The aim of this study was to analyze the complications and outcomes of treatment in a series of previously untreated patients with a primary aneurysmal bone cyst (ABC) who had been treated by percutaneous sclerosant therapy using polidocanol. Between January 2010 and December 2016, 56 patients were treated primarily with serial intralesional sclerosant injections. Their mean age was 20 years (1 to 54). The sites involved were clavicle (n = 3), humeri (n = 11), radius (n = 1), ulna (n = 3), hand (n = 2), pelvis (n = 12), femur (n = 7), tibia (n = 13), fibula (n = 3), and foot (n = 1). After histopathological confirmation of the diagnosis, 3% polidocanol (hydroxypolyaethoxydodecan) was injected into the lesion under image intensifier guidance. Patients were evaluated clinically and radiologically every six to eight weeks. In the absence of clinical and/or radiological response, a repeat sclerosant injection was given after eight to 12 weeks and repeated at similar intervals if necessary.Aims

A lack of supporting clinical studies have been published to determine the ideal length of intramedullary nail in fixation of trochanteric fractures of the hip. Nevertheless, there has been a trend to use shorter intramedullary nails for the internal fixation of trochanteric hip fractures. Our aim was to determine if the length of nail affected the outcome. We randomized 229 patients with a trochanteric hip fracture between two implants: a ‘standard’ nail of 220 mm and a shorter nail of 175 mm, which had decreased proximal angulation (4° vs 7°) and a reduced diameter at the level of the lesser trochanter. Patients were followed up for one year by a nurse blinded to the type of implant used to determine if there were differences in mobility and pain with two nail designs. Pain was assessed on a scale of 1 (none) to 8 (severe and constant) and mobility on a scale of 1 (full mobility) to 9 (immobile).Aims

Early cases of cauda equina syndrome (CES) often present with nonspecific symptoms and signs, and it is recommended that patients undergo emergency MRI regardless of the time since presentation. This creates substantial pressure on resources, with many scans performed to rule out cauda equina rather than confirm it. We propose that compression of the cauda equina should be apparent with a limited sequence (LS) scan that takes significantly less time to perform. In all, 188 patients with suspected CES underwent a LS lumbosacral MRI between the beginning of September 2017 and the end of July 2018. These images were read by a consultant musculoskeletal radiologist. We report ten-year clinical and radiological
follow-up data for the Sigma Press Fit Condylar total knee replacement system
(Sigma PFC TKR). Between October 1998 and October 1999 a total of
235 consecutive PFC Sigma TKRs were carried out in 203 patients.
Patients were seen at a specialist nurse-led clinic seven to ten
days before admission and at six and 18 months, three, five and
eight to ten years after surgery. Data were recorded prospectively
at each clinic visit. Radiographs were obtained at the five- and
eight- to ten-year follow-up appointments. Of the 203 patients,
147 (171 knees) were alive at ten years and 12 were lost to follow-up.
A total of eight knees (3.4%) were revised, five for infection and
three to change the polyethylene insert. The survival at ten years
with an endpoint of revision for any reason was 95.9%, and with
an endpoint of revision for aseptic failure was 98.7%. The mean
American Knee Society Score (AKSS) was 79 (10 to 99) at eight to

Idiopathic scoliosis is the most common spinal deformity in adolescents and children. The aetiology of the disease remains unknown. Previous studies have shown a lower bone mineral density in individuals with idiopathic scoliosis, which may contribute to the causation. The aim of the present study was to compare bone health in adolescents with idiopathic scoliosis with controls. We included 78 adolescents with idiopathic scoliosis (57 female patients) at a mean age of 13.7 years (8.5 to 19.6) and 52 age- and sex-matched healthy controls (39 female patients) at a mean age of 13.8 years (9.1 to 17.6). Mean skeletal age, estimated according to the Tanner-Whitehouse 3 system (TW3), was 13.4 years (7.4 to 17.8) for those with idiopathic scoliosis, and 13.1 years (7.4 to 16.5) for the controls. Methods
Free vascularised fibular grafting has been reported
to be successful for adult patients with osteonecrosis of the femoral
head (ONFH). However, its benefit in teenage patients with post-traumatic
ONFH has not been determined. We evaluated the effectiveness of
free vascularised fibular grafting in the treatment of this condition
in children and adolescents. We retrospectively analysed 28 hips
in 28 patients in whom an osteonecrotic femoral head had been treated
with free vascularised fibular grafting between 2002 and 2008. Their
mean age was 16.3 years (13 to 19). The stage of the disease at
time of surgery, and results of treatment including pre- and post-operative
Harris hip scores, were studied. We defined clinical failure as
conversion to total hip replacement. All patients were followed
up for a mean of four years (2 to 7). The mean Harris hip score
improved from 60.4 (37 to 84) pre-operatively to 94.2 (87 to 100)

Osseointegrated prosthetic limbs allow better mobility than socket-mounted prosthetics for lower limb amputees. Fractures, however, can occur in the residual limb, but they have rarely been reported. Approximately 2% to 3% of amputees with socket-mounted prostheses may fracture within five years. This is the first study which directly addresses the risks and management of periprosthetic osseointegration fractures in amputees. A retrospective review identified 518 osseointegration procedures which were undertaken in 458 patients between 2010 and 2018 for whom complete medical records were available. Potential risk factors including time since amputation, age at osseointegration, bone density, weight, uni/bilateral implantation and sex were evaluated with multiple logistic regression. The mechanism of injury, technique and implant that was used for fixation of the fracture, pre-osseointegration and post fracture mobility (assessed using the K-level) and the time that the prosthesis was worn for in hours/day were also assessed.Aims
