Objectives. The most prevalent disorders of the shoulder are related to the
muscles of rotator cuff. In order to develop a mechanical method
for the evaluation of the rotator cuff muscles, we created a database
of isometric force generation by the rotator cuff muscles in normal
adult population. We hypothesised the existence of variations according
to age, gender and dominancy of limb. Methods. A total of 400 healthy adult volunteers were tested, classified
into groups of 50 men and women for each decade of life. Maximal
isometric force was measured at standardised positions for supraspinatus,
infraspinatus and subscapularis muscles in both shoulders in every
person. Torque of the force was calculated and normalised to lean body
mass. The profiles of mean torque-time curves for each age and gender
group were compared. Results. Our data showed that men gradually gained maximal strength in
the fifth decade, and showed decreased strength in the sixth. In
women the maximal strength was gained in the fourth decade with
gradual decline to the sixth decade of life. The dominant arm was
stronger in most of the tested groups. The torque profiles of the
rotator cuff muscles in men at all ages were significantly higher
than that in women. Conclusions. We found previously unrecognised variations of rotator cuff muscles’
isometric strength according to age, gender and dominancy in a normal
population. The presented data may serve as a basis for the future
studies for identification of the abnormal patterns of muscle isometric

Objectives. The goal of this study was to determine whether intra-articular
administration of the potentially anti-fibrotic agent decorin influences
the expression of genes involved in the fibrotic cascade, and ultimately
leads to less contracture, in an animal model. Methods. A total of 18 rabbits underwent an operation on their right knees
to form contractures. Six limbs in group 1 received four intra-articular
injections of decorin; six limbs in group 2 received four intra-articular
injections of bovine serum albumin (BSA) over eight days; six limbs
in group 3 received no injections. The contracted limbs of rabbits
in group 1 were biomechanically and genetically compared with the
contracted limbs of rabbits in groups 2 and 3, with the use of a
calibrated joint measuring device and custom microarray, respectively. Results. There was no statistical difference in the flexion contracture
angles between those limbs that received intra-articular decorin versus those
that received intra-articular BSA (66° vs 69°;
p = 0.41). Likewise, there was no statistical difference between
those limbs that received intra-articular decorin versus those
who had no injection (66° vs 72°; p = 0.27). When
compared with BSA, decorin led to a statistically significant increase
in the mRNA expression of 12 genes (p <
0.01). In addition, there
was a statistical change in the mRNA expression of three genes,
when compared with those without injection. . Conclusions. In this model, when administered intra-articularly at eight weeks,
2 mg of decorin had no significant effect on joint contractures.
However, our genetic analysis revealed a significant alteration

Objective. This study compared the primary stability of two commercially
available acetabular components from the same manufacturer, which
differ only in geometry; a hemispherical and a peripherally enhanced
design (peripheral self-locking (PSL)). The objective was to determine
whether altered geometry resulted in better primary stability. Methods. Acetabular components were seated with 0.8 mm to 2 mm interference
fits in reamed polyethylene bone substrate of two different densities
(0.22 g/cm. 3. and 0.45 g/cm. 3. ). The primary stability
of each component design was investigated by measuring the peak
failure load during uniaxial pull-out and tangential lever-out tests. Results. There was no statistically significant difference in seating
force (p = 0.104) or primary stability (pull-out p = 0.171, lever-out
p = 0.087) of the two components in the low-density substrate. Similarly,
in the high-density substrate, there was no statistically significant
difference in the peak pull-out force (p = 0.154) or lever-out moment
(p = 0.574) between the designs. However, the PSL component required
a significantly higher seating force than the hemispherical cup
in the high-density bone analogue (p = 0.006). Conclusions. Higher seating forces associated with the PSL design may result
in inadequate seating and increased risk of component malpositioning
or acetabular fracture in the intra-operative setting in high-density
bone stock. Our results, if translated clinically, suggest that
a purely hemispherical geometry may have an advantage over a peripherally

