Our objective in this article is to test the hypothesis that
type 2 diabetes mellitus (T2DM) is a factor in the onset and progression
of osteoarthritis, and to characterise the quality of the articular
cartilage in an appropriate rat model. T2DM rats were obtained from the UC Davis group and compared
with control Lewis rats. The diabetic rats were sacrificed at ages
from six to 12 months, while control rats were sacrificed at six
months only. Osteoarthritis severity was determined via histology
in four knee quadrants using the OARSI scoring guide. Immunohistochemical
staining was also performed as a secondary form of osteoarthritic

Bone sarcomas are rare cancers and orthopaedic
surgeons come across them infrequently, sometimes unexpectedly during
surgical procedures. We investigated the outcomes of patients who
underwent a surgical procedure where sarcomas were found unexpectedly
and were subsequently referred to our unit for treatment. We identified
95 patients (44 intra-lesional excisions, 35 fracture fixations,
16 joint replacements) with mean age of 48 years (11 to 83); 60%

This review considers the surgical treatment
of displaced fractures involving the knee in elderly, osteoporotic patients.
The goals of treatment include pain control, early mobilisation,
avoidance of complications and minimising the need for further surgery.
Open reduction and internal fixation (ORIF) frequently results in
loss of reduction, which can result in post-traumatic arthritis
and the occasional conversion to total knee replacement (TKR). TKR
after failed internal fixation is challenging, with modest functional
outcomes and high complication rates. TKR undertaken as treatment
of the initial fracture has better results to late TKR, but does
not match the outcome of primary TKR without complications. Given
the relatively infrequent need for late TKR following failed fixation,
ORIF is the preferred management for most cases. Early TKR can be
considered for those patients with pre-existing arthritis, bicondylar
femoral fractures, those who would be unable to comply with weight-bearing restrictions,
or where a single definitive procedure is required.
Osteoarthritis is extremely common and many different causes for it have been described. One such cause is abnormal morphology of the affected joint, the hip being a good example of this. For those joints with femoroacetabular impingement (FAI) or developmental dysplasia of the hip (DDH), a link with subsequent osteoarthritis seems clear. However, far from being abnormal, these variants may be explained by evolution, certainly so for FAI, and may actually be normal rather than representing deformity or disease. The animal equivalent of FAI is coxa recta, commonly found in species that run and jump. It is rarely found in animals that climb and swim. In contrast are the animals with coxa rotunda, a perfectly spherical femoral head, and more in keeping with the coxa profunda of mankind. A combined anterior and posterior surgical approach
is generally recommended in the treatment of severe congenital kyphosis,
despite the fact that the anterior vascular supply of the spine
and viscera are at risk during exposure. The aim of this study was
to determine whether the surgical treatment of severe congenital thoracolumbar
kyphosis through a single posterior approach is feasible, safe and
effective. We reviewed the records of ten patients with a mean age of 11.1
years (5.4 to 14.1) who underwent surgery either by pedicle subtraction
osteotomy or by vertebral column resection with instrumented fusion
through a single posterior approach. The mean kyphotic deformity improved from 59.9° (45° to 110°)
pre-operatively to 17.5° (3° to 40°) at a mean follow-up of 47.0
months (29 to 85). Spinal cord monitoring was used in all patients
and there were no complications during surgery. These promising
results indicate the possible advantages of the described technique
over the established procedures. We believe that surgery should
be performed in case of documented progression and before structural
secondary curves develop. Our current strategy after documented
progression is to recommend surgery at the age of five years and

We report a multicentre prospective consecutive
study assessing the long-term outcome of the proximally hydroxyapatite
(HA)-coated ABG II monobloc femoral component in a series of 1148
hips in 1053 patients with a mean age at surgery of 64.77 years

The aim of this study was to identify the incidence of post-operative symptomatic deep-vein thrombosis (DVT), as well as the risk factors for and location of DVT, in 665 patients (701 ankles) who underwent primary total ankle replacement. All patients received low-molecular-weight heparin prophylaxis. A total of 26 patients (3.9%, 26 ankles) had a symptomatic DVT, diagnosed by experienced radiologists using colour Doppler ultrasound. Most thrombi (22 patients, 84.6%) were localised distally in the operated limb. Using a logistic multiple regression model we identified obesity, a previous venous thromboembolic event and the absence of full post-operative weight-bearing as independent risk factors for developing a symptomatic DVT. The incidence of symptomatic DVT after total ankle replacement and use of low-molecular-weight heparin is comparable with that in patients undergoing total knee or hip replacement.

