 patients due to overlying hardware. Using plain radiographs, one patient was found to have an inadequate reduction (> 2mm step and/or > 3mm gap). Post-operative measurement of step and gap by CT scan were 1.2 mm (SD) and 2.3 mm (SD) respectively. Using CT scans, eight patients were found to have either an inadequate reduction, intra-articular hardware or retained fragments. Computed tomography demonstrated 2 times more step and gap compared to plain radiographs. Conclusion: Post-operative CT was found to be more sensitive than plain radiographs to assess the quality of acetabular fracture reduction. Plain radiographs detected only 1 out of 8 cases where further operative intervention may have been beneficial. Given the consequences of missing an unacceptable reduction, intra-articular hardware, or retained intra-articular fragments, it is recommended that all fractures should be assessed postoperatively with CT unless the patient is not a candidate for further surgery for reasons independent of reduction quality. The benefits of post-operative CT imaging in acetabular fractures likely outweigh the cost and radiation exposure associated with its use

Several preoperative planning tools in computer-assisted surgery in acetabular fractures have been proposed. Moreover, all these preoperative planning tools are based on geometrical repositioning with their own limitations. The aim of this study was to evaluate the value of our prototype virtual planning tool using a rigid biomechanical model to predict failure in fracture reduction. Between November of 2015 and June of 2016, 10 patients were operated by the main author for acetabular fracture in our institution. To validate our biomechanical model planning tool, biomechanical simulation was performed for each patient immediately after the surgery. Reduction quality was assessed on post-operative CT scans. A 3D model of the acetabular fracture was build out of the CT images using the non-commercial software Itksnap. Then a biomechanical model implemented within the non-commercial Artisynth framework was used to perform virtual reduction. Surgical approach and surgical strategy according to the operative report were simulated. The simulated reductions and the surgical reductions were compared. The same reductions were obtained during surgery and biomechanical simulation in the 10 cases. For 7 cases, reduction was achieved by anterior surgical approach and so was the simulation. For 3 cases, reduction was achieved by posterior surgical approach and so was the simulation. The biomechanical simulation found similar results using the same surgical strategy with 9 anatomical reductions (90%) and one imperfect reduction (10%). The mean duration to perform acetabular planning surgery was 24 +/− 9 min [16–38]. Our virtual planning tool using a rigid biomechanical model can predict success or failure in fracture reduction according to the surgical approach and the surgical strategy

Metal on metal hip replacements have been one of worst failures in recent years in terms of orthopaedic implants. Some of these devices have had catastrophic failure rates, with reports of 48% failure at 6 years. The failure of these devices has led to considerable suffering, pain and reduction in quality of life; consequently, they have given rise to high costs and multi-million-dollar legal cases. This talk will describe the history of the current metal on metal failure and discuss some of the reasons why might have occurred. It will also consider the reasons that wear debris arising from the trunnion is worse in terms of biological activity then that arising from the bearing surfaces

Aims

The study objective was to prospectively assess clinical outcomes for a pilot cohort of tibial shaft fractures treated with a new tibial nailing system that produces controlled axial interfragmentary micromotion. The hypothesis was that axial micromotion enhances fracture healing compared to static interlocking.

Methods

Patients were treated in a single level I trauma centre over a 2.5-year period. Group allocation was not randomized; both the micromotion nail and standard-of-care static locking nails (control group) were commercially available and selected at the discretion of the treating surgeons. Injury risk levels were quantified using the Nonunion Risk Determination (NURD) score. Radiological healing was assessed until 24 weeks or clinical union. Low-dose CT scans were acquired at 12 weeks and virtual mechanical testing was performed to objectively assess structural bone healing.

Purpose: SWISSspine is a national health technology assessment (HTA) registry for ballon kyphoplasty (BKP) and total disc arthroplasty (TDA). It was initiated to evaluate the safety, efficiency and cost-effectiveness of these products. It is the first mandatory orthopaedic HTA registry of its kind in Switzerland. The aim is to generate evidence for a decision about reimbursement by the Swiss basic health insurance. Methods: During the time between March 2005 until June 2008, 331 interventions with 391 treated levels in 320 patients have been documented. Data was collected in a prospective, observational multicenter mode. The preliminary timeframe for the registry is three years. Data collection is performed preoperative, at 6 weeks, 3 months, 6 months and 1 year followup as well as annually thereafter. Surgery and followup case report forms are administered by spinal surgeons. Comorbidity questionnaires, NASS and EQ-5D forms are completed by patients. Results: Significant and clinically relevant reduction of back pain, improvement of quality of life and reduction of pain killer consumption was seen in the 1 year followup. Improvement of preoperative segmental kyhposis was achieved in the majority of cases. There were three symptomatic cement extrusions with radiculopathy documented. Conclusion: BKP is a safe and effective treatment concerning pain reduction, quality of life improvement and pain killer consumption. The SWISSspine registry proved to be an excellent tool for evaluation of this new technology and its results lead to a permanent coverage of BKP by the basic health insurance

