In pre-operative planning for total hip arthroplasty (THA), femoral offset (FO) is frequently underestimated on AP pelvis radiographs as a result of inaccurate patient positioning, imprecise magnification, and radiographic beam divergence. The aim of the present study was to evaluate the reliability and accuracy of predicting three-dimensional (3-D) FO as measured on computed tomography (CT) from measurements performed on standardised AP pelvis radiographs. In a retrospective cohort study, pre-operative AP pelvis radiographs and corresponding CT scans of a consecutive series of 345 patients (345 hips, 146 males, 199 females, mean age 60 (range: 40-79) years, mean body-mass-index 27 (range: 29-57) kg/m2) with primary end-stage hip osteoarthritis were reviewed. Patients were positioned according to a standardised protocol and all images were calibrated. Using validated custom programmes, FO was measured on corresponding AP pelvis radiographs and CT scans. Inter- and intra-observer reliability of the measurement methods were evaluated using intra-class correlation coefficients (ICC). To predict 3-D FO from AP pelvis measurements, the entire cohort was randomly split in two groups and gender specific linear regression equations were derived from a subgroup of 250 patients (group A). The accuracy of the derived prediction equations was subsequently assessed in a second subgroup of 100 patients (group B). In the entire cohort, mean FO was 39.2mm (95%CI: 38.5-40.0mm) on AP pelvis radiographs and 44.6mm (95%CI: 44.0-45.2mm) on CT scans. FO was underestimated by 14% on AP pelvis radiographs compared to CT (5.4mm, 95%CI: 4.8-6.0mm, p<0.001) and both parameters demonstrated a linear correlation (r=0.642, p<0.001). In group B, we observed no significant difference between gender specific predicted FO (males: 48.0mm, 95%CI: 47.1-48.8mm; females: 42.0mm, 95%CI: 41.1-42.8mm) and FO as measured on CT (males: 47.7mm, 95%CI: 46.1-49.4mm, p=0.689; females: 41.6mm, 95%CI: 40.3-43.0mm, p=0.607). The results of the present study suggest that femoral offset can be accurately and reliably predicted from AP pelvis radiographs in patients with primary end-stage hip osteoarthritis. Our findings support the surgeon in pre-operative templating and may improve offset and limb length restoration in THA without the routine performance of CT

Aims. This study aimed to investigate the incidence of ≥ 5 mm asymmetry in lower and whole leg lengths (LLs) in patients with unilateral osteoarthritis (OA) secondary to developmental dysplasia of the hip (DDH-OA) and primary hip osteoarthritis (PHOA), and the relationship between lower and whole LL asymmetries and femoral length asymmetry. Methods. In total, 116 patients who underwent unilateral total hip arthroplasty were included in this study. Of these, 93 had DDH-OA and 23 had PHOA. Patients with DDH-OA were categorized into three groups: Crowe grade I, II/III, and IV. Anatomical femoral length, femoral length greater trochanter (GT), femoral length lesser trochanter (LT), tibial length, foot height, lower LL, and whole LL were evaluated using preoperative CT data of the whole leg in the supine position. Asymmetry was evaluated in the Crowe I, II/III, IV, and PHOA groups. Results. The incidences of whole and lower LL asymmetries were 40%, 62.5%, 66.7%, and 26.1%, and 21.7%, 20.8%, 55.6%, and 8.7% in the Crowe I, II/III, and IV, and PHOA groups, respectively. The incidence of tibial length asymmetry was significantly higher in the Crowe IV group (44.4%) than that in the PHOA group (4.4%). In all, 50% of patients with DDH-OA with femoral length GT and LT asymmetries had lower LL asymmetry, and 75% had whole LL asymmetry. The incidences of lower and whole LL asymmetries were 20% and 42.9%, respectively, even in the absence of femoral length GT and LT asymmetries. Conclusion. Overall, 43% of patients with unilateral DDH-OA without femoral length asymmetry had whole LL asymmetry of ≥ 5 mm. Thus, both the femur length and whole LL should be measured to accurately assess LL discrepancy in patients with unilateral DDH-OA. Cite this article: Bone Jt Open 2024;5(2):79–86

Purpose: The primary objective of this study was to determine if surgical treatment for spinal stenosis is comparable to total hip arthroplasty in improving patients’ self-reported quality of life.

