The aims of this study were to determine the proportion of patients
with outlier varus or valgus alignment in kinematically aligned
total knee arthroplasty (TKA), whether those with outlier varus
or valgus alignment have higher forces in the medial or lateral
compartments of the knee than those with in-range alignment and
whether measurements of the alignment of the limb, knee and components
predict compartment forces. The intra-operative forces in the medial and lateral compartments
were measured with an instrumented tibial insert in 67 patients
who underwent a kinematically aligned TKA during passive movement.
The mean of the forces at full extension, 45° and 90° of flexion
determined the force in the medial and lateral compartments. Measurements
of the alignment of the limb and the components included the hip-knee-ankle
(HKA) angle, proximal medial tibial angle (PMTA), and distal lateral
femoral angle (DLFA). Measurements of the alignment of the knee
and the components included the tibiofemoral angle (TFA), tibial
component angle (TCA) and femoral component angle (FCA). Alignment was
measured on post-operative, non-weight-bearing anteroposterior (AP)
scanograms and categorised as varus or valgus outlier or in-range
in relation to mechanically aligned criteria.Aims

Patient-specific glenoid guides (PSGs) claim an improvement in
accuracy and reproducibility of the positioning of components in
total shoulder arthroplasty (TSA). The results have not yet been
confirmed in a prospective clinical trial. Our aim was to assess
whether the use of PSGs in patients with osteoarthritis of the shoulder
would allow accurate and reliable implantation of the glenoid component. A total of 17 patients (three men and 14 women) with a mean age
of 71 years (53 to 81) awaiting TSA were enrolled in the study.
Pre- and post-operative version and inclination of the glenoid were
measured on CT scans, using 3D planning automatic software. During
surgery, a congruent 3D-printed PSG was applied onto the glenoid
surface, thus determining the entry point and orientation of the
central guide wire used for reaming the glenoid and the introduction
of the component. Manual segmentation was performed on post-operative
CT scans to compare the planned and the actual position of the entry
point (mm) and orientation of the component (°).Aims

Little biomechanical information is available about kinematically aligned (KA) total knee arthroplasty (TKA). The purpose of this study was to simulate the kinematics and kinetics after KA TKA and mechanically aligned (MA) TKA with four different limb alignments. Bone models were constructed from one volunteer (normal) and three patients with three different knee deformities (slight, moderate and severe varus). A dynamic musculoskeletal modelling system was used to analyse the kinematics and the tibiofemoral contact force. Our aim was to evaluate the radiographic characteristics of patients
undergoing total hip arthroplasty (THA) for the potential of posterior
bony impingement using CT simulations. Virtual CT data from 112 patients who underwent THA were analysed.
There were 40 men and 72 women. Their mean age was 59.1 years (41
to 76). Associations between radiographic characteristics and posterior
bony impingement and the range of external rotation of the hip were
evaluated. In addition, we investigated the effects of pelvic tilt
and the neck/shaft angle and femoral offset on posterior bony impingement.Aims

We undertook a prospective non-randomised radiological study
to evaluate the preliminary results of using magnetically-controlled
growing rods (MAGEC System, Ellipse technology) to treat children
with early-onset scoliosis. Between January 2011 and January 2015, 19 children were treated
with magnetically-controlled growing rods (MCGRs) and underwent
distraction at three-monthly intervals. The mean age of our cohort
was 9.1 years (4 to 14) and the mean follow-up 22.4 months (5.1
to 35.2). Of the 19 children, eight underwent conversion from traditional growing
rods. Whole spine radiographs were carried out pre- and post-operatively:
image intensification was used during each lengthening in the outpatient
department. The measurements evaluated were Cobb angle, thoracic kyphosis,

Femoroacetabular impingement (FAI) has been highlighted and well
documented primarily in Western countries and there are few large
studies focused on FAI-related morphological assessment in Asian
patients. We chose to investigate this subject. We assessed the morphology of the hip and the prevalence of radiographic
FAI in Japanese patients by measuring predictors of FAI. We reviewed
a total of 1178 hips in 695 men and 483 women with a mean age of
58.2 years (20 to 89) using CT images that had been obtained for
reasons unrelated to symptoms from the hip. We measured the lateral
centre edge angle, acetabular index, crossover sign, alpha angle

Malrotation of the femoral component can result in post-operative complications in total knee arthroplasty (TKA), including patellar maltracking. Therefore, we used computational simulation to investigate the influence of femoral malrotation on contact stresses on the polyethylene (PE) insert and on the patellar button as well as on the forces on the collateral ligaments. Validated finite element (FE) models, for internal and external malrotations from 0° to 10° with regard to the neutral position, were developed to evaluate the effect of malrotation on the femoral component in TKA. We aimed to investigate factors related to the technique of medial
opening wedge high tibial osteotomy which might predispose to the
development of a lateral hinge fracture. A total of 71 patients with 82 osteotomies were included in the
study. Their mean age was 62.9 years (37 to 80). The classification
of the type of osteotomy was based on whether it extended beyond
the fibular head. The level of the osteotomy was classified according

This prospective randomised controlled trial was designed to
evaluate the outcome of both the MRI- and CT-based patient-specific
matched guides (PSG) from the same manufacturer. A total of 137 knees in 137 patients (50 men, 87 women) were
included, 67 in the MRI- and 70 in the CT-based PSG group. Their
mean age was 68.4 years (47.0 to 88.9). Outcome was expressed as
the biomechanical limb alignment (centre hip-knee-ankle: HKA-axis)
achieved post-operatively, the position of the individual components
within 3° of the pre-operatively planned alignment, correct planned

