To evaluate the short-term clinical outcomes of patients treated arthroscopically with chitin-based scaffolding for acetabular chondral defects in conjunction with microfracture compared to microfracture alone. This study is a retrospective analysis of prospectively collected data. A review of charts was performed (2014–2016) on all patients who underwent hip arthroscopy and had microfracture +/− scaffolding for acetabular chondral defects, intraoperative details (lesion size, grade, labral repair/reconstruction) and postoperative complications were recorded with a minimum follow-up of 2 years. Clinical outcomes were assessed by analysing iHOT and HOS scores which were obtained pre-operatively, at six months, one year and two years post-surgery. Plain radiographs were assessed for hip osteoarthritis by Kellgren & Lawrence grading. A total of 60 patients (microfracture=25, scaffolding=35) were included. Patients had a mean age of 36.2 years at the time of the index operation. There were no major adverse events of deep vein 36.2 years at the time of the index operation. There were no major adverse events of deep vein thrombosis, blood vessel or nerve damage, hemarthrosis or device related adverse events in both groups. Two patients were readmitted due to pain as a result of an inflammatory reaction in the scaffolding group. Both treatments of microfracture and scaffolding showed significant improvement in outcome score (iHOT) (p < 0 .001) when compared postoperative to preoperative. Both the arthroscopic treatment of chondral acetabular defects with chitin based scaffolding and microfracture demonstrated significant improvement from their pre-operative outcomes

Introduction. Ostochondral lesion of the knee is a common cause of chronic knee pain. Arthroscopic treatment with subcondral microfracture is a widespread technique leading to noticeable improvement of knee function and pain. To improve the effectiveness of this treatment options, we thought to add intra (PRF) or post-operative (PRP) growth factors. Platelet rich plasma (PRP) is obtained by centrifugation of the blood to produce a plasma with high concentration of platelets and growth factors. This latter represents a promising method to manage degenerative cartilage lesion and can be used postoperatively to improve clinical results of patients treated arthroscopically. Platelet Rich Fibrin (PRF) has been presented as a second-generation platelet concentrate, and it is used intraoperatively to cover the microfracuteres’ holes. No literature was found about using of PRF intraoperative in association with arthroscopic microfracture technique. The aim of this study is to compare clinical outcomes of the treatment of knee osteochondral lesion using arthroscopic microfracture technique alone or in association with PRF Intraoperative application using “Vivostat” system or with PRP “ReGen Lab” postoperative injection. Patients & Methods. 90 patients with clinical and radiographic evidence of osteochondral lesion of the medial or lateral compartment of the knee were enrolled. All patients received arthroscopic debridement and Microfractures and were randomised into 3 groups: 30 patients received microfractures and intraoperative PRF “Vivostat” injection(Group A), 30 patients received microfracture and 3 intra-articular injections of 5.5 mL PRP “Regen”(Group B), 30 patients received microfracture only. IKDC, KOOS and VAS score were administered to all patients before starting the treatment, at 1, 6 and 12 months from the end of the management. Results. Patients who received microfracture and PRF intraoperative application provided the best outcomes, showing a significant higher clinical scores (P<0.001) compared to the other two groups. Patients underwent PRP postoperative administration reported significant higher score than those undergoing arthroscopic microfracture alone (P<0.005), but lesser than Intraoperative PRF group at 6 months and 1 year follow up. Discussion/Conclusion. Treatment of osteochondral lesions of the knee using microfracture technique significantly improved functional and pain scores from the pre- to postoperatively time in the overall cohort. Intraoperative application of PRF shows significantly better outcome than postoperative PRP injections. However, additional treatment with intra-articular PRP injection as an adjunct to microfracture technique may offer better clinical outcomes over microfracture technique alone


