Scoliosis is estimated to occur between 21–64% of patients with cerebral palsy (CP), where a subset of patients develops early onset scoliosis (EOS) before the age of ten. Traditional growth friendly (TGF) surgeries in the context of traditional growing rods have been shown to be effective in treating scoliosis in this population, however significant complication rates are reported. Currently, no studies have been done to examine the effect of novel growth friendly surgeries such as magnetically controlled growing rods (MCGR) on EOS in CP patients. The objective of this study is to compare MCGR with TGF surgeries in this patient population, specifically by evaluating radiographic measurements and risk of unplanned reoperations (UPRORs).

Patients with EOS secondary to CP were prospectively identified from an international database, with data retrospectively analyzed. Scoliosis (primary curve), maximum kyphosis, T1-S1 and T1-T12 height were measured pre-operation, immediate post-operation, and at two-years follow-up. The risk and etiology of UPRORs were compared between MCGR and TGF. P < 0.05 was considered statistically significant for all analyses.

Of the 120 patients that met inclusion criteria, 86 received TGF (age 7.5 ± 1.8 years; follow-up 7.0 ± 2.9 years) and 34 received MCGR (age 7.1 ± 2.2 years, follow-up 2.8 ± 0.5 years). Compared to TGF, MCGR resulted in significant improvements in maintenance of scoliosis correction (p=0.04). At final follow-up, UPRORs were 24% for MCGR (8/34 patients) and 43% (37/86 patients) for TGF (p=0.05). To minimize the influence of follow-up period, UPRORs within the first two years post-operation were evaluated: MCGR (21%, 7/34 patients) vs. TGF (14%, 12/86 patients; p=0.37). Within the first two years, etiology of UPROR as a percentage of all patients per group were deep infection (5% TGF, 6% MCGR), implant failure/migration (5% TGF, 9% MCGR), dehiscence (2% TGF, 3% MCGR), and superficial infection (1% TGF, 3% MCGR). The most common etiology of UPROR for TGF was deep infection and implant failure/migration and for MCGR was implant failure/migration.

For patients with CP, at final follow-up, MCGR had superior maintenance of scoliosis correction; however, there was no difference in risk of UPROR within the first two years post-operatively (21% MCGR, 14% TGF).

Purpose: Since the advent of pinning for supracondylar fractures of the humerus, Volkmann syndrome has been exceptional and most of the posttraumatic compartment syndromes observed in children have been seen in the lower limb. We propose an analysis of the causes, the diagnosis, the treatment and the results of treatment of acute posttraumatic compartment syndrome of the leg in children.

Material and methods: Twenty-eight consecutive cases of acute posttraumatic compartment syndrome in 27 children were reviewed. These children were treated in two American paediatric traumatology units over a ten year period. We evaluated the cause of the trauma, associated lesions, clinical course, diagnostic methods, muscle compartment pressures, time from accident to diagnosis, and time from accident to surgery. Results were analysed at last follow-up.

Results: The study population was 24 boys and three girls, aged 4 months to 15 years. Twenty-four children were pedestrian traffic accident victims. Twenty-two had a tibial fracture, four a femoral fracture, and two no fracture. Twenty-five compartment syndromes were diagnosed on the basis of compartment pressure measurements. Mean time from accident to diagnosis was 19 hours (range 2.5–85 hr). At diagnosis, exacerbated pain was observed in 26 children, paraesthesia in eleven, motor deficit in seven, and diminished pulses in three. Mean time from accident to surgery was 21 hours. Mean follow-up was 15 months. The final outcome was remarkably good. At last follow-up, 24 children were pain free, with no functional or sensorial deficit. Aponeurotomy had been performed very late (43, 83, and 86 hr) in the three patients who developed functional deficit. There were no cases of infection even when surgery was performed late.

Discussion: Most children treated for acute posttraumatic compartment syndrome achieve a good result even when the time from accident to treatment is long, often more than 12 hours. All patients with sequellae at the last follow-up in our series had undergone aponeurotomy more than 36 hours after the accident.

Conclusion: This is the first series devoted exclusively to acute posttraumatic compartment syndrome of the leg in children. The results were generally good despite significant time from accident to treatment.

Introduction: We compared femoral head resection (FHR) and traction with femoral head resection and valgus osteotomy (the McHale procedure), in order to determine the effectiveness of these two procedures in the treatment of painful hip subluxation in severely involved individuals with cerebral palsy.

Methods: Retrieval of demographic patient information, operative technique, post-operative complications, and migration of the femoral shaft was obtained from a retrospective review of charts and radiographs. Caregivers were then contacted by telephone and queried regarding post-operative changes in pain, sitting tolerance, and hygiene as well as overall satisfaction with surgical intervention.

Results: 27 patients, 36 hips comprise the study cohort; 26 patients have quadriplegia, one has diplegia and is the only patient who is ambulatory. 16 patients underwent FHR, 11 patients underwent McHale procedures. The average age of surgery was 19 years, range from 8 to 42 years. Average follow-up was 3.4 years from time of surgery. The majority of patients (17) had not undergone reconstructive hip surgery because they were lost to orthopedic follow-up, and missed the opportunity to have the hip relocated before femur was significantly deformed. Six patients had painful hips despite previous attempts at surgery, three patients refused reconstructive surgery, and one patient was not deemed medically stable enough for reconstructive surgery.

Post-operative complications were numerous and included skin breakdown, wound dehiscense, hardware infection or failure, heterotopic ossification, and death. The complication rate was significantly higher in patients who had undergone FHR and traction (13/16) compared with the patients who had a McHale procedure (3/11). The average length of hospitalization was almost twice as long for the FHR group (7 days) as for the McHale group (4 days). Telephone surveys of caregiv-ers often demonstrated equivalent overall satisfaction with surgery in both groups with average scores of 8/10 for the FHR and 7.6/10 for the McHale group (on a scale from 1 to 10, 10 being the most satisfied). Only two of the respondents (one from the FHR group, one McHale) we contacted regretted having had surgery.

Caregivers felt that post-operatively pain relief was achieved in almost all patients. The average time to achieve a more pain-free state was three months. Sitting tolerance improved variably between individual patients, while few caregivers felt that hygiene improved after surgery, although they also felt that hygiene had not been a significant problem pre-operatively.
