Objective. To define the common pathology of the hips with irreducible posterior dislocation combined with femoral head fracture and the outcome of surgical treatment using surgical hip dislocation technique. Design: retrospective observational clinical study. Setting: Level III referral trauma center. Patients/Participants: from January 2011till February 2014 five patients with irreducible posterior hip dislocation and femoral head fracture who underwent operation were included and they followed for at least 18 months. Intervention. Open reduction and internal fixation of fractured femoral head and labral repair by suture anchors using surgical hip dislocation through trochanteric flip osteotomy approach. Main Outcome Measures: Clinical and radiographical findings of the irreducible posterior hip dislocation, intraoperative findings, clinical outcomes using Merle d'Aubigné & Postel and Thompson & Epstein scores, and radiological outcome. Results. All patients presented clinically with a shortened lower limb in neutral or external rotation of the hip (not in Internal rotation). All were Pipkin type II fracture of femoral head with the intact part of the head buttonholed on the posterior wall of the acetabulum through a capsule-labral flap. Postoperative computed tomography revealed perfect reduction except one case with severe comminution with good reduction. Only one patient with delayed operative management developed avascular necrosis and underwent total hip arthroplasty. Conclusion. Irreducible femoral head fracture-dislocation is rare injury with different clinical presentation that shows neutral or externally rotated limb and optimal surgical management is not clear. Surgical hip dislocation gives full access to the femoral head for reconstruction and opportunity to direct repair of the labral tears

Since 1977 we did implant ceramic on ceramic THR in younger and active population. In 1991 we published (JBJS B N°4) data's in a consecutive group of patients under 50 years of age. What about the same patients for more than 30 years? Eighty-six hips in 75 patients, 41 males 34 females, mean age 43 (18 to 50), mean weight 68 kg (36 to 100), Charnley class: 38 A, 28 (38 hips) B, 9 patients (10 hips) C. Sixty-six primary procedures, 20 revisions (18 failed arthroplasties: 6 THR, 5 resurfacing, four single cup, two hemiarthroplasty, one bipolar), one after acetabular fractures. Four hips previously infected. Eight Patients deceased (8 hips) prosthesis still in place, ten lost to follow-up before 2 years, eight hips in 8 patients were revised before the review, partially followed: from 2 to 20 years: 35, completely followed: 25 hips in 23 patients resuming in: No pain in 20, slight pain in 2, severe disability in 3 not related to the hip, no radiolucent lines in 22, radiolucent lines in 3, no osteolysis in 25. Revision for: early sepsis in one, socket loosening in 8 (3 revisions cases), femoral head fractures in 2: one extra small head (22mm) for Crowe 3 DDH, one fractured at 24 years. Inertness, stability related to fibrous tissue generation, no noise

Since its creation, labral repair has become the preferred method among surgeons for the arthroscopic treatment of acetabular labral tears resulting in pain and dysfunction for patients. Labral reconstruction is performed mainly in revision hip arthroscopy but can be used in the primary setting when the labrum cannot be repaired or is calcified. The purpose of this study was to compare the survival between primary labral repair and labral reconstruction with survival defined as no further surgery (revision or total hip replacement). Patients who underwent labral repair or reconstruction between January 2005 and December 2018 in the primary setting were included in the study. Patients were included if they had primary hip arthroscopy with the senior author for femoroacetabular impingement (FAI), involving either labral reconstruction or labral repair, and were within the ages of 18 and 65 at the time of surgery. Exclusion criteria included confounding injuries (Leggs Calves Perthes, avascular necrosis, femoral head fracture, etc.), history of unilateral or bilateral hip surgeries, or Tönnis grades of 2 or 3 at the time of surgery. Labral repairs were performed when adequate tissue was available for repair and labral reconstruction was performed when tissue was absent, ossified or torn beyond repair. A total of 501 labral repairs and 114 labral reconstructions performed in the primary setting were included in the study. Labral reconstruction patients were older (37±10) compared to labral repair (34±11).(p=0.021). Second surgeries were required in 19/114 (17%) of labral reconstruction and 40/501(8%) [odds ratio: 2.3; 95% CI 1.3 to 4.2] (p=0.008). Revision hip arthroscopy were required in 6/114(5%) labral reconstructions and 33/501(6.5%) labral repair (p=0.496). Total hip replacement was required in 13/114 labral reconstructions and 7/501 labral repairs [odds ratio:9.1 95%CI 3.5 to 23] (p=< 0.01). The mean survival for the labral repair group was 10.2 years (95%CI:10 to 10.5) and 11.9 years (98%CI:10.9 to 12.8) in the labral reconstruction group. Conversion to total hip was required more often following primary labral reconstruction. Revision hip arthroscopy rates were similar between groups as was the mean survival, with both over 10 years. Similar survival was seen in labral repair and reconstruction when strict patient selection criteria are followed