Aims. Arthroscopic microfracture is a conventional form of treatment for patients with osteochondritis of the talus, involving an area of < 1.5 cm. 2. However, some patients have persistent pain and limitation of movement in the early postoperative period. No studies have investigated the combined treatment of microfracture and shortwave treatment in these patients. The aim of this prospective single-centre, randomized, double-blind, placebo-controlled trial was to compare the outcome in patients treated with arthroscopic microfracture combined with radial extracorporeal shockwave therapy (rESWT) and arthroscopic microfracture alone, in patients with ostechondritis of the talus. Methods. Patients were randomly enrolled into two groups. At three weeks postoperatively, the rESWT group was given shockwave treatment, once every other day, for five treatments. In the control group the head of the device which delivered the treatment had no energy output. The two groups were evaluated before surgery and at six weeks and three, six and 12 months postoperatively. The primary outcome measure was the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Scale. Secondary outcome measures included a visual analogue scale (VAS) score for pain and the area of bone marrow oedema of the talus as identified on sagittal fat suppression sequence MRI scans. Results. A total of 40 patients were enrolled and randomly divided into the two groups, with 20 in each. There was no statistically significant difference in the baseline characteristics of the groups. No complications, such as wound infection or neurovascular injury, were found during follow-up of 12 months. The mean AOFAS scores in the rESWT group were significantly higher than those in the control group at three, six, and 12 months postoperatively (p < 0.05). The mean VAS pain scores in the rESWT group were also significantly lower than those in the control group at these times (p < 0.05). The mean area of bone marrow oedema in the rESWT group was significantly smaller at six and 12 months than in the control group at these times (p < 0.05). Conclusion. Local shockwave therapy was safe and effective in patients with osteochondiritis of the talus who were treated with a combination of arthroscopic surgery and rESWT. Preliminary results showed that, compared with arthroscopic microfracture alone, those treated with arthroscopic microfracture combined with rESWT had better relief of pain at three months postoperatively and improved weightbearing and motor function of the ankle. Abstract. Introduction. The aim of this study was to determine the factors affecting return to sport (RTS) and career longevity of elite athletes after microfracture of the knee. Methods. A retrospective review of a consecutive series of elite athletes with chondral injuries in the knee treated with microfracture was undertaken. RTS was defined as competing in at least one event at professional level or national/ international level in amateur sport. Demographic, pre, intra and post operative factors affecting RTS were analysed. Results. Fifty six athletes, including 35 (62.5%) footballers and 14 (25.0 %) rugby players, with a mean age 24.8 (+/− 3.9) years, were included. 18 (32.1 %) of chondral injuries were secondary to trauma, 20 (35.7 %) post meniscectomy, 9 (16.1%) post anterior cruciate ligament (ACL) reconstruction and 9 (16.1%) idiopathic. Fifty (89.3%) of athletes RTS at a mean time of 10.0 (+/−5.8) months. 43 (86.0%) athletes were still playing at 2 years. At 5 years 25 (59.5%) of the players who had RTS and had surgery more than 5 years ago were still playing professional sport. Factors associated with not playing at 5 years were cartilage lesions >1.5cm2(52.4% vs 66.7%), more than 1 lesion (35.7% vs74.1%), lateral meniscal surgery (42.1% vs 73.7%) and effusion on RTS (47.7% vs 72.2%). Conclusion. Over 89 % of elite athletes returned to professional sport after microfracture in the knee. However, the ability to continue playing is affected by several factors such as the extent of the chondral damage and recurrent effusions

