Purpose of the study: Palliative treatment for unreparable rotator cuff tears by intra-articular resection of the long head of the biceps provides demonstrated satisfactory results. We hypothesized that associated acromioplasty could be deleterious. Material and method: We conducted a case-control study comparing 24 tenotomies or tenodeses of the long head of the biceps with 24 tenotomies or tenodeses of the long head of the biceps associated with acromioplasty. All patients had an unreparable tear of the rotator cuff. Full-thickness tear of the subscapularis or Hamada and Fukuda radiological stage 3 or 4 tear were exclusion criteria. The two groups were matched for weighted Constant score, Hamada and Fukuda stage, type of tear of the infraspinatus and subscapularis, overall fatty infiltration grade, and follow-up. After the physical examination, we compared the four items of the Constant score, the absolute and weighted score, active anterior elevation, pain score on a visual analogue scale (VAS), and the subjective shoulder score. All patients were examined by a clinician unaware of the group at last follow-up (mean 51 months). Results: The two groups were comparable preoperatively except for the fatty infiltration index which was higher in the acromioplasty group. At 51 months follow-up, active anterior elevation was statistically lower in the acromioplasty group (145 versus 170). The absolute and weighted Constant scores were lower in the acromioplasty group (p< 0.05). There was no difference for pain and the subjective shoulder score (67 versus 71). Four patients in the acromioplasty group required revision total shoulder arthroplasty versus two in the group without acromioplasty. Discussion: Association of acromioplasty with intra-articular resection of the long head of the biceps for palliative treatment of unreparable rotator cuff tears is deleterious for elevation and reduces overall shoulder function. Even though acromioplasty provides good short-term results for average-sized tears, outcome is less satisfactory for massive tears and deteriorates over time. In addition, as described by Wiley, acromioplasty can induce a pseudoparalytic shoulder in patients with unreparable tears

Aims. The aim of this study is to determine the predictors of overall survival (OS) and predictive factors of poor prognosis of conventional high-grade osteosarcoma of the limbs in a single-centre in South Africa. Methods. We performed a retrospective cross-sectional analysis to identify the prognostic factors that predict the OS of patients with histologically confirmed high-grade conventional osteosarcoma of the limbs over ten years. We employed the Cox proportional regression model and the Kaplan-Meier method for statistical analysis. Results. This study comprised 77 patients at a three-year minimum follow-up. The predictors of poor OS were: the median age of ≤ 19 years (hazard ratio (HR) 0.96; 95% confidence interval (CI) 0.92 to 0.99; p = 0.021); median duration of symptoms ≥ five months (HR 0.91; 95% CI 0.83 to 0.99; p < 0.037); metastasis at diagnosis (i.e. Enneking stage III) (HR 3.33; 95% CI 1.81 to 6.00; p < 0.001); increased alkaline phosphatase (HR 3.28; 95% CI 1.33 to 8.11; p < 0.010); palliative treatment (HR 7.27; 95% CI 2.69 to 19.70); p < 0.001); and amputation (HR 3.71; 95% CI 1.12 to 12.25; p < 0.032). In contrast, definitive surgery (HR 0.11; 95% CI 0.03 to 0.38; p < 0.001) and curative treatment (HR 0.18; 95% CI 0.10 to 0.33; p < 0.001) were a protective factor. The Kaplan-Meier median survival time was 24 months, with OS of 57.1% at the three years. The projected five-year event-free survival was 10.3% and OS of 29.8% (HR 0.76; 95% CI 0.52 to 1.12; p = 0.128). Conclusion. In this series of high-grade conventional osteosarcoma of the appendicular skeleton from South Africa, 58.4% (n = 45) had detectable metastases at presentation; hence, an impoverished OS of five years was 29.8%. Objectives: Description of early results using a new modality in musculoskeletal oncology.

Introduction: Radiofrequency as a malignant tumor ablative modality has been employed during the last decade in liver tumors. Extra-hepatic application in malignancies is new and its indications are not precisely defined. It has been used for more than ten years for osteoid osteoma ablation. Radiofrequency allows destruction of a precise sphere around the application tip. Ablation efficiency is monitored by the change in tissue conductance following tissue necrosis.