Methods: An age, sex and time of surgery matched cohort of patients who had undergone elective primary 1–2 level spinal decompression (n=90) with (n=30/90) or without fusion for spinal stenosis (n=40 with degenerative spondylolisthesis) and elective primary total hip arthroplasty for osteoarthritis (n=90) were compared. The primary outcome measure was the preoperative and one year postoperative Medical Outcomes Study Short Form-36 (SF-36) questionnaire.

Results: The mean Physical Component Summary (PCS) / Mental Component Summary (MCS) for the stenosis compared to hip patients were 32.0/43.3 vs. 30.5/46.2 preoperatively (analysis between groups: p = 0.2/0.1) and 39.1/47.3 vs.44.1/46.1 postoperatively (analysis between groups: p = 0.003/0.4). The pre- and postoperative PCS significantly improved for both groups (p < 0.0001);however, the pre- and postoperative MCS improved in the stenosis group only (p = 0.04).

Conclusions: Studies have shown the significant impact on overall patient quality of life and cost-effectiveness of primary total hip arthroplasty. The results of this unique study show that surgical intervention for spinal stenosis also has a very positive overall effect on patients’ self-reported quality of life at one year follow up. This study provides data that supports the need for a long term prospective study and advocacy regarding waiting time initiatives and surgical resources for the treatment of patients with symptomatic spinal stenosis with a similar demographic to those with primary OA of the hip.

In uncemented total hip arthroplasty (THA), the optimal femoral component should allow both maximum cortical contact with proximal load transfer and accurate restoration of individual joint biomechanics. This is often compromised due to a high variability in proximal femoral anatomy. The aim of this on-going study is to assess the variation in proximal femoral canal shape and its association with geometric and anthropometric parameters in primary hip OA.

In a retrospective cohort study, AP-pelvis radiographs of 98 consecutive patients (42 males, 56 females, mean age 61 (range:45-74) years, BMI 27.4 (range:20.3-44.6) kg/m2) who underwent THA for primary hip OA were reviewed. All radiographs were calibrated and femoral offset (FO) and neck-shaft-angle (NSA) were measured using a validated custom programme. Point-based active shape modelling (ASM) was performed to assess the shape of the inner cortex of the proximal femoral meta- and diaphysis. Independent shape modes were identified using principal component analysis (PCA). Hierarchical cluster analysis of the shape modes was performed to identify natural groupings of patients. Differences in geometric measures of the proximal femur (FO, NSA) and demographic parameters (age, height, weight, BMI) between the clusters were evaluated using Kruskal-Wallis one-way-ANOVA or Chi-square tests, as appropriate.

In the entire cohort, mean FO was 39.0 mm, mean NSA was 131 degrees. PCA identified 10 independent shape modes accounting for over 90% of variation in proximal femoral canal shape within the dataset. Cluster Analysis revealed 6 shape clusters for which all 10 shape modes demonstrated a significantly different distribution (p-range:0.000-0.015). We observed significant differences in age (p=0.032), FO (p<0.001) and NSA (p<0.001) between the clusters. No significant differences with regard to gender or BMI were seen.

Our preliminary analysis has identified 6 different patterns of proximal femoral canal shape which are associated with significant differences in femoral offset, neck-shaft-angle and age at time of surgery. We are currently evaluating the entire dataset of 345 patients which will allow a comprehensive classification of variation in proximal femoral shape and joint geometry. The present data may optimise preoperative planning and improve future implant design in THA.