Obesity is an epidemic across both the developed
and developing nations that is possibly the most important current
public health factor affecting the morbidity and mortality of the global
population. Obese patients have the potential to pose several challenges
for arthroplasty surgeons from the standpoint of the influence obesity
has on osteoarthritic symptoms, their peri-operative medical management,
the increased intra-operative technical demands on the surgeon,
the intra- and post-operative complications, the long term outcomes
of total hip and knee arthroplasty. Also, there is no consensus
on the role the arthroplasty surgeon should have in facilitating
weight loss for these patients, nor whether obesity should affect
the access to arthroplasty procedures.
Most problems encountered in complex revision
total knee arthroplasty can be managed with the wide range of implant
systems currently available. Modular metaphyseal sleeves, metallic
augments and cones provide stability even with significant bone
loss. Hinged designs substitute for significant ligamentous deficiencies.
Catastrophic failure that precludes successful reconstruction can
be encountered. The alternatives to arthroplasty in such drastic
situations include knee arthrodesis, resection arthroplasty and
amputation. The relative indications for the selection of these
alternatives are recurrent deep infection, immunocompromised host,
and extensive non-reconstructible bone or soft-tissue defects.
We undertook a retrospective case-control study
to assess the clinical variables associated with infections in open fractures.
A total of 1492 open fractures were retrieved; these were Gustilo
and Anderson grade I in 663 (44.4%), grade II in 370 (24.8%), grade
III in 310 (20.8%) and unclassifiable in 149 (10.0%). The median
duration of prophylaxis was three days (interquartile range (IQR)
1 to 3), and the median number of surgical interventions was two
(1 to 9). We identified 54 infections (3.6%) occurring at a median
of ten days (IQR 5 to 20) after trauma. Pathogens intrinsically
resistant to the empirical antibiotic regimen used (enterococci, Infection in open fractures is related to the extent of tissue
damage but not to the duration of prophylactic antibiotic therapy.
Even for grade III fractures, a one-day course of prophylactic antibiotics

We undertook a retrospective study of 50 consecutive patients (41 male, 9 female) with an infected nonunion and bone defect of the femoral shaft who had been treated by radical debridement and distraction osteogenesis. Their mean age was 29.9 years (9 to 58) and they had a mean of 3.8 (2 to 19) previous operations. They were followed for a mean of 5.9 years (2.0 to 19.0). The mean duration of the distraction osteogenesis was 24.5 months (2 to 39). Pin-track infection was observed in all patients. The range of knee movement was reduced and there was a mean residual leg-length discrepancy of 1.9 cm (0 to 8) after treatment. One patient required hip disarticulation to manage intractable sepsis. In all, 13 patients had persistant pain. Bony union was achieved in 49 patients at a mean of 20.7 months (12 to 35). Although distraction osteogenesis is commonly used for the treatment of infected femoral nonunion with bone defects, it is associated with a high rate of complications.