Methods: 10 patients scheduled to undergo limb sparing surgery were included. During the pre-operative angiography and embolization, tumors were treated by radio-frequency ablation. The precise location of the needle electrode was defined by real-time ultrasonography. The relation of the needle tip to large blood vessels was monitored by comparison of ultrasonography to angiography. Thus, precise ablation of the tumor tissue was possible without risk to the neurovascular bundle.

Results: Tumor necrosis was uniform in the center of the ablative sphere and declined to 60 percent in the periphery. No complications were encountered.

Discussion: This method appears to allow further shrinkage of the tumors prior to limb sparing surgery. This technique is rapid and does not require delay of surgery, as does isolated limb perfusion or neo-adjuvant chemotherapy. The procedure can be performed in sedated patients, thus obviating the need for general anesthesia prior to limb sparing surgery. It can be employed in areas were isolated limb perfusion is not anatomically feasible such as the pelvis. Further studies are required in order to better define the role of this technique as compared to intra-arterial chemotherapy or isolated limb perfusion.

Purpose of the study: For patients aged less than 65 years who have a large rotator cuff tear, potential solutions include anatomic repair, palliative treatment, non-anatomic repair with flaps or cuff prosthesis, and reversed prosthesis. The later solution is not recommended at this age and anatomic repair is not always possible. In this situation, what is best, palliative treatment or flap or prosthesis repair?. Material and method: This retrospective multicentric study included 142 patients, 74 men and 68 women with a large or massive cuff tear. Palliative treatment (group A) involved acromioplasty (n=48) associated as needed with a biceps procedure and partial repair (n=41). Non-anatomic repair (group B) included supra-spinatous translation (n=16), deltoid flaps (n=22), and cuff prostheses (n=15). Preoperatively, the two groups were not statistically different for acromiohumeral height (AH, 6 mm in group A versus 7.5 mm in group B) and percent of fatty infiltration of the infraspinatus > II (55% in group A versus 26% in group B). The Constant score, active and passive range of motion, gain in elevation and external rotation were noted. Results: Mean follow-up was 74 months in group A and 90 months in group B; the Constant score was 64 and 65, active elevation 145 and 147 and external rotation 17 and 26 respectively. The two groups were not significantly different. For patients with deficient elevation (n=46), the gain was 62 without any difference between the two groups. For patients with deficient external rotation (n=37), the gain was nil in both groups. Discussion: Although group A had a more severe condition than group B (narrower AH and more advanced fatty degeneration, the final outcome as assessed by the Constant score and range of motion was similar. Both groups recovered active elevation well, but not external rotation. Nevertheless, there were no cases of latissimus dorsi transfer in this series. Repair with a deltoid flap, supraspinatus translation, or cuff prosthesis does not appear to add any supplementary benefit despite the more aggressive surgery

Introduction. It is an example of tendon to bone healing of rotator cuff tears. Low Level Laser Therapy (LLLT) is used in pain literature, pain palliation, tendinopathies, osteoarthritis treatment, implant osteointegration in jaw surgery, wound healing, fracture healing, tendon healing, nerve healing. But; there is not a study on tendon-bone healing. It is aimed to investigate the effects of tendon to bone healing with the rotator cuff experimental tear model. Material and Methods. 60 Wistor Albino Rat right shoulders were used in our project in four groups. Effectiveness of the study in each study group to increase and use the minimum number of animals that would be significant it was planned to use 15 (6 histology + 9 biomechanical) subjects. In our study, there are 4 groups in total. 1. Group 4. Week sacrified control group 2. Group 4. Week sacrificed LLLT group 3. Group 8. Week sacrified control group 4. Group is postoperative LLLT group. The 4 round SSP tendons have been cut with a full course. A total of eight sessions of biostimulation were performed with 24 j energy per session. Biomechanical tensile test and histopathological examination were performed on rats sacrificed at 4 and 8 weeks. In histological examination, cellularity at the repair site with hemotoxylin-eosin staining, extracellular matrix localization with Masson trichrome staining and fibrosis, TRAP (Tartrate Resistance Acid Phosphatase) and osteoclast activity, collagen fibril organization with picrochucine were evaluated. In immunohistological examination, proliferation activity was evaluated by CD-31 (Abcam, Cambridge, MA, USA) through vascular endothelial cells, Ki-67 (Tucson, AZ) and tendon proliferation index. Failure load for comparison of biomechanical stability between groups drawing will be done. Biomechanical pulling applications Linear pulling force over tendon It will be realized by applying. Last failure load (Newton), elongation (mm) and hardness values was recorded. Results. Two rats died in the postoperative first group and one rat died in the other groups. Six animals from each group were examined histopathologically. While the mean of stifness was higher in the patient who underwent LLLT at the 4th week biomechanically compared to the control group, there was no difference in the 8th week between the control group LLLT biomechanically. Histologically, the group with LLLT at 4 weeks showed increased fibroblastic activity and osteoclastic activity, but at 8 weeks there was no significant difference. Discussion. There are studies on the benefits of biostimulation with LLLT in fracture healing, implant integration, wound healing, tendon healing. Biostimulation experiments with LLLT for tendon healing were evaluated in rotator cuff healing as they were not available in literature. Especially early contributions were thought to be beneficial for postoperative rupture