Fracture of the femoral head usually results from high-energy trauma, particularly motor vehicle accidents. These fractures are often associated with poor functional outcomes. SF-36 and MFA scores were obtained for twenty-five femoral head fractures. The mean SF-36 scores was 57.2 (22.6 to 82) and the mean MFA score was 37.6 (10–72). As expected the scores were negatively correlated (−0.587, p=0.005). The presence of an ipsilateral posterior acetabluar wall fracture negatively affected functional outcome. Fragment excision was associated with a higher functional outcome when compared to internal fixation. Posterior surgical approach resulted in higher scores than patients who underwent an anterior procedure. Fracture of the femoral head is a rare but severe injury. The purpose of this study is to determine the functional outcome of and prognostic factors associated with femoral head fractures. Acetabular wall fracture, surgical approach, and method of surgical treatment are prognostic factors of functional outcome as measured by the SF-36 and the MFA. There is currently no consensus on the management of femoral head fractures and treatment practices have evolved on the basis of a limited series of studies. Functional outcome following femoral head fracture has been evaluated using Epstein’s criteria in several studies, however, only one study has been conducted using a validated outcome measure. Twenty-five femoral head fractures in twenty-four patients were identified from the trauma and orthopaedic databases of two major trauma centres. The patient with bilateral femoral head fractures was not included in the analysis. After obtaining informed consent, the patients’ medical records were reviewed. Functional outcome was assessed using two validated, patient-based outcome measures, the Short Form (SF)-36 and the Musculoskeletal Functional Assessment instrument (MFA). The average age of the patients was 36.8 (std. dev. 11.1) and the mean length of follow-up was 41.4 months (minimum twelve months). Three factors were identified using non-parametric analysis that significantly affected the functional outcome scores:. The presence of an ipsilateral posterior acetabluar wall fracture negatively affected functional outcome (p=0.08). Fragment excision resulted in significantly higher scores when compared with the patients treated by internal fixation (p=0.067). A posterior surgical approach resulted in a better functional outcome than the anterior approach (p=0.013)

Purpose: Fractures of the femoral head are relatively uncommon injuries and usually occur following a traumatic dislocation of the hip joint. The purpose of this study was to evaluate self-reported functional outcome of patients who have sustained a femoral head fracture. Methods: A search of the trauma database at a Level I trauma center between the years 1987–2003 was conducted. Sixty two patients, sustaining 63 femoral head fractures were identified. Two patient-based outcome measures, the Short Form-36 (SF-36) and Short Musculoskeletal Functional Assessment (SMFA) were used to evaluate functional outcome. Forty patients were lost to follow-up including three deaths. Results: To date twenty-three subjects (13 male, mean age 37.3 +/− 15.6), with 24 femoral head fractures have complete functional outcome data. Ten hips (42%) were classified as Pipkin type I, 12 (50%) type II, 1 (4%) type III, and 1 (4%) type IV. The mean follow-up was 10.3 +/− 5.4 yrs. Mechanism of injury included motor vehicle collisions (18/24), and falls from height (6/24). The mean ISS was 11.3 +/− 5.3. Fifteen subjects were treated operatively (13 internal fixation, 1 excision, 1 open reduction only). Four hips failed initial treatment and required delayed total hip arthroplasty (THA). Subjects (n=20) not having a THA functioned below the Canadian normal population for SF-36 physical component score (mean 44.5 +/− 11.0, p < 0.03). Conclusions: This is the first report of patient-based functional outcome following treatment of femoral head fractures. Physical function was significantly lower when compared to Canadian population norms

