Custom triflange acetabular components (CTACs) play an important role in reconstructive orthopaedic surgery, particularly in revision total hip arthroplasty (rTHA) and pelvic tumour resection procedures. Accurate CTAC positioning is essential to successful surgical outcomes. While prior studies have explored CTAC positioning in rTHA, research focusing on tumour cases and implant flange positioning precision remains limited. Additionally, the impact of intraoperative navigation on positioning accuracy warrants further investigation. This study assesses CTAC positioning accuracy in tumour resection and rTHA cases, focusing on the differences between preoperative planning and postoperative implant positions. A multicentre observational cohort study in Australia between February 2017 and March 2021 included consecutive patients undergoing acetabular reconstruction with CTACs in rTHA (Paprosky 3A/3B defects) or tumour resection (including Enneking P2 peri-acetabular area). The primary aim of our study was to assess the influence of age on hip-specific outcome following total hip arthroplasty (THA). Secondary aims were to assess health-related quality of life (HRQoL) and level of activity according to age. A prospective cohort study was conducted. All patients were fitted with an Exeter stem with a 32 mm head on highly cross-linked polyethylene (X3RimFit) cemented acetabulum. Patients were recruited into three age groups: < 65 years, 65 to 74 years, and ≥ 75 years, and assessed preoperatively and at three, 12, 24, and 60 months postoperatively. Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Harris Hip Score (HHS), and Hip disability and Osteoarthritis Outcome Score (HOOS), were used to assess hip-specific outcome. This study aims to assess the feasibility of conducting a pragmatic, multicentre randomized controlled trial (RCT) to test the clinical and cost-effectiveness of an informal caregiver training programme to support the recovery of people following hip fracture surgery. This will be a mixed-methods feasibility RCT, recruiting 60 patients following hip fracture surgery and their informal caregivers. Patients will be randomized to usual NHS care, versus usual NHS care plus a caregiver-patient dyad training programme (HIP HELPER). This programme will comprise of three, one-hour, one-to-one training sessions for the patient and caregiver, delivered by a nurse, physiotherapist, or occupational therapist. Training will be delivered in the hospital setting pre-patient discharge. It will include practical skills for rehabilitation such as: transfers and walking; recovery goal setting and expectations; pacing and stress management techniques; and introduction to the HIP HELPER Caregiver Workbook, which provides information on recovery, exercises, worksheets, and goal-setting plans to facilitate a ‘good’ recovery. After discharge, patients and caregivers will be supported in delivering rehabilitation through three telephone coaching sessions. Data, collected at baseline and four months post-randomization, will include: screening logs, intervention logs, fidelity checklists, quality assurance monitoring visit data, and clinical outcomes assessing quality of life, physical, emotional, adverse events, and resource use outcomes. The acceptability of the study intervention and RCT design will be explored through qualitative methods with 20 participants (patients and informal caregivers) and 12 health professionals.Aims

Bone marrow-derived mesenchymal stem cells obtained from bone marrow aspirate concentrate (BMAC) with platelet-rich plasma (PRP), has been used as an adjuvant to hip decompression. Early results have shown promise for hip preservation in patients with osteonecrosis (ON) of the femoral head. The purpose of the current study is to examine the mid-term outcome of this treatment in patients with precollapse corticosteroid-induced ON of the femoral head. In all, 22 patients (35 hips; 11 males and 11 females) with precollapse corticosteroid-induced ON of the femoral head underwent hip decompression combined with BMAC and PRP. Mean age and BMI were 43 years (SD 12) and 31 kg/m² (SD 6), respectively, at the time of surgery. Survivorship free from femoral head collapse and total hip arthroplasty (THA) and risk factors for progression were evaluated at minimum five-years of clinical follow-up with a mean follow-up of seven years (5 to 8).Aims