The spine is a common site of metastasis. Complications include pathologic fracture, spinal cord compression, and neurological deficits. Vertebroplasty (VP) and Balloon Kyphoplasty (KP) are minimally invasive stabilization procedures used as a palliative treatment to improve mechanical stability, quality of life, and reduce pain. Photodynamic therapy (PDT) is a tumour-ablative modality that may complement mechanical stability afforded by VP/KP. This first-in-human study evaluates PDT safety when applied in conjunction with VP/KP. This dose escalation trial involved one light only control group and four light-drug doses (50,100,150,200J;n=6) delivered at 150mW from a 690nm diode laser by 800-micron optical fibers prior to KP/VP. Patients eligible for VP/KP in treating pathologic fracture or at-risk lesions at a single level were recruited. Exclusion criteria included spinal canal compromise or neurologic impairment. PDT is a two-step binary therapy of systemic drug followed by intravertebral light activation. Light was applied via bone trochar prior to cementation. This study used a benzoporphyrin derivative monoacid (BPD-MA), Verteporfin (VisudyneTm), as the photosensitizer drug in the therapy. Drug/light safety, neurologic safety, generic (SF-36), and disease-specific outcomes (VAS, EORTC-QLQ-BM22, EORTC-QLQ-C15-PAL) were recorded through six weeks. Phototoxicity and the side effects of the BPD-MA were also examined following PDT use. Thirty (10 male, 20 female) patients were treated (13 KP, 17 VP). The average age was 61 and significantly different between genders (Male 70yrs vs. Female 57yrs: p 0.05), and tumour status (lytic vs. mixed blastic/lytic: p>0.05). In most cases, fluence rates were similar throughout PDT treatment time, indicating a relatively stable treatment. Twelve (40%) of patients experienced complications during the study, none of which were attributed to PDT therapy. This included two kyphoplasty failures due to progression of disease, one case of shingles, one ankle fracture, one prominent suture, one case of constipation due to a lung lesion, one case of fatigue, and five patients experienced pain that was surgically related or preceded therapy. Vertebral PDT appears safe from pharmaceutical and neurologic perspectives. KP/VP failure rate is broadly in line with reported values and PDT did not compromise efficacy. The 50J group demonstrated an improved response. Ongoing study determining safe dose range and subsequent efficacy studies are necessary

Breast carcinoma is the most common cause for bony metastases. Skeletal complications in women with meta-static breast carcinoma often occur multiple times in a single patient and significantly contribute to the patient morbidity. We describe a 62 year old lady with a known metastatic breast carcinoma who presented with simultaneous quadruple extremity diaphyseal long bone fractures after a trivial fall. To the author’s best knowledge, similar report has never been previously described in the literature. The wish and general condition of the patient, and concurrent occurrence of four long bone fractures dictated the non-operative mode of treatment in this case.Where the life expectancy is assumed to be less than six weeks, the multidisciplinary team should give careful consideration on selection of best treatment choice between simultaneous or sequential surgical fixation of multiple long bone fractures and conservative palliative treatment. With treatment suited for an end-of-life circumstance, the educational lesson for dissemination to the readers is that in a patient where there is an extremely high likelihood of imminent perioperative mortality after sustaining quadruple extremity diaphyseal proximal long bone fractures simultaneously, conservative palliative treatment should be primarily considered over an aggressive operative fixation