Aims: Published series of traditional plate þxation of the femur note rates of up to 69% primary bone grafting, 13% infection, 15% nonunion, 68% late complications, and 25% secondary surgical procedures. A shift from traditional plating to submuscular plating has thus ensued. This series entails an all-inclusive review of the plate þxation experience by two orthopaedic trauma surgeons in a þve-year period at a university trauma center. Methods: Between June 1996 and May 2001, 40 acute diaphyseal femoral fractures in 37 patients were managed utilizing dynamic compression plating via traditional Ç biological È plating with a formal lateral incision (n = 19), or submuscular plate þxation utilizing only a proximal and distal incision (n = 21). A comparison of reduction quality, union rates, and infection rates between traditional plate þxation and submuscular þxation was performed. Results: Thirty-nine of forty femoral shaft fractures healed without need of a secondary procedure. One nonunion occurred in the ORIF group. There was one infection in each group. Two patients treated by submuscular þxation developed signiþcant heterotopic ossiþcation around the femoral shaft which signiþcantly impaired knee motion. There were 6 cases of malreduction in the submuscular group, and none in the ORIF Group. Conclusions: Although the theoretical advantages of submuscular plating are well established, its utilization in the femoral shaft did not have a clear clinical advantage. In addition, its use appears to be more technically challenging, and is associated with a high rate of sub-optimal reductions

Recent studies have assessed operative skill in surgical trainees “objectively” based on patient outcomes by attempting to statistically separate many contributory variables. Compression hip screw fixation (CHS) for neck of femur fracture (#NOF) is a standard operation commonly performed by orthopaedic trainees of varying experience. Our aim was to determine if trainees could be assessed objectively on their efficiency and aptitude in performance of this operation. A secondary aim was to evaluate the predictors of fixation failure for CHS described in the literature. Records and radiographs for all CHS performed by trainees of all levels for acute adult #NOF were examined retrospectively for 2 calendar years. Preoperative patient and fracture variables were scored. Outcome measures included operative time, scores of accuracy of fracture reduction and fixation, blood loss and complications. Failure of fixation was compared to the scores given to radiographs. Multivariate analysis was used to apportion variance between multiple contributing factors. Three hundred and eight two eligible operations were performed by 26 trainees. Operative time was effected by fracture complexity, trainee level and trainee operator (all p< 0.05). “Tip apex distance”, a measure of depth and centrality of screw placement in the femoral head, known to predict screw cut out was associated with trainee operator. Other outcome scores of fixation on radiographs were not correlated with fracture, patient or operator variables. Blood loss and complications were not associated with operator. The rate of failure of fixation was low and associated with scores of reduction quality only (p< 0.05). Trainees of variable experience perform CHS with a low overall complication rate and the most noticeable performance difference seems to be in speed of surgery

Introduction: Besides having various health consequences for the patients, post-operative wound infections following orthopaedic surgery increase health costs, often markedly. Optimal estimates of such costs require accurate incidence data over a prolonged period. The test group: Five hundreds and eighty consecutive patients undergoing orthopaedic surgery during the period May 1st 1998 to December 31st 1999 were studied. They underwent either of the following procedures: total primary hip replacement; total primary knee replacement; operations for trochanter-major fracture (using Hip Compression Screw); ankle fractures. Methods: The basic cost of the 580 operations was estimated from the aggregated costs of diagnosed related groups (DRG)-costs, applying only major codes. Patients who had to be re-admitted later because of wound infections during 1999 were studied and their secondary, wound-infection related costs was estimated from the aggregated DRG-costs during these hospitalisations. Results: A total of 47 of the 580 patients had a diagnosis of a post-operative wound infection, 14 were deep and 33 were superficial infections; 15 were readmitted for their infections. The additional days of hospitalisation for these re-admissions totaled 270 hospital days with an aggregated cost of 230.000.- Euro, compared to 3.000.000.- Euro for all 580 operations (fig. l shows graphically expenses caused by post operative wound infections as a fraction of the total cost). Discussion: Additional hospital costs related to postoperative wound infections amounted to 7% of the total hospital costs over a period of 20 months. Additional expenses not counted are those related to insurance compensations, sick leave compensations, expenses related to doctor visits and visits to out patient departments besides expenses related to additional nursing at home. Added to strict economic calculations are less easily calculable, short or prolonged reductions in quality of life. It is emphasized that post-operative wound infections after orthopaedic surgery represent a major cause for additional health expenses and reductions in quality of life

