Patients had traditionally relied on health care professionals for advice and treatment options for most orthopaedic conditions including degenerative lumbar spine disease. However the unprecedented access to heath care information offered by the internet is changing the way how patients gather information and make treatment choices. Aims & Methods: The purpose of this study was to determine the influence of the internet on patients presenting to orthopaedic clinics with degenerative lumbar spine disease and its influence on decision making. A power calculation was done to determine appropriate sample size needed for the study. Questionnaires were handed to willing patients who were attending back clinic for more 6 months and diagnosed to have degenerative lumbar spine disease. Each participant filled a 25 point survey and a total of 105 surveys were collected. Results: Out of the 79% who had access to the internet, 55% accessed it from home which is in comparison to the national statistics of 57%. The rest of 24% access internet at other places. Internet usage was found to be directly proportional to education and earnings with 100% in professionals and then dropping significantly. 80% living in urban areas used internet to access health information. Topics commonly searched include causes (74%), symptoms (70%), treatment/surgical options (82%). Only few people looked at the choice of surgeons (30%) and hospitals (30%) as the practice is mostly NHS based. Around 50% made clinical decisions based on their search. 86% used general search engines like yahoo, google, 14% used sites like med line, NHS. UK. Health professionals had more influence in decision making than internet (80%). Conclusions: Internet is evolving as a powerful source of health information and influencing more an more patients in clinical decision making with a 5% growth in internet usage every year (national statistics)

The purpose of this retrospective study, is to demonstrate the survivorship and clinical effectiveness of the Wallis implant, against low back pain and functional disability in patients with degenerative lumbar spine disease. The Wallis Interspinous implant, was developed as a minimally invasive and anatomically conserving procedure, without recourse to rigid fusion procedures. The initial finite element analysis and cadaver biomechanical studies showed that the Wallis ligament improves stability in the degenerate lumbar motion segment. Unloading the disc and facet joints reduces intradiscal pressures at same and adjacent levels allowing for the potential of the disc to repair itself. A total of 157 patients who had wallis ligament insertion between 2003 and 2009 were reviewed, with a mean age of 54 and were followed for 48 months on average. Patients were assessed pre-operatively and post-operatively every 6 months by VAS pain score, Oswestry Disability Index and SF-36. 90% of patients improved, to show a minimal clinical difference, compared to the pre-operative evaluation. There is overall 75-80% good clinical outcome. Low infection rate of 1.1%. Two cases of prolapsed discs at the same level requiring further discectomy, 7 required fusion. No fractures or expulsions. The Wallis implant represents a safe non-fusion stabilisation device in the treatment of degenerative lumbar spine disease with canal stenosis. There is less soft tissue damage, quick rehabilitation, less morbidity and associated low complication rate

Aims & Methods: The purpose of this prospective study is to demonstrate the survivorship and clinical effectiveness of Wallis implant against low back pain and functional disability in patients with degenerative lumbar spine disease. Patients were assessed pre operatively and post operatively every 6 months by VAS pain score, Oswestry Disability Index, SF-36. All the patients had pre operative radiographs, MRI scans and followed up with interval radiographs. The results were assessed in three sub groups. Group-1 is decompression and stabilisation, group-2 is stabilisation alone, and group- 3 is “Topping off” a fusion. Results: A total of 211 Wallis Ligaments were inserted in 203 patients between July 2003 and November 2006. In total 179 patients were reviewed with mean age of 54(24–85) were followed for an average 30 months (6–40). The most common level is L4/5 (59%) followed by L3/4. In all the subgroups pain scores and oswestry disability index improved by 50%. And similarly SF-36 scores improved. There is 75–80% good clinical outcome with a survivorship of 98–99%. Low infection rate of 1.1%. Two cases of prolapsed discs at the same level requiring further discectomy and one case of iatrogenic L4 paraesthesia. Conclusions: The Wallis ligament represents a successful non fusion alternative in treatment of degenerative lumbar spine disease with least soft tissue damage, quick rehabilitation, less morbidity and associated low complication rate. The Wallis implant treats pain, preserves mobility, anatomy and stability while being fully reversible, therefore leaving all subsequent options open