The aim of this study was to assess hypertrophy of the extra-articular
tendon of the long head of biceps (LHB) in patients with a rotator
cuff tear. The study involved 638 shoulders in 334 patients (175 men, 159
women, mean age 62.6 years; 25 to 81) with unilateral symptomatic
rotator cuff tears. The cross-sectional area (CSA) of the LHB tendon
in the bicipital groove was measured pre-operatively in both shoulders
using ultrasound. There were 154 asymptomatic rotator cuff tears
in the contralateral shoulder. Comparisons were made between those
with a symptomatic tear, an asymptomatic tear and those with no
rotator cuff tear. In the affected shoulders, the CSAs were compared
in relation to the location and size of the rotator cuff tear. Aims

The aim of this study was to report the outcome of the non-operative
treatment of high-grade posterior cruciate ligament (PCL) injuries,
particularly Hughston grade III injuries, which have not previously
been described. This was a prospective study involving 46 consecutive patients
who were athletes with MRI-confirmed isolated PCL injuries presenting
within four weeks of injury. All had Hughston grade II (25 athletes)
or III (21 athletes) injuries. Our non-operative treatment regimen
involved initial bracing, followed by an individualised rehabilitation
programme determined by the symptoms and physical signs. The patients
were reviewed until they had returned to sports-specific training,
and were reviewed again at a mean of 5.2 years (3 to 9).Aims

A self-control ratio, the spine-pelvis index
(SPI), was proposed for the assessment of patients with adolescent idiopathic
scoliosis (AIS) in this study. The aim was to evaluate the disproportionate
growth between the spine and pelvis in these patients using SPI.
A total of 64 female patients with thoracic AIS were randomly enrolled
between December 2010 and October 2012 (mean age 13 years, standard
deviation ( No significant difference in SPI was found in different age groups
in the control group, making the SPI an age-independent parameter
with a mean value of 2.219 (2.164 to 2.239). We also found that
the SPI was not related to maturity in the control group. This study, for the first time, used a self-control ratio to
confirm the disproportionate patterns of growth of the spine and
pelvis in patients with thoracic AIS, highlighting that the SPI

There is no consensus about the best method of achieving equal
leg lengths at total hip arthroplasty (THA) in patients with Crowe
type-IV developmental dysplasia of the hip (DDH). We reviewed our
experience of a consecutive series of patients who underwent THA
for this indication. We retrospectively reviewed 78 patients (86 THAs) with Crowe
type-IV DDH, including 64 women and 14 men, with a minimum follow-up
of two years. The mean age at the time of surgery was 52.2 years
(34 to 82). We subdivided Crowe type-IV DDH into two major types
according to the number of dislocated hips, and further categorised
them into three groups according to the occurrence of pelvic obliquity
or spinal curvature. Leg length discrepancy (LLD) and functional

The purpose of the present study was to examine the long-term
fixation of a cemented fixed-bearing polished titanium tibial baseplate
(Genesis ll). Patients enrolled in a previous two-year prospective trial (n
= 35) were recalled at ten years. Available patients (n = 15) underwent
radiostereometric analysis (RSA) imaging in a supine position using
a conventional RSA protocol. Migration of the tibial component in
all planes was compared between initial and ten-year follow-up.
Outcome scores including the Knee Society Score, Western Ontario

Survey, Forgotten Joint Score, and University of California, Los

Radiostereometric analysis (RSA) allows an extremely accurate
measurement of early micromotion of components following arthroplasty. In this study, RSA was used to measure the migration of 11 partially
cemented fluted pegged glenoid components in patients with osteoarthritis
who underwent total shoulder arthroplasty using an improved surgical
technique (seven men, four women, mean age 68). Patients were evaluated
clinically using the American Shoulder and Elbow Surgeons (ASES)
and Constant-Murley scores and by CT scans two years post-operatively. Aims

The purpose of this study was to compare the thickness of the hip capsule in patients with surgical hip disease, either with cam-femoroacetabular impingement (FAI) or non-FAI hip pathology, with that of asymptomatic control hips. A total of 56 hips in 55 patients underwent a 3Tesla MRI of the hip. These included 40 patients with 41 hips with arthroscopically proven hip disease (16 with cam-FAI; nine men, seven women; mean age 39 years, 22 to 58) and 25 with non-FAI chondrolabral pathology (four men, 21 women; mean age 40 years, 18 to 63) as well as 15 asymptomatic volunteers, whose hips served as controls (ten men, five women; mean age 62 years, 33 to 77). Pelvic reconstruction after the resection of a tumour around
the acetabulum is a challenging procedure due to the complex anatomy
and biomechanics. Several pelvic endoprostheses have been introduced,
but the rates of complication remain high. Our aim was to review
the use of a stemmed acetabular pedestal cup in the management of
these patients. The study involved 48 patients who underwent periacetabular reconstruction
using a stemmed pedestal cup (Schoellner cup; Zimmer Biomet Inc.,
Warsaw, Indiana) between 2000 and 2013. The indications for treatment included
a primary bone tumour in 27 patients and metastatic disease in 21
patients. The mean age of the patients at the time of surgery was