Introduction. Over 300,000 patients present with fragility fractures each year. Following a hip fracture, the 30 days mortality is around 8.3 to 9.3%. Mortality increases to 20–24% after the first year and this risk may persist for at least 5 years. The most significant variables that influence mortality are age, gender, fracture type, prefracture residence, prefracture mobility and ASA scores. Common reported postoperative complications responsible for mortality in hip fracture patients include; heart failure, bronchopneumonia and infections. Most of the studies in the literature focus on management variables influencing mortality rather than the encountered postoperative medical problems leading to death. The aim of this study is to retrospectively analyze the postoperative medical complications which led to death in a cohort of patients, following a hip fracture. Methodology. This is a retrospective analysis of a cohort of 82, out of 648, patients with hip fractures who died over a period of 2 years (December 2006–January 2009). Forty nine females and 33 males (ratio 1.5:1) were included in the study with a mean age of 87.8 years (range 59–98 years). For mortality analysis, patients' co-morbidities and medical events leading to death were categorized according to the involved organ system. This included cardiovascular, respiratory, renal, gastrointestinal, neurological and musculoskeletal systems). The timing and decision of DNAR and palliation was noted for each patient. The 30 days and the overall mortality were the set outcome measures to be reviewed. Results. The 30 days mortality was 9.4% and 13.6% overall. The majority of patients had multiple co-morbidities; 75.6% had cardiac morbidities, 28% had respiratory problems, 19.5% had renal problems, 22% had neurological problems, 22% had endocrine problems and 34.1% had miscellaneous problems including cancer in 9 patients. Out of the 82 patients, 67 died postoperatively and 15 died preoperatively. Of the 67 postoperative deaths the main reported events that led to death were; respiratory in 59.7% (90% of which had chest infection), cardiac in 29.9% (60% of which had pulmonary edema) and sepsis in 35.8% (62.5% of which had chest infection). Of the 15 preoperative deaths the lead cause of death was the respiratory problems, reported in 73.3%, of which 81.8% were due to chest infection. X% of the patients had DNAR decision and x% were palliated. Documented decisions for DNAR and palliation were done by a senior house officers and registrars in the majority of cases (x%). Conclusion. The majority of deaths in our series were related to chest infection (54.9%), cardiac failure (15.9%) and sepsis mainly secondary to chest infection (32.9%, of which 62.9% were related to chest infection). Increased awareness of the encountered medical problems that can lead to death consolidates the need of a pre and postoperative orthogeriatric input to optimize these frail patients

The outcome of complex acetabular reconstruction was evaluated in twenty-one patients who were confined to a wheelchair or bed because of pain from acetabular metastases. Reconstruction rings were used where bone loss exceeded 50% of the acetabulum. Six roof reinforcement-rings, eight ilioischial-rings and eight Harrington reconstructions were performed. All but two patients(90%) became ambulatory without pain. Median survival was nine months. Two patients underwent acetabular revision for recurrence. These results support the role of acetabular reconstruction for palliation of pain in appropriate patients with acetabular metastases. Metastatic disease of the acetabulum is painful and disabling. Operative intervention is indicated in certain patients with pathologic fractures, and non-responders to adjuvant treatment. The functional outcome of hip arthroplasty with reconstruction rings was evaluated in twenty-one patients with acetabular metastases between 1989 and 2001. Preoperatively all patients were confined to a wheelchair or bed and used significant narcotic medications. Preoperative radiotherapy was employed in eighteen cases (90%) and 30% had undergone chemotherapy. AAOS classification of the acetabular lesion revealed: six-type II, seven-type III and eight-type IV deficiencies. All cases required a reconstruction ring due to bone loss exceeding 50% of the acetabular dome. Six roof reinforcement rings, eight ilioischial rings and eight Harrington reconstructions with rings were performed in this group. Determination of the reconstructive technique was based on preoperative computerized tomography and intraoperative examination of the acetabular deficiency. All but two patients (90%) became ambulatory without significant pain. Eleven patients used a walker or two canes and nine walked with one or no canes. Median survival was nine months and patients with visceral involvement had a shorter duration. Eight early post-operative complications developed in six patients (29%). In two patients the acetabular construct failed with cup migration due to locally recurrent disease; both were successfully revised. The results of complex acetabular reconstruction for metastatic disease validate its role for palliation of pain and to improve ambulatory status. Preoperative planning with computerized tomography can assist in classifying acetabular bone loss and determining optimal reconstruction technique

