Abnormal knee kinematics following reconstruction
of the anterior cruciate ligament may exist despite an apparent resolution
of tibial laxity and functional benefit. We performed upright, weight-bearing
MR scans of both knees in the sagittal plane at different angles
of flexion to determine the kinematics of the knee following unilateral reconstruction
(n = 12). The uninjured knee acted as a control. Scans were performed
pre-operatively and at three and six months post-operatively. Anteroposterior
tibial laxity was determined using an arthrometer and patient function
by validated questionnaires before and after reconstruction. In
all the knees with deficient anterior cruciate ligaments, the tibial
plateau was displaced anteriorly and internally rotated relative
to the femur when compared with the control contralateral knee,
particularly in extension and early flexion (mean lateral compartment displacement:
extension 7.9 mm ( Our results show that despite improvement in laxity and functional
benefit, abnormal knee kinematics remain at six months and actually
deteriorate from three to six months following reconstruction of
the anterior cruciate ligament.
Gunshot injuries to the shoulder are rare and
difficult to manage. We present a case series of seven patients
who sustained a severe shoulder injury to the non-dominant side
as a result of a self-inflicted gunshot wound. We describe the injury
as ‘suicide shoulder’ caused by upward and outward movement of the
gun barrel as the trigger is pulled. All patients were male, with
a mean age of 32 years (21 to 48). All were treated at the time
of injury with initial repeated debridement, and within four weeks
either by hemiarthroplasty (four patients) or arthrodesis (three patients).
The hemiarthroplasty failed in one patient after 20 years due to
infection and an arthrodesis was attempted, which also failed due
to infection. Overall follow-up was for a mean of 26 months (12
to 44). All four hemiarthroplasty implants were removed with no
feasible reconstruction ultimately possible, resulting in a poor functional
outcome and no return to work. In contrast, all three primary arthrodeses
eventually united, with two patients requiring revision plating
and grafting. These patients returned to work with a good functional
outcome. We recommend arthrodesis rather than replacement as the

In May 2012, in airports across the globe, seven

slowly turned towards their respective aircraft. Filled with expectation
and mild trepidation they stepped into the unknown… the ABC fellowship
of 2012.
Highly active anti-retroviral therapy has transformed HIV into a chronic disease with a long-term asymptomatic phase. As a result, emphasis is shifting to other effects of the virus, aside from immunosuppression and mortality. We have reviewed the current evidence for an association between HIV infection and poor fracture healing. The increased prevalence of osteoporosis and fragility fractures in HIV patients is well recognised. The suggestion that this may be purely as a result of highly active anti-retroviral therapy has been largely rejected. Apart from directly impeding cellular function in bone remodelling, HIV infection is known to cause derangement in the levels of those cytokines involved in fracture healing (particularly tumour necrosis factor-α) and appears to impair the blood supply of bone. Many other factors complicate this issue, including a reduced body mass index, suboptimal nutrition, the effects of anti-retroviral drugs and the avoidance of operative intervention because of high rates of wound infection. However, there are sound molecular and biochemical hypotheses for a direct relationship between HIV infection and impaired fracture healing, and the rewards for further knowledge in this area are extensive in terms of optimised fracture management, reduced patient morbidity and educated resource allocation. Further investigation in this area is overdue.

We used demineralised bone matrix (DBM) to augment re-attachment of tendon to a metal prosthesis in an A significant increase of 23.5% was observed in functional weight-bearing at six weeks in the DBM-augmented group compared with non-augmented controls (p = 0.004). By 12 weeks augmentation with DBM resulted in regeneration of a more direct-type enthesis, with regions of fibrocartilage, mineralised fibrocartilage and bone. In the controls the interface was predominantly indirect, with the tendon attached to the bone graft-hydroxyapatite base plate by perforating collagen fibres.
Peri-prosthetic infection remains a leading cause
of revision surgery. Recent publications from the American Musculoskeletal
Infection Society have sought to establish a definition of peri-prosthetic
infection based on clinical findings and laboratory investigations.
The limitations of their approach are discussed and an alternative
definition is proposed, which it is felt may better reflect the
uncertainties encountered in clinical practice.
Treatment for osteoarthritis (OA) has traditionally
focused on joint replacement for end-stage disease. An increasing number
of surgical and pharmaceutical strategies for disease prevention
have now been proposed. However, these require the ability to identify
OA at a stage when it is potentially reversible, and detect small
changes in cartilage structure and function to enable treatment
efficacy to be evaluated within an acceptable timeframe. This has
not been possible using conventional imaging techniques but recent
advances in musculoskeletal imaging have been significant. In this
review we discuss the role of different imaging modalities in the
diagnosis of the earliest changes of OA. The increasing number of
MRI sequences that are able to non-invasively detect biochemical
changes in cartilage that precede structural damage may offer a
great advance in the diagnosis and treatment of this debilitating