Objectives. The period of post-operative treatment before surgical wounds
are completely closed remains a key window, during which one can
apply new technologies that can minimise complications. One such
technology is the use of negative pressure wound therapy to manage
and accelerate healing of the closed incisional wound (incisional
NPWT). . Methods. We undertook a literature review of this emerging indication
to identify evidence within orthopaedic surgery and other surgical
disciplines. Literature that supports our current understanding
of the mechanisms of action was also reviewed in detail. . Results. A total of 33 publications were identified, including nine clinical
study reports from orthopaedic surgery; four from cardiothoracic
surgery and 12 from studies in abdominal, plastic and vascular disciplines.
Most papers (26 of 33) had been published within the past three
years. Thus far two randomised controlled trials – one in orthopaedic
and one in cardiothoracic surgery – show evidence of reduced incidence
of wound healing complications after between three and five days
of post-operative NPWT of two- and four-fold, respectively. Investigations
show that reduction in haematoma and seroma, accelerated wound healing
and increased clearance of oedema are significant mechanisms of
action. . Conclusions. There is a rapidly emerging literature on the effect of NPWT
on the closed incision. Initiated and confirmed first with a randomised
controlled trial in orthopaedic trauma surgery, studies in abdominal,
plastic and vascular surgery with high rates of complications have
been reported recently. The evidence from single-use NPWT devices
is accumulating. There are no large randomised studies yet in reconstructive

Objectives. We aimed to examine the characteristics of deep venous flow in
the leg in a cast and the effects of a wearable neuromuscular stimulator
(geko; FirstKind Ltd) and also to explore the participants’ tolerance
of the stimulator. Methods. This is an open-label physiological study on ten healthy volunteers.
Duplex ultrasonography of the superficial femoral vein measured
normal flow and cross-sectional area in the standing and supine
positions (with the lower limb initially horizontal and then elevated).
Flow measurements were repeated during activation of the geko stimulator
placed over the peroneal nerve. The process was repeated after the
application of a below-knee cast. Participants evaluated discomfort
using a questionnaire (verbal rating score) and a scoring index
(visual analogue scale). Results. The geko device was effective in significantly increasing venous
blood flow in the lower limb both with a plaster cast (mean difference
11.5 cm/sec. -1. ; p = 0.001 to 0.13) and without a plaster
cast (mean difference 7.7 cm/sec. -1. ; p = 0.001 to 0.75).
Posture also had a significant effect on peak venous blood flow
when the cast was on and the geko inactive (p = 0.003 to 0.69),
although these differences were less pronounced than the effect
of the geko (mean difference 3.1 cm/sec. -1. (-6.5 to 10)).
The geko device was well tolerated, with participants generally
reporting only mild discomfort using the device. Conclusion. The geko device increases venous blood flow in the lower limb,
offering a potential mechanical thromboprolylaxis for patients in

Objectives. Traumatic brachial plexus injury causes severe functional impairment
of the arm. Elbow flexion is often affected. Nerve surgery or tendon
transfers provide the only means to obtain improved elbow flexion.
Unfortunately, the functionality of the arm often remains insufficient.
Stem cell therapy could potentially improve muscle strength and
avoid muscle-tendon transfer. This pilot study assesses the safety
and regenerative potential of autologous bone marrow-derived mononuclear
cell injection in partially denervated biceps. Methods. Nine brachial plexus patients with insufficient elbow flexion
(i.e., partial denervation) received intramuscular escalating doses
of autologous bone marrow-derived mononuclear cells, combined with
tendon transfers. Effect parameters included biceps biopsies, motor
unit analysis on needle electromyography and computerised muscle tomography,
before and after cell therapy. Results. No adverse effects in vital signs, bone marrow aspiration sites,
injection sites, or surgical wound were seen. After cell therapy
there was a 52% decrease in muscle fibrosis (p = 0.01), an 80% increase
in myofibre diameter (p = 0.007), a 50% increase in satellite cells
(p = 0.045) and an 83% increase in capillary-to-myofibre ratio (p
<
0.001) was shown. CT analysis demonstrated a 48% decrease in
mean muscle density (p = 0.009). Motor unit analysis showed a mean
increase of 36% in motor unit amplitude (p = 0.045), 22% increase
in duration (p = 0.005) and 29% increase in number of phases (p
= 0.002). Conclusions. Mononuclear cell injection in partly denervated muscle of brachial
plexus patients is safe. The results suggest enhanced muscle reinnervation