The purpose of this study was to compare the effect of femoral head material (delta ceramic versus metal) on polyethylene wear in a consecutive prospective randomized series of low friction total hip arthroplasty. A total of 110 patients with a mean age of 60.6 ± 9.3 (34–75) years were randomized (power of 90%, alpha of 5%) to receive either a metal (55 hips) or a delta ceramic (55 hips) femoral head. The polyethylene socket was moderately cross-linked (3 Mrads of gamma radiation in nitrogen) and annealed at 130°C in all hips. All other parameters were identical in both groups. The primary criterion for evaluation was linear head penetration measurement using the Martell system, performed by an investigator blinded to the material of the femoral head. Creep and steady state wear values were calculated. At the minimum of 3-year follow-up, complete data were available for analysis in 38 hips at a median follow-up of 4.4 years (3.0 to 5.7), and in 42 hips at a median follow-up of 4.0 years (3.0 to 5.4) in the metal and delta ceramic groups, respectively. The mean creep, measured as the linear head penetration at one year follow-up, was 0.42 ± 1.0 mm in the metal group versus 0.30 ± 0.81 mm in the delta ceramic group (Mann and Whitney test, p = 0.56). The mean steady state penetration rate from one year onwards measured 0.17 ± 0.44 mm/year (median 0.072) in the metal group versus 0.074 ± 0.44 mm/year (median 0.072) in the delta ceramic group (Mann and Whitney test, p = 0.48). No case of delta ceramic femoral head fracture was recorded, and no hip had signs of periprosthetic osteolysis. This study demonstrated that up to 5-year follow-up, delta ceramic femoral head did not significantly influence creep neither wear of a contemporary annealed polyethylene. Longer follow-up is necessary to further evaluate the potential clinical benefits of delta ceramic

Femoral head fractures i.e. Pipkin fractures are uncommon injuries and there are very few large series in literature with reported outcomes. There remain many controversies regarding diagnosis and management. This study, which is the largest single surgeon experience, is an attempt to get answers to some of these controversies. This series is an analysis of 51 patients with femoral head fractures. There were 44 males and seven females. The right side was involved in 36 and left in 16 patients. According to Pipkin classification these were 13 Pipkin-I, 27 Pipkin-11, three Pipkin-111 and eight Pipkin-IV fractures. Thirty-two patients were managed by surgical intervention. The surgical approach was posterior in Pipkin-I and in seven cases of Pipkin-II fractures. Another eight Pipkin II cases were managed surgically by anterior Smith-Peterson approach while another eight fractures were accessed by posterior approach with flip osteotomy. The Pipkin III and IV cases were managed using surgical approaches that varied depending on the pattern of associated acetabular injury. The fractured fragment, if small, was excised and, if large, was re-fixed using small fragment partially threaded cancellous screw. Follow-up of two to eight years was available in 39 cases. Using Thompson and Epstein criteria, 26 patients were rated as having good results, eight fair and five poor results. Early osteoarthritic changes were seen in five patients, avascular necrosis of the femoral head in three patients and one patient had re-fracture in same hip during an epileptic fit with subsequent fixation problems. Of four patients with sciatic nerve injury, two had persisting motor deficit. There was one case of heterotopic ossification. Most Pipkin-I fractures can be managed by closed reduction, Pipkin-II fractures usually require ORIF. The best results have been obtained by a Smith-Peterson approach if the hip has already been reduced, but posterior approach with flip osteotomy offers the best exposure if the hip is still unreduced. Pipkin III patients need hip replacement if presentation is late, while ORIF gives acceptable outcome in Pipkin IV fractures