Aims. The Exeter short stem was designed for patients with Dorr type A femora and short-term results are promising. The aim of this study was to evaluate the minimum five-year stem migration pattern of Exeter short stems in comparison with Exeter standard stems. Methods. In this case-control study, 25 patients (22 female) at mean age of 78 years (70 to 89) received cemented Exeter short stem (case group). Cases were selected based on Dorr type A femora and matched first by Dorr type A and then age to a control cohort of 21 patients (11 female) at mean age of 74 years (70 to 89) who received with cemented Exeter standard stems (control group). Preoperatively, all patients had primary hip osteoarthritis and no osteoporosis as confirmed by dual X-ray absorptiometry scanning. Patients were followed with radiostereometry for evaluation of stem migration (primary endpoint), evaluation of cement quality, and Oxford Hip Score. Measurements were taken preoperatively, and at three, 12, and 24 months and a minimum five-year follow-up. Results. At three months, subsidence of the short stem -0.87 mm (95% confidence interval (CI) -1.07 to -0.67) was lower compared to the standard stem -1.59 mm (95% CI -1.82 to -1.36; p < 0.001). Both stems continued a similar pattern of subsidence until five-year follow-up. At five-year follow-up, the short stem had subsided mean -1.67 mm (95% CI -1.98 to -1.36) compared to mean -2.67 mm (95% CI -3.03 to -2.32) for the standard stem (p < 0.001). Subsidence was not influenced by preoperative bone quality (osteopenia vs normal) or cement mantle thickness. Conclusion. The standard Exeter stem had more early subsidence compared with the short Exeter stem in patients with Dorr type A femora, but thereafter a similar migration pattern of subsidence until minimum five years follow-up. Both the standard and the short Exeter stems subside. The standard stem subsides more compared to the short stem in Dorr type A femurs. Subsidence of the Exeter stems was not affected by cement mantle thickness. Cite this article: Bone Jt Open 2023;4(7):507–515

Evidence is accumulating for the role of bone in the pathogenesis of osteoarthritis (OA). Previous studies have shown a generalised increase in bone mass and hypo-mineralisation in OA patients. However, the molecular and cellular mechanisms involved in the increased bone mass and matrix compositional profiles in OA, at distal skeletal sites to the articular cartilage, have not yet been well defined. This study examined whether gene expression of bone anabolic factors, trabecular bone architecture and matrix mineralisation are altered in human OA and non-OA hipbone. Intertrochanteric (IT) trabecular bone samples were obtained from 15 primary hip OA patients (mean age 65 [48–85] years) and 13 closely age- and gender-matched autopsy controls (mean age 63 [44–83] years). Semi-quantitative RT-PCR analysis revealed elevated mRNA expression levels of alkaline phosphatase (p < 0.002), osteocalcin (p < 0.0001), osteopontin (p < 0.05), collagen type-I α chains COL1A1 (p < 0.0001) and COL1A2 (p < 0.002), in OA bone compared to control, suggesting possible increases in osteoblastic biosynthetic activity and/or bone turnover at the IT region in OA. Interestingly, the ratio of COL1A1:COL1A2 mRNA was almost 2-fold greater in OA bone compared to control (p < 0.001), suggesting the potential presence of collagen type-I homotrimer at the distal site that may associate with hypomineralisation in OA individuals. Using a quantitative backscatter electron imaging technique, mineralisation profiles of IT trabecular bone indicated decreased mineralisation in the OA group compared to the control group (24.2 weight percent calcium [wt%Ca] versus 25.3 wt%Ca). Bone histomorphometric analysis found OA IT bone had increased surface density of bone and decreased trabecular separation compared to control bone. Taken together with a reported increase in diffuse microdamage in OA IT bone (Fazzalari et al. Bone 31:697–702, 2002), possibly due to hypomineralisation, these results are consistent with the altered bone material properties found in OA individuals. The finding of differential gene expression, altered mineralisation and architectural changes in OA bone, at a skeletal site distal to the active site of joint degeneration, supports the concept of systemic involvement of bone in the pathogenesis of OA.

The early revision rate in elective Total Hip Arthroplasty (THA) three years after surgery in elderly patients over 80 years is significantly lower for cemented stems in the German Arthroplasty Register (EPRD): cemented 3,1% (3.0 – 3.2) vs. uncemented 4.2% (4.1 – 4.3; p < 0.001). However, the mortality rate in elderly patients is elevated for cemented fixation. This study presents a detailed analysis of the influence of stem type and fixation on revision and mortality rate in this patient cohort. Elective primary THA cases for primary Coxarthrosis using uncemented cups from the EPRD data base were analysed (n. 0. = 37,183). Four stem type groups were compared: cementless, cementless with collar, cementless short, and cemented. Stems with at least 300 cases at risk three years after surgery were analysed individually. The reference stem was determined as the stem with the lowest revision rate and at least 1000 cases under surveillance 3 years after surgery (n. 3. = 28,637). The revision rate for cemented stems (2.5% [2.2–1.81] was lower than for uncemented (4.5% [4.2–4.9]; p<0.001) and uncemented short stems (4.2% [3.1–5.7]; p=0.002). The revision rate of uncemented collared stems (2.3% [1.5–3.6]) was similar to cemented stems (p=0.89) and lower than for uncemented stems (p=0.02). One year mortality showed no sig. differences between the groups (p>0.17): cemented 3.2% [2.9–3.6], uncemented 3.4% [3.1–3.7], uncemented short 3.5% [2.5–4.9], uncemented collar 2.0% [1.2–3.2]. “Cementless” and “cementless short” stems should not be used in patients over 80 years due to the higher revision risk. If cementing should be avoided, “cementless collared” stems seem to be a good alternative combined with a tendency for a lower one year mortality rate