Aims: To evaluate the outcome of surgery in patients with lumbar spine degenerative disease or isthmic spon-dylolisthesis. Methods: Lumbar spine fusion with or without decompression was performed in 132 consecutive patients. Altogether 115 patients attended to follow-up an average 3.4 years after operation. The overall result was assessed by the patient and by an independent observer. Moreover, the disability was measured using Oswestry questionnaire and fusion rate was assessed from flexion and extension radiographs. Results: The complications of surgery were: misplacement of screws in 2 patients, paresis of peroneal nerve in 1 patient, superficial wound infection in 4 patients, breakage of fixation device in 1 patient and spinal fluid fistulas in 2 patients. According to patient’s own opinion the result of surgery was excellent, good or fair in 86 percent of the patients and even 89.6% of them benefited from the surgery. The average postoperative Oswestry score was 28. According to radiographs the fusion rate was 93 percent. At the time of operation 40 patients were retaired and 66 (93%) out of the 75 patients who were still at working age returned to work. Conclusion: Lumbar spine fusion is an effective and safe treatment of degenerative lumbar spine disease and isthmic spondylolisthesis. However, the careful selection of patients is an important factor for success

Pedicle screw fixation is an effective and reliable method for achieving stabilization in lumbar degenerative disease. The procedure carries a risk of violating the spinal and neural canal which can lead to nerve injury. This audit examines the accuracy of screw placement using intra-operative image guidance.

Retrospective audit of patients undergoing lumbar pedicle screw fixation using image guidance systems over an 18-month period. Case records were reviewed to identify complications related to screw placement and post-operative CT scans reviewed to study the accuracy of screw position.

Of the 98 pedicle screws placed in 25 patients, pedicle violation occurred in 4 screw placements (4.1%). Medial or inferior breach of the pedicle cortex was seen in 2 screws (2%). Nerve root injury as a consequence of this violation was seen in one patient resulting in irreversible partial nerve root dysfunction. Mean set up time for the guidance system was 42 minutes. The mean operative time was 192 minutes.

Violation of either the medial or inferior pedicle cortex during placement of fixation screws is a rare, but potentially serious complication bearing lasting consequences. Image-guided placement can be helpful and possibly improve accuracy; particularly in patients with distorted spinal anatomy.

Due to the presence of megakaryocytes, platelets and clotting factors, bone marrow aspirate (BMA) tends to coagulate. For the first time, starting from our previous studies on mesenchymal vertebral stem cells, it has been hypothesized that coagulated BMA represents a safe and effective autologous biological scaffold for bone regeneration in spinal surgery. The present research involved advanced preclinical in vitro models and the execution of a pilot clinical study. Evaluation of cell morphology, growth kinetics, immunophenotyping, clonogenicity, trilineage-differentiation, growth-factors and HOX and TALE gene expression were analyzed on clotted- and un-clotted human V-BMA. In parallel, a pilot clinical study on ten patients with degenerative spine diseases submitted to instrumented posterior arthrodesis, is ongoing to assess the ability of clotted-V-BMA to improve spinal fusion at 6- and 12-months follow-up. Results demonstrated that clotted-V-BMA have significantly higher growth-factor expression and mesenchymal stem cell (MSCs) viability, homogeneity, clonogenicity, and ability to differentiate towards the osteogenic phenotype than un-clotted-V-BMA. Clotted-V-BMA also highlighted significant reduced expression of PBX1 and of MEIS3 genes negatively involved in osteoblast maturation and differentiation. From December 2020, eight patients have already been enrolled with first promising results that will be finally evaluated in the next two months. The application of V-BMA-clot as carrier of progenitors and cytokines and as natural scaffold with a structural texture represents a point-of-care orthobiologic product to improve spinal fusion. Clinical application seems to be efficacy, and we will confirm and strengthen these data with the final results of the pilot clinical study

