The aim of this study was to compare the maximum
laxity conferred by the cruciate-retaining (CR) and posterior-stabilised
(PS) Triathlon single-radius total knee arthroplasty (TKA) for anterior
drawer, varus–valgus opening and rotation in eight cadaver knees
through a defined arc of flexion (0º to 110º). The null hypothesis
was that the limits of laxity of CR- and PS-TKAs are not significantly
different. The investigation was undertaken in eight loaded cadaver knees
undergoing subjective stress testing using a measurement rig. Firstly
the native knee was tested prior to preparation for CR-TKA and subsequently
for PS-TKA implantation. Surgical navigation was used to track maximal
displacements/rotations at 0º, 30º, 60º, 90º and 110° of flexion.
Mixed-effects modelling was used to define the behaviour of the
TKAs. The laxity measured for the CR- and PS-TKAs revealed no statistically
significant differences over the studied flexion arc for the two
versions of TKA. Compared with the native knee both TKAs exhibited
slightly increased anterior drawer and decreased varus-valgus and
internal-external roational laxities. We believe further study is required
to define the clinical states for which the additional constraint
offered by a PS-TKA implant may be beneficial. Cite this article:
Many aspects of total knee arthroplasty have
changed since its inception. Modern prosthetic design, better fixation techniques,
improved polyethylene wear characteristics and rehabilitation, have
all contributed to a large change in revision rates. Arthroplasty
patients now expect longevity of their prostheses and demand functional
improvement to match. This has led to a re-examination of the long-held
belief that mechanical alignment is instrumental to a successful
outcome and a focus on restoring healthy joint kinematics. A combination
of kinematic restoration and uncemented, adaptable fixation may

A national, multi-centre study was designed in
which a questionnaire quantifying the degree of patient satisfaction
and residual symptoms in patients following total knee replacement
(TKR) was administered by an independent, blinded third party survey
centre. A total of 90% of patients reported satisfaction with the
overall functioning of their knee, but 66% felt their knee to be
‘normal’, with the reported incidence of residual symptoms and functional
problems ranging from 33% to 54%. Female patients and patients from
low-income households had increased odds of reporting dissatisfaction.
Neither the use of contemporary implant designs (gender-specific,
high-flex, rotating platform) or custom cutting guides (CCG) with
a neutral mechanical axis target improved patient-perceived outcomes.
However, use of a CCG to perform a so-called kinematically aligned
TKR showed a trend towards more patients reporting their knee to
feel ‘normal’ when compared with a so called mechanically aligned
TKR This data shows a degree of dissatisfaction and residual symptoms
following TKR, and that several recent modifications in implant

The purpose of this study was to investigate
whether a gender-specific high-flexion posterior-stabilised (PS)
total knee replacement (TKR) would offer advantages over a high-flex
PS TKR regarding range of movement (ROM), ‘feel’ of the knee, pain
and satisfaction, as well as during activity. A total of 24 female
patients with bilateral osteoarthritis entered this prospective,
blind randomised trial in which they received a high-flex PS TKR
in one knee and a gender-specific high-flexion PS TKR in the other
knee. At follow-up, patients were assessed clinically measuring
ROM, and questioned about pain, satisfaction and daily ‘feel’ of
each knee. Patients underwent gait analysis pre-operatively and
at one year, which yielded kinematic, kinetic and temporospatial
parameters indicative of knee function during gait. At final follow-up
we found no statistically significant differences in ROM (p = 0.82).
The median pain score was 0 (0 to 8) in both groups (p = 0.95).
The median satisfaction score was 9 (4 to 10) in the high-flex group
and 8 (0 to 10) in the gender-specific group (p = 0.98). The median
‘feel’ score was 9 (3 to 10) in the high-flex group and 8 (0 to
10) in the gender-specific group (p = 0.66). Gait analysis showed
no statistically significant differences between the two prosthetic
designs in any kinematic, kinetic or temporospatial parameters. Both designs produced good clinical results with

A new generation of knee prostheses has been introduced with the intention of improving post-operative knee flexion. In order to evaluate whether this goal has been achieved we performed a systematic review and meta-analysis. Systematic literature searches were conducted on MEDLINE and EMBASE from their inception to December 2007, and proceedings of scientific meetings were also searched. Only randomised, clinical trials were included in the meta-analysis. The mean difference in the maximum post-operative flexion between the ‘high-flex’ and conventional types of prosthesis was defined as the primary outcome measure. A total of five relevant articles was identified. Analysis of these trials suggested that no clinically relevant or statistically significant improvement was obtained in flexion with the ‘high-flex’ prostheses. The weighted mean difference was 2.1° (95% confidence interval −0.2 to +4.3; p = 0.07).
Bone mineral density (BMD) around the femoral component has been reported to decrease after total knee replacement (TKR) because of stress shielding. Our aim was to determine whether a cemented mobile-bearing component reduced the post-operative loss of BMD. In our study 28 knees receiving a cemented fixed-bearing TKR were matched with 28 receiving a cemented mobile-bearing TKR. They underwent dual-energy x-ray absorptiometry, pre-operatively and at three weeks and at three, six, 12, 18 and 24 months post-operatively. The patients were not taking medication to improve the BMD. The pre-operative differences in the BMD of the femoral neck, wrist, lumbar spine and knee in the two groups were not significant. The BMD of the femur decreased postoperatively in the fixed-bearing group, but not the mobile-bearing group. The difference in the post-operative change in the BMD in the two groups was statistically significant (p <
0.05) at 18 and 24 months. Our findings show that a cemented mobile-bearing TKR has a favourable effect on the BMD of the distal femur after TKR in the short term. Further study is required to determine the long-term effects.
We have examined the results obtained with 72 NexGen legacy posterior stabilised-flex fixed total knee replacements in 47 patients implanted by a single surgeon between March 2003 and September 2004. Aseptic loosening of the femoral component was found in 27 (38%) of the replacements at a mean follow-up of 32 months (30 to 48) and 15 knees (21%) required revision at a mean of 23 months (11 to 45). We compared the radiologically-loose and revised knees with those which had remained well-fixed to identify the factors which had contributed to this high rate of aseptic loosening. Post-operatively, the mean maximum flexion was 136° (110° to 140°) in the loosened group and 125° (95° to 140°) in the well-fixed group (independent These implants allowed a high degree of flexion, but showed a marked rate of early loosening of the femoral component, which was associated with weight-bearing in maximum flexion.
The results of 41 consecutive total knee replacements performed on morbidly obese patients with a body mass index >
40 kg/m2, were compared with a matched group of 41 similar procedures carried out in non-obese patients (body mass index <
30 kg/m2). The groups were matched for age, gender, diagnosis, type of prosthesis, laterality and pre-operative Knee Society Score. We prospectively followed up the patients for a mean of 38.5 months (6 to 66). No patients were lost to follow-up. At less than four years after operation, the results were worse in the morbidly obese group compared with the non-obese, as demonstrated by inferior Knee Society Scores (mean knee score 85.7 and 90.5 respectively, p = 0.08; mean function score 75.6 and 83.4, p = 0.01), a higher incidence of radiolucent lines on post-operative radiographs (29% and 7%, respectively, p = 0.02), a higher rate of complications (32% and 0%, respectively, p = 0.001) and inferior survivorship using revision and pain as end-points (72.3% and 97.6%, respectively, p = 0.02). Patients with a body mass index >
40 kg/m2 should be advised to lose weight prior to total knee replacement and to maintain weight reduction. They should also be counselled regarding the inferior results which may occur if they do not lose weight before surgery.