This study aimed to determine if macrophages can attach and directly affect the oxide layers of 316L stainless steel, titanium alloy (Ti6Al4V), and cobalt-chromium-molybdenum alloy (CoCrMo) by releasing components of these alloys. Murine peritoneal macrophages were cultured and placed on stainless steel, CoCrMo, and Ti6Al4V discs into a 96-well plate. Cells were activated with interferon gamma and lipopolysaccharide. Macrophages on stainless steel discs produced significantly more nitric oxide (NO) compared to their control counterparts after eight to ten days and remained elevated for the duration of the experiment.Aims

Cephalomedullary nails (CMNs) are commonly used for the treatment of intertrochanteric hip fractures. Total hip arthroplasty (THA) may be used as a salvage procedure when fixation fails in these patients. The aim of this study was to analyze the complications of THA following failed intertrochanteric hip fracture fixation using a CMN. Patients who underwent THA were identified from the 5% subset of Medicare Parts A/B between 2002 and 2015. A subgroup involving those with an intertrochanteric fracture that was treated using a CMN during the previous five years was identified and compared with the remaining patients who underwent THA. The length of stay (LOS) was compared using both univariate and multivariate analysis. The incidence of infection, dislocation, revision, and re-admission was compared between the two groups, using multivariate analysis adjusted for demographic, hospital, and clinical factors.Aims

Tantalum (Ta) trabecular metal components are increasingly used
to reconstruct major bone defects in revision arthroplasty surgery.
It is known that some metals such as silver have antibacterial properties.
Recent reports have raised the question regarding whether Ta components

Advocates of debridement, antibiotics and implant retention (DAIR)
in hip periprosthetic joint infection (PJI) argue that a procedure
not disturbing a sound prosthesis-bone interface is likely to lead
to better survival and functional outcome compared with revision.
This case-control study aims were to compare outcome of DAIRs for
infected primary total hip arthroplasty (THA) with outcomes following
primary THA and two-stage revision of infected primary THAs. We retrospectively reviewed all DAIRs, performed for confirmed
infected primary hip arthropasty (n = 82) at out institution, between
1997 and 2013. Data recorded included full patient information and
type of surgery. Outcome measures included complications, mortality,
implant survivorship and functional outcome. Outcome was compared with
two control groups matched for gender and age; a cohort of primary
THAs (n = 120) and a cohort of two-stage revisions for infection

The high revision rates of the DePuy Articular Surface Replacement (ASR) and the DePuy ASR XL (the total hip arthroplasty (THA) version) have led to questions over the viability of metal-on-metal (MoM) hip joints. Some designs of MoM hip joint do, however, have reasonable mid-term performance when implanted in appropriate patients. Investigations into the reasons for implant failure are important to offer help with the choice of implants and direction for future implant designs. One way to assess the performance of explanted hip prostheses is to measure the wear (in terms of material loss) on the joint surfaces. In this study, a coordinate measuring machine (CMM) was used to measure the wear on five failed cementless Biomet Magnum/ReCap/ Taperloc large head MoM THAs, along with one Biomet ReCap resurfacing joint. Surface roughness measurements were also taken. The reason for revision of these implants was pain and/or adverse reaction to metal debris (ARMD) and/or elevated blood metal ion levels.Objectives

Periprosthetic fracture (PF) after primary total hip arthroplasty
(THA) is an uncommon but potentially devastating complication. This
study aims to investigate the influence of cemented stem designs
on the risk of needing a revision for a PF. We analysed data on 257 202 primary THAs with cemented stems
and 390 linked first revisions for PF recorded in the National Joint
Registry (NJR) of England, Wales and Northern Ireland to determine
if a cemented femoral stem brand was associated with the risk of
having revision for a PF after primary THA. All cemented femoral
stem brands with more than 10 000 primary operations recorded in
the NJR were identified. The four most commonly used cemented femoral
stems were the Exeter V40 (n = 146 409), CPT (n = 24 300), C-Stem
(n = 15 113) and Charnley (n = 20 182). We compared the revision risk ratios due to PF amongst the stems
using a Poisson regression model adjusting for patient factors.
Compared with the Exeter V40, the age, gender and ASA grade adjusted
revision rate ratio was 3.89 for the cemented CPT stem (95% confidence
interval (CI) 3.07 to 4.93), 0.89 for the C-Stem (95% CI 0.57 to
1.41) and 0.41 for the Charnley stem (95% CI 0.24 to 0.70).Aims