Recent issues related to trunionosis have created a new paradigm in choosing femoral head material in total hip arthroplasty. While many consider highly-crosslinked polyethylene (XLPE) to be the gold standard currently in acetabular liner bearing surface, the debate remains whether metal or ceramic heads are best paired with XLPE. Wear characteristics are similar within an order of magnitude when comparing cobalt chrome femoral heads with ceramic when used in combination with XLPE. Therefore, discernable differences between the two femoral head materials with respect to outcomes would be the result of other biomechanical factors. Notably the fretting and corrosion of metal heads at the modular taper of femoral components is a serious concern and represents a significant deterrent when considering this material. The fretting corrosion that occurs with metal femoral heads has recently been well documented in multiple reports, and can be associated with adverse local tissue reactions necessitating revision hip arthroplasty. Frictional torque has recently been implicated in taper corrosion at modular junctions. In a recent simulated in vivo study, large diameter CoCr femoral heads were associated with increased frictional torque compared to smaller metal heads, supporting recent taper corrosion retrieval studies. In one recent series, a 1.1% incidence of head-neck taper corrosion with a metal head was reported and the authors recommended use of ceramic femoral heads. The notable downside of ceramic femoral heads is the implant cost and potential for fracture. However, the incidence of femoral head fracture with the newer mixed delta ceramic heads is exceptionally low and infrequent (rate 1.7 per 100,000). Furthermore, the incidence of taper corrosion is negligible with ceramic heads, making it the bearing couple of choice among many surgeons in combination with XLPE

Purpose: Fracture-posterior dislocation of the femoral head is an exceptional hip injury. Emergency reduction is required. Relocation into the acetabular cavity of the displaced femoral head may not be feasible. Irreducibility, instability, and more rarely accidental fracture of the femoral neck may also occur. We encountered this latter complication in four patients and report here its frequency and mechanism and propose preventive therapeutic measures. Material and methods: Seventy dislocations and fracture-dislocations of the hip were treated in our unit from March 1997 to February 2003. Among these cases, fourteen hip dislocations were complicated by femoral head fractures. Fracture of the femoral neck occurred during reduction in four. All four cases occurred in men, mean age 49.7 years, who were traffic accident victims (drivers or passengers). There were two Pipkin IV fracture-dislocations and two Pipkin II. The first reduction, achieved under general anaesthesia in an emergency setting, was performed by an orthopaedic surgeon in one patient and a general surgeon in three patients. Arthroplasty was used to treat the femoral neck fracture in three patients and pinning in one. We reviewed retrospectively the clinical and imaging data before and after reduction. Results: Sub-capital fracture situated 4.0 cm (mean, range 3.5–4.5 cm) from the lesser trochanter occurred in all four cases. The head remained attached above and posteriorly to the acetabulum and was rotated less than 90°. The fragment remaining in the acetabulum was displaced in two cases. In one patient, the fracture-dislocation of the head was associated with a fracture of the posterior rim of the acetabulum. Discussion: Neck fracture during reduction of traumatic hip dislocation is a serious complication. Prevention of this iatrogenic event requires a slow, progressive reduction limiting the trauma to a minimum; first intention open surgery may be required in selected cases