Psoas tendinopathy is a potential cause of groin pain after primary total hip arthroplasty (THA). The direct anterior approach (DAA) is becoming increasingly popular as the standard approach for primary THA due to being a muscle preserving technique. It is unclear what the prevalence is for the development of psoas-related pain after DAA THA, how this can influence patient reported outcome, and which risk factors can be identified. This retrospective case control study of prospectively recorded data evaluated 1784 patients who underwent 2087 primary DAA THA procedures between January 2017 and September 2019. Psoas tendinopathy was defined as (1) persistence of groin pain after DAA THA and was triggered by active hip flexion, (2) exclusion of other causes such as dislocation, infection, implant loosening or (occult) fractures, and (3) a positive response to an image-guided injection with xylocaine and steroid into the psoas tendon sheath. Complication-, re-operation rates, and patient-reported outcome measures (PROMs) were measured. Forty-three patients (45 hips; 2.2%) were diagnosed with psoas tendinopathy according to the above-described criteria. The mean age of patients who developed psoas tendinopathy was 50.8±11.7 years, which was significantly lower than the mean age of patients without psoas pain (62.4±12.7y; p<0.001). Patients with primary hip osteoarthritis were significantly less likely to develop psoas tendinopathy (14/1207; 1.2%) in comparison to patients with secondary hip osteoarthritis to dysplasia (18/501; 3.6%) (p<0.001) or FAI (12/305; 3.9%) (p<0.001). Patients with psoas tendinopathy had significantly lower PROM scores at 6 weeks and 1 year follow-up. Psoas tendinopathy was present in 2.2% after DAA THA. Younger age and secondary osteoarthritis due to dysplasia or FAI were risk factors for the development of psoas tendinopathy. Post-operatively, patients with psoas tendinopathy often also presented with low back pain and lateral trochanteric pain. Psoas tendinopathy had an important influence on the evolution of PROM scores

Objectives. T-cells are considered to play an important role in the inflammatory response causing arthroplasty failure. The study objectives were to investigate the composition and distribution of CD4+ T-cell phenotypes in the peripheral blood (PB) and synovial fluid (SF) of patients undergoing revision surgery for failed metal-on-metal (MoM) and metal-on-polyethylene (MoP) hip arthroplasties, and in patients awaiting total hip arthroplasty. Methods. In this prospective case-control study, PB and SF were obtained from 22 patients (23 hips) undergoing revision of MoM (n = 14) and MoP (n = 9) hip arthroplasties, with eight controls provided from primary hip osteoarthritis cases awaiting arthroplasty. Lymphocyte subtypes in samples were analysed using flow cytometry. Results. The percentages of CD4+ T-cell subtypes in PB were not different between groups. The CD4+ T-cells in the SF of MoM hips showed a completely different distribution of phenotypes compared with that found in the PB in the same patients, including significantly decreased CD4+ T-central memory cells (p < 0.05) and increased T-effector memory cells (p < 0.0001) in the SF. Inducible co-stimulator (ICOS) was the only co-stimulatory molecule with different expression on CD4+ CD28+ cells between groups. In PB, ICOS expression was increased in MoM (p < 0.001) and MoP (p < 0.05) cases compared with the controls. In SF, ICOS expression was increased in MoM hips compared with MoP hips (p < 0.05). Conclusions. Increased expression of ICOS on CD4+ T-cells in PB and SF of patients with failed arthroplasties suggests that these cells are activated and involved in generating immune responses. Variations in ICOS expression between MoM and MoP hips may indicate different modes of arthroplasty failure. Cite this article: Professor P. A. Revell. Introduction & aims. The magnitude and pattern of acceptable migration in clinically successful cementless stems is not well understood. Radiostereometric analysis (RSA) is a well-recognised method of assessing implant migration. Previous studies have reported long term RSA results for cemented stems. The aim of this study was to assess the long-term migration characteristics of the Corail hydroxyapatite-coated cementless stem at 10 years using RSA. Method. A prospective cohort of 30 patients undergoing primary total hip arthroplasty for primary hip osteoarthritis were enrolled into a study to characterise the migration behaviour of a cementless stem. Tantalum markers were attached to the stem and placed in the bone intraoperatively, allowing for RSA measurements to be taken in vivo. Previous 5-year results have been presented. A total of 14 patients (total 15 hips, one bilateral) with mean age 82 years (range, 69–92 years) underwent repeat long-term RSA radiographs at minimum 10 years post op. The mean time to follow up was 13.9 years (range, 13.3–14.4 years). The RSA radiographs were analysed to assess for implant rotation and translation. Results. None of the 14 patients (15 hips) followed up have been revised. The migration (rotation and translation) at 6 months, 1 year, 2 years, and 6 years has been described previously. No stems had additional subsidence of more than 0.25 mm between 6 months and 6 years. The resultant mean subsidence between 2 years and 6 years was 0.03 mm, which is below the limit measurable by RSA. The long-term, 10-year results, on implant rotation and translation will be presented. Conclusions. The Corail stem exhibited variable subsidence within the first 6 months after which there is persistent stabilisation through to 6 years. 10-year results on long-term migration show no further migration. These results can be used as a gold standard for other uncemented stems