Abstract. Objectives. The ability to predict which patients will improve following routine surgeries aimed at preventing the progression of osteoarthritis is needed to aid patients being stratified to receive the most appropriate treatment. This study aimed to investigate the potential of a panel of biomarkers for predicting (prior to treatment) the clinical outcome following treatment with microfracture or osteotomy. Methods. Proteins known to relate to OA severity, with predictive value in autologous cell implantation treatment or that had been identified in proteomic analyses (aggrecanase-1/ ADAMTS-4, cartilage oligomeric matrix protein (COMP), hyaluronic acid (HA), Lymphatic Vessel Endothelial Hyaluronan Receptor-1, matrix metalloproteinases-1 and −3, soluble CD14, S100 calcium binding protein A13 and 14-3-3 protein theta) were assessed in the synovial fluid (SF) of 19 and 13 patients prior to microfracture or osteotomy, respectively, using commercial immunoassays. Levels of COMP and HA were measured in the plasma of these patients. To find predictors of postoperative function, multiple linear regression analyses were performed. Results. Linear regression analyses demonstrated that a lower concentration of HA in pre-operative SF was predictive of improved knee function (higher Lysholm score) following microfracture surgery. Further, lower pre-operative activity of ADAMTS-4 in SF was a significant, independent predictor of higher post-operative Lysholm score (improved joint function) following osteotomy surgery. Conclusion. This study is novel in identifying biomarkers with the potential to predict clinical outcome in patients treated with microfracture or osteotomy of the knee. Lower concentrations of HA and undetectable activity of ADAMTS-4 in the joint fluid of individuals with cartilage defects/early-OA may be used in algorithms to stratify patients to the most appropriate surgery. Surgical microfracture is considered a first line treatment for talar osteochondral defects. Pain reduction, functional improvement and patient satisfaction are described to be 61–86% in both primary and secondary osteochondral defects. However, limited research is available whether improvement of the surgical technique is possible. We do know that the current rigid awls and drills limit the access to all locations in human joints and increase the risk of heat necrosis of bone. Application of a flexible water jet instrument to drill the microfracture holes can improve the reachability of the defect without inducing thermal damage. The aim of this study is to determine whether water jet drilling is a safe alternative compared to conventional microfracture awls by studying potential side effects and perioperative complications, as well as the quality of cartilage repair tissue in a caprine model. 6 mm diameter talar chondral defects were created bilaterally in 6 goats (12 samples). One defect in each goat was treated with microfracture holes created with conventional awls. The contralateral defect was treated with holes created with 5 second water jet bursts at a pressure of 50 MPa. The pressure was generated with a custom-made setup using an air compressor connected to a 300 litre accumulator that powered an air driven high-pressure pump (P160 Resato, Roden, The Netherlands, . www.resato.com. ). Postoperative complications were recorded. After 24 weeks, analyses were performed using the ICRS macroscopic score and the modified O'Driscoll histological score. Wilcoxon ranked sum tests were used to assess significant differences between the two instrument groups using each goat as its own control (p ≤ 0.05). One postoperative complication was signs of a prolonged wound healing with swelling and reluctance to weight bearing starting two days after surgery on the water jet side. Antibiotics were administered which resolved the symptoms. The median total ICRS score for the tali treated with water jets was 9,5 (range: 6–12) and 9 (range 2–11) for Observer 1 and 2 respectively; and for the tali treated conventionally this was 9,5 (range 5–11) and 9 range (2–10). The median total Modified O'Driscoll score for the tali treated with water jets was 15 (range: 7–17) and 13 (range: 3–20) for Observer 1 and 2 respectively; and for the tali treated conventionally was 13 (range: 11–21) and 15 (range: 9–20). No differences were found in complication rate or repair tissue quality between the two techniques. The results suggest that water jet drilling can be a safe alternative for conventional microfracture treatment. Future research and development will include the design of an arthroscopic prototype of the water jet drill. The focus will be on stability in nozzle positioning and minimized sterile saline consumption to further the decrease the risk of soft tissue damage