BACKGROUND: Physical outcomes following surgery for degenerative spine disease have been well studied. It is only relatively recently however that the importance of psychological factors in determining outcome from spine surgery has been addressed. Previous studies suggest that pre-operative psychological distress is a predictor of poor outcome. In the drive to identify patients who will not benefit from spine surgery these patients may in future be denied surgery. AIM: The aim of the current study was to examine the relationship between the severity of physical symptoms, levels of pre-operative psychological distress and out-come in patients with degenerative spine disease undergoing elective spinal surgery. PATIENTS & METHODS: The study was a prospective cohort study. Health status and psychological distress were measured pre-operatively and at 12 months or more post-operatively using the Short Form 36 (SF36) Health Survey Questionnaire and the Hospital Anxiety and Depression Scale (HADS). We compared levels of physical disability (SF-36 physical domain scores) and psychological distress (HADS scores) before and after surgery in this group. Comparisons of pre- and post-operative scores were made using the Kruskal-Wallis, Wilcoxon Signed Rank Test and Mann Whitney U tests as appropriate. RESULTS: A total of 333 patients were included (178 men, 155 women, mean age 54y). Pre-operatively patients with severe levels of anxiety and depression (higher HADS score) had worse (lower) SF-36 physical domain (SF-36 PD) scores compared to those with normal HADS scores (median 181.5 vs 109, p< 0.0001). Both HADS and SF-36 PD scores improved post-operatively (HADS 13 vs 6 p< 0.0001; SF-36 PD 134 vs 250.5 p< 0.0001). Greater reduction in HADS score was observed in patients with severe pre-operative HADS scores compared to those with normal pre-operative HADS scores (p< 0.0001). Patients with severe HADS scores also had greater improvement in SF36 PD scores when compared to patients with normal pre-operative HADS scores (77.24 vs 53.87 p=0.03). CONCLUSION: Poor physical function pre-operatively correlates with severe psychological distress. Both physical and psychological symptoms improve after surgery. Severe levels of anxiety and depression pre-operatively are associated with the greatest symptomatic improvement and psychological morbidity does not worsen outcome

Since total disc replacement (TDR) has broadened the spectrum of surgical treatment of degenerative spine diseases many comparison studies, particularly with interbody fusions (IF), have been done. Even though comparable results concerning functionality, radiologic results and subjective rating of life-quality have been presented, very few data about athletic activity before and after spine surgery exists. Material and Methods: Between 1/2002 and 10/2006 181 patients had interbody fusions and 57 had a total disc replacement. Of 86 IF-patients and 25 patients with TDR we have complete data, which was collected in pre- and postoperative clinic and radiologic routine control with standardised questionnaires containing evaluation of level (frequency) and sort of sport. Patients are matched according to demographic data and preoperative activity beside the most important match of operation method. Results: Patients with Total disc replacement show a later resumption of sports (19 weeks) than fusion patients (14 weeks), but more TDR patients (60%) achieve their preoperative level of sport than IF-patients (36%) do. Vice versa to the Fusion group in the TDR group more patients start a new sport after surgery than to stop one. Percentage of patients doing sports post- compared to preoperative is higher in both groups. Less patients having a TDR complain about technical limitations during practicing sports than fusion patients. Discussion: Despite later resumption of athletic activity TDR seems to be the better surgical treatment of degenerative disc diseases in active patients and athletes due to overall higher sports levels. If long term results can keep up with short time follow ups has to be questioned

Introduction: The role of spinal fusion in patients suffering degenerative spine disease may be scrutinized more as costs of surgical treatment rise. Health-related quality of life (HRQL) measurement instruments enable outcome comparisons following treatment of different medical conditions. Rampersaud et al (1) recently presented the results of a comparative study of HRQL outcomes after surgery for lumbar spinal stenosis and hip and knee total joint arthroplasty. The latter are now accepted benchmarks for improvement in patient health. Methods: A retrospective, observational cohort study was undertaken of 12-item Short Form Health Survey (SF-12) outcome data of 105 consecutive patients of two surgeons (1st and 2nd authors) who underwent single level Posterior Lumbar Interbody Fusion (PLIF) for lumbar spinal stenosis associated with degenerative spondylolisthesis. Minimum 12-month (F/U) data was available for 98 patients (93%). Comparison was made with published SF-12 results of hip and knee total joint arthroplasty (THR and TKR) and with age-related Australian population norms. Analyses were performed using XLSTAT version 7.5.3. Non-parametric statistics were used for assessment of skewed continuous variables. Overlappng 95%CIs were interpreted as indicating lack of significant difference in outcomes between patient and population groups. Results: Median follow-up was 24months (range: 12–60months). Median age was 65 (Interquartile range: 59–75) years. Male:female ratio 2.8:1. Mean (95%CI) pre-op Physical Component Summary score (PCS) was 28.1 (26.6–29.5). This increased at last F/U to 39.3 (36.9–41.7, P< 0.0001). Mean Mental Component Summary score (MCS) was 47.8 (45.5–50.1) pre-op and 52.3 (50.2–54.5) at last F/U (P=< 0.0001). While there was no difference in patient demographics, a significant difference existed in the pre-op SF-12 scores between the patients of the two surgeons (mean PCS: 24.9 (22.7–27.0) vs. 29.6 (27.8–31.5) and MCS: 44.0 (39.3–48.6) vs. 49.5 (46.8–52.1)). No significant difference was found in the improvements in mean SF-12 scores between these two patient groups (PCS: 12.3 (7.6–17.1) vs. 10.8 (8.3–13.3) and MCS: 6.3 (1.8–10.8) vs. 3.0 (0.3–5.6)) or in the SF-12 scores at 12-months (PCS: 37.2 (32.8–41.6) vs. 40.2 (37.2–43.2) and MCS: 52 (48.3–55.7) vs. 52.3 (50.1–54.4)). No significant difference was found between post-op PCS of the less disabled patient group or MCS scores of either group and published SF-12 age-matched population norms (65–74 years: mean PCS of 44.4 (42.7–46.1) and MCS of 53.8 (52.7–55.0)). Three published series (869 patients) were located providing SF-12 data for TKR surgery. Weighted mean age was 69 years and pre-op PCS was 30 (range:27–34). 12-month improvement in PCS was 7.0 (range:7–8.5). For THR, one paper (147 patients from 3 hospitals) containing SF-12 data was found. Mean age was 68 years (range:36–89). Mean pre-op PCS and MCS of 30.5 and 41.4, increased to 45.6 and 49.7 at one year. Discussion: The current study shows that spinal fusion can return patients’ HRQL to that of age-matched population norms and yield outcomes comparable to those of total hip and knee arthroplasty. Strict comparison with the arthroplasty literature was problematic however owing to variations in the methodology of their data presentation. Prospective collaboration with surgical colleagues in other disciplines is required