Purpose. The aim of this study is to describe the influence of sitting and standing posture on sagittal pelvic inclination in preoperative total hip replacement patients to assist with correct acetabular component positioning. Methods. Lateral radiographs of the pelvis and lumbar spine in sitting and standing of preoperative hip arthroplasty patients with primary hip osteoarthritis were extracted. Pelvic tilt was measured using the vertical inclination of a line from the ASIS to pubic tubercle. Sacral inclination was measured as the angle between the anterior surface of the sacrum and a horizontal reference. Figure one is a representation of the pelvic tilt angle and sacrel inclination angle taken during standing. The Cobb angle of the lumbar spine was recorded represented for a sitting patient in figure 2. Hip flexion was recorded (figure 2). Results. 60 patients were identified. Mean age of the cohort was 63. Sacral inclination ranged from 1 to 55 degrees in standing with a mean of 25.7 degrees. In sitting, sacral inclination ranged from 0.3–84.5 degrees with a mean of 24.1 degrees. Pelvic tilt ranged from 30 degrees posteriorly to 21.5 degrees anteriorly in standing. Pelvic tilt in sitting and ranged from 48 posterior to 42 degrees anterior tiltLumbar lordosis ranged from 11.6 to 91.7 degrees in standing. Lumbar lordosis in sitting ranged from 29.5 degrees(kyphosis) to 42 degrees (lordosis). Total hip flexion was 107.4 degrees from standing to sitting. Conclusions. There is wide variability in pelvic orientation between individuals in both postures Orientating acetabular components for total hip arthroplasty should account for postural changes in native version

Aims

Transfusion after primary total hip arthroplasty (THA) has become rare, and identification of causative factors allows preventive measures. The aim of this study was to determine patient-specific factors that increase the risk of needing a blood transfusion.

Methods

All patients who underwent elective THA were analyzed retrospectively in this single-centre study from 2020 to 2021. A total of 2,892 patients were included. Transfusion-related parameters were evaluated. A multiple logistic regression was performed to determine whether age, BMI, American Society of Anesthesiologists (ASA) grade, sex, or preoperative haemoglobin (Hb) could predict the need for transfusion within the examined patient population.