The optimal management of intracapsular fractures of the femoral
neck in independently mobile patients remains open to debate. Successful
fixation obviates the limitations of arthroplasty for this group
of patients. However, with fixation failure rates as high as 30%,
the outcome of revision surgery to salvage total hip arthroplasty
(THA) must be considered. We carried out a systematic review to
compare the outcomes of salvage THA and primary THA for intracapsular
fractures of the femoral neck. We performed a Preferred Reporting Items for Systematic Reviews
and Meta-Analysis (PRISMA) compliant systematic review, using the
PubMed, EMBASE and Cochrane libraries databases. A meta-analysis
was performed where possible, and a narrative synthesis when a meta-analysis

The aim of this cadaver study was to identify
the change in position of the sciatic nerve during arthroplasty
using the posterior surgical approach to the hip. We investigated
the position of the nerve during this procedure by dissecting 11
formalin-treated cadavers (22 hips: 12 male, ten female). The distance
between the sciatic nerve and the femoral neck was measured before
and after dislocation of the hip, and in positions used during the

There is great variability in acetabular component
orientation following hip replacement. The aims of this study were
to compare the component orientation at impaction with the orientation
measured on post-operative radiographs and identify factors that
influence the difference between the two. A total of 67 hip replacements
(52 total hip replacements and 15 hip resurfacings) were prospectively
studied. Intra-operatively, the orientation of the acetabular component
after impaction relative to the operating table was measured using
a validated stereo-photogrammetry protocol. Post-operatively, the
radiographic orientation was measured; the mean inclination/anteversion
was 43° ( This study demonstrated that in order to achieve a specific radiographic
orientation target, surgeons should implant the acetabular component

We aimed to determine whether cemented hemiarthroplasty
is associated with a higher post-operative mortality and rate of
re-operation when compared with uncemented hemiarthroplasty. Data
on 19 669 patients, who were treated with a hemiarthroplasty following
a fracture of the hip in a nine-year period from 2002 to 2011, were extracted
from NHS Scotland’s acute admission database (Scottish Morbidity
Record, SMR01). We investigated the rate of mortality at day 0,
1, 7, 30, 120 and one-year post-operatively using 12 case-mix variables
to determine the independent effect of the method of fixation. At
day 0, those with a cemented hemiarthroplasty had a higher rate
of mortality (p <
0.001) compared with those with an uncemented
hemiarthroplasty, equivalent to one extra death per 424 procedures.
By day one this had become one extra death per 338 procedures. Increasing
age and the five-year co-morbidity score were noted as independent
risk factors. By day seven, the cumulative rate of mortality was
less for cemented hemiarthroplasty though this did not reach significance
until day 120. The rate of re-operation was significantly higher
for uncemented hemiarthroplasty. Despite adjusting for 12 confounding
variables, these only accounted for 15% of the observed variability. The debate about the choice of the method of fixation for a hemiarthroplasty
with respect to the rate of mortality or the risk of re-operation
may be largely superfluous. Our results suggest that uncemented
hemiarthroplasties may have a role to play in elderly patients with