Young patients have been reported to have a higher risk of revision following total hip arthroplasty than older cohorts. This was attributed to the higher activity level which led to increased wear, osteolysis, and component fracture. We prospectively assessed the clinical results, wear and osteolysis, the incidence of squeaking, and the survivorship of ceramic on ceramic THA in patients younger than 50 years (mean age of 42 [18–50] years). The series included 425 THAs in 370 patients with 368 hips followed for a minimum of 2 years (mean 7.1 years, range 2–14 years). All patients received uncemented acetabular components with flush-mounted acetabular liners using an 18 degree taper. No osteolysis was observed in any uncemented construct. There was osteolysis around one loose cemented femoral component. The survivorship for reoperation for implant revision was 96.7%. There were only two acetabular liner fractures (0.47%) and one femoral head fracture (0.24%). Two of the three fractures involved a fall from a significant height. There were no hip dislocations. Five patients (1.17%) noted rare or occasional squeaking. None had reproducible squeaking. In summary, the current study shows that ceramic-on-ceramic THAs in the young patient population are extremely reliable with a very low revision rate and an absence of wear-induced osteolysis. In addition, it shows that both bearing fracture in this young patient population typically occurs with polytrauma and squeaking issues that have been raised relative to ceramic bearings occur very rarely with the flush-mounted ceramic liner design used in this study

This study shows the radiographic results of total hip arthroplasty (THA) using Revelation stem ® (Djo surgical USA) for hip joint disease. We removed cases which we couldn't follow up, the remaining of the patients who had undergone a primary THA July 2007 to December 2009 in our institution using Revelation hip stem® and 58 cases-65 hips (14 men and 44 women) were possible for progress observation on this study, and radiographic evaluation was done at the time of the last observation. The preoperative diagnosis of the hip included osteoarthritis (OA) in 40 patients, idiopatic, necrosis of the hip in 13 patients, Rheumatoid arthritis (RA) in 4 patients, and femoral head fracture in 1 patient. Three patients had undergone femoral head replacement (FHA) and 55 patients had undergone THA. At the time of the last evaluation, spot welds were detected in 63 hips (97%) in zone1 and 7. Demarcation lines, which indicates movement of the stem, were detected in zone3,4,5, but not in the proximal of the stem. Stress shielding were observed in 32 joints, according to Engh's classification, first degree were 27 joints and second degree were 5 joints. Cortical hypertrophy were detected in 13 joints. The revelation stem features a lateral overhang structure (lateral flare) and the stem has an expanded proximal geometry allowing the device to rest on both medial and lateral cortices at the metaphyseal level. Above all the stem has anterior overhand structure (trapezoidal structure). The medullary cavity occupation rate is made to increase by these structures. Above all, the stability of the stem is increased by a proximal portion by three point support. The distal stem is tapered and polished, which prevents the stress loading to a distal portion of the device. Stress shielding more than the third degree is not detected, and spots welds were detected in the proximal of the stem. These facts indicate that the concept of this stem, transmit loading to the femur bone, at the proximal portion of the stem, would be achieved

Purpose of the study: The purpose of this prospective study was to evaluated the risk of fracture of 22.2 mm Delta ceramic heads. Material and methods: A preclinical study was performed on twenty 22.2 mm Delta ceramic femoral heads with a medium neck with 20 22.2 mm Delta ceramic femoral heads with a short neck. A V40TM cone was used in all cases with a 5 40 angle. In vitro tests consisted in the assessment of the fracture force under static pressure before and after stress tests, and with a static force shock test (Charpy model) simulating a microseparation during subluxation phenomena. Between April 2007 and April 200, a consecutive series of 55 composite Delta heads were used in 55 patients undergoing cemented total hip arthroplasty (THA). A polyethylene cup was sterilised under vacuum at 3 Mrads with a post-radiation temper (Duration. ®. ), and a stainless steel femoral piece with a highly polished surface and a V40TM Morse cone (Legend. ®. ). This series issued from a randomised prospective study designed to compare wear with 22.2mm Delta ceramic heads with stainless steel heads with the same diameter. Inclusion criteria were age < 75 years, degenerative disease on naive hip and patient residing in France. Mean patient age was 59.2±6.9 years (range 44–70). Results: The resistance of the 22.2 mm heads was significantly less during the static tests before and after the stress tests. Nevertheless, the mean resistance was higher than the FDA recommendations of 46K. The tests simulating a microseparation showed a significantly superior resistance for the 22.2 mm heads. All patients had from 1 to 2 years follow-up (usual delay for 80% of in vivo ceramic fractures). There were no cases of femoral head fracture in this series. There were no cases of early wear at this same follow-up and no case of femoral or acetabular osteolysis. Discussion and Conclusion: The results of this study indicate that the resistance of the 22.2mm ceramic Delta heads is very much superior to the recommendations for in vitro tests. In this series, the risk of fracture in vivo remained nil to two years follow-up. The pertinence of this ceramic implant for decreasing polyethylene wear in vivo is under evaluation