Objectives. The development of tibiofemoral angle in children has shown ethnic
variations. However this data is unavailable for our population. Methods. We measured the tibiofemoral angle (TFA) and intercondylar and
intermalleolar distances in 360 children aged between two and 18
years, dividing them into six interrupted age group intervals: two
to three years; five to six years; eight to nine years; 11 to 12
years; 14 to 15Â years; and 17 to 18 years. Each age group comprised
30 boys and 30 girls. Other variables recorded included standing
height, sitting height, weight, thigh length, leg length and length
of the lower limb. Results. Children aged two to three years had a valgus angulation with
a mean TFA of 1.8° (. sd. 0.65) in boys and 2.45° (. sd. 0.87)
in girls. Peak valgus was seen in the five- to six-year age group,
with mean TFAs of 6.7° (. sd. 1.3) and 7.25° (. sd. 0.64)
for boys and girls, respectively. From this age the values gradually
declined to a mean of 3.18° (. sd. 1.74) and 4.43° (. sd. 0.68)
for boys and girls, respectively, at 17 to 18 years. Girls showed
a higher valgus angulation than boys at all age groups. Conclusion. This study defines the normal range of the TFA in south Indian
boys and girls using an easy and reliable technique of measurement

Aims. Osteoporosis and abnormal bone metabolism may prove to be significant
factors influencing the outcome of arthroplasty surgery, predisposing
to complications of aseptic loosening and peri-prosthetic fracture.
We aimed to investigate baseline bone mineral density (BMD) and
bone turnover in patients about to undergo arthroplasty of the hip
and knee. Methods. We prospectively measured bone mineral density of the hip and
lumbar spine using dual-energy X-ray absorptiometry (DEXA) scans
in a cohort of 194 patients awaiting hip or knee arthroplasty. We
also assessed bone turnover using urinary deoxypyridinoline (DPD),
a type I collagen crosslink, normalised to creatinine. Results. The prevalence of DEXA proven hip osteoporosis (T-score ≤ -2.5)
among hip and knee arthroplasty patients was found to be low at
2.8% (4 of 143). Spinal osteoporosis prevalence was higher at 6.9%
(12 of 175). Sixty patients (42% (60 of 143)) had osteopenia or
osteoporosis of either the hip or spine. The mean T-score for the
hip was -0.34 (. sd. 1.23), which is within normal limits,
and the mean hip Z-score was positive at 0.87 (. sd. 1.17),
signifying higher-than-average BMD for age. The median urinary DPD/creatinine
was raised in both female patients at 8.1 (interquartile range (IQR)
6.6 to 9.9) and male patients at 6.2 (IQR 4.8 to 7.5). Conclusions. Our results indicate hip and knee arthroplasty patients have
higher BMD of the hip and spine compared with an age-matched general
population, and a lower prevalence of osteoporosis. However, untreated
osteoporotic patients are undergoing arthroplasty, which may negatively
impact their outcome. Raised DPD levels suggest abnormal bone turnover,