Abstract. Objectives. The aim of this work was to compare the different techniques and the different fluid permeability of the tissue following each technique through assessing the flow of radiopaque contrast agent using μCT image analysis and 3D modelling. Methods. Donated human tali specimens (n=12) were prepared through creating a 10mm diameter chondral defect in three different regions of each talus. Each region then underwent one of three surgical techniques: 1) Fine wire drilling, 2) Nanofracture or 3) Microfracture, equidistant sites in each defect to ensure even distribution. Each region then had an addition of 0.1 ml radiopaque contrast agent (Omnipaque™ 300), imaged using a clinical μCT scanner (SCANCO Medical AG, 73.6 μm resolution). Each μCT scan was segmented using Slicer 3D software (The Slicer Community, 2023 3D Slicer (5.2.2)). The segmentation package was used to segment the bone and contrast agent regions in each different surgical site of each sample. Each defect site was created into a cylinder and the ratio of segmented pixels of contrast agent against bone. Results. The μCT analysis indicated that across the 12 samples, eight nanofracture regions demonstrated flow of the contrast agent either to the depth of the fracture site or deeper. Some lateral flow was also observed in these sites. eight microfracture regions demonstrated that the flow of the contrast agent was localised to the fracture site and a preferential flow laterally. In only one sample, did a fine wire drilling region demonstrate any fluid flow. In this sample, contrast agent had permeated through the drilling site to the bottom and some sub-site permeation was observed. However, in all samples that showed no permeation of contrast agent through the fracture site, a layer of contrast agent on the chondral surface or minor permeation through to the sub-chondral surface. Segmentation of each sample site showed a significant increase (n=12, p<0.05) in fluid flow of the contrast agent in the nanofracture sites (11%) compared to microfracture (5%) and fine wire drilling (2%). Conclusions. Nanofracture showed significantly improved fluid permeability throughout the surrounding trabecular structure, when compared to microfracture and fine wire drilling. Microfracture appears to allow some fluid flow, but only confined to the immediate area around the fracture site, while fine wire drilling appears to allow a comparably small amount, if not no fluid flow through the surrounding trabecular tissue. This conclusion is reinforced by previous literature that concluded the damage to the structure of the trabecular tissue is reduced when using nanofracture, compared to the other two techniques. Chondral hip injuries are common secondary to femoroacetabular impingement (FAI). Treatment with arthroscopic procedures including chondroplasty and microfracture is becoming increasingly common but literature is limited to case series at specialist centres. The aim of this study is to compare outcomes of arthroscopic acetabular chondral procedures using the NAHR dataset (UK) which represents the largest series to date. All adult Arthroscopies recorded in the NAHR from Jan 2012 were available for inclusion. Exclusions included significant arthritis and femoral, complex or revision chondral procedures. Patients completed iHOT-12 & EQ-5D Index pre-operatively, 6 and 12 months. Data was analysed using T-test/ANOVA for between group/within group for continuous variables, chi square test for categorical variables and linear regression model for multivariable analysis. 5,752 patients, 60% female. 27% Chondroplasty, 5% Microfracture, 68% no Chondral Procedure. Maximum acetabular Outerbridge classification 14% Grade 1, 15% Grade 2, 17% Grade 3, 8% Grade 4, 9% no damage, not recorded in 37%. Higher proportion of Cam impingement in association with chondral treatments and a larger proportion of patients with no impingement recorded in group with no chondral procedures. There was a significant improvement versus baseline for all groups in iHOT-12 and EQ-5D Index (p<0.0001) including Grade four Outerbridge. There was significantly greater improvement in pre-operative scores in the chondroplasty group compared to the microfracture group at 6 and 12 months (p<0.05). Following hip arthroscopy, patients with chondral procedures experienced improved outcome scores despite Outerbridge 4 chondral damage. Presence of cam lesions are more commonly associated with chondral treatments. Good outcomes were maintained up to 12 months for chondral procedures, regardless of age or impingement pathology however pincers improved less and patients over 40 years took longer to see improvement

Purpose:. To develop a method for depth-wise analysis of subchondral bone that considers the gradient of bone volume, density and organization between the articular surface and the marrow cavity. To understand the interplay between subchondral bone changes and extrinsic cartilage repair after microfracture. Method: Since 30% of patients fail microfracture for contained chondral lesions, our hypothesis was that early subchondral sclerosis increases compaction of bone around microfracture holes, leading to failed cartilage repair. Human osteochondral segments from the knee joint were characterized macroscopically using the Outerbridge score, then imaged at 45 micron resolution using microCT. Regions of interest (ROI) were chosen under normal cartilage and abnormal cartilage (Outerbridge Score=1). Routine Bone mineral density (BMD) analysis was performed on each ROI using GE MicroView™ analysis software. Additional depth-wise analysis of BMD was done by exporting each ROI was a density map, and calculating the mean, standard deviation and rate of change of BMD by slice in the vertical (coronal) plane. Plots of normal and early OA data by depth were compared. Microfracture holes were made in normal and sclerotic subchondral bone, and depth-wise measurements of subchondral compaction around the holes were made were made. Results: Bone under normal versus OA cartilage was very subtly different in microCT images, but ROI microCT analysis showed that the OA samples were more mineralized and contained more bone. Using the depth-wise analysis algorithm, automated detection and measurement of the subchondral bone plate and other discrete structures was possible. The depth-wise analysis confirmed that the osteoarthritic subchondral bone plate had a higher BMD and bone volume fraction, but also showed that the rate of change (gradient) in BMD was greater. Horizontally orientated trabeculae and other anomalies were found in OA bone that contributed a more variable BMD in trabecular bone at up to 5 mm from the articular surface. Bone with early sclerotic changes had significantly (p< .01) more bone volume fraction and BMD (p< .05) around microfracture holes in this ex vivo experiment. Conclusion: An enhanced picture of subchondral bone plate and trabecular bone anomalies can be appreciated using a depth-wise approach to image analysis. Both sclerosis and osteopenia have been reported in OA and models of OA, but this analysis shows that variability and gradient of BMD change adjacent to the articular cartilage is a significant feature of OA. This is consistent with some theories of OA progression that implicate stress concentration between the cartilage and subchondral bone plate leading to cartilage degeneration. More importantly, bone sclerosis has a direct effect on the amount of compaction around microfracture holes, so improvements in microfracture technique are needed to avoid this