Introduction:. Direct anterior approach (DAA) total hip arthroplasty (THA) has been reported to be a muscle sparing approach. The purpose of this study was to compare gait patterns over time between patients undergoing THA via DAA and posterior approach (PA). Methods:. 22 patients with unilateral primary hip osteoarthritis were prospectively enrolled and gait analysis was performed prior to, at 6 months and 1 year following THA via DAA and PA. All PA THA's were performed by a single surgeon from January 2008 to February 2009; all DAA THA's were performed by the same surgeon at the same institution from January 2010 to May 2011 with similar design of uncemented acetabular, femoral components and bearing surfaces. Reflective markers were placed on the lower extremity and motion data collected using six infrared cameras (Qtrac, Qualysis). Ground reaction forces were recorded with a multicomponent force plate (Kistler). A repeated-measures ANOVA was used to compare changes in gait parameters over time. Harris Hip Score was used to quantify pain and function. Results:. There were 11 patients in both groups with similar age, sex and BMI distribution. Postoperatively, both groups demonstrated improvement in flexion/extension range of motion (ROM) (p = 0.006), peak flexion (p = 0.05) and extension moments (p = 0.004) with no differences between groups. Internal/external ROM improved significantly and was higher in DAA group as compared to PA group (p = 0.05). Gait velocity and single-leg stance time improved significantly in PA group (p = 0.001), but they were similar between groups postoperatively. Pain and function scores were also similar. Conclusions:. THA performed via DAA and PA offer similar improvement in gait parameters at 6 months and 1 year follow-up with the exception of internal/external ROM. This might be indicative of altered hip mechanics related to release and repair of external rotators during PA THA

Obesity is a risk factor for acetabular malposition when total hip arthroplasty (THA) is performed with manual orientation techniques. However, conflicting evidence exists regarding the usefulness of computer-assisted surgery for performing THA in obese patients. The purpose of this study was to compare the precision and accuracy of imageless navigation for acetabular component placement in obese versus non-obese patients. After institutional review board approval, 459 THA performed for primary hip osteoarthritis were reviewed retrospectively. The same imageless navigation system was used for acetabular component placement in all THA. During surgery the supine anterior pelvic plane was referenced superficially. THA was performed via posterolateral approach in the lateral position. A hemispherical acetabular component was used, with target inclination of 40° and target anteversion of 25°. Computer software was used to determine acetabular orientation on postoperative anteroposterior pelvic radiographs. Obese patients (BMI ≥ 30 kg/m2) were compared to non-obese patients. A 5° difference in mean orientation angles was considered clinically significant. Orientation error (accuracy) was defined as the absolute difference between the target orientation and the measured orientation. Student's t test was used to compare means. Hartley's test compared variances of the mean differences (precision). Fisher exact tests examined the relationship between obesity and component placement in the target zone (target ± 10°) for inclination and version. All statistical tests were two-sided with a significance level of 0.05. Differences in mean inclination and anteversion between obese and non-obese groups were 1.1° (p=0.02 and p=0.08, respectively), and not clinically significant. Inclination accuracy trended toward improvement for non-obese patients (p=0.06). Inclination precision was better for non-obese patients (p=0.006). Accuracy and precision for anteversion were equal between the two groups (p=0.19 and p=0.95, respectively). There was no relationship between obesity and placement of the acetabulum outside of the target ranges for inclination (p=0.13), anteversion (p=0.39) or both (p=0.99), with a trend toward more inclination outliers in obese patients versus non-obese patients (7.3% versus 3.9%). The observed differences in mean acetabular orientation angles were not clinically significant (< 5°), although inclination orientation was less accurate and precise for obese patients. In contrast to existing literature, we found no difference in the accuracy and precision with regard to anteversion in obese and non-obese patients. We propose that accurate superficial registration of landmarks in obese patients is achievable, and the use of imageless navigation likely improves acetabular positioning in obese and non-obese patients

Resurfacing arthroplasty is advantageous over conventional total hip arthroplasty in that femoral bone stock is preserved. However, there has been controversy over the preservation of acetabular bone stock in resurfacing arthroplasty, with the concern that it may result in excess reaming compared with total hip replacement. This is of concern as the prosthesis is primarily advocated in the young patient, who is likely to face future revision surgery. We prospectively identified a cohort of 68 patients with primary hip osteoarthritis undergoing conventional total hip arthroplasty. During surgery, the excised femoral head and neck diameter was measured, along with the diameter of the final acetabular reamer used to achieve a bed of bleeding cancellous bone. The measured neck diameter was then used to calculate the minimum possible resurfacing head and cup sizes, with corresponding final reamer sizes that could have been used in each patient without neck notching for both Birmingham Hip Resurfacing (BHR, Smith & Nephew, 3rd Generation) and Articular Surface replacement (ASR, De Puy, 4th Generation). Reaming diameter and volume was compared for all 3 groups. Mean reaming diameters for the THR, ASR and BHR groups were 51, 52 and 56mm respectively. Mean reaming volumes were 39, 40 and 47cc. There was a statistically significant difference between the THR and BHR groups for both reamed diameter and volume (p< 0.001). There was also a significant difference between the ASR and BHR groups for both reamed diameter and volume (p< 0.001). This difference was more pronounced with larger neck diameters. Our data shows that the BHR results in more ace-tabular bone loss compared to total hip replacement. An implant with a lower profile acetabular cup and a larger variety of sizes such as the ASR may allow better preservation of acetabular bone stock