Shoulder arthritis in the young adult is a deceptive title. The literature is filled with articles that separate outcomes based on an arbitrary age threshold and attempt to provide recommendations for management and even potential criteria for implanting one strategy over another using age as the primary determinant. However, under the age of 50, as few as one out of five patients will have arthritis that can be accurately classified as osteoarthritis. Other conditions such as post-traumatic arthritis, post-surgical arthritis including capsulorrhaphy arthropathy, and rheumatoid arthritis create a mosaic of pathologic bone and soft tissue changes in our younger patients that distort the conclusions regarding “shoulder arthritis” in the young adult. In addition, we are now seeing more patients with unique conditions that are still poorly understood, including arthritis of the pharmacologically performance-enhanced shoulder. Early arthritis in the young adult is often recognised at the time of arthroscopic surgery performed for other preoperative indications. Palliative treatment is the first option, which equals “debridement.” If the procedure fails to resolve the symptoms, and the symptoms can be localised to an intra-articular source, then additional treatment options may include a variety of cartilage restoration procedures that have been developed primarily for the knee and then subsequently used in the shoulder, including microfracture, and osteochondral grafting. The results of these treatments have been rarely reported with only case series and expert opinion to support their use. When arthritis is moderate or severe in young adults, non-arthroplasty interventions have included arthroscopic capsular release, debridement, acromioplasty, distal clavicle resection, microfracture, osteophyte debridement, axillary nerve neurolysis, and bicep tenotomy or tenodesis, or some combination of these techniques. Again, the literature is very limited, with most case series less than 5 years of follow-up. The results are typically acceptable for pain relief, some functional improvement, but not restoration to completely normal function from the patient's perspective. Attempts to resurface the arthritic joint have resulted in limited benefits over a short period of time in most studies. While a few remarkable procedures have provided reasonable outcomes, they are typically in the hands of the developer of the procedure and subsequently, other surgeons fail to achieve the same results. This has been the case with fascia lata grafting of the glenoid, dermal allografts, meniscal allografts, and even biologic resurfacing with large osteochondral grafts for osteoarthritis. Most surgical interventions that show high value in terms of improvement in quality of life require 10-year follow-up. It is unlikely that any of these arthroscopic procedures or resurfacing procedures will provide outcomes that would be valuable in terms of population healthcare; they are currently used on an individual basis to try to delay progression to arthroplasty, with surgeon bias based on personal experience, training, or expert opinion. Arthroplasty in the young adult remains controversial. Without question, study after study supports total shoulder arthroplasty over hemiarthroplasty once the decision has been made that joint replacement is the only remaining option

Aims

To identify factors influencing clinicians’ decisions to undertake a nonoperative hip fracture management approach among older people, and to determine whether there is global heterogeneity regarding these factors between clinicians from high-income countries (HIC) and low- and middle-income countries (LMIC).

Methods

A SurveyMonkey questionnaire was electronically distributed to clinicians around the world through the Fragility Fracture Network (FFN)’s Perioperative Special Interest Group and clinicians’ personal networks between 24 May and 25 July 2021. Analyses were performed using Excel and STATA v16.0. Between-group differences were determined using independent-samples t-tests and chi-squared tests.


Aims

The primary aim of this study is to compare mobility status of patients receiving oral oxycodone with those receiving subcutaneous alfentanil as analgesic methods prior to mobilization to help physiotherapy compliance after hip fracture surgery. The secondary aims are to assess postoperative pain, health-related quality of life, in-hospital length of stay, total use of analgesia over postoperative days 1 and 2 (POD 1 and POD 2), complication rates within 30 days, and 30-day mortality rates.