Introduction and objectives. Intramedullary nailing is indicated to stabilization of tibia shaft fractures. Intramedullary nailing through an infra-patellar incision is commonly the technique of choice. While intramedullary nailing of simple diaphyseal fracture patterns is relatively easy, proximal tibia fractures, extremely comminuted/segmental tibia fractures, politrauma with multiple fractures in both extremities and reconstruction of bone loss segment with stiffness of the knee joint can be very challenging to treat. A novel technique for intramedullary tibia nailing through the patella-femoral joint is described. This technique allow extension tibia during intervention time and it supplies easier reduction of the pattern of fracture above. The purpose of our investigation was to evaluate the use of this new technique in described above pattern fracture and patient situation; because we have thought that new technique can perform better outcomes in this situations. Materials and Methods. An observational study of tibia fractures or bone defect was performed for consecutive patients who presented: proximal tibia fractures, extremely comminuted/segmental tibia fractures, politrauma with multiple fractures in both extremities and reconstruction of bone loss segment in the Trauma unit of our institution from September 2009 to August 2010. A total of 32 were included in our study, which performed surgery intervention with Trigen tibia nail (Smith & Nephew, Memphis) with suprapatelar device. Demographic data, mechanism of injury, fracture classification, ROM (2 and 6 weeks, and 3 months), consolidation rate, reduction fracture quality and knee pain at 3 months were recorded. Results. Male was the gender most frequent (64%), the average age was 39,5 years and the main mechanism of injury was motor vehicle injury (30,6%). There were 7 proximal fractures, 1 bifocal fracture, 6 politrauma and 2 reconstruction bone loss segment, the others were comminuted and segmental tibia fracture. In all the patients, the fracture was consolidated between 8 to 14 weeks with average of 10,6 weeks. The quality of reduction was correct in all patients in different axis. The average of ROM at 2 weeks was (−2°/95°) at 3 months was (−0,4°/133°). The knee pain (VAS) average was 0,6 only in 2 patients were (4). Poller screw was used in 7 patients. Conclusion. The suprapatelar nailing is optimal technique to resolve complex fracture of the tibia (proximal, segmental conminution, politrauma) that perform correct consolidation rate without misalignment in the patients. Concerns about knee pain (VAS) from the technique not appear in our data. Prospective and clinical trials are needed to validate this approach

Introduction The decision to offer a patient a knee replacement is usually based on a patient’s perception of their reduction of quality of life. The choice between unicompartmental (UCKJR) and total knee joint replacement (TKJR) is usually guided by surgeon preference. This preference has been extensively debated in the literature. Methods The author has a series of 130 UCKJR and 370 TKJR performed over similar time spans. Oxford knee scores have been collected comparing outcomes. Radiological signs of loosening or subsidence have been recorded. Results Short term function is superior in terms of range of motion and capacity for activities of daily living in those with UCKJR compared to TKJR. Three UCKJR and 12 TKJR have been revised. Of the three revisions of UCKJR two have been revised for pain of uncertain cause and one was loose. Three UCKJR are unsatisfactory and will come to revision. Two are showing signs of subsidence and one is painful, presumed loose without radiological signs of looseness. Conclusions Comparing revision rates from one surgeon’s series suggests a slightly higher rate of revision with UCKJR but since prosthesis selection is made on different criteria when making the original decision to proceed, comparisons are difficult. A 95% survival figure can be given to patients about the result of this procedure. The functional outcome, earlier recovery, and shorter hospitalisation can be given as other factors to favor the procedure. The literature supports the satisfactory results of conversion of UCKJR to TKJR. A confounding argument is the frequent reporting of 98% to 100% survival of TKJR at 15 years
