A possible solution for the management of proximal femoral bone
loss is a modular femoral endoprosthesis (EPR). Although the outcome
of EPRs in tumour surgery has been well described, the outcome of
their use in revision hip surgery has received less attention. The
aim of this study was to describe the outcome of using EPR for non-neoplastic
indications. A retrospective review of 79 patients who underwent 80 EPRs for
non-neoplastic indications was performed, including the rates of
complication and survival and the mean Oxford Hip Scores (OHS),
at a mean of five years post-operatively. The mean age at the time
of surgery was 69 years (28 to 93) and the mean number of previous operations
on the hip was 2.4 (0 to 17). The most common indications for EPR
implantation were periprosthetic joint infection (PJI) (n = 40),
periprosthetic fracture (n = 12) and failed osteosynthesis of a
proximal femoral fracture or complex trauma (n = 11).Objectives

The purpose of this study was to compare the clinical and radiographic
outcomes of total ankle arthroplasty (TAA) in patients with pre-operatively
moderate and severe arthritic varus ankles to those achieved for
patients with neutral ankles. A total of 105 patients (105 ankles), matched for age, gender,
body mass index, and follow-up duration, were divided into three
groups by pre-operative coronal plane tibiotalar angle; neutral

The interaction between surgical lighting and laminar airflow
is poorly understood. We undertook an experiment to identify any
effect contemporary surgical lights have on laminar flow and recommend
practical strategies to limit any negative effects. Neutrally buoyant bubbles were introduced into the surgical field
of a simulated setup for a routine total knee arthroplasty in a
laminar flow theatre. Patterns of airflow were observed and the
number of bubbles remaining above the surgical field over time identified.
Five different lighting configurations were assessed. Data were analysed

The use of large-diameter metal-on-metal (MoM)
components in total hip arthroplasty (THA) is associated with an increased
risk of early failure due to adverse local tissue reaction to metal
debris (ARMD) in response to the release of metal ions from the
bearing couple and/or head-neck taper corrosion. The aim of this
paper was to present a review of the incidence and natural history
of ARMD and the forms of treatment, with a focus on the need for
and extent of resection or debulking of the pseudotumour. An illustrative
case report is presented of a patient with an intra-pelvic pseudotumour
associated with a large diameter MoM THA, which was treated successfully
with revision of the bearing surface to a dual mobility couple and

We used the National Joint Registry for England, Wales, Northern Ireland and the Isle of Man (NJR) to investigate the risk of revision due to prosthetic joint infection (PJI) for patients undergoing primary and revision hip arthroplasty, the changes in risk over time, and the overall burden created by PJI. We analysed revision total hip arthroplasties (THAs) performed due to a diagnosis of PJI and the linked index procedures recorded in the NJR between 2003 and 2014. The cohort analysed consisted of 623 253 index primary hip arthroplasties, 63 222 index revision hip arthroplasties and 7585 revision THAs performed due to a diagnosis of PJI. The prevalence, cumulative incidence functions and the burden of PJI (total procedures) were calculated. Overall linear trends were investigated with log-linear regression.Objectives

The aim of this study was to assess the current available evidence
about when patients might resume driving after elective, primary
total hip (THA) or total knee arthroplasty (TKA) undertaken for
osteoarthritis (OA). In February 2016, EMBASE, MEDLINE, Web of Science, Scopus, Cochrane,
PubMed Publisher, CINAHL, EBSCO and Google Scholar were searched
for clinical studies reporting on ‘THA’, ‘TKA’, ‘car driving’, ‘reaction
time’ and ‘brake response time’. Two researchers (CAV and JJT) independently
screened the titles and abstracts for eligibility and assessed the
risk of bias. Both fixed and random effects were used to pool data
and calculate mean differences (MD) and 95% confidence intervals
(CI) between pre- and post-operative total brake response time (TBRT).Aims

Older patients with multiple medical co-morbidities
are increasingly being offered and undergoing total joint arthroplasty
(TJA). These patients are more likely to require intensive care
support, following surgery. We prospectively evaluated the need
for intensive care admission and intervention in a consecutive series
of 738 patients undergoing elective hip and knee arthroplasty procedures.
The mean age was 60.6 years (18 to 91; 440 women, 298 men. Risk
factors, correlating with the need for critical care intervention,
according to published guidelines, were analysed to identify high-risk
patients who would benefit from post-operative critical care monitoring.
A total of 50 patients (6.7%) in our series required critical care
level interventions during their hospital stay. Six independent

including a history of congestive heart failure (odds ratio (OR)
24.26, 95% confidence interval (CI) 9.51 to 61.91), estimated blood

pulmonary disease (13.90, 95% CI 4.78 to 40.36), intra-operative
use of vasopressors (OR 8.10, 95% CI 3.23 to 20.27), revision hip
arthroplasty (OR 2.71, 95% CI 1.04 to 7.04) and body mass index

35 kg/m2 (OR 2.70, 95% CI 123 to 5.94). The model was
then validated against an independent, previously published data
set of 1594 consecutive patients. The use of this risk stratification
model can be helpful in predicting which high-risk patients would
benefit from a higher level of monitoring and care after elective

Instability of the hip is the most common mode of failure after
reconstruction with a proximal femoral arthroplasty (PFA) using
an endoprosthesis after excision of a tumour. Small studies report
improved stability with capsular repair of the hip and other techniques,
but these have not been investigated in a large series of patients.
The aim of this study was to evaluate variables associated with
the patient and the operation that affect post-operative stability.
We hypothesised an association between capsular repair and stability. In a retrospective cohort study, we identified 527 adult patients
who were treated with a PFA for tumours. Our data included demographics,
the pathological diagnosis, the amount of resection of the abductor
muscles, the techniques of reconstruction and the characteristics
of the implant. We used regression analysis to compare patients
with and without post-operative instability.Aims

Currently, there is little information about the need for peri-operative
blood transfusion in patients undergoing shoulder arthroplasty. The purpose of this study was to identify the rate of transfusion
and its predisposing factors, and to establish a blood conservation
strategy. We identified all patients who had undergone shoulder arthroplasty
at our hospital between 1 January 2011 and 31 December 2013. The
rate of transfusion was determined from the patient’s records. While
there were exceptions, patients typically underwent transfusion
if they had a level of haemoglobin of <
7.5 g/dl if asymptomatic,

9.0 g/dl if they had a significant cardiac history or symptoms
of dizziness or light headedness. Multivariable regression analysis was undertaken to identify
predictors of transfusion. High- and low-risk cohorts for transfusion

Massive endoprostheses rely on extra-cortical bone bridging (ECBB)
to enhance fixation. The aim of this study was to investigate the
role of selective laser sintered (SLS) porous collars in augmenting
the osseointegration of these prostheses. The two novel designs of porous SLS collars, one with small pores
(Ø700 μm, SP) and one with large pores (Ø1500 μm, LP), were compared
in an ovine tibial diaphyseal model. Osseointegration of these collars
was compared with that of a clinically used solid, grooved design
(G). At six months post-operatively, the ovine tibias were retrieved and
underwent radiological and histological analysis.Aims

(DAIR) in patients with hip periprosthetic joint infection (PJI)
and the relationship to patient characteristics. The outcome was
evaluated in hips with confirmed PJI and a follow-up of not less

hip arthroplasty register (between 2004 and 2013). Adherence to
criteria for DAIR was assessed according to a previously published