Methods

A single-centre, prospective cohort study of 64 patients will be undertaken. Patients undergoing surgery for femoral neck fractures at the study centre will be recruited. Patients with a hip fracture meeting the inclusion/exclusion criteria will be enrolled on admission. Patients who have been administered oral oxycodone will be compared to those prescribed alfentanil for pain prior to mobilization with physiotherapists on POD 1 and POD 2. Which drug a patient receives is reliant of the prescriptions given by the medical team, and in current practice this varies at approximately 50:50. Mobilization will be defined as the ability to stand on and weightbear both feet with or without assistance.

Aim. European population is ageing concurrently with an increase number of arthroplasties. Prosthetic joint infection (PJI) in the elderly is considered more severe. The aim of this study is to describe PJI's management of patients over 79 years of age. Methods. We conducted a retrospective study including all patients aged over 79 years old consulting for a suspected hip or knee PJI in our community hospital where a complex bone and joint unit is present. Results. From 2007 to 2015, among the 366 patients who consulted for a PJI suspicion, 44 were older than 79. In this group, median age was 81.5 and 52% were women. A significant comorbidity was present in 24 patients among them 9 were diabetic. Location of suspected PJI was hip for 24 patients and 52% of the patients had a PJI background. Median time from the first arthroplasty was 8 years, however 17 had already an exchange. We classified the presentation as early (before 3 months after surgery, n=7), delayed (3 to 24 months, n=9) and late (more than 24 months, n=28). Pain was the first symptom, 9 presented fever and 10 had a sinus tract communication. Median C-reactive protein rate was 64 mg/l. Pre-operative synovial fluid analysis was performed in 34 patients, the concordance with intra-operative samples was 44%. A surgery was performed in 86% of the patients corresponding in five retentions, 17 one-time and 13 two-time exchange, 2 arthrodesis and one resection of arthroplasty. Coagulase-negative Staphylococcus (n=14), Staphylococcus aureus (n=10) and Enterobacteriaceae (n=5) were the principal microorganisms identified. Antibiotherapy median duration was 10 days for intravenous regimens and 45 days for total treatment. We noted 4 catheter-related infections and 9 side effects of antibiotics. A prolonged antibiotic suppressive therapy was performed for 8 patients (18%). With a median time of follow-up of 21.5 months, we notified 13 failures (30%) and 5 deaths (11%). After the episode, 5 patients could not standup, a walking stick was necessary for 11 patients, 2 for 5 patients while 13 recovered a relatively good autonomy. Conclusion. PJI in elderly people is a severe complication with a significant morbidity but palliative treatment is not the first alternative. We showed acceptable outcomes with more invasive managements. These data need to be compared with younger population in a second analysis


Aims

For rare cases when a tumour infiltrates into the hip joint, extra-articular resection is required to obtain a safe margin. Endoprosthetic reconstruction following tumour resection can effectively ensure local control and improve postoperative function. However, maximizing bone preservation without compromising surgical margin remains a challenge for surgeons due to the complexity of the procedure. The purpose of the current study was to report clinical outcomes of patients who underwent extra-articular resection of the hip joint using a custom-made osteotomy guide and 3D-printed endoprosthesis.

Methods

We reviewed 15 patients over a five-year period (January 2017 to December 2022) who had undergone extra-articular resection of the hip joint due to malignant tumour using a custom-made osteotomy guide and 3D-printed endoprosthesis. Each of the 15 patients had a single lesion, with six originating from the acetabulum side and nine from the proximal femur. All patients had their posterior column preserved according to the surgical plan.