The orientation of the acetabular component is
influenced not only by the orientation at which the surgeon implants
the component, but also the orientation of the pelvis at the time
of implantation. Hence, the orientation of the pelvis at set-up
and its movement during the operation, are important. During 67

much the pelvis moved during surgery, and what effect these had

Squeaking arising from a ceramic-on-ceramic (CoC)
total hip replacement (THR) may cause patient concern and in some
cases causes patients to seek revision surgery. We performed a meta-analysis
to determine the incidence of squeaking and the incidence of revision
surgery for squeaking. A total of 43 studies including 16 828 CoC
THR that reported squeaking, or revision for squeaking, were entered
into the analysis. The incidence of squeaking was 4.2% and the incidence
of revision for squeaking was 0.2%. The incidence of squeaking in
patients receiving the Accolade femoral stem was 8.3%, and the incidence

This study compared the primary stability of two commercially
available acetabular components from the same manufacturer, which
differ only in geometry; a hemispherical and a peripherally enhanced
design (peripheral self-locking (PSL)). The objective was to determine
whether altered geometry resulted in better primary stability. Acetabular components were seated with 0.8 mm to 2 mm interference
fits in reamed polyethylene bone substrate of two different densities
(0.22 g/cm3 and 0.45 g/cm3). The primary stability
of each component design was investigated by measuring the peak
failure load during uniaxial pull-out and tangential lever-out tests.Objective

Recent events have highlighted the importance
of implant design for survival and wear-related complications following
metal-on-metal hip resurfacing arthroplasty. The mid-term survival
of the most widely used implant, the Birmingham Hip Resurfacing
(BHR), has been described by its designers. The aim of this study
was to report the ten-year survival and patient-reported functional
outcome of the BHR from an independent centre. In this cohort of 554 patients (646 BHRs) with a mean age of
51.9 years (16.5 to 81.5) followed for a mean of eight years (1
to 12), the survival and patient-reported functional outcome depended
on gender and the size of the implant. In female hips (n = 267)
the ten-year survival was 74% (95% confidence interval (CI) 83 to
91), the ten-year revision rate for pseudotumour was 7%, the mean
Oxford hip score (OHS) was 43 ( This study supports the ongoing use of resurfacing in young active
men, who are a subgroup of patients who tend to have problems with
conventional THR. In contrast, the results in women have been poor
and we do not recommend metal-on-metal resurfacing in women. Continuous
follow-up is recommended because of the increasing incidence of
pseudotumour with the passage of time.
We report the use of a 15° face-changing cementless
acetabular component in patients undergoing total hip replacement
for osteoarthritis secondary to developmental dysplasia of the hip.
The rationale behind its design and the surgical technique used
for its implantation are described. It is distinctly different from
a standard cementless hemispherical component as it is designed
to position the bearing surface at the optimal angle of inclination,
that is, <
45°, while maximising the cover of the component by
host bone.
Pseudotumours are a rare complication of hip resurfacing. They are thought to be a response to metal debris which may be caused by edge loading due to poor orientation of the acetabular component. Our aim was to determine the optimal acetabular orientation to minimise the risk of pseudotumour formation. We matched 31 hip resurfacings revised for pseudotumour formation with 58 controls who had a satisfactory outcome from this procedure. The radiographic inclination and anteversion angles of the acetabular component were measured on anteroposterior radiographs of the pelvis using Einzel-Bild-Roentgen-Analyse software. The mean inclination angle (47°, 10° to 81°) and anteversion angle (14°, 4° to 34°) of the pseudotumour cases were the same (p = 0.8, p = 0.2) as the controls, 46° (29° to 60°) and 16° (4° to 30°) respectively, but the variation was greater. Assuming an accuracy of implantation of ± 10° about a target position, the optimal radiographic position was found to be approximately 45° of inclination and 20° of anteversion. The incidence of pseudotumours inside the zone was four times lower (p = 0.007) than outside the zone. In order to minimise the risk of pseudotumour formation we recommend that surgeons implant the acetabular component at an inclination of 45° (± 10) and anteversion of 20° (± 10) on post-operative radiographs. Because of differences between the radiographic and the operative angles, this may be best achieved by aiming for an inclination of 40° and an anteversion of 25°.