Background. Resveratrol is a polyphenolic compound commonly found in the
skins of red grapes. Sirtuin 1 (SIRT1) is a human gene that is activated
by resveratrol and has been shown to promote longevity and boost
mitochondrial metabolism. We examined the effect of resveratrol
on normal and osteoarthritic (OA) human chondrocytes. Methods. Normal and OA chondrocytes were incubated with various concentrations
of resveratrol (1 µM, 10 µM, 25 µM and 50 µM) and cultured for 24,
48 or 72 hours or for six weeks. Cell proliferation, gene expression,
and senescence were evaluated. Results. SIRT1 was significantly upregulated in normal chondrocytes with
resveratrol concentrations of 25 µM and 50 µM on both two- (2D)
(both p = 0.001) and three-dimensional (3D) cultures (p = 0.008
and 0.001, respectively). It was significantly upregulated in OA
chondrocytes treated with 10 µM, 25 µM and 50 µM resveratrol on
2D cultures (p = 0.036, 0.002 and 0.001, respectively) and at 50
µM concentration on 3D cultures (p = 0.001). At 72 hours, the expression
of collagen (COL)-10, aggrecan (AGG), and runt-related transcription
factor 2 (RUNX2) was significantly greater in both 25 µM (p = 0.011,
0.006 and 0.015, respectively) and 50 µM (p = 0.019, 0.004 and 0.002,
respectively) resveratrol-treated normal chondrocyte cultures. In
OA chondrocytes, expression of COL10 and RUNX2 was significantly
greater in 25 µM (p = 0.004 and 0.024) and 50 µM (p = 0.004 and
0.019) cultures at 72 hours on 3D cultures. Conclusions. At concentrations of 25 µM and/or 50 µM, resveratrol treatment
significantly upregulates SIRT1 gene expression in normal and osteoarthritic
chondrocytes. Resveratrol induces chondrocytes into a hypertrophic

Objectives. Electronic forms of data collection have gained interest in recent
years. In orthopaedics, little is known about patient preference
regarding pen-and-paper or electronic questionnaires. We aimed to
determine whether patients undergoing total hip (THR) or total knee
replacement (TKR) prefer pen-and-paper or electronic questionnaires
and to identify variables that predict preference for electronic
questionnaires. Methods. We asked patients who participated in a multi-centre cohort study
investigating improvement in health-related quality of life (HRQoL)
after THR and TKR using pen-and-paper questionnaires, which mode
of questionnaire they preferred. Patient age, gender, highest completed
level of schooling, body mass index (BMI), comorbidities, indication
for joint replacement and pre-operative HRQoL were compared between
the groups preferring different modes of questionnaire. We then
performed logistic regression analyses to investigate which variables
independently predicted preference of electronic questionnaires. Results. A total of 565 THR patients and 387 TKR patients completed the
preference question. Of the THR patients, 81.8% (95% confidence
interval (CI) 78.4 to 84.7) preferred pen-and-paper questionnaires
to electronic questionnaires, as did 86.8% (95% CI 83.1 to 89.8)
of TKR patients. Younger age, male gender, higher completed level
of schooling and higher BMI independently predicted preference of
electronic questionnaires in THR patients. Younger age and higher
completed level of schooling independently predicted preference
of electronic questionnaires in TKR patients. Conclusions. The majority of THR and TKR patients prefer pen-and-paper questionnaires.
Patients who preferred electronic questionnaires differed from patients
who preferred pen-and-paper questionnaires. Restricting the mode
of patient-reported outcome measures to electronic questionnaires