Cancer associated bone pain (CIBP) is a common event in patients with advanced disease with bone metastases (BM), significantly impairing their quality of life. Treatment options are limited and mainly based on the use of opioids with unacceptable side effects. Local acidosis is a well-known cause of pain since it directly stimulates nociceptors that express acid-sensing ion channels and densely innervate bone. In BM, local acidosis derives from osteoclast bone resorption activity and from the acidification by glycolytic tumor cells. Here we speculated that the pH lowering of intratumoral interstitial fluid also promotes nociceptors sensitization and hyperalgesia through the activation of cells of mesenchymal origin in BM microenvironment that might release inflammatory and nociceptive mediators. As a model of breast cancer that can metastatise to the bone we used MDA-MB-231 (MDA), and a subclone with a higher tendency to form osteolytic BM (bmMDA). We evaluated the basal expression of proton pumps/ion transporters by Real-Time PCR (Q-RT-PCR). To evaluate the effect of extracellular acidosis on mesenchymal tumor-associated stroma, we used human osteoblast primary cultures from healthy donors and cancer-associated fibroblasts isolated with specific immunobeads from the tumor biopsies of patient with BM. We exposed the cells to pH 6.8 medium at different time points (between 3 to 24 hours). After the short-term incubation with acidosis, for the expression of and acid-sensing ion channels, inflammatory cytokines and nociceptive mediators that can produce hyperalgesia, we used both a wide screening through a deep-sequencing approach and Q-RT-PCR, and ELISA. Xenograft for osteolytic BM induced by intratibial injection of bmMDA were treated with Omeprazole and monitored for CIBP through several cognitive tests. We found a significantly higher extracellular proton efflux and expression of proton pumps/ion transporters associated with the acid-base balance, the monocarboxylate transporter 4 (MCT4), the carbonic anhydrase (CA9), and the vacuolar ATPase (V-ATPase) V. 1. G. 1. subunit, and V. 0. c subunitin bmMDA, a subclone that is prone to form BM in respect to the parental cell line MDA-MB-231. In mesenchymal stromal cells, osteoblasts and cancer-associated fibroblasts, the incubation with pH 6.8 induced the expression of the achid-sensing ion channels AISC3/ACCN3 and AICS4/ACCN4, as well as of the nociceptive modulators nerve growth factor (NGF), Brain-derived neurotrophic factor (BDNF), and of the inflammatory cytokines interleukin 6 (IL6) and 8 (IL8), and Chemokine (C-C motif) ligand 5 (CCL5). Furthermore, the targeting of V0c subunit to inhibit intratumoral acidification significantly reduced CIBP in mice model of BM. In this study we demonstrated for the first time that, in addition to the direct acid-sensing neuronal stimulation, the acidic microenvironment of BM causes hyperalgesia through the activation of an inflammatory reaction in the tumor-associated mesenchymal stroma at the tumor site, thereby offering as a new target for palliative treatment in advanced cancer

Despite numerous advances in the management of chronic osteomyelitis, many questions remain. To date, no evidence-based guidelines exist in the treatment of chronic osteomyelitis. In essence the aim is to improve quality of life through either a curative or a palliative treatment strategy. The choice of treatment strategy is based on the physiological status of the host. This process of host stratification is, however, complicated by the fact that the definition of a C-host has never been standardized. Purpose;. The aim of the study was to investigate the short term outcome of the treatment of chronic osteomyelitis in adult patients where selection of a management strategy was based on a refined host stratification system. Methods;. A retrospective review was performed of adult patients with chronic osteomyelitis seen over a one year period. In total 116 patients were included in the study. A modified host stratification system was applied, incorporating predefined major and minor criteria, to determine each patient's host status. Results;. A high prevalence of HIV infection (28.6%) and malnutrition (15%) was present in the study population. Almost half the patients were classified as C-hosts (44.8% or n=52), followed by B-host classification in 39.7% of cases (n=46). At a mean follow-up of one year an overall success rate of 91.4% (95% CI: 84.7–95.8%) was achieved. Host status and outcome (remission, suppression or failure) was significantly dependent (p-value < 0.001). Success was achieved in 92.2% of patients treated curatively and 89.6% of patients treated palliatively. Conclusion;. By integrating the physiological status of the host (based on objective predefined criteria) with the appropriate curative, palliative or alternative treatment strategy we were able to achieve acceptable outcomes in both low and high risk cases and, in addition, avoid unnecessary amputation

We report the case of an 80-year-old woman treated by palliative knee arthrodesis for metastases of the proximal left tibia secondary to bladder carcinoma, using percutaneous femortibial intramedullary arthrodesis nailing. The technique provided a simple alternative to massive allografting, total joint prosthesis or amputation, with advantages of low morbidity, short operating time, minimal blood loss, immediate ambulation and weight bearing, relief of pain, restoration of independence, and ease of nursing care. We are satisfied with the procedure of percutaneous femorotibial intramedullary nailing as a palliative treatment of proximal tibial metastases in an elderly patient