Symptomatic and asymptomatic deep-vein thrombosis
(DVT) is a common complication of knee replacement, with an incidence
of up to 85% in the absence of prophylaxis. National guidelines
for thromboprophylaxis in knee replacement are derived from total
knee replacement (TKR) data. No guidelines exist specific to unicompartmental knee
replacement (UKR). We investigated whether the type of knee arthroplasty
(TKR or UKR) was related to the incidence of DVT and discuss the
applicability of existing national guidelines for prophylaxis following
UKR. Data were collected prospectively on 3449 knee replacements,
including procedure type, tourniquet time, surgeon, patient age,
use of drains and gender. These variables were related to the incidence
of symptomatic DVT. The overall DVT rate was 1.6%. The only variable that had an
association with DVT was operation type, with TKR having a higher
incidence than UKR (2.2% TKR and UKR patients have different risk profiles for symptomatic
DVT. The risk-benefit ratio for TKR that has been used to produce
national guidelines may not be applicable to UKR. Further research
is required to establish the most appropriate form of prophylaxis
for UKR.
Increasing numbers of posterior lumbar fusions
are being performed. The purpose of this study was to identify trends
in demographics, mortality and major complications in patients undergoing
primary posterior lumbar fusion. We accessed data collected for
the Nationwide Inpatient Sample for each year between 1998 and 2008
and analysed trends in the number of lumbar fusions, mean patient
age, comorbidity burden, length of hospital stay, discharge status,
major peri-operative complications and mortality. An estimated 1 288 496
primary posterior lumbar fusion operations were performed between
1998 and 2008 in the United States. The total number of procedures,
mean patient age and comorbidity burden increased over time. Hospital
length of stay decreased, although the in-hospital mortality (adjusted
and unadjusted for changes in length of hospital stay) remained
stable. However, a significant increase was observed in peri-operative
septic, pulmonary and cardiac complications. Although in-hospital mortality
rates did not change over time in the setting of increases in mean
patient age and comorbidity burden, some major peri-operative complications
increased. These trends highlight the need for appropriate peri-operative services
to optimise outcomes in an increasingly morbid and older population
of patients undergoing lumbar fusion.
Despite the increase in numbers of the extreme elderly, little data is available regarding their outcome after surgery for fracture of the hip. We performed a prospective study of 50 patients aged 95 years and over who underwent this procedure. Outcome measures included morbidity, mortality, hospital stay, residential and walking status. Comparison was made with a control group of 200 consecutive patients aged less than 95 years who had a similar operation. The mortality at 28 and 120 days was higher (p = 0.005, p = 0.001) in the patients over 95 years. However, the one-year cumulative post-operative mortality was neither significantly different between the two groups (p = 0.229) nor from the standardised mortality rate for the age-matched population (p = 0.445). Predictors of mortality included the ASA grade, the number of comorbid medical conditions and active medical problems on admission. Patients over 95 were unlikely to recover their independence and at a mean follow-up of 29.3 months (12.1 to 48) 96% required permanent institutional care.
We examined the morphology of mammalian hips asking whether evolution can explain the morphology of impingement in human hips. We describe two stereotypical mammalian hips, coxa recta and coxa rotunda. Coxa recta is characterised by a straight or aspherical section on the femoral head or head-neck junction. It is a sturdy hip seen mostly in runners and jumpers. Coxa rotunda has a round femoral head with ample head-neck offset, and is seen mostly in climbers and swimmers. Hominid evolution offers an explanation for the variants in hip morphology associated with impingement. The evolutionary conflict between upright gait and the birth of a large-brained fetus is expressed in the female pelvis and hip, and can explain pincer impingement in a coxa profunda. In the male hip, evolution can explain cam impingement in coxa recta as an adaptation for running.
Bilateral simultaneous total knee replacement (TKR) has been considered by some to be associated with increased morbidity and mortality. Our study analysed the outcome of 150 consecutive, but selected, bilateral simultaneous TKRs and compared them with that of 271 unilateral TKRs in a standardised fast-track setting. This study was performed to review the safety and outcome of total shoulder replacements in patients who are ≥ 80 years of age. A total of 50 total shoulder replacements in 44 patients at a mean age of 82 years (80 to 89) were studied. Their health and shoulder status, the operation and post-operative course were analysed, including pain, movement, patient satisfaction, medical and surgical complications, radiographs, the need for revision surgery, and implant and patient survival. A total of 27 patients had an ASA classification of III or IV and medical abnormalities were common. Of the 13 shoulders with bony deficiency of the glenoid, nine required grafting. The duration of hospital stay was prolonged and blood transfusions were common. There were no peri-operative deaths. The mean follow-up was for 5.5 years (2 to 12). Pain was significantly reduced (p <
0.001) and movement improved in active elevation and both external and internal rotation (p <
0.001). Using the Neer scale for assessing outcome, 40 (80%) shoulders had an excellent or satisfactory result. There were medical or surgical complications in 17 cases. Four shoulders developed radiological evidence of loosened glenoid components, and three of these had a poor outcome. Three other shoulders required revision, two for instability. By the time of this review 39 of the patients had died from unrelated causes at a mean of 7.5 years (0.8 to 16.4) after surgery. Total shoulder replacement is a relatively effective treatment in this elderly group of patients. However, there is a requirement for more intense patient care in the peri-operative period, and non-fatal medical or surgical complications are common. Most of these elderly patients will have a comfortable functional shoulder for the rest of their lives.
The purposes of this study were to define the range of laxity of the interosseous ligaments in cadaveric wrists and to determine whether this correlated with age, the morphology of the lunate, the scapholunate (SL) gap or the SL angle. We evaluated 83 fresh-frozen cadaveric wrists and recorded the SL gap and SL angle. Standard arthroscopy of the wrist was then performed and the grades of laxity of the scapholunate interosseous ligament (SLIL) and the lunotriquetral interosseous ligament (LTIL) and the morphology of the lunate were recorded. Arthroscopic evaluation of the SLIL revealed four (5%) grade I specimens, 28 (34%) grade II, 40 (48%) grade III and 11 (13%) grade IV. Evaluation of the LTIL showed 17 (20%) grade I specimens, 40 (48%) grade II, 28 (30%) grade III and one (1%) grade IV. On both bivariate and multivariate analysis, the grade of both the SLIL and LTIL increased with age, but decreased with female gender. The grades of SLIL or LTIL did not correlate with the morphology of the lunate, the SL gap or the SL angle. The physiological range of laxity at the SL and lunotriquetral joints is wider than originally described. The intercarpal ligaments demonstrate an age-related progression of laxity of the SL and lunotriquetral joints. There is no correlation between the grades of laxity of the SLIL or LTIL and the morphology of the lunate, the SL gap or the SL grade. Based on our results, we believe that the Geissler classification has a role in describing intercarpal laxity, but if used alone it cannot adequately diagnose pathological instability. We suggest a modified classification with a mechanism that may distinguish physiological laxity from pathological instability.