Objectives. To investigate psychosocial and biomedical outcomes following
total hip replacement (THR) and to identify predictors of recovery
from THR. Methods. Patients with osteoarthritis (OA) on the waiting list for primary
THR in North West England were assessed pre-operatively and at six
and 12 months post-operatively to investigate psychosocial and biomedical
outcomes. Psychosocial outcomes were anxiety and depression, social
support and health-related quality of life (HRQoL). Biomedical outcomes
were pain, physical function and stiffness. The primary outcome
was the Short-Form 36 (SF-36) Health Survey Total Physical Function.
Potential predictors of outcome were age, sex, body mass index,
previous joint replacement, involvement in the decision for THR,
any comorbidities, any complications, type of medication, and pre-operative
ENRICHD Social Support Instrument score, Hospital Anxiety and Depression
scores and Western Ontario and McMaster Universities osteoarthritis index
score. Results. The study included 206 patients undergoing THR. There were 88
men and 118 women with a mean age of 66.3 years (. sd. 10.4;36
to 89). Pain, stiffness and physical function, severity of OA, HRQoL,
anxiety and depression all improved significantly from pre-operative
to 12-month assessment (all p <
0.001), with the greatest improvement occurring
in the first six months (all p <
0.001). The predictors that
were found to influence recovery six months after THR were: pain
(p <
0.001), anxiety (p = 0.034), depression (p = 0.001), previous
joint replacement (p = 0.006) and anti-inflammatory drugs (p = 0.012). Conclusions. The study identified the key psychosocial and biomedical predictors
of recovery following THR. By identifying these predictors, we are
able to identify and provide more support for patients at risk of

Objectives. To determine the morbidity and mortality outcomes of patients
presenting with a fractured neck of femur in an Australian context.
Peri-operative variables related to unfavourable outcomes were identified
to allow planning of intervention strategies for improving peri-operative
care. Methods. We performed a retrospective observational study of 185 consecutive
adult patients admitted to an Australian metropolitan teaching hospital
with fractured neck of femur between 2009 and 2010. The main outcome
measures were 30-day and one-year mortality rates, major complications
and factors influencing mortality. . Results. The majority of patients were elderly, female and had multiple
comorbidities. Multiple peri-operative medical complications were
observed, including pre-operative hypoxia (17%), post-operative
delirium (25%), anaemia requiring blood transfusion (28%), representation
within 30 days of discharge (18%), congestive cardiac failure (14%),
acute renal impairment (12%) and myocardial infarction (4%). Mortality
rates were 8.1% at 30 days and 21.6% at one year. Factors predictive
of one-year mortality were American Society of Anesthesiologists
(ASA) score (odds ratio (OR) 4.2 (95% confidence interval (CI) 1.5
to 12.2)), general anaesthesia (OR 3.1 (95% CI 1.1 to 8.5)), age
>
90 years (OR 4.5 (95% CI 1.5 to 13.1)) and post-operative oliguria
(OR 3.6 (95% CI 1.1 to 11.7)). Conclusions. Results from an Australian metropolitan teaching hospital confirm
the persistently high morbidity and mortality in patients presenting
with a fractured neck of femur. Efforts should be aimed at medically
optimising patients pre-operatively and correction of pre-operative

Objectives. One commonly used rat fracture model for bone and mineral research
is a closed mid-shaft femur fracture as described by Bonnarens in
1984. Initially, this model was believed to create very reproducible
fractures. However, there have been frequent reports of comminution
and varying rates of complication. Given the importance of precise
anticipation of those characteristics in laboratory research, we
aimed to precisely estimate the rate of comminution, its importance and
its effect on the amount of soft callus created. Furthermore, we
aimed to precisely report the rate of complications such as death
and infection. Methods. We tested a rat model of femoral fracture on 84 rats based on
Bonnarens’ original description. We used a proximal approach with
trochanterotomy to insert the pin, a drop tower to create the fracture
and a high-resolution fluoroscopic imager to detect the comminution.
We weighed the soft callus on day seven and compared the soft callus
parameters with the comminution status. Results. The mean operating time was 34.8 minutes (. sd. 9.8). The
fracture was usable (transverse, mid-shaft, without significant
comminution and with displacement <
1 mm) in 74 animals (88%).
Of these 74 usable fractures, slight comminution was detected in
47 (63%). In 50 animals who underwent callus manipulation, slight
comminution (n = 32) was statistically correlated to the amount
of early callus created (r = 0.35, p = 0.015). Two complications occurred:
one death and one deep infection. Conclusions. We propose an accurate description of comminution and complications
in order to improve experiments on rat femur fracture model in the