A prospective study was performed to evaluate the efficacy and safety of percutaneous kyphoplasty in patients with osteolytic tumours of thoracic and lumbar spine. To our knowledge this is the only study so far that has followed a cohort of patients prospectively until death. Prospective study of patients with lytic tumours of spine treated with kyphoplasty. A total of 13 patients with osteolytic tumours of spine were treated with kyhpoplasty. There were 8 female and 5 male patients. The age range was 52-81 years with average age of 65 years. A total of 25 vertebrae, from T2 to L3, were treated. The types of tumours included; non-Hodgkin lymphoma (2), myeloma (2), gastric-carcinoma (1), cervix-carcinoma (1), breast-carcinoma (3), prostate-carcinoma (2), small cell lung-carcinoma (1), bladder-carcinoma (1). Outcome was assessed prospectively by visual analogue scale (VAS) for pain, ECOG performance status, walking distance, standing and sitting time. The preoperative average VAS was 7.5 (range: 2.6 – 10). This dropped to 3.0 five days postoperatively and remained below 5 for the duration of follow-up. Average walking distance, standing and sitting time and ECOG performance score showed improvement. The survival time ranged from 2 to 293 weeks. The average survival time was 82 weeks. All patients were able to return home following the procedure. No patient required reoperation or readmission for spinal metastasis. Kyphoplasty is a suitable palliative treatment option for patients with advanced metastatic disease of the spine

Background. Advances in diagnosis and treatment should mean that hindquarter amputation is now rarely needed. Unfortunately this is not the case. We have performed 166 of these amputations in the past 36 years. We have investigated the reasons why this procedure is still required and the outcomes following it. Method. A retrospective review of data stored on a prospective database. Results. Hindquarter amputation was used as treatment for 15% of all primary bone tumours affecting the pelvis. 146 were performed with curative intent but 20 were performed purely for palliation, usually to relieve pain. 96 of the procedures were needed as part of primary treatment, with the other 70 being needed following failure of local control after other surgical procedures. The indication for amputation in primary disease was almost always due to a significant delay in diagnosis, allowing tumours (particularly chondrosarcomas) to become massive by the time of diagnosis. The peri-operative mortality was 3% and 45% had major wound healing problems or infection. The median survival times after curative and palliative procedures were 36 months and 8 months respectively. The survival after hindquarter amputation for curative intent at 1, 3 and 5 years was 74%, 60% and 48%. Overall survival was better with chondrosarcoma – 52% of the patients surviving more than 10 years had chondrosarcoma. Phantom pain was a significant problem; fewer than 10% use their prosthesis regularly. Despite this functional scores averaged 61% – not significantly worse than patients who had undergone pelvic replacements!. Conclusion. Hindquarter amputation is still regularly required both for primary and salvage treatment in musculoskeletal oncology. Earlier diagnosis of pelvic tumours may avoid its use. Survival is not surprisingly worse than for tumours at other sites

Our aim was to assess the value of external fixation in pathological fractures in selected patients. During 2003–2008 we treated 35 patients with multiple myeloma or disseminated cancer, visceral metastases and pathological fractures with external fixation under sedation and local anaesthetic, because they were not fit for general anaesthesia. We used external fixation on 1 hip fracture, 1 fracture of the second metatarsal, 2 wrist fractures, 4 radial, 5 intertrochanteric, 1 subtro-chanteric, 12 fractures of the humerus, 1 ulna fracture, 4 femoral fractures, 3 tibial fractures and 1 femoral osteolysis. Operating times were 15–35 min, all patients were comfortable, cooperated well and they did not experience any pain during the procedure. In all cases XRT was applied either pre- or post-operatively. On follow up (2–48 months) 4 of the patients were deceased. Fracture stabilization was adequate and X-rays confirmed porosis in 4 fractures; however, two lesions expanded further, despite proper adjuvant bio-pharmaceutical therapy. 5 patients impoved so we could operate them later to treat the fractures definitively. All individuals experienced pain relief, they were adequately mobilized and most function was restored, while there was no major problem with pin tract infections. We suggest external fixation as a palliative treatment in patients with pathological fractures and multiple metastases, who don’t qualify for major surgery because of their critical illness. The later puts under local offer an excelent chance to fix fractures quikly, manage the pain and restore function without the risks of general anaesthesia