While image guidance and neuro-navigation have enabled a more accurate positioning of pedicle implants, robot-assisted placement of pedicle screws appears to overcome the disadvantages of the two first systems. However, recent data concerning the superiority of robots currently available to assist spinal surgeons in the accurate positioning of implants are conflicting. The aim of our study was to evaluate the percentage of accurate positioning of pedicle screws inserted using a new robotic-guidance system. Patients were operated on successively by the same surgeon using robotic-assistance (RA; n=40) or by the freehand conventional technique (FH; n=54). Ten and eleven patients from the robot (RG) and freehand (FHG) groups respectively, age-matched and all suffering from degenerative lumbar spine disease were compared. Patient characteristics as well as the duration of the operation and of exposure to X-rays were recorded. The Gertzbein Robbins classification was used to evaluate implant placement. Data wer compared between the groups. Pedicle screw placement in RG patients was achieved using the ROSA™ (Medtech) robot comprising a compact robotic arm on a floor-fixable mobile base. By permanently monitoring the patient's movements, this image-guided tool helps more accurately to pinpoint the pedicle entry point and to control the trajectory. The mean age of patients in each group (RG and FHG) was 63 years. Mean BMI and operating time among the RG and FHG were respectively 26 and 27 kg/m. 2. , and 187 and 119 min. Accurate placement of the implant (score A-B) was achieved in 97.2% of patients in the RG (n=36) and in 92.6% of those in the FHG (n=54). Four implants in the RG were placed manually following failed robotic assistance. The mean duration of X-ray exposure per patient was 1 min 42s in the RG and 41s in the FHG. We report a higher rate of accuracy with robotic assistance as compared to the FH technique. Exposure time was greater in the RG partly due to the fluoroscopic control of the implants required for this pilot study of feasibility. Limitations of the study include its small sized and non-randomised sample. Nevertheless, these preliminary results are encouraging for the development of new robotic techniques for spinal surgery


Purpose of study. This RCT is to determine whether or not there is a clinical benefit from inserting a dynamic stabilising implant such as the Wallis ligament on the functional recovery of patients who have undergone lumbar decompression surgery. This Interspinous implant was developed as an anatomically conserving procedure without recourse to lumbar spinal fusion surgery. The biomechanical studies have shown that unloading the disc and facet joints reduces intradiscal pressures at same and adjacent levels. The aim of this study was to identify a patential Wallis affect. Methods. Ethicallly approved. Patients were randomized into 2 groups, decompression alone or decompression with wallis interspinous ligament stabilisation. Patients were assessed pre operatively and post operatively every 6 months by VAS pain score and Oswestry Disability Index. Summary of findings. A total of 60 patients were recriuted the study from October 2005. Equal number had been randomized into two groups. The mean age of 54 (24–85) and the average follow is 36 months (6–48). The results were significantly better in decompression plus Wallis group compared to decompression alone, showing a minimal clinical difference compared to the control group. Relationship between findings and existing knowledge: Our results deomonstrate that clincial outcomes are significantly better when a Wallis implant was used in lumbar deompression. Patients experienced less back pain. Overall significance of findings: The Wallis implant represents a successful non fusion stabilisation device in the treatment of degenerative lumbar spine disease with canal stenosis. Minimal soft tissue dissection, quick rehabilitation, low morbidity. The Wallis ligament sucessfully treats spinal stenosis by reducing pain score, preserving mobility, and function