Objective. To study the effect of hyaluronic acid (HA) on local anaesthetic
chondrotoxicity in vitro. Methods. Chondrocytes were harvested from bovine femoral condyle cartilage
and isolated using collagenase-containing media. At 24 hours after
seeding 15 000 cells per well onto a 96-well plate, chondrocytes
were treated with media (DMEM/F12 + ITS), PBS, 1:1 lidocaine (2%):PBS,
1:1 bupivacaine (0.5%):PBS, 1:1 lidocaine (2%):HA, 1:1 bupivacaine (0.
5%):HA, or 1:1 HA:PBS for one hour. Following treatment, groups
had conditions removed and 24-hour incubation. Cell viability was
assessed using PrestoBlue and confirmed visually using fluorescence
microscopy. Results. Media-treated groups had a mean of 1.55×10. 4. cells/well
(. sem. 783). All treated cells showed statistically significant reduced
viability when compared with media alone (all p <

There was no statistical difference between the chondrotoxicities
of PBS (8.49×10. 3. cells/well (. sem. 730) cells/well)
and HA (4.75×10. 3. cells/well (. sem. 886)) (p =
0.294). Conclusions. HA co-administration reduced anaesthetic cytotoxicity with bupivacaine
but not lidocaine, suggesting different mechanisms of injury between
the two. Co-administered intra-articular injections of HA with bupivacaine,
but not lidocaine, may protect articular chondrocytes from local

The management of acetabular defects at the time of revision hip arthroplasty surgery is a challenge. This study presents the results of a long-term follow-up study of the use of irradiated allograft bone in acetabular reconstruction. Between 1990 and 2000, 123 hips in 110 patients underwent acetabular reconstruction for aseptic loosening, using impaction bone grafting with frozen, irradiated, and morsellized femoral heads and a cemented acetabular component. Objective. The objective of this study was to evaluate the rotation and
translation of each joint in the hindfoot and compare the load response
in healthy feet with that in stage II posterior tibial tendon dysfunction
(PTTD) flatfoot by analysing the reconstructive three-dimensional
(3D) computed tomography (CT) image data during simulated weight-bearing. . Methods. CT scans of 15 healthy feet and 15 feet with stage II PTTD flatfoot
were taken first in a non-weight-bearing condition, followed by
a simulated full-body weight-bearing condition. The images of the
hindfoot bones were reconstructed into 3D models. The ‘twice registration’
method in three planes was used to calculate the position of the
talus relative to the calcaneus in the talocalcaneal joint, the
navicular relative to the talus in talonavicular joint, and the cuboid
relative to the calcaneus in the calcaneocuboid joint. Results. From non- to full-body-weight-bearing condition, the difference
in the talus position relative to the calcaneus in the talocalcaneal
joint was 0.6° more dorsiflexed (p = 0.032), 1.4° more everted (p
= 0.026), 0.9 mm more anterior (p = 0.031) and 1.0 mm more proximal
(p = 0.004) in stage II PTTD flatfoot compared with that in a healthy
foot. The navicular position difference relative to the talus in
the talonavicular joint was 3° more everted (p = 0.012), 1.3 mm more
lateral (p = 0.024), 0.8 mm more anterior (p = 0.037) and 2.1 mm
more proximal (p = 0.017). The cuboid position difference relative
to the calcaneus in the calcaneocuboid joint did not change significantly
in rotation and translation (all p ≥ 0.08). . Conclusion. Referring to a previous study regarding both the cadaveric foot
and the live foot, joint instability occurred in the hindfoot in
simulated weight-bearing condition in patients with stage II PTTD
flatfoot. The method used in this study might be applied to clinical
analysis of the aetiology and evolution of PTTD flatfoot, and may
inform biomechanical analyses of the effects of foot surgery in

learning curve for introducing robotic-arm assisted unicompartmental
knee arthroplasty (UKA) into routine surgical practice. The secondary