Bovine and human articular chondrocytes were seeded in 2% alginate constructs and cultured for up to 19 days in a rotating-wall-vessel (RWV) and under static conditions. Culture within the RWV enhanced DNA levels for bovine chondrocyte-seeded constructs when compared with static conditions but did not produce enhancement for human cells. There was a significant enhancement of glycosaminoglycans and hydroxyproline synthesis for both bovine and human chondrocytes. In all cases, histological analysis revealed enhanced Safranin-O staining in the peripheral regions of the constructs compared with the central region. There was an overall increase in staining intensity after culture within the RWV compared with static conditions. Type-II collagen was produced by both bovine and human chondrocytes in the peripheral and central regions of the constructs and the staining intensity was enhanced by culture within the RWV. A capsule of flattened cells containing type-I collagen developed around the constructs maintained under static conditions when seeded with either bovine or human chondrocytes, but not when cultured within the RWV bioreactor.
We examined the mechanical properties of Vicryl (polyglactin 910) mesh Mesh fibres were visible at six weeks but had been completely resorbed by 12 weeks, with no evidence of chronic inflammation. The tendon-implant neoenthesis was predominantly an indirect type, with tendon attached to the bone-hydroxyapatite surface by perforating collagen fibres.
We compared the intracompartmental pressures
(ICPs) of open and closed tibial fractures with the same injury pattern
in a rabbit model. In all, 20 six-month-old New Zealand White male
rabbits were used. They were randomised into two equal groups of
ten rabbits; an open fracture group (group 1) and a closed fracture
group (group 2). Each anaesthetised rabbit was subjected to a standardised
fracture of the proximal half of the right tibia using a custom-made
device. In order to create a grade II open fracture in group 1,
a 10 mm segment of fascia and periosteum was excised. The ICP in
the anterior compartment was monitored at six-hourly intervals for
48 hours. Although there was a statistically significant difference
in ICP values within each group (both p <
0.001), there was no
significant difference between the groups for all measurements (all
p ≥ 0.089). In addition, in both groups there was a statistically
significant increase in ICP within the first 24 hours, whereas there
was a statistically significant decrease within the second 24 hours
(p <
0.001 for both groups). We conclude that open tibial fractures
should be monitored for the development of acute compartment syndrome

Large femoral heads have been used with increasing
frequency over the last decade. The prime reason is likely the effect
of large heads on stability. The larger head neck ratio, combined
with the increased jump distance of larger heads result in a greater
arc of impingement free motion, and greater resistance to dislocation
in a provocative position. Multiple studies have demonstrated clear
clinical efficacy in diminishing dislocation rates with the use
of large femoral heads. With crosslinked polyethylene, wear has
been shown to be equivalent between larger and smaller heads. However,
the stability advantages of increasing diameter beyond 38 mm have
not been clearly demonstrated. More importantly, recent data implicates
large heads in the increasing prevalence of groin pain and psoas impingement.
There are clear benefits with larger femoral head diameters, but
the advantages of diameters beyond 38 mm have not yet been demonstrated
clinically.
Lately, concerns have arisen following the use of large metal-on-metal bearings in hip replacements owing to reports of catastrophic soft-tissue reactions resulting in implant failure and associated complications. This review examines the literature and contemporary presentations on current clinical dilemmas in metal-on-metal hip replacement.
To review the current best surgical practice and detail a multi-disciplinary
approach that could further reduce joint replacement infection. Review of relevant literature indexed in PubMed.Objectives
Methods
This paper reviews the current literature concerning the main clinical factors which can impair the healing of fractures and makes recommendations on avoiding or minimising these in order to optimise the outcome for patients. The clinical implications are described.
Pain, swelling and inflammation are expected
during the recovery from total knee arthroplasty (TKA) surgery.
The severity of these factors and how a patient copes with them
may determine the ultimate outcome of a TKA. Cryotherapy and compression
are frequently used modalities to mitigate these commonly experienced
sequelae. However, their effect on range of motion, functional testing,
and narcotic consumption has not been well-studied. A prospective, multi-center, randomised trial was conducted to
evaluate the effect of a cryopneumatic device on post-operative
TKA recovery. Patients were randomised to treatment with a cryopneumatic
device or ice with static compression. A total of 280 patients were
enrolled at 11 international sites. Both treatments were initiated
within three hours post-operation and used at least four times per
day for two weeks. The cryopneumatic device was titrated for cooling
and pressure by the patient to their comfort level. Patients were evaluated by physical therapists blinded to the
treatment arm. Range of motion (ROM), knee girth, six minute walk
test (6MWT) and timed up and go test (TUG) were measured pre-operatively,
two- and six-weeks post-operatively. A visual analog pain score
and narcotic consumption was also measured post-operatively. At two weeks post-operatively, both the treatment and control
groups had diminished ROM and function compared to pre-operatively.
Both groups had increased knee girth compared to pre- operatively.
There was no significant difference in ROM, 6MWT, TUG, or knee girth
between the 2 groups. We did find a significantly lower amount of
narcotic consumption (509 mg morphine equivalents) in the treatment
group compared with the control group (680 mg morphine equivalents)
at up to two weeks postop, when the cryopneumatic device was being
used (p <
0.05). Between two and six weeks, there was no difference
in the total amount of narcotics consumed between the two groups.
At six weeks, there was a trend toward a greater distance walked
in the 6MWT in the treatment group (29.4 meters A cryopneumatic device used after TKA appeared to decrease the
need for narcotic medication from hospital discharge to 2 weeks
post-operatively. There was also a trend toward a greater distance
walked in the 6MWT. Patient satisfaction with the cryopneumatic
cooling regimen was significantly higher than with the control treatment.