Introduction: In postmenopausal female patients with hip osteoarthritis, osteoporosis as well as altered local trabecular bone architecture may lead to a increased migration of uncemented hip prostheses. 1. ,. 2. The aim of this study was to determine whether 3D bone architecture and mechanical properties of intertrochanteric cancellous bone in the proximal femur predict RSA migration of uncemented femoral stems. Materials and Methods: The study population consisted of 61 female patients with primary hip osteoarthritis. Informed consent was obtained prior to any study-related procedures. The Ethical Committee approved the study protocol. All patients underwent a total hip replacement (ABG II, Stryker) with uncemented proximally hydroxyapatite-coated femoral stem with tantalum RSA markers. Ceramicceramic bearing surfaces were used. A uniplanar type of RSA setup was applied. The RSA examinations were performed postoperatively and at 3, 6, and 12 months. During surgery, cancellous bone biopsy was taken from the proximal femur from the site of stem implantation. The specimens were scanned with micro-CT. 3D analysis of trabecular bone geometry and BMD was performed using CTAn software. After micro-CT imaging, the trochanteric cancellous bone specimens were subjected to a destructive compression test. Maximum force and stiffness were calculated. Linear regression analysis was applied to study correlations between different parameters investigated. Results: The patients exhibited major differences in the density and structural quality of intertrochanteric cancellous bone. Significant correlations were found between the measured biomechanical parameters and the structural parameters calculated from micro-CT data. Unexpectedly, the quality of intertrochanteric cancellous bone did not predict RSA migration of the femoral stems. The femoral stems reached high stability within 3 months and there were no significant differences in the axial and rotational migration of the femoral stems between the patients with normal or poor quality of the intertrochanteric cancellous bone. Discussion: The 3D bone architecture, mineral density and mechanical properties of the local intertrochanteric cancellous bone do not seem to serve as predictors of femoral stem migration. The observation suggests that the significance of cancellous bone quality for the initial stability of uncemented femoral stems has been over-emphasized

Although there are several reports of excellent long-term survival after cemented total hip arthroplasty (THA), cemented acetabular components are prone to become loose when compared with femoral components. On the other hand, the survival of cementless acetabular components has been reported to be equal or better than cemented ones and the use of cementless acetabular components is increasing. However, most of the reports on survival after THA are for patients with primary hip osteoarthritis (OA) and there is no report of 20-year survival of cementless THA for patients with hip dysplasia. It is supposed to be more difficult to fix cementless acetabular components for OA secondary to hip dysplasia than primary OA. The purposes of this study were to review retrospectively the 20-year survival of cemented and cementless THA for hip dysplasia and to compare the effect of fixation methods on the long-term survival for patients with hip dysplasia. We retrospectively reviewed all patients with OA secondary to hip dysplasia treated with a cemented Bioceram hip system between 1981 and 1987, and a cementless cancellous metal Lübeck hip system between 1987 and 1991. We excluded patients aged more than 60 years, males, and Crowe 4 hips. The studied subjects were 70 hips of cemented THA (Group-C) and 57 hips of cementless THA (Group-UC). Both hip implants had a 28-mm alumina head on polyethylene articulation. The mean age at operation was 50.5 years (range, 36–60 years) in Group-C and 50.0 years (range, 29–60 years) in Group-UC. The mean BMI was 23.2 kg/m. 2. in Group-C (range, 17.3–29.3 kg/m. 2. ) and 22.9 kg/m. 2. in Group-UC (range, 18.8–28.0 kg/m. 2. ). There were no significant differences in age and BMI between the two groups. The average follow-up period was 18.0 years in Group-C and 18.4 years in Group-UC. In Group-C, revision was performed in 33 hips due to aseptic cup loosening (30 hips), stem loosening (one hip), and loosening of both components (two hips). In Group-UC, revision was performed in 10 hips due to stem fracture secondary to distal fixation (4 hips), cup loosening (three hips), polyethylene breakage (two hips), and extensive osteolysis around the stem (one hip). The survival at 20 years regarding any revision as the endpoint was 51% in Group-C and 84% in Group-UC. This difference was significant using Log-rank test (P=0.006). The cup survival at 20 years was 54% in Group-C and 92% in Group-UC. This difference was also significant (P = 0.0003). The stem survival at 20 years was 95% in Group-C and 92% in Group-UC. This difference was not significant (P = 0.4826). Cementless THA showed a higher survival rate at 20 years for hip dysplasia than cemented THA because of the excellent survival of the acetabular component without cement. We conclude that cementless THA with the cancellous metal Lübeck hip system led to better longevity at 20 years than cemented THA with the Bioceram for patients with OA secondary to hip dysplasia