Introduction: Chondral lesions are the second most common pathology encountered during hip arthroscopy. Microfracture is a simple and effective technique to treat chondral lesions with proven long term results in the knee. However, there is little evidence to confirm the ability of microfracture to produce repair tissue in hip joint. Methods: Patients with acetabular chondral defect treated with microfracture during primary arthroscopy and who had a subsequent hip arthroscopy enabling visualisation of the treated chondral defect were included in the study. Over a three year period 185 patients had microfracture for treatment of full thickness chondral defect. 11 patients (8 males and 3 females) with a mean age of 35 years (range 17–54 years) who had revision hip arthroscopy form the study population. The size of chondral defect was measured at the time of primary arthroscopy. Microfracture was performed using arthroscopic awls with a standard technique. Postoperatively a strict rehabilitation protocol was followed. The extent and quality of repair tissue was assessed by visual inspection at second look arthroscopy. Results: All patients had chondral lesions confined to the antero-superior aspect of the acetabulum with an associated labral tear. None had diffuse osteoarthritis. The average defect measured 180 mm2 (range 50–300). The mean time interval between primary and revision arthroscopy was 12 months. Excluding one failure the overall percent fill of the defects was 95% (range 75 – 100) with good quality cartilage. Discussion: Only one other series has reported on the macroscopic results of microfracture in the hip. Our series agrees with the results of those authors. These similar results from 2 centres confirm that arthroscopic microfracture is an effective treatment for acetabular chondral lesions in carefully selected patients

Microfracture is a simple and effective technique to treat chondral lesions. Studies have shown good long term results in the knee. However there is little evidence to confirm the ability of microfracture to produce repair tissue in hip joint. Methods: Patients aged 18 years or older who had a full thickness acetabular chondral defect treated with microfracture during primary arthroscopy and who had a subsequent hip arthroscopy enabling visualisation of the treated chondral defect were included in the study. Over a three year period 185 patients had microfracture for treatment of full thickness chondral defect. 11 patients (8 males and 3 females) with a mean age of 35 years (range 17–54 years) who had revision hip arthroscopy for various reasons form the study population. The size of chondral defect was measured at the time of primary arthroscopy. Microfracture was performed using arthroscopic awls with a standard technique. Postoperatively a strict rehabilitation protocol was followed with no weight bearing allowed for six weeks. The extent and quality of repair tissue was assessed by visual inspection at second look arthroscopy. Results: All acetabular chondral lesions were identified in the antero-superior quadrant at primary arthroscopy. The average defect after debridement measured 180 mm2 (range 50–300 mm2). 10 patients had chondral lesions confined to the acetabulum. 1 patient had a large femoral head defect in addition, due to Avascular Necrosis. None of the patients had diffuse osteoarthritis. All patients had an associated labral tear. The mean time interval between the primary and revision arthroscopy was 12 months. Excluding 1 failure the overall percent fill of the defects was 95% (range 75 – 100%) with good quality (Grade 1) cartilage. There was one failure with only a 25% fill. In that particular patient a large flap of delaminated cartilage was not resected at primary arthroscopy. Instead microfracture was done under the flap in the hope of encouraging the cartilage to stick to the underlying bone. Unfortunately the cartilage continued to remain delaminated and also hindered the formation of new repair tissue. Conclusion: Only one other series of second look arthroscopy after microfracture has been reported. Our series agrees with the results of those authors. These similar results from 2 centres confirm that arthroscopic microfracture is an effective treatment for acetabular chondral lesions in carefully selected patients