Aims

Femoroacetabular impingement (FAI) describes abnormal bony contact of the proximal femur against the acetabulum. The term was first coined in 1999; however what is often overlooked is that descriptions of the morphology have existed in the literature for centuries. The aim of this paper is to delineate its origins and provide further clarity on FAI to shape future research.

Methods

A non-systematic search on PubMed was performed using keywords such as “impingement” or “tilt deformity” to find early anatomical descriptions of FAI. Relevant references from these primary studies were then followed up.

Introduction. The purpose of this study was to examine if Sugioka's transtrochanteric rotational osteotomy (TRO) of the femoral head could be a valid option in treating idiopathic osteonecrosis of the femoral head. Methods. Thirty-two TROs performed in 29 patients between 1985 and 2006 were studied. Patient age at operation ranged from 16 to 56 years (average, 36 years). Thirteen hips were in women and 19 hips in men. Height was 165 cm and weight 63 kg (Body Mass Index 23) on average. Risk factors were corticosteroid use in 18 hips and excessive alcohol consumption in 15 hips (2 of them had both backgrounds), while neither of them was found with 1 hip. Bilateral hips were affected in 19 patients and 3 of them underwent bilateral TROs sequentially. The femoral head was rotated anteriorly in 26 hips and posteriorly in 6 hips. For fixation of the osteotomy site, large femoral screws had been used till 1995 (Group 1, 9 hips), and since 1996 an AO angle plate or a compression hip screw has been used (Group 2, 13 hips). Since 2003, pre-operative planning was performed more meticulously and the distal part of the joint capsule was cut after osteotomy as described by Atsumi (Group 3, 10 hips). The average follow-up period was 6.5 years (range, 2 to 21 years). Results. The average Japanese Orthopaedic Association hip score was 55 points pre-operatively, 63 points at 1 year, and 61 points at the latest follow-up. With pain increase, re-collapse of the rotated femoral head, fracture of the proximal fragment, nonunion, trochanteric detachment, joint space narrowing, osteoarthritic changes, or infection as the endpoint, the survival rate was 58% at 1 year, 53% at 4 - 9 years. The endpoint developed in 14 hips (re-collapse of the rotated femoral head in 9, fracture of the proximal fragment in 3, joint space narrowing in 1, and infection in 1). The changes in the surgical techniques were suggested to have improved the 3-year survivorship: 25% in Group 1, 54% in Group 2, and 74% in Group 3. The 4- to 9-year survival of this procedure was 53%. The majority of the endpoints developed within the first year, which may suggest that most of them were surgery-related. Conclusion. This procedure requires precise and meticulous precautions in pre-operative planning, preservation of the posterior column vessels during surgery, capsulotomy, and after-treatment. With those precautions, this procedure could be a valid joint-reserving option in treating idiopathic osteonecrosis of the femoral head. TRO is a highly difficult procedure and should be performed only by surgeons who have full training