Introduction: Double mobility acetabular implant has a semicircular shape, it is covered with hydroxyapatite, the entire surface has a thickness of 3 mm and its centers of rotation are shifted. The polyethylene employed is a high density one and it has some chanfers to avoid the cam effect. Methods and materials: We present the results o an homogeneous series of 200 patients, operated between 2003 and 2007. Clinical and radiographic parameters were analyzed prospectively. The mean follow-up was done during 15 months. Clinical results were evaluated by the HHS at the out patients clinics: previous and post surgery. Results: From the 200 patients operated (130 women-70 men / Mean age: 81 years old): 57,63% had a primary hip osteoarthritis; 5,77 % femoral necrosis ; 1,13 % rheumatoid arthritis; 16,95 % revision surgeries; 13,45% femoral neck fractures; 3,2% acetabular fractures y 1,5 % hip tumours. HHS before surgery was 45, 83 points on average (from 12 to 79) y post surgery HHS was 80, 03 points (from 37 to 100), increasing the total score after the arthroplasty in a mean of 34, 17 points. Post surgery complications were as follow: 3 dislocations (1 after an enormous fall and 2 in patients with Alzheimer. In our series there are 50 patients diagnosed of dementia-Alzheimer); 1 per prosthetic fracture (revision surgery); 4 deep infections (2 acute: lavage+ antibiotherapy; 2 late ones: spacer + antibiotherapy +second time surgery); 2 Deep vein thromboses (Eco Doppler +) ; 10 urinary infections; 2 urinary retentions and 17 deaths. Discussion: Double Mobility acetabular implant has shown good results in all the following indications: Revision surgery, hip osteoarthritis, femoral necrosis, Rheumatoid arthritis, femoral neck and acetabular fractures, hip tumours and as an implant for Computer Assisted Hip Surgery. Conclusions: The complications founded while this acetabular implant is used appeared with the same percentage than others. The dislocation rate is lower than standard acetabular implants, especially in patients with neuromuscular or cognitive illnesses. Those clinical results are hopeful and they could increase the number of actual indications (hip osteoarthritis in people over 70 years old, multiple illnesses associated, iterative dislocations…) for the double mobility implant on the future

Aims

Hip arthroplasty aims to accurately recreate joint biomechanics. Considerable attention has been paid to vertical and horizontal offset, but femoral head centre in the anteroposterior (AP) plane has received little attention. This study investigates the accuracy of restoration of joint centre of rotation in the AP plane.

Methods

Postoperative CT scans of 40 patients who underwent unilateral uncemented total hip arthroplasty were analyzed. Anteroposterior offset (APO) and femoral anteversion were measured on both the operated and non-operated sides. Sagittal tilt of the femoral stem was also measured. APO measured on axial slices was defined as the perpendicular distance between a line drawn from the anterior most point of the proximal femur (anterior reference line) to the centre of the femoral head. The anterior reference line was made parallel to the posterior condylar axis of the knee to correct for rotation.

Aims

The main aims were to identify risk factors predictive of a radiolucent line (RLL) around the acetabular component with an interface bioactive bone cement (IBBC) technique in the first year after THA, and evaluate whether these risk factors influence the development of RLLs at five and ten years after THA.

Methods

A retrospective review was undertaken of 980 primary cemented THAs in 876 patients using cemented acetabular components with the IBBC technique. The outcome variable was any RLLs that could be observed around the acetabular component at the first year after THA. Univariate analyses with univariate logistic regression and multivariate analyses with exact logistic regression were performed to identify risk factors for any RLLs based on radiological classification of hip osteoarthritis.