Objective. To provide a best estimate of the average treatment effect when microfracture was chosen as the intervention of choice in patients with full-thickness cartilage defects of the knee. Design. We focussed on controlled studies which either referred to microfracture alone or in comparison with any other surgical treatment of articular cartilage of the knee. Papers including patients who had been treated by microfracture and concomitant adjuvant procedures like ACL reconstruction or meniscus repair were accepted too, whereas papers reporting on the microfracture technique combined with the implantation of a scaffold were excluded. To achieve a best estimate of the average, to be expected treatment effect we pooled pooled before–after data of study arms using microfracture. Because cartilage studies employ various scales to measure functional improvements, we standardized treatment effects using Hedges' g. To provide clinically meaningful estimates we converted the pooled summary effect back into the respective scales by multiplying the pooled effect with pooled standard deviations of each included clinical scale. Results. A systematic review of the literature revealed six papers including 200 patients with a mean age of 32 years, a mean defect size of 3 cm. 2. and a follow up period from 2 to 5 years. Four of the studies compared microfracture to autologous chondrocyte implantation and two of them to osteochondral autologous transplantation. All patients were treated by the microfracture technique as described by Steadman and by a similar rehabilitation protocol which only allowed crutch-assisted touchdown weight bearing initially. Referring to the individual studies, a comparison of the pooled estimates of Hedges' g revealed that the two papers which evaluated the youngest patients provided the highest treatment effect. On the contrary, those two papers which focussed on the largest lesions, reported the worst improvement. Finally, the remaining two papers whose patients were characterized by similar age and defect size presented comparable results. The individual standardized effect sizes were combined into an overall best estimate. Its value was 1.678, measured in units of standard deviation, with the 95% confidence interval of [1.016; 2.340] resulting in different values of the average, to be expected treatment effect when it is measured in Lysholm Score (22.1), IKDC Score (26.5) and KOOS (15.2) points. Conclusions. Our results offer a clinically intuitive estimation of the average treatment effects on common clinical scales. Compared to the preoperative situation, a significant clinical improvement can be expected for each patient. Nevertheless, the magnitudes of these treatment effects are an approximation and must be interpreted cautiously. Furthermore, we did not succeed to confirm that young age and small lesion size have a beneficial effect on the clinical outcome

Introduction: Microfracture is found to be effective for isolated chondral defect of knee in young adults however controversy exists over the relevance of microfracture treatment in degenerative knee. The purpose of the study is to assess the outcome and patient satisfaction with the arthroscopic microfracture of osteoarthritic knee. Materials: We collected the demographic details of the patient, weight, procedure and follow up details from June 2000 to Dec 2002. We reviewed the case notes retrospectively and assessed the patient satisfaction with Oxford knee score and Lysholm score. Results: There were total of 194 patients but only 76 patients returned the questionnaire. The average patient age was 57 years (range: 27–87 years) with majority of the patients being male patients (43 patients). The average weight of the patient was 82 kg (44–119) and the mean duration of operation was 28 minutes. The patients were evaluated at an average follow-up of 51 months (range: 36–66 months). The average Lysholm score was 49 (4–100) with mean oxford score of 32.5(13–56). 21 patients required knee replacement at an average of 24 months (range: 6–48 months) from the initial microfracture. 17 out of 21 patients had kissing lesion and all showed grade 4 degenerative changes. Discussion: Age and weight does not appear to have any effect on microfracture treatment however more number of female patients seem to have more kissing lesion and poor result. With shorter duration of operation and proper explanation to patient give short term symptomatic relief before replacement surgery. It is worth considering this treatment for isolated grade 4 lesion or less than grade 4 degenerative chondral lesions in appropriately selected patients