Mechanical failure in total hip arthroplasty is usually due to aseptic loosening related to wear particles as seen with polyethylene bearing. Alumina has been proposed for avoiding wear problems. In vitro and mid-term clinical studies showed tribological advantages but early acetabular fixation issues. Since alumina on alumina bearing is currently used with new fixation techniques, updated evaluations of the ancient series are informative regarding the long-term tolerance of alumina in vivo. In this paper, we investigated 104 consecutive lumina on alumina cemented total hip arthroplasties (CER-VAER-OSTEAL, Roissy, France) implanted 20 years ago in 81 patients (from 1979 to 1983). Alumina femoral head was 32 mm in diameter. Alumina acetabular socket and titanium femoral stem were cemented. The clinical evaluation used Postel Merle d’Aubigné score. Radiological wear and appearance of osteolysis or loosening were noted for establishing actuarial curves. When accessible, histological samples from revision procedures were analyzed. Six infected cases were not taken into account later. The average follow-up was 11 years, reaching 18 years in 38 cases. Twenty-three hip were revised for changing 23 acetabular sockets, 12 femoral heads, and 1 femoral stem. We noted 1 femoral head fracture, 24 definite ace-tabular loosenings, 12 probable acetabular loosenings, and 3 definite femoral loosenings. Radiological acetabular osteolysis was present in 4 cases, always limited to De Lee zone 1, and associated with loosening. Radiological wear was below eye detection. Peri-prosthetic tissue showed non-specific histological reaction to cement particles. Survival rate at 20 years was 61.4% in term of revision (57.1% and 95.2% concerning acetabular and femoral defininte loosening). Beside the high rate of cemented fixation failure of the socket, loosened and non loosend cases showed an excellent tolerance of alumina on alumina bearing in the long-term, with minimal wear and osteolysis. This may also have protected the femoral component from complications

A tantalum AVN implant was used in sixteen patients with advanced AVN (Grade 3/4). No reports have been published of use of this implant in advanced disease. Outcomes included radiological, SF36, Harris hip score and secondary surgeries. HHS improved from fifty-two to seventy. SF36 scores approached controls. At over one year average follow-up five patients are revised to THA, however, all hips except one have at least minor pain. Revisions occurred in older patients or those with 100% head involvement. Certain acetabular fractures involve impaction of the weight-bearing dome and medialisation of the femoral head. Intra-operative fracture reduction is made easier by traction on the limb, ideally in line with the femoral neck (lateral traction). However, holding this lateral traction throughout surgery is very difficult for a tiring assistant. We detail a previously undescribed technique of providing intra-operative lateral femoral head traction via a pelvic reduction frame, to aid fixation of difficult acetabular fractures. The first 10 consecutive cases are reviewed (Group 1) and compared with a retrospective control (Group 2, n=18) of case-matched patients, treated prior to introducing the technique. The post-operative X-rays and CT scans were assessed to identify quality of fracture reduction according to the criteria of Tornetta and Matta. Operative time, blood loss and early complication rates were also compared. All cases in both groups were acute injuries with medial and/or superior migration of the femoral head. The majority were either associated both column or anterior column posterior hemi-transverse. There was no statistical difference between the groups in age, time to surgery, BMI or ASA grade. Fracture reduction was assessed as excellent in seven, good in three and poor in one. This was not significantly different from the control group (p=0.093). The mean operative time was 232 minutes in Group 1 and 332.78 minutes in Group 2 (p = 0.0015). There was no difference between the groups for blood loss or complication rates. We conclude that this new technique is at least equivalent to using manual traction and early results suggest it reduces operative time and technical difficulty in treating these complex acetabular fractures