Introduction: Lumbar fusion is one of the most frequently recommended treatments in spinal surgery, whereas ALIF and PLF are discussed controversially. This prospective and consecutive study examined if complications and rehabilitation differed between patients with a degenerated lumbar spine, who had been treated with ALIF or PLF. Methods: Between 04/2002 and 12/2005 clinical data of 39 patients, who were treated with ALIF, and of 296 patients, who received PLF, were submitted to the European Spine Register SSE Spine Tango. Complications and rehabilitation process were documented after a median follow-up time of 9 weeks (IQR 8–24 weeks). Analysis was performed with non-parametric tests. The study corresponds with an EBM-level 3. Results: The median age of patients with ALIF was 56 years (IQR 37-66 yrs) vs. 64 yrs in the PLF group (IQR 54–72 yrs) with a male to female ratio 10:29 vs. 108:188. Patients after ALIF and PLF had an even complication rate (5/39 vs. 26/296, p = 0,41). The types of complications in the ALIF group were less severe (sensory and motoric disturbance vs. implant failure and implant malposition). All five patients needing reintervention belonged to the PLF group. Three quarters of all patients underwent rehabilitation. The proportion of patients with outpatient rehabilitation was higher in the ALIF group (14/39 vs. 50/296 patients, p = 0,05). Conclusions: Patients with degenerative lumbar spine disease have a less severe type of complications after ALIF than after PLF, whereas the complication rate is even. Concerning the higher proportion of patients with outpatient rehabilitation in the ALIF group, one has to consider their lower age compared with patients in the PLF group. Except for the severeness of complications and the proportion of outpatient rehabilitation one cannot conclude an advantage of either of the two surgical methods

Background: Lumbar facet joints are a frequent source of pain in degenerative lumbar spine disease. In many cases, they may actually be the predominant source of pain. Material & Methods: Our target criteria were low back pain (VAS 0 10), back pain related limitation in daily activities and general acceptance of the treatment method. Inclusion criteria: Deep-seated non-sciatic low back pain, failure of conservative measures, positive diagnostic medial branch blocks. Exclusion criteria: Previous spinal surgery, relevant spinal stenosis, activated osteochondrosis, radicular pain. Diagnostic blocks were performed under fluoroscopy, improvement in low back pain of more than 50% for more than 3 hours was considered a positive block. Cryodenervation was performed also under fluoroscopy at a separate appointment. Since June 2002, 52 patients (average age 56) were entered into the study. 2 Patients were lost to follow-up and 2 others had to be excluded, so that 48 patients were available for evaluation. At present, we have a 3-month follow-up for all 48 patients, a 6-month follow-up for 44 patients, a 12-month follow-up for 32 patients and an 18-month follow-up for 19 patients. Results: 2 weeks after treatment, 65 % of patients reported significant improvement, 35 % reported little or no change in pain. The average VAS of the complete study group dropped from 7.7 preoperatively to 3.3 at two weeks and to 3.45 at three and eighteen months postoperatively (p < 0.05). Limitation in daily activities improved parallel to the reduction in low back pain and 33 out of 48 patients would have the procedure performed again while 2 remained undecided. Conclusion: Percutaneous medial branch cryodenervation is a safe and effective means for the treatment of lumbar facet joint pain

Objectives

We performed a systematic review of the literature to determine the safety and efficacy of bone morphogenetic protein (BMP) compared with bone graft when used specifically for revision spinal fusion surgery secondary to pseudarthrosis.

Methods

The MEDLINE, EMBASE and Cochrane Library databases were searched using defined search terms. The primary outcome measure was spinal fusion, assessed as success or failure in accordance with radiograph, MRI or CT scan review at 24-month follow-up. The secondary outcome measure was time to fusion.