Introduction. Bone marrow stimulation has been a successful treatment option in cartilage repair and microfracture was the procedure of choice since the late 1980s. Despite its success in young and active patients, microfracture has inherent shortcomings such as shallow channels, wall compression, and non-standardized depth and diameter. This in vitro study assessed bone marrow access comparing microfracture, 1 and 2mm K-Wires, 1mm drill, and a recently introduced standardized subchondral bone needling procedure (Nanofracture) that creates 9mm deep and 1mm wide channels. Methods. An adult ovine model was used to assess access to bone the marrow spaces as well as effects on bone following microfracture, nanofracture, K-wire, and drilling following ethical clearance. All bone marrow stimulation techniques were conducted on a full thickness articular cartilage defect on the medial femoral condyles by the same surgeon. The same groups were repeated in vitro in 4 paired ovine distal femurs. MicroCT (Inveon Scanner, Siemens, Germany) was performed using 3D reconstruction and 25 micron slice analysis (MIMICS, Materialise, Belgium). Results. Microfracture elicited shallow depth with bone compression surrounding the channels. Trabecular channel access was limited; the channel depth and diameter were non-standardized and highly user and instrument dependent. Nanofracture demonstrated deep cancellous bone perforation with a high number of open trabecular channels. K-Wire drilling with both diameters resulted in well-defined channel walls, outlined by fine osseous deposits. Trabecular channel access was limited. The diameter of bone perforation is standardized, but depth is defined by visual controls. 1mm drill bit reaming demonstrated better osseous evacuation, but still limited trabecular marrow access. Discussion and Conclusion. Nanofracture resulted in thin, fragmented cancellous bone channels without rotational heat generation. Compared to microfracture, drilling and K-Wire stimulation, nanofracture showed superior bone marrow access with multiple trabecular access channels extending 9mm into subchondral bone

Microfracture is a stem cell stimulation technique to promote the healing of full thickness articulate cartilage defects. Sixty-six patients have undergone microfracture for full thickness articulate cartilage defects over a five year period. All procedures were performed by one surgeon following the technique of Steadman. All patients were under 46, had an isolated chondral lesion, had a stable well aligned knee and were a minimum of one year post surgery. Patients were evaluated with a preoperative and follow up IKDC score, WOMAC score, KOOS Score, Tegner activity level and SF 36, VAS pain scores and overall knee function score. Failure was determined by the need for a secondary chondral procedure. Nine patients failed. Thus the overall success rate was 86%. The failure rate was higher with larger lesions. There was a significant improvement in IKDC score, WOMAC score, KOOS Score, Tegner activity level and SF 36, VAS pain scores and overall knee function score. Microfracture has a good success rate in the short to medium term with isolated full thickness articular cartilage lesions in the stable well aligned knee

Introduction: The results of treating chondral lesions with microfracture have been well documented. The lesion heals by fibrocartilage and the functional results tend to deteriorate through time. Hypothesis: The use of steroids an platelet rich plasma (PRP) as coadjuvants to microfracture for the treatment of full thickness chondral lesions improve the results of this marrow stimulating technique. Purpose: To macroscopically, histologically and molecularly evaluate the repair tissue generated after treating full thickness chondral lesions with microfracture and local steroids or PRP in an animal model. Materials: Experimental in-vivo study in 40 femoral condyles (FC) from New Zealand rabbits. Chondral lesions were induced in all the samples and divided into 4 groups:. Group 1: control, lesion left untreated. Group 2: microfracture. Group 3: microfracture + intraarticular betamethasone. Group 4: microfracture + PRP. Animals were sacrificed after 3 months and the samples were evaluated macroscopically, histologically (H and E, Toluidine Blue) and molecularly (RT-PCR for Col1 and Col2). The results were analyzed with ANOVA and Bonferroni tests (p< 0.05). Results: Macroscopy: the control group had no healing tissue. In all the other groups there was a variable presence of a fibrocartilaginous tissue without significant differences among groups. Histology: all the groups had the presence of fibrocartilage. Molecular analysis: all the groups had a significantly poorer Col2/Col1 relation when compared to normal hyaline cartilage, without significant difference among groups. Conclusions: The local use of betamethasone and PRP as coadjuvants to microfracture does not improve the macroscopical, histological and molecular results of the treatment of full thickness chondral lesions