Introduction. Hip resurfacing (HRA) designer centres have reported survivorships between 88.5–96% at 12 years. Arthroplasty Registries (AR) reported less favourable results especially in females gender and small sizes. The aim of this study was to evaluate the minimum 10-year survival and outcome of the Birmingham Hip Resurfacing (BHR) from an independent specialist centre. Methods. Since 1998, 1967 BHRs have been implanted in our centre by a single hip resurfacing specialist. The first 249 BHR, implanted between 1999 and 2001 in 232 patients (17 bilateral) were included in this study. The majority of the patients were male (163; 69%). The mean age at surgery was 50.6 years (range: 17–76), with primary OA as most common indication (201; 81%), followed by avascular necrosis (23; 9.2%) and hip dysplasia (11; 4.4%). Mean follow up was 10.2 years (range: 0.1 (revision) to 13.1). Implant survival was established with revision as the end point. Harris Hip Scores (HHS), radiographs and metal ion levels were assessed in all patients. Sub-analysis was performed by gender, diagnosis and femoral component size (Small: <50 mm; Large: ≥50 mm). Results. Of the 232 patients, 15 were deceased (4 bilateral BHR), 16 lost to follow-up and 9 revised. 205 BHR were evaluated at minimum 10 years postoperatively. Failure modes included 2 component malpositioning, 2 loose femoral heads, 1 fracture, 1 metal sensitivity, 2 impingement and 1 with high metal ions. The overall survival was 95.1% (95% CI: 93.6–96.6) at 12.8 years. The mean HHS was 97.8 (range: 65–100). Survivorship in men was 98.6% (95%CI: 97.4–99.8%) at 13 years. Survivorship in women was inferior to men (log rank = 0.003): 87.9% (95%CI: 84.3–91.5%) at 12 years. There was no difference in HHS between genders in the non-revised cases (p = 0.46). There was no difference in survivorship with different pre-operative diagnosis (log-rank = 0.83) but a significant difference in 12-year survivorship between Small (90.1%) and Large components (97.2%) (log rank = 0.01). After adjusting for head size, the difference in survival between males and females was no longer significant (log-rank = 0.125). The median ion levels were Cr:2.0μg/l; Co:1.0μg/l. In 24 patients the ion levels were undetectable. Four patients (1.9%) had ions above the upper acceptable limits of Cr:4.6μg/l;Co:4.0μg/l for unilateral or Cr:7.4μg/l;Co:5.0μg/l for bilateral HRA. In 67 patients with consecutive ion measurements, levels decreased significantly with time with a mean decrease of 0.97μg/l for Cr and 0.60μg/l for Co. Discussion. This study reports the more than 10-year survival of BHR and reflects an experienced specialist's practice, including his learning curve of the procedure. The overall 12.8-year survival was superior to registry reported figures of THA amongst young patients and corresponded well with reports from designer centres. Survivorship and clinical outcome were excellent in men. In women survivorship was significantly inferior and related to smaller component sizes, but the >10-year clinical outcome in non-revised cases was excellent. In well-functioning BHR, the metal ions decrease significantly with time. The results of this study support the use of HRA with a good design

Introduction and Aims: Aberrations in the balance of chondrocyte metabolism play an integral role in the degeneration of articular cartilage and subsequent osteoarthritis. Gene expression profiling allows a comparison of levels of mRNA expression in large numbers of genes simultaneously. This study compares the mRNA expression from osteoarthritic cartilage in knees and hips with that of normal cartilage. Method: Human cartilage samples were obtained from osteoarthritic knees and hips at the time of joint arthroplasty surgery. ‘Normal’ cartilage was obtained from femoral heads after fracture or from radial heads after trauma. Cartilage samples were either snap frozen in liquid nitrogen or enzymatically digested and established in primary cell culture prior to RNA isolation. The RNA was reverse-transcribed to cDNA, labelled with a fluorochrome and then hybridised to gene chips. Results: In addition to confirming that cells raised in primary cell culture dedifferentiate to a fibroblast-like state and cease to synthesise normal products of cartilage matrix we have also developed a reproducible method of processing snap frozen cartilage samples in order to produce a sufficiently pure quantity of mRNA to be used in gene chip technology. We now have gene chips completed for a ‘normal’ control, a standard osteo-arthritic knee and an osteoarthritic hip with a significant genetic history of early onset osteoarthritis. Early analysis and comparison of the data from these chips identifies some potential candidate genes for further analysis. Conclusion: Human articular cartilage lends itself to gene profiling using cDNA arrays as it contains only one cell type. Thus any changes in gene expression levels can be directly attributable to the chondrocyte. This early data analysis opens the door to a new search for the ‘arthritis gene’. For the data to be meaningful we will need to process gene chips on several more samples of arthritic and ‘normal’ cartilage
