Ankle arthroplasty with custom-made talar component is used to avoid talar subsidence, one of the most common causes of ankle prosthesis failure. We have used Agility ankle system with custom-made talar component to treat young patients with postraumatic arthritis, revision arthroplasty and takedown ankle arthrodesis. Ankle substitution was indicated in young patients who refused arthrodesis and understood that revision or additional surgery would be inevitable in the future. Twelve cases were revised with a minimum of nine months of follow-up, females, 2; males, 10; average age, 42 years. Primary replacements were performed in 9 patients, takedown fusion in 2 and revision arthroplasty in 1. Other additional procedures as subtalar fusion (8 cases), calcaneal osteotomies (6), medial column reconstruction (2), anterior compartment tendon lengthening (2 cases) and TAL or gastrocnemius lengthening (12 cases) and reoperation were also revised. Early complications included a fracture of the malleoli in 1 ankle and a dehiscence of the principal wound in 1 case. The mean postoperative ankle ROM was 32° (range 10°–40°) in comparison with preoperatively (0° –15°). The postoperative functional results were evaluated with the SMFA (Short Musculoskeletal Function Assessment) score system and a visual analog pain scale (VAS Questionnaire). The average preoperative SMFA and VAS scores for all patients was, 40,6 and 8,1 respectively. Postoperatively, these scores averaged 18,9 and 2,0 respectively. Those patients with conversion to ankle arthroplasty presented more stiffness after surgery and had required more rehabilitation time. Despite short-term follow-up, talar stems may provide an excellent alternative for the difficult problem of talar subsidence in young patients in total ankle arthroplasty, with good results and restoration of ankle function.

Introduction: Congenital kyphosis is an uncommon deformity caused by failure of the vertebral bodies to form and/or segment. It is treated surgically. Our purpose is to confirm whether the treatment protocols established years ago are still valid and to assess the benefits of improved implants.

Materials and methods: Between 1985 and 2003, 24 patients underwent surgery in La Paz Hospital. They were classified into three groups: < 5 years (8 p), 6–12 (6 p) and > 13 (10 p). The minimum follow-up was 2 years (2–14). The procedures were posterior or circumferential spinal fusion, with or without instrumentation, and corrective osteotomy. Complications were evaluated radiographically.

Results: In the first group posterior spinal fusion was performed in six patients and circumferential in two, with a correction rate of 55%. In the second group instrumented circumferential spinal fusion was performed in three cases and instrumented posterior in three (30% correction). In the third group eight of the ten patients underwent instrumented anteroposterior spinal fusion (osteotomy in five) and the correction rate was 45%. Complications: 3 pseudoarthrosis, 1 DVT, 2 infections and 3 failed implants.

Conclusions: Congenital kyphosis can be controlled at any age, although early surgery is best. Gradual correction occurs after poster spinal fusion in children and instrumentation prevents revision of the fusion and prolonged immobilisation. Persons over the age of 5 with type I kyphosis usually require circumferential spinal fusion.

Aims: Tension band wiring is a widely accepted method for internal þxation of olecranon fractures. Plate þxation is suggested for the more complex olecranon fractures, but little is known about the speciþc risk factors for failure of tension band wiring. The aim of this study was to analyze the inßuence of fracture comminution, associated elbow instability and fracture extension into the coronoid process on the outcome of tension band wiring for olecranon fractures. Methods: From 1996 to 1998, forty-four olecranon fractures were treated consecutively at out institution using tension band wiring. All patients returned for a clinical and radiographic exam performed by two observers independent of the treating surgeon. Pain and satisfaction were determined using visual-analogue scales (VAS) and clinical results were graded using the Mayo Elbow Performance Score (MEPS) and the DASH questionnaire. Patients were followed for 3 to 6 years. Results: At most recent follow-up, the mean VAS score for pain was 2.0, mean extension was 3.6û and mean ßexion was 137.2û. According to the MEPS the results were graded as good or excellent in 78% of the patients. Five patients were disabled for activities of daily living according to the DASH questionnaire. All but one fracture healed. Fracture comminution did not affect the outcome. Worse results were associated with elbow instability and fracture extension into the coronoid. Conclusions: Tension band wiring provided satisfactory results for the treatment of olecranon fractures in the presence of fracture comminution, but worse results were obtained in the presence of elbow instability and fracture extension into the coronoid.