Purpose: As a one-step surgical procedure, microfracture is frequently considered to be technically easier and associated with less postoperative morbidity than autologous chondrocyte implantation (ACI), which involves both arthrotomy and arthroscopy and therefore safety was assessed in patients with symptomatic cartilage lesions of the knee treated with characterized chondrocyte implantation (CCI) or microfracture. Methods: CCI (n=57) was compared to microfracture (n=61) in patients with grade III–IV symptomatic cartilage defects of the femoral condyles in a Phase III, prospective, multi-center, randomized, controlled trial. Safety assessments included adverse events (AEs), physical examination, vital signs, hematology and clinical chemistry. Results: At 18 months post-surgery, similar proportions of patients experienced AEs in the CCI (88%) and microfracture (82%) groups; 67% and 59%, respectively, experienced AEs considered treatment related. The AE profile was generally similar between groups, with no significant difference for hypertrophy, although significantly more CCI-treated patients had joint swelling (19% versus 4.9%; p=0.022) and treatment-related joint crepitation (12% versus 1.6%; p=0.028). Although the proportion of patients with severe AEs was similar for CCI (12%) and microfracture (13%), slightly more microfracture-treated patients experienced serious (life-threatening or requiring hospitalization) AEs (13% versus 8.8%). No patients discontinued due to AEs or died during the study. Conclusion: Contrary to general opinion, the two-step CCI procedure, involving arthroscopy followed by arthrotomy, has a similar safety profile to that of microfracture, a one-step arthroscopic procedure, for treating cartilage lesions of the knee

Introduction. Freiberg's infarction poses a challenge to foot and ankle surgeons. Several surgical and non surgical treatment methods are described. We performed a dorsal closing wedge osteotomy, debridement and microfracture of the metatarsal head. Dorsal closing wedge osteotomy helps in bringing the smooth plantar articular surface of the metatarsal head to articulate with the phalangeal articular cartilage whilst offloading the damaged dorsal articular cartilage. Debridement and Microfracture of the metatarsal head helps in regeneration of the damaged cartilaage via subchondral stem cells. Materials and Methods. Total of 15 patients (12F, 3M) underwent the above surgery between year 2002 and 2008. Mean age was 35yrs (range14-60). All of them had an extraarticular dorsal closing wedge osteotomy fixed with a single screw along with debridement of the joint and mocrofracture of the damaged cartilage. Post operatively heel weight bearing was allowed with a special shoe for 6 weeks. Serial radiological assessments were done to assess healing of the osteotomy and reshaping of metatarsal head. Patients had a mean follow up of 2.5 yrs (Range 1-6). All patients were assessed using subjective patient satisfaction scores (scale 0-10) and AOFAS scores. Results. 2nd metatarsal was involved in 14 and 3rd in one patient. All the osteotomies healed at a mean period of 10 weeks (range6-18). The mean patient satisfaction score was 8 (range5-10). The mean pre and post operative AOFAS scores were 54 and 82. One patient developed post operative haematoma which resolved spontaneously. No other complications noted. Conclusions. A combination of dorsal closing wedge osteotomy, debridement and microfracture is a simple, reproducible and effective method of treating Freiberg's disease with no major complications

Aims: The technique of microfracture for the arthroscopic treatment of articular cartilage (AC) defects has been shown to result in reparative tissue in the defect, however, retrieved tissues have demonstrated þbrocartilagenous material. The objective of this study was to evaluate the tissue types formed in AC defects in an ovine model treated by microfracture with a collagen- GAG-copolymer trilayer matrix consisting of collagen type I, II and III and autologous cultured cells. Methods: Sixteen adult sheep were used in the study following the protocol accepted by the Animal Care Commitee of the University. Two 4.5-mm diameter defects were produced in the medial condyle of the right knee, all AC was removed without penetrating the subchondral bone. In twelve animals microfracture was performed with a curved pick, in four of them without further treatment, in four the defect was covered by the collagen implant alone, and in four by the cell-seeded implant with cultured autologous chondrocytes from the left knee, 4 defects served as controls. After four months the knees were removed, parafþn sections were stained with H & E, Safranin O/fast green, alcian blue, azan, and antibodies to types I and II collagen. Results: All treatment groups showed better þlling of the defects than untreated knees. Histological analysis revealed the biggest amount of hyaline-like tissue in the cell augmented treatment group. Reparative tissue was predominantly þbrocartilage in the other groups. Conclusions: Collagen implants are able to increase the repair of chondral defects in combination with microfracture
